- Abstract
The long-term physiological consequences of respiratory viral infections, particularly in the aftermath of the COVID-19 pandemic—termed post-acute sequelae of SARS-CoV-2 (PASC)—are rapidly evolving into a major public health concern1–3. While the cellular and molecular aetiologies of these sequelae are poorly defined, increasing evidence implicates abnormal immune responses3–6and/or impaired organ recovery7–9after infection. However, the precise mechanisms that link these processes in the context of PASC remain unclear. Here, with insights from three cohorts of patients with respiratory PASC, we established a mouse model of post-viral lung disease and identified an aberrant immune–epithelial progenitor niche unique to fibroproliferation in respiratory PASC. Using spatial transcriptomics and imaging, we found a central role for lung-resident CD8+T cell–macrophage interactions in impairing alveolar regeneration and driving fibrotic sequelae after acute viral pneumonia. Specifically, IFNγ and TNF derived from CD8+T cells stimulated local macrophages to chronically release IL-1β, resulting in the long-term maintenance of dysplastic epithelial progenitors and lung fibrosis. Notably, therapeutic neutralization of IFNγ + TNF or IL-1β markedly improved alveolar regeneration and pulmonary function. In contrast to other approaches, which require early intervention10, we highlight therapeutic strategies to rescue fibrotic disease after the resolution of acute disease, addressing a current unmet need in the clinical management of PASC and post-viral disease.

5. Peripapillary vessel density in healthy people, primary open-angle glaucoma, and normal-tension glaucoma

- Abstract
Objective To compare peripapillary vessel density using optical coherence tomography angiography (OCT-A) in eyes of healthy people, primary open-angle glaucoma (POAG), and normal-tension glaucoma (NTG).Methods Thirty patients with POAG, 27 patients with NTG, and 29 healthy individuals in the control group were assessed. Capillary vessels in peripapillary retinal nerve fiber layer (RNFL) represented by whole image RPC (radial peripapillary capillary) density in an AngioDisc scan 4.5 × 4.5 mm centered on the optic disc, and ONH morphological variables (disc area, rim area, cup to disc area ratio (CDR)), and average peripapillary RNFL thickness were measured.Results Differences in mean RPC, RNFL, disc area, rim area, and CDR between the groups were statistically significant (P < 0.05). The difference in RNFL thickness and rim area was not significant between NTG and healthy groups, while RPC and CDR showed a statistically significant difference between all pairs. The vessel density in the POAG group was 8.25% and 11.7% lower compared to the NTG and healthy groups, respectively; while the mean difference was less (2.97%) for the NTG and healthy group. In the POAG group, 67.2% of the variation in RPC can be explained by a model containing CDR and RNFL thickness, and in normal eyes 38.8% of the changes using a model containing RNFL.Conclusion The peripapillary vessel density is reduced in both types of glaucoma. The vessel density in NTG was significantly lower than in the healthy eyes, despite the lack of significant difference in RNFL thickness and neuroretinal rim area between them.

6. Evaluation and Comparison of Two Media-Rich Radiology Apps Regarding Pathology of the Lumbar Spine: Lessons for Medical Education App Developers and Content Creators.

- Abstract
Mobile applications have become an increasingly popular component of medical education over the past 2 decades. Visual layout and aesthetics are important considerations in app design given Generation Z's strong preference for visually appealing digital content. In this study, we evaluate students' perceptions of an educational app related to pathology of the lumbar spine before and after a redesign of its workflow and illustrations. A professional user experience and interface designer was recruited for the revision of an iOS app focused on anatomy and pathology of the lumbar spine on abdominal computed tomography (CT). Revisions focused on developing the app to make it more visually appealing, increasing ease of use, and fixing bugs. An IRB-approved study was conducted in which participants were asked to interact with the original and revised versions of the app prior to completing a survey regarding their perception of the graphics, animation style, overall app design, and content. Overall impressions of the revised app were more positive than the original app, with 89% of respondents finding the revised app's graphics to be visually appealing compared to 39% regarding the original app. Further, 95% of respondents indicated that the revised app's layout was organized and user-friendly compared to 73% regarding the original. Both apps received similarly positive ratings for participants' understanding of the presented materials. User feedback is key when designing mobile health and educational applications. Feedback may be sought via formal usability testing or app-specific surveys. The aesthetics, layout, and ease of use of mobile apps are important considerations and, in the absence of content alteration, may influence user engagement. 2 > 0.99). The present work utilizes waste black cardamom (BC) as an inexpensive and environmentally friendly adsorbent for sequestering the Congo Red (CR) dye from aqueous media for the first time. Following a carbonization process at 600 °C, chemical activation with KOH was carried out for waste BC and subsequent black cardamom activated carbon (BCAC) was employed as an absorbent for CR eradication. The effect of experimental factors, including pH, adsorption time, dose and CR initial concentration, was investigated. 96.21 % of CR dye removal was achieved at pH 6 for 100 mg/L of CR concentration having 0.1 g dose at 30 °C. Maximum Langmuir adsorption capacity of BCAC was found to be 69.93 mg/g at 30 °C. The kinetic analyses showed that the CR adsorption over BCAC behaved in accordance with a pseudo-second order kinetic model as high R2 values (0.997–1) were obtained. Thermodynamic parameters (ΔH°, ΔS°, and ΔG°) demonstrated that the CR adsorption over BCAC was feasible, spontaneous and exothermic in nature. In addition, the state-of-the-art machine learning (ML) approaches namely, support vector regression (SVR) and artificial neural network (ANN) were employed for modeling the BCAC adsorbent for CR removal. The statistical analysis revealed high prediction performance of SVR model with AARE value of 0.0491 and RMSE value of 0.4635 while the corresponding values for the ANN model were 0.0781 and 0.5395, respectively. Furthermore, the plots between experimental CR data and ML forecasted data were closely matched (R2 > 0.99). Thus, it can be concluded that BC, an agro waste could be utilized for CR removal and that the adoption of ML approaches can benefit users by providing them with a tool to enhance the design and performance of wastewater treatment operations. 12. Blue luminescent carbon quantum dots derived from diverse banana peels for selective sensing of Fe(III) ions

- Abstract
The eco-friendly production of carbon quantum dots (CQDs) from natural resources remains appealing owing to their superior optical properties. This work presents the synthesis of highly fluorescent CQDs from peels of different varieties of Musa (yellow, green, and red) through a straightforward one-step hydrothermal process, without needing a bit of metal salt or oxidizing agent. The proposed method resulted in quantum yields (QY) of 18.06 %, and 13.06 %, for CQDs from normal yellow banana and green banana, respectively compared to other CQDs derived from natural sources. The QY for the CQDs extracted from the small yellow banana was 7.72 %, while the red banana had a much lower value of 2.6 %. The optical properties of CQDs of different banana peels are also compared. All the CQDs produced a blue color upon exposure to 360 nm UV radiation, and the fluorescence was excitation-dependent. Moreover, each of the four types of CQDs is proven to be an efficient fluorescent probe capable of selectively detecting Fe3+ions. The linear variation of fluorescence with the analyte amount allowed quantification of ions, with a limit of the detection value of 6 μM, across a concentration range of 37–277 μM. Above all, the real-world applications aimed at sensing Fe3+ions in tap water achieved excellent recoveries ranging from 96 to 100 %. Therefore, these tuneable CQDs with good optical properties present an auspicious avenue for developing nano-sensors in real-time applications.

To deal with different important and updated applications like biomedical, pharmaceutical, industrial, agricultural, environmental, food, cosmetics, and even water purification, the synthetic approaches of solid silica nanoparticles (SSiNPs) and its porous variant mesoporous silica nanoparticles (MSiNPs) become a matter of attention. The outstanding non-toxic nature and exceptional properties like bio-mobilizability, easy surface modifiability, stability, micro or meso pore structure, volume, and high surface area make them versatile contributors and potential candidates in human civilization. This review article comprises a brief summarization of the synthesis of SiNPs chemically as well as through a green approach along with its noteworthy characteristics in which it is clearly shown how the functional molecules change the particle size and shape for multiple purposes. This article mainly focuses greatly effective all conventional chemical and green approaches of both silica nanoparticles (SiNPs) and MSiNPs (a total of 21 synthesis techniques). Also, the newly modified methods are cited in this review paper along with challenges and prospects. One of the most important discussions regarding sustainable, reliable, and eco-friendly (green) synthesis techniques and their available sources, advantages, and principles are mentioned in this paper. Finally, several relevant recent applications by many researchers are also noted with possible research in the industrial, chemical, agricultural, and biomedical life-related sectors. This review will help the researchers to gain the necessary information in perfect order which can be applied in the near future to invent and modify new methods on a bigger scale.

- Abstract
This study aims to explore the research landscape of green Lean Six Sigma from a bibliometric perspective. The analysis was conducted over the dataset extracted from Scopus database using Biblioshiny app, a web-interface for bibliometrix. Bibliometrix is an open-source tool for executing science mapping analysis of literature. This study is mainly focused on the application of bibliometric approach to analyse the research progress in the field of green Lean Six Sigma through the identification of research trends, authors' productivity, collaboration, co-occurrence, topmost contributions and trending themes. The results indicate that publications evolved initially in 2013 and the major contributors in this research area are from India. Enablers, barriers and application of green Lean Six Sigma in manufacturing sector are identified as the main driving theme for the future research. This study contributes to the field by presenting research status, thematic focus, and research hotspots along with future direction.

Non-cryptographic (NC) hash functions are generally designed for speed and efficiency, which enables their use in many network security applications that require efficient lookup and counting, such as Bloom Filters and Count-Min (CM) Sketch structures. The performance of these structures heavily relies on underlying hash functions. Therefore, any advancement in the hash function design significantly impacts the overall performance of these structures. This paper presents a novel family of 32-bit NC hash functions (NCGPH-32) evolved using Genetic Programming (GP) and their corresponding implementation on Field Programmable Gate Arrays (FPGAs). This family of NC hash functions generates smaller hash values concatenated to produce larger hash outputs. Inspired by related work on 96-bit NC hash functions with GP, this work optimizes the performance of 32-bit NC hash functions on FPGA while achieving high scores on specific avalanche metrics (avalanche dependence, avalanche weight, and entropy) when considering concatenated 96-bit outputs. This optimization is of utmost importance to address the escalating demand for Terabit Ethernet networks, specifically in processing real-time network flow IDs (identification and monitoring) at line rate. The throughput, latency, operating frequency, and resource utilization are evaluated on an FPGA and compared against 17 state-of-the-art NC hash functions. The results show that the proposed 96-bit concatenated hash function surpasses prior GP-based and other state-of-the-art NC hash functions by at least 36% in operating frequency, 30% in throughput and reduces latency by 27%. The demonstrated improvements in the hash design not only cater to the present demands of Terabit networks but also meet the expected near-future demands. Additionally, we integrate these hash functions into the Standard Bloom Filter (SBF) architecture and demonstrate comparable false positive rates (FPR) to state-of-the-art NC hash functions, affirming their effectiveness and applicability. We have also conducted several statistical tests on hash outputs of NCGPH-32 to demonstrate the high random nature and uniform distribution. • Proposed a novel 32-bit hash family for 96-bit inputs using Genetic Programming (GP). • Concatenated the three best hash outputs from the 32-bit hash family to produce 96-bit output. • The proposed design surpasses the state-of-the-art Non-Cryptographic (NC) hash functions by at least 36% in operating frequency, and 30% in throughput and reduces latency by 27% on FPGA. • Integrated the hash functions in a Standard Bloom Filter and showed that the False Positive Rate is comparable to state-of-the-art NC hash functions. • The proposed NC hash functions exhibit favourable uniformity characteristics. 18. Equity in HIV mental health research: a call to action

The brain remains a key reservoir of latent HIV infection, and people living with HIV (PLWH) face a high risk for cognitive impairment and psychiatric disorders. Although the burden of HIV infection and co-morbidities is greatest in the Global South, a large proportion of HIV mental health research is carried out in the Global North. Large, well-funded observational cohort studies exploring HIV-associated psychopathology generally involve participant groups from WEIRD (Western, educated, industrialised, rich and democratic) settings. The socioeconomic status and institutional access afforded to these participant groups on average does not reflect those of the majority of beneficiaries of HIV mental health research. This misalignment may lead to limitations in generalising findings and developing effective interventions to improve the mental health of PLWH. Here, I offer recommendations to actively cultivate authentic diversity and inclusion in the field, with four focus points: (1) for funding bodies, to actively invest in neuroscientists in the Global South for investigations of HIV-related psychopathology; (2) for scientific publishers, to fund professional support services for researchers in the Global South; (3) for academic institutions, to facilitate meaningful, equitable collaborations with researchers in the Global South and incentivise studies with diverse participant groups; and (4) for individual neuroscientists, to actively cite and converse with colleagues in the Global South, tackle personal biases in those conversations, and avoid overgeneralising findings from primarily WEIRD participant groups.

Increasing expansion rate in cell manufacturing systems would allow faster delivery of life-saving treatments to a greater number of patients. Chaotic printing is a novel biofabrication technology that could improve cell expansion capabilities by producing hydrogel filaments layered with open channels, resulting in significantly higher surface area-per-unit-volume of interface between cells and nutrient media compared to existing systems. In this study, we chaotically printed hydrogel filaments laden with clinically-relevant, bone marrow-derived human mesenchymal stem cells. After the first week of a 21-day expansion period, cell-laden hydrogel filaments containing open channels had an optimal expansion rate and 2.1x as many cells as filaments without channels. AbstractA novel Schiff base was synthesized by the condensation of imidazole-2-carboxaldehyde with l-histidine in an equimolar ratio. The prepared Schiff base was characterized by elemental analysis and spectral characterization techniques. It was then complexed with a series of 3-d metal(II) ions like manganese, iron, cobalt, nickel, copper and zinc. The coordination properties, nature of bonding and stability of the complexes were deduced from elemental analysis, IR, UV–vis, 1H NMR, mass, electronic spectra, magnetic, conductivity and thermogravimetric analysis. IR studies support the tridentate behaviour of Schiff base as well as its coordination to the central metal ion through an azomethine nitrogen, deprotonated carboxylic oxygen and imidazole ring nitrogen atoms of histidine. The electronic spectra and magnetic moment data demonstrate that the complexes have an octahedral geometry, except zinc complex, which has a tetrahedral geometry. In vitro antimicrobial activity of the synthesized compounds has been shown to exhibit excellent antibacterial and antifungal activities. The antibacterial property of the prepared Schiff base was further confirmed by conducting a docking study of target proteins involved in the antibacterial mechanism.Communicated by Ramaswamy H. ABSTRACTBackgroundIn Pakistan, there are a total of 160 neurosurgeons that constitute a ratio of one neurosurgeon for a 5.5 million population. Gilgit Baltistan being a backward region does not have a single tertiary care facility. A new department of neurosurgery was established at Regional Headquarter City Hospital (RHQ) Gilgit with only one neurosurgeon. This study aimed to determine which neurosurgical diagnoses are common and which surgical interventions were performed at RHQ.MethodsThis is a retrospective cross-sectional study conducted at Regional Headquarter Hospital (RHQ) Gilgit in which data of patients operated for neurosurgical diseases between January 2020 and January 2022 was collected from patient record books.ResultsOf the total of 223 patients, 148 (66.3%) were males and 75 (33.6%) were females. A total of 92(41.2%) belonged to the pediatric age group. The top most diagnosis included Neurotrauma (46.6%), NTDs (13.9%) and CSDH (10.3%) while the most routinely performed procedures were craniotomy & hematoma evacuation (22.9%), debridement & elevation of DSF (20.6%), and burrhole evacuation (13.9%). In the pediatric age group, the top diagnosis was Neurotrauma (43.5%), NTDs (32.6%), and Hydrocephalus (19.6%) while in adults, neurotrauma (48.9%) was the leading diagnosis followed by CSDH (17.6%). In the pediatric age group, repair of NTDs (32.6%) was the most frequently performed procedure.ConclusionThis study shows different kinds of neurosurgical cases but because of a lack of diagnostic and therapeutic facilities, very limited operations were performed and many cases were referred to metropolitan cities. The hospitals in the region need further up-gradation to cater to the presenting burden.

23. Neuroinflammation in HIV-associated depression: evidence and future perspectives

- Abstract
People living with HIV face a high risk of mental illness, especially depression. We do not yet know the precise neurobiological mechanisms underlying HIV-associated depression. Depression severity in the general population has been linked to acute and chronic markers of systemic inflammation. Given the associations between depression and peripheral inflammation, and since HIV infection in the brain elicits a neuroinflammatory response, it is possible that neuroinflammation contributes to the high prevalence of depression amongst people living with HIV. The purpose of this review was to synthesise existing evidence for associations between inflammation, depression, and HIV. While there is strong evidence for independent associations between these three conditions, few preclinical or clinical studies have attempted to characterise their interrelationship, representing a major gap in the literature. This review identifies key areas of debate in the field and offers perspectives for future investigations of the pathophysiology of HIV-associated depression. Reproducing findings across diverse populations will be crucial in obtaining robust and generalisable results to elucidate the precise role of neuroinflammation in this pathophysiology.

• Objective and Methodology: the study aimed to develop an explainable machine learning model to predict the conversion of clinically isolated syndrome (CIS) to clinically definite multiple sclerosis (CDMS). The model used demographic, clinical, and imaging data from a public dataset of 273 Mexican mestizo CIS patients with a 10-year follow-up. • Model Performance: the model achieved an accuracy of 83.6 %, AUC of 91.8 %, sensitivity of 83.9 %, and specificity of 83.4 % in cross-validation. In the final testing, the model achieved an accuracy of 78.3 %, AUC of 85.8 %, sensitivity of 75 %, and specificity of 81.1 %. • Significant Variables: nine variables including age, gender, schooling, motor symptoms, infratentorial and periventricular lesions in MRI, oligoclonal bands in cerebrospinal fluid, lesion and symptoms types were significant. • Interpretability: the study emphasized the importance of model interpretability in medical applications. The SHapley additive exPlanations (SHAP) library was used to calculate the feature importance, providing a clear understanding of how each variable influences the prediction. • Conclusion: the developed model effectively stratifies risk for treatment decisions and disability prevention in MS patients. It provides a numerical risk estimate for CDMS conversion, enhancing transparency in clinical decision-making and aiding in patient care. 26. An intelligent lighting control system for individual visual comfort and energy savings in buildings

- Abstract
Office buildings especially workplaces are not tailor-made to meet personal indoor requirements of comfort. Therefore, it is a challenge to meet individual-level comfort in these buildings without compromising energy efficiency. This paper presents and analyzes an intelligent lighting system for the visual comfort of individuals and the energy efficiency of buildings. The proposed lighting system best uses historical user preferences, collaborative user preferences, and historical energy consumption data through a recommender systems-based intelligent algorithm to generate proffered lighting set-point and optimize energy consumption of the lighting system. The system is a wireless sensor–actuator network-based and makes use of illumination sensors and occupancy sensors, which measure the ambient light, detect the user’s presence, and controls the intensity of light on a user’s work area. The Smart Android App allows the user to modify light intensities depending on visual comfort or the activity-based needs of users. Results from the testbed show that the proposed lighting system is around 72% energy efficient than the conventional fluorescent lighting system while providing individual visual comfort to the occupants. This energy efficiency is achieved through the combined effects of using energy-efficient LED lighting system and automatic as well as manual lighting control through intelligent algorithm and smart app, respectively, used in the system.

Mycobacteria tuberculosis(M.tb) the causative agent for tuberculosis has been accredited for a high rate of morbidity and mortality worldwide. The rise in MDR and XDR cases has further created new obstacles in achieving the “End TB Strategy”, which is aimed for 2035. In this article, we have demonstrated the potential of sphingosine-1-phosphate (S1P) analogs in providing an anti-mycobacterial effector response by altering macrophage polarity into M1. Among S1PR1 and S1PR3 analogs, S1PR2 analogs proficiently favor selective polarization of infected human macrophages into M1 phenotypes, marked by increased expression of M1 markers and decreased M2 markers. Furthermore, S1PR1-3 analogs treated macrophages were also able to decrease the secretion of anti-inflammatory cytokine IL-10 and can induce NO secretion in infected macrophages. Lastly, only S1PR2-3 analogs were able to restrict the growth of mycobacteria in human macrophages. Taken together our study reflects the potential of S1PR2-3 analogs in providing host defenses following mycobacterial infection by favoring M1 macrophage polarization.

- Abstract
Answer ALS is a biological and clinical resource of patient-derived, induced pluripotent stem (iPS) cell lines, multi-omic data derived from iPS neurons and longitudinal clinical and smartphone data from over 1,000 patients with ALS. This resource provides population-level biological and clinical data that may be employed to identify clinical–molecular–biochemical subtypes of amyotrophic lateral sclerosis (ALS). A unique smartphone-based system was employed to collect deep clinical data, including fine motor activity, speech, breathing and linguistics/cognition. The iPS spinal neurons were blood derived from each patient and these cells underwent multi-omic analytics including whole-genome sequencing, RNA transcriptomics, ATAC-sequencing and proteomics. The intent of these data is for the generation of integrated clinical and biological signatures using bioinformatics, statistics and computational biology to establish patterns that may lead to a better understanding of the underlying mechanisms of disease, including subgroup identification. A web portal for open-source sharing of all data was developed for widespread community-based data analytics.

30. Skeletonized or Pedicled Harvesting of Left Internal Mammary Artery: A Systematic Review and Meta-analysis.

We sought to compare clinical outcomes in skeletonized versus pedicled left internal mammary artery (LIMA) grafts in elective coronary artery bypass grafting through a systematic review and meta-analysis. A comprehensive electronic literature search of PubMed, Ovid, Embase, and Scopus was conducted from inception to January 2020. Only short-term (30 days) studies which compared both techniques have been included in our analysis. Primary outcomes were post anastomosis flow rate and sternal wound infection rate (SWI); secondary outcomes were conduit length, acute myocardial infarction and 30-day mortality. Thirteen articles with a total of 6222 patients met the inclusion criteria. Except for the prevalence of diabetes mellitus being significantly lower in the skeletonized cohort (odds ratio [OR] 0.77 95% confidence interval [CI] [0.61, 0.97], P = 0.03), there were no differences in the preoperative demographics between the 2 groups. The skeletonized LIMA conduit was significantly longer when compared to the pedicled conduit (weighted mean difference -2.64 cm 95% CI [-3.71, -1.56], P < 0.0001). SWI rates were not significantly different in the skeletonized versus pedicled LIMA group (OR 0.71 95% CI [0.47, 1.06], P = 0.10). New onset of acute myocardial infarction and 30-day mortality rate was similar in the 2 groups (OR 1.04 and 0.97, respectively, P > 0.05 in both). The postanastomoses flow rate was higher in skeletonized LIMA (Weighted Mean Difference -11.51 mL/min 95% CI [-20.54, -2.49], P < 0.01). Harvesting the LIMA using the skeletonized technique is associated with higher postanastomosis flow rates and longer conduit lengths; with no difference in SWI and mortality rates when compared to the pedicled technique. We suggest that this technique should be adopted, particularly for BITA harvesting. However, further research is needed to provide clearer indications for both methods. [ABSTRACT FROM AUTHOR]

Postacute sequelae of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection (PASC) represent an urgent public health challenge and are estimated to affect more than 60 million individuals globally. Although a growing body of evidence suggests that dysregulated immune reactions may be linked with PASC symptoms, most investigations have primarily centered around blood-based studies, with few focusing on samples derived from affected tissues. Furthermore, clinical studies alone often provide correlative insights rather than causal mechanisms. Thus, it is essential to compare clinical samples with relevant animal models and conduct functional experiments to understand the etiology of PASC. In this study, we comprehensively compared bronchoalveolar lavage fluid single-cell RNA sequencing data derived from clinical PASC samples and a mouse model of PASC. This revealed a pro-fibrotic monocyte-derived macrophage response in respiratory PASC, as well as abnormal interactions between pulmonary macrophages and respiratory resident T cells, in both humans and mice. Interferon-γ (IFN-γ) emerged as a key node mediating the immune anomalies in respiratory PASC. Neutralizing IFN-γ after the resolution of acute SARS-CoV-2 infection reduced lung inflammation and tissue fibrosis in mice. Together, our study underscores the importance of performing comparative analysis to understand the cause of PASC and suggests that the IFN-γ signaling axis might represent a therapeutic target.

The model used demographic, clinical, and imaging data from a public dataset of 273 Mexican mestizo CIS patients with a 10-year follow-up.•Model Performance:the model achieved an accuracy of 83.6 %, AUC of 91.8 %, sensitivity of 83.9 %, and specificity of 83.4 % in cross-validation. The SHapley additive exPlanations (SHAP) library was used to calculate the feature importance, providing a clear understanding of how each variable influences the prediction.•Conclusion:the developed model effectively stratifies risk for treatment decisions and disability prevention in MS patients. It provides a numerical risk estimate for CDMS conversion, enhancing transparency in clinical decision-making and aiding in patient care.

Abstract Built-environment, especially open-plan workplaces are often not tailored to meet individual visual comfort needs. Therefore, meeting the need for personalized visual comfort whilst achieving energy efficiency in open-plan office environment has been an open challenge. However, recent technological advancements in distributed sensing, pervasive computing, context-awareness and machine learning is progressively closing this gap. This article introduces ReViCEE –a simple recommender systems based approach to learn both individual and collaborative user-preferences from historical data and offer recommendations for intelligent building lighting controls. The intelligence in this case is achieved by being able to derive set-points to control task lights such that it balances personalized visual comfort without compromising on energy savings. The proposed approach has been developed using Python and implemented on a real test-bed in an university campus office building in National University of Singapore. The evaluation of the proposed approach is carried out for two months using field experiments involving distributed wireless sensor actuator network (WSAN) and multiple occupants having varied visual sensation. The novelty lies in proposing a new inter-disciplinary approach that supports smart and intelligent buildings paradigm by learning and predicting optimum individual user-preferences towards energy efficient control of personalized light. The results obtained from field experiments present a potential energy savings upto 72% when compared to the conventional lighting systems used. 34. Antisynthetase Syndrome and Malignancy: An Association or Mere Coincidence?

- Abstract
Antisynthetase syndrome (ASS) is a rare autoimmune idiopathic disease identified in a subset of inflammatory myopathies which includes dermatomyositis (DM) and polymyositis. ASS is associated with malignancy, but the association is weaker than the association of DM with malignancy. ASS is considered by some to be a protective factor. For these reasons, patients with ASS often undergo less aggressive cancer screenings than patients with DM. We describe a squamous cell carcinoma of unknown primary origin in a patient with ASS. Our case suggests that ASS can also be associated with malignancy. Therefore, these patients require aggressive cancer screening at the time of diagnosis. The case also highlights the safety of using radiation in patients with autoimmune inflammatory disease. [ABSTRACT FROM AUTHOR]

Hintergrund:Die Lungenfunktionsprüfung (LFP) wird wegen der Anordnung der Messgeräte und des Patientenkomforts routinemäßig in aufrechter Position durchgeführt. In der vorliegenden systematischen Übersichtsarbeit wurde der Einfluss der Körperposition auf die Lungenfunktion bei gesunden Probanden und speziellen Patientengruppen untersucht. Methoden:Zur Identifizierung englischsprachiger Publikationen, die zwischen Januar 1998 und Dezember 2017 veröffentlicht wurden, erfolgte eine Suche in MEDLINE und Google Scholar anhand der Suchbegriffe body position, lung function, lung mechanics, lung volume, position change, positioning, posture, pulmonary function testing, sitting, standing, supine, ventilation, and ventilatory change. Bei den eingeschlossenen Studien handelte es sich um quasi-experimentelle Prä-post-Interventionen, in denen mindestens 2 Positionen, einschließlich Sitzen oder Stehen, untersucht wurden und die die Lungenfunktion bei nicht mechanisch beatmeten Probanden ≥ 18 Jahre bewerteten. Primäre Zielkriterien waren das forcierte exspiratorische Volumen in 1 Sekunde (FEV
1 ), die forcierte Vitalkapazität (FVC, FEV1 /FVC), die Vitalkapazität (VC), die funktionelle Residualkapazität (FRC), der maximale exspiratorische Druck (PEmax), der maximale inspiratorische Druck (PImax), der exspiratorische Spitzenfluss (PEF), die totale Lungenkapazität (TLC), das Residualvolumen (RV) und die Kohlenmonoxid-Diffusionskapazität der Lunge (DLCO ). Folgende Positionen wurden untersucht: stehend, sitzend, Rückenlage sowie Rechts- und Linksseitenlage. Ergebnisse:43 Studien erfüllten die Einschlusskriterien. Die Studienpopulationen umfassten gesunde Probanden (29 Studien), Patienten mit Lungenerkrankungen (9), Herzkrankheiten (4), Rückenmarksverletzungen (SCI) (7), neuromuskulären Erkrankungen (3) und Adipositas (4). In den meisten Studien mit gesunden Probanden oder Patienten mit Lungen-, Herz- oder neuromuskulären Erkrankungen oder Adipositas fielen die Werte für FEV1 , FVC, FRC, PEmax, PImax und/oder PEF in den aufrechteren Positionen höher aus. Bei Patienten mit SCI-bedingter Tetraplegie waren die Werte für FVC und FEV1 in Rückenlage höher als im Sitzen. Gesunde Probanden wiesen in Rückenlage eine höhere DLCO auf als im Sitzen und im Sitzen höhere Werte als in Seitenlage. Bei Patienten mit chronischer Herzinsuffizienz fiel der Einfluss der Körperposition auf die DLCO unterschiedlich aus. Schlussfolgerungen: Die Körperposition hat Einfluss auf die Ergebnisse der LFP, doch bestehen Unterschiede hinsichtlich optimaler Position und Ausmaß des Benefits zwischen den Studienpopulationen. Die LFP erfolgt routinemäßig in sitzender Position. Wir empfehlen, bei Patienten mit Rückenmarksverletzung und neuromuskulären Erkrankungen neben der sitzenden Position auch die Rückenlage für die LFP in Betracht zu ziehen. Bei der Behandlung von Patienten mit Herz- oder Lungenerkrankungen, SCI, neuromuskulären Erkrankungen oder Adipositas ist zu berücksichtigen, dass die Physiologie und Funktion der Lunge von der Körperposition beeinflusst werden.- Published

36. Repurposing Glyburide as Antileishmanial Agent to Fight Against Leishmaniasis

- Abstract
Background: Leishmaniasis is caused by a protozoan parasite, Leishmania. It is common in more than 98 countries throughout the world. Due to insufficient availability of antileishmanial chemotherapeutics, it is an urgent need to search for new molecules which have better efficacy, low toxicity and are available at low cost. Objectives: There is a high rate of diabetic cases throughout the world that is why we planned to test the antileishmanial activity of glyburide, an effective sugar lowering drug used for the treatment of diabetes. In this study, glyburide showed a significant decrease in the parasite growth and survival in vitro in a dose-dependent manner. Methods: Anti-leishmanial activity of glyburide was checked by culturing Leishmania donovani promastigotes in the presence of glyburide in a dose and time dependent manner. Docking study against Leishmania donovani-Trypanothione synthetase (LdTrySyn) protein was performed using Autodock Vina tool. Results: Growth reversibility assay shows that growth of treated parasite was not reversed when transferred to fresh culture media after 7 days. Moreover, docking studies show efficient interactions of glyburide with key residues in the catalytic site of Leishmania donovani- Trypanothione synthetase (LdTrySyn), a very important leishmanial enzyme involved in parasite’s survival by detoxification of Nitric Oxide (NO) species, generated by the mammalian host as a defense molecule. Thus this study proves that the drug-repurposing is a beneficial strategy for identification of new and potent antileishmanial molecules. Conclusion: The results suggest that glyburide binds to LdTrySyn and inhibits its activity which further leads to the altered parasite morphology and inhibition of parasite growth. Glyburide may also be used in combination with other anti-leishmanial drugs to potentiate the response of the chemotherapy. Overall this study provides information about combination therapy as well as a single drug treatment for the infected patients suffering from diabetes. This study also provides raw information for further in vivo disease model studies to confirm the hypothesis.

Percutaneous cholecystostomy tube (PTGBD), endoscopic retrograde cholangiopancreatography with transpapillary gallbladder drainage (TP), and endoscopic ultrasound-guided transmural gallbladder drainage (EGBD) using lumen-apposing metal stents (LAMS) have been offered for gallbladder decompression for acute cholecystitis in high-risk surgical patients. Yet, there are limited data comparing these therapies. Our aim was to compare the safety and efficacy of EGBD to TP and PTGBD for gallbladder drainage. We retrospectively collected high-risk surgical patients from six centers with acute cholecystitis who underwent gallbladder drainage by EGBD, TP, or PTGBD. Data included technical success (gallbladder drainage), clinical success (acute cholecystitis resolution), adverse events (AE), and follow-up. From 2010 to 2016, 372 patients underwent gallbladder drainage, with 146 by PTGBD, 124 by TP, and 102 drained by EGBD. Technical (98% vs. 88% vs. 94%; p= 0.004) and Clinical (97% vs. 90% vs. 80%; p< 0.001) success rates were significantly higher with PTGBD and EGBD compared to TP. PTGBD group had statistically significantly higher number of complications as compared to EGBD and TP groups (2 0% vs. 2% vs. 5%; p= 0.01). Mean hospital stay in the EGBD group was significantly less than TP and PTGBD (16 vs. 18 vs. 19 days; p= 0.01), while additional surgical intervention was significantly higher in the PTGBD group compared to the EGBD and TP groups (49% vs. 4% vs. 11%; p< 0.0001). EGBD with LAMS is an effective and safer alternative to TP and PTGBD for treatment of patients with acute cholecystitis who cannot undergo surgery. EGBD with LAMS has significantly lower overall AEs, hospital stay, and unplanned admissions compared to PTGBD. Trial registration: ClinicalTrials.gov Identifier: NCT01522573.

- Abstract
There has been a steady progress in the development of doped conjugated polymers to remarkably improve their photo physical properties for their application as biomarkers. With a view to enhance the spectral, morphological, and photo physical properties of poly(o-phenylenediamine) (POPD), the present work reports the synthesis of poly(o-phenylenediamine) and doping of this polymer using luminol. The formation of luminol-doped POPD was confirmed by infrared and ultraviolet–visible spectroscopies and X-ray diffraction studies. The energy band gap values and oscillator strength of luminol in acidic, basic, and neutral media were computed by density functional theory calculations using the B3LYP/6-31G (d) basis set and were compared with experimental data. The luminol doped POPDs show significant in vitro anti-leishmanial activity. Live cell imaging also proved that these molecules bind with the organelle of Leishmaniaalso. These luminol doped POPDs were found non-toxic at the used concentrations on THP-1 derived human macrophage cells through methyl tetrazolium (MTT) assay. The results revealed that luminol doped POPDs were potentially non-toxic to human cells though exhibited immense potential to be used as a fluorescent marker to label Leishmania donovanifor diagnostic and other studies.

In this study, an attempt was made to develop bi-functional constructs serving both as scaffolds and potential delivery systems for application in neural tissue engineering. The constructs were prepared in two steps. In the first step, the bulks of poly (L-lactic acid) (PLLA) in 1, 4-dioxane/water (87:13) were fabricated using liquid-liquid thermally induced phase separation technique. In the next step, the prepared bulks were coated with chitosan nanoparticles produced by two different techniques of ultrasonication and ionic gelation by grafting-coating technique. In ultrasonication technique, the chitosan solution (2 mg/mL) in acetic acid/sodium acetate buffer (90:10) was irradiated by an ultrasound generator at 20 kHz and power output of 750 W for 100 s. In ionic gelation technique, the tripolyphosphate in water solution (1 mg/mL) was added to the same chitosan solution. The physicochemical properties of the products were characterized by Scanning Electron Microscopy, Attenuated Total Reflection Fourier Transform-Infrared, liquid displacement technique, contact angle measurement, compressive and tensile tests, as well as zeta potential and particle size analysis using dynamic light scattering. Moreover, the cell proliferation and attachment on the scaffolds were evaluated through human glioblastoma cell line (U-87 MG) and human neuroblastoma cell line [BE (2)-C] culture respectively. The results showed that the samples coated with chitosan nanoparticles prepared by ultrasonication possessed enhanced hydrophilicity, biodegradation and cytocompatibility compared with pure PLLA and PLLA coated with chitosan nanoparticles prepared by ionic gelation. This study suggests successful nanoparticles-scaffold systems which can act simultaneously as potential delivery systems and tissue engineering scaffolds.

46. Implication of drainage basin parameters of a tropical river basin of South India

Drainage morphometry provides quantitative description of the drainage system which is an important aspect of the characterisation of watersheds. Chalakudi River is one of the important rivers of the South India which has attracted attention of many environmental scientists recently because of the proposed Athirapally Hydel Project across the river. SRTM (Shuttle Radar Topographic Mission) data were used for preparing DEM (Digital Elevation Model), Aspect Map and Slope Map. Geographical Information System (GIS) was used for the evaluation of linear, areal and relief aspects of morphometric parameters. The study reveals that the terrain exhibits dentritic and trellis pattern of drainage. The Chalakudi River Basin has a total area of 1,448.73 km2and is designated as seventh-order basin. The drainage density of the basin is estimated as 2.54 and the lower-order streams mostly dominate the basin. The high basin relief indicates high runoff and sediment transport. The elongation ratio of the Chalakudi Basin is estimated as 0.48 and indicates that the shape of the basin is elongated. The development of stream segments in the basin area is more or less effected by rainfall. Relief ratio indicates that the discharge capability of watershed is very high and the groundwater potential is meagre. The low value of drainage density in spite of mountainous relief indicates that the area is covered by dense vegetation and resistant rocks permeated by fractures and joints. These studies are helpful in watershed development planning and wise utilization of natural resources.

In recent years, visible-light-driven eco-friendly titania-based photocatalytic nanocomposite materials innovations have sparked a lot of interest in environmental applications such as water treatment, hydrogen production, and sensing to clinical and health monitoring. Many appealing physio-chemical properties are viable with the advent of such materials. However, despite the frontier of research on titania-based nanocomposites rapidly expanding, transforming the technology into the market or large-scale applications is still an open issue due to their performance/cost trade-off factor. From this perspective, researchers working on the development of titania-based materials towards more reliable and better performance in terms of under solar light by altering the energy structures of titania are modified primarily by adjusting the crystal structures and surface properties by using different strategies such as elemental doping and composite materials. Hence, this review mainly focuses on the novel strategies adopted for the realization of improved titania-based nanocomposite products for environmental applications.
