Abstract

We aimed to assess the role of FGF21 in metabolic dysfunction‐associated steatotic liver disease (MASLD) at a multi‐scale level. We used human MASLD pathology samples for FGF21 gene expression analyses (qPCR and RNAseq), serum to measure circulating FGF21 levels and DNA for genotyping the FGF21 rs838133 variant in both estimation and validation cohorts. A hepatocyte‐derived cell line was exposed to free fatty acids at different timepoints. Finally, C57BL/6J mice were fed a high‐fat and choline‐deficient diet (CDA‐HFD) for 16 weeks to assess hepatic FGF21 protein expression and FGF21 levels by ELISA. A significant upregulation in FGF21 mRNA expression was observed in the liver analysed by both qPCR (fold change 5.32 ± 5.25 vs. 0.59 ± 0.66; p= 0.017) and RNA‐Seq (3.5 fold; FDR: 0.006; p< 0.0001) in MASLD patients vs. controls. Circulating levels of FGF21 were increased in patients with steatohepatitis vs. bland steatosis (386.6 ± 328.9 vs. 297.9 ± 231.5 pg/mL; p= 0.009). Besides, sex, age, A‐allele from FGF21, GG genotype from PNPLA3, ALT, type 2 diabetes mellitus and BMI were independently associated with MASH and significant fibrosis in both estimation and validation cohorts. In vitro exposure of Huh7.5 cells to high concentrations of free fatty acids (FFAs) resulted in overexpression of FGF21 (p< 0.001). Finally, Circulating FGF21 levels and hepatic FGF21 expression were found to be significantly increased (p< 0.001) in animals under CDA‐HFD. Hepatic and circulating FGF21 expression was increased in MASH patients, in Huh7.5 cells under FFAs and in CDA‐HFD animals. The A‐allele from the rs838133 variant was also associated with an increased risk of steatohepatitis and significant and advanced fibrosis in MASLD patients.

2. One‐step non‐invasive diagnosis of metabolic dysfunction‐associated steatohepatitis and fibrosis in high‐risk population

Abstract

Type 2 Diabetes mellitus (T2DM), age, and obesity are risk factors for metabolic dysfunction‐associated steatotic liver disease (MASLD). We aimed to assess the performance of non‐invasive tests (NITs) for the diagnosis of metabolic dysfunction‐associated steatohepatitis (MASH) and fibrosis in high‐risk subjects. Multicentre cross‐sectional study that included 124 biopsy‐proven MASLD in more than 50 years‐old patients with overweight/obesity and T2DM. Vibration‐controlled transient elastography, Fibrosis‐4 index (FIB‐4), Non‐alcoholic fatty liver disease fibrosis score (NFS), OWLiver Panel (OWLiver DM2 + Metabolomics‐Advanced Steatohepatitis Fibrosis Score ‐MASEF) and FibroScan‐AST were performed. Sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and area under the receiver operating characteristic curve (AUC) were calculated. NITs were assessed individually and in sequential/parallel combinations. 35 (28.2%) patients had early MASH and 66 (53.2%) had MASH with significant fibrosis (at‐risk MASH). The OWLiver Panel correctly classified 86.1% as MASH, showing an accuracy, sensitivity, specificity, PPV, and NPV of 0.77, 0.86, 0.35, 0.85, and 0.36, respectively. Class III obesity, diabetes control, or gender did not impact on the performance of the OWLiver Panel (p> 0.1). NITs for at‐risk MASH showed an AUC > 0.70 except for NFS. MASEF showed the highest accuracy and NPV for at‐risk MASH (AUC 0.77 [0.68–0.85], NPV 72%) and advanced fibrosis (AUC 0.80 [0.71–0.88], NPV 92%). Combinations of NITs for the identification of at‐risk MASH did not provide any additional benefit over using MASEF alone. One‐step screening strategy with the OWLiver Panel has high accuracy to detect MASH and at‐risk MASH in high‐risk subjects for MASLD.

6. Self-doubt in Nursing Students

Abstract

Self-doubt may negatively affect the development of prelicensure nursing students as they prepare to become practice-ready members of the workforce. Walker and Avant's method of analysis was used to explore generalized self-doubt as well as within the context of a contemporary nursing education framework: Aller's Development of Decision-Making and Self-Efficacy Model (ADD-SEM). Results indicate that several attributes of generalized self-doubt, as a form of low psychological capital, were consistent with this construct of the ADD-SEM and should be considered as nurse educators strive to ensure new nurses are prepared for the complexity of today's health care systems.

9. Early Prelicensure Nursing Students' Perceptions of Case Studies Through the Lens of the Clinical Judgment Measurement Model.

Abstract

Background: Research indicates new nursing graduates struggle with clinical reasoning, despite some related skill development throughout their education. The question remains, "Could an earlier focus on clinical reasoning in nursing programs enhance nursing students' clinical reasoning?" Purpose: This pilot study investigated how first-semester nursing students perceive the use of case studies in clinical reasoning skill development and the perceived benefits of case studies related to the NCSBN Clinical Judgment Measurement Model (NCJMM). Methods: Case studies and written reflections were introduced to a convenience sample (n = 45) of first-semester nursing students. Qualitative analysis was conducted on written reflections. Results: Students found case studies enhanced their knowledge and prepared them for clinical practice as a nurse. Conclusions: Findings suggest early introduction of case studies is a meaningful teaching strategy to develop clinical reasoning aligned with the NCJMM. [ABSTRACT FROM AUTHOR]

Abstract

ObjectiveHyperferritinaemia is associated with liver fibrosis severity in patients with metabolic dysfunction-associated steatotic liver disease (MASLD), but the longitudinal implications have not been thoroughly investigated. We assessed the role of serum ferritin in predicting long-term outcomes or death.DesignWe evaluated the relationship between baseline serum ferritin and longitudinal events in a multicentre cohort of 1342 patients. Four survival models considering ferritin with confounders or non-invasive scoring systems were applied with repeated five-fold cross-validation schema. Prediction performance was evaluated in terms of Harrell’s C-index and its improvement by including ferritin as a covariate.ResultsMedian follow-up time was 96 months. Liver-related events occurred in 7.7%, hepatocellular carcinoma in 1.9%, cardiovascular events in 10.9%, extrahepatic cancers in 8.3% and all-cause mortality in 5.8%. Hyperferritinaemia was associated with a 50% increased risk of liver-related events and 27% of all-cause mortality. A stepwise increase in baseline ferritin thresholds was associated with a statistical increase in C-index, ranging between 0.02 (lasso-penalised Cox regression) and 0.03 (ridge-penalised Cox regression); the risk of developing liver-related events mainly increased from threshold 215.5 µg/L (median HR=1.71 and C-index=0.71) and the risk of overall mortality from threshold 272 µg/L (median HR=1.49 and C-index=0.70). The inclusion of serum ferritin thresholds (215.5 µg/L and 272 µg/L) in predictive models increased the performance of Fibrosis-4 and Non-Alcoholic Fatty Liver Disease Fibrosis Score in the longitudinal risk assessment of liver-related events (C-indices>0.71) and overall mortality (C-indices>0.65).ConclusionsThis study supports the potential use of serum ferritin values for predicting the long-term prognosis of patients with MASLD.

12. Measuring Nursing Student Development Through Computer-Based Simulation Activities.

Background: New nurse attrition jeopardizes patient safety, devastates career plans, and negatively impacts costs to hospitals and patients. Employers and educators recognize the widening transition-to-practice gap in meeting expectations for practice-ready, resilient nurses. Purpose: The purpose of this study was to test the use of computer-based simulation activities (CBSAs) to measure processes in Aller's Development of Decision-Making and Self-Efficacy Model (ADD-SEM). Methods: BSN students (N = 50) in a multisite, cross-sectional study used CBSAs to provide data on decision making (Virtual Patient Lasater's Clinical Judgment Rubric), psychological capital (Nursing Anxiety and Self-Confidence in Decision-Making Scale©), and reflection. Results: Repeated-measures analysis of variance and reflexive thematic analysis revealed that decision-making (DM) scores were highest in noticing and lowest in interpreting with significant differences in cohorts (DM) (juniors: P < .001; seniors: P = .013) and self-confidence/anxiety (P < .001). Conclusion: The CBSAs are an effective means of measuring student development of DM and psychological capital needed to retain resilient nurses. [ABSTRACT FROM AUTHOR]

13. RS2289487 variation in PERILIPIN gene is a predictor of weight loss and protection against impaired glucose metabolism after a meal-replacement diet in postmenopausal obese females.

Abstract

OBJECTIVE: The PERILIPIN1 (PLIN1) gene encodes an adipocyte-associated protein that modulates weight. The objective was to evaluate the role of the rs2289487 genetic variant of the PLIN1 gene on weight loss and glucose metabolism secondary to a partial meal replacement (pMR) hypocaloric diet. PATIENTS AND METHODS: We conducted an interventional study in 111 postmenopausal obese females with body mass index (BMI) > 35 kg/m2. The subjects received two intakes per day of a normocaloric hyperproteic formula for 12 weeks. RESULTS: After the pMR diet, body weight, (BMI), fat mass, waist circumference, fasting insulin levels and HOMA-IR decreased in both genotype groups. The improvements in these parameters were higher in C allele carriers than in subjects with TT genotype. The percentage of patients who achieved 7.5% weight loss was higher in the C carriers (57.4% vs. 27.6%), (adjusted Odds Ratio 2.14, 95% CI = 1.33-9.40; p = 0.02). The decrease in the percentage of diabetes mellitus or impaired fasting glucose decrease was statistically significant in C allele carriers (30.2% vs. 18.9%; p = 0.01) (OR 0.54, 95% CI = 0.22-0.78; p = 0.02). CONCLUSIONS: The C allele of rs2289487 predicts the magnitude of weight loss resulting from a pMR diet. These adiposity improvements produce a better improvement in insulin resistance and the percentage of impaired glucose metabolism. [ABSTRACT FROM AUTHOR]

Abstract

The P450 enzyme CYP121 from Mycobacterium tuberculosiscatalyzes a carbon–carbon (C–C) bond coupling cyclization of the dityrosine substrate containing a diketopiperazine ring, cyclo(l-tyrosine-l-tyrosine) (cYY). An unusual high-spin (S= 5/2) ferric intermediate maximizes its population in less than 5 ms in the rapid freeze-quenching study of CYP121 during the shunt reaction with peracetic acid or hydrogen peroxide in acetic acid solution. We show that this intermediate can also be observed in the crystalline state by EPR spectroscopy. By developing an on-demand-rapid-mixing method for time-resolved serial femtosecond crystallography with X-ray free-electron laser (tr-SFX-XFEL) technology covering the millisecond time domain and without freezing, we structurally monitored the reaction in situ at room temperature. After a 200 ms peracetic acid reaction with the cocrystallized enzyme–substrate microcrystal slurry, a ferric-hydroperoxo intermediate is observed, and its structure is determined at 1.85 Å resolution. The structure shows a hydroperoxyl ligand between the heme and the native substrate, cYY. The oxygen atoms of the hydroperoxo are 2.5 and 3.2 Å from the iron ion. The end-on binding ligand adopts a near-side-on geometry and is weakly associated with the iron ion, causing the unusual high-spin state. This compound 0 intermediate, spectroscopically and structurally observed during the catalytic shunt pathway, reveals a unique binding mode that deviates from the end-on compound 0 intermediates in other heme enzymes. The hydroperoxyl ligand is only 2.9 Å from the bound cYY, suggesting an active oxidant role of the intermediate for direct substrate oxidation in the nonhydroxylation C–C bond coupling chemistry.

17. Double Bus Provider Asymmetrical Half Bridge Converter With a Resonant DCX-Derived Auxiliary Circuit to Supply a LED-Based VLC Postregulator

Abstract

The wide and settled use of Light-Emitting Diodes (LEDs) in lighting has made Visible Light Communication (VLC) an interesting option. Given the way LEDs are driven in VLC, the Two-Input Buck converter fits perfectly as a post regulator for VLC-LED lamps. Its main drawback is that it needs two input voltages with specific requirements tied to the VLC feature. One needs to be regulated according to the LEDs knee voltage thermal drift, while the other needs to be constant. With the objectives of avoiding complex circuits with cross regulation and using two converters performing the bus provider, the present work proposes a converter with two outputs that meet those voltage requirements. This converter is the integration of an Asymmetrical Half Bridge converter and a resonant unregulated Half Bridge converter. This integration is achieved thanks to the primary side similarity of both converters and the integration of both transformers, achieving a solution with fewer components. In addition, one output can be fully regulated by a standard control variable while the other remains naturally constant and independent. This greatly simplifies the control stage, avoiding cross regulation while fully complying with the application requirements. This work presents the circuit concept and how it relates to the intended application. The principle of operation will be explained and used to define the operational region where the topology meets the application requirements. To validate the proposed topology, a 45-W prototype is built and tested, archiving the output voltage requirements and an overall efficiency near 94%.

18. Stability of N-Heterocyclic Carbene Monolayers under Continuous Voltammetric Interrogation

Abstract

N-Heterocyclic carbenes (NHCs) are promising monolayer-forming ligands that can overcome limitations of thiol-based monolayers in terms of stability, surface functionality, and reactivity across a variety of transition-metal surfaces. Recent publications have reported the ability of NHCs to support biomolecular receptors on gold substrates for sensing applications and improved tolerance to prolonged biofluid exposure relative to thiols. However, important questions remain regarding the stability of these monolayers when subjected to voltage perturbations, which is needed for applications with electrochemical platforms. Here, we investigate the ability of two NHCs, 1,3-diisopropylbenzimidazole and 5-(ethoxycarbonyl)-1,3-diisopropylbenzimidazole, to form monolayers via self-assembly from methanolic solutions of their trifluoromethanesulfonate salts. We compare the electrochemical behavior of the resulting monolayers relative to that of benchmark mercaptohexanol monolayers in phosphate-buffered saline. Within the −0.15 to 0.25 V vs Ag|AgCl voltage window, NHC monolayers are stable on gold surfaces, wherein they electrochemically perform like thiol-based monolayers and undergo similar reorganization kinetics, displaying long-term stability under incubation in buffered media and under continuous voltammetric interrogation. At negative voltages, NHC monolayers cathodically desorb from the electrode surface at lower bias (−0.1 V) than thiol-based monolayers (−0.5 V). At voltages more positive than 0.25 V, NHC monolayers anodically desorb from electrode surfaces at similar voltages to thiol-based monolayers. These results highlight new limitations to NHC monolayer stability imposed by electrochemical interrogation of the underlying gold electrodes. Our results serve as a framework for future optimization of NHC monolayers on gold for electrochemical applications, as well as structure–functionality studies of NHCs on gold.

A randomised trial implementing Enhanced Recovery After Surgery (ERAS) for high complexity advanced ovarian cancer (AOC) surgery (PROFAST) demonstrated a reduction of median length of stay and hospital readmissions when compared to patients managed conventionally. One secondary objective was to determine if an ERAS pathway in the perioperative management of advanced ovarian cancer patients led to cost savings. Secondary objective of a prospective randomised trial of patients with suspected or diagnosed advanced ovarian cancer allocated to conventional or ERAS perioperative management, carried out at a referral centre from June 2014 to March 2018. Treatment was determined by a computer-generated random allocation system. Gross counting was employed to estimate the cost of hospitalisation in wards, intensive care unit (ICU) and surgical care, while micro-costing was used to obtain image and laboratory test costs. Mean costs between trial arms were considered. Sensitivity analyses were performed. Ninety-nine patients (n = 50 ERAS group, n = 49 Conventional group) were included. Mean costs per patient were 10,719€ in the ERAS group and 11,028€ in the conventional group, leading to an average saving of 309€ per patient. These results were based on 96 patients, excluding 3 extreme outliers mainly related with very high ICU costs. Savings, which were significant for hospital ward costs (−33% total; 759€ per patient in first hospitalisation, and 914€ per partient/day of readmission) were found as robust in the sensitivity analysis. 21. Explaining the Decay of Nucleic Acid-Based Sensors under Continuous Voltammetric Interrogation

Abstract

Nucleic acid-based electrochemical sensors (NBEs) can support continuous and highly selective molecular monitoring in biological fluids, both in vitro and in vivo, via affinity-based interactions. Such interactions afford a sensing versatility that is not supported by strategies that depend on target-specific reactivity. Thus, NBEs have significantly expanded the scope of molecules that can be monitored continuously in biological systems. However, the technology is limited by the lability of the thiol-based monolayers employed for sensor fabrication. Seeking to understand the main drivers of monolayer degradation, we studied four possible mechanisms of NBE decay: (i) passive desorption of monolayer elements in undisturbed sensors, (ii) voltage-induced desorption under continuous voltammetric interrogation, (iii) competitive displacement by thiolated molecules naturally present in biofluids like serum, and (iv) protein binding. Our results indicate that voltage-induced desorption of monolayer elements is the main mechanism by which NBEs decay in phosphate-buffered saline. This degradation can be overcome by using a voltage window contained between −0.2 and 0.2 V vs Ag|AgCl, reported for the first time in this work, where electrochemical oxygen reduction and surface gold oxidation cannot occur. This result underscores the need for chemically stable redox reporters with more positive reduction potentials than the benchmark methylene blue and the ability to cycle thousands of times between redox states to support continuous sensing for long periods. Additionally, in biofluids, the rate of sensor decay is further accelerated by the presence of thiolated small molecules like cysteine and glutathione, which can competitively displace monolayer elements even in the absence of voltage-induced damage. We hope that this work will serve as a framework to inspire future development of novel sensor interfaces aiming to eliminate the mechanisms of signal decay in NBEs.

22. Bovine campylobacteriosis in bulls: insights in the conventional and molecular diagnosis

Abstract

Campylobacter fetusis a gram-negative motile bacterium, with two subspecies relevant to cattle health: C. fetussubsp. venerealis(Cfv) and C. fetussubsp. fetus(Cff). Both subspecies are associated with reproductive losses in cattle. In this study, we evaluated the identification of C. fetusfor the diagnosis of bovine campylobacteriosis through bacteriological culture, direct immunofluorescence (DIF) and molecular tests in preputial smegma (PS) samples of three Angus bulls challenged with Cfv, Cfv biovar intermedius (Cfvi) or Cff, respectively, in an experiment imitating the natural infection. Two DNA extraction protocols were tested (in-house thermal extraction and commercial kit). Aspiration and scraping collection for PS were compared by conventional tests. Additionally, bacteremia was also evaluated in blood samples. Bulls were challenged by natural mating with heifers that had been experimentally infected with C. fetussubspecies; which led to infection. The Cfv- and Cfvi-bulls were positive for at least 9 months. Although Cff is not considered a venereal strain, in this study it was transmissible to bull from heifers experimentally infected, as evidenced by its colonization and persistence in the preputial cavity for 5 to 6 months. This finding suggests a potential risk of dissemination within herds. The results obtained by bacteriological culture or direct immunofluorescence (DIF) showed no significant differences, regardless the sampling device used (aspiration with Cassou pipette, metal and plastic scraper). C. fetusqPCR, on the other hand, yielded better results with an in-house DNA extraction method than with a commercial kit (75% vs 66.6%). Furthermore, qPCR diagnosis was more efficient than culture (66.6%) or DIF (56%). Bacteremia in whole blood samples was negative by qPCR and bacteriological culture in all samples. Altogether, this study demonstrated the transmission of Cff from heifers to bull and also showed that PCR-based methods are promising for the diagnosis of Bovine Genital Campylobacteriosis from clinical samples of PS.

24. Measuring psychological flexibility in autistic adults: Examining the validity and reliability of the AAQ-II, BEAQ, and VQ.

Abstract

Autistic adults, adults who have an autism spectrum disorder diagnosis, are more likely than their non-autistic peers to experience mental health concerns. A growing body of literature supports interventions that target psychological flexibility as a useful approach for reducing mental health concerns in autistic adults. Despite this, psychometric evidence on measuring psychological flexibility within this population is scant. Accordingly, we determined the validity and reliability of the Acceptance and Action Questionnaire-II (AAQ-II), Brief Experiential Avoidance Questionnaire (BEAQ), and Valuing Questionnaire (VQ) in measuring psychological flexibility in a sample of 461 autistic adults. Each of the measures demonstrated consistent factor structures and internal consistency as was proposed in the original measurement development articles. Convergent, discriminant, and explanatory validity of the AAQ-II, BEAQ, and VQ aligned with a priori hypotheses. The AAQ-II, BEAQ and VQ obstruction subscale demonstrated redundancy in one another suggesting the benefits of selecting a single measure of psychological flexibilty for research. The VQ Progress subscale demonstrated more explanatory validity for key mental health outcomes compared to the other measures. Overall, the VQ scale might be the most useful of the three measures in measuring psychological flexibility in autistic adults in clinical settings, but each included scale appeared reliable and valid for this population. • ACT is increasingly used to improve autistic adults' mental health. • The AAQ-II, BEAQ, and VQ demonstrated adequate validity, reliability, and sensitivity in autistic adults. • We suggest the use of the VQ in measuring psychological flexibility in autistic adults. 29. Observation of Cation Chromophore Photoisomerization of a Fluorescent Protein Using Millisecond Synchrotron Serial Crystallography and Infrared Vibrational and Visible Spectroscopy

Abstract

The chromophores of reversibly switchable fluorescent proteins (rsFPs) undergo photoisomerization of both the trans and cis forms. Concurrent with cis/trans photoisomerisation, rsFPs typically become protonated on the phenolic oxygen resulting in a blue shift of the absorption. A synthetic rsFP referred to as rsEospa, derived from EosFP family, displays the same spectroscopic behavior as the GFP-like rsFP Dronpa at pH 8.4 and involves the photoconversion between nonfluorescent neutral and fluorescent anionic chromophore states. Millisecond time-resolved synchrotron serial crystallography of rsEospa at pH 8.4 shows that photoisomerization is accompanied by rearrangements of the same three residues as seen in Dronpa. However, at pH 5.5 we observe that the OFF state is identified as the cationic chromophore with additional protonation of the imidazolinone nitrogen which is concurrent with a newly formed hydrogen bond with the Glu212 carboxylate side chain. FTIR spectroscopy resolves the characteristic up-shifted carbonyl stretching frequency at 1713 cm–1for the cationic species. Electronic spectroscopy furthermore distinguishes the cationic absorption band at 397 nm from the neutral species at pH 8.4 seen at 387 nm. The observation of photoisomerization of the cationic chromophore state demonstrates the conical intersection for the electronic configuration, where previously fluorescence was proposed to be the main decay route for states containing imidazolinone nitrogen protonation. We present the full time-resolved room-temperature X-ray crystallographic, FTIR, and UV/vis assignment and photoconversion modeling of rsEospa.

30. Acquired Elastotic Hemangioma-like Change of the Vulva Associated With Lichen Sclerosus

Abstract

Acquired elastotic hemangioma (AEH) is a rare variant of hemangioma that usually presents as an asymptomatic, solitary, slow-growing red plaque on a sun-exposed site of an adult. Ultraviolet radiation can contribute to the pathogenesis of this hemangioma. Lichen sclerosus (LS), a scarring disease, may present a prominent accumulation of elastic fibers in the reticular dermis reflecting a reparative process. Vulvar elastosis, a novel diagnostic entity with fibers similar to solar elastosis, is more common in women 45 yr and older and is related to aging and/or hormonal changes. We herein report for the first time a case of AEH-like change located in the vulva, a sun-protected area, associated with genital LS. An 81-yr-old woman presented with a painful vulvar lesion of 1-yr duration. Clinical examination revealed vulvar LS and 1 cm-flat, erythematous, well-defined plaque with increased consistency located on the left labium minus. Histopathology showed a non-neoplastic proliferation of WT1-positive, small vascular channels surrounded and intertwined by intense elastosis in the reticular dermis. Alpha-smooth muscle actin positive pericytes encircled the vascular channels. The lesion can be understood as a reparative process within an LS with the appearance of epidermal hyperplasia, proliferation of small vascular channels, and hyperplastic elastosis. There is a close link between epidermal hyperplasia and angiogenesis in the formation of this reparative lesion. Recognition of this lesion is crucial to avoid confusion with other significant processes especially Kaposi sarcoma and well-differentiated angiosarcoma.

Abstract

Kainate (KA)-type glutamate receptors (KARs) are implicated in various neuropsychiatric and neurological disorders through their ionotropic and metabotropic actions. However, compared to AMPA- and NMDA-type receptor functions, many aspects of KAR biology remain incompletely understood. Our study demonstrates an important role of KARs in organizing climbing fiber (CF)-Purkinje cell (PC) synapses and synaptic plasticity in the cerebellum, independently of their ion channel or metabotropic functions. The amino-terminal domain (ATD) of the GluK4 KAR subunit binds to C1ql1, provided by CFs, and associates with Bai3, an adhesion-type G protein-coupled receptor expressed in PC dendrites. Mice lacking GluK4 exhibit no KAR-mediated responses, reduced C1ql1 and Bai3 levels, and fewer CF-PC synapses, along with impaired long-term depression and oculomotor learning. Remarkably, introduction of the ATD of GluK4 significantly improves all these phenotypes. These findings demonstrate that KARs act as synaptic scaffolds, orchestrating synapses by forming a KAR-C1ql1-Bai3 complex in the cerebellum.

Abstract

Intercollegiate student-athletes, a distinct sub-population of college students accounting for nearly 500,000 individuals across university campuses, may experience greater mental health issues than their non-athlete counterparts. This study examined the differences in student-athlete mental health and wellness by various biopsychosocial factors. Results indicated that female, minoritized, and Division I athletes reported worse mental health concerns (depression, anxiety, stress) and quality of life than their White, male, other-division peers. Implications and future research needs are discussed.

39. A Contemporary Model for Undergraduate Nursing Education: A Grounded Theory Study.

Abstract

Background: Health care trends including advanced technology, higher patient acuity, and shorter employment orientation have impacted the environment in which new-graduate nurses are entering the workforce. These issues are contributing to new nurses leaving the workforce prematurely. Theoretical foundations for nursing education need to be updated to better prepare graduates for entry into this burdened environment. Purpose: The purpose of this study was to explore, from the student perspective, the process of educational development into professional licensure-ready graduates. Method: Grounded theory was used to explore the developmental process of undergraduate nursing education. Results: Two simultaneous developmental processes emerged: decision-making and self-doubt/self-efficacy. Early in the nursing program, students made very few patient care decisions and were plagued with self-doubt. As knowledge and experience were gained, decisions became based on patient conditions, self-doubt decreased, self-efficacy emerged, and students relayed increased comfort, confidence, and competence. 41. Benthic iron flux influenced by climate‐sensitive interplay between organic carbon availability and sedimentation rate in Arctic fjords

Abstract

Benthic iron (Fe) fluxes from continental shelf sediments are an important source of Fe to the global ocean, yet the magnitude of these fluxes is not well constrained. Processing of Fe in sediments is of particular importance in the Arctic Ocean, which has a large shelf area and Fe limitation of primary productivity. In the Arctic fjords of Svalbard, glacial weathering delivers high volumes of Fe‐rich sediment to the fjord benthos. Benthic redox cycling of Fe proceeds through multiple pathways of reduction (i.e., dissimilatory iron reduction and reduction by hydrogen sulfide) and re‐oxidation. There are few estimates of the magnitude and controlling factors of the benthic Fe flux in Arctic fjords. We collected cores from two Svalbard fjords (Kongsfjorden and Lilliehöökfjorden), measured dissolved Fe2+concentrations using a two‐dimensional sensor, and analyzed iron, manganese, carbon, and sulfur species to study benthic Fe fluxes. Benthic fluxes of Fe2+vary throughout the fjords, with a “sweet spot” mid‐fjord controlled by the availability of organic carbon linked to sedimentation rates. The flux is also impacted by fjord circulation and sea ice cover, which influence overall mineralization rates in the sediment. Due to ongoing Arctic warming, we predict an increase in the benthic Fe2+flux with reduced sea ice cover in some fjords and a decrease in the Fe2+flux with the retreat of tidewater glaciers in other regions. Decreasing benthic Fe2+fluxes in fjords may exacerbate Fe limitation of primary productivity in the Arctic Ocean.

42. Relation of a variant in adiponectin gene (rs266729) with metabolic syndrome and diabetes mellitus type 2 in adult obese subjects.

Abstract

OBJECTIVE: Some studies in the literature indicate that ADIPOQ rs266729 polymorphism functionally regulates adiponectin promoter activity and secondarily adiponectin levels. The aim of the present investigation was to describe the association of rs266729 with diabetes mellitus (DM2), components of Metabolic syndrome (MS) and serum adiponectin levels. PATIENTS AND METHODS: The study involved a population of 1004 adult obese subjects. Measurements of anthropometric parameters, blood pressure, fasting blood glucose, C-reactive protein (CRP), insulin concentration, insulin resistance (HOMA-IR), lipid profile, adipokines levels and prevalence of MS and DM2 were recorded. The genotype of ADIPOQ gene polymorphism (rs266729) was evaluated. RESULTS: The distribution of the rs266729 polymorphism in this population was 56.7% (n=569) (CC), 33.1% (n=332) (CG) and 10.2% (n=103) (GG). Insulin and HOMA-IR levels were higher in G allele carriers than non G allele carriers. Adiponectin levels were lower in G allele carriers than non G allele carriers. In total group carriers of G allele, logistic regression analysis showed an increased risk of hyperglycaemia (OR=1.70, 95% CI=1.19-2.76, p=0.03) and prevalence of diabetes mellitus type 2 (OR=1.81, 95% CI=1.13-5.14, p=0.04), after adjusting by body mass index and age CONCLUSIONS: G allele of SNP (rs266729) of the ADIPOQ gene showed high values of insulin and HOMA-IR, and low values of adiponectin levels than non G allele carriers. Abstract

OBJECTIVE: Some studies have demonstrated that the allele A of FTO rs9939609 is related to both higher waist circumference and body mass index. Subsequently, some designs related biochemical variables and body weight changes with this genetic variant. We decide to analyze the effects of rs9939609 genetic variant of FTO gene on metabolic parameters and weight loss secondary to partial meal replacements hypocaloric diets (pMRHDs) in obese subjects. PATIENTS AND METHODS: This was a non-randomized, single-treatment study with a formula-diet in 44 obese subjects. The patients received nutritional education and a pMRHDs with two intakes of normocaloric hyperproteic formula during 12 weeks. Anthropometric parameters and biochemical profiles were measured at basal time and after 12 weeks. The variant of FTO gene rs9939609 was determined. RESULTS: Genotype distribution (n=44) was (16 TT (36.4%), 17 TA (38.6%) and 11 AA (25.0%)). After the pMRHD, body weight, body mass index (BMI), fat mass, waist circumference, serum leptin levels and systolic blood pressure improved in both genotypes without statistical differences in both branches. After dietary intervention with pMRHD, subjects with A allele showed a significant improvement in total cholesterol levels (TT vs. TA+AA) (-3.8±1.4 md/dL vs. -12.6±1.7 mg/dl: p=0.01), LDL-cholesterol (-0.2±1.5 md/dL vs. -10.5±1.9 mg/dl: p=0.02), insulin levels (-1.9±0.2 mU/L vs. -3.8±0.3 mU/L: p=0.02) and HOMA-IR (-0.6±0.2 units vs. -1.1±0.1 units: p=0.01). Abstract

The desire to improve and decentralize diagnostic platforms to facilitate highly precise and personalized medicine has motivated the development of a large number of electrochemical sensing technologies. Such a development has been facilitated by electrochemistry’s unparalleled ability to achieve highly specific molecular measurements in complex biological fluids, without the need for expensive instrumentation. However, for decades, progress in the field had been constrained to systems that depended on the chemical reactivity of the analyte, obstructing the generalizability of such platforms beyond redox- or enzymatically active clinical targets. Thus, the pursuit of alternative, more general strategies, coupled to the timely technological advances in DNA sequencing, led to the development of DNA-based electrochemical sensors. The analytical value of these arises from the structural customizability of DNA and its ability to bind analytes ranging from ions and small molecules to whole proteins and cells. This versatility extends to interrogation methods, as DNA-based sensors work through a variety of detection schemes that can be probed via many electroanalytical techniques. As a reference for those experienced in the field, and to guide the unexperienced scientist, here we review the specific advantages of the electroanalytical methods most commonly used for the interrogation of DNA-based sensors. Abstract

Few studies conducted in primary care setting report about age-adjusted prevalence rates of erectile dysfunction (ED). Aims of SIMETAP-ED study were to determine crude and age-adjusted prevalence rates of ED diagnosis, to compare these rates with other similar studies, and to compare prevalence rates of cardiovascular risk factors (CVRF), cardiovascular diseases (CVD), metabolic diseases and chronic kidney disease (CKD) between populations with and without ED.

47. Chemical Equilibrium-Based Mechanism for the Electrochemical Reduction of DNA-Bound Methylene Blue Explains Double Redox Waves in Voltammetry

Abstract

Methylene blue is widely used as a redox reporter in DNA-based electrochemical sensors and, in particular, it is the benchmark DNA-bound reporter used in electrochemical, aptamer-based sensors (E-ABs). Our group recently published an approach to interrogate E-ABs via cyclic voltammetry, which uses the cathodic to anodic peak-to-peak voltage separation (ΔEP) from methylene blue to report on the electron-transfer kinetics and binding state of these sensors. Although effective at scanning rates ≤10 V·s–1, the method is limited at faster scanning rates because cyclic voltammograms of methylene blue-modified, electrode-bound DNA present double faradaic waves that prevent the accurate estimation of ΔEP. These double waves have been observed in previous works, but their origin was unknown. In response, here we investigated the origin of these redox waves by developing a numerical model that incorporates methylene blue’s chemical equilibria in phosphate buffer to predict the shape and magnitude of cyclic voltammograms with 85% or better accuracy from single- and double-stranded DNA. Our model confirms that the peak splitting observed at scanning rates >10 V·s–1originates from the protonation equilibrium of the radical intermediate species formed after methylene blue receives the first electron. Moreover, the model reveals a strong interaction between the proton transferred during the reduction of methylene blue and the chemical make of blocking self-assembled monolayers typically used in the fabrication of E-ABs. This interaction affects the apparent rate of the first electron-transfer step, accelerating or decelerating it depending on the hydrophobicity and polarity of the blocking monolayer. By expanding our understanding of the effect that monolayer chemistries have on methylene blue’s protonation rates and E-AB signaling, this work may serve the rational design of future sensors with tunable electron-transfer kinetics.

50. Taking Advantage of the Sum of the Light in Outphasing Technique for Visible Light Communication Transmitter

Abstract

Visible light communication (VLC) takes advantage of the widespread use of the light-emitting diodes (LEDs), and by modifying the driver stage, the LEDs are capable of lighting and transmitting information. One of the main drawbacks is the low power efficiency due to the modification of the LED driver stage in order to add the communication capability. Most of the research work related to VLC is toward the communication task, whereas there is a limited work about the improvement on power efficiency. This article proposes a high-efficiency LED driver for VLC working as a transmitter based on the outphasing technique. This technique is used also in radio frequency (RF) communications and increases the efficiency of the amplifiers. The proposed transmitter is made up of two switching-mode power amplifiers that reproduce the signals required for the outphasing technique and a dc/dc converter that biases the LEDs. The proposal exploits the light and, instead of being added electrically, the signals are added in their light form, which leads to a reduction in the complexity of the design. As experimental results, a transmitter was built of two Class E amplifiers reproducing a 16-QAM modulation, achieving a signal-generation efficiency of 78% and an overall efficiency of 92%.
