Introduction.: High-quality patient care requires that providers understand and optimize each health care team member's roles and responsibilities. Review of Literature.: The purpose of this paper was to examine the contemporary knowledge and perceptions of physical therapist (PT) practice and education held by advanced practice interprofessional (IP) team members, including physicians, physician assistants (PAs), and advanced practice registered nurses (APRNs). Subjects.: Physicians, PAs, and APRNs licensed in the state of Minnesota. Methods.: Eligible individuals were invited to participate in an online survey assessing understanding of PT practice and education. Descriptive and nonparametric statistical analyses were used to evaluate respondent demographics, practice characteristics, and perceptions and knowledge of PT education and practice. Results.: Survey responses were analyzed from 442 respondents including 210 APRNs, 182 physicians, 48 PAs, and 2 clinicians of unknown profession. Results demonstrated that most referring providers did not learn about physical therapy during their academic or posttraining education. Those who interacted with PTs more frequently perceived greater benefit for their patients, tended to refer to PTs more often, and demonstrated greater knowledge of PT practice. Poorly understood areas of PT practice and education included PT practice settings, specialties, salary, level of education, and the ability to see patients through direct access. Discussion and Conclusion.: Multiple areas of poor provider knowledge regarding PT education and practice were identified; many unchanged from the literature in the 1980s. Three main areas of growth were identified from which actionable recommendations are made: increase IP interactions with PTs, address areas of poor understanding of PT education and scope, and maximize shifting perceptions of PTs through education and advocacy. This paper illustrates that PT education and practice are not well understood by referring providers, posing a threat to IP collaboration. Purpose To measure radiologic science professionals' current attitudes toward older adults. Methods The Geriatrics Attitude Scale (GAS) paper survey was distributed to radiology and radiation oncology personnel in a large, single teaching hospital system. The GAS provides a global measure of ageist attitudes using 14 questions and 4 subscales. Demographic information also was collected. Results The total sample (N = 74) comprised radiology and radiation oncology practitioners from rural and nonrural facilities in the health system. Of the 14 questions, 3 yielded significant differences between medical imaging and radiation therapy practitioners. There were no significant effects for gender, race, years of experience, or facility type. Discussion Although significant differences were found between medical imaging and radiation therapy professionals for some of the constructs, both groups had positive attitudes overall toward older adults as measured by the GAS and subscales. Interventions to disrupt ageism should be introduced to make positive shifts in attitudes. Conclusion The United Nations has declared 2021 to 2030 the Decade of Healthy Ageing; people are challenged to change how they think, feel, and act toward aging and older adults. Background: Literature shows that in South Africa there are insufficient resources to meet mental healthcare needs. At general or district hospital level, the non-specialist doctor is often responsible for the holistic assessment and management of mental health service users. Such situations inevitably increase doctors' care load as they are required to treat across disciplines. We highlight the particular challenges faced by a community service (CS) doctor in this context. Methods: The presented case study formed part of a larger project that investigated public mental healthcare provision in the Eastern Cape province. Data were collected through a once-off semi-structured interview with the participant. The interview was transcribed and data analysed by utilising thematic analysis to yield results. Results: The study suggests that the CS doctor experiences being overloaded with duties, and feels overwhelmed in a healthcare context that lacks resources needed for service provision, which may lead to inadequate mental healthcare provision to public health service users. Conclusion: Healthcare facilities in rural parts of the Eastern Cape province are in need of assistance. This in-depth account highlighted the consequences of working on the front line of a disadvantaged and under-resourced health system. The presented account can be interpreted as a cry for help by CS doctors for relevant authorities to improve access and provision of mental healthcare in the area. The need to guarantee public health spending sustainability remains on the governments' agenda, despite constant efforts to improve health sector efficiency and to contain health spending growth. In this paper, we analyse citizens', managers' and health professionals' views concerning the choice of alternative National Health Service (NHS)' financing sources and the selection of priority areas to be financed from public funds. The main novelty of this study is the auscultation and the evaluation of different stakeholders' perspectives concerning health spending decisions, namely, health professionals, managers and the general public. An online questionnaire was used to collect data. Methods include descriptive and inferential statistics, a Pareto graph and a factorial analysis. Our results reveal the preferable additional NHS funding sources are lottery and games of chance and the increase in alcohol and tobacco taxes. The respondents defend that priorities should consider the improvement of the population's health status, namely, considering the universality of access, equity, effectiveness and efficiency, in line with the NHS' mission. Health professionals are also concerned about disease prevention and health promotion. This study is an attempt to bridge the gap between the understanding of strengths and challenges of people with autism to operationalize a strength-based approach to serve people with autism effectively. By virtue of being based on firsthand accounts by parents and practitioners the people who spend prolonged periods with people with autism in natural settings, and are privy to direct observation of abilities and challenges, the study yields in-depth information that addresses a continued gap in the existing literature on the subject. Twenty participants (13 parents and seven practitioners) took part in the study through a semi-structured questionnaire and face-to-face interviews. Data were analyzed following qualitative thematic analysis methodology. Strengths and related challenges experienced were extracted as the broad themes, based on which support needs were derived and future directions recommended. Background Foot ulcers are an important cause of multiple hospitalisation and clinical complications in patients with diabetes. Before hospital discharge, the care transition should be planned across multiple care settings. Many problems occur during care transitions, resulting in negative patient outcomes and unnecessary readmissions. Aim This paper presents a protocol for a qualitative study exploring the experience of care transitions from both patients' and clinicians' perspectives in a regional hospital context in Western Switzerland. Methods design A qualitative descriptive design will be used to solicit patients' and clinicians' perceptions of care transitions. Semi-structured interviews will be conducted with six patients with a diabetic foot ulcer (DFU) and six community nurses. Data will be analysed according to Braun and Clarke's thematic analysis. Discussion Exploring participants' experiences will afford information about their role within the process, any barriers to effective care transitions, and any strategies to overcome. Implication for clinical practice The findings of this study will be pivotal to informing and developing new and enhanced transition planning for people with DFU, with the ultimate goal of improving patient outcomes, reducing hospital readmissions and enhancing cost savings. Emerging infectious diseases have affected every aspect of medical practice and the advent of COVID-19 in 2019 created many challenges for basic science research methods. Face-to-face data collection was extremely problematic to undertake in any healthcare environment. The emergency department (ED) in low-resource environments, specifically, was severely impacted by the research restrictions, the psychological state of the ED staff, and the lack of perceived importance of research in the emergency departments. This paper aims to reflect on the challenges in collecting research data in the ED during the COVID-19 pandemic and provides some suggestions and lessons learnt for other researchers embarking on such a path. Notwithstanding challenges inherent to virtual platforms, video-calling may be an option rather than in-person interviewing and can allow for data collection over a large geographical area, even when social distancing measures are not in place. Occupation-based intervention (OBI) in hand therapy has shown superior benefits in patient-reported performance and physical measures; however, only a few studies have used OBI. We developed a decision-aid to promote the use of an injured hand in the real world (Aid for Decision-making in Occupation Choice for hand; ADOC-H) To investigate the clinical utility of the ADOC-H (paper version) in patients with distal radius fractures. A prospective case series and a clinical survey for occupational therapists. This study comprised a prospective patient case series of 8 patients with distal radius fractures, treated using Volar locking plates, and a clinical survey of 4 experienced occupational therapists. No patient or therapist complaints or drop-outs were reported. Active range of motion (wrist), Grip strength, and Disabilities of the Arm, Shoulder, and Hand scores improved for all patients. The ADOC-H induced 158 activities using the injured hand, with activities of daily living (69.8%) selected earlier in the treatment period, and instrumental activities of daily living (63.3%) selected later. The feedback and case studies suggested that the ADOC-H was useful for patients who were afraid of using the hand and, interestingly, patients who were able to use their hand without pain or other problems. The clinical survey showed that most therapists found the ADOC-H effective in facilitating real-life use of an injured hand. The ADOC-H paper version is an useful tool that can be applied to facilitate patients with distal radius fractures to use their injured hands in real-life settings. • We tested the Aid for Decision-making in Occupation Choice–Hand (ADOC-H). • This case series showed that the ADOC-H is applicable to various real-life settings. • ADOC-H promotes distal radius fracture patients' daily use of the injured hand. Introduction A significant proportion of healthcare that is delivered is wasteful, harmful and not evidence-based. There are many wound care-related guidelines, but their implementation in practice is variable. The Society of Tissue Viability (SoTV) published updated seating guidelines in 2017, but there is a lack of theoretical and conceptual clarity about how these guidelines are being used to inform clinical practice. Therefore, the aim of this paper is to generate a theory that can be used to incorporate the SoTV seating guidelines into policy and clinical practice. Methods We critically reflected on data from an evaluation study using a systems thinking approach and informed by implementation and safety science using the wider literature and our expertise to generate a guideline-implementation theory. Discussion Factors that facilitate or hinder the incorporation of the SoTV guidelines into policy and practice were characterised. We conceptualised the implementation of these guidelines into policy and practice into a Translation or Implementation into Policy or Practice (TIPP) theory with distinct stages, which we called 'liminal spaces'. Knowledge of the guidelines, and the agency or authority to effect change, are key factors in the translation of these guidelines into clinical practice. Conclusion Our theory is that there are liminal spaces in the implementation trajectory of these guidelines into practice, and these have their own characteristics. This theory provides a framework that can be used to underpin guidelines' strategies for embedding skin and wound care guidelines into policy and clinical practice to improve patient care. During the COVID-19 pandemic, the UK press featured headlines that heightened concerns around Do Not Attempt Cardiopulmonary Resuscitation (DNACPR) orders, particularly the use of 'blanket' DNACPR orders applied to older people in care settings. The portrayal of DNACPR may impact professional and public understandings with implications for end-of-life care. To explore the portrayal of DNACPR orders in the general and academic press and consider implications for public and professional understandings and practice. Academic papers and articles published in the general press during the first wave of the COVID-19 pandemic in the UK were retrieved. Those pertaining to the use of DNACPR orders were analysed thematically. Analysis of 179 media articles and 11 professional commentaries identified mixed understandings of DNACPR as indicated within three themes: rationing of acute services, championing autonomy in DNACPR decisions, and communication and trust. The call to 'protect the NHS' marginalised palliative and social care services with DNACPR constructed as a rationing tool. This led to ethical challenges around autonomy, DNACPR decisions, communication and trust. Media coverage of DNACPR orders was contentious and raised questions around the value of life and quality of dying, particularly for vulnerable individuals. DNACPR orders were conflated with frailty, futility and rationing of acute services and the marginalisation of palliative care. Nevertheless, media outputs stimulated advocacy and support for human rights and autonomy. Background: There is a constant search across the globe for optimal healthcare solutions with affordability, accessibility, availability, and quality of healthcare services being the burning issue for mankind. The pandemic has further necessitated the need of use of the Telemedicine platform to address healthcare issues which are also non Covid related. Though Telemedicine has been in use for over two decades in India, there have been various challenges and adoption issues which have not yet made the technology an effective solution to address the current healthcare issues. There has been phenomenal growth in the Information and Communication Technology (ICT) over the last decade and its utilization in the healthcare field. Methods: Internet research on the various adoption strategies by healthcare providers coupled with our own experience for using this technology along with guidelines provided by the information and communication technology providers. The Telemedicine Guidelines of 2020 released by the Ministry of Health and Family welfare, India, provides a framework for the implementation of healthcare delivery through this technology. Results: This paper mentions our telemedicine experience in governmental and private institutes and highlights the implementation challenges of this technology and some solutions that made a difference in the execution. However, we discuss to a larger extent the possible challenges and barriers in the implementation of this technology in India. Conclusion: Despite successful work in the field of telemedicine, it is yet to become an integral part of healthcare system because challenges related to adaptability of healthcare users and lack of proper training to fast growing technologies. The future is going to compel the usage of this kind of technology and it is essential for setting up infrastructure and having trained personnel to man these departments to encash the full potential of the telemedicine technology. This discussion paper considers the role of allied health assistants (AHAs) in speechlanguage pathology and provides information regarding the training, role and value that they may bring to the profession. As part of a larger research study into speech-language pathologists' perceptions of working with allied health assistants, a review of the literature was undertaken utilising the search terms assistant, allied health assistant, speech-language pathology list, speech therapy/ist. Only a small number of papers were found directly addressing allied health assistants in speech-language pathology, raising concern regarding the profession's existing knowledge of working with this vocational group. A narrative summary of findings is presented. Greater understanding of the allied health assistant workforce, including training, skills and tasks they can undertake, is required to ensure the speechlanguage pathology profession is prepared to work with allied health assistants, and to utilise them effectively. Working with allied health assistants is a workforce strategy which may assist the profession to provide a broader range of services to those who need it. However, relatively little is known about allied health assistants in Australian speechlanguage pathology, and the value they can bring to speech-language pathology practice. The long-term care workforce is a significant provider of palliative care. The majority of older people being admitted to long-term care have palliative care needs and many are approaching end of life. The long-term care workforce comprises both registered health care professionals and unlicensed health care workers (UHCW) who provide most direct care. Studies that have examined palliative care competence in long-term care have focused on staff knowledge. However, it is also important to understand staff attitudes, beliefs, and values towards palliative care because these attributes influence behaviours related to care provision. The aim of the qualitative evidence synthesis is to identify and appraise the best available qualitative evidence on the attitudes, beliefs, and values of the long-term care workforce towards palliative care. The review will consider original research that reports qualitative findings of long-term care staff and their attitudes, beliefs, and values regarding palliative care for residents of long-term care facilities. The following databases will be searched for eligible papers: CINAHL, EMBASE, PubMed, PsychINFO, and Scopus. Studies that meet the inclusion criteria by addressing all of the phenomena of interest will be reviewed using the Joanna Briggs Institute (JBI) methodology for systematic reviews of qualitative evidence. Two reviewers will independently assess the studies for methodological quality. The data will be extracted using the standardized JBI SUMARI extraction tool. Specific details about authors and publication date, study design, aims, context, population, cultural and linguistic background, location, main findings, limitations, and conclusions will be extracted and a level of credibility assigned. Categories will be developed from the findings. The findings will be presented diagrammatically and accompanied by a narrative to explain categories and synthesised findings. The review of the literature will synthesis key findings pertaining to the attitudes, beliefs, and values of the workforce providing palliative care to older people in long-term care, beyond what is known about palliative care knowledge in this workforce. The role of communication environments in everyday participation is well accepted within speech-language pathology research and practice. However, the evidence base for strategies to support speech-language pathologists (SLPs) in assessing and responding to environmental and community needs are less clearly understood. This paper provides a case study to illustrate the application of ethnography of communication to understanding and responding to communication needs within a dance class for adults with intellectual disability. The case study utilised observation, participant PEERobservation, reflection, and interviews to explore the perspectives and beliefs of the dance students, dance teacher, and disability support workers. Strategies emerged for enhancing the awareness and capabilities of communication partners and addressing barriers and facilitators to inclusive social and physical environments. This study illustrates the potential of ethnography of communication as a tool to support SLPs to gain insight into everyday communication environments and the social norms that influence participation and interaction in order to enhance communication accessibility. The aim of this study was to explore graduate midwives' experiences of completing a Bachelor of Midwifery online theory course and how that experience led to preparation for practice and future employment as a midwife. This study used a questionnaire, with the core research design having a quantitative component using open-ended questions, via the use of an electronic platform. The main themes emerging from the data that the graduates considered important issues were flexibility, isolation and lack of support. Course completion, experience of online learning/preparation for practice, and recommendations for improvement were identified as areas of importance for graduates of the online midwifery theory course. What is already known about the topic (see Table 3) • Limited literature exists on the experiences of midwifery students and the outcomes relating to the transition from traditional on-campus learning to online modes of educational delivery. • The assessment of graduate's experiences is vital to determine the overall success of programs and future directions towards a more contemporary approach to midwifery education. • Higher education institutions have witnessed a growth in enrolments in online midwifery education, as new technologies provide platforms for learning that are mobile, accessible for rural and remote students, and provide flexibility, work-life balance and is cost effective. Introduction: In response to growing demands on health and social care services there is an emphasis on communities addressing the needs of local populations to improve lives and reduce inequalities. Occupational therapists are responding to these demands by expanding their scope of practice into innovative settings, such as working with refugees, the homeless and residents of nursing homes, and within sport and leisure environments. The benefits of sport are widely acknowledged, and this paper argues that occupational therapists could play a pivotal role in enabling people to participate. Method: This qualitative study drew on a phenomenological approach and used interviews and thematic analysis to explore five occupational therapists' experiences of enabling people to participate in sport. Findings: Findings revealed that participants demonstrated the uniqueness of occupational therapy when enabling people to participate in sport and practised according to their professional philosophy. There were opportunities to reach wider communities and promote the value of occupational therapy by collaborating with organisations, but there were also challenges when working outside of traditional settings. Conclusion: The study emphasises the unique skills occupational therapists can bring to this setting. Wound-related pain in people with chronic lower limb wounds is often underestimated and undertreated by clinicians1,2 (Briggs, Bennett, Closs, & Cocks, 2007; Coutts, Woo, & Bourque, 2008). This paper provides an overview of the findings from a mixed methods study exploring the perceptions and experiences of healthcare practitioners (HCP) on the assessment and management of wound-related pain, in an effort to provide insights into the above deficit. Aim The aim of this study was to explore whether assessments of wound pain are undertaken and what the barriers are to wound pain management, from the perspectives of HCP. Method This study encompassed two research activities (an HCP survey and focus groups). An explanatory, sequential mixed method design combining data from the survey and the focus groups was used for the study. Results At present, there is no consistent method for the assessment and management of wound-related pain. The key barriers to effective management of pain identified here are pain language, the attitudes and beliefs of HCP in relation to pain assessment and a lack of knowledge and skills in evidence-based practice in wound-related pain. Conclusion There is a need for a universal clinical multidimensional wound pain assessment tool that incorporates clinical guidelines and prescriptive pathways for managing wound-related pain. It is projected that by 2051, close to a quarter of the Australian population will be over the age of 65 years, 5% of which will be over the age of 85 years (AIHW, 2021). It is not surprising, then, that palliative care has garnered increasing attention in healthcare aims and practices. The cornerstones of healthy ageing and wellbeing are maintaining optimal health and function for as long as possible and supporting individuals with the utmost dignity and care during loss of function and/or progression to end of life. Health professionals, including speech-language pathologists (SLPs), are integral to support healthy ageing and eventual progression to end-of-life care. It is therefore imperative that health professionals such as SLPs are equipped with the necessary skills to provide effective care to individuals receiving palliative care. This paper explores the role of SLPs in end-of-life care, and what is currently known about the preparedness of SLP graduates to support individuals in palliative care settings. The aim of this review was to examine the literature to identify what is known about midwives' experiences as academics and their perspectives of transition from clinical practice to university. Literature on health care practitioners, particularly nurses' transition from clinical experts to novice teachers suggests the transition to academia is challenging.Challenges reported by research studies have identified stress, uncertainty, and lack of a supportive and structured transition. There was a dearth of primary research evidence of midwives' perspectives. A scoping review using the Johanna Briggs Institute three step approach was employed. First, an initial limited search of Google Scholar and CINAHL was undertaken to identify articles on the topic. Second, the text words contained in the titles and abstracts of relevant articles and the index terms used to describe the articles were used to develop a complete search strategy in databases: EMBASE, MIDIRS, Scopus and PubMed. Finally, a manual search of the reference lists of all included sources of evidence was manually searched for additional studies. Ten papers that included midwives as the participants were located from Australia, The United Kingdom (UK) and the United States of America (USA) between 2006 and 2022. Analysis of the findings led to three themes: Theme 1: Challenges associated with the transition; Theme 2: Dichotomy of identity; and Theme 3: Fear of losing clinical credibility. In most studies midwives formed a subset of the sample meaning that the voices of midwifery academics are minimised by the experiences of other health practitioners. However, in depth details were located from four personal reflections which identified that transition from clinical practice to academia can result in a duality of roles while individuals come to terms with their new reality and overcome their fear of losing credibility as clinicians. New academics from health tend to hold onto their identity as clinicians rather than adopting their new persona as academics. Fear of losing credibility stems from the need to maintain competence for continued professional registration as clinicians. Further research is needed which focuses on midwives as the target sample to validate the research evidence from this scoping review. [ABSTRACT FROM AUTHOR]

The world wide web offers the promise and means of continual development and improved access to speech-language pathology services for people with communication disorders. In this paper we describe practices and possibilities for service provision for this population, using telehealth and emergent virtual worlds. We illustrate these technologies with a particular focus on research and developments for people with communication disorders. We then highlight some of the ethical risks associated with the web in terms of the promotion of non-evidence based practices, client-patient relationships and the storage and access of client data. These concerns are discussed with reference to Speech Pathology Australia’s Code of Ethics, and provide guidance to speech-language pathologists regarding the potential dangers associated with service provision over digital platforms. One of the most important ethical challenges in medical care is the complex process involved in the communications about treatment with individual patients, their informal (eg families) support people, and their formal (eg health professionals) healthcare providers. It is important to recognize that communication about treatment is not a discrete and isolated event, but one that involves a cascade of relationships within a social context, with each communication and context affecting a later possibility (Slomka 1992). We aimed for equal representation of individuals in the proposed triad (patient with condition, unpaid support, and formal support person) from each of the selected groups resulting in patients with a diagnosis (n=6); key support persons (n=5); and, physicians and healthcare providers (n=5). Interviews were held with 16 individuals as part of the designated triad. A primary aim of this paper is to look at environmental contexts, according to the perceptions of patients and care providers that might influence communication. Main themes are discussed as they occurred in the data, with recommendations for care proposed in the conclusion. The study was funded through an internal grant from the University of Manitoba. Diversity characteristics such as culture, sexual orientation, and social situation potentially impact the participation of older individuals in healthcare. An awareness of diversity characteristics and their potential to impact on older people's access to health and aged care by professionals through training is important. This paper outlines the development and initial validation of a survey using adapted Delphi methodology and distribution to 195 health and aged care professionals pre- and post-diversity training. An initial pool of 31 items were developed with an expert reference group and reduced to 21 through exploratory factor analysis. A two-factor solution was extracted, measuring skills and attitudes of professionals relative to providing services to older people with diverse characteristics. Internal consistency of the items yielded a Cronbach alpha of 0.78. T-tests conducted on the pre- and post-survey data showed significantly higher scores on the domains of skills and attitudes following the training. Knowledge was not effectively captured using this method, with true-false or free-recall questions being a more appropriate approach. This tool fills a gap in the evaluation literature, providing an important first step to assess outcomes of diversity training in community aged care.
