The purpose of this position paper by the South African Academy of Family Physicians (SAAFPs) is to inform decision making on human resources for health policy in South Africa and the placement of family physicians (FPs) in the district health system. National policies have been marred by misunderstanding of the roles and contribution of FPs; and there is unhelpful variability in how FPs are positioned in the health services between provinces. In the private sector, medical aid schemes have discriminated against FPs by failing to remunerate them as specialists and to recognise their scope of practice. [ABSTRACT FROM AUTHOR]

• Representatives of five expert panels came together to establish conceptual clarity and consensus for what social determinants of health mean for nursing. • We define the social determinants of health as having six key dimensions: economic stability, education access and quality (including health literacy), health care access and quality, neighborhood and the built environment, social and community context, and planetary conditions. • We provide a specific health policy example for each of the six key areas of the social determinants of health to illustrate how nurses can act to improve population health. • We underscore the crucial need to eliminate systematic and structural racism if equity in planetary health-related quality of life is to be attained. • Our conceptual framework addresses the particular importance of nursing actions at the individual, family, and population levels to promote health policies that advance health equity, anti-racism, and planetary health-related quality of life. The 2019-2020 American Academy of Nursing (Academy, 2019) policy priorities document states that "they have a clear and distinct focus on social determinants of health and uses this lens to advance policies and solutions within each of the three overarching priorities" This consensus paper seeks to establish conceptual clarity and consensus for what social determinants of health mean for nursing, with emphasis on examples of health policies that advance planetary health equity and improve planetary health-related quality of life. Volunteers from five Expert Panels of the Academy met via videoconference to determine roles and refine the focus of the paper. After the initial discussion, the first draft of the conceptual framework was written by the first three authors of the paper and, after discussion via videoconference with all the co-authors, successive drafts were developed and circulated for feedback. Consensus was reached when all authors indicated acceptance of what became the final version of the conceptual framework. A conceptual framework was developed that describes how the social determinants of health can be addressed through nursing roles and actions at the individual, family, and population levels with a particular focus on the role of health policy. The paper provides a specific health policy example for each of the six key areas of the social determinants of health to illustrate how nurses can act to improve population health. Nursing actions can support timely health policy changes that focus on upstream factors in the six key areas of the social determinants of health and thus improve population health. When it comes to pandemic response, preparation can be the key. Between 2020 and 2024, the fast-paced development of COVID-19—often compounded by pubic policies' failures to reflect the latest reality and the public's divergent reactions to the pandemic and the policies—means that society should prepare for exit strategies that can reflect the reality of the pandemic and the interests of the people. Yet oftentimes societies only have one exit strategy with limited scope. This paper investigates the dangers of having only one pandemic exit strategy for pandemics like COVID-19. Analyses were based on a review of the literature on COVID-19 exit strategies and our own research. The PubMed literature search focused on two concepts—"COVID-19″ and "exit strategy"—and was limited to peer-reviewed papers published between 2020 and 2024 in English. A total of 31 articles were included in the final review. Analyses showed that existing studies on COVID-19 exit strategies often focused on using the modelling method to gauge one exit strategy. Exit strategies were often discussed in the context of implementing, easing, or lifting specific pharmaceutical or non-pharmaceutical interventions. Staged and country-wide coordinated exit strategies were also discussed in the literature, both of which were often deemed as comparatively rigorous options compared to single or stand-alone exit strategies. Drawing on the overall development of COVID-19 and our own research, we presented and discussed the importance of having multiple exit strategies that are considerate of all possible pandemic trajectories, diverse interests of the public, and the communication challenges officials might face in introducing or implementing pandemic policies. This paper underscored the importance of having multiple exit strategies for societies to prepare for pandemics. The insights of this study can help inform health policies so that they can more comprehensively and compassionately protect the needs and wants of the "public" in public health, particularly in grave times like COVID-19. The paper focuses on the role of the World Health Organization (WHO) in promoting a healthy world population as a generative and robust idea within health policy. The WHO's health credo transcends national boundaries to promote health globally. It is embedded in norms, values, and standards promulgated by the organization and contributes in shaping the health responses of national governments. Ideational robustness refers to the ability of the WHO to adapt its health credo to changing contexts and circumstances, thus promoting the legitimacy of an international health order. Disturbances, including the Covid-19 pandemic, test the credo's robustness, forcing the WHO to constantly work at reframing ideas to adapt to political forces and competing logics that structure the field of international health. Empirically, the paper is based on an historical analysis of the evolution of the health credo of the WHO since its inception. Qualitative content analysis of secondary sources, such as policy documents, explores how ideational work performed by WHO leaders impacts on the organization's position and legitimacy. Ideational robustness appears to be largely influenced by leadership vision, preexisting organizational structure, and the political economy of international health. Influenza, surpassing all other respiratory diseases in both morbidity and mortality, annually triggers seasonal epidemics responsible for approximately 500,000 global deaths. Mathematical epidemic models serve as valuable tools for forecasting potential outbreaks and predicting the trajectory of the disease. This paper represents a comprehensive SEIRVS model tailored to the context of Influenza transmission dynamics in North Macedonia. In this paper the classical Susceptible- Exposed- InfectiousRecovered (SEIR) model is enhanced by incorporating vaccination and a death compartment while examining their impact on the spread of Influenza through the population. Simulations are conducted using data from the 2022/2023 season, focusing on a case study of North Macedonia. The simulations were conducted utilizing both the actual vaccination rate in N. Macedonia for that season and an increased vaccination rate to observe the influence of vaccination. The simulation results emphasize the need to increase the vaccination rate. "Building back better" post-pandemic, as advocated by the Organisation for Economic Co-operation and Development, could advance the realization of health as a human right. However, the COVID-19 pandemic is more likely to represent a tipping point into a new and even more unequal normal, nationally and internationally, that represents a hostile environment for building back better. This paper begins with a brief explanation of the tipping point concept. It goes on to describe the mechanisms by which the pandemic and many responses to it have increased inequality, and then identifies three political dynamics that are inimical to realizing health as a human right even in formal democracies, two of them material (related to the unequal distribution of resources within societies and in the global economy) and one ideational (the continued hegemony of neoliberal ideas about the proper limits of public policy). With the global population ageing, concerns about the incidence, prevalence and prevention of elder abuse have become increasingly pertinent. This study presents a narrative review of empirical research, investigating the elder abuse prevention implemented in public health policies in three selected countries. The narrative review aims to enrich academic critical discussion about the theme and provide insights for policymakers and professional practitioners working towards elder abuse prevention across different regions. Searches in the electronic database CINAHL, Medline, PsycINFO and Web of Science databases were performed to identify and synthesise studies reporting on research aimed at investigating elder abuse prevention. The studies that handle on all prevention levels, had to be published in a peer-reviewed journal, report on empirical research, and are available in full text in Portuguese, Slovak, English or Norwegian and published within a ten-year period (2012 to 2022) were included. A total of 981 studies were retrieved, of which 13 papers met the inclusion criteria for this narrative review. The evidence indicated that proactive approach within primordial and primary prevention, interdisciplinary cooperation within primary and secondary prevention, and education for healthcare professionals within all forms of prevention were implemented to various extent as elder abuse prevention in Brazil, Norway and Slovakia. There is a need to further enhance public awareness of elder abuse through campaigns and advocacy programs; additionally, the routine screening and assessments by healthcare providers for early detection and intervention in suspected abuse cases require improvement; and also, legal and health policy measures must be updated regularly. Different Council of Europe organs have been attentive and reactive to specific human rights issues in the COVID-19 context, quickly alerting on the risks of inequitable access to quality health care, vaccines, or medicines for vulnerable groups. Yet these reactions have mainly taken the form of nonbinding instruments such as declarations, statements, and recommendations. Although these reactions derive from the interpretation of binding Council of Europe conventions, the observance or implementation of these conventions is not always monitored. Strasbourg judges have on several occasions confirmed that European Convention on Human Rights case law must consider other international instruments, especially those of other Council of Europe organs, in order to interpret the guarantees of the convention. As a consequence, soft law rules can sometimes indirectly acquire binding force when used as an interpretation and implementation tool for binding treaties. In this paper, I examine how Council of Europe organs interpret the principle of equitable access to health care of appropriate quality in the context of a pandemic and whether and how this interpretation is being implemented within the Council of Europe's interpretation of binding treaties such as the Medicrime Convention, the European Social Charter, and the European Convention on Human Rights. The significance of mental health inequities globally is illustrated by higher rates of anxiety and depression amongst racial and ethnic minority populations as well as individuals of lower socioeconomic status. The COVID-19 pandemic has further exacerbated these pre-existing mental health inequities. With rising mental health concerns, arts engagement offers an accessible, equitable opportunity to combat mental health inequities and impact upstream determinants of health. As the field of public health continues to shift its focus toward social ecological strategies, the social ecological model of health offers an approach that prioritizes social and structural determinants of health. To capture the impacts of arts engagement, this paper creates an applied social ecological model of health while aiming to advocate that engaging in the arts is a protective and rehabilitative behavior for mental health. The need to guarantee public health spending sustainability remains on the governments' agenda, despite constant efforts to improve health sector efficiency and to contain health spending growth. In this paper, we analyse citizens', managers' and health professionals' views concerning the choice of alternative National Health Service (NHS)' financing sources and the selection of priority areas to be financed from public funds. The main novelty of this study is the auscultation and the evaluation of different stakeholders' perspectives concerning health spending decisions, namely, health professionals, managers and the general public. An online questionnaire was used to collect data. Methods include descriptive and inferential statistics, a Pareto graph and a factorial analysis. Our results reveal the preferable additional NHS funding sources are lottery and games of chance and the increase in alcohol and tobacco taxes. The respondents defend that priorities should consider the improvement of the population's health status, namely, considering the universality of access, equity, effectiveness and efficiency, in line with the NHS' mission. Health professionals are also concerned about disease prevention and health promotion. A persistent problem in cardiothoracic surgery, as in all of medicine, is when to offer or to withhold expensive technologies. The ethical requirement of balancing harms and benefits is often difficult to achieve. The use of LVADs is an example of such technologies, and when to offer it is explored in this paper. In this paper we argue that to tackle complex issues such as urban health, there is a need not only of understanding the limitation of different policy subsystems, but also of the interplay of the ideas, interests, and institutional arrangements that underpin cross-boundary challenges. This paper unpacks the dynamics of policymaking between public health and spatial planning by adopting boundary-spanning policy regime theory to trace the alignment and divergence of urban health issues across the two policy subsystems in England. Greater Manchester, heralded as an exemplar of collaborative governance in England, is used as a case study to test the strength of and tensions within the urban health policy regime. Recognizing law as a determinant of scarcity in health care is vital. This paper underscores the need for a comprehensive approach to manage scarcity beyond intellectual property, using targeted regulations to promote affordability and counter market distortions. I argue that relying on law solely to ensure democratic deliberations for resource allocation overlooks market failures and economic inequalities that contribute to scarcity. I examine different "legal determinants of scarcity" that can be used, on the basis of the right to health, to improve or positively influence the availability and affordability of health technologies through complementary policies such as direct price control, competitive procurement, competition laws, and public-private partnerships. I conclude by asserting that health care affordability must be a central positive human rights obligation in economic and health policies and that states must strive to diversify their approaches to eliminate persistent economic barriers. In 2022, the global commercial surrogacy industry was valued at approximately US$14 billion. This paper explores the issue of surrogacy to reveal how international human rights standards and labor laws treat reproduction as work, building on previous scholarship analyzing similar framing at the grassroots level in Mexico. I argue that the failure to recognize surrogacy as labor is rooted in three lacunae: (1) contemporary policies and practices around surrogacy globally pay little attention to the well-being and rights fulfillment of surrogates themselves, particularly the economic rights of surrogates; (2) the stigma of surrogacy as sexualized care work results in neglect of the labor rights of surrogates in mainstream economic rights discourses; and (3) relevant international rights law has not yet addressed the economic rights of surrogates, nor has it effectively articulated the interdependent relationship between economic rights and reproductive rights. Lastly, I discuss where reproductive rights and economic rights overlap in existing human rights conventions and standards and what possibilities these offer for articulating the interdependence of reproductive and economic rights and for advancing the labor rights of surrogates. Background: Digital health technologies (DHTs) can facilitate the execution of de-centralized trials that can offer opportunities to reduce the burden on participants, collect outcome data in a real-world setting, and potentially make trial populations more diverse and inclusive. However, DHTs can also be a significant source of electronic waste (e-waste). In recognition of the potential health and environmental impact from DHT use in trials, private and public institutions have recently launched initiatives to help measure and manage this e-waste. But in order to develop sound e-waste management policies, it will be necessary to first estimate the current volume of e-waste that results from the use of DHTs in trials. Materials and Methods: A Web Ontology Language (OWL)-compliant ontology of DHTs was created using a list of 500 DHT device names derived from a mixture of public and private sources. The U.S. clinical trials registry, ClinicalTrials.gov, was then queried to identify and classify trials using any of the devices in the ontology. The ClinicalTrials.gov records from this search were then analyzed to characterize the volume and properties of trials using DHTs, as well as estimating the total volume of individual DHT units that have been provisioned (or are planned to be provisioned) for clinical research. Results: Our ontology-driven search identified 2326 unique clinical trials with a reported "actual" enrollment of 200,947 participants and a "planned" enrollment of an additional 4,094,748 participants. The most-used class of DHTs in our ontology was "wearables," (1852 trials), largely driven by the use of smart watches and other wrist-worn sensors (estimated to involve 149,391 provisioned devices). The most-used subtype of DHTs in trials was "subcutaneous" devices (367 trials), driven by the prevalent use and testing of glucose monitors (estimated to involve 17,666 provisioned devices). Conclusion: Thousands of trials, involving hundreds of thousands of devices, have already been completed, and many more trials (potentially involving millions more devices) are planned. Despite the great opportunities that are afforded by DHTs to the clinical trial enterprise, if the industry lacks the ability to track DHT use with sufficient resolution, the result is likely to be a great deal of e-waste. A new ontology of DHTs, combined with rigorous data science methods like those described in this paper, can be used to provide better information across the industry, and in turn, help create a more sustainable and equitable clinical trials enterprise. Public policy in democratic societies aims to create public value by providing quality public services, regulations and state-organized activities with the ultimate goal of creating a better service towards its citizens. Since policy failure has always been a concern for various scientists and researchers, this paper aims to contribute to improving the quality and efficiency of public policies by providing expertise to state institutions to ensure effectiveness, transparency, inclusiveness and accountability of the entire process. The health system is one of the main pillars of public policy of every country today, and this is especially noticeable after the outbreak of the COVID-19 pandemic in the world, a problem which radically changed public policies and how are they percept. Kosovo as a new state faces various political and state-building challenges. The functioning of the health system is one of those challenges, which bared the main responsibility in dealing with pandemic. Therefore, the main focus of this paper will be to analyze the public policies introduced by health institutions and government bodies as response to the COVID 19 pandemic. In this regard we will try to answer some questions which are going to be raised within the paper such as how satisfied were the citizens with health policies in Kosovo, the readiness of the institutions and the professional staff engaged, knowledge and use of best practices. In order to obtain the information needed to justify the main objectives of the thesis, beside the content analysis and comparative methods, we are going to conduct a survey with officials and various institutional experts that were engaged in planning and implementation of the policies in time of Covid19 pandemic, as well as with people from private sector and that of civil society. Background: Malnutrition remains a public health burden in the Philippines, with the country struggling to meet global nutrition targets. Objective: This study aims to review the Philippine Plan of Action for Nutrition (PPAN) and its progress in addressing malnutrition in the Philippines. Methods: A narrative literature review of the PPAN and its implementation from 1974 to 2022 was conducted, including an analysis of relevant data on the nutrition status and programs for Filipinos. Results: The study found that the PPAN has been largely supportive in addressing malnutrition in the Philippines over the years. However, obstacles hindering the achievement of its nutrition-related goals have been identified. Challenges include inadequate coordination of programs at the local level, concerns regarding data quality, and insufficient training of personnel involved in implementing the PPAN. The results also indicate that the country has made limited progress toward achieving the global nutrition targets set by the Sustainable Development Goals 2030. Conclusion: This study highlights the need to revisit and refine the PPAN to respond to the changing nature of malnutrition in the country. Recommendations to address the remaining gaps in the planning, implementing, and monitoring of the PPAN are discussed. Finally, the paper offers some valuable insights from the experiences of the PPAN that can be applied to other countries facing comparable nutrition contexts. The concept of Institutional Racism (IR) gained emphasis in the late 1990s and the 2000s. It became one of the central terms in the discussion on racism and the Black Population Health. In Brazil, this category hegemonically influences the understanding of racism in health studies and debates. This qualitative case study paper analyzes the incorporation of the IR concept in Brazil in implementing the National Comprehensive Health Policy for the Black Population. Government documents from the Ministry of Health and the Institute of Applied Economic Research were analyzed. Although the formal inclusion of the discussion about IR in the public policy agenda in the early 21st century is a great achievement, apprehending this fact in isolation disregards the dialectical interaction between individual, institutional, and structural forms in the racialization dehumanizing process. Thus, combating IR depends on facing the reasons that lead institutions to reproduce racism and its consequences on access to housing, work, education, and health services and actions. Introduction: The Transplantation of Human Organs Act (THOA) provided legal status to brain death as a form of death in 1994. However, even after three decades, the deceased organ donation rate in India stands at a mere 0.52 per million population, crucially due to a lack of brain death awareness, and an absence of institutionalized protocols for deceased donations. Objectives: By taking the case of the state of Haryana, this article seeks to understand the existing state of brain death awareness among professionals working in the medical community, and identify the implementation challenges that would be involved in instituting a state‑level policy for donations from deceased persons. Materials and Methods: This paper adopts a qualitative approach, utilizing in-depth interviews with doctors in the districts of Rohtak and Charkhi Dadri in Haryana, and transplant coordinators across the state to gather a nuanced understanding of the issue. Results: The research finds that brain death awareness is lacking among doctors, along with the necessary resources for brain death declarations to become practice. There is also an evident district-wise variation in the responses of doctors when it comes to the potential challenges of instituting a brain death declaration standard operating procedure. It offers recommendations for improved implementation of organ transplant policies and scaling up of deceased donations. Conclusion: To unearth the potential that deceased organ donation holds, some immediate measures that can be taken include mapping all the functional organ retrieval facilities across the state and their closest civil hospitals or postgraduate medical institutions with trauma centres, and focusing on the sensitization of the medical functionaries with respect to brain death and its identification. The coronavirus disease (COVID-19) pandemic has tested the mettle of governments across the globe and has thrown entrenched fault lines within health systems into sharper relief. In response to the outbreak of the pandemic, governments introduced a range of measures to meet the growth in demand and bridge gaps in health systems. The objective of this paper is to understand the nature and extent of the changes in health systems triggered by the COVID-19 crisis. The paper examines changes in the role of governments in (1) sector coordination, (2) service provision, (3) financing, (4) payment, and (5) regulations. It outlines broad trends and reforms underway prior to the pandemic and highlights likely trajectories in these aspects in the future. The paper argues that while the pandemic has accelerated changes already underway before the crisis, it has made little headway in clearing the path for other or deeper health policy reforms. The SARS-CoV-2 pandemic crisis is ongoing, and the elderly are still affected by the virus, sometimes with fatal consequences. The political leaders are, inter alia, focussed on protecting citizens, including this vulnerable group. This article aims to describe the response of the Norwegian political leadership, with a focus on the public health measures enacted to protect the elderly during the SARS-CoV-2 pandemic crisis in the years 2020 and 2021. This study has a descriptive qualitative research design using thematic-content analysis as a methodology to evaluate data collected from official documents, national statistics and public health documents from Norway. Statistics show that the elderly mortality rate per 100,000 person-weeks during the first wave of the SARS-CoV-2 pandemic was 2.69 in Norway; during the second wave of the pandemic, it was 1.43. The public health measures the Norwegian political leadership instigated were more effective in containing the crisis when compared to other European countries, and this paper investigates the reasons. The results of our study provide public health policy decision-makers with insights into analytical material showing the usefulness of different measures enacted by the Norwegian government. The Norwegian government's responses to the crisis, such as the public health measures, restrictions, social consequences for vulnerable groups such as the elderly, as well as cross-government and cross-community cooperation, will need further in-depth analysis. Based on a unique set of First Republic archival documents from the State Archive Košice – Spišská Nová Ves, the paper presents the conditions and course of a special state sugar food campaign for children of unemployed and underemployed parents in the first half of the 1930s. The text briefly maps the basic parameters of this special state food measures, the method of transport and distribution of sugar using the example of the socially weak eastern Slovak district of Gelnica. This food aid is also a proof of the active approach of the health and social policy of the first Czechoslovak Republic in the field of nutrition of children coming from the most vulnerable social groups. However, contemporary sources show that the sugar contingent allocated by the state was insufficient for this special sugar rationing campaign. [ABSTRACT FROM AUTHOR]

This paper argues that public addresses of heads of state are critical for public health policy during pandemics. Drawing insights from studies that investigate metaphors and frames in political and public health discourses, it explores how the Philippine government, especially President Rodrigo Duterte, framed COVID-19. In doing so, it hopes to broaden the understanding of how political rhetoric may be constitutive of public health policy. The analysis of the public addresses entailed three interrelated levels: (1) a description of broad historical, social, political, and cultural contexts of public addresses under investigation, (2) an explanation of the communicative situation including the production and consumption of these addresses—processes that mediate between the text and context, and (3) a textual analysis, which substantiates how the discursive patterns are realized through the president's rhetorical choices. Our analysis reveals that the president consistently deployed the rhetorical strategies of (1) enemization, (2) legitimization of the incumbent administration, and (3) dismissal of critics. The configuration of these strategies sustains a binaristic discourse structure that lays blame on a political other while the government asserts its legitimacy during a public health crisis. These rhetorical strategies organize support for public health policy by a populist administration to manage COVID-19. This paper addresses the gap in studies which use democratic policy making frameworks to analyze health policy development in Community Health Improvement Plans (CHIPs). The study describes and analyzes three streams of the tobacco free living policy cycle in a Community Health Improvement Plan implemented in a Mid-Western Region of the United States from 2016 to 2020. The roles of public health interest, economic interests, and democratic participatory rights in Tobacco free health policy making are assessed. The policy making process is assessed within an integrated framework of analysis that weaves pluralist, power elite, critical democratic theories, and participatory governance paradigms into Kingdon's (2003), streams framework which includes problems, policy, and political streams. The results of the analysis are as follows:1. There was a deficit in democratic participation, and a preeminence of state institutions and economic interest in tobacco free living policy development to the disadvantage of public health interests; 2. Both the policy making process and policy outcomes are anticipated and explained by the governance-driven democratization paradigm and elite dominance theories 3. Democracy driven governance paradigm, democratic theories, and critical theories, illuminate the shortcomings of the policy making process and design. These frameworks also provide a pathway for improving the policy practice components of Community Health Improvement Plans. Recent work on health system strengthening suggests that a combination of leadership and policy capacity is essential to achieve transformation and improvement. Policy capacity and leadership are mutually constitutive but difficult to assemble in a coherent and consistent way. Our paper relies on the nested model of policy capacity to empirically explore how health reformers in seven Canadian provinces address the question of policy capacity. More specifically, we look at emerging representations of policy capacity within the context of health reforms between 1990 and 2020. Based on the exploration of the scientific and grey literature (legislation, annual reports of Ministries, agencies and organizations, meeting minutes, press, etc.) and interviews with key informants (n=54), we identify how policy capacity is considered and framed within health reforms A series of core dilemmas emerge from attempts by each province to develop policy capacity for and through health reforms. Wu, Howlett, and Ramesh's understanding of policy capacity has been used to identify generalizable strengths and weaknesses of specific jurisdictions and policy sectors such as health. In an extension of this work, Howlett and Ramesh have argued that the mode of governance of a policy sector accentuates the importance of specific elements of policy capacity. In this paper we focus on the implementation of the System Level Measures Framework (SLMF) in New Zealand that has been specifically focused on health systems improvement and which aimed to do so by fostering network governance at the local level. However, this policy is introduced in a context in which there has been significant contestation regarding which mode of governance--network or hierarchy--is dominant in New Zealand health policy. By exploring three divergent local cases of implementation of the SLMF we develop three arguments that contribute to the literature on policy capacity and health. Firstly, local histories of interorganizational play a crucial role in shaping health policy capacity. Secondly, it is crucially important to understand the dynamics and feedback loops between operational, political, and analytical policy capacity. Network and hierarchical governance are characterized by distinct and contrasting understandings of the content of policy capacity elements and of the way in which they are dynamically related. Thirdly, the key challenge in developing policy capacity compatible with network governance is how to facilitate this capacity when connections between operational, political, and analytical policy capacity fail to fire. The policy capacity framework offers relevant analytical ideas that can be mobilized for health system strengthening. However, the employment of this framework in the health field constitutes a relevant interdisciplinary gap in knowledge. This themed issue explores the relationships between the policy capacity framework and health system strengthening, in a multidimensional and interdisciplinary way, in high-income and low-middle-income countries. This introduction unpacks the dynamic interrelationships between the policy capacity framework and health system strengthening, bringing together common and distinct elements from both fields and summarizing possible relationships between them. The analysis shows that both fields together can increase our knowledge on health policies and system's critical themes and reforms. This challenge could be followed by exploring the convergences between them, as far as concepts/themes (types of capacities and other themes) and levels of analysis are concerned. Although in varied ways, papers in this issue (based on European countries, China, Canada, New Zealand, India, Australia, and Brazil) advance the use of the policy capacity framework for health policy or system strengthening. They give two main interdisciplinary contributions. Critical capacities can be incorporated into the policy capacity framework for the analysis of system strengthening--capacity to adapt, contexts of mixed and complex systems, dynamic view of policy capacity, and policy capacity as a relational power. Policy capacity is contextually interpreted (relative to the problem frame) and dynamic and adaptive (processual and relational), in relation to the properties of a health system, particularly with regard to the existing and developing mixed and complex systems. In 2019 the Australian government released a guiding document for maternity care: Woman-centred care strategic directions for Australian maternity services (WCC Strategy), with mixed responses from providers and consumers. The aims of this paper were to: examine reasons behind reported dissatisfaction, and compare the WCC Strategy against similar international strategies/plans. The four guiding values in the WCC strategy: safety, respect, choice, and access were used to facilitate comparisons and provide recommendations to governments/health services enacting the plan. Maternity plans published in English from comparable high-income countries were reviewed. Eight maternity strategies/plans from 2011 to 2021 were included. There is an admirable focus in the WCC Strategy on respectful care, postnatal care, and culturally appropriate maternity models. Significant gaps in support for continuity of midwifery care and place of birth options were notable, despite robust evidence supporting both. In addition, clarity around women's right to make decisions about their care was lacking or contradictory in the majority of the strategies/plans. Addressing hierarchical, structure-based obstacles to regulation, policy, planning, service delivery models and funding mechanisms may be necessary to overcome concerns and barriers to implementation. We observed that countries where midwifery is more strongly embedded and autonomous, have guidelines recommending greater contributions from midwives. Maternity strategy/plans should be based on the best available evidence, with consistent and complementary recommendations. This paper presents the results of a global data analysis that for the first time ever looks at the exact performance distances when combining groups of quantitative health and health care indicators that stand for performance differences between the health care system performances of different countries in every corner of the world. Thus, by using Aspalter’s new Standardized Relative Performance Index, we can now not just rank all health care systems and health policy outcomes (as before) but can also, which is entirely new, measure them exactly and compare them exactly with one to another and/ or different groups of indicators (dimensions) thereof. The World Health Organization, the Organization of Economic Corporation and Development, and the World Bank have yet to use such an indicator (Aspalter’s Index) that is able to add up and directly compare different variables, groups of variables among each other, and, of course, for example, different groups of countries among each other, while adding to the quality and quantity of overall information and knowledge gathered on the research subject(s) in the case of quantitative data analysis. For a more in-depth analytical purpose, the “Ten Worlds of Welfare Regime Theory” (or “ten worlds theory” in short) has been used to arrive at further conclusions and extra valuable information and knowledge on top of the rich comparative data analysis conducted in this study. This paper tests whether women's descriptive representation in American state legislatures explains variance in policies relevant to women. The relationship between women's representation and policy is estimated, controlling for alternative explanations of policy adoption including learning from neighboring and politically-similar states, internal economic and political conditions, and state demographics. Following prior research, a single equation instrumental variables model is used to link descriptive and substantive representation, but results do not support the use of a model with endogenous covariates. A simpler model specification demonstrates that women's descriptive representation in state legislatures improves economic policy but not health policy outcomes. Political party confounds the relationship between gender and health policy outcomes. This paper contributes to the literature by: Using a broader measure of policy outcomes rather than dichotomous measures, examining women's representation in both executive and legislative branches, demonstrating the role of political party, and linking women's descriptive and substantive representation. This paper aims to describe the implementation process of the Prison Primary Care Teams in Rio Grande do Sul, the frequency of tuberculosis notifications in people deprived of liberty, and their chronological relationship. This documentary research is nested in a descriptive cross- sectional study of historical series. We analyzed the decrees on the qualification of Prison Primary Care Teams, the number, and the year of notification of tuberculosis cases from the National System of Notifiable Diseases from 2014 to 2020. The state of Rio Grande do Sul pioneered prison health municipalization and shared financing, which resulted in the implementation of 45 teams, covering approximately 54.5% of people deprived of liberty a closed regime. The teams notified 5,175 cases of tuberculosis from 2014 to 2020, with a progressive increase in the notification of cases and the implementation of new teams. The strategy of implementing Prison Primary Care Teams driven by the National Comprehensive Health Care Policy for People Deprived of Liberty in the Prison System was crucial for increasing TB diagnosis, notification, and control in Rio Grande do Sul. The authors describe district-specific disability-adjusted life-year (DALY) of lung cancer attributable to ambient particulate matter < 2.5 um in diameter (PM2.5) for Guangzhou city in China, so as to help prioritizing environmental health action from geospatial perspective. Comparative risk assessment and satellite-derived PM2.5 concentrations were used to investigate the spatial-temporal distribution of DALY attributable to ambient PM2.5 for lung cancer. Integrated exposure-response model and kriging model were constructed based on estimated relative risk (RR) from risk assessment. Annual mean PM2.5 increased by 25.9% from 2010 (71.1 ug/m3) to 2013 (89.5 ug/m3). Estimated RRs ranged from 1.37 (95% uncertainty interval [UI]: 1.04 ~ 1.86) to 1.99 (95% UI: 1.12 ~ 3.19) among the districts. For lung cancer, DALY attributable to PM2.5 increased by 26.8% from 2010 to 2013, reaching 43352.7 DALYs (95% UI: 8157.9 ~ 62371.7) in 2013. The paper showed that population residing in highly-polluted and aged districts might suffer a higher relative risk for developing lung cancer. Our validated analysis framework also showed that population suffered from a higher loss of lung cancer DALYs, partly due to the higher PM2.5 exposure in some subareas within the city. We reveal that ambient PM2.5 pollution contributed substantially to lung cancer burden, both locally and sub-locally. Background: The sale and utilization of dietary and fitness supplements in America, with industry revenue totaling 140.3 billion in 2020 alone, has proven significant. Unfortunately, these supplements are not held to high standards of manufacturing or marketing, leading to ethical, financial, and physiological repercussions for consumers. Aim: The aim of this study is to discuss specific examples of a prevalent issue within the supplementation industry; we suggest the implementation of regulatory processes in the sale and marketing of such products. Methods: Studies from 2007 to 2021 which illustrate positive or negative effects of specific supplements based on gross revenue or a high level of publicity were analyzed. Results: Within this paper, we outline potential regulations which could assist in mitigating the negative impact that a lack of oversight has precipitated. These regulations include an initial approval request which reviews supplement ingredients, effects, risks, and therapeutic index. Conclusion: If the proposed regulations are introduced, the data collected via supplement applications may be utilized in classifying the supplement by its risk before it is marketed to the general population with supervision by pharmacists when indicated, ultimately reducing the adverse effects of inappropriate supplementation. Since its foundation in 1944, the International Bank for Reconstruction and Development, soon renamed the World Bank (hereafter, "the Bank"), has shifted its initial goal of reconstructing post-World War II Europe to promoting economic development and alleviating poverty in Africa, Asia, Latin America, and the Caribbean. Bank assistance is provided through loans awarded to countries that agree to policy changes that the Bank deems necessary to achieve its stated goals. Bank policies—dubbed Structural Adjustment Programs (SAPs) and, over time, superseded by Poverty Reduction Strategy Papers—have been criticized for not only failing to ameliorate underdevelopment and poverty, but for exacerbating both. The most recent Bank approach to development and poverty alleviation, the Systematic Diagnostic Reports (SDR), attempts to address these criticisms. We appraise the SDR for six Organization of Eastern Caribbean States (OECS). All share daunting economic and social challenges, including an ongoing epidemic of noncommunicable diseases (NCDs). We argue that, contrary to the Bank's claims, these challenges will continue under, and may even be exacerbated by, the policies the Report demands, and we elaborate on their implications for NCDs in the OECS and for social and health equity in the region. The National Institute of Health (NIH) policy, Consideration of Sex as a Biological Variable (SABV) in NIH-funded Research (2015), focuses on the expectation that researchers account for the influence of SABV in vertebrate animal and human studies and provide a strong justification for single-sex investigations. When SABV is considered in the research design, data analyses, and reporting, the rigor and reproducibility of the research are elevated and inform best practices and precision health for all people. Additional recommendations include the appropriate use of terminology, integration into curricula, intersection with social determinants of health, and application of sex and gender equity guidelines when disseminating research. COVID-19 has resulted in new anxieties about the risks and dangers involved in human mobility and forced governments to simultaneously re-engineer policies for temporary health control and longer-term border-crossing and migration policies; characterized by the sanitization of space and mobility. This special issue considers the policies, including health and non-health measures, that have impacts on migrant workers and migration. While COVID control measures are often phrased in medical language and policy discourses, they often serve multiple goals including political and social control. The papers in this issue cover different places in Asia and the Pacific. We propose the "politics of sanitization" as a conceptual framework to examine the multiple dimensions of state governance and the variegated impacts upon migrants, including: (1) sanitizing space and borders, (2) stigmatization and sanitizing migrants' bodies, (3) sanitizing ethnic borders and the national body, and (4) reorganizing the borders of sanitization and membership of society.
