THE MESSAGE to editors and publishers of medical books and journals is growing more insistent: practice preventive medicine with your paper.The problem is the "brittle book syndrome"—too-rapid decay of paper in publications printed since the mid-1800s, when papermaking techniques changed. A subcommittee of the House of Representatives' Education Committee has been conducting hearings on the problem, and the National Library of Medicine (NLM), part of the National Institutes of Health in Bethesda, Md, is equally concerned.Specifically, the NLM wants medical publishers to use acid-free paper. This is "preventive medicine," according to NLM officials and others concerned with preserving the printed medical word, because 76 million books and countless periodicals on the shelves of libraries around the country are in danger of self-destruction from the acidic content of the paper they are printed on.That 76-million volume estimate for books on all subjects is from the Council on Library

During the past three years, from terminals within Beth Israel Hospital, Boston, 3,654 persons used a computer program called Paper-Chase to search the medical literature. While performing 39,022 searches, these persons had 1,976,421 references displayed and selected 449,690 of them for printing. Throughout this period, the program underwent continual modification. Each year the number of searches increased—from 10,678 to 11,541 to 16,803. Each year the percentage of new users who were unsuccessful decreased—from 16 to 13 to 11. By the end of the third year, 549 people had completed 20 or more searches. Paper-Chase enables people to search the medical literature themselves, and thus provides a new experience in computerized bibliographic retrieval. When made available without charge in a teaching hospital, Paper-Chase is widely used.(JAMA 1983;250:2494-2499)

A 27-year-old woman experienced pruritus, eye and throat irritation, hoarseness, shortness of breath, and fatigue within half an hour of exposure to carbonless copy paper. On two separate occasions, she was purposely challenged in a controlled-blinded fashion with portions of the carbonless copy paper. This resulted on both occasions in contact urticaria of the hand that held the paper and changes in pulmonary function flow-volume loops characteristic of upper airway obstruction. To determine if alterations in prostaglandin (PG) metabolism might explain these findings, plasma PGF2α and thromboxane B2 (both capable of causing these symptoms) were measured before and during the second exposure period. Both PGF2α and thromboxane B2 increased substantially. We conclude that the cutaneous and respiratory symptoms induced by carbonless copy paper were probably related to PG release.(JAMA 1984;252:1038-1040)

PAPERMAKING, as the Chinese developed it almost 2,000 years ago, was a simple process using natural ingredients. The basic procedure involved breaking up raw materials such as straw, leaves, and rags into cellulose fibers. These fibers were suspended in water and formed into felted sheets with a filtering device. The modern paper-pulp industry has modified the basic process with various chemicals to refine and alter the wood pulp. It is a highly technical science requiring both natural and synthetic materials. The composition of the final product varies depending on the intended use, whether it be for writing, printing, wrapping, filtering, or cleaning. We report two cases of recurrent episodes of hoarseness, cough, flushing, pruritus, and rash occurring within 30 minutes of topical exposure to carbonless copy paper. Provocative challenges revealed that alkylphenol novolac resin was the ingredient responsible. Video endoscopy of the larynx was performed and plasma histamine levels were obtained prior to and 30 minutes after cutaneous challenge of a patient with alkylphenol novolac resin. We documented marked laryngeal edema and a sixfold increase in plasma histamine levels after challenge. We conclude that topical exposure to carbonless copy paper may cause mast cell/basophil-mediated acute systemic and potentially life-threatening reactions in susceptible patients.(JAMA 1988;260:242-243)

In the past few years the term psychohistory has been applied to a number of works that try to combine the disciplines of psychology and history. Of course, biographers and historians have always been interested in psychological factors, but perhaps it is only since Freud that psychiatrists have tried to interpret history. Freud's own contributions, especially Totem and Taboo and Moses and Monotheism, are essentially works of imagination and psychoanalytic theory, but they stimulated other writers to serious exploration of the relation of history and psychopathology.Beginning in 1966, several of the prominent workers in this field have met each summer in Wellfleet to exchange ideas. This book includes 13 essays that were presented there. IN APRIL 1981, JAMA published an article reporting an association between spermicide use around conception and congenital disorders.1 The authors were careful to note that "the results should be considered tentative until confirmed by other data." Nevertheless, their findings were taken up by the media and lawyers and treated as if they were established facts. Pregnant women who had used spermicides around conception became alarmed, and spermicide manufacturers were sued by former users who had had malformed infants.Some journals publish exciting positive findings but are loathe to publish subsequent negative investigations that contradict them. JAMA, to its credit, published an editorial the following year explaining why the association should be considered tentative or preliminary,2 along with several studies3,4 that did not find any association between spermicides and birth defects. Author Taro Takemi is one of the most remarkable physicians of this or any other century. Consider these factors:1. Fundamentally educated as a physician, he functioned as a research scientist in prewar and wartime Japan.2. Selected by Emperor Hirohito, he visited Hiroshima within 24 hours after the atomic bomb blast on Aug 6, 1945, to investigate the circumstances. Because of his advice to the Emperor, Japan's involvement in the Second World War was ended.3. After the war, when research in Japan was abolished by the unconditional surrender and subsequent occupation, he became a practicing physician/internist.4. He served as personal physician to six postwar prime ministers of Japan.5. He served as President of the Japan Medical Association for 25 consecutive years, retiring in 1982.6. The randomized clinical trial (RCT) is well established among research methodologies, especially in the evaluation of new drugs. Its use for other problems is often more controversial. Advocates point out the theoretical advantages that make positive results obtained by this method so convincing. They stress the doubtful reliability of clinical observation and of descriptive studies, and, to support their position, they keep a list of harmful practices that have been perpetuated for lack of timely RCTs. Detractors emphasize the expense, the attendant time lags, the unrealistic rigidity of protocols, the lack of representativeness of volunteers, potential ethical problems, and the inability of modest-sized trials to detect important effects. They point out that most of the major advances in 20th-century medicine have not been subjected to randomized trials. To the Editor.—Treating acute psychogenic hyperventilation by rebreathing into a paper bag restores arterial carbon dioxide pressure (PCO2) to normal, and leads to relief of symptoms. We compared the retention of CO2 during rebreathing in three different types of bags to determine their clinical usefulness. A plain paper bag, a paraffin-treated ("oily") paper bag, and a clear plastic bag of the same size (No. 12) were tested. Bag gas was sampled continuously through a small catheter, and CO2 concentration was measured with an infrared CO2 analyzer. With the bag held tightly over the mouth and nose, a subject hyperventilated at a constant rate, and the rise in bag CO2 was recorded. When the plain paper bag was used, CO2 reached a plateau of 43±7 (mean±SD) mm Hg after 15 to 20 breaths. In contrast, CO2 rose slightly faster in the "oily" paper

It might be better if editors wrote about auto mechanics or the assembly of children's toys. Both of these subjects are in great need of additional editorial expertise, whereas writing scientific papers, which is what editors do write about, does not need it. After reading several books about preparing manuscripts, one realizes that there are as many right ways to proceed as there are editors writing.This little volume—one of many books by editors on how to write and publish scientific papers—has little to recommend itself over its competitors. It's a pity, because the volume has been prepared with care and is clearly the product of years of experience. To the Editor.—In the letter "The 'Oily' Paper Bag and Hyperventilation" (229:638: 1974), Dr. Riley mentioned that the increased carbon dioxide pressure when rebreathing from the bag held tightly over the mouth and nose lasts for three to four minutes. During this time, the oxygen concentration must fall to quite low levels, inducing a hazard in patients with coronary artery disease, and probably also in others. It would be of interest to know whether the oxygen pressure was measured.

These two volumes review the advances made in the last century in the care of the mentally retarded, from the mid-19th century to the present. Each section is preceded by an introduction by the three editors. The selection opens with the "Memorial to the Legislature of Massachusetts," written by Dorothea L. Dix in 1843, and ends with a 1971 article that, significantly, bears the title "Will there always be an institution?"Led by Dix's moving memorial, the first group of papers deals with rising humanitarian feelings toward the mentally retarded. Huth has written a gem of a book; it will be invaluable for would-be authors of medical journal articles, particularly authors who wish to prepare scientific manuscripts. Department chairmen should consider presenting each member of their faculty with a copy, and, judging from experience, they should peruse the book themselves. The work, written for US physician-scientists, is authoritative and comprehensive in scope. The logical steps in writing and editing original manuscripts are comprehensively outlined, and the subtleties of producing a polished medical communication are painstakingly explained. Such a book, presenting a full and systematic description of the process of preparing medical articles for publication, has been sorely needed. The prose style is plain, lucid, and to the point. It is clear why Huth is widely regarded as the Dean of US medical editors.To demonstrate the vigor of the book, I will cite just a few Huthian pearls:"Scientific papers

The purpose of this paper is to describe and analyze the gross lesions found in the coronary arteries of United States soldiers killed in action in Korea. The histology will be discussed in detail in a subsequent paper as will such pertinent data as race, body build, and personal habits.MATERIALRecently 300 autopsies were performed on United States battle casualties in Korea. Most of these soldiers were killed in action or suffered accidental death in front line areas. The coronary arteries were carefully dissected in all cases. Laymen may outscore physicians in cardiopulmonary resuscitationIf you must have a heart attack, it might be better to do so on a Seattle street than elsewhere. It seems that many citizens of Seattle may be better trained at cardiopulmonary resuscitation (CPR) than many interns and residents.This comparison is not entirely fair, but it is revealing. Well-trained laymen may in fact score higher on basic CPR skills than house officers in a teaching hospital. The physician in training may have been shortchanged on instruction in this basic medical skill.This suggestion (conclusion is too strong a term) came from a group of three papers presented at the recent meeting of the American Heart Association in Miami Beach. In turn, the papers made three major points:Whatever the level of the rescuer's training, immediate CPR can double survival rate after ventricular fibrillation while lessening the neurological damage.

I believe the editors were trying to achieve the following objectives in soliciting the papers of this volume: (1) to focus on a number of important areas in medicine in which controversy over treatment still exists despite many published studies; (2) to distill key unanswered, specific questions out of these broader areas of therapeutic controversy; (3) to review salient features of design as well as published results relative to the better of relevant studies; (4) to critique these studies in terms of the suitability of experimental design and the consequent clinical believability of results; (5) to arrive at tentative recommendations relative to clinical practice; and (6) to make specific suggestions for experimental clinical studies to resolve lingering controversies.The component papers are all interesting and well written, but not uniform in their emphasis.The broad clinical areas covered and the scope of individual reviews (identified by the aforementioned numbers) are

We assessed the ability of a computerized outpatient medical record (MR) system, the Summary Time-Oriented Record (STOR), to communicate information to clinicians in two randomized single-blind studies. In the first study, physicians were better able to predict their patients' future symptom changes and laboratory test results from outpatient visits to an arthritis clinic when STOR was added to the standard MR than when the standard MR was used alone. In a separate study, the removal of the standard MR did not result in important decrease in the physicians' ability to predict their patients' symptoms and laboratory test results if they had the option of using the full paper record when they thought they needed it. In 134 (26%) of 514 visits, the physicians exercised this option. We conclude that for outpatient visits, the computerized record system STOR operationally added information to that supplied by the full paper MR. This improved flow of information could improve the clinical decision process.(JAMA 1985;254:1185-1192)

THE MODERN era of cerebrovascular disease began in 1946 with a landmark paper by Kubik and Adams.1 A description of antecedent symptoms and those of onset as well as of neurological abnormalities of the final illness was provided for each of the 18 patients. The authors did not discuss the possible importance of antecedent symptoms, ie, attacks, in six patients and probable warning symptoms in four others. These episodes are now called transient ischemic (focal cerebral) attacks (TIA).Review of the LiteratureThe report of the nitrous oxide method for the quantitative determination of cerebral blood flow in man in 1948 by Kety and Schmidt2 opened an era that has seen the publication of thousands of papers about cerebral blood flow in animals and man. The Kety and Schmidt method, ie, Fick principle, has been modified in a number of ways, particularly with technical improvements, including computer analysis.

In 1945, few would have predicted the sustained worldwide interest in, and profound influence on, the surgical treatment of heart disease that would follow the report of three children operated upon for a cyanotic malformation of the heart. The paper was submitted for publication when the patients were only a few months postoperative, probably leading some critics to question whether the report of success was a little premature. The authors acknowledged and defended the promptness of the report as follows: "The results are sufficiently encouraging to warrant an early report."1The paper was not a hastily assembled report of a new procedure but included, first, a methodical exposition on the prevailing theories of the mechanism of cyanosis in congenital heart disease, then a step-by-step review of how Dr Taussig developed the concept of surgically increasing the volume of blood to the pulmonary circulation in cyanotic malformations with pulmonary stenosis

IN THE 1960s a number of "free clinics" were developed in various North American cities.1 These clinics had the common objective of providing medical care and counseling in an atmosphere different from the practitioner's office, hospital clinic, or university dispensary. Usually organized by young physicians and health care activists, they were free in the sense of charging little or nothing for their services; using the word in another sense, they were free of the usual structure of health care provision. An attempt was made in these clinics to keep the patient and the patient's needs paramount, to minimize paper work and bureaucracy, and to make the physician a member rather than director of the health team. Particular emphasis was placed on avoiding judgmental attitudes about venereal disease, pregnancy, birth control, or drug use, and on providing patient education in all these areas. During the recent war an intensive search for new antimalarial drugs was conducted in the course of which thousands of chemical substances were investigated under the auspices of the Office of Scientific Research and Development. Studies were designed to find drugs which might have one or more of the following properties: (a) true causal prophylaxis, (b) ability to effect complete cure following treatment, (c) greater efficacy than quinacrine and quinine in suppression and (d) greater efficacy than quinine and quinacrine in the treatment of acute clinical attacks.The present report is concerned with chloroquine (SN 7618): 7-chloro-4-(4-diethylamino-1-methylbutylamino) quinoline, which is one of a series of new active antimalarial agents. The impact of the article "The Battered-Child Syndrome" by Drs Henry Kempe, Frederic Silverman, Brandt Steele, William Droegemueller, and Henry Silver, which appeared in The Journal on July 7, 19621 —and the speed with which this impact was felt throughout the land —were truly astonishing. Within four years of the publication of this LANDMARK ARTICLE, all but one state had adopted child abuse reporting statutes.2Others had suspected the existence of child abuse, and a few previous articles on this subject had been published in the medical literature. Two things made this article different. First, the title. How can a nation ignore an entity called "the battered-child syndrome"? Second, the paper was submitted to the widely circulated Journal of the American Medical Association so that the majority of practicing physicians read about child abuse and the press quickly took note.This perspective will cover four topics: (1) What

While most of the deaths occurring during labor and in the early puerperium are said to be due to hemorrhage, toxemia, heart disease or other less common obstetric and medical causes, a few fatalities remain unexplained even after careful postmortem examination.1 In the absence of pathologic chemical and anatomic changes such deaths are often considered, without proof, to be on a functional basis. It is the object of this paper to attempt to fill this gap in our knowledge and to point out that some of these obscure deaths form a disease entity with its characteristic causation, symptoms and pathologic changes by (a) presenting the clinical histories of 8 patients to show that they form a distinct group, (b) demonstrating in them a pathologic-anatomic basis for this series of symptoms and (c) demonstrating the experimental production of this disease in animals, the disease in them having a similar symptomatology

Whether it is a stage in the dissection of a cadaver, a pathology specimen, a histologic section, or an unusual finding during a physical examination, the desire to capture an observation, before it fades, is compelling.One young medical student in the late 1920s was uniquely capable of synthesizing the medical knowledge he was accumulating and then of rendering these complex facts in an astonishingly clear pictorial representation. During the first year of operation, the New England Regional Hypothyroidism Screening Program determined the concentration of thyroxine (T4) levels in dried blood on filter paper from 129,028 infants born in Connecticut, Maine, Massachusetts, New Hampshire, and Rhode Island. Specimens from approximately 3,800 neonates were found to have levels of T4 below the lower limit of normal (<6.0 μg/dl) and were assayed for thyroidstimulating hormone (TSH) content. Levels of TSH were elevated (>20 μU/ml) in 31 infants, 23 of whom eventually proved to have unequivocal hypothyroidism. Based on these figures and including two hypothyroid infants who were not tested, the incidence of congenital hypothyroidism in the New England region is approximately 1:5,200 births. The estimation of T4 values supplemented by measurement of TSH values on specimens with low T4 values has proved to be a satisfactory approach to large-scale screening for congenital hypothyroidism.(JAMA 239:2348-2351, 1978)

The other, the offensive operation to secure Canada for the Whig cause, resulted in a disastrous and demoralizing defeat. In both instances medical factors strongly influenced the result.In this paper I explore the major medical aspects of the four phases of the Canadian campaign: Arnold's epic march through the Maine wilderness; Montgomery's offensive against St Johns, Chambly, and Montreal; their joint siege of Quebec; and the doleful retreat into upper New York state. I also suggest several pertinent comparisons with the medical practice and problems of the Continental Army besieging Boston.

The history of the Medical Department of the Continental Army is a chronicle of intrigue, controversy, and sullied reputations. The brief administration of the first Surgeon-General ended inauspiciously with his imprisonment for treason; his successor, John Morgan, was dismissed abruptly by Congress for alleged incompetence; while Morgan's archrival, William Shippen, Jr, who followed him in office, was eventually tried by court-martial for "malpractice and misconduct." Only John Cochran, the fourth Surgeon-General, managed to leave the service with an unblemished record and the admiration of the entire corps. During the Spanish Civil War (1936-1939), one of us (F.B.) worked as a physician and the other (A.B.) organized homes for children who had been evacuated from combat zones. Many of these children were pathetically shocked and grieved. Although we are neither "art therapists" nor psychologists, we felt that describing their experiences, in words or pictures, might help release the children from their memories. We offered them paper and pencils, but it was only several months later that they had recovered enough to be able to draw.1 We attempted to hide these 10 000 drawings when the German army invaded Paris in 1940, but almost all were discovered and destroyed by the Gestapo. Over the next few years, computer-stored medical records will become technically and economically feasible on a broad scale. Hybrid systems that include computer and traditional paper versions of the medical record and obtain their data from existing ancillary service systems will soon be widely available. Completely electronic medical records will follow. However, standards for exchanging clinical information between independent computers are needed to eliminate the reentry or interfacing costs otherwise required to obtain data from computerized ancillary services. Three kinds of benefits may be expected: (1) improved logistics and organization of the medical record to speed care and improve care givers' efficiency, (2) automatic computer review of the medical record to limit errors and control costs, and (3) systematic analysis of past clinical experience to guide future practices and policies.(JAMA 1988;259:3433-3440)

Much has been said and written about the peer review process for scientific journals. A simple search of the MEDIS database reveals 435 articles that contain the term "peer review." The first International Congress on Peer Review in Biomedical Publication, sponsored by the American Medical Association and to be held in 1989 in Chicago,1 will delve into the effectiveness of this process extensively. But there is another side to the peer review process. The administration of a peer review file and its daily use for reviewing manuscripts is complicated.2 Management of such a large body of data has been made somewhat easier through computerization. Our peer reviewer file and manuscript processing procedures were converted from paper to electronics in 1988 through a difficult and tedious process. This paper concerns the treatment in a series of forty-five cases of pernicious anemia in which the patients were given a special form of diet. While the problem of diet in the treatment of pernicious anemia is by no means new, in our opinion its possible importance has not heretofore been generally recognized. In 1863, seven years after the publication of Addison's second, but best known, description of the disease now called pernicious anemia, Habershon1 wrote concerning this condition: "Many patients at an early stage completely recover under the influence of bracing air and a nutrient and stimulating diet." Other early investigators of the disease, as Biermer2 in 1872, and Pepper,3 in 1875, appreciated the desirability of prescribing easily digested foods as a form of medication, but no greater emphasis was placed on the value of diet. It is my purpose in this paper to report three cases and experiments which furnish additional evidence to show that the blood platelets play a part in stopping hemorrhage, and that one type of hemorrhagic disease may be attributed to an extreme reduction in the number of platelets. The cases possibly explain the relief which sometimes follows transfusion in hemorrhagic disease. It is my purpose also to describe a method for studying hemorrhage called the bleeding time, and to describe briefly a simple method for determining the coagulation time.In the cases there was marked hemorrhagic diathesis, a normal coagulation time, and almost an absence of platelets. Transfusion was performed in each case. After transfusion there was a marked increase in the number of platelets and remarkable relief of hemorrhage.
