CIES2020 - XVII Congresso Ibérico e XIII Congresso Ibero-americano de Energia Solar RESUMEN: Los cuadernos abiertos (Open Notebooks), como los que pueden realizarse en el entorno Jupyter, son una herramienta excelente, no solo para documentar los programas que se implementan para realizar tal o cual cálculo, sino también para: a) facilitar la docencia sobre el asunto de que se trate, b) facilitar que terceros verifiquen con facilidad los cálculos realizados, c) posibilitar el cálculo interactivo. En el contexto del proyecto Europeo GRECO, dedicado al desarrollo de la ciencia e innovación responsable (RRI) aplicado al campo de la energía solar fotovoltaica, estamos desarrollando un “Open Notebook” para modelar analíticamente la denominada “célula solar de tres terminales de tipo transistor bipolar de heterounión”. En este trabajo describimos cómo acceder a dicho cuaderno, describimos el modelo utilizado para modelar dicha célula y comentamos algunas de las lecciones aprendidas en relación con su uso y el desarrollo de la ciencia abierta. In the context of the European GRECO project, dedicated to the development of responsible research and innovation (RRI) in the field of photovoltaics, we are developing an “Open Notebook” to analytically model the so-called “three terminal heterojunction bipolar transistor solar cell”. Summary Right ventricular (RV) involvement during acute inferior myocardial infarction is common and considered to be of prognostic relevance. It was speculated that particularly those patients with RV involvement would profit most from the use of thrombolytic therapy and thus are the key patient group to understanding controversial results on the overall clinical benefit of using thrombolysis during acute inferior myocardial infarction. In 200 consecutive patients with acute inferior myocardial infarction we assessed the eligibility for thrombolytic therapy and the prognostic impact of RV infarction in patients eligible or non-eligible for reperfusion therapy. Prognostic analyses were based on the in-hospital period and a 1–6-year follow-up (mean: 36±11 months). When based on ST-segment elevation in V4R, known as a reliable predictor for RV infarction (sensitivity 88%; specificity 78%; diagnostic efficiency 83%), 107 patients (54%) presented with evidence of RV ischemia. Seventy-one patients (36%) were found eligible for thrombolytic therapy. These patients showed a lower mortality (6/71 patients, 8%) and complication rate (22/71 patients, 31%) compared to patients non-eligible for thrombolysis (mortality: 32/129 patients, 25%; P RV infarction is a common complication of acute inferior myocardial infarction, determines an unfavourable inhospital prognosis and identifies patients who will profit most from using thrombolytic therapy.

Tesis (Magíster en Enseñanza del Inglés como Lengua Extranjera) The aim of this study is to explore the use of Facebook as an effective tool to improve EFL earners' writing skills in an informal context. To this end, this research will consider a sample of 10 EFL students of a gth grade in Peñalolen. Santiago. A mixed-method approach, within a qualitative paradigm is employed. For research purposes, documentary analysis and observation will be used to gather information regarding the usefulness of a Facebook Group (FBG). First, a FBG will be defined as a web page created within the Facebook social networking site, based around a reallife interest among learners of a gth grade. Second, Informal writing will be conceptualized within a process approach and it will be understood as a set of visible or tactile signs used to represent units of language in a systematic way that has, as its purpose, communication. Finally, improve will be approached as to raise participants' writing skills to a more excellent quality. To analyze learners' improvements of the written discourse, Participants will take a pre-test and after a month of being working and completing different tasks on a FBG, a post test will be taken. The aim is to contrast and analyze participants' results by using a focused holistic rubric. For instante, this study intends to assess the relevance of using social media to improve learners' writing skills. Patent foramen ovale (PFO) is a relatively frequent remnant of the fetal circulation, causing transient right-to-left shunt. Its potential consequences include arterial hypoxemia and paradoxical embolism. Echocardiographic contrast imaging techniques make the detection of PFO possible during life. Over the past 20 yr, clinical research focused on the association between PFO and cryptogenic stroke, peripheral embolism, decompression sickness, platypnea-orthodeoxia, and fat embolism. However, although right-to-left shunt through a PFO can occur in the presence of normal right-side hemodynamics, patients with pulmonary embolism (PE) and elevated pressures in the right ventricle and atrium may be at particularly high risk to suffer hypoxemia and paradoxical embolism. In a study performed by our group, PFO was diagnosed in 35% of the patients with PE. These patients had an in-hospital death rate of 33% as opposed to 14% in patients without a PFO. Logistic regression analysis demonstrated that the only independent predictors of mortality were a PFO and arterial hypotension at presentation. Patients with a PFO also had a higher incidence of ischemic stroke and peripheral embolism. Moreover, significant differences were observed in arterial oxygen tension depending on the presence or absence of a PFO. Overall, the risk of a complicated in-hospital course was 5.2 times higher in patients with a PFO. Thus, in patients with PE and right ventricular dysfunction, PFO indicates a particularly high risk of death and arterial thromboembolic complications, and it is, in fact, one of the independent predictors of an adverse clinical outcome.

There is increasing evidence for a fatal interaction of myocardial ischemia, ventricular arrhythmias and sudden cardiac death in some patients with coronary artery disease. Evidence comes from autopsy studies, from the evaluation of patients who survived an episode of sudden cardiac death, from follow-up data of these patients either treated or not by revascularization therapy and/or an implantable cardioverter-defibrillator and indicate that reducing the individual ischemic burden will be beneficial to reduce the incidence of sudden cardiac death. Studies in patients with stable and especially with unstable angina using Holter monitoring could demonstrate that there is a close and causal relationship between myocardial ischemia inducing or aggravating life-threatening ventricular arrhythmias and sudden cardiac death particularly in patients with unstable and postinfarction status. This review summarizes some of our clinical knowledge on this topic and indicates that preventive strategies for myocardial ischemia are the antiarrhythmic treatment of choice in patients with severe coronary artery disease and patients with evidence or at risk for ischemic proarrhythmia.

Ketanserin and calcium antagonists are frequently used for the treatment of arterial hypertension in the elderly. The possibility that combined treatment with ketanserin and the calcium antagonist nifedipine have a pro-arrhythmic effect was investigated in 20 normal volunteers aged60 years with normal or slightly elevated blood pressure. Each subject received monotherapy with ketanserin or nifedipine for 1 week and the combined treatment during the following week. Clinical and biochemical parameters, ECG and 24-hour ECG recording were monitored before and at the end of the first and second treatment weeks. Ketanserin and nifedipine given in monotherapy or in combination did not modify, on average, blood pressure, heart rate, the biochemical variables and the QT interval. In the 24-hour ECG recordings, 2 normal subjects developed a marked increase in the frequency of ventricular ectopics, couplets and ventricular tachycardia after combined treatment. Therefore, the present investigation does not exclude the possibility that combined treatment with ketanserin and nifedipine could increase the prevalence of arrhythmia in some elderly patients.

The antiarrhythmic efficacy and safety of oral flecainide were assessed during a controlled 2-week and a subsequent 48-week long-term trial. Fifteen patients with frequent (more than 30 per hour) and complex ventricular arrhythmias (Lown grade IVA or IVB) who had been resistant or intolerant to 2 or more antiarrhythmic agents, were included in the study. Antiarrhythmic efficacy was controlled by 24-hour Holter monitoring at 2, 12, 24 and 48 weeks. The administration of 100 to 200 mg flecainide twice daily resulted in more than 90% suppression of VPCs and of complex ventricular arrhythmias in 14 of 15 patients. The minimum effective therapeutic dose could be titrated in 9 of 14 patients to 100 mg twice daily, in 3 of 14 patients to 150 mg twice daily and in 2 of 14 patients to 200 mg twice daily. During this therapy and a mean plasma concentration of 886 ± 103 ng/ml, PQ and QRS duration, as well as QTc time and JTc interval were not significantly changed. Side effects (gastrointestinal complaints, nausea, obstipation, dizziness, visual disturbances, headache and impaired potency) were seen in 5 of 14 patients after 12 weeks, in 3 of 4 patients after 24 weeks and in only 2 of 14 patients after 48 weeks. Side effects were described as mild and tolerable and did not limit flecainide therapy except in 1 patient, who had discontinued therapy with flecainide after 3 days because of intense gastrointestinal symptoms. In conclusion, flecainide is highly effective and well tolerated in the long-term treatment of serious ventricular arrhythmias.

Meinertz et a1. (1984) investigated the long term efficacy and tolerability of flecainide in patients with ventricular arrhythmias. 15 patients were studied, 8 women and 7 men, aged 19 to 74 (mean 58.7) years, with more than 30 ventricular premature complexes (VPCs) per hour, and ventricular arrhythmias of Lown grade IVA or IVB. 11 had coronary artery disease and 2 had idiopathic dilated cardiomyopathy. Patients were excluded on the basis of sustained ventricular tachycardia (VT) [defined as ~ 3 consecutive beats at a rate of> 100 beats/min]; myocardial infarction within the last 6 months; unstable angina; severe congestive heart failure; or conduction abnormalities. Previously unsuccessful antiarrhythmic treatments included: mexiletine (13 patients), propafenone (11), disopyramide (9), lignocaine (lidocaine) [4], lorcainide (3) and ajmaline (5).

Aims Current long QT syndrome (LQTS) therapy, largely based on beta-blockade, does not prevent arrhythmias in all patients; therefore, novel therapies are warranted. Pharmacological inhibition of the serum/glucocorticoid-regulated kinase 1 (SGK1-Inh) has been shown to shorten action potential duration (APD) in LQTS type 3. We aimed to investigate whether SGK1-Inh could similarly shorten APD in LQTS types 1 and 2. Methods and results Human-induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs) and hiPSC-cardiac cell sheets (CCS) were obtained from LQT1 and LQT2 patients; CMs were isolated from transgenic LQT1, LQT2, and wild-type (WT) rabbits. Serum/glucocorticoid-regulated kinase 1 inhibition effects (300 nM–10 µM) on field potential durations (FPD) were investigated in hiPSC-CMs with multielectrode arrays; optical mapping was performed in LQT2 CCS. Whole-cell and perforated patch clamp recordings were performed in isolated LQT1, LQT2, and WT rabbit CMs to investigate SGK1-Inh (3 µM) effects on APD. In all LQT2 models across different species (hiPSC-CMs, hiPSC-CCS, and rabbit CMs) and independent of the disease-causing variant (KCNH2-p.A561V/p.A614V/p.G628S/IVS9-28A/G), SGK1-Inh dose-dependently shortened FPD/APD at 0.3–10 µM (by 20–32%/25–30%/44–45%). Importantly, in LQT2 rabbit CMs, 3 µM SGK1-Inh normalized APD to its WT value. A significant FPD shortening was observed in KCNQ1-p.R594Q hiPSC-CMs at 1/3/10 µM (by 19/26/35%) and in KCNQ1-p.A341V hiPSC-CMs at 10 µM (by 29%). No SGK1-Inh-induced FPD/APD shortening effect was observed in LQT1 KCNQ1-p.A341V hiPSC-CMs or KCNQ1-p.Y315S rabbit CMs at 0.3–3 µM. Conclusion A robust SGK1-Inh-induced APD shortening was observed across different LQT2 models, species, and genetic variants but less consistently in LQT1 models. This suggests a genotype- and variant-specific beneficial effect of this novel therapeutic approach in LQTS.
