If a hypothetical elementary particle called an axion exists, to solve the strong CP problem, a 57Fe nucleus in the solar core could emit a 14.4-keV monochromatic axion through the M1 transition. If such axions are once more transformed into photons by a 57Fe absorber, a transition edge sensor (TES) X-ray microcalorimeter should be able to detect them efficiently. We have designed and fabricated a dedicated 64-pixel TES array with iron absorbers for the solar axion search. In order to decrease the effect of iron magnetization on spectroscopic performance, the iron absorber is placed next to the TES while maintaining a certain distance. A gold thermal transfer strap connects them. We have accomplished the electroplating of gold straps with high thermal conductivity. The residual resistivity ratio (RRR) was over 23, more than eight times higher than a previous evaporated strap. In addition, we successfully electroplated pure-iron films of more than a few micrometers in thickness for absorbers and a fabricated 64-pixel TES calorimeter structure., 5 pages, 5 figures, published in IEEE Transactions on Applied Superconductivity on 8 March 2023

Background The effects of empagliflozin in patients with chronic kidney disease who are at risk for disease progression are not well understood. The EMPA-KIDNEY trial was designed to assess the effects of treatment with empagliflozin in a broad range of such patients. Methods We enrolled patients with chronic kidney disease who had an estimated glomerular filtration rate (eGFR) of at least 20 but less than 45 ml per minute per 1.73 m2 of body-surface area, or who had an eGFR of at least 45 but less than 90 ml per minute per 1.73 m2 with a urinary albumin-to-creatinine ratio (with albumin measured in milligrams and creatinine measured in grams) of at least 200. Patients were randomly assigned to receive empagliflozin (10 mg once daily) or matching placebo. The primary outcome was a composite of progression of kidney disease (defined as end-stage kidney disease, a sustained decrease in eGFR to Results A total of 6609 patients underwent randomization. During a median of 2.0 years of follow-up, progression of kidney disease or death from cardiovascular causes occurred in 432 of 3304 patients (13.1%) in the empagliflozin group and in 558 of 3305 patients (16.9%) in the placebo group (hazard ratio, 0.72; 95% confidence interval [CI], 0.64 to 0.82; PConclusions Among a wide range of patients with chronic kidney disease who were at risk for disease progression, empagliflozin therapy led to a lower risk of progression of kidney disease or death from cardiovascular causes than placebo. 5. やせや体重減少のある児童生徒に対する養護教諭の対応頻度と知識 : 小児摂食障害に焦点をあてて

本研究では,次の3点を明らかとするため調査を行った。第一に,養護教諭はやせや体重減少のある児童生徒に対しどのくらい対応しているか。第二に,やせや体重減少に関する知識を持っているか。第三に対応頻度と知識の有無に関係はあるのか,である。その結果,A市養護教諭への調査で次の3点が明らかとなった。①肥満度−20%以下と抽出され,かつ肥満度−30%以下になっていない児童生徒にかかわる養護教諭の対応頻度については,「対応群」は対応の種類によって32.1%~69.8%であり,本人への対応は先行研究と比較して低い対応水準であった。②「知識あり群」は知識の内容によって24.6%~58.5%であり,先行研究と比較し低い知識水準であった。③やせや体重減少による重症度の見極めについて「知識あり群」であったとしても,月経の有無や脈拍の状態という指標を伴うと「知識なし群」と比較し対応頻度に有意な差はみられない,である。これらから,A市の養護教諭にとってやせや体重減少のある児童生徒本人への対応に結びつかない要因があること,A市の養護教諭の身近にある発育評価資料では,肥満度−20%以下と抽出され−30%以下になっていない児童生徒について医療側の求める対応水準・知識水準に到達できないこと,成長曲線に関わる養護教諭向けの資料に月経の有無や徐脈などの観察項目の必要性とその意義について記載するとよいことが示唆された。

We sought to design a data visualization platform to represent the Movement Disorder Society- Unified Parkinson’s Disease Rating Scale (MDS-UPDRS) item scores in an easy-to-use display without modification of the raw data or summary scores. Score items for Parts I, II, and IV were arranged as separate inline blocks, while Part III item blocks were arranged in an anatomical fashion. A color scale was created to represent symptom severity and changes observed from one exam to another. We have found the visualization helpful for quickly defining the most troublesome symptoms and their anatomical location enabling communication of the results and interpretations.

Buruli ulcer is the third most common mycobacterial infection worldwide and is mainly diagnosed in tropical regions. Globally, this progressive disease is caused by Mycobacterium ulcerans; however, Mycobacterium ulcerans subsp. shinshuense, an Asian variant, has been exclusively identified in Japan. Because of insufficient clinical cases, the clinical features of M. ulcerans subsp. shinshuense–associated Buruli ulcer remain unclear. A 70-year-old Japanese woman presented with erythema on her left backhand. The skin lesion deteriorated without an apparent etiology of inflammation, and she was referred to our hospital 3 months after disease onset. A biopsy specimen was incubated in 2% Ogawa medium at 30 °C. After 66 days, we detected small yellow-pigmented colonies, suggesting scotochromogens. Matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI Biotyper; Bruker Daltonics, Billerica, MA, USA) indicated that the organism was Mycobacterium pseudoshottsii or Mycobacterium marinum. However, additional PCR testing for the insertion sequence 2404 (IS2404) was positive, suggesting that the pathogen was either M. ulcerans or M. ulcerans subsp. shinshuense. Further examination by 16S rRNA sequencing analysis, focusing on nucleotide positions 492, 1247, 1288, and 1449–1451, we finally identified the organism as M. ulcerans subsp. shinshuense. The patient was successfully treated with 12 weeks of clarithromycin and levofloxacin treatment. Mass spectrometry is the latest microbial diagnostic method; however, it cannot be used to identify M. ulcerans subsp. shinshuense. To accurately detect this enigmatic pathogen and uncover its epidemiology and clinical characteristics in Japan, more accumulation of clinical cases with accurate identification of the causative pathogen is essential.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spike protein (S protein) is highly N-glycosylated, and a "glycan shield" is formed to limit the access of other molecules; however, a small open area coincides with the interface to the host's receptor and also neutralising antibodies. Most of the variants of concern have mutations in this area, which could reduce the efficacy of existing antibodies. In contrast, N-glycosylation sites are relatively invariant, and some are essential for infection. Here, we observed that the S proteins of the ancestral (Wuhan) and Omicron strains bind with Pholiota squarrosa lectin (PhoSL), a 40-amino-acid chemically synthesised peptide specific to core-fucosylated N-glycans. The affinities were at a low nanomolar level, which were ~ 1000-fold stronger than those between PhoSL and the core-fucosylated N-glycans at the micromolar level. We demonstrated that PhoSL inhibited infection by both strains at similar submicromolar levels, suggesting its broad-spectrum effect on SARS-CoV-2 variants. Cryogenic electron microscopy revealed that PhoSL caused an aggregation of the S protein, which was likely due to the multivalence of both the trimeric PhoSL and S protein. This characteristic is likely relevant to the inhibitory mechanism. Structural modelling of the PhoSL-S protein complex indicated that PhoSL was in contact with the amino acids of the S protein, which explains the enhanced affinity with S protein and also indicates the significant potential for developing specific binders by the engineering of PhoSL.

Floating treatment wetlands (FTWs) use plants' roots for water quality improvement. The plants are supported by a buoyant structure deployed at the water surface. The roots form a porous zone beneath the structure and remove pollutants carried in suspension through filtering, absorption and uptake. This paper used CFD simulation to model FTWs arranged in series and spanning the channel width and to study the effects of root length and spacing between FTWs on flow distribution and mass removal. The root zone was modelled as a porous media, and removal was computed using first-order decay, for which a range of removal constants was tested. Longer roots increased the reactive volume of the root zone, which increased the fraction of pollutant inflow entering the FTWs. Increasing the distance between FTWs allowed greater mixing between water that went through and beneath the upstream FTW. This increased the concentration entering each FTW, which enhanced mass removal per FTW. However, a larger distance between FTWs reduced the number of FTWs in the channel, reducing the reactive volume. In the tradeoff between mixing and reactive volume, the reactive volume was more important, such that total removal in the channel increased with longer roots and more units of FTW (shorter gap distance). However, removing the gap entirely was detrimental, as FTWs in series removed more mass than a continuous FTW of same volume. This study points to two design recommendations for FTWs in series. First, if resources for building FTWs are not limiting, but the channel length is, it is preferable to prioritize higher reactive volume (shorter gap distance) to achieve maximum removal per channel length. Second, if resources for FTWs are limiting, but channel length is not, it is better to place the FTWs with a longer gap distance, preferably along enough to allow mixing over the full depth between FTWs, as this will achieve maximum removal per FTW. 18. A Study of Factors Affecting Febrile Urinary Tract Infection Following Ureterorenoscopic Lithotripsy

Febrile urinary tract infection (f-UTI) is a common complication after ureterorenoscopic lithotripsy (URSL) but is sometimes lethal. In this article, we analyzed the factors of post URSL f-UTI. We retrospectively evaluated the association between the development of f-UTI and patients, stones, and perioperative factors in 695 cases in which URSL was performed at our institution from September 2015 to 2018. Seventy-six of the 695 patients (10.9%) had postoperative f-UTI. Elderly (p=0.013), female (p=0.02), and hypertension (p=0.001) patients had significantly higher rates of f-UTI. Renal stone (p=0.001) cases showed significantly higher rates of f-UTI. Preoperative urine positive culture (p=0.045), preoperative f-UTI (p<0.001), URSL procedure using flexible ureteroscopy (p=0.048), non-stone-free (p=0.006), long operation time (p=0.011), preoperative urinary stent insertion due to preoperative f-UTI (p<0.001), were factors associated with post-operative f-UTI. Multivariate analysis revealed that hypertension (OR=2.08, p=0.008) and preoperative f-UTI (OR=3.739, p=0.033) were independent factors of postoperative f-UTI. Patients with hypertension or preoperative f-UTI should be managed more carefully during the perioperative period, suspecting that they are more likely to develop postoperative f-UTI.

Small flares frequently occur in the quiet Sun. Previous studies have noted that they share many common characteristics with typical solar flares in active regions. However, their similarities and differences are not fully understood, especially their thermal properties. In this study, we performed imaging spectroscopic observations in the H$\alpha$ line taken with the Solar Dynamics Doppler Imager on the Solar Magnetic Activity Research Telescope (SMART/SDDI) at the Hida Observatory and imaging observations with the Atmospheric Imaging Assembly onboard Solar Dynamics Observatory (SDO/AIA). We analysed 25 cases of small flares in the quiet Sun over the thermal energy range of $10^{24}-10^{27}\,\mathrm{erg}$, paying particular attention to their thermal properties. Our main results are as follows: (1) We observe a redshift together with line centre brightening in the H$\alpha$ line associated with more than half of the small flares. (2) We employ differential emission measure analysis using AIA multi-temperature (channel) observations to obtain the emission measure and temperature of the small flares. The results are consistent with the Shibata & Yokoyama (1999, 2002) scaling law. From the scaling law, we estimated the coronal magnetic field strength of small flares to be 5 --15 G. (3) The temporal evolution of the temperature and the density shows that the temperature peaks precede the density peaks in more than half of the events. These results suggest that chromospheric evaporations/condensations play an essential role in the thermal properties of some of the small flares in the quiet Sun, as does for large flares., Comment: 14 pages, 12 figures, accepted for publication in MNRAS

This study examined the longitudinal association between dementia, activity participation, the coronavirus disease 2019 pandemic period, and 1-year mental health changes. We obtained data from the National Health and Aging Trends Study in the United States. We included 4,548 older adult participants of two or more survey rounds between 2018 and 2021. We identified baseline dementia status, and assessed depressive symptoms and anxiety at baseline and follow-up. Dementia and poor activity participation were independently associated with an increased prevalence of depressive symptoms and anxiety. Dementia care and support should address emotional and social needs under continued public health restrictions.

Several methods have been proposed for postoperative pain management, including administration of opioid analgesics, epidural analgesia, and perineural and infiltrative techniques; however, data are lacking on the relationship between pain intensity, patients' age and gender, and surgery duration.Prospective, observational, single-center study.The study included patients greater than or equal to 18 years old who underwent surgery with different anesthesia types, grouped according to the American Society of Anesthesiologists' physical status classification score. The McGill Pain Questionnaire was used to assess postoperative pain intensity. The postoperative pain evaluation was performed in the first 5 minutes on entering the postanesthesia care unit (PACU), and at 30 minutes and 24 hours after the operation.Our results showed a significant negative relationship between pain intensity as assessed at 5 and 30 minutes postoperatively and age. Postoperative pain intensity at 24 hours was significantly lower after low-risk surgeries lasting up to 1 hour; pain intensity was also significantly lower at 30 minutes following epidural anesthesia. When nonsteroidal anti-inflammatory drugs were not administered in the PACU, pain intensity was significantly lower at 5 minutes, 30 minutes, and 3 hours.Postoperative analgesic administration should be conducted in accordance with age and surgery type. Additionally, epidural anesthesia can reduce the immediate postoperative pain intensity.

Th is study aimed to examine the depth of learning in health guidance through health guidance on blood pressure associated with each subject area. Health guidance was created in connection with the study of each subject area. Health Guidance I and II were conducted with junior high school students, and students' descriptions after the guidance were qualitatively analyzed. By using the extracted categories, each of the health guidance was evaluated. In Health Guidance I, questions about blood pressure itself and events that occur in daily life were described. In Health Guidance II, there were some description s that connected the content of health guidance to events that occur to them in their daily lives and some description that connected the content of health guidance to what they had already learned in science, home economics, and health and physical education. It was suggested that by relating the content to other subjects, health guidance could become a place to use knowledge of previously studied topics and deepen learning.

Bullying is still prevalent in Japan, leading to serious problems such as suicides of victims. Although the Ministry of Education, Culture, Sports, Science and Technology (MEXT) has thus far attempted to address this problem, it has resulted in failure. Even the Act for the Promotion of Measures to Prevent Bullying that was enacted in 2013 cannot stop this problem. In this paper, first, the real figures regarding bullying were clarified using various statistical materials, and then the definition of bulling was discussed referring to previous definitions within and outside Japan. Thereafter, various facilitating factors to bullying were introduced, elucidating many detrimental influences on victims, bullies, and bystanders. Regarding the causes that make bullies conduct bullying, they were discussed in terms of both developmental and information-processing factors, and the approaches to address bullying problems were revealed in each of both factors. Other than bullies’ characteristics, the influence from bystanders was underscored clarifying why they stand by while noticing bullying. Additionally, it was suggested that if school as a system can effectively function against bulling, bullying will be stopped before getting serious, discussing how such a school system should be established. However, the school system at present cannot cultivate characteristics by which students do not conduct bullying in any place and in any time, which is a fundamental purpose school education aims to achieve. So, finally, the possibilities and necessities of prevention for bullying that cultivates antibullying characteristics were suggested with an introduction of an effective prevention program.

In recent years, the incidence of myopia in children has been increasing, so it is important to determine the effects of environmental and lifestyle factors at an early stage and take countermeasures. The purpose of this study was to examine in detail the changes in the use of electronic devices and the changes in the visual acuity of children Comparing the good vision group and the poor vision group, no significant differences were observed in the frequency of attending cram schools, the frequency of exercising outside, the amount of time spent using electronic devices, and sleep. Although no significant difference was observed in stress response, the poor visual acuity group had a higher physical reaction score, and symptoms such as headache and fatigue we re observed compared to the good visual acuity group. Children's use of electronic devices was predicted to be longer than before COVID 19, but no significant change was observed in this study. It is believed that he lived under the supervision of his parents. In the future, it is necessary to investigate parents' ideas and measures to maintain their eyesight, clarify the relationship with children's eyesight, and consider guidance not only for children but also for parents.

Objectives: Maxillomandibular advancement (MMA) for obstructive sleep apnea (OSA) is considered the useful treatment; however, its wide effectiveness is unclear. Thus, this study aimed to explore factors affecting the efficacy of MMA for OSA using a 3-D morphological and computed fluid dynamics (CFD) analysis of the upper airway (UA). Design: Retrospective study Settings and participants: Twenty consecutive patients (six women, mean age 34.2 ± 12.4 years) who underwent MMA because of persistent OSA at our center. Main outcome measures: Cone-beam computed tomography images were captured before and after MMA. We assessed the maxilla and mandibular positions, cross-sectional areas of the nasal airway (NA) and pharyngeal airway (PA), and PA space (PAS). The negative pressure of the PA, NA, and UA were measured at inspiration using CFD. We performed paired t-tests and Wilcoxon signed-rank test to compare values before and after MMA. The relationship between the airway size and pressure was evaluated using Spearman correlation coefficients and a non-linear regression analysis. Results: PAS significantly expanded from 6.5 ± 3.4 mm to 12.2 ± 3.8 mm. NA obstruction significantly improved from -312.6 ± 265.0 Pa to -76.2 ± 129.0 Pa. Moreover, the patients were divided into four types according to the PAS size and presence of NA obstruction, namely narrow PAS with NA obstruction, narrow PAS, NA obstruction, and wide PAS and without NA obstruction. Conclusions: We identified variations in the OSA types. However, MMA substantially improved NA obstruction and PA expansion. Thus, we considered that MMA for OSA was effective for various types.

39. Defects of Simile in Rudraṭa’s Poetic Treatise

- Abstract
In Sanskrit poetics, theorists lay down the rule that a simile expressed in the form of a sentence (vākyopamā) must be constructed in such a way that the subject of comparison can grammatically agree with the object of comparison. However, it should be noted that in poetry, there are many examples of vākyopamā not conforming to this rule, which makes it necessary for theorists to modify the rule. Focusing on the poetic treatise Kāvyālaṃkāra, written by the theorist Rudraṭa (ca. ninth-century CE), this paper aims to answer the question of what grammatical requirements are necessary to construct vākyopamā. According to the classification in Kāvyālaṃkāra 8.4–16, vākyopamā has six subtypes: (a) those in which the subject and object of comparison, the common property between them, and an indicator of comparison are explicit; (b) those in which the subject and object of comparison and an indicator of simile are explicit; (c) those in which the subject and object of comparison are interchangeable (ubhayopamā); (d) those in which the subject of comparison is identical to the object of comparison (ananvayopamā); (e) those in which the common property between the subject and object of comparison is inferred from the modifiers that modify the subject and object of comparison (kalpitopamā); (f) those in which the subject of comparison is compared to the unreal object of comparison, assumed to be real in a subjunctive clause (utpādyopamā). In the verse used as an example of subtype (f) Rudrat.a compares a piece of clothing that covers breasts (stanāvaraṇam) to a skin (tvak). It is worth noting that the former is neuter in gender, whereas the latter is feminine. In Kāvyālaṃkāra 11.25–26, Rudrat.a states that in vākyopamā, the subject of comparison must agree with the object of comparison in gender (liṅga), number (vacana), tense (kāla), participant of an action (kāraka), and nominal or verbal ending (vibhakti), so that the modifier denoting the common property between the subject and object of comparison can modify both without changing its grammatical form. Observation of this fact leads us to the following conclusion: Rudraṭa allows a poet to construct vākyopamā in which the subject of comparison does not grammatically agree with the object of comparison, as long as the modifier denoting the common property is implicit. The findings were mainly the followings: (1) 8 categories were found as categories related to science teaching competence corresponding to diverse learning. (2) It was implied that after having each learner think about their predictions, by having them present and share their predictions for all groups, teacher trainees will be able to notice various predictions and think about how to respond to such diversity. (3) From the viewpoint of the UDL guidelines, some descriptions were found related to each of the three principles; these are 'Provide multiple means of representation', 'Provide multiple means of action & expression', and 'Provide multiple means of engagement'.

Three young teachers participated in semi-structured interviews about the causes that emotionally drain teachers and the factors that reduce their feelings of emotional exhaustion. Their utterance were analyzed and put into theoretical descriptions using SCAT. The common features of the theoretical descriptions are as follows. First, “emotional exhaustion varies depending on the students and the classes they take charge of, and the characteristics of their tasks.” Second, “they feel emotional exhaustion when dealing with students’ problematic behaviors.” Third, “they cite social support from other teachers and staff members as a factor that reduces their emotional exhaustion.” Fourth, “they expect effective support for student guidance in their training programs.” These factors show the following. They all experience emotional exhaustion due to stress from their interactions with others although the quality and quantity of their stressors vary from person to person. They wish that there were training programs providing effective support for guiding students. To reduce the emotional exhaustion of young teachers, it is necessary to provide them with continuous social support in their daily duties.

49. Hypoxia-Inducible Factor-Prolyl-Hydroxylase and Sodium-Glucose Cotransporter 2 Inhibitors for Low-Risk Myelodysplastic Syndrome-Related Anemia in Patients with Chronic Kidney Disease: A Report of Three Cases

Although daprodustat, a hypoxia-inducible factor prolyl hydroxylase inhibitor, and dapagliflozin, a sodium-glucose cotransporter 2 inhibitor, have been approved for the treatment of renal anemia in Japan, their efficacy and safety for patients aged 80 years or older with low-risk myelodysplastic syndrome (MDS)-related anemia have not been demonstrated. Our case series comprised two men and one woman aged >80 years with low-risk MDS-related anemia and diabetic mellitus (DM)-related chronic kidney disease who were dependent on red blood cell transfusions and in whom erythropoiesis-stimulating agents had been insufficient. All three patients received daprodustat and additional dapagliflozin achieved red blood cell transfusion independence and were followed up for >6 months. Daily oral daprodustat was well tolerated. There were no fatalities or progression to acute myeloid leukemia during the >6-month follow-up after daprodustat initiation. On the basis of these outcomes, we consider 24 mg of daprodustat combined with 10 mg of dapagliflozin daily an effective form of treatment for low-risk MDS-related anemia. Further studies are required to clarify the synergistic effects of daprodustat and dapagliflozin, which correct chronic kidney disease-related anemia by promoting endogenous erythropoietin production and normalizing iron metabolism to manage low-risk MDS in the long term.
