Objectives The aim of this AFIRE (Atrial Fibrillation and Ischemic Events With Rivaroxaban in Patients With Stable Coronary Artery Disease) trial subgroup analysis was to examine rivaroxaban monotherapy benefits and their relation to the time between stenting and enrollment among patients after coronary stenting. Background Of 2,215 patients with atrial fibrillation and stable coronary artery disease in the AFIRE trial, rivaroxaban monotherapy was noninferior to rivaroxaban plus antiplatelet therapy (combination therapy) in terms of efficacy and superior for safety endpoints. However, thrombotic risk after antiplatelet therapy cessation remained a concern among 1,444 patients who had undergone coronary stenting >1 year before enrollment. Methods The benefits of rivaroxaban monotherapy in coronary stenting subgroups were assessed for efficacy (a composite of stroke, systemic embolism, myocardial infarction, unstable angina requiring revascularization, or death of any cause), safety (major bleeding defined according to International Society on Thrombosis and Haemostasis criteria), ischemic endpoints, net adverse clinical event, and time between stenting and enrollment. Results Efficacy and safety endpoints for monotherapy were superior to combination therapy, with HRs of 0.70 for efficacy (95% CI: 0.50-0.98; P = 0.036) and 0.55 for safety (95% CI: 0.33-0.92; P = 0.019). For ischemic endpoints, the HR was 0.82 (95% CI: 0.58-1.15; P = 0.240). The HR became smaller with longer time between stenting and enrollment (efficacy, P for interaction = 0.158; safety, P = 0.097). Conclusions In patients with atrial fibrillation after coronary stenting, the benefits of rivaroxaban monotherapy for efficacy and safety endpoints were consistent with those in the whole AFIRE trial population. The benefits became apparent with longer time between stenting and enrollment. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I p steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at ∼8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I p beam injection was coupled to a radiative divertor. The hybrid core scenario was achieved directly, without the need for anomalous current diffusion, using off-axis current drive actuators. Also, a controller to assess proximity to stability limits and regulate β N in the ITER baseline scenario, based on plasma response to probing 3D fields, was demonstrated. Finally, innovative tokamak operation using a negative triangularity shape showed many attractive features for future pilot plant operation.

Background Severe inflammation with necrosis and fibrosis of the gallbladder in acute cholecystitis increases operative difficulty during laparoscopic cholecystectomy. This study aimed to assess the use of preoperative MRI in predicting pathological changes of the gallbladder associated with surgical difficulty. Methods Patients who underwent both preoperative MRI and early cholecystectomy for acute cholecystitis between 2012 and 2018 were identified retrospectively. On the basis of the layered pattern of the gallbladder wall on MRI, patients were classified into three groups: high signal intensity (HSI), intermediate signal intensity (ISI), and low signal intensity (LSI). The endpoint was the presence of pathological changes of the gallbladder associated with surgical difficulty, such as necrosis, abscess formation and fibrosis. Results Of 229 eligible patients, pathological changes associated with surgical difficulty were found in 17 (27 per cent) of 62 patients in the HSI group, 84 (85 per cent) of 99 patients in the ISI group, and 66 (97 per cent) of 68 patients in the LSI group (P, Variations in the layered pattern of the gallbladder wall on MRI were significantly associated with fibrosis and necrosis of the gallbladder wall in acute cholecystitis. Intermediate to low signal intensity of the gallbladder wall on MRI had a sensitivity of 90 per cent and a specificity of 73 per cent for predicting pathological changes in the gallbladder wall associated with surgical difficulty during laparoscopic cholecystectomy for acute cholecystitis. The results indicate that MRI could be a novel method of predicting surgical difficulty in laparoscopic cholecystectomy. MRI could possilby predict surgical difficulty of cholecystectomy.

To fabricate low-consumption and high-resolution gas sensors, especially for methane (CH4) gas (which has absorption around wavelength of 3.3 μm), mid-infrared LEDs are required. Accordingly, to reduce Shockley Read Hall recombination and achieve high luminous intensity at 3.3 μm, an AlxIn1-xSb LED with dislocation filter layers was developed. Dislocation density of the active layer was significantly reduced from 3.5 × 108 to 1.1 × 108/cm2, and luminous efficiency was doubled compared to that of conventionally structured LEDs. To our knowledge, emission intensity at 3.3 μm under injection current of 100 mA is the highest of any commercial LEDs.

Peak detection of any time serial signals can be done using different methods. However, exact positions of the peaks cannot be achieved, if the method does not have a high sensitivity detection capability. It is known that various errors appear between the real peak value and the measured value; therefore, a true peak value can be obtained from the measured value by using a highly sensitive detection method. For this reason, spline interpolation is used to estimate the peak points from measured values. In this paper, highly sensitive peak detection of high-frequency signals based on FPGA is proposed using spline algorithm, which is capable of calculating the middle points using the first and last points on the signal. To evaluate the system, the histograms, the mean, and the variance of the peak values before and after spline interpolation are compared. Simulation results show that calculated peak points using splines and real peak values of a signal are very close to each other since the spline algorithm has very high calculation sensitivity. In addition, spline results show us how much error the existing peak values have.

Solitary fibrous tumour (SFT) is a rare mesenchymal tumour with intermediate malignant potential. Although this tumour arises in several sites, prostatic SFT is an extremely rare neoplasm and may prove confusing owing to the lack of clinical experience because of tumour rarity. The diagnosis may be further difficult because SFTs can manifest positive immunoreactivity for CD34 and progesterone receptor, which are known markers of prostatic stromal tumours. Herein, we describe a case of prostatic SFT that was difficult to differentiate from a prostatic stromal tumour of uncertain malignant potential because of positive immunoreactivity to CD34 and progesterone receptor.A 40-year-old Japanese man presented with lower abdominal pain. Computed tomography revealed a prostatic mass; furthermore, prostate core needle biopsy revealed proliferating bland spindle cells, without necrosis or prominent mitoses. Tumour cells were positive for CD34 and progesterone receptor on immunohistochemical analysis; thus, a prostatic stromal tumour of uncertain malignant potential was initially suspected. However, as the tumour cells showed positive immunoreactivity for STAT6, the final diagnosis was an SFT of the prostate. The patient underwent tumour resection, and at the 6-month postoperative follow-up, neither local recurrence nor distant metastasis occurred.For an accurate diagnosis of an SFT of the prostate, STAT6 immunohistochemistry should be conducted for all mesenchymal tumours of the prostate. When STAT6 immunohistochemical analysis is unfeasible, pathologists should be aware that the morphological and immunohistochemical characteristics of SFT variable from case to case and diagnose with combined analysis of several immunohistochemical markers.

In Japan, inguinal hernia repair is widely performed with local anesthesia. The objective of this study was to evaluate safety and efficacy of intravenous dexmedetomidine as a sedation agent with local anesthesia in inguinal hernia repair. We performed this randomized, single-blind study for 200 patients who were scheduled to undergo inguinal hernia repair with local anesthesia. Patients were randomly divided into two groups (dexmedetomidine group: Group D, midazolam group: Group M). The primary outcome was to evaluate the safety of intravenous dexmedetomidine. Secondary outcomes were to analyze results of operators’ surveys and patients’ questionnaires and evaluate implementation of conscious sedation. Incidence of respiratory depression was significantly higher in Group M than Group D (p = 0.03). Other adverse events examined did not differ significantly. All three operators’ questionnaires indicated that results were better in Group D than Group M. More than 70% of patients in both groups were satisfied with the surgery. More than 80% of Group D patients and 74% of Group M patients achieved a state of conscious sedation. This study demonstrated that intravenous dexmedetomidine during hernia repair with local anesthesia is safe and the results were satisfactory to both operators and patients.

Background and Aims Cohort studies in Japan indicate that citrus fruit and β-cryptoxanthin–rich Satsuma mandarin are candidate foods for lowering cardiovascular risk; however, there have been few intervention studies of β-cryptoxanthin and Satsuma mandarin supplementation. Methods and Results A randomized, placebo-controlled, double-blind parallel trial was performed. Apparently healthy men and women were randomly assigned to two groups: one receiving concentrated β-cryptoxanthin–rich Satsuma mandarin juice (β-cryptoxanthin 4 mg per 125 mL/day: active group) and one receiving β-cryptoxanthin–deprived Satsuma mandarin juice (β-cryptoxanthin 0 mg per 125 mL/day: control group) for 12 weeks. The primary outcome was brachial-ankle pulse wave velocity (baPWV), which assesses arterial stiffness. An oxidative stress biomarker and clinical outcomes were also evaluated. Effect of the intervention on the outcome variables was analyzed using generalized linear mixed models. After the trial, a prespecified 8-month follow-up examination was conducted. One hundred eighteen subjects were randomly assigned, and 117 completed the intervention. Serum β-cryptoxanthin concentration was increased among the active group after β-cryptoxanthin–rich Satsuma mandarin juice supplementation. Both supplementations lowered baPWV and oxidative stress biomarker levels; however, there was no additional effect in the active group compared with the control group. At the 8-month examination, the increase in baPWV among the active group was smaller than that among the control group. Conclusion Satsuma mandarin juice supplementation is potentially beneficial for reducing cardiovascular risk; however, additional β-cryptoxanthin supplementation did not enhance this effect. Further study is needed to evaluate the long-term effect of β-cryptoxanthin supplementation. Registration UMIN-CTR, UMIN000013579.

Reverse roll coating is probably the most widely used coating operation, yet its full potential has not been exploited as it is shown in this paper which considers operation with a negative gap. We demonstrate through a wide range of experimental data that such operation can yield very thin and stable films with no ribbing or cascade instabilities when low viscosity fluids are used. Typically, stable film thickness less than 5 μm can be obtained at speeds up to 150 m/min when a rubber roller is used at −100 μm gap with fluids of viscosity in the range 10–200 mPa s. These film thicknesses can be made to decrease further down to 1 or 2 µm with a judicious choice of speed ratios (applicator to metering roller) and rubber hardness. Such new findings make this simple coating method an attractive roll to roll technique for application in the newer coating technologies, such as in the production of solar cells and plastic electronics. The data obtained in this study have been underpinned by a model based on the classical lubrication theory, well developed for such flow situations. Essentially it is shown that the film thickness non dimensionalised with respect to the set negative gap is controlled through a single parameter, the elasticity number Ne which combines all the operating parameters. Of course, this flow problem has complexities, particularly at high speed ratios and at zero gap so the data obtained here can serve as a basis for more comprehensive modelling of this classical fluid mechanic problem.

Aims The EURO-ENDO registry aimed to study the management and outcomes of patients with infective endocarditis (IE). Methods and results Prospective cohort of 3116 adult patients (2470 from Europe, 646 from non-ESC countries), admitted to 156 hospitals in 40 countries between January 2016 and March 2018 with a diagnosis of IE based on ESC 2015 diagnostic criteria. Clinical, biological, microbiological, and imaging [echocardiography, computed tomography (CT) scan, 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT)] data were collected. Infective endocarditis was native (NVE) in 1764 (56.6%) patients, prosthetic (PVIE) in 939 (30.1%), and device-related (CDRIE) in 308 (9.9%). Infective endocarditis was community-acquired in 2046 (65.66%) patients. Microorganisms involved were staphylococci in 1085 (44.1%) patients, oral streptococci in 304 (12.3%), enterococci in 390 (15.8%), and Streptococcus gallolyticus in 162 (6.6%). 18F-fluorodeoxyglucose positron emission tomography/computed tomography was performed in 518 (16.6%) patients and presented with cardiac uptake (major criterion) in 222 (42.9%) patients, with a better sensitivity in PVIE (66.8%) than in NVE (28.0%) and CDRIE (16.3%). Embolic events occurred in 20.6% of patients, and were significantly associated with tricuspid or pulmonary IE, presence of a vegetation and Staphylococcus aureus IE. According to ESC guidelines, cardiac surgery was indicated in 2160 (69.3%) patients, but finally performed in only 1596 (73.9%) of them. In-hospital death occurred in 532 (17.1%) patients and was more frequent in PVIE. Independent predictors of mortality were Charlson index, creatinine > 2 mg/dL, congestive heart failure, vegetation length > 10 mm, cerebral complications, abscess, and failure to undertake surgery when indicated. Conclusion Infective endocarditis is still a life-threatening disease with frequent lethal outcome despite profound changes in its clinical, microbiological, imaging, and therapeutic profiles.

Background Previous studies have shown proximal extension (PE) rate of ulcerative proctitis (UP) is approximately 50%. This rate teach us the importance of treating UP adequately. Basically, we have to treat them with topical 5-ASA. But in clinical practice, we sometimes experience the case whose inflammation remain endoscopically, even though the symptoms have been relieved. Should we strengthen our treatment at this situation, especially inflammation at lower rectum? We aimed to know PE rate of lower rectal UP and optimize the treatment. Methods We retrospectively investigated the medical charts of patients with UP from 2010 to 2020 at Kenseikai Nara Coloproctology Center. We excluded the patients with UP shrinking from left-sided or pancolitis as a result of the treatment. The cases with missing value were excluded too. Variables of interest included gender, onset age, disease location, initial mayo endoscopic subscore (MES) and treatment, and the time to PE. To compare the cohorts we used Fisher’s exact test and Mann-Whitney test. Proximal extension free survival (PEFS) was calculated using the Kaplan-Meier method. Results Sixty-five patients were recruited. Mean age was 42 years old, the ratio of males to females was 1.17. The number of lower rectal UP patients at first examination was 24. We observed MES 1 inflammation for 30 patients, and MES 2 for 35. Median follow-up duration was 73.5 months. PE occurred in 34 patients: 28 patients had left-sided colitis and 6 had pancolitis. Fifty-four patients were medicated and other 11 patients were just observed. Among the patients medicated, 23 patients were took only topical drugs, 18 were took only oral drugs and other 13 were took both. We couldn’t find any relationships between initial medication and PE rate. The multivariate analysis revealed that having peri-appendiceal red patch and lower rectal UP were reduce the risk of PE. MES was not related to PE (table 1). PEFS at 1 year, 2 year are 81%, 81% for lower rectal UP (Rb group), and 66%, 61% for the patients having the disease beyond middle Houston’s valve (Not-Rb group), though we couldn’t find significant difference (Fig.1). Among 34 patients having PE, we used biologics for only five patients and got their inflammation under control. Conclusion Patients with lower rectal UP is less likely to extent their disease location. There’s no difference between the route of treatment and PE rate, but it’s important to carefully check PE within first one year.

Lipid-coated microbubbles (MBs) with an indocyanine green (ICG) derivative were fabricated for ultrasound and near-infrared (NIR) fluorescence dual imaging. We characterized the NIR-fluorescence intensity, stability and viscoelastic properties of the encapsulating lipid shell, focusing on the influence of the ICG derivative and lipid compositions. In terms of the NIR fluorescence intensity, the fluorescence intensity of the MBs (with the ICG derivative) was significantly affected by the lipid composition of the MB shell. Regarding the contrast agent used for ultrasound imaging, the stability of the MBs and viscoelastic properties of shell also depended on the lipid compositions, while the incorporation of the ICG derivative into the MB shells had a negligible effect. The performance of this contrast agent for ultrasound and NIR fluorescence dual-imaging exhibited a significant trade-off relationship for the lipid composition.

30. Synchronicity of the East Asian Summer Monsoon variability and Northern Hemisphere climate change since the last deglaciation

Understanding of the mechanism of the East Asian Summer Monsoon (EASM) is required for the prediction of climate change in East Asia in a scenario of modern global warming. In this study, we present high-resolution climate records from peat sediments in Northeast Japan to reconstruct the EASM variability based on peat bulk cellulose δ13C since the last deglaciation. We used a 8.8 m long peat sediment core collected from the Tashiro Bog, Northeast Japan. Based on 42 14C measurements, the core bottom reaches ~15.5 ka. δ13C, accumulation rate and accumulation flux time-series correlate well to Greenland ice core δ18O variability, suggesting that the climate record in Northeast Japan is linked to global climate changes. The δ13C record at Tashiro Bog and other paleo-EASM records at Northeast and Southern China consistently demonstrate that hydrological environments were spatially different in mid-high and mid-low latitude regions over the last 15.5 kyr. During global cooling (warming) periods, mid-high and mid-low latitude regions were characterized by wet (dry) and dry (wet) environments, respectively. We suggest that these climatic patterns are related to the migration of the EASM-related rain belt during global climate changes, as a consequence of variations in intensity and location of both the Intertropical Convergence Zone (ITCZ) and the Western Pacific Subtropical High (STH). The location of the rain belt largely influences the East Asian hydrological environment. Our δ13C time-series are characterized by a 1230 yr throughout the Holocene and a 680 yr periodicity during the early Holocene. The 1230 yr periodicity is in agreement with North Atlantic ice-rafted debris (IRD) events, suggesting a teleconnection between the Northeast Japan and the North Atlantic during the Holocene. In addition, it is the first evidence that the Bond events were recorded in terrestrial sediment in Japan. On the other hand, the 680 yr periodicity between 10.0 and 8.0 kyr is consistent with a prominent 649 yr solar activity cycle, suggesting that solar activity affected EASM precipitation during the Hypsithermal, when orbital-scale solar insolation was at a maximum in the Northern Hemisphere.

BACKGROUND AND PURPOSE Diphenylarsinic acid (DPAA) intoxication caused by drinking contaminated well water was found in Kamisu, Japan. The symptoms indicated cerebellar-brainstem and temporo-occipital involvement. However, it remains unclear how it affects the human brain. To elucidate the effect of DPAA on the human brain, we analyzed cerebral blood flow (CBF) data after the drinking of DPAA-contaminated water was stopped and investigated the correlation between DPAA exposure level and CBF by single-photon emission computed tomography (CBF-SPECT). METHODS The DPAA-exposed inhabitants (n = 78) were divided into 35 symptomatic and 43 asymptomatic subjects and compared with 38 healthy controls. The DPAA concentration in nails or hair and well water was measured using a high-performance liquid chromatography system and coupled plasma mass spectrometry after adequate extraction treatment. CBF-SPECT data, obtained within 1 year after the drinking of contaminated well water was stopped, were analyzed by statistical parametric mapping. We also examined the relationship between variations in CBF-SPECT signals and variations in DPAA concentrations in the hair or nails of the subjects. RESULTS Compared with control subjects, CBF in symptomatic DPAA-exposed subjects was significantly lower in the occipital lobe, including the cuneus and inferior occipital gyri. The DPAA concentration in the nails or hair of subjects was inversely and significantly related to their CBF. CONCLUSION These data suggest that CBF-SPECT may be useful as a clinical marker to infer the effect of accumulated DPAA on the brain.

We report on investigations of the quantum Hall chiral edge states using a near-field scanning optical microscope that enables us to irradiate circularly polarized light from the probe tip with spatial resolution below the diffraction limit. We have found a clear evidence for the formation of spin-split incompressible strips near the edge of a two-dimensional electron system in high magnetic fields.

This paper introduces a new design of disaster state presentation system using large size and ultra high definition displaying system to satisfy requirement of quick and wide collecting and sharing disaster state information from disaster areas to make decision by the officers as decision makers who work in the headquarters. The collected disaster state information are processed as workspace and interactively controlled using workspace controller to display on the Tiled Display Wall (TDW). We designed and implemented our system using current available technology as a prototype.

On large scale disasters, such as the Eastern Japan Great Earthquake and Kumamoto Earthquake, due to mud slides of cliff and floodwater of rivers, many disaster areas are physically isolated and relief and support activities of victims become quite difficult. The communication means are also destructed in those areas and cannot transmit the disaster information. In order to resolve those problems, we propose an information communication system which consisted of Multiple Unmanned Aerial Vehicles (UAV). In this system, multiple UAVs have wireless communication units and organize a wireless mesh network in the sky to transmit the information among the isolated areas. Its network configuration and architecture are explained. A prototype system is constructed to evaluate its function and performance.

An Auger-electron–ion coincidence method is applied to study the site-specific formation of metastable CO 2 2+ in the C1s and O1s Auger decays of CO 2 . It is found that only dication states with the electronic configuration of 1π g −2 are relevant to the metastable formation. These dication states are favorably produced in the O1s Auger decay, because of the charge distribution of the 1π g orbital localizing at the O site. Fragmentation channels of dissociative CO 2 2+ states are also investigated.

Reverse roll coating is probably the most widely used coating operation, much less investigated than its counterpart and inherently unstable forward roll coating. A new data to complement earlier work which was limited to large gaps and thus “thick” films is presented. The intention is to assess the feasibility of reverse roll coating to yield very thin films ( 1 m/s) for application in the newer technologies, such as the production of solar cells and plastic electronics. The data obtained demonstrate this is possible but at the lowest permissible gap (25–50 μm) with low-viscosity fluids (∼7 mPa s). The study also developed a new understanding of how instabilities are controlled. It was seen that the size of the inertia forces generated by the applicator roller in relation to surface tension, as expressed by the Weber number and not the applicator Capillary number (viscous forces/surface tension) which is the critical parameter. © 2013 American Institute of Chemical Engineers AIChE J, 59: 3083–3091, 2013
