Neuromuscular blocking agents (NMBAs) like atracurium and rocuronium as well as fluoroquinolones (FQs) cause mast cell-mediated anaphylaxis by activating Mas-related G protein-coupled receptor X2 (MRGPRX2), but many questions remain unanswered. Here, we address three of them, namely whether primary human mast cells show similar activation by these drugs as murine mast cells and mast cell lines, how sugammadex protects from atracurium-induced MRGPRX2-mediated mast cell activation, and why some but not all patients treated with rocuronium develop anaphylaxis. We used peripheral blood-derived cultured mast cells from healthy donors and patients, assessed mast cell activation and degranulation by quantifying intracellular calcium and CD63 expression, respectively, and made use of MRGPRX2-silencing, via electroporation with Dicer-substrate small interfering RNAs, and single cell flow cytometric analyses. Atracurium, ciprofloxacin, and levofloxacin activated and degranulated primary human mast cells, but only MRGPRX2-positive and not MRGPRX2-negative or -silenced mast cells. Sugammadex attenuated the atracurium-induced and MRGPRX2-mediated activation and degranulation of human mast cells by reducing free atracurium levels. The mast cells of patients with IgE-independent anaphylaxis to rocuronium were similar, in their MRGPRX2 expression and function, to those of patients with IgE-mediated anaphylaxis. These findings further improve our understanding of the role and relevance of MRGPRX2-driven mast cell activation in anaphylactic reactions to NMBAs and FQs and may help to improve their prediction, prevention, and treatment.

We have read with interest the paper by Loli‐Ausejo and colleagues about the basophil activation test (BAT) in the diagnosis of hypersensitivity to fluoroquinolones (FQs) 1. The authors report on a heterogenous group of 19 patients who experienced a hypersensitivity reaction reported as immediate, accelerated/non‐immediate or of unknown time of onset. Based on clinical history, the authors posit these reactions to have resulted from levofloxacin, moxifloxacin or ciprofloxacin and stratified the patients in three severity grades according to Brown.

Unverified and false “β-lactam allergies”, mainly to the first-line preparations natural penicillin and aminopenicillins, have evolved into a worldwide plague with serious medical and financial consequences. From this thesis it appears that as much as 12% of the individuals attending the outpatients’ clinic of Allergology and Pediatric Allergology of the Antwerp University Hospital claim to have a “penicillin allergy”. However, in 91% of these individuals this claim appears to be false, as they tolerated these antibiotics during a controlled drug challenge. Importantly, as these cases could be safely delabelled, we were able to improve their antibiotic stewardship, mainly by safeguard them against the unnecessary risk for use of suboptimal and expensive second-line treatments. From the literature and our own experience it is clear that judicious diagnostic work-up by a trained physician is absolutely necessary in every case of both witnessed or self-reported “penicillin allergy”. However, diagnosis of β-lactam hypersensitivity reactions is not always straightforward and optimization of the diagnostic approach of β-lactam hypersensitivities is needed. Therefore, we studied the performance of new or recently introduced diagnostics. First, in the context of nonimmediate hypersensitivity reactions to amoxicillin and amoxicillin clavulanic acid, we showed that a CD154-based flow cytometric lymphocyte activation test is a safe and quick diagnostic test, especially in cases with mild cutaneous nonimmediate drug hypersensitivity reactions to amoxicillin or amoxicillin clavulanic acid, and that a prolonged drug challenge extending over several consecutive days is of limited use. In our country cefazolin has evolved to one of the major causes of perioperative anaphylaxis. We explored the performance of a recently developed specific (s)IgE assay to cefazolin. We observed that neither sIgE to cefazolin nor a sIgE/tIgE ratio were of added value in the diagnosis of cefazolin hypersensitivity. Finally, it is known that cross-reactivity among β-lactam antibiotics is unpredictable but mostly related to the R1-side chain. However, we demonstrated that fine structural differences in the R1-side chain can suffice to explain absence of cross-reactivity and clinical tolerance. Clearly, molecules with a similar, but not identical R1-side chain, should not be precluded or administered using a desensitization protocol, provided they test negative in skin testing. In conclusion, self-reported “penicillin allergy” has grown into a vast epidemic with unacceptable consequences for the individual patient and the society. Further efforts are required to optimize and harmonize a cost-conscious diagnostic approach of “penicillin allergy” and “β-lactam allergy” in general.

Oralemijtenanafylaxie (OMA), ook het pancake syndrome genoemd, is een aandoening die gekenmerkt wordt door (ernstige) allergische reacties ten gevolge van de inname van voedsel waarin bloem zit gecontamineerd met mijten. De kliniek van OMA is zeer verscheiden en wordt bepaald door de plaats en de mate van mastcellen/basofielendegranulatie en treedt doorgaans op binnen het uur na de ingestie. De meest voorkomende symptomen zijn kortademigheid, angio-oedeem, wheezing en neusloop. Vooralsnog blijven de allergenen verantwoordelijk voor OMA niet geïdentificeerd. De diagnose van OMA berust op een suggestieve anamnese verenigbaar met een allergische reactie naar aanleiding van de inname van voedsel dat meel bevat, aangevuld met het aantonen van een sensibilisatie ten opzichte van mijten, het aantonen van mijten in de verdachte bloem, positieve huidtesten met de verdachte bloem en negatieve huidtesten met niet-gecontamineerde bloem. Niet zelden, zoals bij de in deze tekst besproken patiënten, is de gecontamineerde bloem niet meer beschikbaar, waardoor de correcte diagnose bemoeilijkt wordt. In dit manuscript wordt er voor het eerst simultane OMA beschreven bij een vader en zijn dochter, waarschijnlijk toe te schrijven aan beignetbloem gecontamineerd met mijten. Een literatuuroverzicht leert dat de diagnose van OMA gemakkelijk gemist wordt en overwogen moet worden in geval van anafylaxie sine causa apparente.

De ziektegeschiedenis van een patiënte met een aardbeienallergie wordt beschreven. Na een grondige anamnese aangevuld met in-vitro- en in-vivodiagnostiek wordt er achterhaald dat de aardbeienallergie geen alleenstaand fenomeen is, maar dat de patiënte ondertussen verschillende gevoeligheden voor fruit en groenten vertoont die hoogstwaarschijnlijk berusten op een kruisallergie voor Cannabis sativa. Zoals in dit tijdschrift reeds werd beschreven, lijkt deze kruisallergie tussen cannabis en fruit/groenten geassocieerd met een gevoeligheid voor een non-specific lipid transfer protein (ns-LTP). De identificatie van deze kruisallergie werd daarbij verricht aan de hand van componentgeoriënteerde diagnostiek. Het strikt vermijden van enig gebruik van en passieve blootstelling aan cannabis werd aangeraden, evenals het vermijden van vruchten en groenten die ondertussen verantwoordelijk gebleken zijn voor min of meer ernstige allergische reacties.

Allergieën voor genotsmiddelen zoals drugs en alcohol worden zelden gemeld. Vermoedelijk betreft het een onderrapportering mede door het illegale karakter van sommige ervan. Toch is er een toenemend aantal meldingen betreffende allergieën voor cannabis, opioïden, cocaïne, kava, amfetaminen, ketamine, efedra, khat en nootmuskaat.
