Previous cross-sectional studies examining whether John Henryism (JH), or high-effort coping with socioeconomic adversity, potentiates the inverse association between socioeconomic position (SEP) and cardiovascular health have focused mainly on hypertension in African Americans. We conducted the first longitudinal test of this hypothesis on incident acute myocardial infarction (AMI) using data from the Kuopio Ischemic Heart Disease Risk Factor Study in Finland (N = 1405 men, 42-60 years). We hypothesized that the expected inverse gradient between SEP and AMI risk would be stronger for men scoring high on JH than for those scoring low. John Henryism was measured by a Finnish version of the JH Scale for Active Coping. Four different measures of SEP were used: childhood SEP, education, income, and occupation. AMI hazard ratios (HR) by SEP and JH were estimated using COX proportional hazard models, before and after adjustment for study covariates. 205 cases of AMI occurred over a median of 14.9 years. Men employed in lower rank (farmer, blue-collar) occupations who scored high on JH had significantly higher age-adjusted risks of AMI than men in higher rank (white-collar) occupations (HR = 3.14, 95% CI: 1.65-5.98 for blue collar; HR = 2.33, 95% Cl: 1.04-5.22 for farmers) who also scored high on JH. No socioeconomic differences in AMI were observed for men who scored low on JH (HR = 136, 95% CI: 0.74 2.47 for blue collar; HR = 0.93, 95% CI: 0.59-1.48 for farmers; p = 0.002 for the SEP x JH interaction). These findings persisted after adjustment for sociodemographic, behavioral, and biological factors. Results for other SEP measures were in the same direction, but did not reach statistical significance. Repetitive high-effort coping with adversity (John Henryism) was independently associated with increased risk for AMI in Finnish men, underscoring the potential relevance of the John Henryism hypothesis to CVD outcomes other than hypertension and to populations other than African Americans. Alcohol dependence and alcoholism are modulated by environmental factors and genetic predisposition. Rat models have been invaluable in the investigation of several aspects of alcoholism in humans. The rodents exhibit a wide range of genetically determined alcohol-drinking preferences. Selective rat breeding programs with different alcohol preference has produced stable lines of rats that reliably exhibit high and low voluntary alcohol consumption (termed here AA and ANA, respectively). Alcohol consumption is also strongly dependent on environmental conditions. Stressful events evoke an extensive multisystem and integrative physiological response, where a major component is the activation of the hypothalamic-pituitary-adrenal (HPA) axis. Testosterone has been implicated as mediator of the rewarding effect of alcohol, and the testosterone level is predictive of future alcohol consumption. The objective of the present thesis is to examine in greater detail the relations between alcohol, testosterone and neuroendocrine stress responses, and alcohol drinking. In the first study (I), the interrelations between endogenous and alcohol-mediated effects on testosterone and corticosterone levels and voluntary alcohol consumption were studied in a crossbred F2 generation of the original AA and ANA rat lines. The second study (II) was a reinvestigation of the effect of subchronic nandrolone decanoate treatment, which in an earlier study had been shown to increase alcohol consumption, and the relation of the effect on the HPA and hypothalamic-pituitary-gonadal (HPG) axes. The third study (III) examined the possible role of benzyl alcohol, present as a preservative in the nandrolone product used in the earlier study, as a confounding factor that could explain differences between the results of the earlier study and the present one. In the fourth study (IV) the interrelations between the effect of corticosterone on alcohol-mediated testosterone changes and alcohol consumption in AA, ANA, F2 and Wistar rats were examined. The results of Study I shows connections between testosterone elevation and increased alcohol drinking, as well as between testosterone reduction and decreased alcohol drinking, which is in line with the original data of the AA and ANA lines. Elevated endogenous testosterone levels and higher frequencies of alcohol-induced testosterone increases were found in high consumption groups compared to low consumption groups. In Study II, subchronic nandrolone administration led to a reduction in alcohol-mediated testosterone levels, correlating with reduced voluntary alcohol consumption in the alcohol-preferring AA rat line. On the other hand, Study III showed that benzyl alcohol increases voluntary alcohol intake at least in the low-consumption rats, which may explain earlier discrepancies among studies. The results of Study IV were consistent with the hypothesis that corticosterone is involved in the regulation of alcohol-mediated testosterone changes. In conclusion, our present results suggests that corticosterone is a balancer, which regulates both the alcohol-mediated testosterone increase followed by reinforcement and increased voluntary alcohol drinking in high-drinking rats, and the alcohol-mediated testosterone reduction followed by disinforcement and reduced alcohol intake in low-drinking rats. Tutkimukset ovat osoittaneet yhteyden testosteronin ja alkoholin kulutuksen välillä sekä testosteronin että palkitsemiseen liittyvien hermostollisten vasteiden välillä. Testosteronitaso voi olla myös ihmisen tulevaa alkoholin käyttöä ennakoiva tekijä. Tämän tutkimuksen kohteena oli tarkemmin selvittää alkoholin, testosteronin ja elimistön fysiologisen stressivasteen välisiä suhteita. Ensimmäisessä tutkimuksessa (I) selvitettiin sisäsyntyisen testosteroni- ja kortikosteronitason, alkoholiannoksen sekä vapaaehtoisen alkoholin kulutuksen aiheuttamien muutosten keskinäisiä yhteyksiä. Tutkimuksessa käytettiin alkoholin kulutukseltaan erilaisiksi jalostettujen rottakantojen F2-sukupolven yksilöitä. Toinen tutkimus (II) tehtiin aikaisempien löydösten perusteella, joissa oli havaittu nandrolon dekanoaatti-käsittelyn lisäävän alkoholin kulutusta. Tutkimuksen tavoitteena oli selvittää, miten tämä vaikutus on yhteydessä HPA- ja HPG-akseleihin. Kolmannessa kokeessa (III) selvitettiin, oliko alkoholin kulutusta lisännyt aikaisemmassa tutkimuksessa säilöntäaineena käytetty bentsyylialkoholi, mikä selittäisi ristiriitaiset tulokset nykyiseen tutkimukseen verrattuna. Neljännen tutkimuksen (IV) tavoitteena oli selvittää tarkemmin kortikosteronin ja alkoholin aiheuttaman testosteronitason muutosten välisiä yhteyksiä AA, ANA, F2 ja Wistar rottakannoilla. Tutkimuksen I tulosten perusteella paljon ja vähän alkoholia kuluttavat rotat eroavat toisistaan testosteronin perustasojen suhteen kuten alkuperäiset AA ja ANA rottakannat. Paljon juovilla rotilla havaittiin korkeammat testosteronitasot ja enemmän alkoholin aiheuttamia testosteroninousuja kuin vähän alkoholia kuluttavilla rotilla. Tutkimuksen II tulosten perusteella havaittiin, että nandrolonikäsittely lievensi alkoholin aiheuttamaa testosteronitason nousujen määrää AA-rotilla ja vähensi alkoholin kulutusta kontrollieläimiin verrattuna. Tutkimus III osoitti, että bentsyylialkoholilla on alkoholin kulutusta lisäävä vaikutus ensisijaisesti vähän juovilla Wistar-rotilla. Tämä selittää ristiriitaa aikaisemman tutkimuksen ja nykyisen tutkimuksen II välillä. Bentsyylialkoholin vaikutusta stressivasteeseen ja testosteronitasojen nousuun ei kuitenkaan havaittu. Tutkimuksessa IV havaittiin kortikosteronin perustason olevan yhteydessä alkoholin aiheuttamaan testosteronitason muutokseen. Tutkimuksen tulosten perusteella voidaan esittää, että kortikosteroni on yhteydessä sekä alkoholin aiheuttamaan testosteronitason kohoamiseen, mikä voi lisätä alkoholin palkitsevaa vaikutusta paljon kuluttavalla rottakannalla että testosteronitason alenemiseen, mikä voi vähentää alkoholin palkitsevaa vaikutusta vähän kuluttavalla rottakannalla.

Today's medical education faces the challenge of developing students competencies to resolve ever more complex problems in collaboration with other professionals. Problem-based learning (PBL) has proven useful for developing many of the competencies needed in modern healthcare. In PBL students collaboratively construct knowledge to explain and solve problems related to real-life situations. In such knowledge construction, dealing with conflicting ideas and knowledge has the potential to enhance student learning while also developing teamwork and critical thinking, skills that are central to multi-professional healthcare teams. The studies that make up the present doctoral thesis explored how students deal with conflicts on knowledge and how they are facilitated to handle such conflicts in PBL tutorial discussions. In addition, the thesis focuses on how to help students learn teamwork skills and critical thinking. Study I examined how to teach teamwork skills to first-year medical students and how to motivate them to learn these skills. The teamwork skills focused on verbal communication in PBL tutorial sessions and in healthcare teams. Feedback on the teamwork skills module from three consecutive classes of first-year students was analysed. Students motivation to learn teamwork skills increased significantly when the introduction to the topic was improved by more clearly pointing out the clinical relevance of such skills. Study II focused on how conflicts on knowledge were dealt with in PBL tutorial group discussions. Four video-recorded tutorial sessions including 33 first-year medical and dental students were analysed. Conflicts on knowledge were found to be relatively rare and generally fairly brief. This was due to a lack of collaborative and thorough argumentation, as well as a lack of questions that would elicit elaboration on the issues. Study III examined tutor facilitation during tutorial discussions, and particularly how the facilitation helped students to collaboratively resolve conflicts on knowledge. The study focused on the tutors in the same video-recorded tutorial sessions as in Study II. The tutors typically intervened by confirming what the students had said or by giving explanations, but they rarely asked questions that would stimulate elaboration on knowledge. During conflicts on knowledge the tutors gave more explanations, but did little to encourage the students to elaborate on conflicting ideas. Study IV focused on medical students conceptions of critical thinking in preclinical PBL. The aim was to find out how the students defined critical thinking, how they perceived it in preclinical PBL and what they expected it to be in clinical practice. The students typically understood critical thinking as judging the reliability of sources of information. Few students understood critical thinking to mean reflecting on their own thinking or viewing things from different perspectives. Students conceptions of critical thinking may have prevented them from seeing the connection between critical thinking in preclinical PBL and critical thinking in clinical practice. The present thesis sheds light on the processes of collaborative knowledge construction related to dealing with conflicting knowledge and ideas in PBL tutorial discussions. The results confirmed prior research findings, which have shown that students rarely deal with conflicting ideas and knowledge, and they point to the central role of the tutor in facilitating students to address these matters in tutorial discussions. The findings also revealed that engaging in deep inquiry during conflicts on knowledge was challenging for both students and tutors. The results further underlined the importance of clearly pointing out to students how they will benefit from the skills learned in preclinical PBL, such as teamwork and critical thinking, in their future professions. Based on these findings, new ideas for improving learning from conflicting ideas in small-group discussions are introduced. Future studies are encouraged to continue exploring the many exciting avenues opened by the present doctoral thesis. Nykypäivän lääketieteen koulutuksella on haastava tehtävä kehittää opiskelijoiden valmiuksia ratkaista entistä monimutkaisempia ongelmia yhteistyössä muiden alojen ammattilaisten kanssa. Ongelmalähtöinen oppiminen (PBL) on osoittautunut toimivaksi opetusmenetelmäksi kehittää monia nykyajan terveydenhuollossa vaadittavia taitoja. Ongelmalähtöisessä oppimisessa opiskelijat rakentavat yhdessä tietoa, jolla selitetään ja ratkaistaan tosielämän tilanteita jäljitteleviä ongelmatapauksia. Ristiriitaisten ideoiden ja tiedon käsittely tiedonrakentamisprosessin aikana tarjoaa mahdollisuuden sekä edistää opiskelijoiden oppimista että tiimityötaitoja ja kriittistä ajattelua, jotka ovat keskeisiä taitoja moniammatillisissa hoitotiimeissä. Tämän väitöskirjan osatutkimukset keskittyivät tarkastelemaan sitä, miten opiskelijat käsittelevät tiedollisia ristiriitoja ja sitä, miten heitä tuettiin käsittelemään tiedollisia ristiriitoja PBL-ryhmäistunnoissa. Lisäksi tarkasteltiin, miten voidaan auttaa opiskelijoita oppimaan yhteisöllisessä tiedonrakentamisessa tarvittavia tiimityö- ja kriittisen ajattelun taitoja. Osatutkimus I tarkasteli tiimityötaitojen opettamista ensimmäisen vuoden lääketieteen opiskelijoille ja sitä, miten heitä voi motivoida oppimaan näitä taitoja. Tiimityötaitojen opetus keskittyi kielelliseen vuorovaikutukseen PBL-ryhmäistunnoissa sekä hoitotiimeissä. Tiimityöopetusta koskeva opiskelijapalaute ensimmäisen vuoden vuosikurssilta analysoitiin kolmena peräkkäisenä vuotena. Opiskelijoiden motivaatio oppia tiimityötaitoja lisääntyi merkittävästi sen jälkeen, kun aiheen esittelyä kehitettiin osoittamalla selkeämmin tiimityötaitojen kliininen merkitys. Osatutkimus II tutki sitä, miten opiskelijat käsittelivät tiedollisia ristiriitoja PBL-ryhmäistuntojen keskusteluissa. Tutkimuksessa analysoitiin neljän ryhmäistunnon videotallennetut keskustelut, joihin osallistui yhteensä 33 lääketieteen ja hammaslääketieteen opiskelijaa. Tilanteet, joissa tiedollisia ristiriitoja käsiteltiin, osoittautuivat melko harvinaisiksi ja ajalliselta kestoltaan varsin lyhyiksi. Tiedollisten ristiriitojen vähäinen käsittely johtui yhdessä tapahtuvan, huolellisen argumentoinnin puutteesta sekä siitä, että keskusteluissa esitettiin vähän sellaisia kysymyksiä, jotka olisivat johtaneet aiheen syvällisempään tarkasteluun. Osatutkimus III keskittyi tarkastelemaan tutorin toimintaa ryhmäistuntojen keskusteluissa. Tutkimus keskittyi erityisesti siihen, miten tutorin toiminta auttoi opiskelijoita yhdessä ratkaisemaan tiedollisia ristiriitoja. Tutkimuksessa käytettiin samoja videotallennettuja keskusteluja kuin toisessa osatutkimuksessa. Tyypillisesti tutorit puuttuivat keskusteluun vahvistamalla mitä opiskelijat olivat sanoneet tai selittämällä asioita opiskelijoille. Tutorit esittivät harvoin kysymyksiä, jotka olisivat stimuloineet aiheen syvällisempään tarkasteluun. Tiedollisten ristiriitojen aikana tutorit selittivät vielä enemmän asioita opiskelijoille, mutta eivät juuri ohjanneet opiskelijoita ristiriitaisten ajatusten syvällisempään käsittelyyn. Osatutkimus IV tarkasteli lääketieteen opiskelijoiden käsityksiä kriittisestä ajattelusta prekliinisen vaiheen ongelmalähtöisessä oppimisessa. Tavoitteena oli selvittää, miten opiskelijat määrittelivät kriittisen ajattelun sekä, millaisena he pitävät sitä prekliinisen vaiheen ongelmalähtöisessä oppimisessa ja kliinisessä työssä. Tyypillisesti opiskelijat ymmärsivät kriittisen ajattelun informaatiolähteiden luotettavuuden arviointina. Harvempi hahmotti kriittisen ajattelun olevan omaan ajatteluun kohdistuvaa reflektointia tai asioiden tarkastelua eri näkökulmista. Opiskelijoiden käsitys kriittisestä ajattelusta saattoi estää heitä näkemästä yhteyttä prekliinisen vaiheen ongelmalähtöisessä oppimisessa tapahtuvan ja kliinisessä työssä tapahtuvan kriittisen ajattelun välillä. Tämä väitöskirja tuottaa tietoa, jolla voidaan kehittää yhteisölliseen tiedonrakentamiseen liittyvien tiedollisten ristiriitojen käsittelyä PBL-ryhmäistunnoissa. Tulokset vahvistivat aiempia tutkimustuloksia siitä, että opiskelijat käsittelevät tiedollisia ristiriitoja melko vähän sekä havaintoja tutorin keskeisestä roolista auttaa opiskelijoita käsittelemään tiedollisia ristiriitoja. Löydökset myös osoittivat, että ristiriitaisten ajatusten syvällisempään käsittelyyn johtavien kysymysten esittäminen on haastavaa sekä opiskelijoille että tutoreille. Tulokset myös korostivat että opiskelijoille on tärkeää osoittaa miten he tulevat hyötymään prekliinisessä PBL:ssä oppimistaan taidoista heidän tulevassa työssään. Löydösten perusteella tutkimuksessa esitettiin uusia ideoita kehittää tiedollisista ristiriidoista oppimista pienryhmien keskusteluissa. Tulevat tutkimukset toivottavasti jatkavat tässä väitöskirjassa esitettyjen uusien avausten tarkastelua.

Experienced stress in childhood might have been so severe that it has effects throughout the life course. It has been suggested that early life stress may extend consequences on psychological and physical well-being. Previous findings focusing upon consequences of early life stress are however limited and mostly based upon retrospective studies. Still little is known about the consequences of early life stress, such as war separation on physical health from a longitudinal aspect. This thesis aimed to study cardiovascular mortality and morbidity in late adulthood among people who experienced separation without their parents to temporary foster care during World War II (WWII). Other study aims were to examine cardiovascular health, blood pressure levels and physical and psychosocial functioning in the separated participants. People who were not separated served as controls. Participants in this study belong to the Helsinki Birth Cohort Study (HBCS) which includes 13,345 people born in Helsinki between 1934 and 1944 and visited child welfare clinics in the city. Of these 13.5% had experienced evacuation abroad. Data on parental separation was obtained from National Archives, and information on use of medication or events on coronary heart disease (CHD), hypertension, stroke and cardiovascular deaths are obtained from national registers during a follow-up period from 1971-2002. Clinical study cohort included 2003 men and women who participated in the detailed clinical examination during the years 2001-2004. All these participants are included the cardiovascular health sub-study and of them 16.0% were separated participants. Howick et al. have reported the findings of a survey that addressed the use of placebos among primary care practitioners in the United Kingdom. They adopted methodology similar to that used in previous studies performed in other countries; however, the use of this approach also means that they repeated the conceptual confusion of the previous surveys. Therefore the findings are not useful. ... The paper’s main finding “placebos are commonly used in UK primary care” is not correct. Only 0.9% of the responding general practitioners reported using pure placebos frequently. The frequency with which impure placebos are used is irrelevant because the concept is useless, as described above. Misleading a patient by administering inert substances without the explicit consent of the patient is unethical. The authors' proposal to “develop ethical and cost-effective placebos” is not possible because saving money by misleading patients is unethical. There is substantial conceptual confusion in the area of placebo and placebo-effect research, and the paper by Howick et al. does not help to reduce this confusion.

In 1975 Thomas Chalmers analyzed the possible effect of vitamin C on the common cold by calculating the average difference in the duration of cold episodes in vitamin C and control groups in seven placebo-controlled studies. He found that episodes were 0.11 +/- 0.24 (SE) days shorter in the vitamin C groups and concluded that there was no valid evidence to indicate that vitamin C is beneficial in the treatment of the common cold. Chalmers' review has been extensively cited in scientific articles and monographs. However, other reviewers have concluded that vitamin C significantly alleviates the symptoms of the common cold. A careful analysis of Chalmers' review reveals serious shortcomings. For example, Chalmers did not consider the amount of vitamin C used in the studies and included in his meta-analysis was a study in which only 0.025-0.05 g/day of vitamin C was administered to the test subjects. For some studies Chalmers used values that are inconsistent with the original published results. Using data from the same studies, we calculated that vitamin C (1-6 g/day) decreased the duration of the cold episodes by 0.93 +/- 0.22 (SE) days; the relative decrease in the episode duration was 21%. The current notion that vitamin C has no effect on the common cold seems to be based in large part on a faulty review written two decades ago. Non

Anesthesiologists, according to some studies, are highly stressed, die at a significantly earlier age than their colleagues and the general population,and are among the leaders in physicians' suicide records. Data are,however, sparse and contradictory. The aim of this study was to discover details of the work-related well-being of Finnish anesthesiologists. In 2004, a cross-sectional postal survey including all 550 working Finnish anesthesiologists produced a total of 328 responses (60%); 53% were men. The anesthesiologists had the greatest on-call workload among Finnish physicians. Their average in-hospital on-call period lasted 24 hours (range 14 to 38). Over two-thirds felt stressed. The most important causes of stress were work and combining work with family. Their main worries at work were: excessive workload and time constraints, especially being on call, organizational problems, and fear of harming patients. On-call workload correlated with burnout. Being frequently on call was correlated with severe stress symptoms--symptoms associated with sick leaves. Women were more affected by stress than men. High job control and organizational justice seemed to mitigate hospital-on-call stress symptoms. The respondents enjoyed fairly high job and life satisfaction. Job control and organizational justice were the most important correlates of these wellness indicators. Work-related factors were more important in males, whereas family life played a larger role in the well-being of female anesthesiologists. Women had less job control, fewer permanent job contracts, and a higher domestic workload. Of the respondents, 31% were willing to consider changing to another physician's specialty and 43% to a profession other than medicine. The most important correlates for these job turnover attitudes were conflicts at the workplace, low job control, organizational injustice, stress, and job dissatisfaction. One in four had at some time considered suicide. Respondents with poor health, low social support, and family problems were at the highest risk for suicidality. The highest risks at work were conflicts with co-workers and superiors, on-call-related stress symptoms, and low organizational justice. If a respondent had several risk factors, the risk for suicidality doubled with each additional factor. On-call work-burden, job control, fairness of decision-making procedures,and workplace relationships should be the focus in attempts to increase the work-related well-being of anesthesiologists. Anestesialääkärit ovat eräiden tutkimusten mukaan stressaantuneita, kuolevat muita lääkäreitä ja muuta väestöä nuorempina ja ovat lääkärien itsemurhatilastojen kärkipäässä. Aiempien tutkimusten tulokset ovat kuitenkin osittain ristiriitaisia. Väitöskirjatyössäni pyrin selvittämään anestesialääkäreiden työhön liittyvää subjektiivista vointia ja siihen vaikuttavia tekijöitä. Poikkileikkaustutkimuksen aineisto koottiin postikyselyllä v. 2004. Tutkimukseen otettiin kaikki Suomessa työskentelevät anestesiaerikoislääkärit (N = 550). 60 % heistä (328) vastasi. Vastaajista 53 % oli miehiä. Anestesialääkäreillä on suomalaisista lääkäreistä suurin päivystystaakka. Sairaalapäivystysrupeama saattoi kestää jopa 38 tuntia. Yli kaksi kolmasosaa vastanneista koki olevansa stressaantunut. Suurimmat stressin syyt olivat työ ja työn ja perheen yhteen sovittaminen. Työssä eniten huolestuttivat kiire ja työn määrä etenkin päivystäessä, organisaatioon liittyvät ongelmat sekä pelko potilaan vahingoittamisesta. Runsas päivystäminen oli yhteydessä jopa vakavaan stressioireiluun, joka puolestaan liittyi sairauslomalla oloon. Hyvä työn hallinta ja organisaation oikeudenmukaisuus näytti vähentävän raskaisiin sairaalapäivystyksiin liittyvää päivystysstressioireilua. Naiset kärsivät stressioireilusta miehiä enemmän. Huolimatta stressaantuneisuudestaan anestesialääkärit olivat melko tyytyväisiä työhönsä ja elämäänsä. Oman työn hallinta ja organisaation oikeudenmukaisuus olivat tärkeimmät tekijät työhyvinvoinnissa. Työhön liittyvät tekijät olivat tärkeämpiä miehillä, kun taas perhe-elämä oli tärkeämpää naisten hyvinvoinnille. Naisilla oli miehiä vähemmän työnhallintamahdollisuuksia sekä pysyviä työsuhteita, mutta enemmän kotitöitä. Lähes kolmannes vastaajista oli halukkaita harkitsemaan ammattinsa vaihtamista toiseen lääkärin ammattiin ja 43 % muuhun kuin lääkärin ammattiin. Tärkeimmät tällaiseen suhtautumiseen vaikuttavista tekijöistä olivat konfliktit työyhteisössä, huono työn hallinta, organisaation epäoikeudenmukaisuus, stressi ja tyytymättömyys työhön. Neljäsosa vastaajista raportoi harkinneensa itsemurhaa. Suurimmat riskit itsemurha-ajattelulle olivat huono terveys, vähäinen sosiaalinen tuki ja perhe-ongelmat. Työhön liittyvistä riskeistä suurimmat olivat konfliktit työyhteisössä, päivystysstressi ja organisaation epäoikeudenmukaisuus. Riskitekijöiden kumuloituessa riski kaksinkertaistui jokaisen uuden tekijän myötä. Päivystysrasitetta tulisi vähentää, ammatillista itsemääräämisoikeutta lisätä sekä parantaa organisaation oikeudenmukaisuutta ja työpaikan ihmissuhteita haluttaessa kohentaa anestesialääkärien työhyvinvointia.

The risk of upper respiratory infections (URIs) is increased in people who are under heavy physical stress, including recreational and competitive swimmers. Additional treatment options are needed, especially in the younger age group. The aim of this study was to determine whether 1 g/day vitamin C supplementation affects the rate, length, or severity of URIs in adolescent swimmers. We carried out a randomized, double-blind, placebo-controlled trial during three winter months, among 39 competitive young swimmers (mean age 13.8 ± 1.6 years) in Jerusalem, Israel. Vitamin C had no effect on the incidence of URIs (rate ratio = 1.01; 95% confidence interval (CI) = 0.70-1.46). The duration of respiratory infections was 22% shorter in vitamin C group, but the difference was not statistically significant. However, we found a significant interaction between vitamin C effect and sex, so that vitamin C shortened the duration of infections in male swimmers by 47% (95% CI: -80% to -14%), but had no effect on female swimmers (difference in duration: +17%; 95% CI: -38% to +71%). The effect of vitamin C on the severity of URIs was also different between male and female swimmers, so that vitamin C was beneficial for males, but not for females. Our study indicates that vitamin C does not affect the rate of respiratory infections in competitive swimmers. Nevertheless, we found that vitamin C decreased the duration and severity of respiratory infections in male swimmers, but not in females. This finding warrants further research.
