Introdução: na classificação da OMS de 2017, o fibroma cemento-ossificante se enquadra no grupo de tumores odontogênicos benignos mesenquimais/ectomesenquimais, sendo definido como uma neoplasia rara de tecido conjuntivo fibroso maduro, clinicamente se apresenta como lesão exofítica bem delimitada na gengiva, de superfície lisa, formando nódulos macios ou sólidos, com base séssil ou pediculada de coloração rósea, consistência chegando por vezes à dureza óssea. Objetivo: mostrar caso clínico de paciente gênero feminino de 32 anos, apresentando massa nodular, pediculada, com coloração normocrômica em região vestibular e lingual de rebordo mandibular esquerdo, tamanho de 5cm, com consistência firme à palpação. Material e método: por exames de imagem de tomografia computadorizada foi observada lesão hipodensa nodular, expansiva e corticalizada, em vertente esquerda do corpo mandibular. Para elucidação diagnóstica, foi realizada bópsia excisional, sendo a peça foi submetida à análise histopatológica. Resultado: feita a interrelação das características clínicas, imageológicas e exame histopatológico, obteve-se diagnóstico de fibroma odontogênico cemento-ossificante periférico. Conclusão: o controle dos prováveis fatores causais auxilia na prevenção, para pacientes já acometidos e tratados, faz-se necessário o acompanhamento e orientações pós-operatória para evitar recidiva.

Os mixomas odontogênicos são neoplasias benignas, localmente agressivas, de crescimento lento, que provavelmente derivam do ectomesênquima odontogênico, caracterizadas por invasão local e tendência a recidiva. Vários tratamentos são propostos, variando de enucleação conservadora a ressecção radical, de acordo com as características da lesão. O presente artigo apresenta o relato de caso de uma paciente do sexo feminino, 23 anos de idade, diagnosticada com mixoma odontogênico no corpo mandibular esquerdo. Clinicamente, não se observou expansão cortical digna de nota, apenas discreto apinhamento dentário com mucosa normocorada. O exame radiológico evidenciou lesão hipodensa, multilocular, de aproximadamente 3,5 cm, com aspecto de “bolhas de sabão”, que se estendia por todo o corpo mandibular esquerdo. Foi realizada ressecção cirúrgica da lesão, com margem de segurança de 1,5 cm, e fixação com placa de reconstrução do sistema 2.7. A ressecção ocorreu satisfatoriamente, obedecendo a margem de segurança preconizada, com os cotos macroscopicamente livres de lesão. O paciente evoluiu bem, com funções fisiológicas e motoras preservadas e, no acompanhamento pós-operatório de seis anos, não foi observada recidiva da lesão. Pode-se concluir que é imprescindível que a escolha do tratamento de mixoma odontogênico, utilizando abordagem radical ou conservadora, leve em consideração critérios individuais clínicos e radiológicos de cada caso, para que haja maior efetividade no tratamento, com segurança ao paciente.

O Fibroma Ossificante Juvenil (FOJ) são lesões fibro-ósseas dos maxilares benignas de origem mesenquimal. Apresenta comportamento clínico mais agressivo e, comumente, acomete crianças e jovens na faixa etária de 5 à 15 anos e é mais comum em maxila. As lesões necessitam de remoção cirúrgica como forma de tratamento. Histologicamente, essa lesão caracteriza-se por estroma fibroso densamente celularizado, exibindo depósitos de osteoide desprovido de margem osteoblástica, associados a trabéculas de osso imaturo mais típicas, no caso de FOJT, e ocasionais estruturas psamomatoides, evidenciadas em FOJP. Este artigo apresenta o tratamento cirúrgico de dois casos, de Fibroma Ossificante Juvenil sendo um Fibroma Ossificante Juvenil Trabecular e outro Fibroma Ossificante Juvenil Psamomatoide. Em um dos casos houve a necessidade de reconstrução com placa de titânio para devolver estética e função ao paciente.

A queilite actínica (QA) é uma lesão comum do lábio inferior causada principalmente pela exposição com frequência elevada ou crônica à radiação solar. É considerada uma lesão cancerizável, onde o diagnóstico precoce pelo profissional é de extrema importância para o bom prognóstico do caso visto que a QA muitas vezes é a lesão precursora do carcinoma espinocelular de lábio inferior. Na área da saúde, é importante realizar um atendimento integral do paciente, criando uma interseção entre sua queixa principal e as necessidades que ele apresenta para que se consiga reestabelecer a saúde geral. O presente estudo visa apresentar um caso clínico de um paciente do gênero masculino, 38 anos de idade, que buscou o serviço odontológico para o tratamento de uma disfunção temporomandibular mas que, ao seu atendimento integralizado, percebeu-se a presença de uma lesão nodular em ventre lingual e de uma mácula leucoplásica em vermelhão de lábio inferior. Houve indicação para biópsia excisional da lesão nodular e de biópsia incisional para a lesão leucoplásica. Após o diagnóstico de neuroma traumático em língua e de queilite actínica em lábio inferior, com a confirmação histopatológica, foi realizada uma biópsia excisional simples. Após a remoção da lesão o paciente retornou para remoção de sutura e para proservação do caso, onde apresentou boa recuperação.

A sialadenite esclerosante crônica foi descrita pela primeira vez na Alemanha por Kuttner, em 1896. É uma lesão benigna semelhante a um tumor que afeta predominantemente a glândula submandibular e é uma doença inflamatória crônica relativamente incomum da glândula salivar. Esta lesão é caracterizada histologicamente por atrofia acinar, fibrose periductal e infiltração de glândulas por linfócitos com formação de folículos linfóides, perda de ácino e, eventualmente, esclerose acentuada da glândula salivar e geralmente ocorre em idosos e adultos de meia idade com ligeira predominância masculina. O relatório descreve um paciente, R.J.R.C, um homem de 53 anos, apresentando um histórico de dois anos de massa sublingual moderada e indolor, relacionada a um edentulismo parcial dos dentes 46 e 47.

As malformações vasculares são anomalias estruturais dos vasos sanguíneos sem proliferação endotelial, estão presentes ao nascimento e persistem ao longo da vida. Elas podem ser categorizadas de acordo com o tipo de vaso envolvido (capilar, venoso, arteriovenoso) e de acordo com as características hemodinâmicas (baixo fluxo ou alto fluxo). Possuem uma etiologia variável, como fatores químicos, mutações genéticas ou alterações relacionadas a síndromes, fluxo sanguíneo, resistência da parede vascular e trauma. Neste caso, a malformação vascular apresentada pela paciente tem relação com o trauma em mucosa jugal devido a atividade parafuncional presente. Atividade parafuncional refere-se a qualquer atividade que não seja considerada funcional, como apertar e/ou ranger os dentes durante o dia e/ou à noite, mascar chicletes, morder bochecha, lábios e língua. Algumas destas atividades podem ser responsáveis por concorrer para a Disfunção Temporomandibular (DTM), pois promovem hiperatividade muscular mastigatória. Dor muscular, restrição de abertura de boca, sensação de plenitude auricular, zumbido na orelha e vertigem foram alguns dos sintomas apresentados pela paciente. Diante desse quadro, além de tratar a malformação vascular com uma combinação de escleroterapia e excisão cirúrgica, foi também tratado o quadro disfuncional, para seu controle e resolução além da prevenção de recidiva da lesão, com a utilização de placa miorrelaxante combinada com a realização de exercícios fisioterápicos, mostrando-se eficazes, contendo a sintomatologia, restringindo a atividade parafuncional e reduzindo o trauma na mucosa.

Introdução: O osteoma é um tumor benigno, de crescimento lento e indolor, composto de osso maduro que pode ser compacto e/ou esponjoso, afetando geralmente os ossos gnáticos. Objetivo: mostrar um caso clínico de osteoma trabecular, que afetou região edêntula do elemento 36 em paciente do sexo feminino, feoderma de 36 anos onde, há aproximadamente 3 anos, o elemento fora extraído devido à destruição por cárie e não feito nenhum tratamento reabilitador. Material e método: Realizado exame clínico e tomográfico, sendo neste observado em corpo mandibular, na região edêntula do elemento 36, uma imagem hiperdensa irregular, mas bem delimitada de aproximadamente 5 cm em seu maior diâmetro, circunscrita por área hipodensa. A interrelação desses exames levou às hipóteses diagnósticas de osteoma, osteoblastoma e odontoma complexo, que, após biópsia excisional e análise histopatológica, se confirmou o diagnóstico de osteoma. Resultado: Realizada a interrelação clínica, imagenológica e histopatológica, obteve-se o diagnóstico de osteoma trabecular. Conclusão: a remoção completa da lesão foi importante para a confirmação diagnóstica e configurou seu tratamento, sendo raros os casos recidivantes.

Introdução: A doença de Paget (DP) ou osteíte deformante é uma doença osteometabólica benigna, caracterizada pela reabsorção e deposição anormal e anárquica de osso, este osso é menos resistente e mais suscetível a fraturas. Objetivo: mostrar caso clínico de manifestação atípica de DP que afetou área de seio maxilar (maior intensidade) e outras regiões como cóccix, tubérculo ilíaco direito e esquerdo e articulação sacroilíaca direita e esquerda, numa paciente do sexo feminino de 63 anos que queixava dor em hemi-maxila esquerda com aumento progressivo local desde a juventude. Esta acometia região dos dentes remanescentes ipsilaterais 23 e 24 com doença periodontal crônica grave. Material e método: Por exames de imagem observou hiperdensidade que preenchia quase toda a extensão do seio maxilar esquerdo com áreas de hipodensidade e na cintilografia apresentava maior captação de radiofármaco também nesta localização. Para elucidação diagnóstica, cujas hipóteses contemplavam, além de DP, também tumor marrom do hiperparatiroidismo e osteossarcoma, foram solicitados exames laboratoriais: TSH, creatinina, PTH, T3, T4, fosfatase alcalina, albumina, microalbumina, TGP, cálcio sérico, aldolase e calcitonina que não apresentaram nenhuma alteração, sendo indicada a biopsia incisional para análise histopatológica e conclusão diagnóstica. Resultado: Feita a interrelação das características clínicas, laboratoriais, imagenológicas e exame histopatológico, obteve-se diagnóstico de DP. Conclusão: Após diagnóstico, paciente foi encaminhada para tratamento reumatológico. O tratamento ordinariamente transcorre com o uso de bifosfonatos para redução da remodelação óssea, sendo amplamente usado o ácido zoledrônico além de analgésicos para o controle da dor.

A mucocele é considerada uma lesão comum da mucosa oral, que resulta da ruptura de um ducto de glândula salivar e extravasamento de mucina para dentro dos tecidos moles adjacentes. Frequentemente encontrada no lábio inferior, o presente trabalho descreverá um caso de mucocele em mucosa do palato, considerada um região atípica, para muitos autores. Paciente do gênero feminina, 51 anos de idade, melanoderma, foi encaminhada para a Policlínica odontológica da UEA, com hipótese diagnóstica de hemangioma. Na anamnese, paciente relatou tempo de evolução de 5 meses, ausência de sintomatologia dolorosa, negou possuir hábitos de sucção e não ter traumatizado a área durante a alimentação ou escovação, além de não possuir hábito tabagista, etilista ou fazer uso de prótese, e sem histórico de doença sistêmica. Diante das informações clinicas, estabelecemos hipóteses diagnósticas: Carcinoma Mucoepidermóide e Mucocele. O diagnóstico foi confirmado através de biopsia excisional da lesão e exame histopatológico. Portanto, mesmo se tratando de uma lesão simples, podemos nos deparar com dificuldade diagnóstica, quando a localização da lesão é incomum. Entretanto, cabe ressaltar a importância de uma análise diferencial do caso, que para o presente trabalho, foi de grande importância para se chegar ao diagnóstico e assim proporciona um tratamento eficaz para a paciente.

Sialolitíases de pequenas proporções localizadas na porção terminal do ducto podem ser difíceis de serem eliminadas de forma passiva, causando problemas ao paciente como episódios de dor e desconforto, podendo evoluir para complicações como infecção ou retenção mucosa. O presente trabalho relata um caso de sialolitíase de uma paciente de 35 anos. Ao exame clínico constatou-se a presença de uma elevação nodular na região de carúncula sublingual, formato cilíndrico, indolor e móvel à palpação, endurecida, coloração amarelada, aumento de volume sensível no pescoço durante algumas refeições, e evolução de sete meses. Ao exame tomográfico, foi possível observar a presença de uma massa hiperdensa na região de assoalho bucal. Inicialmente foi realizado uma ordenha associada a colocação de cone de guta percha, mas não se obteve sucesso. O tratamento realizado foi a remoção cirúrgica do sialólito sem necessidade de síntese associado ao uso tópico de Mud Oral juntamente com a orientação da ordenha para estimulação do fluxo salivar. A hipótese diagnóstica para esta lesão foi de sialolitíase e a mesma foi confirmada através da análise histopatológica. A paciente, continuou em acompanhamento pós-operatório durante 3 semanas, durante este período, a mesma não desenvolveu nenhuma sintomatologia relevante, sendo dispensada após 21 dias. Portanto, diante das possibilidades clínicas e situação da paciente optou-se por métodos mais conservadores e menos invasivos associados a orientações de cuidados pós-operatórios para que se obtivesse uma rápida recuperação e um bom prognóstico.

O carcinoma espinocelular (CEC) é a neoplasia maligna mais comum da cavidade oral, sendo seus agentes etiológicos principais: o álcool, tabaco e o Papilomavirus Humano (HPV) de alto risco, porém, existem outras possibilidades etiopatogênicas ainda não amiudadas pela ciência. Este trabalho relata caso de paciente do gênero feminino, 48 anos, não tabagista, não etilista e sem sinal de infecção pelo vírus HPV, com queixa de lesão nodular em ventre lingual. Relatou hábito de “chupar a língua” desde tenra idade, boca seca e ardência bucal há alguns meses e, ao exame físico, apresentou ordenha negativa das glândulas parótidas, baixa coleta nas sublinguais e submandibulares. Apresentava sinais de comprometimento muscular cervical, massetérico e, por conseguinte, bucinatório, condição que pode concorrer fisicamente para estenose do ducto parotídeo e redução da secreção parotídea. Após diagnóstico de CEC, a paciente foi submetida a tratamento cirúrgico, radio e quimioterápico. Para o controle do hábito parafuncional, provavelmente relacionado à lesão que evoluiu para o CEC, foram realizados: uso de dispositivo interoclusal, fisioterapia e crioterapia com prescrição de saliva artificial e creme reparador para as lesões mucosas, decorrentes da hipo/assialia. Consideramos que a associação do trauma crônico, fator modificador clássico, resultante da atividade parafuncional e a redução do fluxo salivar promovida- entre outras razões orgânicas primárias, prévias ao tratamento radio e quimioterápico- pelo potencial estreitamento do ducto de Stenon em razão da disfunção mastigatória, como promotora da perda de homeostasia local e não controle de iniciadores endógenos e/ou exógenos, concorreram para o desenvolvimento da condição neoplásica.

Diante das neoformações malignas de cabeça e pescoço, o carcinoma mucoepidermoide apresenta-se como uma das manifestações de grande ocorrência, sendo a região do palato a mais envolvida. Podendo o tratamento resultar em mutilações na cavidade oral, ocasionando comunicação bucosinusal, a qual poderá ser restaurada por intermédio de próteses obturadoras. O objetivo do presente trabalho é relatar um caso clínico de confecção de prótese obturadora palatina oca em paciente portador de carcinoma mucoepidermoide após maxilectomia parcial, e, a técnica de moldagem facial prévia a tratamento cirúrgico de recidiva. Paciente buscou atendimento odontológico, mencionando apresentar sua prótese total obturadora mal adaptada devido o tempo de uso. Seu plano de tratamento, consistiu na confecção da prótese obturadora pela técnica da clonagem e prótese parcial removível convencional. Após alguns meses, detectou-se clinicamente a recidiva da lesão, onde foi realizado novo procedimento cirúrgico, o qual não obteve sucesso. Então optou-se por outra abordagem, desenvolvendo a moldagem facial, pois uma cirurgia mais invasiva seria necessária, podendo trazer consequências na face. A paciente encontrava-se em tratamento radioterápico, sendo a nova intervenção médica escolhida. A prótese obturadora é uma alternativa eficaz na reabilitação de pacientes submetidos a ressecções cirúrgicas, as quais resultaram em mutilações. Sendo assim, a escolha pela técnica da clonagem com algumas modificações, nesse caso além do custo-benefício, as etapas clínicas e laboratoriais foram reduzidas e como consequência a substituição da prótese foi mais rápida eficaz. Além disso, é de grande relevância o papel dessa reabilitação na reintegração do indivíduo no meio biopsicossocial, o restabelecimento da função, estética e conforto.

Introduction: Necrotizing sialometaplasia (NS) is a rare locally destructive inflammatory benign disease that commonly affects the minor salivary glands. It is frequently associated with the glands located in the posterior portion of the hard and soft palates. Low-level laser therapy (LLLT), also called photobiomodulation therapy (PBMT), has been deemed a substantial method for the regenerative wound process. Case Presentation: A 32-year-old male patient was referred with a chief complaint of two asymptomatic crater-like ulcers measuring approximately 1.5 cm wide on the right side of the hard and soft palates, and another measuring 0.3 cm wide on the left side of the hard palate. The lesion had two weeks of evolution followed by a previous infectious "sore throat" event that kept the patient hospitalized for 4 days. A clinical diagnosis of NS was made. LLLT was applied during 2 sessions per week, favoring the total wound healing within 2 weeks. At 3 months of clinical followup, the patient did not present any complication or relapse and was thus released. Conclusion: This is, to our knowledge, the first clinical report of LLLT applied for the management of NS. Large palatal ulcers caused by NS usually have long healing periods. The shortened healing period observed in this case encourages the inclusion of LLLT in any treatment protocol for similar lesions.

Pediatric mandibular tumors present an aggressive biological behavior and difficult diagnosis. A wide range of odontogenic and nonodontogenic tumors comprise the spectrum of these lesions. We report a case of a 1-year-old male child patient showing facial asymmetry symptomatic of an expansive lesion extending throughout the body and ramus of the left hemimandible with a diameter of 8 cm. The histopathological report suggested a high-grade mucoepidermoid carcinoma (MEC), recommending further immunohistochemical investigation of the ectomesenchymal or neuroectodermal origin of the tumor cells. The patient evolved with extensive bilateral pleural effusion followed by metastasis in the middle third of the right humerus, and died 2 months after the first biopsy procedure by acute renal failure with tubular necrosis, before a final inconclusive immunohistochemical report was reached. The lack of resources for less-favored regions of Brazil impairs rapid biomolecular examinations such as immunohistochemical resulting in delay of appropriate therapeutic procedures.

Methodology: Twenty extracted human maxillary incisors with PGG were subjected to macroscopic analysis and scanning electron microscopy analysis (SEM). The following characteristics of the PGG were evaluated: depth, point of origin in the cingulum, extension and position on the lingual surface. Furthermore, the presence of calculus, communications between the root canal system and the PGG, and root resorptions were investigated. The root canals were subsequently instrumented with K-files of three consecutive sizes. The teeth were sectioned, and the axial plane of each tooth section was imaged using SEM before and after instrumentation. The distance between the root canal walls and the PGG was calculated according to the location. Additionally, the distance between canal walls and cementum was measured at three different sites, to verify if instrumentation influenced dentine removal on a specific wall in teeth with PGG. Statistical analysis was performed using the Mann–Whitney or Student’s t-test (P 0.05). Conclusions: Palatogingival grooves were characteristically deep and originated from the distal margin of the cingulum. Although it has been associated with a thinner root wall, root canal preparation did not influence the thickness of the specific wall in the maxillary incisors with PGG. Background Oral leukoplakia is the most common potentially malignant lesion, the etiology of this change is still uncertain which makes it difficult to predict when this will undergo malignant transformation. Recent studies have examined the involvement of mast cells in this process, it is believed that these cells through the release of pro-angiogenic factors influence the processing mechanism of such lesions through promoting angiogenesis. It was the objective of this study to evaluate the inflammatory process in oral leukoplakia comparing it to a non-cancerous lesion correlating the intensity of mast cells with inflammatory infiltrate and vascularization in these lesions. Methods A total of 20 selected cases divided into two groups, oral leukoplakia and inflammatory fibrous hyperplasia (IFH). Histochemical Technique of Toluidine Blue was applied to enhance mast cells and histomorphometry for quantifying mast cells, blood vessels and inflammatory cell infiltration by capturing digital images of the samples and analysis software Image Java version 1.48v. Mast cells and blood vessels were counted in 10 different areas and the intensity of the inflammatory infiltrate was measured in three areas. The analysis of variance (ANOVA) was used for between-group variation, a P value of 0.05 for statistical significance. Results It was observed a reduction in the concentration of mast cells in oral leukoplakia compared fibrous inflammatory hyperplasia. In contrast to the concentration of blood vessels were present in higher concentrations in the Oral leukoplakia lesions. The inflammatory infiltrate in most cases was considered scarce, except in a case where that IFH was intense and a case of moderate inflammatory infiltrate in oral leukoplakia. Conclusions The results showed reduction in the number of mast cells in the oral leukoplakia and greater intensity in the inflammatory process in the IFH, suggesting a possible carcinogenic influence in the microenvironment oral leukoplakia.

No. of bitstreams: 0 Previous issue date: 2022-04-01 Fundação de Amparo à Pesquisa do Estado do Amazonas The current research aims to perform a comparative evaluation of vegetable matter involved lesions with oral parasitic infections found in oral mucosa, presenting histochemical methods to differentiate their microscopic similarities. Eight cases were selected out of a sample of 1.975 reports from a single Oral and Maxillofacial Pathology Service of the author's institution from 2012 to 2019. Specimens were examined by hematoxylin and eosin staining (HE), periodic acid-Schiff (PAS) staining, Gomori–Grocott staining, Ziehl–Neelsen staining, Giemsa, and mucicarmine staining. Microscopic analysis included fluorescence, polarized light, and confocal microscopy. Microscopically, in HE coloration, hookworm eggs showed as eosinophilic. Inflammatory multinucleated giant cells and lymphocytes, were usually related to the nematode eggs, forming an intense inflammatory infiltrate. Biofluorescent properties of eggs and larvae revealed to be sensitive in the detection of parasitic structures contrasting with the inflamed connective tissue. Vegetable presence was confirmed by polarized light microscopy and it was found to be associated with microbial biofilms. Confocal microscopy has showed to be an excellent method for morphotype differentiation of parasitic eggs. Parasitic infection and vegetable matter displayed similarities in the inflammatory response, but the latter can rot and agglomerate biofilms. The microscopic diagnosis of such infections requires the interpretation of challenging morphological features since the parasites are usually sectioned and mixed with an inflammatory reaction. These histochemical approaches proved to be excellent to distinguish both lesions. Background: We aimed to present a minimally invasive clinical approach to collect a suitable sample for the laboratory diagnosis of desquamative gingivitis. Materials and Methods: The proposed technique involves Nikolsky's test for the collection of tissue samples. It consists of the histopathological analysis of the bullous membrane formed in cases with positive Nikolsky's sign (NS). Three patients without a previous diagnosis of the disease agreed to undergo this protocol before a biopsy. Results: The diagnoses of the three cases reported here were mucous pemphigoid (MP), pemphigus vulgaris (PV), and lichen planus (LP). The tissue samples collected using this technique were sufficient to microscopically observe partial-thickness epithelium (diagnosing PV) or full-thickness epithelium (diagnosing MP). The diagnosis of LP was only possible by conventional biopsy. Conclusion: We observed, for different cases, some advantages of the proposed technique: minimally invasive, potentially superior to exfoliative cytology, easily performed, and with low costs.

Endodontics has gained emphasis in the scientific community in recent years due to the increase in clinical and in animal models studies focused on endodontic medicine, which aims to evaluate the interrelationship between systemic and periapical tissues pathological conditions. These studies have shown that systemic changes can boost the pathogenesis of endodontic infection, favoring its development and progression. A contrary relationship is reported in numerous studies that affirm the potential of endodontic infection to trigger systemic damage and may lead to the worsening of pre-existing pathologies. Recently, the potential of filling materials to develop systemic changes such as neurological alterations had been evaluated, also showing that systemic diseases can negatively influence tissue responses to filling materials after endodontic treatment. Despite advances in endodontic medicine studies, there are still gaps in knowledge on the mechanisms of interactions between apical periodontitis (AP) and systemic diseases and much research to be done. In this sense, this critical narrative literature review aimed to show the evolution of studies in endodontic medicine to help the endodontist to know the role of systemic diseases in the pathogenesis of AP and the possible interference in the repair of periapical tissues after endodontic treatment, as well as to evidence the systemic complications that can be triggered or aggravated in the presence of endodontic infection.

The purpose of this study was to investigate the tissue reaction stimulated by BaSO4 - and Bi2 O3 -containing White MTA Angelus, in comparison with Bi2 O3 -containing white Portland cement, and white ProRoot MTA. Thirty-six adult male Wistar rats (Rattus norvegicus), weighing between 250 and 300 g, were distributed into three groups (n = 12) in accordance with the period of sacrifice (15, 30, and 60 days). Four polyethylene tubes filled with the tested cements were implanted into the dorsum of each rat. Lateral wall of the tubes served as the negative control. After the experimental periods, the animals were euthanized by overdose of pentobarbital anesthetic solution, and the specimens were prepared for microscopic analysis under ×50, ×100, and ×400 magnifications. The investigation of periodontal disease requires a clinical approach and complete intraoral radiographs, even with increasing concerns about radiation exposure. Thus, this study assesses pixel linear analysis accuracy using digital radiography via Digora® in detecting alveolar bone destruction in diabetic rats with periodontal disease. Methodology: 40 rats were divided into groups (n = 10): control (C), rats with periodontal disease (PD), experimental diabetic rats (ED), experimental diabetic rats with periodontal disease (ED-PD). Diabetes was induced by streptozotocin and periodontal disease by periodontal ligature. After 30 days, maxillae bone destruction was obtained by linear analysis of vertical bone loss using digital radiography and then assessed by micro-CT and histology. Data were analyzed by ANOVA and Tukey's test (p < 0.05). Results: Radiographic, micro-CT and histological analysis presented accurate and similar results. PD and ED-PD groups showed higher bone destruction than C and ED groups (p < 0.05). Moreover, the ED-PD group had higher bone loss than the PD group (p < 0.05). Conclusion: The pixel linear analysis via digital radiography was an accurate, low-cost alternative in detecting alveolar bone loss in this rat model. Micro-CT and histological analysis may also be used to obtain linear measures to assess and compare periodontal bone destruction in diabetic rats. A hiperplasia palatina por câmara de sucção (HPCS) ou hiperplasia do palato está associada ao uso de próteses totais mucosuportada superiores com câmara de vácuo. A Câmara de sucção é uma depressão que os protéticos ou práticos realizavam na porção interna da prótese total superior em sua porção central, com o objetivo de promover uma maior estabilidade. O objetivo deste trabalho foi relatar o diagnóstico de um caso clínico odontológico de uma paciente com Hiperplasia Palatina por câmara de sucção, atendida na Disciplina de Diagnóstico Bucal, do curso de Odontologia, da Universidade do Estado do Amazonas evidenciando os aspectos clínicos gerais e bucais. Foi feito apenas o diagnóstico da lesão já que a paciente estava de viagem marcada para outro estado, não contribuiu para a troca das próteses superior e inferior. Ademais foi deixado bem claro, que deveria ser feito uma cirurgia para remoção da lesão, e também, a confecção de uma nova prótese, já que a prótese em questão não era mais condizente para o uso. Early diagnosis of neurofibromatosis is significant to start the multidisciplinary approach of this type of patient. This syndrome may be first identified in pediatric dental care. This study is about a 9-year-old patient who was diagnosed with neurofibromatosis type 1 from a plexiform neurofibroma displayed in the left hemi-maxilla. From the microscopic diagnosis of the lesion the investigation of other related injuries began, according to the diagnostic criteria of the National Institute of Health (NIH), leading to the diagnosis of systemic alterations. The parental awareness about the condition and the establishment of multidisciplinary care are important for the treatment to be more conservative and the patient has fewer complications with better quality of life. The use of the diagnostic criteria in this case shows how methodization is important for reaching more accurate and reliable diagnoses.

Background: Maxillary sinus mucocele is a benign cystic lesion caused due to mucus retention. The aim of this article is to relate a singular case of MSM development after dental extraction. We believe that previous dental extraction procedure of teeth 26, must have established some degree of Oroantral Comunnication, allowing the maxillary sinus epithelial inclusion into the alveolar socket. Case presentation: A 32-year-old male patient presented to the Oral Medicine service in the authors’ institution, with a chief complaint of left-sided facial swelling with slow growth and development for 2 years. Intra oral examination revealed a symptomatic circumscribed lesion of approximately 5–6 cm without mobility, firm on palpation, located in the vestibular sulcus on left-sided maxilla. It was situated in the region between teeth 25 to 27, with the teeth 26 being absent The lateral wall was surgically removed from the cystic cavity, followed by conservative decompression allowing gradual pneumatization of the maxillary sinus. The lesion response comprised regression in size, swelling, and pain, preserving the affected site and benefiting the patient with possible rehabilitation in the future. At the 2-year follow-up, the patient reported no complications. Conclusion: This study case demonstrated that MSM could be confused with tumors from an odontogenic nature, sometimes presenting an aggressive growth pattern. Although numerous treatment modalities exist for the treatment of MSM, the conservative technique promotes effective tissue preservation and benefits to the patient.

O fibroma odontogênico periférico é uma massa relativamente rara, benigna, não encapsulada e gengival de tecido conjuntivo fibroso, considerada a contrapartida extra-óssea do fibroma odontogênico central. Ele foi anteriormente bastante comumente confundido com o fibroma ossificante periférico, até a classificação da OMS (1992), classificado em tumores odontogênicos. O objetivo deste artigo é relatar um caso de fibroma odontogênico periférico gigante de tipo rico em epitélio, avaliando e analisando, segundo a literatura, as suas características. Utilizou-se como ferramenta de diagnóstico a coloração Picrossírius vermelho em campo claro, luz polarizada e fluorescência, uma técnica histológica comumente usada para visualizar colágeno pela determinação da histoquímica da cor de polarização.

No. of bitstreams: 0 Previous issue date: 2018-12-01 Pilomatrixoma is considered a rare benign tumor arising from the hair follicle, most common in the head and neck region, but it is rarely diagnosed on a clinical basis. This report describes a new case of giant pilomatrixoma in a 36-year-old female patient. The nodule was localized in the preauricular area on the right side, appearing as a slow-growing, fixed, painless, with a hardened consistency, unusual giant (4.5 cm). A cone-beam computed tomography (CBCT) examination showed a slightly hyperdense lesion, and fine-needle aspiration cytology (FNAC) revealed peripheral blood and mononucleated inflammatory cells. After enucleation of lesion, a diagnosis of pilomatrixoma was confirmed. The differential diagnosis of pilomatrixoma is broad, because its characteristics also can be found in other lesions common to the head and neck. Thus, a lesion in the head and neck, adherent to the skin, and well demarcated, mainly in the young and in females, should be suspected as pilomatrixoma. Inflammatory pseudotumors are a group of lesions of unknown etiology that mimic clinically and radiographically neoplasms. In the maxilla, inflammatory pseudotumors are presented with bone alterations of erosion, remodeling, and sclerosis. The diagnosis is of exclusion, where multiple biopsies are required. The present study aims to report the case of a male patient who presented with increased volume in the left maxillary region, with diagnosis after total left maxillectomy being inflammatory pseudotumor. The patient did not present recurrences with 3 years of preservation and underwent by multidisciplinary treatment with esthetic and functional rehabilitation with the preparation of a bucomaxilo prosthesis. Despite presenting some suggestive clinical features, the inflammatory pseudotumor has a difficult and of exclusion diagnosis, where multiple biopsies are required. They are lesions that simulate clinically and radiographically neoplasms. If it is surgically accessible, the treatment of choice is complete surgical resection.

We report a unique case of a potassium aluminum sulfate oral dissolution leading to palatal chemical necrosis and extensive chemical ulcers on the tongue. The patient, a 47-year-old white, blind male, denied using cocaine or other illegal drugs that could cause such lesions. His self-medication started as a treatment for a traumatic ulcerative lesion on the hard palate. After palatal perforation, he started another self-medication routine, mixing corticoid cream and tandrilax tablets with a gauze obturator. Our treatment comprised the removal of all chemical factors, a surgical debridement, and a prosthetic obturator to resolve the communication. The 1-year follow-up showed no complications.

Objective This study aimed to determine the correlation between the vascular mast cells and inflammatory changes in the potentially malignant disorders, compared to the oral squamous cell carcinoma (OSCC) in varying degrees of dysplasia. Materials and Methods Thirty samples were selected: 10 of OSCC, 10 of oral leukoplakia, and 10 of actinic keratosis. The toluidine blue technique was used on the mast cells, and hematoxylin and eosin were used for analyzing the lymphocytes, mitosis, and vessels. The quantification was performed using the ImageJ software after obtaining the images by light microscopy with a × 40 objective. Analysis of variance with p < 0.05 was considered for statistical significance. Results Mast cells (p < 0.0158), vessels (p < 0.9431), lymphocytes (p < 0.0001), and mitoses (p < 0.0009) were found in OSCC. In potentially malignant disorders, a lower density of these structures and a higher concentration of mitosis in the actinic keratosis were observed. Conclusion The results showed evidence of a positive correlation between mast cells and vascularization in the OSCC, suggesting the aggression of the disease. Intense inflammatory infiltrate indicates that other molecular events are involved in the carcinogenesis process, and further studies are necessary for a better understanding of it.

Facial cosmetic procedures are on increasing demand, for plastic surgeons and dental professionals now-a-days. New techniques applied for the renewal offacial contours have become the first treatment option before surgical corrections. This case report highlights some of the risks related to such procedures, reporting a case of a painful fluctuating nodule in the midline of the vestibular sulcus, extending towards the upper lip. The patient mentioned a 2-year history of progression of the lesion. The nodule was 0.8 cm wide, well defined, with normal colour. The excisional biopsy was proposed with initial suspect of pleomorphic adenoma, or other soft tissue tumour. Microscopic evaluation of the excised lesion revealed a foreign body granuloma caused by multiple spherules of transparent synthetic material of uniform diameter, identified as Polymethylmetacrylate (PMMA). The patient then recalled that he underwent an aesthetic procedure of rhinoplasty with facial filling that he did not correlate with the symptoms.

Background Sialadenoma papilliferum is a benign rare condition of salivary glands showing a characteristic papillary growth of the ductal epithelium that ends up being confused with more frequent lesions of the oral cavity. Objectives: To perform a bibliometric analysis of all articles about Sialadenoma papilliferum in the oral cavity and to add a singular case report of Sialadenoma in the lower lip. Material and methods A total of 36 publications referring to Sialadenoma papilliferum in the oral cavity from the PubMed platform was reviewed. The specific data were collected, and a bibliometric analysis was performed using Microsoft Excel. The results obtained were then compared with this new case report. Results The people most affected with sialadenoma were white males at the average age of 56. The lesion was asymptomatic, usually white or red, with an average size of 1.4 cm, and the palate was by far the most affected site. The majority of the lesions were excised, and only two cases indicated recurrence. Conclusions With surgical removal, Sialadenoma papilliferum has a favorable prognosis and no further treatment is required. Due to few recorded cases of recurrence, a long follow-up period is recommended to ensure that the lesion does not redevelop. We report an unusual case of a 1.5-cm dark macule on the edentulous posterior mandibular alveolar ridge. The lesion affected a 62-year-old woman referred by a clinician to the oral medicine service with suspect of melanoma. Clinical investigation found no signs of pain and discomfort. The patient stated she had not noticed the lesion previously. An excisional biopsy was performed and during the excision a loose brownish granular material within the submucosa and over the mandibular bone surface was seen. This material was curetted and submitted for microscopic evaluation. The histopathologic report described a loosely organized connective tissue with blackened granular agglomerates that also stained the elastic collagen fibers dispersed in the matrix and in the vessel wall leading to a foreign body–type granuloma with focal argyrosis (amalgam tattoo). Postsurgical follow-up of 11 months was uneventful.

Kuttner tumor chronic sclerosing sialadenitis is graded in 4 stages, but most of the lesions are usually diagnosed in the 2 intermediary stages. We present a case affecting a female patient, 57 years old and white, who sought the oral medicine service because of a small tendered nodule on the buccal mucosa. Intraoral examination revealed a sessile, painless, and normal-colored fluctuating nodule, resembling a mucocele. After excisional biopsy, the material was submitted to a microscopic evaluation that revealed dilated salivary ducts with ectasia and peripheral fibrosis with intense hyalinization in addition to a discrete periductal lymphocytic infiltrate in the middle of a stroma of adipose tissue of usual appearance. That suggested diagnosis of advanced stage chronic sclerosing sialadenitis (Kuttner tumor). Further clinical and laboratory examinations ruled out the other immunomediated diseases. Postoperative follow-up showed no signs of recurrence, and the patient was released.

The present study reports a rare case of anaplastic malignant melanoma in a 23-year-old female patient, in the anterior maxilla. On clinical examination, the lesion presented as a 5-cm sessile nodule, with brownish irregular macules on the mucosal surface, painless, with a firm consistency ranging from teeth 14 to 24 with palatal and nasal extension causing facial asymmetry and dental mobility. The computed tomography showed irregular lytic bone reabsorption, without dental involvement. The incisional biopsy followed by microscopic examination suggested primitive neuroectodermal tumor, due to the presence of round blue cells, periodic acid-Schiff (PAS)-positive clear cells, with considerable pleomorphism, atypical mitosis, hemorrhagic areas, and coagulative necrosis. Immunohistochemistry revealed high-grade epithelioid neoplasm and a diagnosis of invasive melanoma was concluded; the melanoma was positive for S-100 protein, gp100, and SOX 10 antigens. The patient underwent partial maxillectomy with no postsurgical complications after 3-month follow-up. The aggressive nature of the lesion and its prognosis are discussed.

A 54-year-old black female patient sought dental care due to a sessile, fluctuating nodule on the left hemiface, normal colored, with an 8-mm width. During clinical examination, a large lump on the dorsum was noticed, which was suggested to be a lipoma by the patient. Fine needle aspiration cytology was carried out followed by an excisional biopsy. Microscopic evaluation diagnosed a nodular neurofibroma, and advised further investigation to rule out the chance of being a case of neurofibromatosis. Additional clinical examination found cafe-au-lait spots, Lisch nodules, multiple subcutaneous nodules on other parts of the body, and enlarged fungiform papillae of the tongue. Neurofibromatosis type I is the most common manifestation of the syndrome, but as shown in the present study, its diagnosis can be difficult because of the discrete presentation of its stigmata, caused by skin and eye color, as well as absence of superficial neurofibromas.

Intraneural perineurioma is a rare lesion of the peripheral nervous system with unique ultrastructural features, and it is distinct from Schwann cell-derived entities (schwannoma and neurofibroma). The present case aims to report a lesion in the lower lip in a 33-year-old female patient. Clinical examination showed a nodular growth, sessile, and smooth surfaced from an undetermined etiology. Histopathologic examination revealed slender spindle cells, arranged in 2 fascicles, and focal areas showing a distinct storiform pattern, surrounding by histiocytes. Histopathologic investigation comprised of toluidine blue, Masson trichrome, and picrosirius red showed presence of mast cells, collagen organization, and type III prevalence. Based on clinical and histopathologic findings, the final diagnosis was intraneural perineurioma. Eight-month follow-up showed no recurrence. We will discuss histopathologic and histochemical aspects of interest for intraneural perineurioma diagnosis.

O mioespasmo é uma alteração involuntária sustentada por mediação central. É uma alteração em que o músculo permanece contraído por alguns segundos, minutos ou mesmo dias e também é frequentemente observado durante tratamento em que necessitam de sessões prolongadas, como, por exemplo, tratamentos endodônticos de molares. Depois de determinado tempo, o paciente mostra instabilidade para manter a boca aberta, apresentando episódios de fechamento repentinos. Mioespasmos são facilmente reconhecidos pela disfunção estrutural que produzem. Descrever o diagnóstico de um caso de mioespasmo em paciente com mordida aberta anterior, otalgia e dor na própria ATM. A paciente M.M.S.R, 33 anos, gênero feminino, cor parda, compareceu ao atendimento na Policlínica da UEA para exame estomatológico de rotina. Durante a anamnese a paciente relatou que sentia dores de cabeça, dor de ouvido, apertamento dos dentes durante a noite e possuía o elemento 46 fraturado, relatou também ter ido ao dentista fazer a restauração do elemento dental 46 há aproximadamente um ano e que, passado alguns dias do procedimento começou a ter dores de cabeça frequentemente, porém a paciente relatou ser muito estressada. Por conseguinte, a etiologia dos mioespasmos não tem sido bem documentada. É provável que muitos fatores se combinem para promover o mioespasmo. Condições musculares locais certamente aparecem para criar mioespasmos: as dores de cabeça, de ouvido, dor facial, má oclusão aguda, dentre outros. Introduction Ranulas are lesions that affect the salivary glands, and they are generally treated by excision; however, they can recur in some cases where the gland is not removed and they may progress to acute suppurative sialadenitis, yet such cases are rare, especially in children. Aims To report clinical management of occurrence of acute suppurative sialadenitis secondary to excisional biopsy of ranula in a child. Case report An 11-year-old female patient was diagnosed with ranula where excisional biopsy was performed. Subsequently, the patient developed infection, suggesting Ludwig's angina, after reevaluation, she was diagnosed with acute suppurative sialadenitis in the right mandibular gland; during the follow-up period, antibiotic therapy was introduced. Conclusion The close relationship of the biopsy site with the submandibular space and other structures of the neck required the completion of antibacterial prophylaxis associated with preoperative care, transoperative care, and postoperative care to combat opportunistic infections and their subsequent complications. Clinical Significance Suppurative sialadenitis and its subsequent complications may be considered, although rare, as a complication in cases of ranula.
