Backgrounds and aimsPain is the main reason for hysteroscopy failure. In day-surgical settings, hysteroscopy procedures are commonly performed with the patient under sedation. Hypoxia is the most common adverse event during sedation and can lead to severe adverse events. This study aimed to compare the incidence of hypoxia when using high-flow nasal oxygen (HFNO) with that when using regular nasal oxygen in patients undergoing hysteroscopy with sedation.Materials and methodsIn this single-center, prospective, randomized, single-blinded study, 960 female patients undergoing elective diagnostic or operative hysteroscopy were randomly enrolled into the following two groups: the regular nasal group [O2 (3–6 L/min) covered by an HFNO] and the HFNO group [O2 (30–60 L/min)] from September 2021 to December 2021. All women were sedated with propofol (1.5 mg/kg) and remifentanil (1.5 μg/kg) in the operating room. The primary outcome was the incidence of hypoxia (75% ≤ SpO2 < 90%, < 60 s).ResultsHFNO decreased the incidence of hypoxia (75% ≤ SpO2 < 90%, < 60 s), subclinical respiratory depression (90% ≤ SpO2 < 95%) and severe hypoxia (SpO2 < 75% for any duration or 75% ≤ SpO2 < 90% for ≥ 60 s) from 24.38 to 0.83%, from 11.25 to 1.46% and from 3.75 to 0%, respectively (P < 0.001).ConclusionIn procedures conducted to treat female infertility, HFNO can reduce hypoxia during hysteroscopy in patients sedated with propofol, and it can prevent the occurrence of subclinical respiratory depression and severe hypoxia.

However, a diverse range of factors are involved in CPSP development, which complicates efforts to predict and treat this disease. To advance research in this field, our study aimed to use bibliometric analysis to quantitatively and qualitatively evaluate CPSP research and predict research hot spots over the last 10 years.Methods: Relevant publications between 2011 and 2020 were extracted from the Web of Science Core Collection database. CiteSpace software (v5.7.R2) and the Online Analysis Platform of Literature Metrology were used to analyze research attributes including countries and authors, keywords and co-occurrence, and burst detection to predict trends and hot spots.Results: A total of 2493 publications were collected with the number of annual publications showing nearly threefold increase over the past decade. Articles were the primary publication type with the United States as the leading country and the center of national collaboration. Johns Hopkins University provided the leading influence within the CPSP field. Postoperative pain, multimodal analgesia, quality of life, opioid, microglia, cesarean delivery, inguinal hernia, chronification, genetic polymorphism, and lidocaine were the top 10 clusters in co-occurrence cluster analysis. Moreover, burst detection was shown that epidural analgesia, nerve injury, total hip arthroplasty were the new hot spots within the CPSP field.Conclusion: Bibliometric mapping not only defined the overall structure of CPSP-related research but its collective information provides crucial assistance to direct ongoing research efforts. The prominent keywords including ”risk factor” and “multimodal analgesia” indicate that CPSP prevention and new treatment methods remain hot spots. Nonetheless, the recognition that CPSP is complex and changeable, proposes comprehensive biopsychosocial approaches are needed, and these will be essential to improve CPSP interventions and outcomes.Keywords: chronic postsurgical pain, bibliometric analysis, CiteSpace, co-citation analysis, burst detection with keywords

Supplemental Digital Content is available in the text, To examine the characteristics and short-term outcome of perioperative myocardial infarction (PMI), a single-center retrospective study was carried out. The electronic medical records of 278,939 patients aged 45 years or older who underwent non-cardiac surgery at Renji Hospital from January 2003 to December 2015 were screened based on diagnostic codes (ICD121, ICD121.0, ICD121.1, ICD121.2, ICD121.3, ICD121.4, or ICD121.9). The incidence and characteristics of PMI and mortality risk factors were analyzed after non-cardiac surgery. PMI was reported in 45 patients, with an incidence rate of 1.61 per 10,000 and a mortality rate of 75.6% (34/45). The PMI incidence rate increased significantly with age. The PMI incidence rate was the highest for vascular surgery (5.82 per 10,000 cases). PMI occurs mainly within 48 h of surgery, with most cases showing an onset in the general wards. Logistic analysis showed that the use of nitrates is the independent protective factor for the outcomes of patients with PMI. The incidence of PMI in non-cardiac surgery is approximately 2 of 10,000 in patients aged 45 years or older, and increased significantly with age. The use of nitrates might be helpful for their survival.

This study tests the hypothesis that low hematocrit (Hct) worsens cerebral injury after prolonged hypothermic circulatory arrest (HCA) in rats, and the mechanism involves variable expression of the genes C-Fos, Bcl-2 and Bax. A rat HCA model was developed, and 40 animals were randomly assigned to four groups: Sham (sham) group, or Hct groups of Hct 10%, Hct 20% and Hct 30%. After 90 min of HCA at 18°C, physiologic variables were recorded and brain morphological changes were evaluated with light and electron microscopy. Expressions of C-Fos, Bcl-2, Bax in various brain areas were measured by the reverse transcriptase polymerase chain reaction and standard immunohistochemistry techniques. The number of injured neurons in the hippocampus CA1 and parietal cortex in the Hct 10% group (CA1: 11.44 ± 2.52; cortex: 13.65 ± 2.31) exceeded the mean number of injured neurons in the Hct 20% group (CA1: 8.29 ± 1.31; cortex: 10.68 ± 1.24;P < 0.05) and the Hct 30% group. Mean mitochondrial injury scores were greatest at lower Hct levels, while the expression of C-Fos and Bax were highest in the Hct 10% group and lowest in the Hct30% group (P < 0.05). In contrast, the expression of the Bcl-2 mRNA was greatest in the Hct 30% group (P < 0.05). Low Hct worsens cerebral injury after prolonged HCA and CPB in rats, which may relate in part to the variable expression of the genes C-Fos, Bcl-2 and Bax.

It has been demonstrated that [D-Ala2, D-Leu5] enkephalin (DADLE), a delta opioid agonist, protected neuron from hypoxic neuronal injury by activating the delta opioid receptor (DOR). However, whether DADLE can prevent neuronal injury induced by severe hypoxia like oxygen-glucose deprivation (OGD) is not clear. Here, we investigated whether DADLE has a protective effect against neuronal injury induced by oxygen-glucose deprivation. Neuron viability was measured by MTT and neuron injury was assessed by lactate dehydrogenase (LDH) release. Protein expression was examined by Western blot. The results showed that DADLE protected the cortical neuron in a dose-dependent way from OGD injury. And this neuroprotective effect could be completely blocked by delta 2 opioid antagonist Naltrindole. DADLE increased phosphorylation of ERK and prevented OGD-induced p38 phosphorylation. Neither DADLE nor Naltrindole had any appreciable effect on phosphorylation of JNK. One of the protective mechanisms of DADLE on OGD neurons may be due to the dynamic balance between the activation of ERK and the p38.

Blocking or impairment of the sphenopalatine ganglia (SPG) is an effective therapy of cluster headache and other pain syndromes. Contrarily, unilateral SPG-stimulation reduces infarction size in the rat permanent suture model. Well, what are the effects of the SPG damage on the following brain ischemia? This study was aimed to investigate the effects of resection of the nerves bundle from the SPG of rat on the brain lesions following middle cerebral artery occlusion (MCAO), and evaluated the roles of the nitric oxygen synthase (NOS) immunoreactive perivascular nerves of cerebral arteries in MCAO. We found that 7 days after bilateral resections of the nerves bundle from the SPG, the NOS activity perivascular nerves in the middle cerebral arteries disappeared, and the infarction volume and the TUNEL positive cells increased significantly after 24 h MCAO, which implicated that the NOS contained nerves from the SPG maybe have an important role in the MCAO.
