Peer-to-peer (P2P) energy trading is a promising market scheme to accommodate the increasing distributed energy resources (DERs). However, how P2P to be integrated into the existing power systems remains to be investigated. In this paper, we apply network charge as a means for the grid operator to attribute transmission loss and ensure network constraints for empowering P2P transaction. The interaction between the grid operator and the prosumers is modeled as a Stackelberg game, which yields a bi-level optimization problem. We prove that the Stackelberg game admits an equilibrium network charge price. Besides, we propose a method to obtain the network charge price by converting the bi-level optimization into a single-level mixed-integer quadratic programming (MIQP), which can handle a reasonable scale of prosumers efficiently. Simulations on the IEEE bus systems show that the proposed optimal network charge is favorable as it can benefit both the grid operator and the prosumers for empowering the P2P market, and achieves near-optimal social welfare. Three suspected resistant (R1, R2, and R3) corn marigold populations collected from winter cereal fields located in central Greece were studied to confirm and elucidate the mechanisms of resistance to acetolactate synthase (ALS) inhibitors and their competitive ability against barley. Whole-plant dose–response assays proved that the three suspected R populations were highly cross-resistant to the ALS inhibitors tribenuron, pyroxsulam + florasulam, and imazamox, whereas their control with synthetic auxin plus ALS inhibitors co-formulated mixtures was increased in the order of tritosulfuron + dicamba < florasulam + clopyralid < tribenuron + mecoprop-P < florasulam + aminopyralid. The ALS gene sequence revealed a point mutation in 11 plants of the R1, R2, and R3 populations, which resulted in the substitution of Pro-197-Thr or Trp-574-Leu. By contrast, all three sequenced plants of the susceptible (S) population were found with the wild-type allele encoding Pro-197 and Trp-574. This is the first report of ALS-inhibitor resistance in corn marigold. The competition study between barley and four densities of the S, R2, or R3 populations indicated similar biomass rates for all three populations, suggesting lack of association between the competitive ability of the R populations and the target-site resistance mechanism, which was also confirmed by the similar biomass reduction rates of barley grown in competition with S or R populations.

Conical neck may affect endovascular aneurysm repair (EVAR) outcomes. The aim of this study was to present EVAR neck adverse events [endoleak type Ia (ET Ia) and graft migration], in patients with conical neck morphology compared to patients with non-conical necks. An additional analysis of the factors that may affect neck adverse events in patients with conical necks, during the initial post-operative year, was executed.A retrospective analysis of prospectively data was conducted, including patients that underwent elective EVAR, between 2017 and 2019. All patients completed the clinical and imaging follow-up of the initial 12 months. Regarding imaging, all cases underwent computed tomography angiography (CTA), preoperatively, at the 1The cohort included 150 patients; 66 (44%) presented conical neck morphology. No significant difference was detected regarding the pre-operative anatomic characteristics between the conical and non-conical groups. Only distal (15mm) neck diameter was wider in the conical group (p.001). Supra-renal active fixation was used in 63.3% of the total cohort; 59.5% in patients with non-conical necks and 68.2% in patients with conical morphology (p=.275). Graft oversizing was 18.2% and 18.7% in the non-conical and conical group, respectively (p=.248). Oversizing20% was equal between groups [37.8% vs 33.3%% (p=.608) while oversizing ≥30% was more common among patients with conical necks (3.5% vs. 10.6%, p.001, 3.2 OR, 95% CI 0.79, 12.91). Regarding ET Ia and migration, no difference was recorded between the groups. In a sub-analysis among patients with conical necks, a lower graft migration rate was detected among patients with higher oversizing rate (p=.037).EVAR may offer similarly good midterm outcomes in patients with conical and non-conical neck anatomy. An oversizing to the higher suggested rate may be preventive of graft migration during the first post-operative year in necks with conical morphology. Aggressive oversizing (20%) do not offer any benefit regarding the prevention of adverse events among patients with conical necks.

Building performance simulation is an important tool in building design and operations. Its purpose is to evaluate and optimize energy use, environmental impact, and occupant comfort of buildings. However, the current state of building performance simulation tools is highly fragmented, and the models themselves can be of low quality. In this paper, we present a platform-based design paradigm for building performance models. This approach offers a standardized design flow to ensure that the models are developed in a consistent and systematic way. Additionally, our approach addresses the lack of model performance metrics, allowing for the quantification of model performance. We explore the design flow and model performance quantification with a case study, demonstrating the use of the platform-based design paradigm.

Malaria is an infectious disease transmitted by mosquitos, whose control is hampered by drug resistance evolution in the causing agent, protist parasites of the genus Plasmodium, as well as by the resistance of the mosquito to insecticides. New approaches to fight this disease are, therefore, needed. Research into targeted drug delivery is expanding as this strategy increases treatment efficacies. Alternatively, targeting the parasite in humans, here we use single-chain polymer nanoparticles (SCNPs) to target the parasite at the ookinete stage, which is one of the stages in the mosquito. This nanocarrier system provides uniquely sized and monodispersed particles of 5-20 nm, via thiol-Michael addition. The conjugation of succinic anhydride to the SCNP surface provides negative surface charges that have been shown to increase the targeting ability of SCNPs to Plasmodium berghei ookinetes. The biodistribution of SCNPs in mosquitos was studied, showing the presence of SCNPs in mosquito midguts. The presented results demonstrate the potential of anionic SCNPs for the targeting of malaria parasites in mosquitos and may lead to progress in the fight against malaria.

Proximal sealing zone has been the main interest in endovascular abdominal aortic aneurysm repair (EVAR), although the distal landing zone remodeling may also affect EVAR durability. The aim of this study was to assess iliac anatomy and its potential impact on distal landing zone adverse events after EVAR during the 12-month follow-up.A prospective data collection of patients treated with standard bifurcated EVAR devices for abdominal aortic aneurysm was undertaken between 2017 and 2019. Patients that received extension to the external iliac artery were excluded. Follow-up included computed tomography angiography (CTA) at the 1st and 12th month postoperatively. The common iliac artery (CIA) diameter was assessed in three levels: origin (just below the aortic bifurcation), distally (just above the iliac bifurcation) and the middle of the distance between these two landmarks. Iliac angle, tortuosity indexes, relining and oversizing were also analyzed. Distal landing zone-related adverse events were any limb related re-intervention, endoleak type Ib, graft migration, limb stenosis, or occlusion.In total, 268 iliac limbs (134 patients) were included. In all three levels, the mean iliac artery diameters increased at 12-month follow-up. At the origin of the CIA, the diameter increased from 18.7 ± 10.5 mm to 19.9 ± 9.4 mm (P = 0.04), at the middle portion of the CIA, the diameter changed significantly from 15.5 ± 5.1 mm to 17.4 ± 5.4 mm (P 0.001) and at the distal CIA, from 14.6 ± 3.3 mm to 15.1 ± 3.9 mm (P = 0.03). The iliac angle remained stable (P = 0.14) while the CIA index decreased significantly from 1.17 ± 0.13 to 1.11 ± 0.09 (P 0.001). The mean value of oversizing was 21.5 ± 14.5% and affected distal iliac diameter increase (P 0.001). The composite outcome of distal landing zone adverse events was not associated to diameter changes at any level. In 57 cases, a distal iliac diameter ≥18 mm was recorded. The estimated oversizing was lower (16.3 ± 11.8%) compared to18 mm arteries (22.5 ± 14.9%, P = 0.01). At 12-month follow-up, iliac diameters remained stable in the ≥18 mm group. Endoleak type Ib was more common in iliac arteries ≥18 mm [3 (5.3%) vs. 1 (0.5%) (P = 0.04)] at 12-months.Post-EVAR iliac artery dilation does not seem to have an impact on distal landing zone adverse events during the 12-month follow-up. Aggressive oversizing may be related to iliac dilation. EVAR patients with iliac arteries ≥18 mm are at higher risk for ET Ib.

Al–Pt compounds have been systematically studied as electrocatalysts for the oxygen evolution reaction (OER). Considering the harsh oxidative conditions of the OER, all Al–Pt compounds undergo modifications during electrochemical experiments. However, the degree of changes strongly depends on the composition and crystal structure of a compound. In contrast to Al-rich compounds (Al4Pt and Al21Pt8), which reveal strong leaching of aluminum, changes in other compounds (Al2Pt, Al3Pt2, rt-AlPt, Al3Pt5, and rt-AlPt3) take place only on the surface or in the near-surface region. Furthermore, surface modification leads to a change in the electronic structure of Pt, giving rise to the in situ formation of catalytically more active surfaces, which are composed of intermetallic compounds, Pt-rich AlxPt1−x phases and Pt oxides. Forming a compromise between sufficient OER activity and stability, Al2Pt and Al3Pt2 can be considered as precursors for OER electrocatalysts.

This paper investigates how the credit to private sector affects the impact of household debt on economic growth in 25 European Union countries over the period 1995-2018. The findings reveal that the positive effect of household debt on economic growth turns to negative with the onset of the 2008 global financial crisis (GFC) and beyond a certain point at around 58% of GDP, thus suggesting that their relationship is non-linear. Interestingly, the adverse effect subjects to the increased pressure of the credit to private sector when it is above 70%, and the pressure becomes even higher when the ratio is above 90%.

Type A intramural hematoma (TAIMH) is an acute aortic disease characterized by the presence of hematoma in the aortic media and involving the ascending aorta. Open repair seems to be the first treatment approach, although recent evidence highlights that the best management of TAIMH is controversial. This review will focus on the current concept for TAIMH management and factors affecting the decision making.Recent studies have evaluated the role of open and endovascular repair, as well as conservative management in patients with TAIMH. More specific imaging findings seem to affect decision making for urgent repair.Despite TAIMH's acute nature, conservative management seems to represent a valid option for urgent approach, presenting similar mortality to open and endovascular repair. Comparative data are limited, however, in experienced centers, any approach may be applied with encouraging results. Endovascular management, which is mainly applied to manage retrograde TAIMH, is related to lower mortality and morbidity compared to open repair in this group of patients while aortic remodeling seems beneficial with this approach. Imaging findings, as ulcer-like lesions, hematoma thickness, concomitant dissection and aortic diameter, related to higher complication rate, set the indication for interventional management. Further research, including prospective data and registries, and ideally, randomized data may further clarify the best approach and factors indicating urgent repair.

This paper presents the problem of soil pollution in the vicinity of a former chemical plant, given that production activity was stopped for more than thirty years ago. The old chemical plant was located in the southern part of Braila town, SE Romania, just a few kilometers from the left bank of the Danube River. Prior to 1990, there was a significant production of industrial chemical goods, such as sulfuric acid, carbon dioxide for synthetic fibers, chlorine gas and hydrochloric acid obtained by electrolysis processes with mercury electrodes and heavy metal catalysts (Cd, Ni, Zn), sodium sulfide and sulphate, etc. For the good development of the experimental program, a number of 10 soil samples were collected from the north, northwest, west, southwest and south of the former chemical plant. The multi-element analysis method used to determine the trace elements in the soil samples was energy-dispersive X-ray fluorescence spectrometry (EDXRF), using a Genius XRF portable spectrometer manufactured by Skyray Instruments Inc., equipped with a large surface Si with a Be window and a 40 kV/100 µA miniature X-ray tube excitation source with Ag target. The spectrometric system has an energy resolution up to 139 eV, and the detection limits of the order of ppm. A total of 21 chemical elements were identified in the target soil samples, such as: K, Ca, Ti, V, Cr, Mn, Fe, Ni, Cu, Zn, As, Hg, Rb, Sr, Y, Zr, Nb, Mo, Sn, Sb, Pb. The compositional analyses of contaminated industrial soils were completed by using the AAS and ICP-MS high performance and selectivity techniques.

We therefore investigate the relation between natural and artificial light, based on geographical position as well as social/cultural habits of different countries. The aim is that this approach will inform the user on how different lighting scenarios can improve the use of the space. The investigations are based on end user’s preferences and illustrate how awareness of different lighting cultures can be used in different lighting design scenarios. The findings from this specific project are meant to support the development of scenarios for future lighting fixtures and control systems based on a deeper understanding of cultural and geographical parameters.

Connective tissue disease (CTD) syndromes involve the ascending, aortic arch, and thoracoabdominal aorta and are associated with higher risk of aortic aneurysm or dissection. Currently, vascular societies generally recommend open repair as the first option for aortic disease in patients with CTD. However, the implementation of endovascular techniques for patients with CTD with aortic pathologies seems to have increased in recent years, mainly in patients of high surgical risk or in urgent situations. Endovascular treatment of aortic arch pathologies in patients with CTD have been feasible in experienced centers; however, the evidence is scarce. Thoracic endovascular aneurysm repair in patients with CTD is more evident; in 15 studies, 304 patients with CTD were treated with thoracic endovascular aneurysm repair with high technical success rates (88% to 100%) and a low early mortality rate (1.6%). During the median follow-up, 33 patients died and 64 patients underwent a re-intervention. In 6 studies, 26 patients with CTD were treated with fenestrated/branched endovascular aneurysm repair for thoracoabdominal aortic aneurysm, with a technical success rate of 100%, without early mortality and morbidity. The endovascular approach to thoracoabdominal aortic aneurysm, especially in post-dissection patients, mandates adjunctive techniques to achieve false lumen thrombosis with various approaches; in our experience, the Candy-Plug technique has been proven to be technically feasible with good outcomes. Endovascular treatment of aortic pathologies in patients with CTD seems to be feasible and safe in high-risk and urgent patients. Re-intervention remains an issue. The constant development of endovascular techniques and devices may provide improved mortality and morbidity outcomes.

The main purpose of the study was the absorption of heavy metals in the leaves of forest tree species, which were planted in two different plots for forestry use and environmental restoration. Four species were studied Pinus brutia, Robinia pseudoacaccia, Quercus trojana and Fraxinus ornus. Forty-eight leaf samples were collected which consisted of six samples from each species at each plot. The heavy metal concentrations in the leaves were measured for the following nine heavy metals: iron (Fe), copper (Cu), chromium (Cr), nickel (Ni), cadmium (Cd), manganese (Mn), zinc (Zn), cobalt (Co) and lead (Pd). The determinative estimation of metal concentration was carried out in the clear filtrate, using ICP-OES. Statistically significant differences in the concentrations of the heave metals were found among the species, as well as between the two plots. It was only in Robinia peudoacacia’s leaves that the cadmium concentration showed a statistical difference among the other species. The same applied for manganese in Quercus trojana’s leaves and zinc for Pinus brutia. The careful selection and planting of the appropriate forest tree species provides for an overall improvement in the environment in heavy metal polluted sites, such as those resulting from thermal power plants.

Context: Both isokinetic testing and functional tests are often used during patient rehabilitation programs. Most researchers have assessed the relationship of isokinetic testing and functional tests in parameters such as peak moment, work, or power. However, little information exists regarding the acceleration and deceleration parameters and their relationship to function. Design: The purpose of the study was to investigate the concept of acceleration and deceleration of the knee flexor and extensor muscle groups during isokinetic testing and its relationship to function. Methods: Twenty-six healthy individuals (14 men and 12 women) participated in the study. Isokinetic bilateral concentric knee flexion and extension muscular performance was assessed isokinetically at the angular velocities of 60°/s, 180°/s, and 300°/s. Functional performance was also assessed using (1) single hop for distance, (2) triple hop for distance, and (3) single timed hop. Functional data were normalized to body weight. A P value Results: Fair and moderate to good correlations (r = .396, P = .045 to r = .705, P = .001) were discovered between flexors’ and extensors’ acceleration/deceleration variables and the normalized to body weight functional tests. Conclusions: Appropriate understanding of isokinetic parameters as described in this study, including acceleration/deceleration values, may provide useful information about the functional ability of the lower limb muscular system.

VTE is a multifactorial condition that results from the interaction of genetic, acquired, and environmental factors. Physiological changes during pregnancy increase the risk of VTE as they express Virchow’s triad (increased coagulation factors, decreased fibrinolysis, trauma, and venous stasis). Moreover, pregnancy-related risk factors, such as advanced maternal age, obesity, multiple gestations, and cesarean delivery, further increase the risk of VTE. Managing VTE in pregnancy is challenging due to the complexity of balancing the risks and benefits of anticoagulant therapy for both the mother and the fetus. A multidisciplinary approach involving obstetricians, hematologists, and neonatologists, is necessary to ensure optimal outcomes for both the mother and baby. The development of an electronic voting system that would replace traditional election procedures is a research topic of great interest for many years. Blockchain technology could provide some guarantees and fulfill strong requirements for electronic voting platforms, such as transparency, immutability, and confidentiality. From time to time research is conducted to address problems in voting systems. Many research works attempt to implement secure and reliable voting systems, which address known security, anonymity, and fraud issues that might threaten such systems. This paper presents a proposal of a secure electronic voting system, the EtherVote, using the Ethereum Blockchain network that focuses deeply on the field of identification of eligible citizens. The proposed system will be entirely based on Blockchain without any central authority servers or databases, thus improving security, privacy, and election cost. Nasal irrigation is thought to decrease the viral load present in the nasal cavity. Our aim was to assess the effect of a hypertonic seawater solution [with algal and herbal natural ingredients (Sinomarin®)] on the viral load of nasopharynx in patients hospitalized with severe COVID-19 pneumonia. We conducted a prospective, randomized, controlled trial from June 2022 to December 2022. We allocated 56 patients with COVID-19 pneumonia into two groups (28 in each group)—the hypertonic seawater group [nasal irrigations with a hypertonic seawater solution (Sinomarin®) every 4 h for 16 h per day, for two consecutive days] and the control group (no nasal irrigations). A second nasopharyngeal swab was collected 48 h after the baseline nasopharyngeal swab (8 h after the last wash in the hypertonic seawater group) to estimate the SARS-CoV-2 viral load as determined by cycle threshold (Ct) values. In the hypertonic seawater group, the mean Ct values significantly increased two days after the initial measurement [ΔCt 48−0 h = 3.86 ± 3.03 cycles, p < 0.001 (95%CI: 2.69 to 5.04)]. No significant differences in the Ct values were observed in the control group [ΔCt 48−0 h = −0.14 ± 4.29, p = 0.866 (95%CI: −1.80 to −1.52)]. At follow-up, 17 patients from the hypertonic seawater group had negative test results compared to only 9 patients from the control group (p = 0.03). Nasal irrigations with a hypertonic seawater solution containing algal and herbal natural ingredients significantly decreased nasopharyngeal viral load and the detection time of SARS-CoV-2 in the nasal cavity.

SummaryDetection and quantitation of the RNA-interacting proteome is commonly achieved by applying SILAC labeling, following by data-dependent acquisition (DDA) of proteomic data. However, the limited sensitivity of the DDA approach often restricts detection to relatively abundant proteins, necessitating peptide fractionation prior to mass spectrometry. Here we report a pipeline for SILAC analysis using data-independent acquisition (DIA). The spectral library is constructed using gas-phase window separation of light material, followed byin silicoprediction of heavy spectra. The resulting DIA datasets gave 30-40% greater protein detection relative to the same biological materials analyzed using DDA, while abolishing the requirement for fractionation of peptides following pre-MS reverse-phase separation. For proteins detected in both acquisitions, lower inter-replicate variation was seen with DIA. As a test, we determined the effects of arsenite treatment on the RNA-bound proteome of HEK293T cells, recovered following total RNA-associated proteome purification (TRAPP). The DIA dataset yielded clear GO term enrichment for RNA-binding proteins involved in cellular stress responses, while enrichment in the DDA dataset was relatively weak. Dataset normalization of with Cyclic Loess slightly improved the DDA-DIA correlation over median normalization for DIA datasets, but not for DDA dataset relative to default MaxQuant normalization. Overall, the DIA SILAC approach improved both protein detection and biological significance over conventional DDA SILAC.

Optical activity (polarization rotation of light) is one of the most desired features of chiral media, as it is important for many polarization related applications. However, in the THz region, chiral media with strong optical activity are not available in nature. Here, we study theoretically, and experimentally a chiral metamaterial structure composed of pairs of vertical U-shape resonators of "twisted" arms, and we reveal that it demonstrates large pure optical activity (i.e. optical activity associated with negligible transmitted wave ellipticity) in the low THz regime. The experimental data show polarization rotation up to 25 (deg) for an unmatched bandwidth of 1 THz (relative bandwidth 80 %), from a 130 um-thickness structure, while theoretical optimizations show that the rotation can reach 45 (deg). The enhanced chiral response of the structure is analyzed through an equivalent RLC circuit model, which provides also simple optimization rules for the enhancement of its chiral response. The proposed chiral structures allow easy fabrication via direct laser writing and electroless metal plating, making them suitable candidates for polarization control applications., Comment: 17 pages, 7 figures

Sarcoidosis is an inflammatory granulomatous disease of unknown etiology involving any organ or tissue along with any combination of active sites, even the most silent ones clinically. The unpredictable nature of the sites involved in sarcoidosis dictates the highly variable natural history of the disease and the necessity to cluster cases at diagnosis based on clinical and/or imaging common characteristics in an attempt to classify patients based on their more homogeneous phenotypes, possibly with similar clinical behavior, prognosis, outcome, and therefore with therapeutic requirements. In the course of the disease's history, this attempt relates to the availability of a means of detection of the sites involved, from the Karl Wurm and Guy Scadding's chest x-ray staging through the ACCESS, the WASOG Sarcoidosis Organ Assessment Instruments, and the GenPhenReSa study to the 18F-FDG PET/CT scan phenotyping and far beyond to new technologies and/or the current “omics.” The hybrid molecular imaging of the 18F-FDG PET/CT scan, by unveiling the glucose metabolism of inflammatory cells, can identify high sensitivity inflammatory active granulomas, the hallmark of sarcoidosis—even in clinically and physiologically silent sites—and, as recently shown, is successful in identifying an unexpected ordered stratification into four phenotypes: (I) hilar–mediastinal nodal, (II) lungs and hilar–mediastinal nodal, (III) an extended nodal supraclavicular, thoracic, abdominal, inguinal, and (IV) all the above in addition to systemic organs and tissues, which is therefore the ideal phenotyping instrument. During the “omics era,” studies could provide significant, distinct, and exclusive insights into sarcoidosis phenotypes linking clinical, laboratory, imaging, and histologic characteristics with molecular signatures. In this context, the personalization of treatment for sarcoidosis patients might have reached its goal.

The molecular and functional contributions of intratumoral nerves to disease remain largely unknown. We localized synaptic markers within tumors suggesting that these nerves form functional connections. Consistent with this, electrophysiological analysis shows that malignancies harbor significantly higher electrical activity than benign disease or normal tissues. We also demonstrate pharmacologic silencing of tumoral electrical activity. Tumors implanted in transgenic animals lacking nociceptor neurons show reduced electrical activity. These data suggest that intratumoral nerves remain functional at the tumor bed. Immunohistochemical staining demonstrates the presence of the neuropeptide, Substance P (SP), within the tumor space. We show that tumor cells express the SP receptor, NK1R, and that ligand/receptor engagement promotes cellular proliferation and migration. Our findings identify a mechanism whereby intratumoral nerves promote cancer progression.

Despite advances in prevention, detection, and treatment, cardiovascular disease is a leading cause of mortality and represents a major health problem worldwide. Artificial intelligence and machine learning have brought new insights to the management of vascular diseases by allowing analysis of huge and complex datasets and by offering new techniques to develop advanced imaging analysis. Artificial intelligence–based applications have the potential to improve prognostic evaluation and evidence-based decision making and contribute to vascular therapeutic decision making. In this scoping review, we provide an overview on how artificial intelligence could help in vascular surgical clinical decision making, highlighting potential benefits, current limitations, and future challenges.

Sarcopenia is the loss of muscle mass and strength. It manifests as decreased core muscle area in axial abdominal computed tomography scans. The predictive value of sarcopenia in outcome research has been widely discussed. A systematic review was conducted to assess sarcopenia as a biomarker in patients undergoing endovascular abdominal aortic aneurysm repair (EVAR) and its association with long-term survival.A systematic search of the English medical literature, published from 1991 to 2021, was conducted, using PubMed, EMBASE, and CENTRAL, as per the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA, 2020) guidelines. The study protocol was registered to the International Prospective Register of Systematic Reviews (CRD 42021260192). Observational studies reporting on sarcopenic and nonsarcopenic patients undergoing EVAR were included. The Risk Of Bias In Non-Randomized Studies - of Interventions (ROBINS-I) risk of bias evaluation tool was used for the quality assessment. The outcomes were summarized as odds ratio (OR) along with their 95% confidence intervals (CI), through a paired meta-analysis. The primary outcome was 5-year survival following EVAR.Eleven observational studies, including 2,385 patients (89.6% males) treated with EVAR, between 1999 and 2018, were included in the qualitative synthesis. The mean age was 72.9 years (range 70-76.4 years). Nine of the studies reported on a negative prognostic value of sarcopenia on survival during a long-term follow-up. Six studies stratified their cohorts in sarcopenic versus nonsarcopenic patients (40.3% sarcopenic). The cutoff values (mmThe available data derived from observational studies suggest an association between sarcopenia and worse long-term survival in patients treated with EVAR. There is a lack of a universally accepted definition for sarcopenia and future reporting standards should address this issue. Prospective studies are necessary to establish this negative prognostic value of sarcopenia on long-term survival.

Fenestrated endovascular aneurysm repair (F-EVAR) has allowed successful treatment of patients with complex aortic aneurysms. Custom-made devices (CMDs) are manufactured by companies and tailored to the patient's anatomy to incorporate target vessels, while there is also the "off label" alternative with the devices modified by a surgeon in the operating room: surgeon-modified FEVAR (sm-FEVAR). This study aims to present and compare technical durability of CMDs- and sm-FEVAR for complex abdominal and thoracoabdominal aortic pathologies.A retrospective cohort study was undertaken including all consecutive patients treated with sm- or CMD-FEVAR during a 3-year period in a single center. Only cases with at least 3 reno-visceral target vessels were included. Primary outcomes were technical success, and freedom from endoleak (EL) (Ia or III; all branch related) and re-intervention during follow-up period. Mortality and morbidity were also recorded.Thirty-two sm-FEVAR patients (81,3% male) and 79 CMD-FEVAR patients (77,2% male) were included. Indication for sm-FEVAR was exclusively urgent, while all CMD- FEVAR were elective. Technical success was similar in sm-FEVAR (100%) and CMD-FEVAR (98,7%) (P = 0.523). Mean follow-up was 16.3 ± 13 and 20 ± 17.3 months for sm-FEVAR and CMD- FEVAR, respectively (P = 0.28). The freedom from EL Ia rate was 91.7% [standard error (SE) 5.7%] at 12 months in sm-FEVAR, while it was 97.7% (SE 2.2%) and 92.3% (SE 4.3%) at 12 and 24 months, respectively in CMD-FEVAR (P = 0.69). The freedom from EL III rate was 95.5% (SE 4.4%) and 88% (SE 8.2%) at 12 and 24 months, respectively in sm-FEVAR, while it was 92.1% (SE 3.8%) and 89.2% (SE 4.7%) at 12 and 24 months, respectively in CMD-FEVAR (P = 0.68). The freedom from re-intervention rate was 91.6% (SE 5.7%) and 84.6% (SE 8.6%) at 12 and 24 months, respectively in sm-FEVAR, while it was 91.7% (SE 4%) and 83.4% (SE 5.9%) at 12 and 24 months, respectively in CMD-FEVAR (P = 0.90). The survival rate was 87.5% (5.8%) and 76.3% (7.9%) at 1 and 12 months, respectively in sm-FEVAR, while it was 93.7% (2.7%) at 1 month in CMD-FEVAR without any other death during FU period. No bridging stent occlusions were noted during follow-up period in any patient.Sm-FEVAR offers good technical success and mid-term clinical outcomes in urgent cases of complex aortic pathologies. Its durability is acceptable and comparable to CMD-FEVAR with a relatively low re-intervention rate.

Spinel-type catalysts are promising anode materials for the alkaline oxygen evolution reaction (OER), exhibiting low overpotentials and providing long-term stability. In this study, we compared two structurally equal Co2FeO4 spinels with nominally identical stoichiometry and substantially different OER activities. In particular, one of the samples, characterized by a metastable precatalyst state, was found to quickly achieve its steady-state optimum operation, while the other, which was initially closer to the ideal crystallographic spinel structure, never reached such a state and required 168 mV higher potential to achieve 1 mA/cm2. In addition, the enhanced OER activity was accompanied by a larger resistance to corrosion. More specifically, using various ex situ, quasi in situ, and operando methods, we could identify a correlation between the catalytic activity and compositional inhomogeneities resulting in an X-ray amorphous Co2+-rich minority phase linking the crystalline spinel domains in the as-prepared state. Operando X-ray absorption spectroscopy revealed that these Co-rich domains transform during OER to structurally different Co3+-rich domains. These domains appear to be crucial for enhancing OER kinetics while exhibiting distinctly different redox properties. Our work emphasizes the necessity of the operando methodology to gain fundamental insight into the activity-determining properties of OER catalysts and presents a promising catalyst concept in which a stable, crystalline structure hosts the disordered and active catalyst phase.

$\textbf{Background:}$ At the onset of a pandemic, such as COVID-19, data with proper labeling/attributes corresponding to the new disease might be unavailable or sparse. Machine Learning (ML) models trained with the available data, which is limited in quantity and poor in diversity, will often be biased and inaccurate. At the same time, ML algorithms designed to fight pandemics must have good performance and be developed in a time-sensitive manner. To tackle the challenges of limited data, and label scarcity in the available data, we propose generating conditional synthetic data, to be used alongside real data for developing robust ML models. $\textbf{Methods:}$ We present a hybrid model consisting of a conditional generative flow and a classifier for conditional synthetic data generation. The classifier decouples the feature representation for the condition, which is fed to the flow to extract the local noise. We generate synthetic data by manipulating the local noise with fixed conditional feature representation. We also propose a semi-supervised approach to generate synthetic samples in the absence of labels for a majority of the available data. $\textbf{Results:}$ We performed conditional synthetic generation for chest computed tomography (CT) scans corresponding to normal, COVID-19, and pneumonia afflicted patients. We show that our method significantly outperforms existing models both on qualitative and quantitative performance, and our semi-supervised approach can efficiently synthesize conditional samples under label scarcity. As an example of downstream use of synthetic data, we show improvement in COVID-19 detection from CT scans with conditional synthetic data augmentation.

Background Recurrent laryngeal nerve identification is the ‘gold standard’ in thyroidectomy, to determine nerve function security and prevent severe complications. This study assessed the topographical relationship between the recurrent laryngeal nerve and the inferior thyroid artery in patients undergoing total thyroidectomy, and determined its clinical impact. Methods A retrospective study was performed of patients undergoing total thyroidectomy in a single tertiary centre over a six-month period. Results Sixty-four patients were included. Among the 128 recurrent laryngeal nerve dissections, the nerve was identified traversing the inferior thyroid artery anteriorly in 27.3 per cent, with equal distribution between the two sides. No significant sex association was reported. One patient had transient vocal fold palsy, and hypocalcaemia was observed in 21.9 per cent, yet there was no statistical association with the topographical variation of the recurrent laryngeal nerve. Conclusion Almost one-third of patients had an anatomical variation in which the recurrent laryngeal nerve ran superiorly to the inferior thyroid artery. Recurrent laryngeal nerve variation had no clinical impact on local complications or hypocalcaemia.

Hypoxemia is a defining feature of acute respiratory distress syndrome (ARDS), an often-fatal complication of pulmonary or systemic inflammation, yet the resulting tissue hypoxia, and its impact on immune responses, is often neglected. In the present study, we have shown that ARDS patients were hypoxemic and monocytopenic within the first 48 h of ventilation. Monocytopenia was also observed in mouse models of hypoxic acute lung injury, in which hypoxemia drove the suppression of type I interferon signaling in the bone marrow. This impaired monopoiesis resulted in reduced accumulation of monocyte-derived macrophages and enhanced neutrophil-mediated inflammation in the lung. Administration of colony-stimulating factor 1 in mice with hypoxic lung injury rescued the monocytopenia, altered the phenotype of circulating monocytes, increased monocyte-derived macrophages in the lung and limited injury. Thus, tissue hypoxia altered the dynamics of the immune response to the detriment of the host and interventions to address the aberrant response offer new therapeutic strategies for ARDS.

Background Asthma epidemics associated with thunderstorms have had catastrophic impacts on individuals and emergency services. Seasonal allergic rhinitis (SAR) is present in the vast majority of people who develop thunderstorm asthma (TA), but there is little evidence regarding risk factors for TA among the SAR population. Objective We sought to identify risk factors for a history of TA and hospital presentation in a cohort of individuals with SAR. Methods This multi-centre study recruited adults from Melbourne (Australia) with a past diagnosis of TA and/or self-reported SAR. Clinical information, spirometry, white blood-cell count, ryegrass pollen specific IgE (RGP-spIgE) and fractional exhaled nitric oxide (FeNO) were measured to identify risk factors for a history of TA in individuals with SAR. Results From a total of 228 individuals with SAR, 35% (80/228) reported SAR only (I-SAR), 37% (84/228) reported TA symptoms but had not attended hospital for treatment (O-TA) and 28% (64/228) had presented to hospital for TA (H-TA). All H-TA patients reported a previous asthma diagnosis. Logistic regression analysis of factors associated with O-TA and H-TA indicated that lower forced expiratory volume in one second (FEV1) and asthma control questionnaire (ACQ) score >1.5 were associated with H-TA. Higher blood RGP-spIgE, eosinophil counts, and FeNO were significantly associated with both O-TA and H-TA. Receiver-operating curve (ROC) analysis showed RGP-spIgE >10·1 kU/L and pre-bronchodilator FEV1 ≤90% to be biomarkers of increased H-TA risk. Conclusion Clinical tests can identify risk for a history of TA in individuals with SAR and thereby inform patient-specific treatment recommendations.

BackgroundThe proliferation antigen Ki-67 has been widely used in clinical settings for cancer staging for many years but investigations on its biological functions have lagged. Recently, Ki-67 was shown to regulate both the composition of the chromosome periphery and chromosome behaviour in mitosis as well as to play a role in heterochromatin organisation and gene transcription. However, a role for Ki-67 in regulating cell cycle progression has never been reported. The progress towards understanding Ki-67 function have been limited by the tools available to deplete the protein coupled to its abundance and fluctuation during the cell cycle.ResultsHere we have used an auxin-inducible degron tag (AID) to achieve a rapid and homogeneous degradation of Ki-67 in HCT116 cells. This system, coupled with APEX2 proteomics and phospho-proteomics approaches, allowed us to show for the first time that Ki-67 plays a role during DNA replication. In its absence, DNA replication is severely delayed, the replication machinery is unloaded, causing DNA damage that is not sensed by the canonical pathways and dependant on HUWE1 ligase. This leads to replication and sister chromatids cohesion defects, but it also triggers an interferon response mediated by the cGAS/STING pathway in all the cell lines tested.ConclusionsWe have unveiled a new function of Ki-67 in DNA replication and genome maintenance that is independent of its previously known role in mitosis and gene regulation.

BACKGROUND: There is increasing evidence, in transgenic mice and in vitro, that inhibitory killer cell immunoglobulin-like receptors (iKIRs) can modulate T cell responses. Furthermore, we have previously shown that iKIRs are an important determinant of T cell-mediated control of chronic virus infection and that these results are consistent with an increase in CD8+ T cell lifespan due to iKIR-ligand interactions. Here we test this prediction and investigate whether iKIRs affect T cell lifespan in humans in vivo. METHODS: We used stable isotope labelling with deuterated water to quantify memory CD8+ T cell survival in healthy individuals and patients with chronic viral infections. RESULTS: We showed that an individual's iKIR-ligand genotype is a significant determinant of CD8+ T cell lifespan: in individuals with two iKIR-ligand gene pairs, memory CD8+ T cells survived on average for 125 days, in individuals with four iKIR-ligand gene pairs then memory CD8+ T cell lifespan was doubled to 250 days. Additionally, we showed that this survival advantage is independent of iKIR expression by the T cell of interest and further that iKIR-ligand genotype altered CD8+ and CD4+ T cell immune aging phenotype. CONCLUSIONS: Together these data reveal an unexpectedly large impact of iKIR genotype on T cell survival. The freeze-in mechanism has been shown to allow the simultaneous generation of cosmic dark matter and a viable matter-antimatter asymmetry in the universe. When the underlying interactions are described by higher-dimensional, non-renormalizable operators, the relevant freeze-in processes take place close to the highest considered cosmic temperatures. In this paper we study how the presence of a fluid that temporarily dominates the energy content of the early universe affects the predictions of this ``Ultraviolet Freeze-In Baryogenesis'' scenario. We find that this additional cosmic component has a significant impact on the predictions of concrete microscopic models, allowing for reheating temperatures which are much lower than those required in the simplest cosmological scenario. Moreover, we show that inflationary observables can constrain the parameter space of such models, once the latter are examined in conjunction with concrete models of inflation., 25 pages, 6 figures
