Data regarding changes in HbA1c, fasting plasma glucose (FPG), and postprandial glucose (PPG) levels from observation periods of 3 to 6 months were assessed.Results: Out of 358 patients that received BOT/glulisine, 182 had a high BeAM value. Patients with a high BeAM value were older and had a longer diabetes duration than patients with a medium BeAM value. Significant reductions in HbA1c (7.5% to 7.2% [59 to 55 mmol/mol], p< 0.0001) and PPG (202 to 143 mg/dl, p< 0.0001) levels were documented. The proportion of patients with a high BeAM value achieving an HbA1c < 7% [53 mmol/mol], alone or in combination with no hypoglycaemia, was lower than that of patients with a medium BeAM value.Conclusion: The analysis indicates that the supplementation of BOT with a single daily injection of prandial insulin is safe and effective for reducing HbA1c and PPG levels in patients with a high BeAM value (more than 50 mg/dl). 2. Two clinical cases of adjunctive use of a SGLT-2 inhibitor in type 1 diabetes

Abstract: Type 1 diabetes mellitus (T1DM) prevalence is increasing and despite all available modern treatment options, an overall small but noticeable increase of mean HbA1c was recently observed in various registries. Authorized adjunctive pharmacological treatment options to insulin therapy are still scarce for T1DM. In February 2019, the European Medicines Agency (EMA) approved dapagliflozin as first in class sodium/glucose co-transporter 2 inhibitor (SGLT-2i) adjunctive therapy to insulin in patients with T1DM, which is currently still not approved by the FDA in the United States. SGLT-2is have shown significant improvement in HbA1c, reducing body weight and increasing time-in-range by reducing glycaemic variability, as well as reductions in total daily insulin dose in the trials in persons with T1DM. The cases presented here translate some of the observations gained from clinical trials into a real-world environment. They demonstrate that even highly practised and educated patients can benefit from the addition of a SGLT-2i as adjunctive treatment to insulin in T1DM. In summary, these cases demonstrate that a variety of patients with T1DM in a real-world setting may benefit from SGLT-2i treatment, as they have the potential to improve HbA1c, excess of body weight and increasing TiR among other things. 3. Clinical benefits of switching to insulin degludec irrespective of previous basal insulin therapy in people with Type 1 or Type 2 diabetes: evidence from a European, multicentre, retrospective, non-interventional study (EU-TREAT)

Abstract: Aims: To investigate whether the benefits of switching to insulin degludec observed in the European retrospective chart review study EU-TREAT were dependent on the previous basal insulin used. Methods: People with Type 1 or Type 2 diabetes were switched to insulin degludec from other basal insulins ≥6 months before data collection. Participants were stratified into three groups based on their previous basal insulin: insulin glargine 100 units/ml (Type 1: n=888; Type 2: n=259); insulin detemir (Type 1: n=726; Type 2: n=415); and neutral protamine Hagedorn (Type 1: n=53; Type 2: n=95). Their glycaemic control and hypoglycaemia incidence at 6 and 12 months post-switch vs pre-switch was then evaluated. Results: Significant HbA1c reductions were achieved in all previous basal insulin groups for participants with Type 1 diabetes [insulin glargine 100 units/ml: −2.08 mmol/mol (−0.19%); insulin detemir: −2.40 mmol/mol (−0.22%)] and those with Type 2 diabetes [insulin glargine 100 units/ml: −5.90 mmol/mol (–0.54%); insulin detemir: −6.01 mmol/mol (−0.55%); neutral protamine Hagedorn: −2.73 mmol/mol (−0.25%)] at 6 months, except for the relatively small neutral protamine Hagedorn group in those with Type 1 diabetes [−1.75 mmol/mol (−0.16%)], where statistical significance was not reached. At 6 months in the Type 1 diabetes group, switching to insulin degludec from insulin glargine 100 units/ml resulted in significantly lower hypoglycaemia rates across all hypoglycaemia categories; for the insulin detemir group, this significance was also observed for severe and nocturnal non-severe hypoglycaemia, while the low number of people in the neutral protamine Hagedorn group resulted in nonsignificant reductions in hypoglycaemia rates. At 6 months in the people with Type 2 diabetes, switching to insulin degludec resulted in significantly lower rates of hypoglycaemia across all categories for all groups. Similar outcomes were observed at 12 months. Conclusions: Switching to insulin degludec from other basal insulins can improve glycaemic control and/or reduce hypoglycaemia risk in people with diabetes (although there was a nonsignificant reduction in HbA1c and hypoglycaemia rates for the neutral protamine Hagedorn group in Type 1 diabetes) under routine care. © 2019 The Authors. In T2DM patients on either basal insulin-supported oral therapy (BOT) or supplementary insulin therapy (SIT) presenting with HbA1c values above individual targets for 3–6 months, therapy should be intensified. This study investigated effectiveness and tolerability of an intensification of BOT or SIT to a basal–bolus therapy (BBT) regimen in T2DM patients in daily clinical practice. Methods: This noninterventional, 8-month, prospective, multicenter study evaluated parameters of glucose control, occurrence of adverse events (eg, hypoglycemia), and acceptance of devices in daily clinical practice routine after 12 and 24 weeks of intensifying insulin therapy to a BBT regimen starting from either preexisting BOT with insulin glargine (pre-BOT) or preexisting SIT with ≥3 daily injections of insulin glulisine (pre-SIT). Results: A total of 1,530 patients were documented in 258 German medical practices. A total of 1,301 patients were included in the full analysis set (55% male, 45% female; age median 64 years; body mass index median 30.8 kg/m2; pre-BOT: n=1,072; pre-SIT: n=229), and 1,515 patients were evaluated for safety. 9. The voice of struggle : an interpretive study of factors influencing success or failure of microenterprises in Brazil's urban favelas

Abstract: I Third World countries and in particular in Latin America the high birth rate and the forced rural exodus of farm hands has caused a extremely high unemployment rate among the migrants who have flocked into the peripheries of the big cities. Without jobs many of these squatters remain in a desperate waiting position of sheer survival, but those that are more enterprising they to make their living by starting and developing a business of their own. Although this form of activity dates back to the very beginning of the worlds economic history, it has dramatically increased if the countries of the Southern hemisphere during the last three or four decades. Policy makers and scholars have been stirred up by the phenomenon. While ' new terms, like °microentrepreneur° and microenterpn`se° have been coined, a better insight into the labours of the struggling community of microentrepreneurs is still being sought after. This study, in its first chapter, tries to set the stage to uncover the factors that lead to success or failure of microbusinesses as seen from the unique point of view of the microentrepreneurs while doing business in the environment of the Brazilianfavela. The second chapter introduces the background literature on the relevant issues of the thesis. It presents a short historical overview and sheds light on the current social and economic situation of Brazil, on the general structure of microenterpises, on the importance of active entrepreneurship and on the personal traits of a entrepreneur. A vast set of information on the subject of help to microenterpises has been published by many support organizations, but as the predominant concern of the emerging enterprises seemed to be the lack of capital, most publications are restricted on how to collect and administer ftmds. These publications express the view of the donors, but almost no literature exists on the views of the receivers. It is the purpose of this study to l this apparent gap. The present research is geared to bringing to light views and perceptions at the grass root level. It intends to penetrate into the microenterprise culture of the favelas of Brazils megalopolises. A array of possible methodological approaches for achieving this aspired goal is presented in the third chapter and the final decision to use grounded theory, as a theory generating approach, is explained. The next chapter presents the scope of the field studies. Out of the almost homogeneous body of rural-urban migrants that constitute the majority of Brazils favela tenants, forty two microentrepreneurs were selected to form the target group of the thesis. I order to achieve a geographic spread, the megalopolises of Porto Alegre, Säo Paulo, and Salvador da Bahia were selected. The h chapter presents as its core section the extensive narratives of twelve micro-entrepreneurs, who represent a continuum of outcomes from cases of encouraging success to cases of complete failure. I the next chapter, chapter six, the data is screened using a extensive and methodical cross case analysis. From the statements of the interviewees nine distinct propositions emerge as the core findings of the study. These propositions match with the factors that influence the success or failure of the microenterprises as seen by the microentrepreneurs themselves: Drive and dedication, Schooling and competence, Family ties and other networks, Capital and loans, Business project, Chance and risk, Support, Rules and regulations, and Environment and context. The study postulates that the factors Drive and dedication, Schooling and competence, and Family ties and other networks are the most important. Chapter seven covers the effects of the demographical attributes of the microentrepreneurs and describes the casual relationships between the nine selected factors. Special attention is paid to the relative importance of each factor. The final chapter, the Conclusion, offers a summary of the findings and describes how they contribute to the three domains of knowledge: the structure and the agency of enterprises, the insight into the microentrepreneurs perceptions, and the development theory of micro- enterprises. It also highlights the implications of the findings for practitioners, support organisations, and official institutions, as well as making suggestions for further study. The final chapter ends with words of encouragement and caution.

Abstract: 11 pp. (1997)., Using the ARGUS detector at the e + e − storage ring DORIS II at DESY, we have found evidence for the production of the excited charmed baryon state Λ c (2593) + in the channel Λ c + π + π − . Its mass was determined to be (2594.6±0.9±0.4) MeV/c 2 , and the natural width measured to be Γ = (2.9 −2.1−1.4 +2.9+1.8 ) MeV. The production cross section times the branching ratios of σ ( e + e − → Λ c (2593) + X ) × Br ( Λ c (2593) + → Λ c + π + π − ) × Br ( Λ c + → pK − π + ) was measured to be (0.25 −0.13 +0.24 ±0.13) pb. The fractions of Λ c (2593) + decays proceeding through the Σ c 0 π + and Σ c ++ π − channels were determined to be 0.29±0.10±0.11 and 0.37±0.12±0.13, respectively.

Abstract: 27 pp. (1996)., A fast second level trigger has been developed for the ARGUS experiment which recognizes tracks originating from the interaction region. The processor compares the hits in the ARGUS Micro Vertex Drift Chamber to 245760 masks stored in random access memories. The masks which are fully defined in three dimensions are able to reject tracks originating in the wall of the narrow beampipe of 10.5\,mm radius.

Abstract: 14 pp. (1996)., Using the ARGUS detector at the e + e − storage ring DORIS II at DESY, we have made two measurements of the mixing parameter χ d using kaons as flavour tags. Using D ∗+ K ± correlations we found χ d = 0.20 ± 0.13 ± 0.12 and from the study of (D ∗+ ℓ − ) K ± correlations we obtained χ d = 0.19 ± 0.07 ± 0.09. The branching ratio for B → D ∗+ X has been updated: Br( B → D ∗+ X) = (19.6 ± 1.9) %. We have also determined the average multiplicity of charged kaons in B 0 decays to be 0.78 ± 0.08.

Abstract: 9 pp. (1995)., Using the ARGUS detector at the storage ring DORIS II we have measured τ decays into three charged mesons containingK* mesons. Exploiting the good particle identification capabilities of the detector we have determined the following branching ratios:$Br\left( {\tau ^ -\to \overline {K^{*0} } \pi ^ -v_\tau} \right) = \left( {0.25 \pm 0.10 \pm 0.05} \right)\% $,Br(τ−→K*0K−vτ)= (0.20±0.05±0.04)%, andBr(τ−→K*−X0vτ) =(1.15±0.15−0.18+0.13)%.

Abstract: 9 pp. (1993)., Using the ARGUS detector at the e+e− storage ring DORIS II, we have determined the Michel parameters ϱ and ξ of τ → μννandτ → eνν decays. From a data sample with 333 events/pb around s= 10GeV, we select 3230 events ǂe+e−→ τ+τ−→ (μ±μμ)(eǂvv) and determine ϱ from the e and μ momentum spectra and ξ from the correlations between e and μ momenta. For ϱ we obtain ϱτ→e = 0.79 ± 0.08 ± 0.06 and ϱτ→μ = 0.76 ± 0.08 in accordance with the V - A structure of the decays. The momentum correlations are only sensitive to the product ξτ→eξτ→μ. Setting ξτ→e = ξτ→μ, we obtain |ξτ| = 0.90 ± 0.15 ± 0.10 also in accordance with V - A. The updated decay fractions are B(τ →evv) = (17.5 ± 0.3 ± 0.5)% and B(τ → μvv) = (17.4 ± 0.3 ± 0.5)%.

Abstract: 19 pp. (1993)., Using the detector ARGUS at the $e^+e^−$ storage ring DORIS II, we have investigated inclusive momentum spectra of charged pions, kaons, and protons from decays of the υ(4S) meson. The kaon spectra have been measured in two independent ways, by coherently exploiting the detector's particle identification capabilities, and by detecting decays in-flight. The extracted mean multiplicities for charged hadrons are 7.17±0.05±0.14 pions, 1.56±0.03±0.05 kaons and 0.110±0.010±0.007 protons per υ(4S) decay, where pions and protons fromKso and Δ decays have been subtracted.

Abstract: 8 pp. (1993)., Using the ARGUS detector at the e+e− storage ring DORIS II at DESY, we have observed a new charmed baryon state in the channel Λc+π+π−. (All references to a specific charged state also imply the charge conjugate state.) The mass of this state was measured to be (2626.6 ± 0.5 ± 1.5) MeV/c2. The product of the production cross section and branching ratio for this channel was determined to be (11.5 ± 2.5 ± 3.0) pb, and the natural width estimated to be smaller than 3.2 MeV/c2 at 90% CL.

Abstract: 22 pp. (1993)., Using the ARGUS detector at the DORIS II $e^+e^−$ storage ring we have studied the three-pion hadronic final state in the decay $τ^- → π^-π^-π^+v_τ$. From about 7500 events we conclude that the three-pion system is dominated by the $J^P=1^+$ resonancea1 (1260) decaying preferentially via $ρ^0π^-$. Our data restrict a possible ρπ contribution of three-pion states without intermediate resonances to less than 6% at 95% CL. Fitting a model by Isgur et al. to the data yieldsma1=(1.211±0.007±0.0500.0)GeV/c$^2$ and $Γ$a1=(0.446±0.0021±0.1400.0)GeV/c$^2$. The second errors reflect the model dependence of the result. This is the currently most significant determination of thea1 resonance parameters. The ratio ofS- and D-wave amplitudes for the ρ0π- intermediate state of thea1− decay at the nominala1− mass was found to beD/S=−0.11±0.02. Using thisD/S ratio we update our former measurement of the parity violating asymmetry parameterγ $AV=2gAgV/(g^2_A+g^2_V)$ to 1.25±0.23±0.080.15. The branching ratio of the decay $τ^- → π^-π^-π^+v_τ$ is determined to be Br($τ^- → π^-π^-π^+v_τ$)=(6.8±0.1±0.5)%.

Abstract: 16 pp. (1993)., A search has been performed for rareB meson decays intoDs− mesons arising fromb→u transitions,W exchange modes,B+ annihilation processes, and decays where theDs+ is not produced via aW→c s¯s¯ quark pair coupling, using the ARGUS detector operating on the Ψ(4S) resonance at thee+e− storage ring DORIS II. Upper limits for individual decay modes are obtained. In addition, from a study ofDs+ℓ- correlations an upper limit of BR (B→Ds+ℓ-X) (90% CL) is determined.

Abstract: 13 pp. (1993)., Using the ARGUS detector at the DORIS-II electron-positron storage ring at DESY, we have investigated the exclusive decay $D∗+→ D^0 π^+, D^0→ K_S^0 π^+π^−$. From a partial wave analysis of the $K_S^0 π+^π^−$ system we find that (71.8± 4.2 ± 3.0)% are $D^0→ K∗^−π^+$ and (22.7 ± 3.2 ± 0.9)% are $D^0→K^0_ϱ 0$ with a relative phase of (−137 ± 7 ± 3)° between the channels. The remaining fraction can be described by several channels involving excited resonances, but not by a three-body phase space decay, thus giving first evidence for D0 decays into $K^0_∗(1430)^−π^+,K^0f^0(1975), K^0f^2(1270), and K^0f^0(1400)$.

Abstract: 19 pp. (1993)., The inclusive production cross sections of η′ (958) andfo (975) mesons are measured ine+e− annihilation in the nonresonant continuum around s√=10s=10 GeV and in decays of the υ resonances using the ARGUS detector. For η′ (958) mesons, a production ratio of η′ (958)/ηdir=0.35±0.24, with ηdir=η−BR(η′→ηX)·η′, is determined in direct υ(1S) decays, which can be partially explained by the pseudoscalar singlet/octet mixing. Forfo(975) production, we obtain a production ratio offo(975)/p(770)°=0.17±0.030 in direct υ(1S) decays. In its production features, thefo(975) behaves like an ordinary meson, though aK K¯K¯ molecule nature cannot be excluded. The substantial production yield of thefo(975) meson demonstrates the important effect of feeddown from mesons beyond the basic multiples on pseudoscalar and vector meson production

Abstract: 18 pp. (1992)., Using the ARGUS detector at the $e^+e^−$ storage ring DORIS II, we have studied nine decay modes of theD0 meson. In addition to a higher-precision determination of the branching ratios for these modes, several new channels, the decays $D^0 \to \bar K^0 \eta ',D^0 \to \bar K^{*0} \omega$ and $D^0 \to \bar K\pi ^ + \omega$ , have been observed for the first time.
