Lipoid pneumonia is a rare form of pneumonia which was initially described to be caused by inhalation or aspiration of fatty substances. Certain autopsy studies have reported the incidence to be 1.0-2.5%. Based on the mode of lipid acquisition, it has been classified into endogenous, exogenous or idiopathic types. Almost 50% of the patients with lipoid pneumonia are asymptomatic, and may be discovered by chance during routine chest imaging. In symptomatic patients, the symptoms are non- specific. However, it can produce inflammatory pneumonitis that can progress to irreversible pulmonary fibrosis as seen in our case. We present a case of a 53-year-old deceased male. A piece of one of his lungs was received after autopsy, which appeared normal grossly. There was no history of any illness before death. Microscopy revealed interstitial fibrosis with collection of foamy macrophages in alveolar spaces and cholesterol crystals surrounded by inflammatory reaction including occasional giant cells. The clinical picture and radiologic changes in cases of lipoid pneumonia can mimic bacterial pneumonia and tuberculosis. The occupational history is of extreme importance and should always be investigated.

Background and Objective: The currentstudy aimed at investigating the histomorphological spectrum of cervical intraepithelial and invasive lesions assessing the diagnostic significance of P16/INK4a and Ki-67 in such lesions, andcorrelatingP16/ INK4a and Ki-67 immunoexpression with histologic type and grade. Methods: A total of 60 cases were selectedcomprising 10 cases withchronic cervicitis, 29 cases withcervical intraepithelial neoplasia(CIN), and 21 cases withsquamous cell carcinoma. These cases were evaluated morphologically and immunohistochemically with P16 and Ki-67. Results: There was no expression of P16 and Ki-67 in 10 (100%) cases withchronic cervicitis while in CIN, it was expressed in 25 (86.20%) cases and in carcinoma it was expressed in 20 (95.23%) cases. Ki-67 was expressed in 28 (96.55%) cases withCIN and in 100% of cases withcarcinoma. Conclusion: Cervical carcinoma is a significant contributor to cancer-related morbidity and mortality worldwide. Identification of bio-markers in cervical neoplasia is necessary to distinguish CIN from other non-neoplastic cervical lesions to prevent under treatmentor overtreatment as the histomorphological features alone are not sufficient.Significant upregulation of P16, cyclin dependent kinase inhibitor, and Ki-67, a nuclear non-histone protein, was observed in carcinoma cervix and with the increasing severity of CINs. Correlation between grades of P16 and Ki-67 among cervical pre-neoplasia and neoplasia showed an increasing P16 expression with consistently increasing Ki-67 labelling index in the groups with theincreasing severity.

Primary soft tissue sarcomas of the breast constitute less than 5% of all soft tissue sarcomas and less than 1% of malignant breast cancers. The rarity of this tumor limits most studies to small retrospective case reviews and case reports. Primary breast sarcomas are locally aggressive tumors as evidenced by the high rate of local recurrence when excisional surgery is performed. A contemporary multidisciplinary approach to therapy including surgery, radiation, and chemotherapy is advocated. Herein, we report a case of 45-yr-old female, who presented with a large ulcerated breast mass and was diagnosed as carcinoma breast on fine needle aspiration. Modified radical masectomy was performed and was diagonsed with primary breast stromal sarcoma on histopathology, which is a rare entity.

Brunner’s gland hamartoma is a rare benign tumour of the duodenum. It was first described by Cruveilhier in 1835. Presently around 200 cases have been reported in literature. No sex predilection is seen. Patients usually present in the fifth to sixth decades of life. They may be clinically silent or may present with variable symptoms and occasionally obstructive symptoms and chronic pancreatitis. Endoscopic presentation can be nodular, polypoid or diffuse glandular proliferation with thickening of duodenal wall and hence can be misdiagnosed as malignancy. We describe a case of duodenal tumor reported outside (on biopsy) as well differentiated adenocarcinoma which out as Brunner gland hamartoma upon complete resection. Brunner gland hamartoma may sometimes have a very unusual presentation. Extensive pre-operative evaluation is necessity to avoid radical surgical procedure.

Objective: Pancytopenia is a clinicohematological problem with wide spread discriminated diagnosis. A through evaluation of patient is necessary to identify the cause as large number of patients has reversible etiology and early diagnosis may be lifesaving. Diagnosis of pancytopenia requires microscopic examination of bone marrow aspirates to assess the overall cellularity and morphology. This study was conducted with the aim to find out the cause of pancytopenia on the basis of bone marrow findings.Methods: The present study was conducted in the department of pathology over a period of one year. A total of 169 patients were included in the study that fulfilled the criteria of pancytopenia. A detailed clinical history and physical examination followed by complete blood count, peripheral smear examination and bone marrow aspiration was done in all cases.Results: There was slight male predominance with male to female ratio of 1.2:1. The majority of patients were in second and third decade. The main cause of pancytopenia was megaloblastic anemia followed by mixed nutritional deficiency anemia and others.Conclusion: This study emphasized that in developing countries like India majority of the patients had reversible etiology and patients can be put on a trial of hematinics and close haematological follow up.Key words: Pancytopenia, bone marrow, megaloblastic anemia, aplastic anemia, Amaç: Pansitopeni geniş bir ayırıcı tanı yelpazesi olan klinik ve hematolojik bir problemdir. Hastaların detaylı bir incelemesi gerekmektedir çünkü etiyolojide geri dönüşümlü bir etiyoloji olabileceği gibi erken patolojinin hayat kurtaracağı ciddi bir patolojide sorumlu olabilir. Pansitopeni tanısında kemik iliğinin hücresel ve morfolojik incelenmesi için kemik iliği aspirasyonu gerekmektedir. Bu çalışma, kemik iliği bulguları temelinde pansitopeninin nedenini bulmak amacıyla yapılmıştır. Yöntemler: Bu çalışma bir yıl süre içinde patoloji bölümünde gerçekleştirilmiştir. Pansitopeni kriterlerini karşılayan 169 hasta çalışmaya dahil edildi. Tüm olgularda, tam kan sayımı, periferik yayma incelemesi ve kemik iliği aspirasyonu takiben ayrıntılı klinik öykü ve fizik muayene yapıldı.Bulgular: Erkek kadın oranında erkek lehine hafif bir baskınlık vardı: 1/1.2. Hastaların büyük bir çoğunluğu, 10 lu ve 20 li yaşlarındaydılar. Pansitopenin ana nedeni megaloblastik anemi ve ardından beslenme yetersizliği anemisi ve diğer nedenler olarak görüldü.Sonuç: Bu çalışma göstermiştir ki Hindistan gibi gelişmekte olan ülkelerde pansitopeni etiyolojisi çoğunlukla geri dönüşümlü nedenlere bağlıdır ve bu ülkelerde hastalara kan yapıcı vererek yakın hematolojik takipleri önemlidir

Follicular dendritic cells or dendritic reticulum cells are important components of the immune system essential for antigen presentation. Malignancies arising from these cells are uncommon and the first case was reported in 1986. The most common sites of follicular dendritic cell sarcomas are lymph nodes, especially cervical, axillary and mediastinal regions, but extranodal sites including head and neck and gastrointestinal tract may be affected in one-third of patients. Immunohistochemistry plays an important role in its diagnosis to differentiate it from morphologically similar malignancies The present report describes a case of follicular dendritic cell sarcoma in a patient with chronic myeloid leukemia (CML) treated with imatininb mesylate for 6 years. This case deserves reporting due to rarity of the disease and hitherto unreported association with CML. Furthermore, the pathological diagnosis is challenging and requires a close-knit effort between the pathologist and haematologist.

Breast cancer is the commonest cancer of Indian women. Estrogen and Progesterone expression is seen in benign breast lesions and in breast carcinoma associated with good prognostic parameters and it correlates well with response to hormone therapy. Although a lot of studies have been conducted in the past on hormone receptor expression in breast cancer and few have correlated them with other prognostic parameters of breast cancer, the present study was intended to document the prevalence of hormone receptor positive breast carcinomas in our population; their importance in benign breast diseases; to document a reliable scoring system of hormone receptors expression by Quick scoring; to correlate them with most of the proven prognostic parameters of breast carcinoma.Tissue specimens from 25 patients with benign breast disease and 50 patients with breast carcinoma were assayed for estrogen (ER) and progesterone (PR) receptors using Quick scoring. ER/PR expression in breast carcinomas was correlated with various prognostic parameters including patients' age, menopausal status, tumor size, type, MBR grade, NPI, lymphatic vessel invasion, lymph node stage, lymphomononuclear invasion, elastosis and HER2/neu status.Scoring of steroid receptors paralleled intensity of hyperplasia in benign breast diseases but in breast carcinoma, it was inversely correlated with grade of tumor, NPI, HER2/neu status, tumor necrosis, lymphomononuclear infiltrate and elastosis. We found no relationship with tumor size, lymph node status or age.Assessment of hormone receptors for clinical management of breast cancer patients is strongly advocated to provide prognostic information and best therapeutic options.

Chediak-Higashi Syndrome is a rare autosomal recessive disease, characterized by oculocutaneous albinism, recurrent pyogenic infections of skin, mucosa and respiratory system, prolonged bleeding time, easy bruisability and neurologic deficit. The hallmark of this syndrome is the presence of abnormal intracytoplasmic giant granules in all granule containing cells including leukocytes in blood and bone marrow. We here report a case of Chediak-Higashi Syndrome in a 3 year old girl child who had recurrent chest infections, pancytopenia, hypopigmentation, lymphadenopathy and hepatosplenomegaly.

Leiomyomatosis peritonealis disseminata (LPD) is an exceedingly rare, usually benign condition that clinically simulates a disseminated malignancy, occurring predominantly in women of childbearing age. We present a case of LPD in a postmenopausal woman who had undergone hysterectomy 8 years back for fibroids along with simultaneous presence of pelvic metastasis from breast carcinoma.

Fetus-in-fetu is a rare condition in which a fetiform calcified mass is often present in the abdomen of its host; a newborn or infant. We present the case of a three-month-old male baby with acute intestinal obstruction and abdominal mass. X-ray abdomen and ultrasonogram revealed a cystic mass with calcification. On laparotomy, a well encapsulated retroperitoneal mass causing high intestinal obstruction was identified. Total excision of the mass was done. Diagnosis of fetus-in-fetu was confirmed on histopathology. Postoperative recovery was uneventful.

Xanthogranulomatous cystitis (XC) is a rare benign disease of unknown etiology. A case of XC in a 30-year-old male is presented due to sparcity of such case report in medical literature. Patient evaluation included clinical, biochemical and radiological studies before treatment. Histological study revealed the rare diagnosis. Patient was asymptomatic at eight weeks follow-up after treatment.
