Hellen CS Römkens, Henny JM Beckers, Jan SAG Schouten, Tos TJM Berendschot, Carroll AB Webers Department of Ophthalmology, Maastricht University Medical Center+, Maastricht, the Netherlands Purpose: The aim of this study is to generate reference values for anterior chamber morphometrics to facilitate the interpretation of anterior chamber images acquired with swept-source optical coherence tomography (SS-OCT) in a Caucasian population.Materials and methods: Non-glaucomatous Caucasian subjects, who were newly referred to the outpatient department of the University Eye Clinic Maastricht UMC+, were asked to undergo anterior segment assessment with SS-OCT imaging prior to routine eye examination (including visual acuity and refractive error measurements, Goldmann applanation tonometry, slit lamp examination and funduscopy).Results: A total of 647 Caucasian subjects, aged 40–80 years, were included. Mean age (±standard deviation) was 61.7±10.3 years, and 294 were male (45%). Mean refractive error was -0.63±3.17 D. OCT images and measurements were obtained in all cases. The mean temporal trabecular iris angle at 500 µm (TIA500) was 27.04°±12.04°, the mean nasal TIA500 was 27.43°±11.75°, the mean anterior chamber depth (ACD) was 2.75±0.39 mm and the mean lens vault (LV) was 0.44±0.30 mm. A smaller temporal TIA500, shallower ACD and higher LV were found in female and hyperopic subjects.Conclusion: Reference values for anterior chamber morphometrics in Caucasians were generated, quantified and presented. These values offer tools for the interpretation of anterior segment images of Caucasians and differentiation between presumed mechanisms of angle closure. P Mokhles, JSAG Schouten, HJM Beckers, CAB Webers University Eye Clinic Maastricht, Maastricht, the Netherlands Purpose: Blindness in glaucoma is difficult to assess with merely the use of the current World Health Organization (WHO) definition (a visual field restricted to 10° in a radius around central fixation), as this criterion does not cover other types of visual field loss that are encountered in clinical practice and also depict blindness. In this study, a 5-point ordinal scale was developed for the assessment of common visual field defect patterns, with the purpose of comparing blindness as outcome to the findings with the WHO criterion when applied to the same visual fields. The scores with the two methods were compared between two ophthalmologists. In addition, the variability between these assessors in assessing the different visual field types was determined.Methods: Two glaucoma specialists randomly assessed a sample of 423 visual fields from 77 glaucoma patients, stripped of all indices and masked for all patient variables. They applied the WHO criterion and a 5-point ordinal scale to all visual fields for the probability of blindness.Results: The WHO criterion was mostly found applicable and in good agreement for both assessors to visual fields depicting central island of vision or a temporal crescent. The percentage of blindness scores was higher when using the ordinal scale, 21.7% and 19.6% for assessors A and B, respectively, versus 14.4% and 11.3% for the WHO criterion. However, Kappa was lower, 0.71 versus 0.78 for WHO.Conclusions: The WHO criterion is strictly applied and shows good agreement between assessors; however, blindness does not always fit this criterion. More visual fields are labeled as blind when a less stringent criterion is used, but this leads to more interobserver variability. A new criterion that describes the extent, location, and depth of visual field defects together with their consequence for the patient’s quality of life is needed for the classification of glaucoma blindness. De aanwezigheid van antipolymeer antistoffen (APA) is in de literatuur beschreven bij vrouwen met ernstige ziekteverschijnselen en een siliconen-borstimplantaat (SBI). Het doel van het in dit rapport beschreven onderzoek is vast te stellen of er in Nederland een populatie vrouwen met een SBI bestaat met ernstige ziekteverschijnselen, en een hoge prevalentie van APA. Deelneemsters werden geselecteerd middels een uitgebreide vragenlijst met vragen over spier- en gewrichtspijn, vermoeidheid, en vragen over het lichamelijke en psychosociale gevoel van welbehagen. Bovendien werd van de huisarts informatie gevraagd over de klinische symptomen van de SBI vrouwen. In totaal namen 42 vrouwen deel aan het onderzoek. De leeftijd varieerde van 31-73 jaar. De gemiddelde blootstelling aan een SBI was 16 jaar. Bij de vrouwen werd klinisch onderzoek uitgevoerd en bloed afgenomen. Tevens werden bloedmonsters onderzocht van 80 vrouwen uit de algemene bevolking in dezelfde leeftijdscategorie (studiegroep plus of min 5 jaar) met een leeftijd van 26-69 jaar. De studiegroep werd ingedeeld in vier subgroepen met respectievelijk minimale, geringe, matige, en ernstige ziekteverschijnselen. De indeling vond plaats op basis van de afname van functionele capaciteit (zelfredzaamheid, wat kan men nog wel en niet meer doen in het dagelijkse leven), en de algemene beoordeling door de studie-arts wat betreft de ziekte-activiteit en een algemene schatting van de pijn. De meeste vrouwen met SBI (31 van de 42) werden ingedeeld in de groep met minimale ziekteverschijnselen. Vijf van de 42 SBI draagsters (11.9%, 95% betrouwbaarheidsinterval 4.0%-25.6%) hadden een positieve waarde voor de aanwezigheid van APA in het serum. Alle APA positieve reacties werden waargenomen bij vrouwen in de groep met minimale ziekteverschijnselen. In de controle groep werd bij vijf van de 80 vrouwen (6.3%, 95% betrouwbaarheidsinterval 2.1%-14.0%) een positieve APA waarde waargenomen. Het geringe verschil tussen de SBI draagsters en de controle groep is statistisch niet significant. Dit onderzoek kan de in de literatuur gerapporteerde resultaten over de aanwezigheid van antipolymeer antistoffen (APA) in het bloed bij vrouwen met een SBI en ernstige symptomen noch bevestigen noch ontkennen. Het feit dat het niet mogelijk bleek in twee studies voldoende vrouwen met een SBI en ziekteverschijnselen op te nemen, geeft aan dat het aantal vrouwen met een SBI en ernstige symptomen in Nederland gering is. Ook bij een normale populatie Nederlandse vrouwen wordt er een behoorlijk aantal vrouwen met een positieve waarde voor antipolymeer antistoffen in het bloed gevonden. Dientengevolge kan het gebruik van de "antipolymeer antistof" (APA) test niet aanbevolen worden als hulpmiddel bij de klinische evaluatie van vrouwen met een SBI en ernstige klachten/symptomen.

Het doel van het hier beschreven onderzoek is vast te stellen of er een populatie Nederlandse vrouwen is met een hoge prevalentie van antipolymeer antistoffen (APA) en ernstige klachten/ziekte-verschijnselen en blootstelling aan een siliconenborstimplantaat. Omdat de antigeenspecificiteit van de antipolymeer antistoffen (APA) nog niet is aangetoond, gebruiken wij de term polymeer bindende immunoglobulinen. De studiepopulatie werd geselecteerd uit een bestand van de Consumentenbond, Den Haag, waar vrouwen met een SBI zich op vrijwillige basis hadden laten registreren. De definitieve selectie werd uitgevoerd middels een vragenlijst met zelf gerapporteerde klachten. In totaal werden 42 vrouwen in de studie opgenomen. Bij deze vrouwen werd klinisch onderzoek uitgevoerd en bloed afgenomen. In de onderzoeksgroep werd bij een gering aantal vrouwen een verhoogd gehalte aan polymeer bindende immunoglobulines waargenomen. Ook bij een groep vrouwen zonder siliconenborstimplantaat, waarvan het serum onderzocht werd als controle op de uitvoering van de laboratorium test, werd bij een aantal vrouwen een verhoogd gehalte aan polymeer bindende immunoglobulines waargenomen. Met een gemiddelde implantatietijd van 17 jaar kan geconcludeerd worden dat blootstelling aan siliconen op zich geen inductie geeft van polymeer bindende immunoglobulinen. De studiepopulatie werd wat betreft ziekteverschijnselen in subgroepen (minimaal, gering, matig, ernstig) onderverdeeld op basis van afname van functionele capaciteit (zelfredzaamheid, wat kan men nog wel en niet meer doen in het dagelijkse leven) en algemene beoordeling door een arts wat betreft de ziekte-activiteit en een globale schatting van de pijn. De meeste vrouwen met SBI (34 van de 42) werden ingedeeld in de groep met minimale ziekteverschijnselen. De onderzoekspopulatie bleek wat betreft de ernst van de ziekteverschijnselen niet overeen te komen met de groep vrouwen waarbij in de literatuur een verhoogde prevalentie van polymeer bindende immunoglobulines was gevonden.

This report presents the prevalences of low back problems among a random sample of the general population aged 20-59 living in three towns in different regions in the Netherlands (Amsterdam, Doetinchem and Maastricht). The description includes different characteristics of the low back problems, its consequences and the high risk groups. Almost half of the population reports low back problems during the last 12 months and chronic low back problems are reported by one-fifth. Low back problems form a considerable public health problem, not only because of the high prevalences but also because of the major consequences in terms of disabilities, use of health services and work absenteeism.

Musculoskeletal complaints represent a big public health problem however data on the total population are scarce.Using cross-sectional data from a national population-based study on persons of 25 years and olderw we give estimates on prevalences of different musculoskeletal complaints, their consequences and risk groups in the Dutch population.The period prevalence (12 months) and point prevalence (Pp) of pain complaints of ten different anatomical sites were assessed in five groups: 1. neck, shoulder or higher part of the back, 2. elbow or wrist/hand, 3. lower part of the back, 4. hip or knee, 5. ankle and foot. Additional data included demographics, characteristics of the complaints (duration, severity, course) and consequences (perceived health, (work) disability, use of health care services). Data were weighted to present data on the Dutch population aged 25 years and over.The top 3 of self-reported musculoskeletal complaints was: 1. low back pain, Pp=26.9%, 2. shoulder pain, Pp=20.9% 3. neck pain, Pp=20.6%. In most cases the pain was described as continuous or repetitive, non-severe pain. In every 3 out of 10 cases the pain complaints were accompanied with limitation in daily life. Between 33% and 42% of those with complaints consulted their general physician for their pain. With exception of those who are work disabled, general socio-demographic characteristics cannot be used to identify high risk groups. Musculoskeletal pain is common among every subgroup in the population and varies from a non-severe pain complaint to a severe health problem with consequences for work and use of health care.

Using the simulation model context we analysed data the consistency of data on incidence, prevalence and mortality for specific chronic diseases so as to construct an appropriate data set for chronic disease models. These simulation models integrate data from different sources and are used to estimate the public health effects of trends in and interventions on disease risk factors. Disease incidence and prevalence data are derived from disease registration in general practice, epidemiological surveys and health-care registration. Disease-specific mortality data are collected from national statistics, while remission and case fatality rates are taken from the scientific literature. The successive steps followed in the consistency analyses were: (visual) comparison of data from different sources, calculation of remission and mortality rates from given disease incidence and prevalence rates and comparison of results with remission and mortality data from other sources. Data was evaluated on lung cancer, asthma and COPD, coronary heart diseases and congestive heartfailure, diabetes mellitus, and dementia and stroke. From the results it can be concluded that differences between data from several sources can often be explained by differences in registration coding rules. For most diseases, calculated excess mortality rates are much greater than the mortality where the disease is the primary cause of death, registered in national statistics. In general, the estimated mortality parameters are in agreement with data from the literature, with the estimation of age-specific rates being complicated by time-trends.
