Abstract

Item does not contain fulltext BACKGROUND: The bacille Calmette-Guérin (BCG) vaccine has immunomodulatory "off-target" effects that have been hypothesized to protect against coronavirus disease 2019 (Covid-19). METHODS: In this international, double-blind, placebo-controlled trial, we randomly assigned health care workers to receive the BCG-Denmark vaccine or saline placebo and followed them for 12 months. Symptomatic Covid-19 and severe Covid-19, the primary outcomes, were assessed at 6 months; the primary analyses involved the modified intention-to-treat population, which was restricted to participants with a negative test for severe acute respiratory syndrome coronavirus 2 at baseline. RESULTS: A total of 3988 participants underwent randomization; recruitment ceased before the planned sample size was reached owing to the availability of Covid-19 vaccines. The modified intention-to-treat population included 84.9% of the participants who underwent randomization: 1703 in the BCG group and 1683 in the placebo group. The estimated risk of symptomatic Covid-19 by 6 months was 14.7% in the BCG group and 12.3% in the placebo group (risk difference, 2.4 percentage points; 95% confidence interval [CI], -0.7 to 5.5; P = 0.13). The risk of severe Covid-19 by 6 months was 7.6% in the BCG group and 6.5% in the placebo group (risk difference, 1.1 percentage points; 95% CI, -1.2 to 3.5; P = 0.34); the majority of participants who met the trial definition of severe Covid-19 were not hospitalized but were unable to work for at least 3 consecutive days. In supplementary and sensitivity analyses that used less conservative censoring rules, the risk differences were similar but the confidence intervals were narrower. There were five hospitalizations due to Covid-19 in each group (including one death in the placebo group). The hazard ratio for any Covid-19 episode in the BCG group as compared with the placebo group was 1.23 (95% CI, 0.96 to 1.59). No safety concerns were identified. CONCLUSIONS: Vaccination with BCG-Denmark did not result in a lower risk of Covid-19 among health care workers than placebo. Abstract

International audience; We use spin-lattice dynamics simulations to study the possibility of modeling the low-temperature magnetic hysteresis behavior of a bulk ferromagnetic material. The temporal evolution of the magnetic and mechanical degrees of freedom is obtained through a set of two coupled Langevin equations. Hysteresis loops are calculated for different angles between the external field and the magnetocrystalline anisotropy axes. The influence of several relevant parameters is studied, including the field frequency, magnetic damping, magnetic anisotropy (magnitude and type), magnetic exchange, and system size. The role played by a moving lattice is also discussed. We find that the exchange and lattice dynamics barely affect the loops, while the field frequency and magnetic damping have a large effect on it. The influence of the anisotropy magnitude and symmetry are found to follow the expected behavior. We show that a careful choice of simulation parameters allows for an excellent agreement between the spin-lattice dynamics measurements and the paradigmatic Stoner-Wohlfarth model. Furthermore, we extend this analysis to intermediate and high temperatures, reaching values close to the Curie temperature. The present study opens the possibility for more accurate inclusion of lattice defects and thermal effects in hysteresis simulations.

Abstract

OBJECTIVES To analyse the frequency and causes of treatment discontinuation in patients who were treated with an integrase strand transfer inhibitor (INSTI), with a focus on neuropsychiatric adverse events (NPAEs). METHODS Patients in 18 HIV reference centres in France were prospectively included in the Dat'AIDS cohort. Data were collected from all patients starting an INSTI-containing regimen between 1 January 2006 and 31 December 2016. All causes of INSTI-containing regimen discontinuations were analysed, and patients' characteristics related to discontinuation due to NPAEs were sought. RESULTS INSTIs were prescribed to 21315 patients: 6274 received dolutegravir, 3421 received elvitegravir boosted by cobicistat, and 11620 received raltegravir. Abstract

Os objetivos desta pesquisa foram simular, analisar e quantificar o impacto econômico da frequência média anual de mastite clínica em rebanhos bovinos leiteiros. A pesquisa foi realizada por meio de simulação no sistema computacional CU$TO MASTITE, considerando rebanhos leiteiros com 1, 7 e 15% de mastite clínica. Foram consideradas como prevenção as despesas com monitoramento (cultura e antibiograma, contagem de células somáticas no tanque e contagem de células somática individual), pré e pós dipping, vacinação, tratamento de vacas secas e manutenção de ordenhadeira. Como medida curativa considerou-se apenas tratamento de casos clínicos. O impacto da mastite foi estimado como sendo o total em perdas acrescido das despesas com prevenção e tratamento de casos clínicos. O aumento da frequência média anual de mastite influenciou diretamente no impacto econômico da mastite, evidenciando a necessidade de monitoramento da mastite clínica e subclínica e adoção de medidas preventivas para diminuir os prejuízos ocasionados por essa enfermidade. As despesas com tratamento preventivo representaram, no máximo, 19,7% do impacto econômico, o que demonstra vantagens em investir nessa prática, pois ela irá contribuir significativamente para reduzir o impacto econômico da mastite. The objectives of this research were to analyze and quantify the economic impact of the annual average frequency of clinical mastitis in dairy cattle. The research was carried through data simulation in the CU$TO MASTITE computational system, considering dairy cattle with 1, 7 and 15% of clinical mastitis. The expenditures considered as prevention included those of monitoring (culture and antibiogram, bulk tank somatic cell count and individual somatic cell count) pre- and post-dipping, vaccination, treatment of dry cows and maintenance of the milking machine. The measures considered as healing consisted only of the treatment of clinical cases. The impact of the mastitis was estimated as being the total in losses in addition to the expenditures with prevention and treatment of clinical cases. The increase of the annual average frequency of mastitis directly influenced the economic impact of the mastitis. The higher economic impact evidences the need for the monitoring of clinical and subclinical mastitis and the adoption of preventative measures to diminish the damages caused by this disease. The expenditures with preventive treatment represented, at maximum, 19.7% of the economic impact, which demonstrates the advantages of investing in this practice, since it contributes significantly toward reducing the economic impact of mastitis.

5. Estudo de fundente e argilito com potencial para uso em formulações de porcelanato

Abstract

O porcelanato se destaca no mercado atual de revestimentos cerâmicos por apresentar excelentes características técnicas e estéticas. No presente trabalho foram selecionadas matérias-primas cerâmicas tradicionais e adotada uma formulação padrão, convencionalmente reportada na literatura, visando a substituição do componente fundente (feldspato) por uma matéria-prima formadora de fase líquida em alta temperatura, e a substituição da fração argilosa por um argilito de menor refratariedade. O objetivo do estudo foi avaliar a utilização destas matérias-primas como alternativas técnicas e de menor custo, uma vez que, em uma massa de porcelanato, a quantidade de fundente é muito grande (representa aproximadamente 50% do total das matérias-primas utilizadas). A nefelina sienito foi à matéria-prima natural substituta ao feldspato escolhida por pertencer à família dos feldspatóides e por possuir características físico-químicas semelhantes, sobretudo, elevada fundência. Para a substituição da fração argilosa foi escolhido um argilito de ocorrência natural, abundante na região de Santa Gertrudes, SP. As substituições ao feldspato e à fração argilosa ocorreram de maneira total e/ou parcial, por meio de combinações entre os fundentes e argilitos. As massas foram processadas a partir da mistura de pós, moagem a úmido, secagem, desaglomeração, compactação e queima. Além das curvas de gresificação, técnicas como dilatometria, medidas de porosidade aparente, ensaio de resistência mecânica à flexão a três pontos, microscopia eletrônica e difração de raios X foram utilizadas para a caracterização dos materiais após queima em diversas temperaturas. Os principais resultados mostraram que, a princípio, é possível a combinação das matérias-primas nefelina e argilito como material alternativo ao feldspato potássico e à argila S. Simão, pois elas proporcionam a gresificação em temperaturas mais baixas. Composições com ambas as matérias-primas apresentaram baixos valores de absorção de água ( 45 MPa), apesar de maior deformação piroplástica a 1150 ºC. The porcelain tile is distinguished in the current market for ceramic tiles, with excellent technical and esthetic characteristics. In the present study traditional ceramic raw materials were selected and a standard formulation conventionally reported in the literature was adopted aiming to replace the melting phase (feldspar) of the composition for a raw material that forms liquid phase at high temperature, and replacement of the clay fraction by a mudstone of lower refractoriness. The aim of this study was to evaluate the use of these materials as alternative techniques and lower cost, since, in a mass of porcelain tile, the amount of flux is very large (representing approximately 50% of total raw materials used). The nepheline syenite was the natural raw material selected to replace the feldspar due to the fact that it belongs to the family of feldspathoids, and has similar physicochemical characteristics, therefore, a high fluxing efficiency. To replace the clay fraction was chosen a mudstone naturally occurring, abundant in the region of Santa Gertrudes, SP. Substitutions to the feldspar and clay fraction were completely and/or partially by means of combinations between the flux and mudstones. The masses were processed from the mixing of powders, wet milling, drying, disagglomeration, compaction and firing. In addition to the vitrification curves, techniques such as dilatometry, measurements of porosity, flexure strength test, electron microscopy and X-ray diffraction were used to characterize the material after firing at various temperatures. The results showed that, in principle, it is possible the combination of raw materials such as nepheline and mudstone to replace potassium feldspar and S. Simão clay, as they provide the vitrification at lower temperatures. Compositions with both materials showed lower values of water absorption ( 45 MPa), despite higher pyroplastic strain at 1150 ºC.

RESUMO Os objetivos desta pesquisa foram analisar e quantificar a influência da contagem de células somáticas (CCS) sobre o impacto econômico da mastite. A pesquisa foi realizada por meio de simulação no sistema computacional CU$TO MASTITE, considerando rebanhos leiteiros com média de 250.000; 500.000; 750.000 e 1.000.000 células somáticas/mL de leite. Foram consideradas como prevenção as despesas com monitoramento (cultura e antibiograma, CCS do tanque e CCS individual), pré e pós dipping, vacinação, tratamento de vacas secas e manutenção de ordenhadeira. Como medidas curativas foram considerados os tratamentos com casos clínicos, cuja percentagem foi de 7% das vacas em lactação. O impacto da mastite foi estimado como sendo o total em perdas acrescido das despesas com prevenção e tratamento de casos clínicos. O aumento da contagem de células somáticas no tanque influenciou diretamente no impacto econômico da mastite. O elevado impacto evidencia a necessidade de monitoramento da mastite clínica e subclínica e adoção de medidas preventivas para diminuir os impactos ocasionados. As despesas com prevenção repre-sentaram, no máximo, 10,8% do impacto econômico, o que demonstra vantagens em investir nessa prática, pois irá contribuir significativamente para diminuição da CCS no tanque e, consequen-temente, para reduzir o impacto econômico da mastite.
