Background/Aim: Telemedicine, the remote delivery of healthcare services, represents a great opportunity for cancer pain management. A care model of telemedicine that combines remote visits and hospital access could be an effective and safe strategy for pain management of cancer patients. Patients and Methods: A retrospective study was conducted using the dataset of the telemedicine program at the Istituto Nazionale Tumori of Naples, Italy for assessing the efficacy of a telehealth-based model of care. Demographic, clinical, and process variables were investigated. Results: A total of 226 cases and 489 visits were included in the analysis. The mean age of patients was 63.4 years (SD=12.4 years), and no sex differences were observed. Approximately 55% of patients were ECOG-PS ≤2 and 87% suffered from metastatic disease. More than half of the patients were treated with high doses of opioids. Each patient had a mean of 2 remote visits and half of the patients had more than 1 telehealth consultation. The dropout ratio was 5.3%. Most visits (n=472) were conducted on patients in the Campania Region, Italy. The maximum covered distance from the Cancer Center and the patients’ location was 555.22 Km. A significant difference in the overall number of visits (p=0.006) and the number of pro-capita remote visits (p=0.010) was found, in favor of the group of patients treated before the end of the Covid-19 emergency in Italy, compared to those treated after the pandemic. Conclusion: Despite various positive outcomes, the analysis highlights several weaknesses, such as the need to assist patients with advanced and non-advanced disease located outside the regional territory. Overall, the telehealth processes should be adapted to post-pandemic scenarios towards their implementation in routine clinical practice.

The introduction of glycoconjugate vaccines marks an important point in the fight against various infectious diseases. The covalent conjugation of relevant polysaccharide antigens to immunogenic carrier proteins enables the induction of a long-lasting and robust IgG antibody response, which is not observed for pure polysaccharide vaccines. Although there has been remarkable progress in the development of glycoconjugate vaccines, many crucial parameters remain poorly understood. In particular, the influence of the conjugation site and strategy on the immunogenic properties of the final glycoconjugate vaccine is the focus of intense research. Here, we present a comparison of two cysteine selective conjugation strategies, elucidating the impact of both modifications on the structural integrity of the carrier protein, as well as on the immunogenic properties of the resulting glycoconjugate vaccine candidates. Our work suggests that conjugation chemistries impairing structurally relevant elements of the protein carrier, such as disulfide bonds, can have a dramatic effect on protein immunogenicity.

In August 2021, an extremely productive seismic sequence took place in northeastern Italy. Within 1 month, we identified and localized 407 microearthquakes in the area monitored by the local Collalto Seismic Network. This is about 20 times the average monthly rate of events detected in the last decade; 92% of them are clustered near the village of Refrontolo at about 9 km depth (main event a ML2.5, MW2.4). None of the Refrontolo sequence earthquakes were felt by the population. The earthquakes, mostly identified by automatic procedures, were post-processed by manual picking of P, S-phases and polarities; they were localized by different techniques and define a small SE dipping volume that departs from the brittle surface previously enlightened by microseisms and assigned to the NNW dipping Montello Thrust. We interpret the sequence ruptured pre-stressed patches near to failure of sub-vertical, antithetically oriented faults. The unusual productivity rate and precursory foreshock activity support the hypothesis that the Montello system has a relevant aseismic component. We believe that this episode, of no importance from an energetic point of view, is of greatest interest for deciphering the seismic potential of this area and for strengthening automatic microearthquake detection and location procedures.

The assessment of fetal heart rate variability (FHRV) is fundamental for evaluating fetal well-being during pregnancy. Through cardiotocography (CTG), it is possible to monitor both the fetal heart rate (FHR) and the uterine contractions (UC) and, through the use og computerized analysis, it is also possible to extract synthetic parameters of the FHRV. Among the available FHRV indices, the use of symbolic dynamics (SD) arises as a promising nonlinear indicator to investigate the complexity in FHRV signals. However, there is not enough evidence of the reliability of such indices and no consensus on the proposed formula to compute them. Based on previous works on this topic, here we aim at carrying out a sensitivity analysis of one of the proposed SD parameters for FHRV analysis, namely the Variability Index (VI). A database of 583 CTG signals has been used as basis for this study. The signals were preprocessed and analyzed to extract the FHRV series and calculate the VI according to a predefined set of weights. Then, the weights have been systematically varied and the sensitivity of the obtained VI in discriminating different fetal conditions and signal characteristics was assessed by means of a statistical approach. The results obtained show that the choice of weights can be decisive for specific purposes, e.g. in evaluating fetal development and distinguishing cesarean from spontaneous sections. On the other hand, weights do not alter the VI sensitivity in discriminating between active and rest states, presence and absence of accelerations.

Emergency medicine is a new discipline that is rapidly developing and spreading in medical practice. The central themes of emergency medicine are resuscitation, laboratory and diagnostic imaging. In the emergency department, different parameters can be associated with a prolonged Length of Stay (LOS) as, for example; the protact use of computed tomography (TAC), radiology techniques, the need for advice from external consultants (1). To improve the efficiency of the emergency department and the assessment effectiveness of the state of healthy patient, it is important to identify the factors that affect the LOS (2).This work was based on the evaluation of the impact of demographic factors, clinical information and diagnostic techniques on the LOS in emergency medicine (LOS-ED). The dataset was carried out at the Emergency Medicine Unit of the hospital “San Giovanni di Dio e Ruggi d’Aragona” of Salerno. Multiple Linear Regression model was optimized considering the hospital stay after diagnostic procedures (dLOS) as dependent variable.

Maintenance of kidney health is a global priority. This reflects the vital role that the kidneys play. Indeed, although often underestimated, the kidneys, perform various functions essential for our survival as maintaining water homeostasis through the removal of excess fluids and being responsible for the removal of toxic substances. Furthermore, they are involved in the regulation of blood pressure, the production of erythropoietin, activation of vitamin D to maintain bone health and the acid-base balance control to maintain the blood pH within normal values. Kidney damage at date is a very common condition and its identification and management are necessary in order to prevent its progression, comorbidities outcomes, reduce the mortality risks improving patient safety and medicines management. In this scenario, we propose a case study at University Hospital “San Giovanni di Dio e Ruggi d’Aragona” in Salerno (Italy) in the period between October 2018 and December 2020 on 847 patients with different kidney injuries considering their age and the presence of related pathologies. Aim of our wok is a better understand of the length of hospital stay (LOS) in order to improve the prediction of the disease stage, the quality of life of patients reducing hospitalization and mortality. Furthermore, our final goal is to understand the key mechanism related to the managing of patients reducing the costs.

Prolonged length of stay (LOS) is one of the most significant issues that hospitals must face since it can determine an increase of costs and risk of complications and a decrease of patient satisfaction. The capability of accurately predicting LOS is a valuable tool for helping hospital administrators in resource planning, encouraging quality improvement actions and providing support for medical practice. In particular, delayed hospital discharge in pediatric population is something that needs to be carefully considered because of their vulnerability and complexity from the medical viewpoint. Predicting the use of hospital resources and beds in pediatric departments could also help to better dimension hospitalizations management. This work has the aim to determine the predictive factors for the length of stay for patients admitted to the Complex Operative Units of Pediatrics and Pediatric Surgery at the “San Giovanni di Dio e Ruggi d’Aragona” University Hospital of Salerno and to build a classification model of LOS exploiting the potential of Machine Learning. Different algorithms are implemented, and their evaluation metrics are assessed and compared together to develop a prediction model with high performances.

Emerging evidence is offering significant insights into the effectiveness and safety of the vaccination against the Coronavirus Disease 2019 (COVID-19), but another crucial aspect of the current global vaccination campaign is the time trend of the antibody response to COVID-19 vaccines over a longer period and the resulting duration of the protection offered.1Here, we present data on the dynamics of antibodies that bind SARS-CoV-2 spike (S) protein receptor binding domain (RBD)—the most critical target for SARS-CoV-2-specific antibodies within the S1 sub-unit2—after 6months from the administration with BNT162b2 vaccine. This analysis, built as a longitudinal observational design, is part of the VASCO project (‘Monitoraggio della risposta alVaccinoAnti-SARS-CoV-2/COVID-19 negli operatori sanitari del Pineta Grande Hospital’), which defines an ongoing broad study on the response to BNT162b2 mRNA COVID-19 vaccine in a sample of healthcare workers (HCWs) of the Pineta Grande Hospital (Castel Volturno, Caserta, Italy), investigating effectiveness, immunogenicity and safety of the vaccination. Complete methods of the VASCO project have been presented elsewhere.3 In this survey, HCWs who were administered the two-dose BNT162b2 mRNA vaccine 21days apart between December 2020 and January 2021 were invited to undertake a quantitative serology test for the research of SARS-CoV-2 S-RBD-specific immunoglobulins G (IgG). Seroconversion, defined as the development of any detectable SARS-CoV-2 S-RBD-specific IgG in serum sample, was evaluated through Snibe—Maglumi®SARS-CoV-2 S-RBD IgG chemiluminescent immunoassay (CLIA).4Reactivity was intended as an antibody level equal to or greater than 1.0AU/ml. According to the manufacturer’s recommendations, the Maglumi®SARS-CoV-2 S-RBD IgG CLIA presented sensitivity of 100% (95% confidence interval [CI], 99.9–100%) and specificity of 99.6% (95% CI, 98.7–100%) after the 15th day from symptom onset.4HCWs underwent six longitudinal serological assays every 30days, the first of which was performed within 1 month after completing the vaccination cycle. If a HCW had had a previous infection with SARS-CoV-2 6 months prior to the vaccination or if he/she had contracted the infection after the administration of the first vaccine dose, the cycle was considered complete with a unique dose, as per Italian Ministry of Health guidelines and according to literature findings.5,6CLIA results were expressed as median IgG value and interquartile range (IQR). Differences between medians were assessed through Mann–Whitney U test; multivariate regression analyses were built to investigate the association between the level of the vaccine-elicited antibodies and potential predictors, such as sex, age, previous SARS-CoV-2 infection and post-first dose infection. AP-value of 0.05 was set as significance level. Overall, we analyzed the sera of 162 subjects, being mostly women (58.0%) with a mean age of 42.5years (±11.9 SD). Twenty-eight HCWs had a history of previous SARS-CoV-2 infection. At the first serum sample, the median anti-S-RBD IgG reached 540.0AU/ml (IQR 64.5–1102.0). In the following tests, a progressive decay of antibodies was seen, up to the value of 55.7AU/ml (IQR 26.2–84.7) at the 6-month follow-up (Figure 1). No significant associations were found according to vaccinees’ sex and age. Within 1 month from the vaccination, there was a significant higher S-RBD-reactive antibody response in those subjects with previous SARS-CoV-2 infection (medians: 1534.9 [IQR 1142.0–2000.0] vs. 407.7 [IQR 60.0–846.6];P = 0.001) and the significance remained after adjusting for age and sex (β = 1762.2; 95% CI 1022.9–2501.6;P R2adj = 0.29). Differences in IgG titres between those with previous SARS-CoV-2 infection and those without were no longer significant at the following serological surveys. Three vaccinees tested positive at the RT-PCR assay for qualitative detection of SARS-CoV-2 nucleic acid on nasopharyngeal swabs before the administration of the second vaccine dose; no statistical association was detected between the infection after the first dose and humoral response. No other infections were reported in the follow-up passive surveillance described previously.3

The present paper provides theoretical insights regarding the determinants of firms’ incentives to invest in a Circular Economy. The analysis relies on a Cournot model disaggregating the disposal cost in the production function. In a non-simultaneous sequential game, two risk-neutral firms are endowed with a green innovation project that, if successful, would reduce the overall production costs and implement a Circular Economy. Firms are plagued by asymmetric information about the exact value of the other firm’s innovation. In this setting, the R&amp, D investment in a Circular Economy, by affecting the distribution of production and disposal costs, influences the production decisions of both the innovating and the rival firms. The sign of the impact depends on the firms’ strategy in the product market. Furthermore, the analysis points out that cooperation in R&amp, D of firms competing in the product market reinforces incentives to invest in green innovation. This suggests that governments aimed to advance a Circular Economy should encourage firms’ cooperation.

De fato, estudos teóricos e medidas de campo mostram que uma considerável proporção do tráfego com acesso a uma segunda célula pode também se comunicar com uma terceira. Este trabalho de tese explora este atributo e propõe um modelo de tráfego onde assinantes com acesso a duas ou três estações rádio base sejam direcionados convenientemente para uma ou outra célula de acordo com as regras de decisão da estratégia adotada. Em particular, a estratégia estudada é a Variação do Limiar de Bloqueio. O processo de tráfego é modelado utilizando-se a Cadeia de Markov, através do Processo de Nascimento e Morte tridimensional aplicado a três células. O desempenho do sistema é avaliado através de análise numérica e alguns casos particulares podem ser tratados pela fórmula Erlang-B. Verifica-se, de uma maneira geral, que o uso de um terceiro caminho de rádio, ao invés de dois apenas, pode melhorar o desempenho de - tráfego do sistema. This study aimed to elucidate how the genetic alterations caused by Down Syndrome (DS) impact cases of involvement by SARS-CoV-2, in addition to showing how social isolation can negatively affect the lives of individuals with DS. The study adopted the qualitative literary review model, through the Google Academic, SciELO and PubMed databases, using the descriptors “Down Syndrome” and “Coronavirus”, with twenty articles being selected. It was noticed that DS is the most common genetic form of intellectual disability and the changes that trisomy 21 cause significantly impact the disease course of people with DS infected with Coronavirus, since their immune system is overactivated, with ability to generate a shower of cytokines, which will facilitate the entry of the virus into cells, thus favoring viral replication. Furthermore, it was noted that, even though social isolation is effective in combating the pandemic, it can cause several damages to the health of the population with DS, which can aggravate certain pre-existing comorbidities in this group. Therefore, the surveillance and training of this population and their families is very important, so that they can correctly prevent themselves – with an effective hand washing technique and use of barrier equipment – ​​minimizing exposure to the virus; and socialization and physical activity – even during quarantine periods and remotely – in order to preserve the emotional and physical health of these individuals Este estudio tuvo como objetivo dilucidar cómo las alteraciones genéticas provocadas por el Síndrome de Down (SD) impactan en los casos de afectación por SARS-CoV-2, además de mostrar cómo el aislamiento social puede afectar negativamente la vida de las personas con SD. El estudio adoptó el modelo de revisión literaria cualitativa, a través de las bases de datos Google Academic, SciELO y PubMed, a través de los descriptores “Síndrome de Down” y “Coronavirus”, seleccionándose veinte artículos. Se notó que el SD es la forma genética más común de discapacidad intelectual y las alteraciones que ocasiona la trisomía 21 impactan significativamente en el curso de la enfermedad de las personas con SD infectadas por Coronavirus, ya que su sistema inmunológico está sobreactivado, con capacidad para generar una lluvia de citocinas. lo que facilitará la entrada del virus en las células, favoreciendo así la replicación viral. Percebeu-se que a SD é a forma genética mais comum de deficiência intelectual e as alterações que a trissomia do cromossomo 21 acarretam, impactam significativamente no curso da doença de pessoas com SD infectadas pelo Coronavírus, já que o sistema imune delas é hiperativado, com capacidade para gerar uma chuva de citocinas, que facilitará a entrada do vírus nas células, favorecendo, assim, a replicação viral. Ademais, notou-se que, mesmo o isolamento social sendo eficaz no combate à pandemia, ele pode acarretar diversos danos à saúde da população com SD, o que pode agravar certas comorbidades pré-existentes nesse grupo. Portanto, é muito relevante a vigilância e a capacitação dessa população e de suas famílias, para que previnam-se de forma correta – com técnica eficaz de lavagem de mãos e uso de equipamentos de barreira – minimizando a exposição ao vírus; e a socialização e a prática de atividade física – mesmo durante os períodos de quarentena e de forma remota – de maneira que preserve-se a saúde emocional e física desses indivíduos.

Esta investigação pretendeu perceber e analisar como o projeto de animação para a leitura” Contos Contigo”, desenvolvido em contexto de Biblioteca Escolar, direcionado para os alunos de 1º ciclo do ensino básico, poderá ser uma prática eficaz para incentivar hábitos de leitura. O projeto surgiu a partir de uma situação concreta, ou seja, da necessidade imperativa de agir para desenvolver competências leitoras, motivar para a leitura e o gosto pelo livro, face a alunos que não manifestavam interesse particular pela leitura. Foi nosso propósito, com este estudo de caso, analisar e refletir sobre os resultados obtidos relativamente à procura do livro, criação de hábitos de leitura e examinar a influência educacional relacionada com a animação em contextos lúdicos de aprendizagem da leitura para a promoção de hábitos leitores. Com a análise dos dados obtidos, através dos instrumentos de recolha de informação utilizados e das atividades realizadas, verificámos que a Biblioteca Escolar, através do projeto de animação “Contos Contigo”, desempenhou um papel relevante para a promoção da leitura e contribuiu desta forma para uma mudança significativa de atitudes em relação ao gosto pelo livro por parte do público-alvo. This research aimed to understand and analyse how the reading animation project "Contos Contigo", developed in the context of the School Library, developed with students of Primary School education, can be an effective practice to encourage reading habits. This project arose from a concrete situation, that is, from the imperative need to act developing reading skills, and motivating students who reject reading. And also to develop interest for reading books with children who had no particular interest in reading. It was our purpose, with this case study, to analyze and reflect on the results obtained in relation to the search for the book, creation of reading habits and to examine the educational influence related to animation in playful contexts of learning to read for the promotion of reading habits. The analysis of the data obtained through data-collecting instruments used and the activities carried out, showed that the School Library, through the "Contos Contigo" animation project, played a relevant role in promoting reading and thus contributed to a significant change in attitudes regarding the taste for books on the part of the target public.

Aim of study: To demonstrate the virulence of a Mycobacterium bovis local pig isolate in order to contribute to a better understanding of the pathological and immunological consequences of M. bovis infection in previous sensitized animals. Area of study: Buenos Aires, Argentina Material and methods: One group of ten pigs received two oral doses of killed M. bovis suspension and a comparative intradermal tuberculin test (CIT) (multiple sensitized) and then was infected with the M. bovis strain. Another group only received the CIT (single sensitized) and the infective dose. Humoral immune response was followed monthly, and gross pathology, histopathological and bacteriological analysis were performed at necropsy 100 days after infection. Main results: M. bovis oral infection induced lesions and allowed bacterial growth in most of the animals. Previous sensitization with killed M. bovis suspension slightly raised the intensity of the response, as the multiple sensitized group showed higher lesion scores and humoral response. Research highlights: Although the differences in lesion scores were not statistically significant, oral route infection after sensitization can modify the course of infections towards a fast development of lesions with a higher fibrotic component suggestive of increased resistance to infection in the right conditions. Atmospheric particulate matter is a concern in most of the European towns because it has potential negative effects on human health (Davidson et al., 2005; Lelieveld et al., 2015). Although the toxic effects of PM have been correlated with some of its chemical and physical properties, the toxicity mechanisms are not yet fully known. Different in vitro toxicological tests are often necessary to characterise potential health effects and, often, it is found significant correlation only among a few of the possible tests. In addition, contrasting results could be obtained comparing in vitro tests with acellular assays like those used to determine oxidative potential (Steenhof et al., 2011; Van Den Heuvel et al., 2018). The aim of the work was to study the oxidative potential (OP) of PM10, determined with the acellular DTT assay, in relationship with its ecotoxicological and cytotoxicological potential. The study was carried out on aqueous extracts of 10 samples of airborne PM10 randomly selected among the samples collected between 16/09/2017 and 25/12/2017. Samples were collected using a low-volume (2.3 m3 /h) sampler (SWAM, Fai Instruments srl) on 47 mm quartz fibre filters (Whatman) pre-fired at 700 °C for 2 hours in order to reduce contamination. Samples were exposed for 24 hours (starting at midnight) at the Environmental-Climate Observatory of ISAC-CNR in Lecce (Southern Italy), regional station of the Global Atmosphere Watch (GAW) network, characterised as an urban background site (Cesari et al., 2018). The aqueous extraction was performed in an ultrasonic bath for 80 min using 10 ml Milli-Q water. The ecotoxicological potential of PM10 was assessed by the bioluminescence inhibition assay based on the Gram-negative non-pathogenic bacterium Vibrio fischeri (Microtox® test), which physiologically emits light as a results of its metabolic activity. The natural bioluminescence of V. fisheri is inhibited by the exposure to a number of chemical pollutants, including organic and inorganic compounds (Abbass et al., 2018). Different exposure times (5, 15, and 30 mins) were used and inhibition results, obtained with five repetitions, are reported as a net effect corrected using field blanks. The cytotoxicological potential of PM10 was assessed on the same extracts by the MTT assay on the cell line A549. The MTT assay is based on a colorimetric reaction dependent on mitochondrial respiration of the cells and indirectly allows assessing the cellular energy capacity of a cell (Stockert et al., 2012). The MTT assay was applied to the A549 cell line, representative of the alveolar type II pneumocytes of the human lung (Foster et al., 1998). Cell mortality after 24h exposition is evaluated, in relative terms, considering the net effect of PM10 using field blanks for correction. Six repetitions were done. The water-soluble fraction of PM10 was also used for the analysis of the OP, performed with the dithiothreitol assay (DTT), a surrogate for cellular antioxidants, which analyses the rate of DTT depletion catalysed by chemical species present in the PM (Chirizzi et al., 2017). An aliquot of the extracts was diluted with deionised water (1:4 factor). Diluted samples were incubated at 37 °C with DTT (0.1 mM) in 0.1 M potassium phosphate buffer at pH 7.4 for times varying from 5 to 90 min. At designated times (specifically at 5, 10, 15, 20, 30, 45, 60, and 90 min) an aliquot of incubation mixture was picked up and 10% trichloroacetic acid was added to stop the reaction. Then, this reaction mixture was mixed with a solution containing 10 mM DTNB. The concentration of the formed 5-mercapto-2-nitrobenzoic acid was measured by its optical density absorption at 412 nm using a Eon BioTek Microplate Spectrophotometer. The consumption of DTT over time was determined through the linear fitting of the absorbance with the time in which it was made the withdrawal. The DTT depletion rate was used to determine OP values as DTT-activity 21 normalized in terms of sampled air volume (OPV) or in terms of mass of collected aerosols (OPM). The OPV and OPM values were comparable with previous measurements in this area or in other Italian towns (Chirizzi et al., 2017). In all the 10 samples analysed a significant inhibition of the Vibrio fisheri bioluminescence was observed as a results of the exposure of bacteria to the undiluted extracts for 5, 15 and 30 min, suggesting the presence in the PM10 of components able to induce an ecotoxic effect. Four samples (samples n. 2,3,4, and 7) showed a % of inhibition ranging from 30% to 50%, ascribable to a slight toxic effect, while six samples (samples 1,5,6, 8,9, and 10) showed a % of inhibition above 50% after 30 min exposure, suggesting the presence of a toxic effect. Determinar la diversidad molecular de Mycobacterium avium subsp. paratuberculosis (MAP) en muestras ambientales de hatos lecheros colombianos. Materiales y métodos. Las muestras ambientales de 25 hatos lecheros positivos a MAP por IS900-qPCR se cultivaron por duplicado en medio de yema de huevo de Herrold con micobactina J para obtener aislamientos. Las colonias sospechosas fueron confirmadas para MAP por IS900-qPCR. El ADN positivo se subtipó utilizando técnicas de unidades micobacterialess repetitivas intercaladas - número variable de repeticiones en tándem (MIRU-VNTR) y técnicas de repeticiones de multilocus de secuencia corta (MLSSR) para analizar las diferencias genéticas entre los aislamientos. Resultados. El subtipado reveló dos genotipos diferentes por MIRU-VNTR (INMV 2 e INMV 36). La técnica de MLSSR se realizó para aumentar el poder discriminatorio de lo obtenido por MIRU-VNTR, pero no se observaron diferencias entre los aislamientos recuperados. Conclusiones. To determine Mycobacterium avium subsp. paratuberculosis (MAP) molecular diversity in invironmental samples from Colombian dairy herds. Materials and methods. Environmental samples from 25 IS900-qPCR MAP-positive dairy herds were cultured by duplicate in Herrold´s egg yolk medium with mycobactin J to obtain isolates. Suspicious colonies were confirmed by MAPIS900-qPCR. Positive DNA was sub-typed using mycobacterial interspersed repetitive units-variable number of tandem repeat (MIRU-VNTR) and multilocus short sequence repeats (MLSSR) techniques to analyze the genetic differences between the isolates. Results. Sub-typing revealed two different genotypes by MIRU-VNTR (INMV 2 and INMV 36). MLSSR technique was carried out to increase the discriminatory power from what was obtained by MIRU-VNTR, but no differences were observed among the recovered isolates. Conclusions. Non-communicable chronic diseases (NCCDs) are the main cause of morbidity and mortality globally. Demographic aging has resulted in older populations with more complex healthcare needs. This necessitates a multilevel rethinking of healthcare policies, health education and community support systems with digitalization of technologies playing a central role. The European Innovation Partnership on Active and Healthy Aging (A3) working group focuses on well-being for older adults, with an emphasis on quality of life and healthy aging. A subgroup of A3, including multidisciplinary stakeholders in health care across Europe, focuses on the palliative care (PC) model as a paradigm to be modified to meet the needs of older persons with NCCDs. This development paper delineates the key parameters we identified as critical in creating a public health model of PC directed to the needs of persons with NCCDs. This paradigm shift should affect horizontal components of public health models. Furthermore, our model includes vertical components often neglected, such as nutrition, resilience, well-being and leisure activities. The main enablers identified are information and communication technologies, education and training programs, communities of compassion, twinning activities, promoting research and increasing awareness amongst policymakers. We also identified key ‘bottlenecks’: inequity of access, insufficient research, inadequate development of advance care planning and a lack of co-creation of relevant technologies and shared decision-making. Rethinking PC within a public health context must focus on developing policies, training and technologies to enhance person-centered quality life for those with NCCD, while ensuring that they and those important to them experience death with dignity.

Six months of stratospheric aerosol observations with the European Aerosol Research Lidar Network (EARLINET) from August 2017 to January 2018 are presented. The decay phase of an unprecedented, record-breaking stratospheric perturbation caused by wildfire smoke is reported and discussed in terms of geometrical, optical, and microphysical aerosol properties. Enormous amounts of smoke were injected into the upper troposphere and lower stratosphere over fire areas in western Canada on 12 August 2017 during strong thunderstorm–pyrocumulonimbus activity. The stratospheric fire plumes spread over the entire Northern Hemisphere in the following weeks and months. Twenty-eight European lidar stations from northern Norway to southern Portugal and the eastern Mediterranean monitored the strong stratospheric perturbation on a continental scale. The main smoke layer (over central, western, southern, and eastern Europe) was found at heights between 15 and 20 km since September 2017 (about 2 weeks after entering the stratosphere). Thin layers of smoke were detected at heights of up to 22–23 km. The stratospheric aerosol optical thickness at 532 nm decreased from values > 0.25 on 21–23 August 2017 to 0.005–0.03 until 5–10 September and was mainly 0.003–0.004 from October to December 2017 and thus was still significantly above the stratospheric background (0.001–0.002). Stratospheric particle extinction coefficients (532 nm) were as high as 50–200 Mm−1 until the beginning of September and on the order of 1 Mm−1 (0.5– 5 Mm−1 ) from October 2017 until the end of January 2018. The corresponding layer mean particle mass concentration was on the order of 0.05–0.5 µg m−3 over these months. Soot particles (light-absorbing carbonaceous particles) are efficient ice-nucleating particles (INPs) at upper tropospheric (cirrus) temperatures and available to influence cirrus formation when entering the tropopause from above. We estimated INP concentrations of 50–500 L−1 until the first days in September and afterwards 5–50 L−1 until the end of the year 2017 in the lower stratosphere for typical cirrus formation temperatures of −55 ◦C and an ice supersaturation level of 1.15. The measured profiles of the particle linear depolarization ratio indicated a predominance of nonspherical smoke particles. The 532 nm depolarization ratio decreased slowly with time in the main smoke layer from values of 0.15–0.25 (August–September) to values of 0.05–0.10 (October–November) and < 0.05 (December–January). The decrease of the depolarization ratio is consistent with aging of the smoke particles, growing of a coating around the solid black carbon core (aggregates), and thus change of the shape towards a spherical form. We found ascending aerosol layer features over the most southern European stations, especially over the eastern Mediterranean at 32–35◦ N, that ascended from heights of about 18–19 to 22–23 km from the beginning of October to the beginning of December 2017 (about 2 km per month). We discuss several transport and lifting mechanisms that may have had an impact on the found aerosol layering structures.

What are the constitutive elements of drama? In this innovative analysis of theatre history through the lens of system theory, this paper shows how this approach can support the idea of an intrinsically relational nature of the theatre praxis itself, its genesis and the origin of modern dramaturgy, meaning the act of organizing the various elements of a mise-en-scene, both on and off stage. The aim of this research consists also in showing that what we call drama is an emergent property that is generated by the connection between the actors and the audience.

Working in small groups, students in ISCI 3A12 examine the science behind winemaking, from the art of viticulture to its eventual consumption by the public. Students perform literature-based reviews and original research in order to understand the wine industry and its complexity; including the environmental requirements and consequences of winemaking, the short- and long-term health effects of drinking wine, and what factors contribute to the quality, aroma, and taste of wine. Nos últimos anos ocorreu um aumento significativo na demanda de tomate (Lycopersicum esculentum Mill) em decorrência de um processo de reeducação alimentar que vem ocorrendo, onde as pessoas estão cada vez mais em busca de produtos saudáveis para sua alimentação. Os tomates possuem componentes essenciais para saúde como vitamina C, provitaminas A, traços de potássio, fósforo, ferro, ácido acético e ácido málico, além ser um alimento que possui baixo teor calórico. Este trabalho objetivou caracterizar atributos físico-químicos do tomate da variedade conhecida popularmente como “italiano” produzido em três locais: 1- Município Mojuí dos Campos, 2- comunidade Estrada Nova e 3- comunidade Tipizal no município de Santarém, na região Oeste do Pará, através de parâmetros físicos de massa, diâmetro, comprimento e firmeza dos frutos, e parâmetros químicos de potencial hidrogeniônico (pH), acidez titulável (AT) e sólidos solúveis (SS). O delineamento experimental foi o inteiramente casualizado, composto por três tratamentos e cinco repetições, onde cada repetição constitui-se de um fruto, analisados pelo teste de Tukey a 5% de probabilidade pelo software Assistat® beta 7.7. Os frutos não apresentaram diferenças estatísticas (p>0,05) nos parâmetros físicos e em pH. Diferenciaram-se nos teores de SS expresso em °Brix, AT expresso em % de ácido cítrico e relação entre SS/AT. Os tomates produzidos na região Oeste do Pará possuem características físico-químicas dentro dos padrões do Centro de Qualidade em Horticultura. This work objected characterize the physico-chemical characteristics of the variety known as "Italian", produced in three different locations: 1 – In city Mojuí dos Campos, 2 – Community Tipizal and 3 – Community Estrada Nova in the city Santarém, western region of Pará state, through of physical parameters of weight, diameter, length, firmness of fruits and parameters chemical of hydrogen potential (pH), titratable acidity (AT) and solids soluble (SS). Glioblastoma can avoid immune surveillance and induce tumor tolerance, through inhibitory molecules, e.g. PD-L1. Ionizing radiation (IR), used to treat this tumor, is known to increase tumor expression of PD-L1, thus inducing resistance mechanisms. Finding molecular determinants involved in IR-induced PD-L1 may provide a target for preventing such an effect and improve radiotherapy outcomes. We demonstrated that the short isoform of the cochaperone FKBP51 (FKBP51s) regulated PD-L1 expression in melanoma. In glioma, FKBP51s was expressed at high levels, together with PD-L1 and its silencing reduced PD-L1 levels. Conversely, overexpression of FKBP51s increased PD-L1. Different PD-L1 isoforms were observed by immunoblot. A lower band (~37 kDa) corresponding to the naïve protein and two upper bands (~50, ~68 kDa) ascribable to post-translationally modified isoforms. FKBP51s was found mainly bound to the heaviest bands of PD-L1, reasonably mature protein, while the canonical isoform FKBP51 appeared to bind only the naïve protein. Mature PD-L1 protein consists in carbohydrates addition, the principal chemical modification to most plasma membrane proteins, and, particularly, N-glycosylation. Treatment of immunoprecipitated PD-L1 protein with PNGaseF produced a decrease of the highest band and the appearance of a lower band, corresponding to the naïve PD-L1, in accordance with the concept that the heaviest band of PD-L1 is a glycosylated form. Moreover, following subcellular fractionation to obtain extracts from ER and Golgi compartments, we found that naïve 37 kDa PD-L1 was detectable in the ER, but not in the Golgi. The PD-L1 glycosylated band was expressed in ER in a small proportion and mostly in the Golgi. FKBP51s, but not the canonical FKBP51, was found in ER. Co-IP of FKBP51s and PD-L1 from ER extract confirmed the two proteins interacted each other in ER. Our results show that naïve PD-L1 colocalized in the ER of glioma cell complexed with FKBP51s, while the PD-L1 glycosylated form was measured in the Golgi apparatus. Treatment of glioma cell with increasing doses of IR upregulated PD-L1 expression, in a dose-response manner. Particularly, we found a significant increase in PD-L1 expression at 4 and 8 Gy, in comparison with unirradiated glioma cell. Moreover, IR induction of mature PD-L1 was efficiently counteracted by FKBP51s silencing. Subcellular fractionation of glioma cell subjected to IR in kinetics showed an early and transitory decrease in FKBP51s ER levels at 3hrs, in line with a reduction of the glycosylated band in the whole lysate. After 8 hrs from IR, FKBP51s rose up again in the ER inducing a full maturation of PD-L1. These findings suggested that FKBP51s has a role in catalyzing PD-L1 folding, an essential step to glycosylation, through which it controls the affinity for PD1. This study identifies FKBP51s as an essential element that regulates PD-L1 expression on glioma cell, which is exploited by the tumor to resist to IR. Consequently to the diffusion of these systems, new software and complex signal processing methodologies are spreading and are gaining more and more interest. For testing the performances of new software and/or techniques, the availability of synthetic signals with a-priori known characteristics can be very helpful. In this paper we describe an updating of a procedure previously published for the computerized simulation of fetal heart rate signals, realized in order to obtain more realistic signals. Obtained results, analyzed by a team of experts, are satisfying, in that new simulated signals appear less regular and more complex, and hence more similar to real signals. Nevertheless, further research is necessary for obtaining very realistic signals. The future research will be initially focused on the study of floatingline and uterine contractions simulation.

Gli "Atti della difesa nel processo penale" sono pensati per offrire un solido strumento operativo in vista dell'esperienza giudiziaria, terreno sempre più scivoloso - tra l'altro - a causa dell'evoluzione normativa e giurisprudenziale. L'Opera non costituisce un semplice formulario, ma torna utile in tre diverse prospettive. In linea di fondo, essa presenta un insieme aggiornato di norme (Codice di procedura penale - con le disposizioni di attuazione, coordinamento e transitorie - la disciplina sul processo minorile e quella dinanzi al giudice di pace) dove ogni articolo reca in calce eventuali pronunce d'illegittimità costituzionale; fungono da premessa la Costituzione, la Convenzione europea dei diritti dell'uomo, il Patto internazionale dei diritti civili e politici, con i relativi Protocolli, i Trattati dell'UE. In secondo luogo, l'Opera contiene l'analisi delle norme (secondo gli orientamenti della giurisprudenza) raggruppata per "Quadri essenziali", settori in cui si concentra la disamina delle disposizioni utili a formulare gli "Atti difensivi". Il volume costituisce la sistematizzazione teorica e metodologica di alcuni momenti della riflessione e di un itinerario euristico di Luigi Caramiello e dei suoi collaboratori, sviluppati nell'ambito del corso di "Sociologia dell'Arte e della letteratura" all' Università degli Studi di Napoli Federico II. La raccolta, per certi aspetti originale e atipica, intende far dialogare i presupposti epistemologici fondamentali per un approccio corretto alla vicenda dell'espressività, con l'analisi delle concrete esperienze prodottesi sul piano sociologico e storico. Il testo si propone, quindi, come una tappa rilevante per ricostruire e comprendere, non solo alcuni momenti essenziali nella vicenda delle arti plastiche e visive e del loro rapporto con le loro dinamiche fruitive e sociali, ma anche per indagare i complessi meccanismi che sottintendono alla costruzione dei "mondi dell'arte", nell'ottica di quel work-in-progress tipico di ogni viaggio intrapreso sui sentieri della scienza.

Among the high-grade malignant gliomas (III-IV), grade IV glioma, or glioblastoma (GBM), is the most common and destructive form of brain cancer. Despite rigorous molecular, preclinical, and clinical research these tumors remain a challenge to treat and 70% of patients diagnosed with GBM will succumb to the disease within 2 years. New immune-based cancer treatments are currently in development, among which, anti-PD-1 is in a phase II clinical trial for recurrent glioblastoma. Tumor cells exploit PDL-1/PD1 pathway to induce T cell exhaustion and evade host antitumour immunity. PDL-1 expression on glioblastoma has been shown to correlate with a bad prognosis. There is strong demand for knowledge about the mechanisms regulating PDL-1 expression in glioblastoma. Our research group, has previously identified a relevant role for FKBP51, and its spliced isoform FKBP51s, in PDL-1 expression and melanoma resistance to the therapy. FKBP51 is an immunophilin capable of immunosuppression, originally cloned in lymphocytes. Because FKBP51 was identified as highly expressed in glioblastoma, correlating with tumor aggressiveness, we investigated whether this protein and/or its spliced isoform FKBP51s could be involved in regulation of PDL-1 expression in glioblastoma. In addition, we looked at the role of these protein-isoforms in glioblastoma resistance to chemotherapy. To this aim, we used two different glioblastoma cell lines termed D54 and U251. Our results suggest that, differently from melanoma, in which PDL-1 is inducible and associated with FKBP51 splicing, glioblastoma cell lines express high basal levels of PDL-1. Interestingly, both the canonical and the spliced FKBP51 isoforms were highly expressed in the glioblastoma cell lines. Expression of PDL-1 was regulated by FKBP51s knockin and knockout and the levels changed in accordance with FKBP51s levels. Moreover, in glioblastoma cells silenced for FKBP51s, the decrease in PDL-1 levels was associated with a significant increase in apoptosis, either spontaneous apoptosis and the one stimulated by etoposide or temozolomide. Two recently developed compounds, selective inhibitor of FKBP51, termed SAFit1 and SAFit2 were able to reduce expression of PDL-1 on glioblastoma cell lines. Particularly, SAFit 2 was found to be more effective, in accordance with the notion that it is more brain-permeable than SAFit1. SaFit 2 was also shown to increase sensitivity to etoposide of glioblastoma cell, although to a lesser extent when compared to FKBP51siRNAs. In conclusion, our preliminary results suggest that FKBP51s plays a relevant role in glioblastoma resistance and PDL-1 expression. Future studies are needed to shed light on the mechanisms underlying FKBP51s-induced PDL-1 expression and signaling pathway downstream to FKBP51s/PDL-1 that sustain glioblastoma resistance.

OBIETTIVO: La tossicità cutanea è uno dei principali effetti collaterali causati dalla radioterapia, sia a breve ma soprattutto a lungo termine, con comparsa di xerosi, ittiosi, rugosità e lesioni precancerose e cancerose. Lo scopo del presente protocollo osservazionale è quello di valutare l’efficacia e la tollerabilità di una crema contenente acido ialuronico sale sodico 2 mg/g e sulfadiazina argentica 10 mg/g (Connettivina plus) nel trattamento della radiodermite acuta. METODI: Tre centri dermatologici coinvolti per un totale di 40 pazienti; i pazienti sono stati divisi in due gruppi: il gruppo A, composto da 20 pazienti, ha applicato la crema in studio due volte al giorno, a partire dalla prima visita dermatologica fino a 7 giorni dopo l’ultima seduta di radioterapia; il gruppo B, composto dagli altri 20 pazienti selezionati, ha applicato una crema comunemente consigliata dai centri dermatologici ai pazienti in trattamento, contenente olio di argan. RISULTATI: È stato eseguito un t-test per dati appaiati. Dai risultati ottenuti si evince come la crema in studio a base di acido ialuronico e sulfadiazina argentica abbia determinato: una riduzione del grado di eritema, valutato mediante immagini digitali; una diminuzione della TEWL, valutata mediante Tewameter; un miglioramento dell’idratazione cutanea valutato sia mediante corneometria sia mediante D-squame®; un’ottima tollerabilità valutata mediante apposito questionario. CONCLUSIONI: Nei pazienti sottoposti a terapia radiante l’applicazione del prodotto in studio rappresenta un valido presidio terapeutico in grado di migliorare e prevenire gli effetti collaterali della radioterapia.

Jazz, immaginario, ideologia" analizza il fenomeno della musicalità popolare moderna mediante un approccio sociologico, caratterizzato in senso storico-evolutivo, capace di rendere conto della complessità del fenomeno, con l’obiettivo di individuare le molteplici e diverse variabili, che contribuiscono a fare della musica un autentico mediatore sociale e una potente dimensione significante dei mondi di cui è espressione.
