Autonomous locomotion is a ubiquitous phenomenon in biology and in physics of active systems at microscopic scale. This includes prokaryotic, eukaryotic cells (crawling and swimming) and artificial swimmers. An outstanding feature is the ability of these entities to follow complex trajectories, ranging from straight, curved (circular, helical...), to random-like ones. The non-straight nature of these trajectories is often explained as a consequence of the asymmetry of the particle or the medium in which it moves, or due to the presence of bounding walls, etc... Here, we show that straight, circular and helical trajectories emerge naturally in the absence of asymmetry of the swimmer or that of suspending medium. Our first proof is based on general considerations, without referring to an explicit form of a model. We show that these three trajectories correspond to self-congruent solutions. Self-congruency means that the states of the system at different moments of time can be made identical by an appropriate combination of rotation and translation of the coordinate space. We show that these solutions are exhibited by spherically symmetric particles as a result of a series of pitchfork bifurcations as the activity is increased. Self-congruent dynamics in one and two dimensions are analyzed as well. Finally, we present a simple explicit nonlinear exactly solvable model of fully isotropic phoretic particle that shows the transitions from a non-motile state to straight motion to circular motion to helical motion as a series of spontaneous symmetry-breaking bifurcations. Whether a system exhibits or not a given trajectory only depends on the numerical values of parameters entering the model, while asymmetry of swimmer shape, or anisotropy of the suspending medium , or influence of bounding walls are not necessary., Comment: 23 pages, 6 figures

Locomotion is essential for living cells. It enables bacteria and algae to explore space for food, cancer to spread, and immune system to fight infections. Motile cells display trajectories of intriguing complexity, from regular (e.g. circular, helical, and so on) to irregular motions (run-tumble), the origin of which has remained elusive for over a century. This dynamics versatility is conventionally attributed to the shape asymmetry of the motile entity, to the suspending media, and/or to stochastic regulation. We propose here a universal approach highlighting that these movements are generic, occurring for a large class of cells and artificial microswimmers, without the need of invoking shape asymmetry nor stochasticity, but are encoded in their inherent nonlinear evolution. We show, in particular, that for a circular and spherical particle moving in a simple fluid, circular, helical and chaotic motions (akin to a persistent random walk) emerge naturally in different regions of parameter space. This establishes the operating principles for complex trajectories manifestation of motile systems, and offers a new vision with minimal ingredients. The reduced evolution equations based on symmetries are consistent with those derived for a model of an autophoretic particle including diffusion, emission/absorption at the particle surface and hydrodynamics, and provide qualitative and quantitative agreement., Comment: 11 pages, 3 figures

We propose new mathematical models of inventory management in a reverse logistics system. The proposed models extend the model introduced by Nahmias and Rivera with the assumption that the demand for newly produced and repaired (remanufacturing) items are not the same. We derive two mathematical models and formulate unconstrained and constrained optimization problems to optimize the holding cost. We also introduce the solution procedures of the proposed problems. The exactness of the proposed solutions has been tested by numerical experiments. Nowadays, it is an essential commitment for industries to reduce greenhouse gas (GHG) emissions as well as energy consumption during the production and remanufacturing processes. This paper also extends along this line of research, and therewith develops a three-objective mathematical model and provides an algorithm to obtain the Pareto solution., Comment: Submitted

International audience; Cancer chemotherapeutic drug containing PEGylated lipidic nanocapsules (D-LNCs) were formulated by the controlled addition of organic phase (combined solution of paclitaxel and curcumin in a mixture of oleic acid and MPEG2000-DSPE (90:2.5 molar ratio) in acetone) to the aqueous phase (consist of Poloxamer 407 as emulsifying agents and glycerol as a co-solvent) at a temperature of 55-60°C followed by evaporation of organic solvent. The obtained pre-colloidal dispersion of D-LNCs was processed through high pressure homogenization to get more uniformly and nano-sized particles. Effect of concentration of emulsifying agent and process variables of high pressure homogenization (pressure and number of cycles) on average particle size and entrapment efficiency was further investigated by constructing Box-Behnken experimental design to achieve the optimum manufacturing process. D-LNCs were characterized by dynamic light scattering, scanning and transmission electron microscopy, Fourier transform infrared spectroscopy, and differential scanning calorimetry. In vitro release studies showed a sustained release pattern of drug from the PEGylated D-LNCs, whereas in vivo pharmacokinetic studies after a single-dose intravenous (i.v.) administration of paclitaxel (15mg/kg) in Ehrlich ascites tumor (EAT)-bearing female Swiss albino mice showed a prolonged circulation time and slower elimination of paclitaxel from D-LNCs as compared with marketed formulation (Paclitec®). Purpose: The objective of the study is to determine the direct cost of treatment of ischemic heart disease patient in tertiary care hospital in Karachi. Methods: This is a retrospective cost of illness study conducted in tertiary care hospital in Karachi between May 2015 to October 2015. All patients were included in the study diagnosed with IHD during admission at hospital between a period of August 2014 to June 2015. The research study conducted in a 100 beds tertiary care hospital attached with a medical university located in Karachi. All in patients were included in the study with an age group of (>18 years old) who were diagnosed, or suffered from IHD (Ischemic heart disease) (with respect to the patient file) were included in the study. Over all we evaluated 700 files of cardiovascular disease patients from which we separated the IHD patients’ files. From separated IHD files we selected the most relevant 75 IHD patient files. Patient clinical information were obtained from the patients ‘medical file, and added with the unit cost of services to determine expense. Results: Overall most relevant 75 patient files were selected for the study purpose from the 700 files of cardiovascular disease patients’ files. Most of the patients were male, aged from 30 to 85 years. Co morbidity was found in 60 (80%) patients, a single co morbid condition consist of 26 (43.3%) patients and two or more consist of 31(51.6%) patients. Hypertension and diabetes were most commonly found co morbid condition in ischemic heart disease patients. Chest pain, Shortness of breath were the most commonly symptoms found in ischemic heart disease patients. Most commonly prescribed medication were aspirin 77 (61.60%), clopidogrel 73 (58.40%), nitro-glycerine 35 (28%), Enoxaparin 38 (30.40%) and Atorvastatin 41 (32.80%). The mean total direct cost was Rs. 359975. The cost component included were length of stay cost Rs. 27697; laboratory and diagnosis cost Rs. 37684; medication cost Rs. 21019 and surgical procedure cost Rs. 273574. The percentage of the total expense according to the each component was as follows: length of stay expense (7.69%), laboratory and diagnosis cost (10.47%), hospital stay cost (7.69%), surgical procedures (75.99%). Surgical procedure was main component of cost of treatment of Ischemic heart disease patients. The median hospital stay was 2 days in ischemic heart disease patients. Conclusion: IHD is associated with high economic burden on society. Among different component of direct cost of treatment of IHD, surgical procedure was the main component of cost of treatment of ischemic heart disease patients.

11. Cucurbit Extracts Augment Biofilm Formation by Probiotic Lactobacilli: An In Vitro Study

Gastrointestinal tract predominantly harbor trillions of microorganisms including probiotics which maintain enteric microbial homeostasis. Some dietary cucurbits were investigated for their effects on biofilm formation by Lactobacillus rhamnosus (L. rhamnosus), Lactobacillus plantarum (L. plantarum), Lactobacillus acidophilus (L. acidophilus), Escherichia coli (E. coli) and Salmonella enterica typhimurium (S. enterica typhi). Aqueous and methanol extracts of Lagenaria siceraria (Ls), Luffa cylindrica (Lc) and Cucurbita maxima (Cm) were prepared and evaluated their effective concentrations on these bacterial strains. Effective concentrations for methanol and aqueous extracts were found 93.60 μg/mL-115.40 μg/mL and 103.67 μg/mL-121.00 μg/mL, respectively. For the both types of extracts toxicity was determined up to 1 mg/mL concentration and found no microbicidal effects on probiotic strains. However, marginal inhibition on the growth of biofilms of pathogenic bacteria was observed. Extracts were found to support growth of biofilms of probiotics. Ls and Lc exhibited marginal inhibition on biofilm formation by E. coli and S. enterica typhi. Therefore, on the basis of our results it may be said that Lagenaria siceraria (bottle guard) and Luffa cylindrica (sponge guard) are safe, non-toxic and may be recommended as nutraceutical.

A bacterial strain Azohydromonas australica DSM 1124 has been chosen, which is accumulated intracellular poly- β-hydroxybutyrate particles, each method which has been merit and demerit for employed in PHB extraction. Our study, we have selected chloroform-sodium hypochlorite method. It is one of the methods for extracting PHB forms Azohydromonas australica DSM 1124. Polyhydroxybutyrate is a biodegradable and biocompatible thermoplastic with many interesting applications in medicine, food packaging and tissue engineering materials. That does not produce any toxins or residues in the environment like petroleum based plastics. The present study was emphasized on enhanced production of PHB by Azohydromonas australica using sucrose as a sole carbon source and estimation of biomass and sucrose content in the media.The batch kinetics analysis of Azohydromonas australica was done an interval of 3 h. Batch cultivation with optimized media recipe in a 7 L bioreactor exhibited a maximum biomass 1.71 g/L and PHB concentration 2.67 g/L for A. australica. The characterization of PHB was done by growth kinetics studies, UV-Spectrophotometer and Fourier transforms infrared spectroscopy (FTIR).

BACKGROUND: Statin therapy reduces low-density lipoprotein (LDL) cholesterol levels and the risk of cardiovascular events, but whether the addition of ezetimibe, a nonstatin drug that reduces intestinal cholesterol absorption, can reduce the rate of cardiovascular events further is not known.METHODS: We conducted a double-blind, randomized trial involving 18,144 patients who had been hospitalized for an acute coronary syndrome within the preceding 10 days and had LDL cholesterol levels of 50 to 100 mg per deciliter (1.3 to 2.6 mmol per liter) if they were receiving lipid-lowering therapy or 50 to 125 mg per deciliter (1.3 to 3.2 mmol per liter) if they were not receiving lipid-lowering therapy. The combination of simvastatin (40 mg) and ezetimibe (10 mg) (simvastatin-ezetimibe) was compared with simvastatin (40 mg) and placebo (simvastatin monotherapy). The primary end point was a composite of cardiovascular death, nonfatal myocardial infarction, unstable angina requiring rehospitalization, coronary revascularization (≥30 days after randomization), or nonfatal stroke. The median follow-up was 6 years.RESULTS: The median time-weighted average LDL cholesterol level during the study was 53.7 mg per deciliter (1.4 mmol per liter) in the simvastatin-ezetimibe group, as compared with 69.5 mg per deciliter (1.8 mmol per liter) in the simvastatin-monotherapy group (PCONCLUSIONS: When added to statin therapy, ezetimibe resulted in incremental lowering of LDL cholesterol levels and improved cardiovascular outcomes. The aim of this study was to know the clinical and haematological changes in female rabbits following the treatment with cypermethrin (CY). In this study, 24 female rabbits were divided randomly into four equal groups A, B, C and D. Groups A, B and C were injected intraperitoneally CY @ 25, 50 and 75 mg/Kg b. wt., respectively, while the group D served as control and received normal saline. Rabbits in each group received four treatments at an interval of five days. Blood samples with EDTA were collected after two days of each treatment for haematological studies. All the animals were monitored twice daily for clinical signs. Signs like increased urination, licking of legs, jerky movements, ataxia, incoordination, staggering gait and dizziness persisted for 30 to 90 minutes after each dose. The correct use of cardiovascular drugs in patients has been shown to decrease the risk associated with cardiovascular morbidity and mortality. The objective of this study was to determine the drug utilization pattern in cardiovascular diseases. For this purpose we conducted this study in various tertiary care setups located in various areas of Karachi, from January to March, 2014. We collected the data from 100 patients having different age groups. The collected data was assessed to determine the prescribing trends. Results indicated that hypertension and ischemic heart diseases were mostly diagnosed and mostly diseases were treated by giving the drugs in combinations. The use of Beta Blockers, Diuretics, Calcium Channel Blockers and Angiotensin Converting Enzymes (ACE) inhibitors was very common. Also prescribing errors related to dosing frequency and prescribed dose were also determined. Results were analyzed with SPSS 20 using Chi square model to calculate the study outcome. Furthermore standard error and statistical variance for the given data is also calculated by descriptive analytical procedure. Present study will help the health practitioners to optimize the appropriate use of cardiovascular drugs.

Drawing on the results of a set of surveys into needs analysis, students´ peer response and student feedback, it is suggested that SA can play a significant role by providing wider input into the content, design and implementation of an EAP course by creating opportunities to engage students in interesting and meaningful classroom experiences and providing essential data for reviewing and evaluating the course to improve and promote its effectiveness. Este artículo investiga un aspecto particular de la participación del estudiante - análisis de los estudiantes en un programa de comunicación oral del currículo de un pregrado en negocios y comercio de la Universidad Sultan Qaboos del Sultanato de Oman - y examina su rol en el mejoramiento y promoción de la efectividad del aprendizaje en el discurso del aula de inglés con propósitos académicos. Gram positive cocci are responsible for a large number of infections, involving the skin and skin structures, respiratory tract, bloodstream etc., both in the community as well as in the hospital settings. However, the recent emergence of multidrug resistant strains has compromised therapeutic options as well as made therapy less effective and costlier. As medicine continues to evolve to combat these pathogens, they always seem to be a step ahead of us. Vancomycin, the drug of choice for resistant strains of gram positive cocci, has also seen the development of bacteria resistant to it. The introduction of ceftobiprole, a novel fifth-generation cephalosporin, has brought with it new hope for combating these pathogens. It exerts its antibacterial effect by binding to the PBP (penicillin-binding protein), blocking formation of the bacterial cell wall and ultimately leading to cell lysis and death. It has also got a wide antibacterial spectrum covering many gram negative bacteria as well as anaerobes. Ceftobiprole has been evaluated in various clinical trials including the multicentric STRAUSS 1 and 2 trials, and the results have demonstrated favourable efficacy of ceftobiprole against gram positive cocci. Thus, although ceftobiprole provides us with another option in our battle against the microbes, its judicious use is imperative so that we do not run out of therapeutic options in the near future.Key words: ceftobiprole, MRSA, cephalosporin, cocci.

BackgroundMyocarditis is a highly morbid complication of immune checkpoint inhibitor (ICI) use that remains inadequately characterized. The QRS duration and the QTc interval are standardized electrocardiographic measures that are prolonged in other cardiac conditions; however, there are no data on their utility in ICI myocarditis.MethodsFrom an international registry, ECG parameters were compared between 140 myocarditis cases and 179 controls across multiple time points (pre-ICI, on ICI prior to myocarditis, and at the time of myocarditis). The association between ECG values and major adverse cardiac events (MACE) was also tested.ResultsBoth the QRS duration and QTc interval were similar between cases and controls prior to myocarditis. When compared with controls on an ICI (93±19 ms) or to baseline prior to myocarditis (97±19 ms), the QRS duration prolonged with myocarditis (110±22 ms, pConclusionsThe QRS duration is increased in ICI myocarditis and is associated with increased MACE risk. Use of this widely available ECG parameter may aid in ICI myocarditis diagnosis and risk-stratification.

Introduction: myocarditis is a potentially fatal complication of immune checkpoint inhibitors (ICI). While corticosteroids are the cornerstones of the treatment, there are no data to guide the dose and timing. Methods: from an international registry of patients with ICI myocarditis diagnosed between 2013 and 2019, data on the type, dose (in methylprednisolone equivalent dose) and timing of steroids were extracted. Major cardiovascular events (MACE) were a composite of cardiovascular death, cardiogenic shock, cardiac arrest, and hemodynamically-significant complete heart block. Results: in total, 143 ICI myocarditis patients (67±13 years old, 29% women) were included. Among them, 125 received corticosteroids (87%), with the initial agent being either methylprednisolone (95, 76%), prednisone (25, 20%), hydrocortisone (2, 1.6%) or dexamethasone (3, 2.4%). The rates of overall MACE (by admission time tertile 1: 45.8%, tertile 2: 43.8%, tertile 3: 38.3%, P=0.746) and individual elements of MACE were unchanged from 2013 to 2019. The initial corticosteroid dose was categorized as low (500mg).
