PT.PQR adalah perusahaan penyedia jasa minyak dan gas di Indonesia yang memiliki beberapa jembatan sebagai infrastruktur penunjang transportasi dalam pengeboran minyak yang tersebar di Provinsi Riau. Jembatan PCK-03-B yang dibangun pada tahun 1959 menjadi obyek penelitian dengan fokus pada pipa-pipa penyangganya yang memiliki sambung­an pengelasan yang rentan mengalami korosi. Penelitian ini bertujuan untuk meminimalisasi tingkat risiko yang ditimbulkan akibat kerusakan material pada pipa penyangga jembatan sebagai bagian dari kegiatan pemeliharaan terhadap kondisi material pipa penyangga. Metode Risk-Based Maintenance (RBM) digunakan dengan menentukan Probability of Failure (PoF) dan Consequence of Failure (CoF). CoF terdiri dari Stand by Availability, Financial Model dan Location Model yang dihitung untuk mendapatkan hasil berupa Criticality Matrix. Pipa penyangga PL 2 C dijadikan acuan kondisi keseluruhan jembatan PCK-03-B karena memiliki laju korosi terkritis sebesar 0,151 mm/tahun dengan sisa umur layan jembatan paling pendek yaitu 9 tahun mendatang jika dibandingkan dengan yang lainnya. Hasil analisis menunjukkan bahwa pipa penyangga PL 2 C diperoleh nilai PoF sebesar 2 dan CoF sebesar 10 yang menunjukkan dalam kategori 2D dengan tingkat kerusakan kategori sedang. Rentang penjadwalan perbaikan jembatan PCK-03-B ditetapkan pada 8 tahun (per-2019) mendatang yaitu di tahun 2027, sebelum masa umur pakai habis, sehingga dapat meng­hindari risiko terjadinya kecelakaan yang tidak diharapkan.

This study aims to determine the risks that occur in the Construction Project of the Integrated Laboratory Building and Man 1 Library, Central Maluku. The method used is a qualitative analysis method. The subjects in this study were workers in the Integrated Laboratory Building Development Project and the Man 1 Library in Central Maluku totaling 30 samples. The primary data collection techniques are through questionnaires and secondary data collection is through work items while the data analysis methods used are Validity Test, Reality Test, and data analysis with a risk matrix., Penelitian ini bertujuan untuk mengetahui risiko-risiko yang terjadi pada Proyek Pembangunan Gedung Laboratorium Terpadu dan Perpustakaan Man 1 Maluku Tengah. Metode yang digunakan adalah metode analisis kualitatif. Subjek dalam penelitian ini adalah pekerja di Proyek Pembangunan Gedung Laboratorium Terpadu dan Perpustakaan Man 1 Maluku Tengah yang berjumlah 30 sampel. Teknik pengumpulan data primer melalui kuesioner dan pengumpulan data sekunder melalui item pekerjaan sedangkan metode analisis data yang digunakan adalah Uji Validitas, Uji Realitas, dan analisis data dengan matriks risiko.

Este es un proyecto comunitario mancomunado entre la Universidad Santo Tomás y su carrera de ingeniería industrial y la empresa Megaproyectos de la ciudad de Tunja busca la capacitación en conocimientos teórico-prácticos a la especialidad de ebanistería de la Institución Educativa San Jerónimo Emiliani en los procesos de producción, preoperacionales, costos, seguridad y salud en el trabajo. Este trabajo está enmarcado bajo la metodología acción-participativa de corte cualitativo y dispondrá de dos etapas. La primera contará con una observación directa estructurada donde se establecerán las necesidades teórico-prácticas a través de encuestas, lo que permitirá ver falencias y ventaja de la población seleccionada. La segunda fase será participativa donde se establecerán los planes de capacitación y trabajo con cada uno de los estudiantes ebanistas. Allí el trabajo se dividirá en tres fases: corte, enchape y ensamble; el resultado será el mobiliario para los niños/adolescentes del internado San Jerónimo. This study aims to develop and validate the checklist instrument and risk assessment matrix (CMAR) in mountain sports, namely hiking, canyoning, and climbing. The development of the CMAR was carried out through document analysis, literature review, and expert consultation. The content validation of the CMAR was done using the Delphi technique, involving 10 experts (aged: 35-58 years, 42.30±7.70 years; experience in the area: 17.00 ± 6.8 years), and the construct validation using exploratory factor analysis, involving 548 technicians and practitioners of hiking (n182), canyoning (n:183), and climbing (n:183). The 1st CMAR version proved to be relevant, clear and objective (ICCspecialists: 0.85-0.90) and included 51 items (risk factors) grouped into 3 dimensions. The final version of the CMAR included 36 items structured in 4 dimensions (human, materials and equipment, environmental, and safety and emergency), to be evaluated in a risk matrix of 5x5 points (probability x consequence), which leads up to a stratified risk level corresponding to a recommendation, action, and treatment (KMOmodel: 0.816; justified variance model: 56.4%). CMAR demonstrated good reliability and good-to-acceptable values for construct validation, enabling its use in risk analysis and management in mountain sports, such as hiking, canyoning, and climbing., O objetivo deste estudo foi desenvolver e validar o instrumento checklist e matriz de avaliação do risco (CMAR) em desportos de montanha, nomeadamente, pedestrianismo, canyoning e escalada. O desenvolvimento do CMAR realizou-se mediante análise documental, revisão de literatura e consulta de especialistas. A validação de conteúdo do CMAR efetuou-se recorrendo à técnica Delphi, envolvendo 10 especialistas (idade: 35-58 anos, 42,30±7,70 anos; experiência na área: 17,00 ± 6,8 anos), e a validação de constructo recorrendo à análise fatorial exploratória, envolvendo 548 técnicos e praticantes de pedestrianismo (n:182), canyoning (n:183) e escalada (n:183). A 1|ª versão do CMAR revelou-se pertinente, clara e objetiva (ICCespecilaistas: 0,85-0,90) e incluiu 51 itens (fatores de risco) agrupados em 3 dimensões. A versão final do CMAR incluiu 36 itens estruturados em 4 dimensões (humana, materiais e equipamentos, ambiental, e segurança e emergência), a serem avaliados numa matriz de risco de 5x5 pontos (probabilidade x consequência), que resulta num nível de risco estratificado correspondente a uma recomendação, ação e tratamento (KMOmodelo: 0,816; variância justificada modelo: 56,4%). O CMAR evidenciou valores bons de confiabilidade e bons a aceitáveis de validação de construto, viabilizando a sua utilização na análise e gestão de risco nos desportos de montanha pedestrianismo, canyoning e escalada.

Introduction The Royal College of Pathologists of Australasia Quality Assurance Programs (RCPAQAP) Key Incident Monitoring and Management Systems (KIMMS) program has found that some existing Quality Indicators are too broad or not well defined. The risk matrix in use does not allow changes in incident Detection or Probability. In 2020, a review was performed: what issues should KIMMS include as Key Incidents and how could risk measurement be improved? Materials and methods Twenty-seven networked and stand-alone laboratories enrolled in KIMMS during 2020 were surveyed on 45 current and new indicators of risk in the total testing process. They were asked which indicators they considered were significant in causing patient harm. Existing risk matrices in use by members of the KIMMS Advisory Committee laboratories were reviewed regarding their size or structure (3x3 or 5x5) and the descriptions of consequences and probability. Results Thirteen participants indicated 21 indicators should be monitored, and the KIMMS Advisory committee added a further 13 (11 from the remaining 24 and 2 new). Of the five risk matrices reviewed, all consistently used a 5x5 matrix to estimate Consequences vs Probability of harm. The KIMMS advisory committee added a third parameter to the calculation of Risk, Detectability. Conclusion All 34 pre- and post- indicators should be monitored, covering all aspects of the total testing cycle other than analytical. The risk measurement can be improved by introducing a 5x5 risk matrix to evaluate harm (consequences x probability) and then evaluating risk by adding detectability; risk equals harm x detectability.

The glass manufacturing includes operations, such as batch forming using raw materials, melting, forming, annealing, quality check and package. Due to risky processes in glass manufacturing, significant health hazards for workers are present in the glass industry. Risk assessment is effective way to prevent accidents and protect workers from serious accidents during glass manufacturing. To assess health hazards associated with glass manufacturing, in this study Risk Matrix and The Technique for Order Preferences by Similarity to an Ideal Solution (TOPSIS) method are integrated under Interval-Valued Intuitionistic Fuzzy (IVIF) environment to prioritize risk factors and suggest required preventive and protective measures. Suggested preventive and protective measures provide technical, economic and environmental challenges for glass manufacturing firms. Once the importance weight of risk parameters in Risk Matrix’ are determined, the risk factors are assessed by performing IVIF-TOPSIS method during glass manufacturing. In order to verify the validity and stability of the proposed risk assessment model, sensitivity and comparative analysis are accomplished at the end of the study.

PT. XYZ is a company engaged in construction and manufacturing. The company focuses on paving production. Where there is a problem with paving printing machines in carrying out production, there are many productions that do not reach the target or the production process does not run optimally. The purpose of this study is to find out the performance of paving molding machines, especially in the components that most often experience damage to paving molding machines. Based on historical data on machine damage, a paving molding machine is a machine that has had the highest frequency of damage for 5 years, because this machine operates continuously to meet production targets and paving demand from customers. Reliability and Risk Centered Maintenance (RRCM) methods to be used to analyze a historical engine malfunction. The results obtained in the form of 7 proposed maintenance tasks are 2 scheduled on-condition tasks, 3 scheduled restoration tasks, 2 scheduled waste tasks. In addition, the average maintenance time interval for pillow blocks is 7 months, iron molding is 11 months, and chain is 4 months. The total cost of proposed maintenance is lower than IDR 52,938,750 or 41.7% compared to the existing maintenance cost of PT. XYZ. The development of this research is to increase the application of RCM as a basis for maintaining the balance between preventive maintenance and corrective maintenance while optimizing maintenance costs to improve engine reliability by considering risk factors as an analysis.

ilustraciones Hemos desarrollado un software que utiliza conceptos de redes complejas y el método de la matriz de riesgo para realizar análisis de riesgo en servicios de radioterapia. Además de medir la importancia de las barreras y proyectar la implementación de barreras faltantes según su robustez, el software permite el estudio de la dinámica en servicios o modelos sometidos a fallas y ataques en las barreras. Para definir la red de sucesos y barreras, utilizamos una red bipartita y pesada que consta de un grupo de nodos que representan los sucesos iniciadores y otro que representa las barreras, cuyas conexiones son ponderadas por la robustez de las barreras. Se crearon estas redes para los modelos de errores y fallos potenciales (FMEA) en radioterapia convencional (MARR) y con técnicas avanzadas (MARRTA), y se utilizó un algoritmo para determinar los riesgos a partir de estas redes. La definición de la red permitió estudiar la dinámica de los modelos y servicios reales cuando se someten a fallas o ataques en sus barreras, definiendo cuatro fases según la fracción de barreras que fallan o se atacan, donde cada fase corresponde al dominio global de un nivel de riesgo específico. Durante la implementación del software, se obtuvieron análisis de riesgo de los modelos MARR y MARRTA, y se comprobó que los resultados obtenidos coinciden con los reportados. También realizamos el análisis de un caso hipotético donde el nivel base de frecuencia de los iniciadores del modelo MARRTA se aumentó en un nivel, encontrando que existe una fase de dominio del nivel de riesgo muy alto cuando se generan fallas en el 99% de las barreras. Además, estudiamos tres servicios reales identificando que la implementación de barreras con una robustez normal resultó en una mayor disminución de riesgos en comparación con la implementación de las barreras con mayor robustez. (Texto tomado de la fuente) We have developed a software that uses complex network concepts and the risk matrix method to perform risk analysis in radiotherapy services. In addition to measuring the importance of barriers and projecting the implementation of missing barriers according to their robustness, the software allows the study of dynamics in services or models subjected to failures and attacks on barriers. To define the event and barrier network, we use a weighted bipartite network consisting of a group of nodes representing initiating events and another representing barriers, whose connections are weighted by the robustness of the barriers. We create these networks for potential error and failure modes (FMEA) in conventional radiotherapy (MARR) and with advanced techniques (MARRTA), and then use an algorithm to determine risks from these networks. The network definition allowed us to study the dynamics of real services and models when subjected to failures or attacks on their barriers, defining four phases according to the fraction of barriers that fail or are attacked, where each phase corresponds to the global domain of a specific risk level. During the software implementation, we obtained risk analyses of the MARR and MARRTA models, verifying that we obtain the same results as reported. We also conducted the analysis of a hypothetical case where the base frequency level of the MARRTA model initiators was increased by one level, finding that there is a phase of very high-risk level dominance when failures occur in 99% of the barriers. Additionally, we studied three real services identifying that the implementation of barriers with normal robustness resulted in a greater decrease in risks compared to the implementation of barriers with higher robustness. Este trabajo está enfocado en analizar la percepción de la paciente gestante sobre la atención de medicina occidental vs la medicina ancestral en la IPS intercultural indígena de los cabildos del gran Cumbal Panan y Mayasquer. Metodológicamente se basa en un enfoque cualitativo por ser un estudio de caso, desde una lógica o método inductivo en donde se recolecto información suministrada por el referente de seguridad del paciente y calidad de atención de la institución; para así mismo generar la aplicación de herramientas gerenciales como la Matriz DOFA, plan de control y el modelo diagnóstico de gestión clínica; se aplicó una encuesta a la población gestante que asistió al servicio de consulta de medicina occidental y ancestral, durante el periodo comprendido entre julio de 2022 a febrero del 2022 en donde se encuestaron 120 pacientes, posteriormente la información obtenida fue tabulada a través de Excel en términos de gráficas y porcentajes para un adecuado análisis de datos. Se obtuvo como resultado que la IPS intercultural indígena de los cabildos del gran Cumbal, Panan y Mayasquer le falta implementar un modelo de atención que articule e integre la medicina occidental y ancestral donde las dos no se contradicen sino complementan, por lo cual se propone la implementación de una matriz de riesgo institucional con el fin de que se pueda realizar un diagnóstico y futuros planes de mejoramiento en pro de garantizar un programa de seguridad en el paciente exitoso. This paper proposes a 3D (three dimensions) continuous risk matrix to assess the operational safety in inland container terminals (ICTs). In this paper, based on the operational features of ICTs and relevant literature, the risk factors (RFs) affecting the operational safety in ICTs are first investigated. A 3D continuous risk matrix based on a fuzzy AHP (Analytic Hierarchical Process) approach is then proposed to assess the operational safety in ICTs. Unlike the traditional risk matrix constructed by risk measures of likelihood and consequence, the proposed risk matrix considers one more risk measure: preventive-ability of an organization that can provide more accurate information for decision makers in risk assessments. Finally, the ICTs in Taiwan were empirically investigated to validate the model. Based on the results, theoretical and managerial implications are discussed.

El objetivo de este trabajo es destacar la importancia de las Asociaciones Publico privadas y el rol activo que juega la Entidad Estatal concedente en la adecuada distribución, administración y mitigación de riesgos en estos contratos, especialmente lo relacionado con el “Riesgo social”, toda vez que el mismo están relacionados con la imposibilidad de instalación y ubicación del peajes, contingencia que es relevante considerando que los recursos del recaudo de los peajes son una fuente fundamental de recursos para apalancar financieramente este tipo de contratos, por ello mediante la metodología cualitativa, con el estudio de casos, se brindan recomendaciones, para evitar la ocurrencia del riesgo social en la instalación de peajes en contractos de concesión vial bajo la modalidad APP, así mismo recomendamos a las Entidades Públicas repensar el proceso de prevención y administración del riesgo social, adaptando herramientas fundamentadas en la sostenibilidad de proyectos las cuales han sido utilizadas en matrices de riesgos de otras entidades como el Ministerio de Relaciones Exteriores (programa de implementación de sostenibilidad del plan fronteras) y la Veeduría Distrital (Programa de Bogotá Como Vamos). Launched in June 2015, the Seirich software is a tool that helps companies to assess their chemical risks, informs them of their regulatory obligations and contributes in setting up a prevention action plan. In order to follow the regulatory updates, to provide ergonomic evolutions and to add new functionalities, several modifications have been made in different versions of Seirich. The aim of this project is to improve the chemical risk assessment algorithms of Seirich. The research approach is based on 3 main phases: (1) constitution of a matrix of work situations associated to a chemical risk value assessed by experts. Work situations with their descriptive determinants were collected from reports written by professionals from public French health insurance. (2) Improvement proposals based on the comparison of Seirich risk score with the experts’ assessment; the examination of other risk/exposure assessment models and the feedback from Seirich users. (3) evaluation of the impact of each improvement modification on the risk assessment was evaluated using nearly 10,000 real work situations provided by companies In the first phase, Eighty-eight French work situations were collected and assessed. Various types of companies are represented by these work situations. The most represented activity sectors were: building and construction (16 %), automotive (11 %), metallurgy (11 %), and printing (7 %). The experts have given risk scores comprised between 40 and 60 to 50% of the work situations. The uncertainty levels rang from 11% to 33% with an average of 20%. During the second phase, a list of 14 limits was elaborated. Criteria of relevance, feasibility and strategy were applied to identify those to be treated. Seven limits were selected and a solution was proposed. Finally, The results of the impact analysis show that the risk score has changed for 35% of the work situations with a modification in the risk level for 6% of them. Afin de suivre les mises à jour réglementaires, d'apporter des évolutions ergonomiques et d'ajouter de nouvelles fonctionnalités, plusieurs modifications ont été apportées dans différentes versions de Seirich. L'objectif de ce projet est d'améliorer les algorithmes d'évaluation des risques chimiques de Seirich. La démarche de recherche repose sur 3 phases principales : (1) la constitution d'une matrice des situations de travail associées à une valeur de risque chimique établi par expertise selon la méthode de Delphes. Chaque situation de travail a été ensuite évaluée et un score de risque chimique a été défini par 21 experts 2) établissement de propositions d'amélioration basées sur la comparaison du score de risque Seirich avec ceux des experts ; l'examen d'autres modèles d'évaluation des risques/expositions et le retour d'expérience des utilisateurs de Seirich.(3) Evaluation de l'impact de chaque amélioration sur l'évaluation des risques a été évalué à partir de près de 10 000 situations réelles de travail fournies par les entreprises. En terme de résultats, quatre-vingt-huit situations de travail ont été recueillies et évaluées. Différents types d'entreprises sont représentés par ces situations de travail. Les secteurs d'activité les plus représentés étaient: le BTP (16 %), l'automobile (11 %), la métallurgie (11 %) et l'imprimerie (7 %). Les experts ont donné des scores de risque compris entre 40 et 60 pour 50 % des situations de travail. Les niveaux d'incertitude variaient de 11 % à 33 % avec une moyenne de 20 %. Au cours de la deuxième phase, une liste de 14 limites de Seirich a été élaborée. Des critères de pertinence, de faisabilité et de stratégie ont été appliqués pour identifier les limites à traiter. Sept ont été sélectionnés et une solution basée pour chacune d’elles. Enfin, l'analyse d'impact de l'ensemble des propositions montre que le score de risque a évolué pour 35% des situations de travail avec une modification du niveau de risque pour 6% d'entre elles. Ces améliorations proposées et leur étude d'impact ont été présentées au comité de pilotage du logiciel qui a statué sur leur application. 5/7 améliorations ont finalement été acceptée et les deux restantes sont toujours en cours de discussion.

Work accidents are things that must be avoided, so that's not inflicted loss on the company and the workers. However, several workers still not pay attention to occupational health and safety regulations and in the work area still found several potential hazards. The research is conducted at PT Pindo Deli Pulp and Paper Mills Indonesia in Karawang Regency and focuses on preventive and corrective maintenance activities that prevent paper machine breakdown. The purpose of this study is to measure how big the potential hazard and impact by using Hazard and Operability Study (HAZOP) method. This method is a systematic study, which aims to find the factors that causing work accidents and find out how big the potential accidents and their impacts. The data is collected by distributing questionnaires to the employee. The results show that hazards caused by the work area and the workers themselves are 18% and 82%, respectively. Based on the severity, the number of extreme, high, medium and low risk hazards are 1, 8, 1 and 1, respectively.

Construction project of building frequently has risk. Often occurring risk often is job accident. Job accident may be one of causes inhibiting project job activity. According to the data of Social Health Organizing Agency for labor, the job accident rate was 130.926 in 2017. Application of a good Construction Safety Management System is expectable to minimize resulting risk probability. This study used method of Danger Identification and Opportunity Risk Assessment (IBPRP) and Job Safety Analysis (JSA) deriving from PURR Government Regulation Number 21/2019. This study aimed at finding IBPRP and JSA designs as well as analogy of problem identification and control variables of foundation job in the Construction Project of Learning Service Building (FISIP) of General Soedirman University of Purworejo. The results of IBPRP and JSA analysis indicated that risk identification and risk rate of foundation job in construction project of Learning Service Building (FISIP) of General Soedirman University of Purwokerto, the foundation job had 48 risks identified from 11 job sequences. All of the 49 identified risks were in low risk rate, where rate value of matrix 2 (two) and 14 risks had rate value of matrix 4 (four). Research and identification of a serum cryoglobulin is a current laboratory test. This work made it possible, thanks to the development of a risk matrix taking into account all pre-analytical, analytical and post-analytical phases, to stress the importance of technical proficiency and management of critical equipment . With this approach, the laboratory also checked homogeneity of results validation practices.

Este proyecto se llevó a cabo con el fin de desarrollar el Sistema de Gestión de Seguridad y Salud en el Trabajo en los talleres de panadería, expendio, asadero y reciclaje del establecimiento penitenciario de mediana seguridad carcelaria de Villa Hermosa en la ciudad de Cali, ya que se evidenció los diferentes peligros que afectan a los trabajadores dentro de los diferentes talleres. Fue necesario realizar una evaluación de los riesgos para conocer el verdadero panorama de lo que estaban expuestos los trabajadores, una vez se realizó la evaluación de cada riesgo identificado, todo esto a través de una lista de chequeo la matriz de riesgo dada en la GTC45. Y una vez desarrollado la evaluación inicial, la identificación del peligro y la valoración del riesgo, se elaboró el diseño del sistema, iniciando con la elaboración del plan de trabajo anual, posteriormente con la conformación del COPASST Y Comité de Convivencia. This project was carried out to develop the Occupational Health and Safety Management System in the bakery, vending, barbecue, and recycling workshops of the Medium Security Prison Establishment of Villa Hermosa in the city of Cali, since the different dangers that affect the workers within the different workshops were evidenced. It was necessary to carry out a risk assessment to know the true picture of what the workers were exposed to, once the evaluation of each identified risk was carried out, all this through a checklist and the risk matrix given in the GTC45. Software risk management is an important factor in ensuring software quality. Therefore, software risk assessment has become a significant and challenging research area. The aim of this study is to establish a data-driven software risk assessment model named DDERM. In the proposed model, experts’ risk assessments of probability and severity can be transformed into basic probability assignments (BPAs). Deng entropy was used to measure the uncertainty of the evaluation and to calculate the criteria weights given by experts. In addition, the adjusted BPAs were fused using the rules of Dempster–Shafer evidence theory (DST). Finally, a risk matrix was used to get the risk priority. A case application demonstrates the effectiveness of the proposed method. The proposed risk modeling framework is a novel approach that provides a rational assessment structure for imprecision in software risk and is applicable to solving similar risk management problems in other domains.

Summary Goals: With increased wildfires in recent years, winemakers now regularly need to interpret results from analyses for smoke related marker compounds to determine if smoke intrusion has impacted their wines. The goal of this study was to examine naturally occurring baseline levels of smoke exposure marker compounds in un-oaked Pinot noir to enable winemakers to better understand smoke exposure results. This study also sought to understand how baseline concentrations of smoke marker compounds change from year to year. Key Findings: Pinot noir wines from California and Oregon had detectable baseline levels of free and bound marker compounds, including guaiacol, 4-methylguaiacol, 4-ethylphenol, and, m-, p-, and o-cresol. In 2019, the baseline free guaiacol concentration ranged from 1.2–2.3 µg/L, while total (free and bound) guaiacol ranged from 6.4–12.0 µg/L. The concentration of free guaiacol was greater in older vintages, while total guaiacol largely did not change from year to year. The ratio of free to total guaiacol approached 1:2 after 5 years, suggesting baseline guaiacol may approach an equal ratio of free to bound over time. Impact and Significance: While baseline levels identified here were low, winemakers should become familiar with the baseline concentrations in their wines to better understand risk during smoke impacted vintages. With increased analysis of non-smoke impacted, baseline samples, it may be possible to create a risk matrix by wine variety for smoke exposure. Additionally, as wines age, free volatile smoke marker compounds may increase due to normal changes to baseline compounds and may not always represent smoke-related glycosides releasing free volatiles.

PurposeThis paper aims to develop a process for prioritizing project risks that integrates the decision-maker's risk attitude, uncertainty about risks both in terms of the associated probability and impact ratings, and correlations across risk assessments.Design/methodology/approachThis paper adopts a Monte Carlo Simulation-based approach to capture the uncertainty associated with project risks. Risks are prioritized based on their relative expected utility values. The proposed process is operationalized through a real application in the construction industry.FindingsThe proposed process helped in identifying low-probability, high-impact risks that were overlooked in the conventional risk matrix-based prioritization scheme. While considering the expected risk exposure of individual risks, none of the risks were located in the high-risk exposure zone; however, the proposed Monte Carlo Simulation-based approach revealed risks with a high probability of occurrence in the high-risk exposure zone. Using the expected utility-based approach alone in prioritizing risks may lead to ignoring few critical risks, which can only be captured through a rigorous simulation-based approach.Originality/valueMonte Carlo Simulation has been used to aggregate the risk matrix-based data and disaggregate and map the resulting risk profiles with underlying distributions. The proposed process supported risk prioritization based on the decision-maker's risk attitude and identified low-probability, high-impact risks and high-probability, high-impact risks.

The fuzziness and randomness are significant epistemic uncertainty within the qualitative categorization of the two critical factors, the frequency and severity, which are not fully considered in the traditional risk matrix. This paper mainly proposes a cloud risk matrix method for the risk assessment of process safety considering the epistemic uncertainty in expert elicitation. The cloud model is employed to provide a mathematical expression for the fuzziness and randomness in the linguistic variables by its two numerical characteristics entropy En and hyperentropy He . An adjusted Mamdani inference algorithm is constructed for the determination of an integrated risk cloud given the value of input variables. And the centroid method is improved to be adapted to the calculation of risk index from the enormous cloud droplets in the integrated risk cloud. A case study of risk assessment for distillation column unit is performed to illustrate the process of cloud risk matrix in detail, and the validity and rationality are verified by contrast to the results from the fuzzy risk matrix. Results indicate the proposed method can handle the randomness of qualitative concepts, which is more suitable to the practical condition. Moreover, the effect of the hyperentropy He on the randomness of risk index is also investigated and discussed for the reference to parameter selection. The proposed cloud risk matrix can provide an effective risk inference tool in a wide range of engineering fields.

The aim of the article is to identify the main components of government overall liabilities based on the Fiscal Risk Matrix classification introduced by the World Bank in 1999, and to estimate the amount and structure of these liabilities in European Union countries (EU Fiscal Risk Matrix). The climate liabilities definition and methodology included in the EU Fiscal Risk Matrix is also a novelty of the research. The study covered EU member states in the period 2018–2019, taking into account available data from the Eurostat database. On this basis, the EU Fiscal Risk Matrix was developed with the estimated structure of the burden of government liabilities for individual countries and the EU as a whole. The article used statistical and comparative analysis. The major conclusion of our research involves the proposal to implement a unified European methodology of government overall liabilities classification based on the EU Fiscal Risk Matrix to assess the fiscal debt burden and transparency of fiscal policy.

The novel coronavirus or COVID-19 has extended its spread across the globe, and most of the countries have reported infections. In what started as a first case single reported in Egypt has now magnified and close to about 355000+ infections have been reported across the African region as on 26-6-2020. The transmission trajectory of COVID-19 across the globe remains a mystery, and much remains to be learned. The fear of spread among nations is a cause for concern, especially among African countries with weaker governance, high poverty levels, weak health systems etc. The density of population in urban areas could be a trigger factor and could be devastating. The management and control of COVID-19 are critical to check the spread, and most of it is reliant on the health facilities in countries to carryout repeated tests. This paper aims to analyses the various parameters and tabulates a risk matrix and places it analysis using SPSS based on the available data. Due to the limitation of assessors’ knowledge and the uncertainty of risks, risk assessment data reasonably are given in the form of linguistic terms, safety risk assessment in petrochemical industry is often a multi-criterion and multi-expert information fusion based on linguistic terms(MCMEIF-LT) problem. A novel model dealing with the MCMEIF-LT problem is presented in this paper. Firstly, the individual linguistic assessment distributions are fused to collective distributions and multiple criteria are fused to a comprehensive criterion. In the fusion process, the objective weights of assessment experts are calculated with using the credibilities of assessment data and the attitudes of decision makers are considered. Secondly, a Fuzzy Number Weighted Ordered Weighted Aggregation(FN-WOWA) operator which can transform a fuzzy number into a crisp value is proposed. In the FN-WOWA operator, the utility function can incorporate the assessors’ loss-based risk attitudes and the membership function can reflect the importance of the values in the integrated fuzzy number. Based on crisp values, a risk matrix is constructed. Finally, a real application is demonstrated to show the flexibility and practicality of the MCMEIF-LT model.

Object and purpose of research. This paper discusses forecasting methods for technical deterioration risks of complex marine technology. The purpose of the study was to formulate the proposals on development and induction of a system for non-stop monitoring, data acquisition and statistical analysis of key performance parameters of ship equipment so as to highlight its current condition and trends, as required by Russian Maritime Register of Shipping. Materials and methods. Statistical simulation was used to study the method of quantile regression analysis for target performance parameters of ship equipment, as well as applicability of this method to assessment of technical deterioration risks based on risk matrices and standard reliability schemes. Main results. Quantile regression analysis, risk matrices of target parameters and structural schemes of ship equipment reliability were used to suggest and justify the method, as well as describe the software based on it and intended to perform technical diagnosis and forecasting of ship equipment with calculation of corresponding risks. Conclusion. The method suggested in this paper to forecast technical deterioration risks of sophisticated marine technology complies with the requirements and recommendations of ITTC, IMO and Russian Maritime Register of Shipping.

Distinguishing and diminishing risks in today’s projects are crucial for project success. Almost every project is facing several risks throughout the project timeline. Construction projects in Finland are also facing project risks due to the complexity of the project. To minimize the impact of risks, an effective risk management approach must be incorporated into every project which also includes the effectiveness and measurement of its performance. Managing the risks is an important job but measuring the RM performance is crucial. Thus, the objective of this study is to analyze the risk management (RM) and risk management performance measurement (RMPM) through an indepth empirical analysis of two complex construction projects of Finland. To achieve the objective, a qualitative case study is followed by the authors of this article to identify the RM processes, major and minor risks of the projects, RM strategies to mitigate them and RM performance measurement strategies. Overall, this article provides a comparative analysis of RM and RMPM for construction projects and it can be used as a basis for further research into RM perspective in complex construction projects.

This paper develops a risk index based on an indicator of risk assessment in terms of coastal activity location and accident type. The risk index is derived from a formula which adds the consequence of failure to a vulnerability value, then subtracts the mitigation value. Specifically, the consequence of failure is the number of casualties in coastal activity locations. An indicator of vulnerability refers to coastal environment elements and social elements. A pointer of mitigation includes managerial and organizational elements that indicate the capabilities of coastal activities. A risk rating of coastal activity location is found from a risk matrix consisting of the accident location and type. The purpose of this study is to prevent accidents at coastal activity locations by allowing the Coastal police guard to monitor effectively and inform visitors of potential risks.

Não recebi financiamento The public service of national infrastructure, from the mid-1990s, began to be provided jointly to the private sector through contracts through the bidding law. From 1995 onwards, with the enactment of the concession law, the State sought private partners to carry out these services, assuming all the risk of the enterprise and public works within a certain period. The objective of this research is to identify and compare the allocation of contractual risks in a risk matrix in the Federal and Paulista Highway Concessions from 2009 to 2020. This research investigated 16 concession contracts, 8 federal and 8 state, corresponding to the period from 2009 to 2020. 2020, with textual analysis of risk allocations between the Grantor and Concessionaire, in order to describe relationships of differences and similarities between contracts. During these periods, the dimensions of the highways, contracted values, economics and political aspects of the federal and state governments were disregarded. Contracts were consulted by the portals of the federal and state transport regulatory agencies, and the contractual clauses were listed in the construction of a risk matrix that was paired and compared by reading and analyzing the convergences and divergences regarding the normative isomorphic aspect and based on the differentiation of the highways in the State of São Paulo due to the quality of its highways, which is among the 10 best in the country. The results found are a convergence of 57% of the contractual clauses, with occasional divergences in the amount of 19% in federal contracts with exclusive clauses; and 17% in the states. It was observed that the contracts seek a trend in relation to contracts in São Paulo, but some aspects diverge in relation to the quality of services. The contribution of this research lies in the practical application of the concepts of the new public governance such as: networking, new actors, accountability, accountability, transparency and accountability. More specifically, the risk matrix as a management tool can contribute to the effective use of resources, mapping contractual risks. This research highlights the convergences in the observation of risks of contracts drawn up by the federation and the State of São Paulo, allowing other entities of the federation to evaluate them in similar contracts. A partir de 1995 com a edição da lei de concessões houve a procura pelo Estado de parceiros privados para execução desses serviços assumindo todo o risco do empreendimento e obra pública em determinado prazo. O objetivo desta pesquisa é identificar e comparar a alocação dos riscos contratuais em matriz de riscos nas Concessões Rodoviárias Federais e Paulistas no período de 2009 a 2020. Esta pesquisa investigou 16 contratos de concessão, 8 federais e 8 estaduais, correspondentes ao período de 2009 a 2020, com análise textual das alocações de riscos entre o Poder Concedente e Concessionária, a fim de descrever relações de diferenças e semelhanças entre contratos. Desconsiderou-se nestes períodos, dimensões das rodovias, valores contratados, economia e vertentes políticas dos governos federal e estadual. Foram consultados contratos pelos portais das agências reguladoras de transportes federal e estadual, e elencadas as cláusulas contratuais na construção de matriz de riscos que foi pareada e comparada por leitura e análise das convergências e divergências quanto ao aspecto isomórfico normativo e por base a diferenciação das rodovias do Estado de São Paulo devido à qualidade de suas rodovias constando entre as 10 melhores do país. Os resultados encontrados são de convergência de 57% das cláusulas contratuais, havendo divergências pontuais na monta de 19% nos contratos federais com cláusulas exclusivas; e de 17% nos estaduais. Observou-se que os contratos buscam uma tendência em relação aos contratos paulistas, porém alguns aspectos divergem em relação à qualidade dos serviços. A contribuição desta pesquisa está na aplicação prática dos conceitos da nova governança pública como: trabalho em rede, novos atores, accountability, prestação de contas, transparência e responsabilização. Mais especificamente, a matriz de riscos como ferramenta de gestão pode contribuir para o uso eficaz dos recursos, mapeando os riscos contratuais. Nesta pesquisa são destacadas as convergências na observação de riscos dos contratos elaborados pela federação e pelo Estado de São Paulo, permitindo que outros entes da federação os avaliem em contratações semelhantes.

La industria de motocicletas en Colombia es una de las más grandes en el país con ventas superiores a 52.000 motos al año, con esto se tiene en cuenta el SGA (Sistema Globalmente Armonizado) ya que se basa en etiquetar y clasificar las sustancias químicas de acuerdo con los peligros físicos para la salud ya que de esta manera puede facilitar el uso de las sustancias químicas minimizando el riesgo. Se plantea este problema que por concernientes a los mantenimientos se usen sustancias químicas para el trabajo, lo cual no se controla regularmente y estas generan irritación y otras falencias físicas, la finalidad del proyecto es contrarrestar bajo la normatividad vigente la manipulación adecuada de los agentes químicos, y su respectiva eliminación. Necesariamente se debe contar con los estándares concretos el cual concientice a los empleadores de los derechos y obligaciones con el empleado, con lo anterior dicho el objetivo es evaluar y reducir este riesgo, específicamente reconocer la zona, describir la exposición e implementar una matriz de riesgos. La metodología de carácter cualitativa descriptiva explica las características y resultados del proyecto teniendo en cuenta todos los procedimientos y variables que se plantean dentro de la metodología, se analizaran los resultados las gráficas y otros procesos, como resultado se reduce los riesgos a los que están expuestos los trabajadores The motorcycle industry in Colombia is one of the largest in the country with sales of more than 52,000 motorcycles per year; this takes into account the GHS (Globally Harmonized System) as it is based on labeling and classifying chemicals according to the physical health hazards as this can facilitate the use of chemicals by minimizing the risk. This problem arises due to the use of chemical substances for maintenance work, which is not regularly controlled and generates irritation and other physical failures. The purpose of the project is to counteract under the current regulations the proper handling of chemical agents and their respective elimination. It is necessary to have concrete standards that make employers aware of their rights and obligations with the employee, with the above mentioned, the objective is to evaluate and reduce this risk, specifically to recognize the area, describe the exposure and implement a risk matrix. For the investigation it was of vital importance to know what types of investigations were carried out in this guild, the characteristics of these investigations were of greater importance to know if there was a control over these substances, faculties such as engineering and other universities such as the nueva granada and the UNIMINUTO allow to evaluate the diverse similarities and substances that surround us. The referential framework is the research process taking into account the legal importance and the characteristics of the chemical substances, the adequate handling of them and other processes. The analytical descriptive methodology will explain the characteristics and results of the project taking into account all the procedures and variables that are raised within the methodology, the results, graphs and other processes will be analyzed.

Risk matrices are a common way to communicate the likelihood and potential impacts of a variety of risks. Until now, there has been little empirical work on their effectiveness in supporting understanding and decision making, and on how different design choices affect these. In this pair of online experiments (total n = 2699), we show that risk matrices are not always superior to text for the presentation of risk information, and that a nonlinear/geometric labeling scheme helps matrix comprehension (when the likelihood/impact scales are nonlinear). To a lesser degree, results suggested that changing the shape of the matrix so that cells increase in size nonlinearly facilitates comprehension as compared to text alone, and that comprehension might be enhanced by integrating further details about the likelihood and impact onto the axes of the matrix rather than putting them in a separate key. These changes did not affect participants' preference for reducing impact over reducing likelihood when making decisions about risk mitigation. We recommend that designers of risk matrices consider these changes to facilitate better understanding of relationships among risks.

With the rapidly increasing demands of e-government systems in smart cities, a myriad of challenges and issues are required to be addressed. Among them, security is one of the prime concerns. To this end, we analyze different e-government systems and find that an e-government system built with container-based technology is endowed with many features. In addition, overhauling the architecture of container-technology-driven e-government systems, we observe that securing an e-government system demands quantifying security issues (vulnerabilities, threats, attacks, and risks) and the related countermeasures. Notably, we find that the Attack Tree and Attack–Defense Tree methods are state-of-the-art approaches in these aspects. Consequently, in this paper, we work on quantifying the security attributes, measures, and metrics of an e-government system using Attack Trees and Attack–Defense Trees—in this context, we build a working prototype of an e-government system aligned with the United Kingdom (UK) government portal, which is in line with our research scope. In particular, we propose a novel measure to quantify the probability of attack success using a risk matrix and normal distribution. The probabilistic analysis distinguishes the attack and defense levels more intuitively in e-government systems. Moreover, it infers the importance of enhancing security in e-government systems. In particular, the analysis shows that an e-government system is fairly unsafe with a 99% probability of being subject to attacks, and even with a defense mechanism, the probability of attack lies around 97%, which directs us to pay close attention to e-government security. In sum, our implications can serve as a benchmark for evaluation for governments to determine the next steps in consolidating e-government system security.

The PRISM (partial risk map) methodology is a novel risk assessment method developed as the combination of the failure mode and effect analysis and risk matrix risk assessment methods. Based on the concept of partial risks, three different aggregation functions are presented for assessing incident risks. Since the different aggregation functions give different properties to the obtained PRISM numbers and threshold surfaces (convex, concave, linear), the description of these properties is carried out. Similarity analyses based on the sum of ranking differences (SRD) method and rank correlation are performed and robustness tests are applied related to the changes of the assessment scale lengths. The PRISM method provides a solution for the systematically criticized problem of the FMEA, i.e., it is not able to deal with hidden risks behind the aggregated RPN number, while the method results in an expressive tool for risk management. Applying new aggregation functions, proactive assessment can be executed, and predictions can be given related to the incidents based on the nature of their hidden risk. The method can be suggested for safety science environments where human safety, environmental protection, sustainable production, etc., are highly required.

Face failure is a typical phenomenon in longwall coal mines that can have a wide range of consequences. Face failure, which includes wall spall and roof collapse occurrences, is a hazardous virus that, if not managed, spreads fast across all stages of coal mining and has the potential to disable the mine. Based on this research, face failure may have a detrimental influence on technical, environmental, community, safety, and economic concerns, and its negative effects will leave an unfavourable legacy for the future. As a result, these impacts can be mitigated by effective management and risk management approaches. The quantitative and qualitative face failure risk model provided in this study has a considerable potential as a suitable tool for decision makers to analyse failure risk. Face failure-related high-risk variables can be discovered using this approach, which also makes comparing various mines easier from a face failure aspect. For validation, the model was evaluated in the Parvadeh, Negin and Pabedana coal mines. The study’s findings revealed that Parvadeh’s face failure risk factor was 5058, indicating a high risk in this mine due to mechanized mining. HAZID risk analysis is the most effective tool to reduce the risks of serious consequences in the operation and development of new projects for the export of hydrocarbons. The purpose of the work is a detailed study of the algorithm for conducting the HAZID method, determining the goals and objectives of its implementation. Subsequently, for each risk scenario identified, a risk ranking is developed using a risk matrix, including frequency and severity rankings.

Launched in June 2015, the Seirich software is a tool that helps companies to assess their chemical risks, informs them of their regulatory obligations and contributes in setting up a prevention action plan. In order to follow the regulatory updates, to provide ergonomic evolutions and to add new functionalities, several modifications have been made in different versions of Seirich. The aim of this project is to improve the chemical risk assessment algorithms of Seirich. The research approach is based on 3 main phases: (1) constitution of a matrix of work situations associated to a chemical risk value assessed by experts. Work situations with their descriptive determinants were collected from reports written by professionals from public French health insurance. (2) Improvement proposals based on the comparison of Seirich risk score with the experts’ assessment; the examination of other risk/exposure assessment models and the feedback from Seirich users. (3) evaluation of the impact of each improvement modification on the risk assessment was evaluated using nearly 10,000 real work situations provided by companies In the first phase, Eighty-eight French work situations were collected and assessed. Various types of companies are represented by these work situations. The most represented activity sectors were: building and construction (16 %), automotive (11 %), metallurgy (11 %), and printing (7 %). The experts have given risk scores comprised between 40 and 60 to 50% of the work situations. The uncertainty levels rang from 11% to 33% with an average of 20%. During the second phase, a list of 14 limits was elaborated. Criteria of relevance, feasibility and strategy were applied to identify those to be treated. Les secteurs d'activité les plus représentés étaient: le BTP (16 %), l'automobile (11 %), la métallurgie (11 %) et l'imprimerie (7 %). Les experts ont donné des scores de risque compris entre 40 et 60 pour 50 % des situations de travail. Les niveaux d'incertitude variaient de 11 % à 33 % avec une moyenne de 20 %. Au cours de la deuxième phase, une liste de 14 limites de Seirich a été élaborée. Des critères de pertinence, de faisabilité et de stratégie ont été appliqués pour identifier les limites à traiter. Sept ont été sélectionnés et une solution basée pour chacune d’elles. To this end, the URRAN system has been created, operated and developed in the Russian railway transport -a system for managing the reliability, safety, and resources of transport facilities based on risk assessment. Within the framework of this system, the tasks of operational collection, processing and analysis of the current state of reliability and safety of transport facilities, the activities of structural units are solved on the basis of risk assessment of maintenance management, assignments of major repairs, modernization and modification of transport facilities. The reliability and safety of transport facilities are managed within the framework of Big Data using artificial intelligence methods. Fire safety management is carried out using an automated fire risk management system, which is part of the URRAN system. It allows, based on the results of the fire risk forecast, to make a decision on the need for repair, replacement or maintenance of transport facilities and their fire safety system.

Road network type and its surroundings play an important role in road traffic accidents. This heterogeneity is due to the diverse nature of the driving population, traffic characteristics, weather conditions, roadside features, and design considerations. Previous research investigation has found different factors associated with accidents, but there is a lack of research in the area-wise categorization of risk factors. In this prospect, the present study endeavors to find factors responsible for the accident in various zones in Nagpur City. And to identify the pattern and impact caused by the individual characteristics features on multiple zones. Overall, three types of zones have been selected based on accident risk level viz. high-risk zone, medium risk zone, and a low-risk zone: and represented as Zone-I, Zone-II, and Zone-III. The process of zone formation is performed in ArcGIS software using the kernel density estimator function. The accident data is obtained from the Nagpur traffic police accident database from 2016 to 2018. The fault tree analysis gave the risk factor. And risk matrix was employed to assess the risk level associated with each risk factor in a specific zone. The result shows that zone-I has more probability of a fatal crash at night-time than at daytime; Zone-II has more injury crash probability at day time. Zone-III is a low crash occurrence area but has a slightly more fatal crash probability at night-time. The average possibility of a fatal crash at night-time in zone-I is 2.97 times compared to that in the daytime.

The impacts of climate change are anticipated to become stronger in the future, leading to higher costs and more severe accidents in the oil industry’s facilities and surrounding communities. Motivated by this, the main objective of this paper is to develop, for the oil industry, a risk assessment methodology that considers future climate projections. In the context of an action research effort, carried out in a refinery in Greece, we adapted the organization’s extant risk management approach based on the Risk Assessment Matrix (RAM) and suggested a risk quantification process that incorporates future climate projections. The Climate Risk Assessment Matrix (CRAM) was developed to be used to assess the exposure of the facility’s assets, including human resources, to future climate risks. To evaluate CRAM, a comparison with RAM for the specific organization for the period 1980–2004 was made. Next, the application of CRAM for the period 2025–2049 indicated that, even though the resilience of the operations of the company to extreme conditions seems adequate at present, increased attention should be paid in the future to the resilience of refinery processes, the cooling system, and human resources. Beyond the specific case, the paper provides lessons for similar organizations and infrastructures located elsewhere.
