Abstract

Background: Delirium is thought to be common across various settings of care; however, still little research has been conducted in rehabilitation. Aim: We investigated the prevalence of delirium, its features and motor subtypes in older patients admitted to rehabilitation facilities during the three editions of the “Delirium Day project”. Methods: We conducted a cross-sectional study in which 1237 older patients (age ≥ 65 years old) admitted to 50 Italian rehabilitation wards during the three editions of the “Delirium Day project” (2015 to 2017) were included. Delirium was evaluated through the 4AT and its motor subtype with the Delirium Motor Subtype Scale. Results: Delirium was detected in 226 patients (18%), and the most recurrent motor subtype was mixed (37%), followed by hypoactive (26%), hyperactive (21%) and non-motor one (16%). In a multivariate Poisson regression model with robust variance, factors associated with delirium were: disability in basic (PR 1.48, 95%CI: 1.17–1.9, p value 0.001) and instrumental activities of daily living (PR 1.58, 95%CI: 1.08–2.32, p value 0.018), dementia (PR 2.10, 95%CI: 1.62–2.73, p value < 0.0001), typical antipsychotics (PR 1.47, 95%CI: 1.10–1.95, p value 0.008), antidepressants other than selective serotonin reuptake inhibitors (PR 1.3, 95%CI: 1.02–1.66, p value 0.035), and physical restraints (PR 2.37, 95%CI: 1.68–3.36, p value < 0.0001). Conclusion: This multicenter study reports that 2 out 10 patients admitted to rehabilitations had delirium on the index day. Mixed delirium was the most prevalent subtype. Delirium was associated with unmodifiable (dementia, disability) and modifiable (physical restraints, medications) factors. Identification of these factors should prompt specific interventions aimed to prevent or mitigate delirium.

Abstract

Introduction Delirium and sarcopenia are common, although underdiagnosed, geriatric syndromes. Several pathological mechanisms can link delirium and low skeletal muscle mass, but few studies have investigated their association. We aimed to investigate (1) the association between delirium and low skeletal muscle mass and (2) the possible role of calf circumference mass in finding cases with delirium. Methods The analyses were conducted employing the cross-sectional “Delirium Day” initiative, on patient 65 years and older admitted to acute hospital medical wards, emergency departments, rehabilitation wards, nursing homes and hospices in Italy in 2017. Delirium was diagnosed as a 4 + score at the 4-AT scale. Low skeletal muscle mass was operationally defined as calf circumference ≤ 34 cm in males and ≤ 33 cm in females. Logistic regression models were used to investigate the association between low skeletal muscle mass and delirium. The discriminative ability of calf circumference was evaluated using non-parametric ROC analyses. Results A sample of 1675 patients was analyzed. In total, 73.6% of participants had low skeletal muscle mass and 24.1% exhibited delirium. Low skeletal muscle mass and delirium showed an independent association (OR: 1.50; 95% CI 1.09–2.08). In the subsample of patients without a diagnosis of dementia, the inclusion of calf circumference in a model based on age and sex significantly improved its discriminative accuracy [area under the curve (AUC) 0.69 vs 0.57, p Discussion and conclusion Low muscle mass is independently associated with delirium. In patients without a previous diagnosis of dementia, calf circumference may help to better identify those who develop delirium.

Objectives Delirium is commonly seen in older adults with multimorbidity, during a hospitalization, resulting from the interplay between predisposing factors such as advanced age, frailty, and dementia, and a series of precipitating factors. The association between delirium and specific multimorbidity is largely unexplored so far although of potential key relevance for targeted interventions. The aim of the study was to check for a potential association of multimorbidity with delirium in a large cohort of older patients hospitalized for an acute medical or surgical condition. Design This is a cross-sectional study nested in the 2017 Delirium Day project. Setting and Participants The study includes 1829 hospitalized patients (age: 81.8, SD: 5.5). Of them, 419 (22.9%) had delirium. Methods Sociodemographic and medical history were collected. The 4AT was used to assess the presence of delirium. The Charlson Comorbidity index was used to assess multimorbidity. Results The results identified neurosensorial multimorbidity as the most prevalent, including patients with dementia, cerebrovascular diseases, and sensory impairments. In light of the highest co-occurrence of 3 neurosensorial chronic conditions, we could hypothesize that a baseline altered brain functional and neural connectivity might determine the vulnerability signature for incipient overall system disruption in presence of acute insults. Conclusions and Implications Eventually, our findings moved a step forward in supporting the key importance of routine screening for sensory impairments and cognitive status of older patients for the highest risk of in-hospital delirium. In fact, preventive interventions could be particularly relevant and effective in preventing delirium in such vulnerable populations and might help refining this early diagnosis.

Abstract

Background although frailty and delirium are among the most frequent and burdensome geriatric syndromes, little is known about their association and impact on short-term mortality. Objective to examine, in hospitalized older persons, whether frailty is associated with delirium, and whether these two conditions, alone or in combination, affect these patients’ 30-day survival. Design observational study nested in the Delirium Day project, with 30-day follow-up. Setting acute medical wards (n = 118) and rehabilitation wards (n = 46) in Italy. Subjects a total of 2,065 individuals aged 65+ years hospitalized in acute medical (1,484 patients, 71.9%) or rehabilitation (581 patients, 28.1%) wards. Methods a 25-item Frailty Index (FI) was created. Delirium was assessed using the 4AT test. Vital status was ascertained at 30 days. Results overall, 469 (22.7%) patients experienced delirium on the index day and 82 (4.0%) died during follow-up. After adjustment for potential confounders, each FI score increase of 0.1 significantly increased the odds of delirium (odds ratio, OR: 1.66 [95% CI: 1.45–1.90]), with no difference between the acute (OR: 1.65 [95% CI: 1.41–1.93]) and rehabilitation ward patients (OR: 1.71 [95% CI: 1.27–2.30]). The risk of dying during follow-up also increased significantly for every FI increase of 0.1 in the overall population (OR: 1.65 [95% CI: 1.33–2.05]) and in the acute medical ward patients (OR: 1.61 [95% CI: 1.28–2.04]), but not in the rehabilitation patients. Delirium was not significantly associated with 30-day mortality in either hospital setting. Conclusions in hospitalized older patients, frailty is associated with delirium and with an increased risk of short-term mortality.

Abstract

Increased interest in the use of word embeddings, such as word representation, for biomedical named entity recognition (BioNER) has highlighted the need for evaluations that aid in selecting the best word embedding to be used. One common criterion for selecting a word embedding is the type of source from which it is generated; that is, general (e.g., Wikipedia, Common Crawl), or specific (e.g., biomedical literature). Using specific word embeddings for the BioNER task has been strongly recommended, considering that they have provided better coverage and semantic relationships among medical entities. To the best of our knowledge, most studies have focused on improving BioNER task performance by, on the one hand, combining several features extracted from the text (for instance, linguistic, morphological, character embedding, and word embedding itself) and, on the other, testing several state-of-the-art named entity recognition algorithms. The latter, however, do not pay great attention to the influence of the word embeddings, and do not facilitate observing their real impact on the BioNER task. For this reason, the present study evaluates three well-known NER algorithms (CRF, BiLSTM, BiLSTM-CRF) with respect to two corpora (DrugBank and MedLine) using two classic word embeddings, GloVe Common Crawl (of the general type) and Pyysalo PM + PMC (specific), as unique features. Furthermore, three contextualized word embeddings (ELMo, Pooled Flair, and Transformer) are compared in their general and specific versions. The aim is to determine whether general embeddings can perform better than specialized ones on the BioNER task. To this end, four experiments were designed. In the first, we set out to identify the combination of classic word embedding, NER algorithm, and corpus that results in the best performance. The second evaluated the effect of the size of the corpus on performance. The third assessed the semantic cohesiveness of the classic word embeddings and their correlation with respect to several gold standards; while the fourth evaluates the performance of general and specific contextualized word embeddings on the BioNER task. Results show that the classic general word embedding GloVe Common Crawl performed better in the DrugBank corpus, despite having less word coverage and a lower internal semantic relationship than the classic specific word embedding, Pyysalo PM + PMC; while in the contextualized word embeddings the best results are presented in the specific ones. We conclude, therefore, when using classic word embeddings as features on the BioNER task, the general ones could be considered a good option. On the other hand, when using contextualized word embeddings, the specific ones are the best option.

Abstract

Ecological economics was formally established in 1989 with the ambitious vision of developing a new economic paradigm embedding the social and economic systems in the biophysical world. Ecological economics had its roots in the biophysical understanding of economics that pioneers of the field Georgescu-Roegen (1971), Odum (1971), Daly (1977), Jansson (1984), Martinez-Alier (1987) and others developed while studying the thermodynamic bedrocks shared by natural and social systems. Subsequently, however, ecological economics has explicitly but controversially adopted a ‘big umbrella’ approach to methodological pluralism, focusing more on topics such as valuing nature and its functions rather than attempting to understand and quantify the biophysical roots of economic activities. Biophysical economics was established in response to these concerns, focusing more on developing analyses and models of the transformations of nature to generate wealth and approached mostly from an energy and material flows perspective. The present paper argues that to achieve its original vision ecological economics must return to its biophysical roots. Collaboration between the two fields will help the next generation of thinkers to address the socio-ecological challenges of the 21st century requiring an energy transformation called for by aspirations such as the Green New Deal and the UN Sustainable Development Goals.

Sono esposti i risultati di una modellazione numerica 2D sforzideformazioni che ha consentito di simulare l’evoluzione temporale degli spostamenti del versante N del M. Toc indotti dalle variazioni di livello dell’invaso del Vajont nel periodo tra marzo 1960 e il 9 Ottobre 1963. Il comportamento tensio-deformativo del versante è stato riprodotto adottando un modello costitutivo del tipo strain-softening per il materiale detritico interessato dalla rottura basale. L’analisi mostra che i materiali ghiaioso-argillosi coinvolti nella rottura basale sono stati caratterizzati da una notevole deformazione plastica localizzata (17.5–22.5%) prima di giungere al collasso finale. Alla fine dei tre cicli di invaso-svaso si è determinato un sensibile decremento della resistenza al taglio causato dal progressivo passaggio dalle condizioni di attrito di picco (Φp = 21.7°) a quello residuo (Φres = 20.2°), per effetto delle deformazioni plastiche raggiunte in corrispondenza della futura superficie di rottura. Tale perdita di resistenza è stata determinante nel raggiungimento della condizione di collasso verificatasi il 9 Ottobre 1963, durante l’ultima manovra di svaso del serbatoio.

Abstract

Stress and anxiety of university science students (Chemistry) was evaluated in basal conditions and during exams using validated stress and anxiety questionnaires. The relations between the data obtained and various biochemical markers were established. Results showed that the evaluated students did not experience stress increase as a consequence of exams but suffered a significant increase in anxiety. The psychological findings agree with the urinary biomarkers studied. It is known that anxiety is related to partial magnesium reduction associated with a urinary magnesium excretion increase, as observed in the present data. Nevertheless, stress also correlates with a urinary calcium increase which was not detected in the present study.

Abstract

This paper analyzes the mechanical behavior of the unstable Mt. Toc slope before the 1963 catastrophic collapse, considering both the measured data (surface displacements and microseismicity) and the updated geological model of the prehistoric rockslide. From February 1960 up to 9 October 1963, the unstable mass behaved as a brittle–ductile ‘mechanical system,’ characterized by remarkable microseismicity as well as by considerable surface displacements (up to 4–5 m). Recorded microshocks were the result of progressive rock fracturing of distinct resisting stiff parts made up of intact rock (indentations, undulations, and rock bridges). The main resisting stiff part was a large rock indentation located at the NE extremity of the unstable mass that acted as a mechanical constraint during the whole 1960–1963 period, inducing a progressive rototranslation toward the NE. This large constraint failed in autumn 1960, when an overall slope failure took place, as emphasized by the occurrence of the large perimetrical crack in the upper slope. In this circumstance, the collapse was inhibited by a reblocking phenomenon of the unstable mass that had been previously destabilized by the first reservoir filling. Progressive failure of localized intact rock parts progressively propagated westwards as a consequence of the two further filling– drawdown cycles of the reservoir (1962 and 1963). The characteristic brittle–ductile behavior of the Vajont landslide was made possible by the presence of a very thick (40–50 m) and highly deformable shear zone underlying the upper rigid rock mass (100–120 m thick).

Abstract

In the present work the 1963 Vajont landslide has been back-analyzed in detail to examine the influence of performed reservoir operations on the Mt. Toc slope stability. Coupledseepage stability analyses show that the main trigger of the Vajont rockslide was the reservoir-induced water table which formed as a consequence of rapid seepage inflow. The permeability of the shear zone material occurring at the base of the Vajont rockslide is evaluated at 5·10 -4 m/s and back-calculated values of the friction angles φ range from 17.5° to 27.0°. Considering mountain reservoirs, slope failures can occur during both filling and drawdown phases. In the Vajont case, due to highly permeable materials of the shear zone, the slope stability decreased during filling and increased during drawdown. Another time-dependent phenomenon of mechanical nature (progressive failure) has to be considered to understand the final slope behaviour related to the last drawdown. © Springer-Verlag Berlin Heidelberg 2013.

O crescimento infantil é um dos melhores indicadores de saúde e seu acompanhamento constitui uma das prioridades na atenção a esse grupo populacional. O objetivo geral desse trabalho foi realizar uma avaliação antropométrica de crianças de seis a vinte e três meses de idade residentes na área de abrangência de uma unidade básica de saúde (UBS) de Londrina-PR e identificar fatores associados aos desvios nutricionais. Como objetivo complementar, comparou-se a avaliação do estado nutricional utilizando-se os referenciais NCHS de 1977 e OMS de 2006. Estudo transversal, com amostra de 268 crianças identificadas no SINASC, Ficha A e outros registros do serviço. Os desfechos estudados foram excesso de peso e baixa estatura, definidos por meio de percentil e escore z, segundo referencial proposto pela OMS em 2006. As variáveis independentes foram as relacionadas às crianças, às mães, às condições sociossanitárias, ao acesso ao serviço e a algumas características alimentares. Realizou-se análise descritiva, e, para a associação entre as variáveis, foi utilizado o teste de qui-quadrado, com correção de Yates e teste exato de Fisher. Entre as crianças avaliadas, observou-se maior proporção de sexo masculino (53,7%), de peso adequado de nascimento (94,8%) e de crianças que são cuidadas pela mãe (63,1%). A maioria das crianças faz acompanhamento na UBS (75,4%), e 91,7% iniciaram a puericultura antes de 30 dias de vida. Encontrou-se também uma frequência alta de ingestão de biscoitos salgados industrializados (59,3%), refrigerante (59,7%) e doces (69,4%). Na avaliação antropométrica verificou-se prevalência de 14,9% de risco de sobrepeso, 7,1% de sobrepeso e obesidade, 2,6% de déficit de peso e 4,1% de baixa estatura. Na comparação entre os dois referenciais, verificou-se maior sensibilidade do referencial OMS de 2006 tanto no diagnóstico de excesso de peso na relação peso para estatura, quanto no de baixa estatura. Os fatores que se associaram significativamente na análise bivariada ao excesso de peso no índice peso para estatura foram: quem cuida a maior parte do tempo da criança não é a mãe, trabalho materno e a não convivência da mãe com o companheiro. Em relação ao déficit de estatura, a prevalência do desfecho foi significativamente mais elevada entre as crianças cujo cuidador não é a mãe. A maior importância do excesso de peso em relação ao déficit sugere transição nutricional em curso, e o seu envolvimento com doenças crônicas não transmissíveis aponta a necessidade de ações efetivas em faixas etárias de vida cada vez mais precoces. Infant growth is one of the best health indicators and its monitoring is one of the priorities in caring for this population group. The aim of the present study was to evaluate the anthropometric profile of six to twenty-three-month-old children in a Basic Health Unit (UBS) of Londrina-PR and identify factors associated with nutritional problems. The supplementary aim of this study compared the nutritional status using the 1997 NCHS references and 2006 WHO (World Health Organization). Cross-sectional study with a sample of 268 children identified at SINASC, Form A and other service records. The outcomes studied were under weight and low height; they were defined by the z score according to 2006 WHO. The independent variables were related to children, mothers, socioeconomic conditions, service access and some alimentary characteristics. Descriptive analysis was performed and regarding the association among the variables a chi-square test with Yates correction and Fisher exact test were used. A large proportion of male gender (53.7%) was observed among the children studied of appropriate birth weight (94.8%) and children who are looked after by their mother (63.1%). The majority of children have UBS supervision (75.4%) and 91.7% started childcare before 30 days of life. A high frequency of industrialized salted crackers (59.3%), soda (59.7%) and sweets (69.4%) intake was perceived. The anthropometric profile assessment found prevalence of 14.9% of risk overweight, 7.1% children with overweight and obesity, prevalence of 2.6% underweight and 4.1% low height. Comparing the two frames, there was greater sensitivity of 2006 WHO reference, in both overweight diagnosis regarding weight for stature, and short stature. The variables that were significantly associated in bivariate analysis to overweight in connection between weight for stature were: working mother, who looks after most of the time and living with a partner. Regarding the outcome of low height, the outcome prevalence was significantly higher among children whose caregiver is not the mother. The most important of overweight in relation to the deficit suggests ongoing nutritional transition, and its involvement with non-communicable chronic diseases highlights the need for effective action in increasingly precocious age groups.
