Mitralis regurgitatie is een veel voorkomende aandoening onder ouderen. Het is de oorzaak van verminderde kwaliteit van leven, vroeger overlijden en toename hospitalisatie. Zich bewezen en langer bestaande behandelstrategieën zijn medicamenteuze en chirurgische mitralis klep (MK) behandeling. Medicamenteuze behandeling heeft als voordeel dat het de integriteit van het lichaam niet beschadigd, maar medicijnen kunnen het mechanische defect van de MK niet repareren. Chirurgische therapie kan dit wel, maar dit kan enkel plaatsvinden door het lichaam tijdens operatie (gecontroleerd) te beschadigen wat weer kan leiden tot forse complicaties. Sinds 10 jaar combineert de MitraClip het beste van beide werelden door de MK te repareren door middel van een minimaal invasieve procedure. Hoewel de MitraClip zijn waarde heeft bewezen, is het nog niet geheel duidelijk wanneer en bij wie deze behandeling het beste toegepast kan worden. In dit proefschrift bespreken we, voorafgegaan door een introductie in hoofdstuk één, in de tweede sectie het huidige gebruik van de MitraClip in Nederland en de peri-procedurele uitkomsten van de behandeling. De derde sectie gaat in op functionele uitkomsten alsmede prognostische uitkomsten waarbij gekeken wordt naar de overleving ná behandeling met de MitraClip. Het hoofdstuk eindigt met een directe vergelijking van MitraClip behandeling met medicamenteuze of chirurgische behandeling. Het vierde hoofdstuk gaat in op uitkomsten van de MitraClip behandeling bij specifieke patiëntenpopulaties. Tevens wordt in dit hoofdstuk een risicomodel gepresenteerd voor patiënten die MitraClip behandeling ondergaan. Het vijfde en laatste hoofdstuk vat de voorgaande secties samen en bespreekt de bevindingen en mogelijke implicaties voor de kliniek.

This thesis concerns the application of three new procedures in the field of structural heart disease. Firstly, we investigate the mineral invasive transcatheter tgechnique to treat high-surgicval risk patients with severe symptomatic MR using edge-to-edge technique with the MitraClip system. Secondly, we describe a percutaneous device (Watchman) for exclusion of the LAA from the systemic circulation and hereby reducin gthe risk of stroke and abolishing the need for VKA and associated bleeding risk. Thirdly, we focus on a minimal invasive treatment of symptomatic peri-prosthetic leaks using a transapicla access in patients with a high operative mortality risk of who are denied for surgery. Finally, the additional value of 3DTEE for patient selection and guidance of these transcatheter cardiac interventions throughout this thesis is shown.
