Objectives The goal of this study was to evaluate the feasibility of [ 18F]Galacto-RGD positron emission tomography (PET)/computed tomography (CT) imaging of αvβ3 expression in human carotid plaques. Background The integrin αvβ3 is expressed by macrophages and angiogenic endothelial cells in atherosclerotic lesions and thus is a marker of plaque inflammation and, potentially, of plaque vulnerability. [ 18F]Galacto-RGD is a PET tracer binding specifically to αvβ3. Therefore, [18F]Galacto-RGD PET/CT imaging of αvβ3 expression in human carotid plaques might provide a novel noninvasive biomarker of plaque vulnerability. Methods [18F]Galacto- RGD PET/CT imaging was performed in 10 patients with high-grade carotid artery stenosis scheduled for carotid endarterectomy. Tracer uptake was measured in the stenotic areas of the carotid arteries, as well as on the contralateral side, and was corrected for blood pool activity, measured in the distal common carotid artery (target-to-background [TB] ratio). TB ratio was correlated with immunohistochemistry of αvβ3 expression (LM609), macrophage density (CD68), and microvessel density (CD31) of the surgical specimen. In addition, ex vivo autoradiography of the surgical specimen with [18F]Galacto-RGD and competition experiments with an unlabeled αvβ3-specific RGD peptide were performed. Results [18F]Galacto-RGD PET/CT showed significantly higher TB ratios in stenotic areas compared with nonstenotic areas (p = 0.01). TB ratios correlated significantly with αvβ3 expression (R = 0.787, p = 0.026) and intensity of ex vivo autoradiography (R = 0.733, p = 0.038). Binding to atherosclerotic plaques was efficiently blocked in ex vivo competition experiments. A weak-to-moderate correlation was found with macrophage density (R = 0.367, p = 0.299) and microvessel density (R = 0.479, p = 0.176), which did not reach statistical significance. Conclusions [ 18F]Galacto-RGD PET/CT shows specific tracer accumulation in human atherosclerotic carotid plaques, which correlates with αvβ3 expression. Based on these initial data, larger prospective studies are now warranted to evaluate the potential of molecular imaging of αvβ3 expression for assessment of plaque inflammation in patients.

Background: In a substantial number of mainly diabetic patients isolated crural arterial lesions are found to be the underlying cause for severe ischaemic foot lesions. Without revascularisation, patients with this specific occlusion pattern will inevitably face major amputation. To attain limb salvage in this setting, since the early eighties short vein grafts were used to bypass the occluded infrapopliteal arteries. More recently, percutaneous transluminal angioplasty (PTA) was also attempted to avoid limb loss in selected patients. Patients and methods: Since May 1986 in 125 patients 130 autologous bypass grafts from the BK-popliteal artery or the proximal tibioperoneal arteries to malleolar vessels were performed in the presence of extended crural arterial occlusions and critical foot ischaemia (rest pain 3, tissue loss 127). In another series in 89 limbs (rest pain 5, tissue loss 84) of 84 patients PTA was done to treat 168 focal stenoses of > 50% diameter reduction and 11 short occlusions in a total of 135 crural arteries. Results: Using life-table analysis, primary and secondary cumulative patency rates for short vein grafts with distal graft origin were 90% and 98% at 30 days, 76% and 83% at one year and 46% and 49% at seven years, respectively. The corresponding limb salvage rates amounted to 95%, 80% and 63%. Initial complete or partial technical success after PTA of crural arteries could be obtained in 93%: The limb salvage rates achieved were 95% at 30 days, 82% at one year and 63% at six years. Conclusion: Our results suggest that – depending on the extent of lesions – both short vein grafts as well as PTA are successful complementary treatment modalities to avoid limb loss in predominantly diabetic patients with infrapopliteal artery disease and critical ischaemia.

In the present work, we develop a three-dimensional isotropic finite-strain damage model for abdominal aortic aneurysm (AAA) wall that considers both the characteristic softening of the material caused by damage and the spatial variation of the material properties. A strain energy function is formulated that accounts for a hyperelastic, slightly compressible, isotropic material behavior during the elastic phase, whereas the damage process only contributes to the material response when the elastic limit of the AAA wall is exceeded. Material and damage parameters are obtained by fitting the strain energy function to the experimental data obtained by uniaxial tensile tests of freshly harvested AAA wall samples. The damage model extends the validity of the material law to a strain range of up to 50%. Purely elastic material laws for AAA wall are only valid for a strain range of up to 17%. In a series of finite element simulations of patient-specific AAAs, serving as numerical examples, we investigate the applicability of the damage model. The use of the damage model does not yield a more distinct identification of ruptureprone AAAs than other computational-based risk indices. However, the benefit of the finite-strain damage model is the potential capability to trigger growth and remodeling processes in mechanobiological models.

9. Correlation of biomechanics to tissue reaction in aortic aneurysms assessed by finite elements and [18F]-fluorodeoxyglucose-PET/CT

Mechanobiological interactions are essential for the adaption of the cardiovascular system to altered environmental and internal conditions, but are poorly understood with regard to abdominal aortic aneurysm (AAA) pathogenesis, growth and rupture. In the present study, we therefore calculated mechanical AAA quantities using nonlinear finite element methods and correlated these to [18F]-fluorodeoxyglucose (FDG)-metabolic activity in the AAA wall detected by positron emission tomography/computed tomography (PET/CT). The interplay between mechanics and FDG-metabolic activity was analyzed in terms of maximum values and the three-dimensional spatial relationship, respectively. Fluorodeoxyglucose-positron emission tomography/computed tomography (FDG-PET/CT) data sets of n=18 AAA patients were studied. Maximum FDG-uptake (SUVmax) in the AAA wall varied from 1.32 to 4.60 (average SUVmax 3.31 +-0.87). Maximum wall stresses and strains ranged from 10.0 to 64.0 N/cm2 (38.2 +- 13.8 N/cm2) and from 0.190 to (0.260 .0.222+-0.023), respectively. SUVmax was significantly correlated to maximum wall stress and strain (SUVmax to stress: r=0.71, p = 0.0005; SUVmax to strain: r = 0.66, p = 0.0013). To evaluate the three-dimensional spatial interaction between FDG-uptake and acting wall stress, element-wise correlations were performed. In all but 2 AAAs, positive element-wise correlation of FDG-uptake to wall stress was obtained, with the Pearson’s correlation coefficient ranging from -0.168 to 0.738 (0.372+-0.263). The results indicate that mechanical stresses are correlated quantitatively and spatially to FDG-uptake in the AAA wall. It is hypothesized that unphysiologically increased loading in the AAA wall triggers biological tissue reaction, such as inflammation or regenerative processes, causing elevated FDG-metabolic activity. These findings strongly support experimental hypotheses of mechanotransduction mechanisms in vivo.

The widely accepted hypothesis that mechanobiological transduction plays a central role in pathogenesis of abdominal aortic aneurysm (AAA) is plausible, but so far was never directly proven in vivo at the organ level for methodical reasons. At present, stresses and strains acting in AAA wall can be assessed by computational finite element analyses (FEM) [1,2]. Independently, it has also been reported that glycolytic activity in AAA wall non-invasively assessed by [18F]flourodeoxyglucose positron emission tomography/CT (FDG-PET-CT) is associated with increased proteolytic activity, structure-protein-degradation, AAA progression and consequently AAA wall instability as well as rupture risk [3]. Both methods were studied qualitatively by our research group in an individual AAA patient in [4] as well as by other groups in [6] and [7]. Here, the correlation of computational biomechanics with metabolic activity assessed by FDG-PET-CT is analyzed in a larger patient cohort (n=18) in a quantitative manner for the first time [5].

The Finite Element Method (FEM) is a promising tool to evaluate rupture risk of abdominal aortic aneurysms (AAA) and thus support clinical decision making. However, knowledge on spatial distribution of wall thickness, elastic properties and strength is prerequisite for meaningful FEM simulation results. In this study, mixed-effects models (comparable to [1]) for the distributions of these quantities are first created and then applied to a set of patient-specific AAAs.

Increased blood pressure and arterial wall degradation lead to formation of abdominal aortic aneurysms (AAA). As elective AAA surgery is not without potential risk it is common practice to balance risk of rupture against risk of intervention. Usually a max. AAA diameter is the clinically accepted parameter where diameter > 55mm indicates intervention. However, it has been shown that computational wall stress analyses is a better indicator for rupture risk, e.g. by Fillinger et al 2003. In the presented study we use highly advanced FEA and statistical wall strength models to investigate whether current computational biomechanics analyses yields significant results when applied to groups of AAA with matched diameter in the decision-critical regime of approx. 55mm. For this purpose diameter matched groups of asymptomatic and ruptured/symptomatic AAA are examined and significance of computational rupture risk predictors is studied

The interplay of biomechanical stresses to subsequent histopathological response in the wall of abdominal aortic aneurysm (AAA) is poorly understood and investigated. Meanwhile individually acting forces can be calculated precisely by wall stress analyses as well as biologic activity of AAA wall can be studied in-vivo by 18F-fluorodeoxyglucose positron emission tomography. For better insights in stresstissue interactions and methodical reasons we studied therefore the correlation of metabolic activity with the results of computational wall stress analyses in two patients. Thereby, areas of highest metabolic activity were well correlated to regions with highest computational peak wall stress for these two patients. To proof these findings further analyses in 25 patients are planned and the results will be presented.

Summary Background Among asymptomatic patients with severe carotid artery stenosis but no recent stroke or transient cerebral ischaemia, either carotid artery stenting (CAS) or carotid endarterectomy (CEA) can restore patency and reduce long-term stroke risks. However, from recent national registry data, each option causes about 1% procedural risk of disabling stroke or death. Comparison of their long-term protective effects requires large-scale randomised evidence. Methods ACST-2 is an international multicentre randomised trial of CAS versus CEA among asymptomatic patients with severe stenosis thought to require intervention, interpreted with all other relevant trials. Patients were eligible if they had severe unilateral or bilateral carotid artery stenosis and both doctor and patient agreed that a carotid procedure should be undertaken, but they were substantially uncertain which one to choose. Patients were randomly allocated to CAS or CEA and followed up at 1 month and then annually, for a mean 5 years. Procedural events were those within 30 days of the intervention. Intention-to-treat analyses are provided. Analyses including procedural hazards use tabular methods. Analyses and meta-analyses of non-procedural strokes use Kaplan-Meier and log-rank methods. The trial is registered with the ISRCTN registry, ISRCTN21144362. Findings Between Jan 15, 2008, and Dec 31, 2020, 3625 patients in 130 centres were randomly allocated, 1811 to CAS and 1814 to CEA, with good compliance, good medical therapy and a mean 5 years of follow-up. Overall, 1% had disabling stroke or death procedurally (15 allocated to CAS and 18 to CEA) and 2% had non-disabling procedural stroke (48 allocated to CAS and 29 to CEA). Kaplan-Meier estimates of 5-year non-procedural stroke were 2·5% in each group for fatal or disabling stroke, and 5·3% with CAS versus 4·5% with CEA for any stroke (rate ratio [RR] 1·16, 95% CI 0·86–1·57; p=0·33). Combining RRs for any non-procedural stroke in all CAS versus CEA trials, the RR was similar in symptomatic and asymptomatic patients (overall RR 1·11, 95% CI 0·91–1·32; p=0·21). Interpretation Serious complications are similarly uncommon after competent CAS and CEA, and the long-term effects of these two carotid artery procedures on fatal or disabling stroke are comparable.
