[eng] Introduction: Overweight is a global public health problem. It is considered a major risk factor for chronic conditions such as hypertension, diabetes, hyperlipidaemia and certain cancers. It affects all categories of the population. To this end, the study aims to investigate the prevalence of obesity and overweight as well as the associated factors among health care personnel in a regional hospital in southern Morocco. Methods: This was a descriptive cross-sectional study of nurses at the regional hospital. Anthropometric measurements were carried out in accordance with WHO recommendations. Weight was measured using a MEDISANA RETRO PSD scale and height using a wall-mounted scale graduated in cm. The data were processed using IBM SPSS version 20 software. Results: The mean BMI in women was 26.18 (SD ±3.77) compared to 25.76 in men (SD ±3.62). Almost two thirds (62.57%) were overweight, of which 16% were obese. The study showed that 65% of the weight load was recorded among nurses (of which 17.20% were obese). In addition, 64.9% (of which 19% were obese) of the overweight was recorded among those who were on call or on-call, followed by those who were on regular duty. Se considera un importante factor de riesgo de enfermedades crónicas como la hipertensión, la diabetes, la hiperlipidemia y ciertos cánceres. Afecta a todas las categorías de la población. Con este fin, el estudio pretende investigar la prevalencia de la obesidad y el sobrepeso, así como los factores asociados, entre el personal sanitario de un hospital regional del sur de Marruecos. Métodos: Se trata de un estudio descriptivo transversal de las enfermeras del hospital regional. Se realizaron mediciones antropométricas de acuerdo con las recomendaciones de la OMS. El peso se midió con una báscula MEDISANA RETRO PSD y la altura con una báscula de pared graduada en cm. Los datos se procesaron con el programa informático IBM SPSS versión 20. Resultados: La media del IMC en las mujeres fue de 26,18 (DE ±3,77) frente a 25,76 en los hombres (DE ±3,62). Casi dos tercios (62,57%) tenían sobrepeso, de los cuales el 16% eran obesos. El estudio mostró que el 65% de la carga de peso se registraba entre las enfermeras (de las cuales el 17,20% eran obesas). Además, el 64,9% (de los cuales el 19% eran obesos) del sobrepeso se registró entre los que estaban de guardia o de guardia, seguidos de los que estaban de servicio regular. Conclusión: El trabajo por turnos altera los patrones regulares de sueño, alimentación y ejercicio, lo que puede dificultar el mantenimiento de un peso saludable.

The objective of this study was to describe the clinical and laboratory manifestations, triggers factors, treatment, and outcome of MAS complicating SLE.We retrospectively analyzed the medical records of adult patients with SLE for a period of 8 years (2009-2016) and identified patients who had developed MAS. We conducted statistical analysis to identify factors associated with MAS.Among 208 consecutive lupus patients, 20 patients (19 women) were identified having MAS. The mean age of patients was 35.4 ± 10 years. MAS revealed lupus in 7 patients. In the others, the delay between diagnosis of SLE and MAS was 33,3 months. All cases required hospital admission, and 2 patients were admitted to the intensive care unit. An anemia (hemoglobin 10 g/dL) was found in all patients. A thrombopenia was observed in 19 (95%) cases. Hypertriglyceridemia and hyperferritinemia were present in all patients. All patients had anti-nuclear antibodies and anti-double-stranded DNA antibodies. Bone marrow aspiration showed hemophagocytosis in 15 (94%) cases. The mean SLEDAI was 20.95 corresponding to an SLE of a very high activity. The mean H-Score was 233.85. MAS was associated with a lupus flare in 13 patients. Documented bacterial infections, viral infections, and a breast cancer were respectively diagnosed in 4, 3, and 1 cases respectively. The corticosteroids were administered in all patients. Intravenous cyclophosphamide was used together with corticosteroids in 6 patients, mycophenolate mofetil in 2 cases and azathioprine in 2 cases. Intravenous immunoglobulin was given in 4 cases, etoposide in one case and rituximab was used as the third line treatment in one patient. All infectious episodes were also treated by broad spectrum antibiotics. All patients had a good outcome without any mortality at the management, with a mean follow-up of 24 months. The clinical parameters significantly associated with MAS were fever (p = 0,001), splenomegaly (p 0.0001), lymphadenopathy (p 0.0001), oral and/or nasopharyngeal ulceration (p = 0.04), arthritis (p = 0.017), and pulmonary signs (p = 0.003). Laboratory parameters associated with MAS were anemia (p 0.0001), thrombopenia (p 0.0001), hyperferritinemia (p 0.0001), hypertriglyceridemia (p 0.0001), SLEDAI (p 0.0001), and H-Score (p 0.0001). Receiver operating characteristic (ROC) analysis identified optimal cutoff values of ferritin ( 695 ng/mL) and SLEDAI ( 13.5) to predict the occurrence of MAS in SLE.MAS was observed in 9.62% Moroccan adult patients with SLE. SLE flare and infection were the common triggers of MAS in our study. Our study indicates that the occurrence of unexplained fever, splenomegaly, lymphadenopathy, profound cytopenia, hyperferritinemia, hypertriglyceridemia, high SLEDAI, and H-Score should raises the possibility of the diagnosis of MAS in SLE patients. Early diagnosis and urgent therapeutic management improves the overall prognosis. Key Points • Macrophage activation syndrome (MAS) is an underdiagnosed complication of systemic lupus erythematosus (SLE). The prevalence of this complication in this study is nearly 10%. • The diagnosis of MAS represents a major challenge for clinicians, as it could mimic a SLE flare up or be confused with infections. Validated diagnostic criteria for MAS in adults secondary to SLE are urgently needed. • In this study, the H-score calculate the individual risk of adult patients having reactive MAS. The cut-off value for the H-score was 190.5 (sensitivity 96.7%, specificity 97.6%). • The prognosis of MAS with SLE is good in our study. However, in the literature MAS may be a fatal condition in SLE patients. Prospective studies are necessary to confirm these results.

Objective The aim of this study is to validate the Moroccan version of The Caregivers Oncology Quality of Life (CarGoQol) questionnaire to measure the caregiver’s quality of life of palliative cancer patients in Morocco. Results The psychometric properties of CarGoQoL were generally good. A Cronbachα coefficient of 0.82 was obtained indicating a good internal consistency except for the private life dimension (α = 0.14). A good reliability of the test re-test was also observed, with an item internal consistency of 0.9. Missing data and ceiling effect were low; some ceiling effects were observed for Relationship with healthcare (38.9%); correlations were confirmed between the different dimensions that corresponding to the same domain; significant association was identified for caregiver and patient characteristics.

Parkinson’s disease (PD) is the second most common neurodegenerative disorder after Alzheimer disease. Five to ten percent of patients have monogenic form of the disease, while most of sporadic PD cases are caused by the combination of genetic and environmental factors. Microtubule-associated protein tau (MAPT) has been appointed as one of the most important risk factors for several neurodegenerative diseases including PD. MAPT is characterized by an inversion in chromosome 17 resulting in two distinct haplotypes H1 and H2. Studies described a significant association of MAPT H1j subhaplotype with PD risk, while H2 haplotype was associated with Parkinsonism, particularly to its bradykinetic component. We report here an isolated case displaying an akinetic-rigid form of PD, with age of onset of 41 years and a good response to levodopa, who developed dementia gradually during the seven years of disease progression. The patient does not carry the LRRK2 G2019S mutation, copy number variations, nor pathogenic and rare variants in known genes associated with PD. MAPT subhaplotype genotyping revealed that the patient has the H1j/H2 diplotype, his mother H1j/H1j, his two healthy brothers H1j/H1v and his deceased father was by deduction H1v/H2. The H1j/H2 diplotype was shown in a total of 3 PD patients among 80, who also did not have known PD-causing mutation and in 1 out of 92 healthy individual controls. The three patients with this diplotype all have a similar clinical phenotype. Our results suggest that haplotypes H1j and H2 are strong risk factor alleles, and their combination could be responsible for early onset of PD with dementia.

In Morocco, diabete is a common life-long disease . About 80 % of cases of diabete have type 2 , and this is likely attributed to lifestyle and type of alimentation. Starting from 2011, the Ministry of health (MH) set up a new system for diabete case reporting at the primary healthcare centers level, that many regional health departments adopted. This study aims at assessing diabete epidemiology after the establishment of such a system in the province of Fquih Ben Saleh (FBS) between 2012 and 2016. This latter is a predominantly rural agriculture area where eating habits and physical activities are different than those in prefectures. The number of cases by commune, by age, and by gender were provided by the provincial Deleguation of MH in FBS. Data were re-organized, cross-checked and analyzed using both excel and a statistical software. Diabete prevalence, incidence, proportion by age group, sex ratio, and evolution were determined both at the province level and by commune (13 rural and 3 small urban communes made up the province of FBS), during the period between 2012 and 2016. Correlation was assessed by means of Kendall’s test. The global prevalence rate was 1.9 per 100 population/year and remains quite lower than the national rate (12 per 100 population in 2015). The global incidence rate was 4 per 1000 population/year during the study period in FBS. There was an increase in both prevalence rate (taux de kendall’s =0.8 ; p-Value= 0,040) and incidence rate ( (taux de kendall’s =1 ; p-Value= 0,008) from 2012 and 2016. During this study period, increase in incidence rate was shown in two rurale communes, namely Had Boumoussa (taux de kendall’s =1 ; p-Value= 0,008) and Kerifate (taux de kendall’s =0,74; p-Value= 0,042). It was also shown in one urban commune namely souk sebt (taux de kendall’s = 0,8; p-Value= 0,042). The age groups 40 to 59 and over 60 year were more affected by diabete than other age groups (i.e., below 19 and 20 to 39). Diabete affects about two times as many women as men (risk ratio = 1.68) in FBS. Il s’agit d’une étude transversale réalisée au laboratoire d’hématologie du CHU Ibn Sina entre Juin 2012 et Mai 2014. Parmi 104 cas de LA diagnostiqués, 52% étaient des garçons avec un sex-ratio H/F= 1,32 et l’âge médian de 5,7 ans. La répartition des différents types de LA était : LA lymphoïde (LAL) (74%), LA myéloïde(LAM) (20,2%), LA biphénotypique(LAB) (65,8%). Parmi les LAL ,78% ont été classé LAL B et 22% comme LALT. Les signes cliniques étaient principalement présentés par le syndrome tumoral (73,1%),la fièvre (61%) et syndrome hémorragique (50%).Les anomalies de l’hémogrammeles plus fréquents étaient : thrombopénie (89,4%), anémie (86,5%), hyperleucocytose (79,8%).Le taux des blastes périphérique et médullaires était statistiquement élevé pour LAL que pour LAM et LAB (p

Résumé : Considérée comme un problème de santé publique à l’échelle mondiale, la violence faite aux femmes ne cesse de menacer leur bien être physique et psychique. Souvent perpétrée par un partenaire intime dans le cadre conjugal mais cette violence a plusieurs formes qui touchent la femme depuis la vie intra-utérine jusqu’à un âge avancé. Background Recently women’s lung cancer mortality rates have dramatically increased in developed countries, contrasting with a levelling off or decrease among men. Descriptive epidemiological data on primary lung cancer in women is scarce in Morocco. The aim of this study, conducted in the National Institute of Oncology in Rabat, was to describe the epidemiological profiling especially for the smoking status, to determine the most frequent type of lung cancer, and to analyse the survival of Moroccan women with lung cancer diagnosis. Results We found 101 women among 1680 (male and female) cases of lung cancer. The never-smokers were estimated to 75 %. The proportion of adenocarcinoma among never and passive smokers was higher than that of squamous cell carcinoma (SCC) (69.4 versus 30.6 %), while among women who were smokers, the most frequent histological type was SCC (63.6 %). The Cox regression analysis showed that smoking and passive smoking were not significantly associated with survival [HR: 0.62 (95 % CI 0.31, 1.30); p = 0.19] [HR: 0.56 (95 % CI 0.29, 1.08); p = 0.08] respectively. Adenocarcinoma was significantly associated with shorter survival [HR: 1.73 (95 % CI 1.05, 2.85); p = 0.03]. Conclusions The majority Moroccan women affected by lung cancer have never smoked (75 %). Environmental exposures, genetic predisposition, hormonal factors, and viral infection may all play a role in lung cancer in this category. The relation between histological type and tobacco found in our series concurred with those reported in the literature—adenocarcinoma appears to be the most frequent cell type affecting never and passive smokers. Adenocarcinoma is significantly associated with poorer survival.

Background Surgical clipping or endovascular coiling are the main procedures used in the treatment of cerebral aneurysms, with a preference for endovascular coiling. In Morocco, the number of patients needing endovascular coiling is growing, but many of them do not have access to this technique. The aim of this study was to determine the main parameters associated with variations in the total cost of this procedure in order to establish the amount (lump sum) that may be reimbursed by health insurance funds. Methods One hundred and seventeen patients with 124 aneurysms were admitted for treatment of one or more intracranial aneurysms between January 2010 and December 2015. The overall cost of hospitalization was assessed by using the micro-costing technique. The calculation was based on the tariffs of medical procedures as defined by the Ministry of Health in Morocco. A regression analysis was used to define the correlation between the overall cost and the various parameters. Results Univariate linear regression showed that the total cost was influenced by overall duration of hospitalization, ICU duration of hospitalization and size of aneurysm. On the other hand, univariate linear regression showed that the total cost was not influenced by sex, localization of aneurysm, and size of the aneurysm’s neck. However, multivariate linear regression showed that the total cost was influenced by one type of insurance health, overall duration of hospitalization, ICU duration of hospitalization, size of the aneurysm, and size of the aneurysm’s neck. Conclusion Setting a rate for reimbursement of patients who have had coiling treatment for a cerebral aneurysm should take into account the results of our study in order to limit the costs borne by patients. The parameters that influence the overall cost must be reimbursed in each case while the parameters that do not influence treatment costs could be included in a lump sum.
