Abstract

Factitious disorder imposed on another, or medical child abuse, has been rarely reported to have primary ocular presentations. We report an unusual and difficult diagnosis of factitious disorder imposed by a mother on her infant resulting in bilateral blindness. An infant was referred with a history of recurrent periorbital cellulitis and sanguineous discharge associated with seizure-like episodes. Symptoms have been going on for more than 14 months, and child had been treated by different physicians from different specialties without a clear ophthalmic diagnosis. The right eye was previously enucleated at an outside hospital for secondary complications of similar symptoms. He was admitted for exhaustive diagnostic tests and multiple surgical treatments, and his hospital stay was complicated with multiple corneal perforations and apnoeic episodes despite optimal treatment. After suspicion of factitious disease, continuous electroencephalography and video monitoring revealed evidence of the mother inflicting physical harm to her child.

Abstract

This study sets out to investigate the effects of COVID-19 on the tourism industry in Jordan by studying the satisfaction of tourists after the lockdown. This study focuses on three main areas that may influence tourists' satisfaction. The tourist satisfaction study was conducted using a questionnaire designed and distributed among tourists who used the services. Other than that, all the tourist guides from the six travel agencies involved in this study were interviewed for cross-checking purposes. There was a shift in the satisfaction level after the pandemic's peak. The three main areas that determine satisfaction are services provided by travel agencies, facilities for tourists at their destinations, and accommodations. The findings suggested the tourists demanded proper COVID-19 standard operating procedures in addition to the basic requirements of the services provided. Data will inform stakeholders on the preparation of services provided for the post-COVID-19 tourism industry.

21. Management of Central Serous Chorioretinopathy Using Focal Laser Photocoagulation: A Case Report

Abstract

Introduction: Central serous chorioretinopathy (CSC) has the main symptoms of choroidal thickening, detachment of retinal pigment epithelium (RPE), and presence of subretinal fluid. Treatment of CSC is indicated for cases with SRF persisting for more than 4 months, decreased visual acuity, history of recurrence, or history of previous CSC in other eyes with poor visual outcome. This case report aims to describe the management of CSC using focal laser photocoagulation. Case presentation: A man, 41 years old, living in the city, came with complaints of sudden blurred vision in his left eye when he woke up 3 months ago. This complaint is accompanied by a decrease in the ability to see in the middle. There are no vision complaints such as flashes of light, no blurred vision such as tunnels or closed curtains, no red eyes, pain, or eye discharge. On ophthalmological examination, there was a decrease in visual acuity oculi sinistra (OS) 6/12, intraocular pressure within normal limits, and orthophoria. On examination of the posterior segment of the OS, a decreased positive foveal reflex was found. Fluorescent angiography fundus photo examination showed central serous chorioretinopathy OS. This patient was diagnosed with central serous chorioretinopathy oculi sinistra. Management is given to patients in the form of disease-related education, laser focal photocoagulation OS, clobazam 10 mg/24 hours, eye drops oxymetazoline HCl 1 drop in the left eye/8 hours. Conclusion: Treatment with laser focal photocoagulation is beneficial in the clinical improvement of cases of central serous chorioretinopathy.

Abstract

Objective: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) hospital outbreaks have been common and devastating during the coronavirus disease 2019 (COVID-19) pandemic. Understanding SARS-CoV-2 transmission in these environments is critical for preventing and managing outbreaks. Design: Outbreak investigation through epidemiological mapping and whole-genome sequencing phylogeny. Setting: Hospital in-patient medical unit outbreak in Toronto, Canada, from November 2020 to January 2021. Participants: The outbreak involved 8 patients and 10 staff and was associated with 3 patient deaths. Results: Patients being cared for in geriatric chairs at the nursing station were at high risk for both acquiring and transmitting SARS-CoV-2 to other patients and staff. Furthermore, given the informal nature of these transmissions, they were not initially recognized, which led to further transmission and missing the opportunity for preventative COVID-19 therapies. Conclusions: During outbreak prevention and management, the risk of informal patient care settings, such as geriatric chairs, should be considered. During high-risk periods or during outbreaks, efforts should be made to care for patients in their rooms when possible.

30. Measuring the Implementation Preferences of Emergency Medical Services Clinicians Using Discrete Choice Experiments

Abstract

Prehospital research and evidence-based guidelines (EBGs) have grown in recent decades, yet there is still a paucity of prehospital implementation research. While recent studies have revealed EMS agency leadership perspectives on implementation, the important perspectives and opinions of frontline EMS clinicians regarding implementation have yet to be explored in a systematic approach. The objective of this study was to measure the preferences of EMS clinicians for the process of EBG implementation and whether current agency practices align with those preferences. This study was a cross-sectional survey of National Registry of Emergency Medical Technicians registrants. Eligible participants were certified paramedics who were actively practicing EMS clinicians. The survey contained discrete choice experiments (DCEs) for three EBG implementation scenarios and questions about rank order preferences for various aspects of the implementation process. For the DCEs, we used multinomial logistic regression to analyze the implementation preference choices of EMS clinicians, and latent class analysis to classify respondents into groups by their preferences. A total of 183 respondents completed the survey. Respondents had a median age of 39 years, were 74.9% male, 89.6% White, and 93.4% of non-Hispanic ethnicity. For all three DCE scenarios, respondents were significantly more likely to choose options with hospital feedback and individual-level feedback from EMS agencies. Respondents were significantly less likely to choose options with email/online only education, no feedback from hospitals, and no EMS agency feedback to clinicians. In general, respondents’ preferences favored classroom-based training over in-person simulation. For all DCE questions, most respondents (66.2% - 77.1%) preferred their survey DCE choice to their agency’s current implementation practices. In the rank order preferences, most participants selected “knowledge of the underlying evidence behind the change” as the most important component of the process of implementation. In this study of EMS clinicians’ implementation preferences using DCEs, respondents preferred in-person education, feedback on hospital outcomes, and feedback on their individual performance. However, current practice at EMS agencies rarely matched those expressed EMS clinician preferences. Collectively, these results present opportunities for improving EMS implementation from the EMS clinician perspective.

Abstract

Introduction: Chronic myeloid leukemia (CML) is a progressive myeloproliferative disorder resulting from forming a chimeric BCR-ABL gene. The proteins derived from this gene can affect some genes from various signaling pathways such as PI3K/AKT/Wnt/catenin/JAK/Stat involved in proliferation, differentiation, cell death, and genes related to autophagy. Imatinib is the first-line treatment for CML patients, with durable and proper responses in Iranian children and adult CML patients. Hence, we aimed to evaluate the mRNA expression of some selected key genes from those pathways in patients with CML before and under treatment. Methods: In the case-control study, the mRNA expression of PTEN, LEF1, JAK3, LC3 and p62 genes were measured in 51 CML patients (6 patients before treatment and 45 patients under treatment with imatinib mesylate) and 40 healthy controls using the Real-time PCR method. Results: The mRNA expression of PTEN and P62 were significantly higher in newly diagnosed patients than in controls (P Conclusion: Our findings showed that the changes in expression of these genes were related to the patient’s treatment. Due to the key role of these genes in proliferation, differentiation and tumor suppression, it is proposed that these genes may be helpful for follow-up of treatment in CML patients.

32. Development of Corynebacterium glutamicum as Staphylococcal-Targeting Chassis via the Construction of Autoinducing Peptide (AIP)-Responsive Expression System

Abstract

Despite increasing reports of antimicrobial activities of commensal and non-pathogenic bacteria such as Corynebacterium spp., previous studies on bioengineered therapeutics traditionally employed probiotics and food-grade bacteria which limits further advancements into microbial therapeutics research. In this study, Corynebacterium glutamicum, a generally recognised as safe (GRAS) and model bacterium was employed as a new chassis for the development of bioengineered corynebacterial chassis tailored towards Staphylococcus sp. via autoinducer peptide (AIP)-based quorum sensing (QS) interactions. To develop C. glutamicum as a staphylococcal-targeting chassis, the bacteria were transformed with the pResponse plasmid harboring AIP-responding accessory regulatory proteins agrAC and red fluorescent protein (RFP) genes under the control of the PaceA and P3 promoter, respectively, which was expected to stimulate the production of fluorescence signals in the presence of AIPs. Fluorescence activity of the C. glutamicum pResponse strain was compared to control C. glutamicum pRFP strain containing only the P3-RFP gene without the agrAC gene cassette. Using AIP-I as the input biomolecule, C. glutamicum pResponse strain fluoresced under different concentrations of AIP-I whereas no fluorescence was observed in the control C. glutamicum pRFP strain. When tested with S. aureus culture supernatant, the pResponse strain exhibited increasing fluorescence over the incubation period with the highest fluorescence signal of 183 relative fluorescence units (R.F.U) was observed at a 48 h point thereby demonstrating a functional QS-responsive protein expression system in bioengineered C. glutamicum. These findings demonstrated the feasibility and promising potential of developing bioengineered C. glutamicum as a staphylococcal-responsive and -targeting chassis.

Abstract

The violations of regulatory rules, martial and religious laws, and social mores were among the reasons that led to the spread of various social phenomena. The latter contributed to social communication disorder and created immense abysses within societies such as immorality, violence and delinquency. Based on the explanatory theories of criminology or criminal sociology, the stigma of deviance was classified as a psychosocial disorder that the stigmatized suffers from, which had been confirmed by various researches starting from 1930s till 1950s. However, sociologists and experts in historical anthropology surpassed the concepts of linguistic terms, which were chiefly investigated by psychologists, to examine the features of stigmatization from a scientific perspective. Through the discussion of stigmatization, this research examines the key implications of stigma in relation not only to individuals but also to communities. It also investigates the reasons that lead some sectors of society to renounce and marginalize the stigmatized individual despite his efforts to go out the indictment division that crippled him and to reintegrate within his society. عدّ الخروج عن الأطر التّنظيميّة، وقوانين الأحكام العرفيّة والشّرعيّة، والعوائد المجتمعيّة من أسباب تفشّي عدّة ظواهر اجتماعيّة ساهمت في خرق أسس التّواصل، وخلقت هوّة سحيقة في صلب المجتمعات الحاضنة لتلك الظّواهر، ولعلّ الانحلال الأخلاقي، وتنامي أشكال العنف والانحراف هما أبرز النّتائج المتمخّضة عنها. ويتنزّل الوصم الانحرافي اِستنادا إلى النّظريّة التّفسيريّة في علم الإجرام، أو علم الاجتماع الجنائيّ ضمن خانة الخلل النّفسيّ الاجتماعيّ الذي يعاني منه الموصوم بالانحراف، وهو ما أكّدته عديد الأبحاث التي خصّصت حيّزا كبيرا من اهتمامها لدراسة الظّاهرة منذ ثلاثينيّات القرن العشرين حتّى منتصف القرن ذاته، بيد أنّ علماء الاجتماع تجاوزوا مفاهيم المصطلحات الّلغويّة التي أشبعت بحثا مع المتخصّصين في علم النّفس للخوض في صفة الوصم من النّاحية العلميّة. وحاول هذا البحث من خلال التّطرق إلى ظاهرة الوصم، الخوض في أهمّ التّداعيّات المترتّبة عنها سواء في علاقة بالأفراد، أو في علاقة بالمجموعات، والأسباب التي دفعت بعض شرائح المجتمع إلى نبذ الموصوم وتهميشه رغم محاولاته الخروج من دائرة الاتّهام التي أحاطت به، والاندماج من جديد في المجتمع المنتمي إليه رغم المضايقات المسلّطة عليه.

Rather than improving our understanding of the past, however, prevailing ways of handling the category Islam tend to weaken our grasp of historical processes. They are problematic and require critical attention to disentangle webs of meanings across a vast number of pertinent texts and contexts, medieval and modern.

Abstract

In this article, a real case of a one MW PV substation design problem is described. The impact of the tolerances of power components (mainly inductors and capacitors) on the design is highlighted through two studies using two different control strategies that are selective harmonic elimination (SHE) and selective harmonic modulation (SHM). The obtained results show that the components’ tolerances have a significant effect on the design, especially for the SHE strategy. While the design using SHM control shows a weak sensitivity to the components’ tolerance. In this study, an upgrade of the most sensitive control strategy is proposed in order to improve its sensitivity against components’ tolerance. The results show that the strategy upgrading leads not only to improving the sensitivity but also to generating better power quality.

38. Real-world effectiveness of COVID-19 vaccination in liver cirrhosis: a systematic review with meta-analysis of 51,834 patients

Abstract

SARS-CoV-2 vaccinations were found to be highly effective in phase 3 clinical trials. However, these trials have not reported data regarding the subgroup of liver disease or excluded patients with liver disease. The effectiveness of COVID-19 vaccines among liver cirrhosis (LC) patients is unclear. We conducted this meta-analysis to assess the effectiveness of SARS-CoV-2 vaccination in LC patients. A comprehensive literature search was conducted to include all the relevant studies that compared the outcomes of LC patients who received SARS-CoV-2 vaccines vs. unvaccinated patients. Pooled risk ratios (RRs) with 95% confidence intervals (CIs) were calculated by the Mantel-Haenszel method within a random-effect model. Four studies with 51,834 LC patients (20,689 patients received at least one dose vs 31,145 were unvaccinated) were included. COVID-19–related complications, including hospitalization (RR 0.73, 95% CI 0.59–0.91, P = 0.004), mortality (RR 0.29, 95% CI 0.16–0.55, P = 0.0001), and need for invasive mechanical ventilation (RR 0.29, 95% CI 0.11–0.77, P = 0.01), were significantly lower in the vaccinated group compared to the unvaccinated group. SARS-CoV-2 vaccination in LC patients reduced COVID-19–related mortality, intubation, and hospitalization. SARS-CoV-2 vaccination is highly effective in LC. Further prospective studies, preferably randomized controlled trials, are necessary to validate our findings and determine which vaccine is superior in patients with LC.

41. The Characteristics of Primary Open Angle Glaucoma Patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia

Introduction. Many people in Indonesia are unfamiliar with glaucoma, even though it causes blindness. This fact is because the damage process of glaucoma occurs slowly. If glaucoma is not treated immediately, it can cause irreversible vision loss and blindness. This study aims to present the characteristics of primary open-angle glaucoma patients at Dr. Mohammad Hoesin General Hospital Palembang, Indonesia. Methods. This study is a descriptive observational study. A total of 33 subjects participated in this study. Data analysis was carried out with SPSS for Windows. Results. Most participants were men aged 40-69 years old with a history of high school education. Most patients had clinical symptoms with a mild cup-to-disc ratio with early degrees of visual field defects and intraocular pressure less than 21 mmHg with therapy. Most patients had no history of hypertension, no history of diabetes mellitus and no family history of similar disorders. Most patients have refractive disorders and received medical therapy. Conclusion. The characteristics of primary open-angle glaucoma patients at Dr. Mohammad Hoesin General Hospital have a clinical form of a mild cup-to-disc ratio, refractive disorders and receive medical therapy to control increased intraocular pressure.

Abstract

Photovoltaic panels are used to generate electricity directly or indirectly. in the case of indirect, the energy should be saved in batteries and then it can be used and in this case, the inverter is required to change the direct current of the batteries(DC) to operate alternating current(AC) loads. This paper concentrates on saving energy in batteries and investigating the process of charging and discharging the batteries to operate selective loads in the paper. Using the photovoltaic panels to store energy in batteries, the results reveal that one photovoltaic panel of( 80 W) is placed towards south at a tilt angle of 36o; it could generate (224 W.hr) of energy that was obtained by charging a( 65 A.hr) lead-acid battery for 13 hours in April. Using two solar panels each of( 80 W )is connecting parallel,( 915.6 W.hr) of energy generated in May while using three units in parallel connection the electricity generated was near to that of two panels as the battery reaches nearly the full charge. Ambient temperature rise harms solar panels and leads to a decrease in the efficiency of the solar panel.

Especially these problems that often occur in Islamic Religious Education teachers in Islamic boarding schools who occupy a strategic position in determining the flow of progress of the times in accordance with the values of Islamic teachings. This study aims to explore the form of digital media for PAI learning and find teacher competencies in the implementation of ICT-based PAI instructional media at the Nurul Haramain Narmada Islamic Boarding School in Narmada West Lombok. This research uses a qualitative-descriptive approach with data collection techniques from interview, observation and documentation. There are three activities in qualitative data analysis, namely Data Condensation, Data Display and Drawing and Verifying Conclusions. In this study, researcher found forms of digital media for PAI at the Nurul Haramain Narmada Islamic Boarding School in West Lombok, Based on the policies of the cottage leaders and principals through several programs, namely Tugu Sasak program, CBT (Computer Base Test) program, CBC (Computer Base Classes) program, PBT (Paper Based Test) program, LAC (language Advisory Council) program, HMC (Haramain Media Center) program and IT department program of students organization with several supporting facilities are computer laboratories, language laboratories, multimedia classes, data base centers, work programs of students organization, language development centers, digital libraries, art creativity centers and eco-theology of pesantren. The competence of PAI teachers at the Nurul Haramain Narmada Islamic Boarding School West Lombok in implementation of instructional media based on ICT is to have pedagogic competence, namely the interaction between teachers and students well with relevant learning models and methods.

Abstract

The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135–15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359–5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138–5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184–5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598–9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090–6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286–5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p p = 0.018; 95% CI 0.138–0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143–0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible. 49. Atypical Neuralgia of Lingual Nerve after General Anesthesia: A Case Report

Abstract

Objectives: Injury of lingual nerve is a rare complication of general anesthesia. The causes of lingual nerve injury following general anesthesia are multifactorial; possible mechanisms may include difficult laryngoscopy, prolonged anterior mandibular displacement, improper placement of the oropharyngeal airway, macroglossia and tongue compression. Case Report: A case of atypical neuralgia of lingual nerve that was associated with orotracheal intubation for 54 years old female. Treatment: Carbamazepine was effectively aided the recovery of the injured lingual nerve. Conclusion: Lingual nerve injury following orotracheal intubation is rare but can cause irritation to patients.
