Snoring may be an important predictor of sleep-disordered breathing. Factors related to snoring among First Nations people are not well understood in a population with high rates of smoking and excess body weight. An interviewer-administered survey was conducted among 874 individual participants from 406 households in 2012 and 2013 in two Canadian First Nations communities. The survey collected information on demographic variables, individual and contextual determinants of respiratory health and snoring (classified as present versus absent) and self-reported height and weight. Multiple logistic regression analyses were conducted to examine relationships between snoring and potential risk factors adjusting for age and sex. Snoring was present in 46.2% men and 47.0% women. Considering body mass index, 259 people (30.3%) were overweight and 311 (36.4%) were considered obese. The combined current/former smoking rate was 90.2%. Being overweight, obesity, sinus trouble, current smoking status and former smoking were significantly associated with snoring. Exposure to home dampness and mold were suggestive of an association with snoring. To the degree that snoring may be a predictor of possible sleep-disordered breathing, these results indicate that environmental conditions such as smoking and home exposures may be important factors in the pathogenesis of these conditions.

OBJECTIVE: To explore how access to a family medicine clinic co-locating with the Children’s Aid Society (CAS) of Hamilton in Ontario helped meet the unique needs of children in care. DESIGN: Qualitative research using semistructured face-to-face and telephone interviews. SETTING: The CAS of Hamilton. PARTICIPANTS: Nineteen foster parents. METHODS: Stakeholders were invited to participate with flyers posted in the clinic, notices that were mailed to foster parents, personal invitations that were distributed during clinic visits, and an internal memo that was distributed to the CAS staff. Informed consent and assent where appropriate was obtained before an interview was started. Interviews were audiorecorded when and where feasible, transcribed, and subsequently underwent inductive, thematic analysis. Common themes evolved by consensus. MAIN FINDINGS: Foster parents valued the family medicine clinic co-locating with the CAS. The co-location helped children in care to know that there were others in similar circumstances. Foster parents learned from and shared parenting skills with one another, which resulted in developing confidence in the care they provided. The clinic became a neutral place for children in care, foster parents, and birth parents. The clinic team gathered the children’s complete health records and was responsible for sharing this information when appropriate. CONCLUSION: Access to a family medicine clinic designed specifically for children in care that co-located with the CAS enhanced not only the planning, management, and evaluation of care, but also provided a consistency that was not found in other parts of the children’s lives; this helped generate trusting relationships over time. The co-location provided a strong spoke in the circle of care.

To identify the essential components of a mentorship program as the first step in the ongoing development of a mentorship program for primary care physicians.Mixed-methods study.Saskatchewan.Forty-nine of 170 physicians responded positively to a letter of invitation. Of these, 25 physicians were purposively sampled based on location, sex, and experience. Fourteen participants practised in urban areas and 11 in rural settings; 11 were men and 14 were women; and 10 were junior physicians and 15 were senior. Junior physicians were defined as those who had graduated from medical school after 1995, and senior physicians were those who had graduated before 1980.This study employed qualitative, in-depth, semistructured interviews. Interview questions, based on an environmental scan, were developed then pilot-tested with a family physician. Interviews lasted approximately 60 minutes and were audiotaped. Digital audio files were transcribed verbatim and analyzed thematically.Family physicians described positive and negative aspects of mentoring, or having a lack of experience with mentoring. They also outlined key components of a potential mentorship program: matching mentees with mentors; integrating formal and informal mentorship; and the evaluation process of the mentorship relationship and program.Based on the feedback from family physicians, mentorship is viewed as an important and meaningful program of action that regional health stakeholders and medical educators in Saskatchewan could implement. A pilot test of a mentorship program model will be the culmination of this study. Further research will be undertaken to evaluate the model once it is implemented. This will have important implications for establishing a national mentorship program for family physicians across the country.
