Case presentation A 77-year-old man was seen in our follow-up clinic one year after implantation of his Medtronic Viva Quad XT CRT-D device. Previously, the single implant procedure of device and leads* was uneventful. Defibrillator and cardiac resynchronization therapy was given in primary prevention for a dilated ischemic cardiomyopathy with NYHA class III heart failure, low left ventricle ejection fraction (LVEF 25%), and prolonged QRS duration (190ms). Although an underlying atrial rhythm of permanent atrial fibrillation, continuous 100% biventricular pacing was maintained due to optimized drug therapy and device programming in VVIR mode at 70-120 bpm. Previous device check-up 6 month ago showed a good battery life status (7.8 years longevity), with normal measurements for sensing/stimulation thresholds and impedances.
