2. Obstacles to home-based dietary management for caregivers of children with citrin deficiency: a qualitative study

Background Dietary management is the most important and effective treatment for citrin deficiency, as well as a decisive factor in the clinical outcome of patients. However, the dietary management ability of caregivers of children with citrin deficiency is generally poor, especially in East Asia where carbohydrate-based diets are predominant. The aim of this study was to identify the difficulties that caregivers encounter in the process of home-based dietary management, and the reasons responsible for these challenges. Results A total of 26 caregivers of children with citrin deficiency were recruited, including 24 mothers, one father, and one grandmother. Grounded theory was employed to identify three themes (covering 12 sub-themes) related to the dilemma of dietary management: dietary management that is difficult to implement; conflicts with traditional concepts; and the notion that children are only a part of family life. The first theme describes the objective difficulties that caregivers encounter in the process of dietary management; the second theme describes the underlying reasons responsible for the non-adherent behavior of caregivers; the third theme further reveals the self-compromise by caregivers in the face of multiple difficulties. Conclusions This study reflects the adverse effects of multi-dimensional contradictions on the adherence of caregivers to dietary management. These findings reveal that the dietary management of citrin deficiency is not only a rational process, rather it is deeply embedded in family, social, and dietary traditions.

With the development of Internet of Things technology, outdoor positioning technology has been investigating extensively. Based on the brief analysis and comparison of the advantages and disadvantages of various tracking systems designed for community correction targets, this paper proposes to establish a LoRa outdoor positioning system used for community correction based on TDOA algorithm with low power consumption. The experimental results show that this system can achieve a positioning accuracy of 40m (with 95% confidence coefficient). This paper analyzes and summarizes the advantages of the system which is applicable to community correction and able to meet the requirements for tracking the correction targets.

Background: Citrin deficiency is an autosomal recessive genetic disorder caused by SLC25A13 genetic mutations. Understandably, Neonatal Intrahepatic Cholestasis caused by Citrin Deficiency (NICCD) was one of its clinical phenotypes, usually occurred in the neonatal period or infancy. The effective treatment of dietary intervention and symptomatic support may provide an avenue for clinicians to reduce liver damage and improve clinical prognosis. Objective: We report a 5-month-old infant diagnosed with Neonatal Intrahepatic Cholestasis caused by Citrin Deficiency (NICCD), coexisting Respiratory Syncytial Virus (RSV) infection. Method: The patient presented with rhinorrhea and cough, jaundiced skin and sclera, fat faces and hepatomegaly. Simultaneously, the child’s mother performed obvious anxious emotion. Infection control, nutrition management, and psychological guidance for the caregiver during hospitalization to discharge were achieved. Meanwhile, effective health education also plays a good role in these interventions. Result: After half a month, the patient’s infection was given controlled. Jaundice and hepatomegaly were improved gradually. Followeing up for 2 months, the clinical symptoms for NICCD were disappeared and the caregiver’s anxiety was alleviated effectively. The importance of follow-up and support longitudinally to improve the quality of life for patients is emphasized. Conclusion: Therefore, this study provided primary while valuable nursing care experience for NICCD with RSV infection patients.

6. Investigation of Anxiety and Its Influencing Factors in Postpartum Nurses Returning to Work from Giving Birth to a Second Child

Objective: We attempt to investigate the anxiety and its influencing factors in postpartum nurses returning to work from second childbirth so as to provide reference basis for making intervening measures to promote physical and psychological health of the postpartum nurses. Methods: We conducted a network questionnaire survey. Firstly, convenience sampling was used to select postpartum nurses who return to work from giving birth to a second child in public hospitals affiliated to Jiangmen City from September 1st 2018 to December 31st 2018. Then we adopted Self-Rating Anxiety Scale (SAS) to conduct a cross-sectional study of the anxiety of postpartum nurses returning to work from giving birth to a second child. We later used Logistics regression analysis to analyze the influencing factors of anxiety of the postpartum nurses. Results: the average score of 702 postpartum nurses on anxiety was (48.64±11.57), and the incidence of anxiety was 42.1%, among which the incidence of mild anxiety was 25.7%, moderate anxiety was 11% and severe anxiety was 5.4%. The Logistics regression analysis showed that duty, coping style, family function and social support were the factors that influenced anxiety of the postpartum nurses (R2=59.41%). Conclusions: The situation of anxiety of postpartum nurses returning to work after giving birth to a second child is severe, and family function is a major influencing factor of their anxiety. Nursing managers can help reduce the influence of family function on anxiety of postpartum nurses through providing psychological adjustment for them, help for their family and support for their work.

7. Nursing Care of an Extremely Preterm Infant with Sodium Taurocholate Cotransporting Polypeptide Deficiency: A Case Report

Sodium taurocholate cotransporting polypeptide (NTCP) is a protein encoded by the solute carrier family 10 member 1 gene and expressed in the basolateral membrane of the hepatocyte to uptake conjugated bile acids from the plasma. This study reported an extremely premature infant (EPI) with NTCP deficiency (NTCPD). The patient presented with jaundice, persistently elevated total bile acids, 25-(OH)-VitD deficiency, and cleft palate. Nursing care mainly focused on disease surveillance, jaundice care, nutrition, kangaroo mother care, and psychological care. In response to 5 months of the nursing care, the infant’s weight reached 3.324 kg, the jaundice was alleviated, and the infant gained the ability to suck milk. However, his TBA levels were abnormally elevated, and 25-(OH)-VitD was still deficient when the patient was discharged from hospital. The nurses taught the patient’s parents the importance of monitoring liver function and trace elements during a routine outpatient visit. Telephone follow-up 1 year later showed that the patient was in good health with no obvious clinical manifestations. This article reports our experiences in caring for an EPI with NTCPD. To the best of our knowledge, this is the first case report worldwide in NTCPD nursing care.

Objective: The article aims to explore the effect of continuous skin temperature monitoring on nursing for flap transplantation for repairing hand skin soft tissue defect. Methods: We selected 70 patients who received flap transplantation for hand skin soft tissue defect repair from April 2016 to April 2018 in a hospital and averagely randomized them into control group and observation group. Both groups were given routine nursing and the observation group also received continuous skin temperature monitoring. The incidence of vascular crisis, length of stay and duration of wound healing were compared between the two groups. Purpose: To systematically review the real experience of nurses in the fight against the coronavirus disease 2019 (COVID-19). Methods: PubMed, EMBASE, Cochrane Library, CBM, CNKI, Wan-Fang Data, and VIP from inception to April 2020. Qualitative research on real experience of nurses in the fight against the COVID-19 were collected. The study was screened by two researchers according to the criteria. After obtaining the data, the study quality was evaluated according to the JBI Critical Appraisal Tool, and the results were integrated by meta-synthesis. Results: Ten qualitative studies were included, involving 29 clear results and 7 categories, then 3 integrated themes were extracted: bear the burden of mind and body; desire for support and approval; self-worth is reflected. Conclusions: Nurses play an important role in the battle against the new coronavirus pneumonia, and nursing managers should dissolve the negative emotions of nurses in time. From the national and policy level, a scientific guarantee system should be formed for the materials, talent reserve, logistics, professional knowledge and other powerful guarantees. The community needs to foster a culture of respect for health care workers so that nurses can achieve their own value and gain a sense of professional identity and honor.

10. Development of a Diet Management Scale for Pediatric Patients with Citrin Deficiency

Objective: This study aimed to develop a scale to assess the caregiversability to manage the dietary needs of pediatric patients with citrin deficiency (CD). Methods: 24 experts from 11 hospitals in mainland China participated in the Delphi research. Item analysis was performed on 61 caregivers of CD patients who were enrolled in the testing of the draft scale, and 153 caregivers attended the validity and reliability testing. The initial scale was made according to the suggestions by the 24 experts. Retest reliability method and Cronbach’s α coefficient method were used to evaluate the external and internal reliability of the scale, respectively; the scale validity was evaluated by content validity and construct validity. Results: A consensus was reached by 24 experts after two rounds of investigation, and a preliminary draft of the scale was formed consisting of three dimensions: Special dietary knowledge, Healthy dietary attitude and Dietary behavior. The expert authority coefficient was 0.887, and the Kendall coefficient was 0.201. The final scale Cronbach's alpha coefficient (α = 0.713) showed good internal consistency and balance stability. The retest reliability coefficient of the scale was 0.975, indicating good external stability. The total content validity index (S-CVI) was 0.981, and the content validity index (I-CVI) of each item varied between 0.913 and 1.0. On confirmatory factor analysis, the indicators CMIN/DF, RESAE, PNFI, NFI, TLI, IFI and CFI for the construct validity of the scale were 1.191, 0.053, 0.628, 0.796, 0.940, 0.954, and 0.951, respectively. Conclusion: The scale developed in this paper exhibited promising reliability and validity for the clinicians to evaluate the dietary management ability of the caregivers of pediatric CD patients.

Background: Progressive familiar intrahepatic cholestasis (PFIC) is a group of rare cholestatic liver diseases whose main features are itching, jaundice and even liver failure. Many studies have been reported in clinical reports, but the care of PFIC is rarely described. Purpose: To provide a summary and recommendations for the setup of strategies for PFIC patient care. Material and methods: A non-systematic review of PFIC nursing research until July 12, 2020. Use online search engines (PubMed, Medline, Embase, Web of Science, and CINAHL) to find PFIC nursing-related content, sort and summarize nursing-related content. Results: As a rare disease, there is limited research in PFIC nursing. The main care measures of PFIC including nursing interventions and evaluation tools. Nursing intervention is divided into general symptom care and nursing care of postoperative complications. Evaluation tools mainly focus on quality of life assessment and evaluation of itching. Conclusions: Nurses should pay attention to the use of scales when caring for PFIC patients, and observe whether specific nursing measures can help improve patients’ quality of life. PFIC nursing interventions and evaluation tools should be tailored according to PFIC clinical manifestation. A multidisciplinary collaborative approach is encouraged for proper management and tailoring therapy according to clinical manifestation.

12. Continuous Quality Improvement of Radiation Safety Management for Treatment of Differentiated Thyroid Carcinoma with Iodine-131

Purpose: To explore the effect of continuous quality improvement (CQI) of radiation safety management for patients administerediodine-131 after thyroid cancer surgery. Methods: A total of 103 patients with differentiated thyroid carcinoma were randomly divided into control and experimental groups containing 51 and 52 patients, respectively. In the control group, the drug was administered according to the operating procedure for iodine-131 treatment of thyroid carcinoma. In the experimental group, CQI was adopted to manage the radiation safety of care in addition to the conventional iodine-131 thyroid cancer treatment procedures. We also improved radiation safety protection measures prior to drug administration, developed a flow chart of drug administration for patients, established a patient preview of the drug administration process, and enhanced health education and psychological intervention for the patient. Results: The environmental radiation around the drug delivery window was reduced (P< 0.05), total duration of exposure to the radiation was shortened (P< 0.05), drug drop rate was decreased to 0%, and patient satisfaction was improved in the experimental group compared to the control group. Conclusions: Application of CQI for management of radiation safety when treating thyroid cancer withiodine-131 can improve patient treatment, quality of care, and satisfaction and reduce radiation pollution in the surrounding environment and radiation injury to staff.

Cervical spinal cord injury without fracture and dislocation (CSCIWFD) is a severe disabling injury with acute illness, slow recovery, and many complications that significantly increase patients’ pain and the difficulty of disease management. We introduce the nursing care of a 52-year-old man with multiple complications of CSCIWFD caused by a car accident. Nursing mainly focused on posture and safety, nutritional support, respiratory management, limb function training, medication, psychological nursing, and actively preventing and managing complications such as central fever, obstinate hyponatremia, klebsiella pneumoniae infection, venous thrombosis, and pressure ulcers, among others. The nurses closely observed the patient's condition, strictly managed his 24-hour access volume, and regularly evaluated his treatment and nursing effects. In addition to superior treatment and nursing, we should invite multidisciplinary cooperation for diagnosis and treatment and encourage family members to participate in disease management, which improves patient’s later functional recovery and his quality of life. After 53 days of hospitalization, the patient’s condition gradually stabilized, he and his family members were satisfied with the treatment and nursing results, and he was transferred to another hospital for rehabilitation. The prognosis of patients with CSCIWFD is closely related to the quality of nursing. Personalized comprehensive care is of great significance to reduce and timely control complications and promote patients' recovery.

Background: Catheter failure means that the arterial catheter loses its original real-time and accurate function of monitoring the blood pressure due to various causes, such as blockage, buckling, displacement of the catheter, edema, bleeding, and oozing of the punctured limb requiring catheter removal. However, there have been few studies about the catheter failure and its related influencing factors. The present study aims to explore the status and influencing factors of failed ultrasound-guided invasive arterial catheterization in ICU patients. Methods: 264 patients who underwent ultrasound-guided invasive arterial catheterization from January 2019 to December 2019 were analyzed retrospectively to observe the status and failure of the arterial catheterization. Statistical analysis was performed using SPSS22.0 software. Results: Among the 264 cases with invasive arterial catheterization, 59 cases had catheter failure, the incidence was 22.34%. 15. Impact of the Internet+ Transitional Care on Knowledge, Attitudes, and Practice of Family Caregivers of Patients with Alzheimer’s Disease: A Randomized Controlled Trial

Purpose: To determine the impact of the internet+ transitional care on knowledge, attitudes, and practice (KAP) of family caregivers of patients with Alzheimer’s disease (AD) and analyze the practicability of the internet+ Medical Messenger and WeChat network support platforms. Background: The number of AD patients in China has exceeded 10 million, ranking first in the world. Home care is the main way of care for AD patients. We use the Internet platform to implement “online” combined with “offline” continuous care for AD patients and home caregivers, and provide personalized health education and guidance, so that caregivers can easily master disease-related knowledge, establish positive care beliefs, improve care ability, and promote the improvement of patients’ caregivers’ knowledge, attitude and practice. Method: The family caregivers of 77 patients with AD were randomly divided into two groups (intervention group [40 patients] and control group [37 patients]). The intervention group adopted the internet + transitional care, while the control group adopted traditional transitional care. The KAP of the two groups of family caregivers before intervention, 3 months after intervention, and 6 months after intervention were analyzed. Results: The KAP scores of the 77 family caregivers before intervention were 17.39 ± 2.92, 33.42 ± 4.77, and 57.65 ± 13.84. There was no significant difference in the knowledge scores in the care (t=1.89, P=0.062) and symptom (t=1.78, P=0.078) dimensions 3 months after intervention, while there was a significant difference in the other KAP dimensions (P < 0.05). Conclusions: The internet + transitional care improves the level to of KAP among family caregivers of patients with AD and the effect was proportional to time.

16. The Application of a New Type of Catheter-Specific Pants for Patients with Indwelling Catheter After Transurethral Prostate Resection

To explore the new catheter-special pants design and the application for patients with indwelling catheter after transurethral prostate resection, 156 patients after transurethral prostate resection were divided into two groups randomly: experimental group wearing newly designed pants and control group wearing normal pants after transurethral prostate resection; to compare the state of restlessness and the times patients tearing the catheter secondary fixation tape and pulled out personally unplanned. Experimental group wearing newly designed pants showed only one case of catheter pulled out, none of fixation tape was teared, while in the control group, 9 cases of catheter were pulled out personally, 21 cases of tape teared, causing some degrees of urethral injury, delayed the retention time of catheter. There were significant differences between the two groups. The application of newly designed pants shielding the catheter decreases the incidence of catheter pullout after transurethral prostate resection surgery, avoids urethral injury and increased the retention time, improves the functional recovery with no increased cost, which is worth promoting the application in ordinary clinical nursing.
