Abstract

A limited ileocaecal resection is the most frequently performed procedure for ileocaecal CD and different anastomotic configurations and techniques have been described. This manuscript audited the different anastomotic techniques used in a national study and evaluated their influence on postoperative outcomes following ileocaecal resection for primary CD. This is a retrospective, multicentre, observational study promoted by the Italian Society of Colorectal Surgery (SICCR), including all adults undergoing elective ileocaecal resection for primary CD from June 2018 May 2019. Postoperative morbidity within 30days of surgery was the primary endpoint. Postoperative length of hospital stay (LOS) and anastomotic leak rate were the secondary outcomes. 427 patients were included. The side to side anastomosis was the chosen configuration in 380 patients (89%). The stapled anastomotic (n = 286; 67%), techniques were preferred to hand-sewn (n = 141; 33%). Postoperative morbidity was 20.3% and anastomotic leak 3.7%. Anastomotic leak was independent of the type of anastomosis performed, while was associated with an ASA grade ≥ 3, presence of perianal disease and ileocolonic localization of disease. Four predictors of LOS were identified after multivariate analysis. The laparoscopic approach was the only associated with a reduced LOS (p = 0.017), while age, ASA grade ≥ 3 or administration of preoperative TPN were associated with increased LOS. The side to side was the most commonly used anastomotic configuration for ileocolic reconstruction following primary CD resection. There was no difference in postoperative morbidity according to anastomotic technique and configuration. Anastomotic leak was associated with ASA grade ≥ 3, a penetrating phenotype of disease and ileo-colonic distribution of CD. Abstract

Background and Aims: The Diverticular Inflammation and Complication Assessment (DICA) endoscopic classification of diverticulosis and diverticular disease (DD) is currently available. It scores severity of the disease as DICA 1, DICA 2 and DICA 3. Our aim was to assess the agreement on this classification in an international endoscopists community setting. Methods: A total of 96 doctors (82.9% endoscopists) independently scored a set of DD endoscopic videos. The percentages of overall agreement on DICA score and a free-marginal multirater kappa (κ) coefficient were reported as statistical measures of interrater agreement. Results: Overall agreement in using DICA was 91.8% with a free-marginal kappa of 88% (95% CI 80-95). The overall agreement levels were: DICA 1, 85.2%; DICA 2, 96.5%; DICA 3, 99.5%. The free marginal κ was: DICA 1 = 0.753, DICA 2 = 0.958, DICA 3 = 0.919. The agreement about the main endoscopic items was 83.4% (k 67%) for diverticular extension, 62.6% (k 65%) for number of diverticula for each district, 86.8% (k 82%) for presence of inflammation, and 98.5 (k 98%) for presence of complications. Conclusions: The overall interrater agreement in this study ranges from good to very good. DICA score is a simple and reproducible endoscopic scoring system for diverticulosis and DD.

The Italian project “INNOVARE - Innovative plants for distributed poly-generation by residual biomass”, funded by the local Ministry of Economic Development (MISE), involves the synergic activities of research institutes and industrial companies to pursue the aim of improving micro-cogeneration technologies fueled by biomass, in order to provide effective solutions of short-range biomass-to-energy chains, characterized by high overall efficiency and flexibility. A place for operational demonstration is chosen in the Municipality of Laurino, in the Cilento National Park, located in the Campania region, in the South of Italy. Main problems today still hindering a sustainable use of energy systems powered by biomass in various contexts are related the resource availability over a continuous time scale and to the necessity of extending their operating range in the absence of faults, to assure reliability. Within the project INNOVARE, a micro combined heat and power (mCHP) unit produced and marketed by the Italian Company CMD, the CMD ECO20 system, made of a gasifier, a syngas cleaning system and a spark ignition (SI) internal combustion engine (ICE) working as a co-generator, is chosen as the case study to assess the whole study. The availability of local biomasses is established by studying the agro-industrial production and by identifying forest areas to be properly managed through an original approach using the satellite location system based on the microwave technology. Woodchip from wood maintenance and olive-pomace from oil mills are also identified as possible feedstock for the system. A detailed experimental characterization of the selected cogeneration system is performed in order to identify its main inefficiencies. Numerical models of each component, validated against experimental data, are developed and used to optimize the performance of the whole conversion process. An improvement of the 5.8% on the electrical power is reached by identifying the optimal spark timing strategy. The waste heat recovery loop is modified in order to have at disposal the excess heat when the request for thermal load gets lower than the production capacity of the plant, with an increase in reliability even in the absence of thermal users. 9. Assessment of the impact of ship emissions on the air quality in Naples

Abstract

The paper report the results of a monitoring campaign of SO2, NO2 and Benzene carried out in the port of Naples and inside the nearby urban canopy from 20th January to 8th March 2016. The activity is a continuation of a previous monitoring campaign realized by the authors in 2012. About 40 passive samplers were placed and average concentration in the period for the three pollutants was evaluated. Concentration levels observed are lower than limit values established by European directives both for SO2 and NO2. Benzene in some place of measure exceeded the limit value (5 ?g/m3) that is fixed, however, as annual average. Results have been interpreted by an interpolation algorithm using the software SURFER® obtaining contour maps that show significant differences among the three pollutants. In fact, maximum levels of SO2 occur inside the harbour area, those of NO2 inside the urban area while Benzene maximum average concentrations in the period occur at the boundary between port and urban canopy. This result has been interpreted on the basis of the occurrences of wind direction and speed measured in the same observation period.

12. Performances and Emissions of a Small Size Diesel Powered Generator Fueled with Vegetable Oil and its Water-in Emulsions

Abstract

Diesel engines are worldwide used in the transport, electricity generation and shaft power and they burn oil products which can be partially or totally replaced by alternative fuels, such as vegetable oils. Vegetable oils have physical characteristics close to those of diesel oil and as they are of plant origin, they have the potential to significantly reduce CO2 emissions into the atmosphere. Nowadays fuelling compression ignition engine with vegetable oil is a feasible solution, with a relevant number of working plants, mainly for medium size electric generators; not the same can be told about small size diesel generators. Few marine propulsion examples are also reported. In this paper, a rapeseed oil and some its water-in emulsions, in different percentage 5, 10 and 15% v/v, have been fuelled in a small size Diesel engine used as power generator in a co-generation system. Tests were conducted in a naturally aspirated Scania DC09 direct injection four-stroke 250kW, 5-cylinders, 9.3 dm3 displacement Diesel engine from low load to 120kW as load conditions. Emulsions were produced on site by a device that adds steam to vegetable oil, so continuously feeds the engine with emulsified oil at about 60°C; tests were also carried out with sample emulsions made offline with an ultrasonic device. Gaseous (CO, HC, NO/NOx, CO2) and particulate emissions have been characterized in steady state conditions. CO, HC and particulate emissions are strongly reduced by fuelling vegetable oil emulsions compared to commercial diesel fuel. NOx emission are not significantly affected, rather a small increase is observed in some conditions; authors believe that an engine optimized for reducing NOx with diesel oil requires a different injection tuning, not feasible during the experiments.

Abstract

Emissione stagionale di uova di parassiti gastrointestinali in cinghiali (Sus scrofa) e in mufloni (Ovis ammon) di un’area mediterranea (Italia centrale). E' stata studiata l'emissione stagionale di uova (o oocisti) di parassiti nelle feci di ungulati selvatici in una zona mediterranea protetta, il Parco dei Monti Livornesi (Livorno, Toscana, Italia Centrale). Per un anno sono stati raccolti mensilmente campioni di feci di Cinghiale (Sus scrofa) e di Muflone (Ovis ammon). Gli andamenti osservati di emissione di uova sono stati analizzati tenendo conto delle variazioni di temperatura e piovosità stagionali, del ciclo biologico e della strategia di sopravvivenza dei parassiti, e delle condizioni sanitarie dell'ospite. E’ risultato che in una zona mediterranea come quella considerata i picchi di emissione di uova appaiono in mesi differenti in relazione alla biologia e alle strategie di sopravvivenza dei diversi parassiti. Seasonal egg (or oocyst) output of gastro-intestinal parasites of wild ungulates was studied in a Mediterranean protected area, the Monti Livornesi Park (Livorno, Tuscany region, Central Italy). Samples of faeces of wild boars (Sus scrofa) and mouflons (Ovis ammon) were collected monthly for one year. The observed trends of egg output were analysed taking into account seasonal variations of temperature and rainfall, life-cycle and survival strategy of parasites, and health condition of hosts. In our Mediterranean study area, the peaks of egg output appear in different months according to different biology and survival strategies of parasites.

Abstract

Purpose To evaluate in a prospective study the efficacy of intrasurgical autologous plasmin enzyme (APE) in producing posterior vitreous detachment. Design Consecutive, interventional case series. Methods A group of 12 eyes of 11 patients with diabetic macular edema secondary to posterior vitreous cortex contraction was considered. A quantity of 0.1- to 0.2-ml containing 0.8 to 1.2 IU of APE prepared by our Coagulation Service was injected into the vitreous body 25 minutes before surgery. The efficacy of the APE was subjectively evaluated. A control group of 10 eyes with the same clinical characteristics underwent the same surgery without APE injection. Complete eye examinations, including optical coherence tomography, were performed on all patients before surgery and during the 1-year follow-up period. Results During surgery in the APE-treated group, the posterior vitreous cortex was judged adherent in three cases, partially detached in six cases, and totally detached in three cases. In two cases a complete collapse of the vitreous body was observed. During surgery in the non–APE-treated group, the posterior vitreous cortex was judged still adherent in nine of 10 eyes and partially detached in 1 eye. Comparing the postoperative results between the APE-treated group and the non-APE-treated group, we found no significant differences in final postoperative retinal thickness (P = .2552), whereas we found a significant difference in final visual acuity (P = .0121). Conclusions Autologous plasmin enzyme was useful in inducing a pharmacologic posterior vitreous detachment and in facilitating surgery. It did not seem to interfere with the final retinal thickness, and it ameliorates the final visual acuity.
