2. Morphometric analysis of the optic canal and the superior orbital fissure in a Brazilian sample – study in CT scans

The posterior wall of the orbit is composed by the sphenoid bone and exhibits the optic canal (OC) and the superior orbital fissure (SOF). The comprehensive knowledge of anatomical and morphometric observations of OC and SOF is vital for an accurate diagnosis and management of local pathology. The aim of this study was to conduct a morphometric analysis of the OC and the SOF in CT scans in a Brazilian population. A total of 40 computed tomography (CT) scans of dry human skulls were used (20 males and 20 females). The images were submitted to a segmentation in which the bony structures of interest in the orbit were selected. A three-dimensional reconstruction of the region and the measurements of the perimeter (mm) of the SOF and the volume (mm3) of the OC were performed. The statistical analysis was performed to verify if there was a difference in sex on each side for each anatomical structure. Regarding the OC, for the left side, there was a statistical difference between the sexes. For the SOF, neither the right side nor the left side showed statistical difference between the sexes. The present study showed new data about anatomical structures of the human orbit, bringing relevant knowledge for surgical and diagnostic procedures in the region. Especially for those anatomical structures evaluated that allow the passage of blood vessels and nerves, specific knowledge of their dimensions in different populations is valuable to avoid injuries during procedures in the orbital region.

Background: We assessed the efficacy and safety of mechanical thrombectomy (MT) in adult stroke patients with anterior circulation large vessel occlusion presenting in the late time window not fulfilling the DEFUSE-3 (Thrombectomy for Stroke at 6 to 16 Hours With Selection by Perfusion Imaging trial) and DAWN (Thrombectomy 6 to 24 Hours After Stroke With a Mismatch Between Deficit and Infarct trial) inclusion criteria. Methods: Cohort study of adults with anterior circulation large vessel occlusion admitted between 6 and 24 hours after last-seen-well at 5 participating Swiss stroke centers between 2014 and 2021. Mismatch was assessed by computer tomography or magnetic resonance imaging perfusion with automated software (RAPID or OLEA). We excluded patients meeting DEFUSE-3 and DAWN inclusion criteria and compared those who underwent MT with those receiving best medical treatment alone by inverse probability of treatment weighting using the propensity score. The primary efficacy end point was a favorable functional outcome at 90 days, defined as a modified Rankin Scale score shift toward lower categories. The primary safety end point was symptomatic intracranial hemorrhage within 7 days of stroke onset; the secondary was all-cause mortality within 90 days. Results: Among 278 patients with anterior circulation large vessel occlusion presenting in the late time window, 190 (68%) did not meet the DEFUSE-3 and DAWN inclusion criteria and thus were included in the analyses. Of those, 102 (54%) received MT. In the inverse probability of treatment weighting analysis, patients in the MT group had higher odds of favorable outcomes compared with the best medical treatment alone group (modified Rankin Scale shift: acOR, 1.46 [1.02–2.10]; P =0.04) and lower odds of all-cause mortality within 90 days (aOR, 0.59 [0.37–0.93]; P =0.02). There were no significant differences in symptomatic intracranial hemorrhage (MT versus best medical treatment alone: 5% versus 2%, P =0.63). Conclusions: Two out of 3 patients with anterior circulation large vessel occlusion presenting in the late time window did not meet the DEFUSE-3 and DAWN inclusion criteria. In these patients, MT was associated with higher odds of favorable functional outcomes without increased rates of symptomatic intracranial hemorrhage. These findings support the enrollment of patients into ongoing randomized trials on MT in the late window with more permissive inclusion criteria.

Currently, one of the most important trends in concrete technology is the use of inert fine particles (fillers) to decrease cement content, increase packing in the paste and increase flowability, with gains in technological, environmental and cost aspects. The advance in dispersion technology (using superplasticisers) is one of the technical issues that allows pastes to be mixed without the expected increase of water content – which decreases compressive strength – caused by the filler's high surface area. However, the use of no more than the optimal dispersant (superplasticiser) content (lowest content that provides the lowest viscosity) is crucial owing to cost constraints. This paper evaluates the possibility of predicting optimal dispersant content in cement–limestone filler systems from the optimal dispersant content previously measured in the pure raw materials. Owing to the need for only a few sets of tests, this can optimise the design timing and accuracy of efficient pastes for lower binder concretes.

5. Endovascular therapy outcome in isolated posterior cerebral artery occlusion strokes: A multicenter analysis of the Swiss Stroke Registry

Purpose: There is little data on the safety and efficacy of endovascular treatment (EVT) in comparison with intravenous thrombolysis (IVT) in acute ischemic stroke due to isolated posterior cerebral artery occlusion (IPCAO). We aimed to investigate the functional and safety outcomes of stroke patients with acute IPCAO treated with EVT (with or without prior bridging IVT) compared to IVT alone. Methods: We did a multicenter retrospective analysis of data from the Swiss Stroke Registry. The primary endpoint was overall functional outcome at 3 months in patients undergoing EVT alone or as part of bridging, compared with IVT alone (shift analysis). Safety endpoints were mortality and symptomatic intracranial hemorrhage. EVT and IVT patients were matched 1:1 using propensity scores. Differences in outcomes were examined using ordinal and logistic regression models. Findings: Out of 17,968 patients, 268 met the inclusion criteria and 136 were matched by propensity scores. The overall functional outcome at 3 months was comparable between the two groups (EVT vs IVT as reference category: OR = 1.42 for higher mRS, 95% CI = 0.78–2.57, p = 0.254). The proportion of patients independent at 3 months was 63.2% in EVT and 72.1% in IVT (OR = 0.67, 95% CI = 0.32–1.37, p = 0.272). Symptomatic intracranial hemorrhages were overall rare and present only in the IVT group (IVT = 5.9% vs EVT = 0%). Mortality at 3 months was also similar between the two groups (IVT = 0% vs EVT = 1.5%). Conclusion: In this multicenter nested analysis, EVT and IVT in patients with acute ischemic stroke due to IPCAO were associated with similar overall good functional outcome and safety. Randomized studies are warranted.

6. A Perioperative Intervention to Prevent and Treat Emergence Delirium at a Veterans Affairs Medical Center

Emergence delirium (ED) is a temporary condition associated with a patient awakening from an anesthetic and/or adjunct agent (e.g., sedatives and analgesics). During the condition, patients risk harming themselves or staff by engaging in dangerous behavior, which may include thrashing, kicking, punching, and attempting to exit the bed/table. A multidisciplinary team at Veterans Affairs Pittsburgh Healthcare System (VAPHS) developed and implemented a multicomponent intervention to reduce the severity and occurrence of ED. The intervention consists of a training component and 21 clinical components. The 21 clinical components are implemented on a patient-by-patient basis and include routine screening for risk factors, enhanced communication among staff, adjusting the environment, following a specific medication strategy, and application of manual restraint (hands-on). The authors provide 15 online Supplemental Materials (S1–S15) to promote replication of the intervention. To our knowledge, this is the first manuscript that describes this type of multicomponent intervention in sufficient detail to allow others to replicate it. Following implementation of the intervention at VAPHS, perioperative staff reported that they observed a substantial reduction in the occurrence and severity of ED, ED-related patient and staff injuries, and ED-related loss of intravenous access and airway patency. Despite staff’s reported success of the intervention, rigorous research is needed to evaluate the efficacy of the intervention.

Background and Objectives: The European Medicine Agency extended the use of Comirnaty, Spikevax, and Nuvaxovid in paediatrics; thus, these vaccines require additional real-world safety evidence. Herein, we aimed to monitor the safety of COVID-19 vaccines through Covid-19 Vaccine Monitor (CVM) and EudraVigilance surveillance systems and the published pivotal clinical trials. Methods: In a prospective cohort of vaccinees aged between 5 and 17 years, we measured the frequency of commonly reported (local/systemic solicited) and serious adverse drug events (ADRs) following the first and second doses of COVID-19 vaccines in Europe using data from the CVM cohort until April 2022. The results of previous pivotal clinical trials and data in the EudraVigilance were also analysed. Results: The CVM study enrolled 658 first-dose vaccinees (children aged 5–11 years; n = 250 and adolescents aged 12–17 years; n = 408). Local/systemic solicited ADRs were common, whereas serious ADRs were uncommon. Among Comirnaty first and second dose recipients, 28.8% and 17.1% of children and 54.2% and 52.2% of adolescents experienced at least one ADR, respectively; injection-site pain (29.2% and 20.7%), fatigue (16.1% and 12.8%), and headache (22.1% and 19.3%) were the most frequent local and systemic ADRs. Results were consistent but slightly lower than in pivotal clinical trials. Reporting rates in Eudravigilance were lower by a factor of 1000. Conclusions: The CVM study showed high frequencies of local solicited reactions after vaccination but lower rates than in pivotal clinical trials. Injection-site pain, fatigue, and headache were the most commonly reported ADRs for clinical trials, but higher than spontaneously reported data.

Metabolic demands of skeletal muscle are substantial and are characterized normally as highly flexible and with a large dynamic range. Skeletal muscle composition (e.g., fiber type and mitochondrial content) and metabolism (e.g., capacity to switch between fatty acid and glucose substrates) are altered in obesity, with some changes proceeding and some following the development of the disease. Nonetheless, there are marked interindividual differences in skeletal muscle composition and metabolism in obesity, some of which have been associated with obesity risk and weight loss capacity. In this review, we discuss related molecular mechanisms and how current and novel treatment strategies may enhance weight loss capacity, particularly in diet-resistant obesity.

System design is an essential subject taught in information systems and has become a core course in its curriculum. Unified modelling language (UML) has been broadly adopted, and it is common to support the system design process using different diagrams. Each diagram serves a purpose by focusing on a specific part of a particular system. Design consistency ensures a seamless process, as the diagrams are generally interrelated. However, creating a well-designed system takes a lot of work, especially for university students with work experience. To overcome this challenge, aligning the concepts across diagrams is essential, which can help achieve better consistency and management of the design system, especially in an educational setting. This article is an extension of our previous work, as we have discussed a simple scenario of Automated teller machines to demonstrate the alignment concepts between UML diagrams. From a more technical perspective, the current contribution provides a Java program that aligns concepts by converting text-based use cases to text-based sequence diagrams. Then, the text is transformed in PlantUML to generate its graphical representation. The developed alignment tool is expected to contribute to helping students and instructors during the system design phases to be more consistent and practical. Limitations and future work are presented.

Although knowledge management relying on data from social networks has become an integral part of common practices, there needs to be a well-defined body of knowledge that explicitly addresses the process and the value generated. Sustainable knowledge management practices, which promote responsible and ethical knowledge sharing between different stakeholders, can also be facilitated through social media. This can foster a culture of continuous learning and innovation while considering the social implications of knowledge sharing. The main goal of this study is to critically and holistically discuss the impact of social media analysis in the knowledge management process holistically and maximize its value in a given context. More concretely, we conducted a systematic literature review (2012–2022) based on the PRISMA guidelines. We first approached the ideal phases of the knowledge management process and then discussed key issues and challenges from an application perspective. Overall, the study points out the positive impact of social network analysis on knowledge sharing, creativity and productivity, knowledge formulation, building trust, and cognitive capital. Additionally, value is provided in knowledge acquisition by simplifying and massively gathering information, reducing uncertainty and ambiguity, and organizing knowledge through storage, retrieval, and classification practices. At an application level, such knowledge may improve the quality of services and encourage creativity. Finally, this study analyzed specific domains, such as healthcare, marketing, politics, tourism, and event management, focusing on the potential and added value.

Grout is a material usually applied to enhance the mechanical properties of structural masonry. As such, the demand for grout has been growing parallel to the demand for structural masonry. Without guidelines for grout mixing design, these products are often prepared on-site based on the same concepts used for preparing ordinary Portland cement-based concretes. Several grout properties still need to be investigated and understood due to this mixing-design approach, and consequently, the production of ecoefficient grouts is a secondary priority. Due to these obstacles, there is a demand for technical-scientific studies to aid with developing grout design guidelines. This work evaluates the rheological and hardened properties of grout compositions that simulate in-field conditions to show the inadequacy of the mixing design method and a way to understand this lack in the technology. The research investigates how the changes in the water-to-cement ratio, to increase the strength, would affect the fresh and hardened state properties of grouts for structural masonry. The grout’s mixing was done in a rheometer which made it possible to determine the rheological behaviour and parameters during mixing and under different shear conditions. Compressive strength, porosity, and air-permeability were evaluated in the hardened state. The results showed that compositions with more cement content does not necessarily reflect enhanced mechanical properties since the differences were not statistically significant, because other variables were also changed. The research findings suggest that the design method used in practice may not be appropriate and potentially result in the waste of cementitious materials and no-eco-friendly compositions. Resumo O graute é um material normalmente aplicado para melhorar as propriedades mecânicas da alvenaria estrutural. Assim, a demanda pela utilização do grautes vem crescendo paralelamente à demanda por alvenaria estrutural. Sem diretrizes específicas para dosagem, esses produtos geralmente são preparados no local com base nos mesmos conceitos para preparo de concretos comuns. Várias propriedades do graute ainda precisam ser investigadas e compreendidas como ineficiência dos parâmetros de dosagem, resultando na produção de grautes não ecoeficientes. Desta forma, existe uma demanda por estudos técnico-científicos que auxiliem no desenvolvimento de diretrizes para dosagens de grautes. Este trabalho avalia as propriedades reológicas e do endurecidas de composições de grautes que simulam condições de campo para mostrar a inadequação do método de dosagem, demonstrando uma maneira de entender essa lacuna na tecnologia. A pesquisa investiga como as mudanças na relação água/cimento, para aumentar trabalhabilidade, afetariam as propriedades de estado fresco e endurecido de grautes para alvenaria estrutural. A mistura dos grautes foi realizada em um reômetro que permitiu determinar o comportamento e os parâmetros reológicos durante a mistura e em diferentes condições de cisalhamento. A resistência à compressão, a porosidade e a permeabilidade ao ar foram as propriedades avaliadas no estado endurecido. Os resultados mostraram que composições com maior teor de cimento não necessariamente refletiram propriedades mecânicas melhores, uma vez que não houve alterações estatística, pois outras variáveis também foram alteradas. Os resultados da pesquisa sugerem que o método de projeto usado em obra pode não ser apropriado o que pode resultar no desperdício de materiais cimentícios e composições não ecoeficientes.

Distributed optimization is an essential paradigm to solve large-scale optimization problems in modern applications where big-data and high-dimensionality creates a computational bottleneck. Distributed optimization algorithms that exhibit fast convergence allow us to fully utilize computing resources and effectively scale to larger optimization problems in a myriad of areas ranging from machine learning to power systems. In this work, we introduce a new centralized distributed optimization algorithm (ECADO) inspired by an equivalent circuit model of the distributed problem. The equivalent circuit (EC) model provides a physical analogy to derive new insights to develop a fast-convergent algorithm. The main contributions of this approach are: 1) a weighting scheme based on a circuit-inspired aggregate sensitivity analysis, and 2) an adaptive step-sizing derived from a stable, Backward-Euler numerical integration. We demonstrate that ECADO exhibits faster convergence compared to state-of-the art distributed optimization methods and provably converges for nonconvex problems. We leverage the ECADO features to solve convex and nonconvex optimization problems with large datasets such as: distributing data for logistic regression, training a deep neural network model for classification, and solving a high-dimensional problem security-constrained optimal power flow problem. Compared to state-of-the-art centralized methods, including ADMM, centralized gradient descent, and DANE, this new ECADO approach is shown to converge in fewer iterations.

Smart Technology is a quickly and constantly evolving concept; it has different applications that cover a wide range of areas, such as healthcare, education, business, agriculture, and manufacturing. An effective application of these technologies increases productivity and performance within complex systems. On one side, trends show a lack of appeal for rural environments as people prefer to move to cities, looking for better opportunities and lifestyles. On the other side, recent studies and reports show that the attractiveness of rural areas as places with opportunities is increasing. Sustainable solutions are needed to enhance development in the rural context, and technological innovation is expected to lead and support the stability for people and organizations in rural regions. While Smart City is progressively becoming a reality and a successful model for integrating Smart Technology into different aspects of everyday life, its effective application in a rural context according to a Sustainable Development approach is not yet completely defined. This study adopts comparative and categorial content analysis to address the different applications and the specific characteristics of rural regions, which often present significant peculiarities depending on the country and the context. The main goal is to investigate and discuss how the Smart City model may be adopted and effectively applied within rural contexts, looking at major gaps and challenges. Additionally, because of the complexity of the topic, we provide an overview of the current adoption of Smart Technology in the different applications in rural areas, including farming, education, business, healthcare, and governance. The study highlights the huge difficulties in rural life and the potentiality of Smart Technology to enhance their Sustainable Development, which is still challenging. While the holistic analysis clearly points out a gap, there is no specific strategic roadmap to re-use or adapt existing models, such as Smart City. The study does not address fine-grained indicators.

- Abstract
Background Acute ischemic stroke attributed to basilar artery occlusion (BAO) results in high rates of death and significant morbidity. Endovascular thrombectomy an effective treatment for BAO, but imaging parameters that predict a favorable response to thrombectomy are not well defined. We determined which imaging parameters were associated with poor outcome in patients with BAO treated by thrombectomy. Methods We performed a retrospective cohort study of patients with BAO who underwent thrombectomy at multiple international stroke centers. All patients underwent computed tomography or magnetic resonance perfusion imaging before treatment. Clinical and imaging variables were measured and correlated to poor functional outcomes (modified Rankin scale score ≥4) after thrombectomy. Imaging variables included the following: Critical Area Perfusion Score, Posterior Circulation Alberta Stroke Program Early Computed Tomography Score, ischemic core volume (30% cerebral blood flow on computed tomography perfusion or diffusion‐weighted imaging), and volume of time to maximum >10 seconds. Clinical and imaging variables associated with poor functional outcome were assessed by a multivariable binary logistic regression analysis. Results A total of 102 patients were included in the study. Median patient age was 66.5 years (interquartile range [IQR], 55–78), median presentation National Institutes of Health Stroke Scale score was 14 (IQR, 7–23), and the median time from last seen normal was 4 hours (IQR, 1:52–9:20). Patient age (odds ratio [OR], 1.37 per 5‐year increment [95% CI, 1.08–1.72]; P=0.008), presentation National Institutes of Health Stroke Scale score (OR, 1.11 [95% CI, 1.04–1.18]; P=0.001), successful reperfusion after thrombectomy (OR, 0.03 [95% CI, 0.003–0.25]; P=0.002), Posterior Circulation Alberta Stroke Program Early Computed Tomography Score ≤6 (OR, 11.40 [95% CI, 1.73–75]; P=0.011), and Critical Area Perfusion Score >3 (OR, 26.22 [95% CI, 1.07–642]; P =0.045) independently predicted poor outcome after BAO thrombectomy. Ischemic core volume (30% cerebral blood flow) and volume of time to maximum >10 seconds did not predict poor outcome. Conclusion Age, National Institutes of Health Stroke Scale presentation, unsuccessful reperfusion, Critical Area Perfusion Score >3, and Posterior Circulation Alberta Stroke Program Early Computed Tomography Score ≤6 are independently associated with poor outcome after BAO thrombectomy.

Recent years have seen a rich literature of data-driven approaches designed for power grid applications. However, insufficient consideration of domain knowledge can impose a high risk to the practicality of the methods. Specifically, ignoring the grid-specific spatiotemporal patterns (in load, generation, and topology, etc.) can lead to outputting infeasible, unrealizable, or completely meaningless predictions on new inputs. To address this concern, this paper investigates real-world operational data to provide insights into power grid behavioral patterns, including the time-varying topology, load, and generation, as well as the spatial differences (in peak hours, diverse styles) between individual loads and generations. Then based on these observations, we evaluate the generalization risks in some existing ML works causedby ignoring these grid-specific patterns in model design and training.

Cyberthreats are an increasingly common risk to the power grid and can thwart secure grid operation. We propose to extend contingency analysis (CA) that is currently used to secure the grid against natural threats to protect against cyberthreats. However, unlike traditional N-1 or N-2 contingencies, cyberthreats (e.g., MadIoT) require CA to solve harder N-k (with k >> 2) contingencies in a practical amount of time. Purely physics-based solvers, while robust, are slow and may not solve N-k contingencies in a timely manner, whereas the emerging data-driven alternatives to power grid analytics are fast but not sufficiently generalizable, interpretable, or scalable. To address these challenges, we propose a novel conditional Gaussian Random Field-based data-driven method that is bothfast and robust. It achieves speedup by improving starting points for the physical solvers. To improve the physical interpretability and generalizability, the proposed method incorporates domain knowledge by considering the graphical nature of the grid topology. To improve scalability, the method applies physics-informed regularization that reduces the model size and complexity. With the increased penetration of renewable sources, power grids are becoming stressed due to fluctuating generation. To alleviate stress from inconsistent sources, utilities employ energy storage systems alongside renewable sources and rely on dispatching synchronous generators. However, optimal dispatch of such devices is limited by traditional AC optimal power flow methods that do not account for time-dependent constraints, such as the state of charge of energy storage systems and generator ramping constraints. Multi-period optimal power flow is proposed as a large non-convex non-linear problem to optimally dispatch and control generators and energy storage elements across multiple time periods. In this paper, we introduce a scalable, robust framework to solve multi-period optimal power flow using a differential dynamic programming scheme that makes it capable of scaling to large systems containing energy storage devices. We demonstrate the efficacy of this solution by optimizing the SyntheticUSA testcase over a set of time periods. A robust homotopy method is applied to achieve fast simulation times that can be parallelized for further improvements.

Published by Elsevier Ltd., Background: Differences in the distribution of individual-level clinical risk factors across regions do not fully explain the observed global disparities in COVID-19 outcomes. We aimed to investigate the associations between environmental and societal factors and country-level variations in mortality attributed to COVID-19 among people with rheumatic disease globally. Methods: In this observational study, we derived individual-level data on adults (aged 18-99 years) with rheumatic disease and a confirmed status of their highest COVID-19 severity level from the COVID-19 Global Rheumatology Alliance (GRA) registry, collected between March 12, 2020, and Aug 27, 2021. Environmental and societal factors were obtained from publicly available sources. The primary endpoint was mortality attributed to COVID-19. We used a multivariable logistic regression to evaluate independent associations between environmental and societal factors and death, after controlling for individual-level risk factors. We used a series of nested mixed-effects models to establish whether environmental and societal factors sufficiently explained country-level variations in death. Findings: 14 044 patients from 23 countries were included in the analyses. 10 178 (72·5%) individuals were female and 3866 (27·5%) were male, with a mean age of 54·4 years (SD 15·6). In a context of constant evolution and proliferation of AI technology, Hybrid Intelligence is gaining popularity to refer a balanced coexistence between human and artificial intelligence. On the other side, the concept has been extensively used in the past two decades to define models of intelligence involving more than one technology. This paper aims to provide (i) a concise and focused overview of the adoption of Ontology in the broad context of Hybrid Intelligence regardless of its definition and (ii) a critical discussion on the possible role of Ontology to reduce the gap between human and artificial intelligence within hybrid intelligent systems. Beside the typical benefits provided by an effective use of ontologies, at a conceptual level, the analysis conducted has pointed out a significant contribution to quality and accuracy, as well as a more specific role to enable extended interoperability, system engineering and explainable/transparent systems. On the other side, an application-oriented analysis has shown a significant role in present systems (70+% of the cases) and, potentially, in future systems. However, a proper holistic discussion on the establishment of the next generation of hybrid-intelligent environments with a balanced co-existence of human and artificial intelligence is fundamentally missed in literature. Last but not the least, there is currently a relatively low explicit focus on automatic reasoning and inference.

29. Evaluation of a stakeholder advisory board for an adolescent mental health randomized clinical trial

Introduction Community engagement in research is widely accepted as best practice, despite gaps in existing frameworks to evaluate its process, context, and impact on research. The Screening in High Schools to Identify, Evaluate, and Lower Depression (SHIELD) study evaluated the use of a school-based major depressive disorder screening tool in the identification of symptoms and treatment initiation among adolescents, and was developed, implemented, and disseminated in partnership with a Stakeholder Advisory Board (SAB). We summarize outcomes of the evaluation strategy applied through our partnership with the SAB and explore gaps in the available engagement evaluation tools for mixed stakeholder populations including youth. Methods SHIELD study SAB members (n = 13; adolescents, parents, mental health and primary care providers, and professionals from education and mental health organizations) advised on study design, implementation, and dissemination over a three-year period. Both SAB members and study team members (i.e., clinician researchers, project managers) were invited to quantitatively and qualitatively evaluate stakeholder engagement after each project year. At the conclusion of the study, SAB members and study team members were asked to evaluate the application of engagement principles in overall stakeholder engagement across the study period, using portions of the Research Engagement Survey Tool (REST). Results SAB members and study team members responded similarly when evaluating engagement process (i.e., valued on team, voice represented); means ranged from 3.9 to 4.8 out of 5 points across all three project years. Reported engagement within study-specific engagement activities (i.e., meetings, study newsletter) varied from year to year, with some discrepancy between SAB member and study team evaluations. Using REST, SAB members reported the alignment of their experience with key engagement principles the same or higher than study team members. Qualitative feedback at the conclusion of the study generally matched quantitative measures; adolescent SAB members, however, reported disengagement from stakeholder activities that was not accurately or effectively captured in evaluation strategies employed across the study period. Conclusions Challenges exist in effectively engaging stakeholders and evaluating their engagement, particularly among heterogenous groups that include youth. Evaluation gaps should be addressed through the development of validated instruments that quantify the process, context, and impact of stakeholder engagement on study outcomes. Consideration should be given to collecting parallel feedback from stakeholders and study team members to fully understand the application and execution of engagement strategy.

Introduction: Endovascular treatment (EVT), in particular mechanical thrombectomy, has dramatically improved the clinical outcome of patients with large vessels occlusion (LVO) of the anterior circulation. In contrast, the benefits of EVT cannot be applied to lacunar strokes (LS). The absence of a LVO in LS has been historically associated with a more favorable outcome, but in the new era of EVT, this assumption may not be correct anymore. We aimed to test the general assumption that LS have a better prognosis than other stroke subtypes, and analyze the outcome of LS treated with IVT as compared to strokes with LVO treated with IVT and/or mechanical thrombectomy in the multicentric Swiss Stroke Registry. Methods: retrospective propensity score matching analysis of patients from the Swiss stroke registry (SSR) with lacunar stroke treated with thrombolysis versus LVO of the medial cerebral artery treated with EVT (with or without thrombolysis). Primary endpoint was a shift analysis of mRs at 90 days after stroke. Secondary outcomes were favorable functional outcome (mRS 0-1), independence (mRS 0-2), survival with high disability (mRS 3-5) and mortality (mRS 6) at 90 days. Results: From 13'227 patients, the propensity score method matched (sex, age, NIHSS, time to treatment, prior anticoagulation, pre-stroke modified Rankin Score, mRs) 538 patients (269 in each group. There was no difference between groups in mRs shift analysis at 90 days after stroke (OR=0.99, 95%CI=0.73-1.35, p=0.952). There was also no significant difference in mRS 0-1 (59.9% vs 55.8% respectively; OR=0.79, 95%CI=0.55-1.16, p=0.75). LS+IVT patients showed a non-significant trend towards mRS 3-5 (OR=1.23, 95%CI=0.78-1.94, p=0.38), while patients treated with EVT had a slightly, non-significant, higher mortality (6.3% vs 1.9%, p=0.38). Conclusions: Lacunar strokes - which cannot directly benefit from thrombectomy therapy - when treated with IVT and matched on key prognostic variables, seem to have similar outcomes as LVO strokes treated with EVT. This observation confirms that lacunar stroke is not a benign entity and therefore suggests that reperfusion therapy should be addressed with the same intensity as LVO stroke.

We recently discovered that the expression of PRKN, a young-onset Parkinson disease-linked gene, confers redox homeostasis. To further examine the protective effects of parkin in an oxidative stress model, we first combined the loss of prkn with Sod2 haploinsufficiency in mice. Although adult prkn−/−//Sod2± animals did not develop dopamine cell loss in the S. nigra, they had more reactive oxidative species and a higher concentration of carbonylated proteins in the brain; bi-genic mice also showed a trend for more nitrotyrosinated proteins. Because these redox changes were seen in the cytosol rather than mitochondria, we next explored the thiol network in the context of PRKN expression. We detected a parkin deficiency-associated increase in the ratio of reduced glutathione (GSH) to oxidized glutathione (GSSG) in murine brain, PRKN-linked human cortex and several cell models. This shift resulted from enhanced recycling of GSSG back to GSH via upregulated glutathione reductase activity; it also correlated with altered activities of redox-sensitive enzymes in mitochondria isolated from mouse brain (e.g., aconitase-2; creatine kinase). Intriguingly, human parkin itself showed glutathione-recycling activity in vitro and in cells: For each GSSG dipeptide encountered, parkin regenerated one GSH molecule and was S-glutathionylated by the other (GSSG + P-SH $$\to$$ → GSH + P-S-SG), including at cysteines 59, 95 and 377. Moreover, parkin’s S-glutathionylation was reversible by glutaredoxin activity. In summary, we found that PRKN gene expression contributes to the network of available thiols in the cell, including by parkin’s participation in glutathione recycling, which involves a reversible, posttranslational modification at select cysteines. Further, parkin’s impact on redox homeostasis in the cytosol can affect enzyme activities elsewhere, such as in mitochondria. We posit that antioxidant functions of parkin may explain many of its previously described, protective effects in vertebrates and invertebrates that are unrelated to E3 ligase activity.

The use of pre-hydrated cement in formulation of mortar and concrete is common because there is not always effective control on the cement production, grinding, transportation and subsequent storage. This paper presents a case study on the optimization of admixture for use with prehydrated cement by assessing changes in the rheological properties of Portland cement artificially pre-hydrated. The cement was artificially pre-hydrated by exposure to relative humidity (RHs) of 90%, in an environment of NH4Cl saturated solution. Additionally, the cement pastes were evaluated with and without superplasticizer. Stepped flow test using a parallel-plate geometry was the method choose to evaluate the rheological behavior, apparent viscosity, yield stress and hysteresis area of each paste at the early age. Oscillatory rheometry was conducted to evaluate the storage modulus (G’), comparing the consolidation with the hydration kinetics obtained by calorimetric evaluation of the same suspensions. Initial rheometer results indicate that the pre-hydrated sample presents lower level of shear stress than the reference sample, because of the higher reactivity of the non-pre-hydrated sample. Resumo A utilização de cimento pré-hidratado na formulação de argamassas e concretos é comum, pois nem sempre há um controle efetivo na produção do cimento, moagem, transporte e posterior armazenamento. Este trabalho apresenta um estudo de caso sobre a otimização de aditivo para uso com cimento pré-hidratado, avaliando as mudanças nas propriedades reológicas do cimento Portland pré-hidratado artificialmente. O cimento foi pré-hidratado artificialmente por exposição à umidade relativa (URs) de 90%, em ambiente de solução saturada de NH4Cl. Adicionalmente, as pastas de cimento foram avaliadas com e sem superplastificante. O teste de fluxo escalonado usando uma geometria de placas paralelas foi o método escolhido para avaliar o comportamento reológico, viscosidade aparente, tensão de escoamento e área de histerese de cada pasta em idade precoce. A reometria oscilatória foi realizada para avaliar o módulo de armazenamento (G'), comparando a consolidação com a cinética de hidratação obtida pela avaliação calorimétrica das mesmas suspensões. Os resultados iniciais do reômetro indicam que a amostra pré-hidratada apresenta menor nível de tensão de cisalhamento do que a amostra de referência, devido à maior reatividade da amostra não pré-hidratada.

The ATLAS detector is installed in its experimental cavern at Point 1 of the CERN Large Hadron Collider. During Run 2 of the LHC, a luminosity of $\mathcal{L}=2\times 10^{34}\mathrm{cm}^{-2}\mathrm{s}^{-1}$ was routinely achieved at the start of fills, twice the design luminosity. For Run 3, accelerator improvements, notably luminosity levelling, allow sustained running at an instantaneous luminosity of $\mathcal{L}=2\times 10^{34}\mathrm{cm}^{-2}\mathrm{s}^{-1}$, with an average of up to 60 interactions per bunch crossing. The ATLAS detector has been upgraded to recover Run 1 single-lepton trigger thresholds while operating comfortably under Run 3 sustained pileup conditions. A fourth pixel layer 3.3 cm from the beam axis was added before Run 2 to improve vertex reconstruction and $b$-tagging performance. New Liquid Argon Calorimeter digital trigger electronics, with corresponding upgrades to the Trigger and Data Acquisition system, take advantage of a factor of 10 finer granularity to improve triggering on electrons, photons, taus, and hadronic signatures through increased pileup rejection. The inner muon endcap wheels were replaced by New Small Wheels with Micromegas and small-strip Thin Gap Chamber detectors, providing both precision tracking and Level-1 Muon trigger functionality. Tile Calorimeter scintillation counters were added to improve electron energy resolution and background rejection. Upgrades to Minimum Bias Trigger Scintillators and Forward Detectors improve luminosity monitoring and enable total proton-proton cross section, diffractive physics, and heavy ion measurements. These upgrades are all compatible with operation in the much harsher environment anticipated after the High-Luminosity upgrade of the LHC and are the first steps towards preparing ATLAS for the High-Luminosity upgrade of the LHC. We introduce a new workflow for unconstrained optimization whereby objective functions are mapped onto a physical domain to more easily design algorithms that are robust to hyperparameters and achieve fast convergence rates. Specifically, we represent optimization problems as an equivalent circuit that are then solved solely as nonlinear circuits using robust solution methods. The equivalent circuit models the trajectory of component-wise scaled gradient flow problem as the transient response of the circuit for which the steady-state coincides with a critical point of the objective function. The equivalent circuit model leverages circuit domain knowledge to methodically design new optimization algorithms that would likely not be developed without a physical model. We incorporate circuit knowledge into optimization methods by 1) enhancing the underlying circuit model for fast numerical analysis, 2) controlling the optimization trajectory by designing the nonlinear circuit components, and 3) solving for step sizes using well-known methods from the circuit simulation. We first establish the necessary conditions that the controls must fulfill for convergence. We show that existing descent algorithms can be re-derived as special cases of this approach and derive new optimization algorithms that are developed with insights from a circuit-based model. The new algorithms can be designed to be robust to hyperparameters, achieve convergence rates comparable or faster than state of the art methods, and are applicable to optimizing a variety of both convex and nonconvex problems.

Background We report here for the first time, a comprehensive characterization of biological and clinical features of early-stage triple negative Invasive Lobular Carcinomas(TN-ILCs) Methods We analyzed all consecutive patients with early-stage TN-ILC operated at two reference cancer-centers between 1994 and 2012. Primary objective was to assess the invasive disease-free survival(iDFS). Co-primary objective was to assess biological features of TN-ILCs, including molecular intrinsic subtypes based on PAM-50 assay, expression of androgen receptor (AR) and mutational status of ERBB2-gene. Additionally, DNA mutational status of an independent cohort of 45 TN-ILCs from three databases were analyzed, to confirm mutations in ERBB2-gene and to identify other recurrently mutated genes. Results Among 4152 ILCs, 74(1.8%) were TN and were analyzed. The iDFS at 5 and 10 years of FUP were 50.4%(95%CI,38.0–61.6) and 37.2%(95%CI,25.5–48.8), respectively. The molecular subtype was defined through PAM50-classifier for 31 out of 74 TN-ILCs: 48% were Luminal-A(15/31), 3% luminal-B(1/31), 32% HER2-enriched (10/31), and only 16% basal-like(5/31). Luminal tumors expressed AR more frequently than non-luminal tumors (AR≥1% in 94% of luminal tumors versus 53% in non-luminal tumors; p-value = 0.001). 20% of TN-ILCs analyzed(7/35), harbored a pathogenetic and actionable mutation in the ERBB2-gene. Analysis of the independent cohort of 45 TN-ILCs from three different databases, confirmed similar percentage of pathogenetic and actionable mutations in ERBB2-gene(20%; 9/45). Among the top 10 molecular pathways significantly enriched for recurrently mutated genes in TN-ILCs(FDR, Highlights • Triple-negative ILCs are distinct from both TN-IDCs and endocrine-responsive ILC. • TN-ILCs are enriched for actionable mutations in ErbB2 gene and DNA Damage Response genes. • The Luminal Androgen Receptor (LAR) is the most prevalent subtype in TN-ILCs.

Introduction: Several studies in the adult population have shown that obesity is an independent risk factor for elevated intraocular pressure (IOP), whereas data in the paediatric population are sparse and controversial. The purpose of the present study is to investigate the relationship between body mass index (BMI), blood pressure (BP), and IOP in healthy school children. Methods: The survey was conducted among a random sample of 8-year-old Italian students. Data were collected on their health status and behaviours related to obesity (physical activity, food and drinking habits, etc.). Physical examinations, conducted at school, included measurements of height, weight, BP, and IOP. Results: Five hundred and seventy-six subjects were recruited (92.8% response rate); 42.4% were overweight or obese, 58.9% consumed inadequate daily servings of fruit and vegetables, and 87.5% were involved in sedentary activities. Elevated BP/hypertension (HTN) affected 3.6% and high IOP was revealed in 12.5% of the children. In the multivariate analysis, elevated BP/HTN was the only significant determinant of ocular HTN (OR 5.36, 95% CI 1.95–14.73, p = 0.001). Conclusions: Our results show that high IOP affects 12.5% of 8-year-old school children and appears to be associated with high BP related to a high BMI.

41. Perioperative Delirium/Agitation Associated With the Use of Anesthetics and/or Adjunct Agents: A Study of Patient Behaviors, Injuries, and Interventions to Mitigate Risk

Anesthetics and adjunct agents have a long history of being associated with patients engaging in delirious or agitated behavior in a perioperative setting. Prior to this study, few have explored the topic with a focus on safety for both the patient and staff. We explored the Pennsylvania Patient Safety Reporting System (PA-PSRS) database for event reports to identify bouts of delirium/ agitation associated with anesthetics and/or adjunct agents that occurred during the pre-, intra-, or postoperative period. We identified 97 event reports from 63 healthcare facilities over a two-year period. Patients’ ages ranged from 1 to 91 years and 66% of the patients were reported as male. Also, 8% of the delirium/agitation occurred preoperatively, 8% intraoperatively, and 84% postoperatively. Across all three operative periods, 62% of the reports described dangerous/nonviolent behavior and 26% described dangerous/violent behavior. Additionally, 40% of the event reports described one or more patient injuries (e.g., cardiopulmonary arrest, asphyxiation, hematoma, prolapse/dehiscence, progressive ischemia) and 36% of the patients required additional healthcare services or monitoring (e.g., intra- or interfacility transfer, additional surgical procedure). Finally, 54% of the event reports described patient behavior that created an immediate and high risk of staff harm. Overall, the current study provides novel insight into how delirium/agitation has varying safety implications depending on the operative period. We encourage readers to review Table 5, which proposes a four-phase intervention package to prevent, treat, and de-escalate bouts of delirium/agitation.

OBJECTIVE Perfusion imaging identifies anterior circulation stroke patients who respond favorably to endovascular thrombectomy (ET), but its role in basilar artery occlusion (BAO) is unknown. We hypothesized that BAO patients with limited regions of severe hypoperfusion (time to reach maximum concentration in seconds [Tmax] > 10) would have a favorable response to ET compared to patients with more extensive regions involved. METHODS We performed a multicenter retrospective cohort study of BAO patients with perfusion imaging prior to ET. We prespecified a Critical Area Perfusion Score (CAPS; 0-6 points), which quantified severe hypoperfusion (Tmax > 10) in cerebellum (1 point/hemisphere), pons (2 points), and midbrain and/or thalamus (2 points). Patients were dichotomized into favorable (CAPS ≤ 3) and unfavorable (CAPS > 3) groups. The primary outcome was a favorable functional outcome 90 days after ET (modified Rankin Scale = 0-3). RESULTS One hundred three patients were included. CAPS ≤ 3 patients (87%) had a lower median National Institutes of Health Stroke Scale score (NIHSS; 12.5, interquartile range [IQR] = 7-22) compared to CAPS > 3 patients (13%; 23, IQR = 19-36; p = 0.01). Reperfusion was achieved in 84% of all patients, with no difference between CAPS groups (p = 0.42). Sixty-four percent of reperfused CAPS ≤ 3 patients had a favorable outcome compared to 8% of nonreperfused CAPS ≤ 3 patients (odds ratio [OR] = 21.0, 95% confidence interval [CI] = 2.6-170; p 3 patients had a favorable outcome, regardless of reperfusion. In a multivariate regression analysis, CAPS ≤ 3 was a robust independent predictor of favorable outcome after adjustment for reperfusion, age, and pre-ET NIHSS (OR = 39.25, 95% CI = 1.34->999, p = 0.04). INTERPRETATION BAO patients with limited regions of severe hypoperfusion had a favorable response to reperfusion following ET. However, patients with more extensive regions of hypoperfusion in critical brain regions did not benefit from endovascular reperfusion. ANN NEUROL 2021.

Objectives To evaluate the disease activity before and after COVID-19 and risk factors associated with outcomes, including hospitalization, intensive care unit (ICU) admission, mechanical ventilation (MV) and death in patients with spondylarthritis (SpA). Methods ReumaCoV Brazil is a multicenter prospective cohort of immune-mediated rheumatic diseases (IMRD) patients with COVID-19 (case group), compared to a control group of IMRD patients without COVID-19. SpA patients enrolled were grouped as axial SpA (axSpA), psoriatic arthritis (PsA) and enteropathic arthritis, according to usual classification criteria. Results 353 SpA patients were included, of whom 229 (64.9%) were axSpA, 118 (33.4%) PsA and 6 enteropathic arthritis (1.7%). No significant difference was observed in disease activity before the study inclusion comparing cases and controls, as well no worsening of disease activity after COVID-19. The risk factors associated with hospitalization were age over 60 years (OR = 3.71; 95% CI 1.62–8.47, p = 0.001); one or more comorbidities (OR = 2.28; 95% CI 1.02–5.08, p = 0.001) and leflunomide treatment (OR = 4.46; 95% CI 1.33–24.9, p = 0.008). Not having comorbidities (OR = 0.11; 95% CI 0.02–0.50, p = 0.001) played a protective role for hospitalization. In multivariate analysis, leflunomide treatment (OR = 8.69; CI = 95% 1.41–53.64; p = 0.023) was associated with hospitalization; teleconsultation (OR = 0.14; CI = 95% 0.03–0.71; p = 0.01) and no comorbidities (OR = 0.14; CI = 95% 0.02–0.76; p = 0.02) remained at final model as protective factor. Conclusions Our results showed no association between pre-COVID disease activity or that SARS-CoV-2 infection could trigger disease activity in patients with SpA. Teleconsultation and no comorbidities were associated with a lower hospitalization risk. Leflunomide remained significantly associated with higher risk of hospitalization after multiple adjustments.
