Background The SARS-CoV-2 pandemic put under pressure all the world's health systems, to the point that it was a severe threat to their stability. At the same time, this scenario confirmed the importance of primary health care to guarantee effective care for patients who suffer from complex and chronic diseases. From these considerations and in the light of the funding provided by the European Union for enhancing the health care system in Italy, our working group has decided to analyse various organisational models of Primary Health Care founded around the world to set up innovative Primary care community Centers in Italy, called Community Houses. Methods A scoping review of the international literature was conducted on Pubmed, searching for primary care models based on integration and co-location of services. Each organisational model was then evaluated using different levels of multidimensional integration inspired by the taxonomy work of P. P. Valentijn, such as clinical, professional, organisational, system, functional and normative integration levels. Results The search produced 2053 results, initially screened by title and abstract and, subsequently, by full-text, finally obtaining 116 articles. When a model is characterised by integrating services with external stakeholders, it also presents more integration levels than the others. In particular, these models are, on average, about 20% more likely to have an organisational, functional and normative integration in the model. Moreover, by stratifying for population complexity, we can find an increase in integration levels for populations suffering from chronic diseases with a higher degree of complexity, such as diabetes or cancer. Conclusions From these preliminary results, we can conclude that it is necessary to prefer primary care models with more integration levels to deliver better healthcare for people with complex or chronic diseases, improving the performance of the Health Care System, especially in Italy. Potrebbe destare stupore richiamare l'attenzione dei cultori dell'Igiene e Sanità pubblica su una tra le opere più significative delle circa settanta che costituiscono il Corpus ippocratico - probabilmente assemblato nella biblioteca di Alessandria, all'inizio del III secolo a.C. - cioè sull' "Aria, Acqua, Luoghi", che identifica nell'ambiente, nelle condizioni metereologiche, nelle sostanze presenti nell'acqua da bere e in altri fattori molto concreti le possibili cause delle malattie, anziché attribuire loro un'origine soprannaturale. Questa intuizione millenaria attribuita ad Ippocrate, padre della Scienza medica, ha improntato tutta la storia dell'Igiene e Sanità pubblica dando luogo ad un immenso corpus dottrinale che, seguendo man mano le nuove conoscenze, ha dimostrato la natura dinamica e il continuo ampliamento della disciplina. Disciplina che sta ora sempre più interessando il mondo non solo scientifico ma anche politico e popolare impressionato dalla manifestazione di vistosi fenomeni naturali attribuibili ad una modificazione di equilibri tra i componenti dei tre regni della natura: animale, vegetale e minerale.

Non vi è alcun dubbio che, sotto la spinta degli effetti sanitari della pandemia da COVID19 e della approvazione (il 13/7/2021) del Piano Nazionale di Ripresa e Resilienza (PNRR), si è in presenza ancora una volta di un tentativo di riorganizzazione del Servizio sanitario nazionale. Alcuni significativi elementi di questa volontà politica sono stati già indicati negli Editoriali di questa Rivista pubblicati negli ultimi due anni - dalla dichiarazione dello stato di emergenza nazionale per il contenimento della diffusione dell'epidemia da COVID-19 (D.L. 25/3/2020, n. 19) a tutt'oggi. Gli esperti diScienze politiche applicate alla sanità hanno fatto rilevare che le proposte di riorganizzazione - soprattutto tendenti a rafforzare la sanità territoriale (cure primarie e servizi territoriali)2 - risalgono addirittura alla legge istitutiva del Servizio sanitario nazionale e sono state "rinnovellate"da numerosi successivi atti legislativi mai completamente realizzati. La motivazione è facile da comprendere perché da tempo è fin troppo noto che non è possibile modificare un sistema, come quello sanitario (ma non solo questo), unicamente attraverso cambiamenti strutturali3. Una riforma per avere successo deve innescare un'interazione virtuosa tra il dispositivo normativo e il sistema sociale di realizzazione, composto dagli esecutori e dai beneficiari della riforma. L'antica metafora botanica della semina può rendere l'idea: anche la migliore semenza non può attecchire su un terreno arido o poco soleggiato e se le radici non trovano nel suolo il necessario alla crescita della pianta.

Aquot;la Scienza e l'Arte di prevenire le malattie, di prolungare la vita e di promuovere la salute attraverso gli sforzi organizzati della societàquot; sono stati attribuiti molti nomi, variamente associati tra loro: Sanità pubblica, termine onnicomprensivo della multidisciplinarietà della materia, ma anche Igiene, Medicina preventiva, Medicina sociale, Medicina di comunità a seconda del prevalente campo di attività messo in atto. L'inserimento della dimensione comunitaria nella teoria e nella pratica della Medicina richiede però la definizione di comunità, il coinvolgimento nell'azione di tutela della salute (promozione, conservazione e recupero), la localizzazione territoriale (dove la gente vive e lavora), la disponibilità di risorse finalizzate e così via. Una esigenza quanto mai necessaria specialmente ora che l'ormaiquot;miticoquot; Recovery Plan - ovvero Piano Nazionale di Ripresa e Resilienza (PNRR) - cita ripetutamente laquot;Comunitàquot; facendone un elemento di rilievo dell'azione progettuale. Uno degli interventi previsti è infatti la realizzazione entro il 2026 di 1.288 Case della Comunità. Queste sono ideate come strutture sanitarie, promotrici di un modello di intervento multidisciplinare, nonché luoghi privilegiati per la progettazione di interventi di carattere sociale e di integrazione sociosanitaria esprimendo una nuova modalità di presa in carico delle persone e delle comunità per prevenire le malattie, prolungare la vita e promuovere la salute. Ne consegue che la sede della Casa della Comunità deve essere visibile e facilmente accessibile per la comunità di riferimento (struttura di prossimità) perché è il luogo dove il cittadino può trovare una risposta adeguata alle più diverse esigenze sanitarie o sociosanitarie. La soluzione organizzativa raccomandata (in informaticaquot;hubspokequot;) prevede un elemento centrale di una rete per le cure primarie e per i supporti sociali e assistenziali, proponendosi come luogo di offerta, ma contestualmente come luogo di relazione e di attenzione a tutte le dimensioni di vita della persona e della comunità.

La pandemia da COVID 19 sembra somigliare sempre più ad un "buco nero galattico" in cui vengono inghiottite tutte le energie e tutti i pensieri dell'intera comunità nazionale. Quest'ultima, al pari di un drogato, sembra attendere ansiosamente la quotidiana "dose" di notizie, in una condizione, cioè, di trovarsi gradualmente a non poter controllare il desiderio di assumere una certa sostanza nonostante il danno fisico, psicologico, affettivo, emotivo o sociale che può conseguire a tale assunzione.

Per la Sanità pubblica l'anno 2020 appena trascorso si può rappresentare con due avvenimenti: la grande pandemia da coronavirus SARS-CoV-2 (così chiamato per la sua immagine circondata da "spine") e il bicentenario della nascita di Florence Nightingale (così chiamata perché nata a Firenze il 12 maggio 1820). L'abbinamento di questi due avvenimenti è meno lontano di quanto sembri prestandosi ad alcune istruttive considerazioni.

La parola italiana "disinformazione" abbraccia i due termini inglesi di "disinformation" e "misinformation" avvalorando la maggiore precisione terminologica della lingua anglosassone. Il primo inteso come informazioni infondate che non sono intenzionalmente false ma credute vere. La storia della Medicina non è altro che la cronaca di un susseguirsi di disinformazioni corrette progressivamente nel tempo mano a mano che venivano acquisite nuove conoscenze, Il secondo termine indica invece informazioni intenzionalmente false o fuorvianti, diffuse con qualche intento, di norma per ottenere vantaggi politici, economici, o elogiativi.

Non è il caso di ricordare che la Sanità pubblica è nata e si è sviluppata per tutelare la salute "come fondamentale diritto dell'individuo e interesse della collettività". Certamente l'attualequot;Ab occulta coeli influentiaquot;2 da Coronavirus SARS-CoV-2 o COVID-19 ha messo a dura prova la tutela della salute da parte della struttura e organizzazione della Sanità pubblica.3.

Antimicrobial resistance (AMR) is the phenomenon whereby a microorganism (bacteria, fungi, viruses, and parasites) is resistant to an antimicrobial, previously effective for the treatment of infections generated by it. Although the emergence of AMR is a natural phenomenon, the lack of implementation of infection prevention and control hygiene measures and the excessive and inappropriate use of antimicrobials are among the causes linked to an accelerated appearance and spread of microorganisms resistant to their action, with a consequent loss of efficacy of the treatments and serious risks for public health. Antimicrobial resistant organisms are found in people, food, animals, plants and the environment and can spread among different ecosystems in a global way without respecting geographical borders. Antimicrobial resistance is therefore a global public health problem that requires a global solution. Without proper control and action, AMR will result in enormous human and economic costs. Currently, according to the most recent data available, it is estimated that the AMR is responsible for around 700 thousand deaths worldwide per year (including 230 thousand deaths from multidrug-resistant tuberculosis) and of the 33 thousand deaths that occur in Europe about 11 thousand concern our country, especially for infections contracted in hospital. In the next 35 years, in the absence of measures, it is assumed that worldwide there will be about 300 million people who died prematurely (10 million deaths per year until 2050) with a loss of 100 thousand billion dollars of economic production; in high-income countries it is estimated that between 2015 and 2050 about 2.4 million people could die in the absence of sustained contrast measures over time. The World Health Organization has stated that AMR represents one of the top ten threats to public health worldwide, as it compromises our ability to treat even minor infections, seriously jeopardizes surgical procedures and other advanced medical interventions such as chemotherapy and transplants and more generally endangers the Sustainable Development Goals (SDGs) related to human health and indirectly also the progress of the SDGs aimed at reducing poverty and inequality. We all have a role to play in the fight against AMR, a coordinated effort to address this threat to our health, to our economies is essential: lives around the world depend on this commitment.

The increased use of vaccination has led to a significant decrease or even to the disappearance of serious infectious diseases. Currently, vaccines are often evaluated by their potential side effects while ignoring the benefits. This happens because of memory loss. The memory of the damage caused by vaccine-preventable diseases has been lost. This paper illustrates the epidemiology of the major vaccine-preventable diseases and the risk of reintroduction of many pathologies in case the vaccine coverage continues to decrease.

High rates of lost to follow-up (LTFU) in the Prevention of Mother-To-Child Transmission of HIV (PMTCT) programs in Cameroon will only contribute in hindering the successful implementation of the program. The objective of this study was to determine the reasons of LTFU of mother-child pairs enrolled in the PMTCT program in Dschang District Hospital (DDH): Cameroon. Methods: A retrospective cohort study was carried out in HIV+ exposed children delivered in the DDH from1st Jan 2012 - 31st Dec 2014, who were greater than or equal to 18 months at the study period (1st August - 30th September 2016) and whose mothers were enrolled in the PMTCT program for at least 3 months. Children were considered LTFU if they did not return to the hospital for the establishment of their HIV status at 18 months. A complementary cross-sectional study was done whereby a structured questionnaire was administered to the LTFU group via telephone calls, in order to determine the causes of LTFU. Data was collected from hospital registers and analyses done using Epi info 7.1.3.3 software. Results: A total of 141 mother-child pairs were eligible for the study, 76 were reachable via phone calls and 36 (47.37%) met the case definition of LTFU. Out of the 36 (47.37%) children LTFU, lack of information, 19(65.52%); lack of support from male partners, 4(11.11%); Poor behavior of health personnel, 2 (5.56%); forgetfulness, negligence/time wasting, unavailability of results, fear of child being infected, and family problems were all listed at equal proportions of, 3(6.90%) as well as financial problem, 1 (2.78%) were all causes of LTFU identified in this study.

Contrary to what has happened so far, hospitals should become a setting which jointly exercise Clinical and Public Health Medicine. The areas of activity that require the presence of multidisciplinary teams and can bring benefits both to the patients and to the community is briefly described.

In the poor zones of sub-Saharan Africa, the conventional drinking water network is very weak. The populations use alternative groundwater sources which are wells and springs. However, because of urbanization, the groundwater sources are degrading gradually making pure, safe, healthy and odourless drinking water a matter of deep concern. There are many pollutants in groundwater due to seepage of organic and inorganic pollutants, heavy metals as well as microbiological contamination. This study was carried out in October 2012 and aimed to analyze the practices and behaviors of populations related to water supply that may constitute potential risks of microbiological contamination and emergence of waterborne diseases.This study was carried out on a sample of 285 households, distributed in twelve principal quarters of the Douala V municipality in Cameroon. After data collection through the questionnaires, the selection of vulnerable quarters was done by the tabulation of some specific results on the questionnaire. The microbiological analysis was carried out using an innovative rapid test calledquot;Micro Biological Survey (MBS)quot; that has been developed and patented by MBS srl, a recent spin-off of the University of Roma Tre.We found out that 42.30% of households used water from drilled wells (forages), 33.80% from Cameroon National Water Company (CDE) distribution network, 9.50% from spring, and 9.40% used other source of water such as buying industrial mineral water. However, it should be noted that, as dangerous as it may be, wells (quot;puitsquot; in French) water is used as permanent source of drinking water by 5% of households. In general, 63.2% of households affirmed not to have benefited the fitting of public water point. Moreover, among the quarters were the households affirmed to have benefited from the development of a public water point, 52.5% of these water point were no longer functional. From the obtained data we have assessed that 83% of wells analyzed are faecally contaminated and all the forages analyzed (100%) are not contaminated. The most affected people by water-related diseases are children under 5 years (43%).We highly recommend the City Council of Douala V to restrict the use of the contaminated wells and provide alternative water sources such as forages for public use and to ensure that microbiological quality of the new and already existing water sources is controlled regularly by the Technical Service.

Hepatitis A disease is endemic in Albania even though records from the Medical Authority show outbreaks involving only limited numbers of people. In the city of Lac, 200 people became ill following an outbreak of hepatitis A started on November 2002. The age distribution of the cases shows a peak in the age group 5-9 (43.5%) followed by the age group 10-14 (28%) and the age group 1-4 (18%). No cases were recorded in the age group 0-1. Drinking water and sewage samples were collected, using electropositive cartridges, during the outbreak. Rotavirus was identified by RT-PCR in two out of five drinking water samples; however, all the samples tested negative for HAV and astrovirus. Rotavirus was also identified in three out of five sewage samples from which four were HAV positive. HAV-RNA was identified in 28 (62.2%) out of 45 IgM anti-HAV positive sera collected during the outbreak. Genotype IA was expressed from all the amplified samples. Sequence analysis of the overlapping VP1-2A region shows 97-99% homology with three Italian strains IT-SCH-00, IT-ZAM-01, and IT-CAP-00 and one Spanish strain (Sa 30/06/95), whereas the phylogenetic tree built from the 168 bp sequence of the VP1-2A shows four clusters, one including 24 out of 28 RT-PCR positive sera. Considering the deduced amino acid sequence, only one substitution was identified and reported previously for genotype IB.

The magnitude of the present Ebola epidemic and the resonance in the media has led to the need to draw clear guidelines for the health personnel potentially involved, since the fears raised by the high lethality of the disease may create inefficiencies Here we present the guidelines for a medium-sized hospital, where, at present, the chance to confront a case of Ebola virus disease is rare, but not impossible. The role and activities of each professional involved and the procedures have been set out. We think that this exercise will be useful for all structureswith characteristics similar to ours.

In times of economic crisis and a national healthcare system that absorbs more than 7% of the gross domestic product, there is a need to "rethink" healthcare practice. EHealth is part of this process of improving accessibility to services, use of available resources and coordination of program choices, and is an indispensable tool for implementing this cultural and management revolution. How realistic is it, today, to think of implementing a digitalized healthcare practice network? To settle this question is essential for today's national healthcare system.

Aim of the study: The present study was designed to evaluate the possible co-infection, with other enteric viruses, during an outbreak of hepatitis A (HA). Material and Methods: Forty-two stool samples and sera were collected during an outbreak of hepatitis A. Sera were analysed by the Abbott test for IgG-IgM anti-HAV antibodies. Stool samples were used to identify the presence of enteric viruses. HAV genome was identified by a RT-PCR test, other enteric viruses were identified, after cell passage and seroneutralization test on BGM cells, by RT-PCR and RFLP assay. Results: The samples were obtained from 27 employees of an industrial plant, nine household contacts and six non-employee controls. The attack rate was 12.5%, whereas the overall prevalence was 63%. In the employee group, 12 out of 27 stool samples were positive for the presence of HAV by reverse transcriptase polymerase chair reaction (RT-PCR). All the other samples (30) were negative. Five samples from employees, three from household contacts and one from non-employees were also found positive for enteroviruses. These viruses were classified by seroneutralization as poliovirus and RFLP assay as Sabin poliovirus type l. Four samples were positive both for HAV and poliiovirus. Conclusions: This study confirms co-infection with different enteric viruses may occur and also emphasizes the wide circulation of HAV and the existence of silent infection with poliovirus.

Random sera, in a total of 192, were collected in the Fever Hospital of Alexandria, Egypt, and analysed for the presence of antibodies against polioviruses. The results show good antibody levels, only three sera (1.5%) were negative for poliovirus type 1, 5 (2.6%) for poliovirus type 2 and 10 (5.2%) for poliovirus type 3; one subject was completely negative.

A total of 202 serum and stool samples from acute hepatitis patients attending the Fever Hospital of Alexandria, Egypt, have been studied to reveal markers of hepatitis virus infection. Anti-HAV IgM were detected in 21 out of 202 sera (10.4%), whereas 201 sera (99.5%) had anti-HAV IgG. The first age attack was in the class-age 0-9 years with 64.7% of anti-HAV IgM positive sera. Among 202 patients, anti-hepatitis E IgG (sample/over cut off > 1.0) was identified in 90 patients (44.5%). The anti-HEV seropositivity ranged from 17.6% to 60.0% in the different age groups, with the highest level in the class-age 20 29 years. Anti-hepatitis E IgM were identified in 49 patients with the first age attack in the class-age 10-19 years (39.4%). HAV RNA was identified by nested PCR in 7 samples out of 15, whereas HEV RNA was present in 4 out of 75 stool samples. Direct DNA sequence of the latter PCR products confirmed the presence of the HEV genome; comparison of the sequences of the isolates from Egypt with those in data banks revealed the highest homology to the Burma strain. Our data confirm that HAV and HEV are common causes of acute sporadic hepatitis in Alexandria but with different peak age positivity. Occasionally, but not infrequently, dual infections (HAV-HEV and HEV-enteric viruses) were also found. The risk analysis indicates that patients living in rural areas are exposed to a higher risk of hepatitis E infection compared to the urban population, whereas the presence of anti-HEV IgG was significantly associated with consumption of common village water and use of indoor dry pit and oral therapy for schistosomiasis.

Healthcare-associated infections (HAIs) are a significant cause of patient morbidity and mortality. They represent one of the biggest Public Health problems today as they prolong hospital stay, reduce quality of life and increase mortality. The frequency and severity of HAIs have increased due to increased severity of patients being admitted to hospitals, increased use of vascular catheterization and progressive spread of antibioticresistant strains. In Italy, there are wide regional variations in antibiotic consumption and in the incidence of infections by multiresistant germs. This suggests the need to not only limit consumption of antibiotics but also to improve their appropriate use. Unfortunately, not all HAIs are preventable: it is therefore advisable to selectively monitor those that are attributable to problems in the quality of care. Prevention of HAIs is obviously aimed at protecting the health of patients; however, occupational exposure of healthcare workers (HCW) has also been receiving increased attention. This has resulted in the dissemination of guidelines and operating instructions aimed at minimizing the risk of exposure to biological agents, through the implementation of general and specific infection control measures. The authors also highlight that litigation related to HAIs is a trend that should be kept in mind by every HCW, especially in light of the fact that the frequency of malpractice liability claims is increasing.

The aim of our study was to assess knowledge, attitudes, and practices (KAP) regarding HIV/AIDS in the city of Kribi, southern region of Cameroon. In November 2012, a questionnaire composed of 20 items was administered by trained staff from the Biomedical Sciences Department of the University of Dschang to 200 students selected from four population groups: high school students, local traders, tourism personnel (staff of bars, restaurants, hotels, nightclubs), and motorcycle taxi drivers. A cluster sampling method was used to select the first three groups while motorcycle taxi drivers were selected by the method of all comers. KAP regarding HIV/AIDS was found to be fragmentary in the studied population. Only 6.5% systematically uses condoms, 59% believe that AIDS can be cured by traditional medicine and religious faith and 40.9% developed stigmatizing behaviour toward HIV infected people. Among participants there is a wide discrepancy between knowledge and social behaviours toward HIV/AIDS. Strategic and continuous awareness campaigns that are culturally and socially tailored are urgently needed.

During the years 1992-1994, we tested 948 individuals from different population groups for HIV-1 and HIV-2 infections by ELISA and WB and for HCV infection by ELISA. Repeated ELISA reactivity for HIV was found in 2.12% of blood donors, 2.95% of fire brigade personnel and 1.61% of prisoners. Western blotting studies, however, showed that these samples were non-reactive or indeterminate to either HIV-1 or HIV-2. In contrast, anti-HCV antibodies were detected in 39% of fire brigade personnel, 31.4% of prisoners and 20.8% of blood donors. The analysis of risk factors for acquiring HCV infection showed a strong association between a past history of parenteral therapy for schistosomiasis and anti-HCV seropositivity (p < 0.0001). The implementation of preventive strategies is at the moment the mandatory choice to stop a further spread of the HCV infection. Meanwhile the same preventive measures could avoid spreading of the HIV disease.

Clinical governance of healthcare and community services by healthcare organizations requires the use of validated tools for identifying the specific healthcare needs of the local population. The population served by a local health organization may be large and although data regarding this population as a whole is useful for a preliminary evaluation, it may be too generic for an accurate estimation of the healthcare needs at the district level since different districts may face different challenges and have profoundly different realities. In this context, it can be strategically useful to use a system of indicators targeted at districts, the latter regarded as the basic unit of the health care system and characterized by a relatively constant structure and size.A set of district indicators has been developed and adopted by a local health authority in Rome (Italy) "ASL Roma B", as part of a collaborative project with the Public Health Agency of the Lazio region. In this paper, we present the main results of the first four years of implementation of the system (from 2007 to 2010).The data shows that even within a metropolitan health organization serving an apparently homogeneous population, health needs, provision of services and outcomes may vary greatly between different districts suggesting the adoption of diverse operational strategies.
