Background: An oxyntic gland neoplasm confined to the mucosal layer (T1a) is classified as an oxyntic gland adenoma, whereas that with submucosal invasion (T1b) is defined as gastric adenocarcinoma of the fundic gland type (GA-FG). Methods: To reveal the differences in clinical features between them, we retrospectively investigated 136 patients with 150 oxyntic gland adenoma and GA-FG lesions. Results: The univariate analysis revealed that the mean size (GA-FG vs. oxyntic gland adenoma, 7.7±5.4 vs. 5.5±3.1 mm), the prevalence of elevated morphology (79.1% vs. 51.8%), black pigmentation within the lesion (23.9% vs. 9.6%), and non or closed-type atrophy (81.2% vs. 65.1%) were different between the two groups. A multivariate logistic regression analysis revealed that ≥5 mm lesion size (odds ratio, 2.96; 95% confidence interval: 1.21–7.23), elevated morphology (odds ratio, 2.40; 95% confidence interval: 1.06–5.45), and no or closed-type atrophy (odds ratio, 2.49; 95% confidence interval: 1.07–5.80) were factors in distinguishing GA-FG from oxyntic gland adenoma. When oxyntic gland neoplasms with no or one feature were judged as oxyntic gland adenomas and those with two or three features were judged as GA-FG, the sensitivity and specificity were 85.1% and 43.4% for GA-FG, respectively. Conclusions: We identified three possible distinctive features of GA-FG compared to oxyntic gland adenoma: lesion size ≥5 mm, elevated morphology, and no or closed-type atrophy.

Objective Although Barrett's adenocarcinoma (BA) remains a minor disease in Japan, its incidence has been gradually increasing. We analyzed the characteristics of BA in Japanese populations. Methods We retrospectively reviewed medical records and analyzed the clinicopathological differences between short-segment Barrett's esophagus (SSBE) and long-segment Barrett's esophagus (LSBE), as well as metastasis. Local recurrence and metachronous lesions were analyzed only in patients who underwent endoscopic resection (ER). Patients Consecutive patients who had pathological T1 BAs resected by ER or surgery from January 2003 Results A total of 168 patients were analyzed, including 139 with SSBE and 29 with LSBE. In total, 67% of the SSBE lesions and 32% of the LSBE lesions were located between 0 and 3 o'clock (p=0.0014). No patients who achieved pathological margin-free resection (pR0) and 17% of patients who did not achieve pR0 experienced local recurrence (p=0.0131). None of the patients without lymphovascular involvement, a poorly differentiated component, lesion size of >30 mm, and submucosal invasion of >500 mu m experienced metastasis. The 5-year cumulative incidence rate of metachronous BA after ER was 0% in patients with SSBE and 40% in patients with LSBE (p=0.0005). Conclusion Superficial BA was likely to be detected at the right anterior wall of SSBE in the Japanese population. The risk for metachronous BA after ER was high in Japanese patients with LSBE, as in Western patients.

A 71-year-old man was referred to our hospital for treatment of a 2 cm-sized right renal mass incidentally found by computed tomography (CT) and was diagnosed with right renal cell carcinoma cT1aN0M0. Contrast-enhanced CT revealed that the aorta was completely occluded below the inferior mesenteric artery origin, and Leriche syndrome was diagnosed. CT angiography showed several collateral arteries along the abdominal wall. A robot-assisted laparoscopic partial nephrectomy was performed to treat renal cell carcinoma. Preoperatively, we marked the collateral arteries using ultrasonography to avoid injury during trocar insertion. We did not observe any decrease in blood flow in the right leg during the operation. The pathological diagnosis was clear cell renal cell carcinoma. Leriche syndrome is a chronic occlusive disease involving the infrarenal aorta and the iliac arteries. Since lower limb blood flow is dependent on collateral circulation, it is important to avoid injuring the collateral arteries during surgery., 本論文の要旨は，第240回日本泌尿器科学会関西地方会にて発表した．

A 41-year-old female who suffered local recurrence of cervical cancer after receiving chemoradiotherapy underwent radical hysterectomy, radical vaginal resection, and pelvic and paraaortic lymph node dissection. After surgery, bilateral hydronephrosis due to right ureteral stenosis and left uretero-vaginal fistula occurred. We therefore placed a bilateral ureteral stent. Thereafter, we continued to replace the bilateral ureteral stent once every 3 months, but the replacement of the right ureteral stent became impossible three years after the initial placement. We thus performed bilateral upper urinary tract reconstruction using an ileal ureter with the aim of both eliminating the left ureteral vaginal fistula and resolving the right ureteral stricture.

Chaque chapitre du Degré zéro de l’écriture nous a fait mentionner bien des fois le problème suivant : certaines phrases chainées et autres appositions de mots induisent une lecture erronée désactivant littéralement la compréhension. Le fait que le langage soit linéaire ne signifie pas que l’activité de penser le soit aussi. Habituellement, dans un texte, la causalité de deux phrases se présente de manière telle que l’une est l’effet et l’autre la cause. Cependant, il va sans dire qu’il existe plusieurs, pour ne pas dire de nombreux choix virtuels avant la décision finale. En réfléchissant devant un tableau noir, on ne place pas sur une seule et même ligne les mots-clés en les reliant avec une flèche, mais on les dispose tantôt au-dessus tantôt en-dessous selon sa disposition d’esprit, toujours changeante. Il arrive même qu’on les efface ou qu’on les réécrive. On peut donc dire que, dans la réalité, la pensée n’est jamais linéaire : ses composants s’organisent de manière parallèle, voire multi-dimensionnelle. Lorsque Barthes paraphrase une idée, il lui en associe une autre tellement éloignée que ce rapprochement induit d’importantes erreurs dans les traductions japonaises. Et il semble utiliser consciemment l’interaction entre les deux éléments de sa comparaison pour en atteindre un troisième, invisible. Il en est ainsi du concept d’« écriture », qui a surgi chez lui entre les deux autres concepts que sont le « style » et la « langue », qui recouvrent respectivement le personnel et le social. L’archétype de cette manière de pensée est assurément le « zéro », illusion épistémologique entre « +1 » et « -1 ». Les OEuvres Complètes de Barthes contiennent tous ses essais, mais ne contiennent aucune des traductions françaises du jeune Barthes avant qu’il n’entre dans sa carrière d’écrivain, car elles ne sont pas considérées comme le résultat d’une création originale. On sait que Barthes a beaucoup étudié les tragédies grecques à la Sorbonne. Ce travail, tout mineur qu’il soit, signifie naturellement qu’il n’a pas seulement lu des textes en grec ancien, mais aussi les traductions françaises qu’il avait à portée de main. De plus, il a mis en scène ces drames avec ses camarades. Il a donc forcément été obligé de traduire les textes grecs en français pour en rédiger les scénarios. On peut dire que les nombreux textes rédigés à partir d’une seule tragédie sont des variantes du même nombre – et c’est dans la lecture comparative de ces lignes parallèles qu’il devait imaginer les scènes à réaliser. Cette première forme d’étude parallèle pourrait bien expliquer l’attitude objectale de Barthes. D’ailleurs, cette démarche parallèle de lecture et d’écriture peut faire penser à sa pratique du piano. Pendant ses heures quotidiennes de piano, il jouait ses partitions favorites. Mais il ne les a pas mémorisé, sans pour autant préciser pourquoi. Il n’a donc jamais cessé de lire de la musique avec ses yeux. Les deux portées parallèles sur la partition, qui représentent les deux mains collaborant, ressemblent bien à des chaînes de paraphrasés incomplets et de paraphrasants incomplets qui se combinent pour projeter une image complète ; dans le même temps, les accords, notes frappées simultanément par plusieurs doigts, ressemblent à des appositions de mots, alignés sans causalité, comme parallèles, mais dessinant un seul concept., 補遺 pp. S-1～S-19

A 79-year-old woman who presented with right hydronephrosis was referred to our hospital. Abdominal computed tomography (CT) showed a right ureteral tumor (cT3N0M0). Right nephroureterectomy and partial cystectomy were performed. Pathological examination revealed small cell carcinoma (mixed type ; INFb, pT3, ly1, v1, u-rt0, ur0, RM0). Cystoscopy showed intravesical recurrence of the tumor 3 months after the surgery. Transurethral resection was performed, and histopathological examination revealed small cell carcinoma (pT2). We recommended postoperative chemotherapy ; however, the patient and her family refused consent for chemotherapy. Liver and lymph node metastases developed, and the patient died 2 months after the transurethral resection.

A 72-year-old female with left renal cell carcinoma and lymphadenopathy had undergone hand-assisted laparoscopic left nephrectomy and dissection of the lymph node (papillary renal cell carcinoma, type 2, pT3a pN2 M1). She had been treated with adjuvant chemotherapy with sunitinib, temsirolimus and pazopanib. However, the patient was started on nivolumab due to disease progression. After receiving 5 cycles of nivolumab, she was admitted to our emergency room for chest discomfort and appetite loss. Since computed tomographic (CT) scan showed pericardial effusion, we performed pericardiocentesis. Cytological examination of the pericardial effusion demonstrated leukocytes and no malignant cells. CT scan two weeks after cardiocentesis showed no recurrent pericardial effusion. She became stable with nivolumab, but the administration of nivolumab was discontinued and she started receiving axitinib.

A 34-year-old male came to us with congenital spina bifida, vesicoureteral reflux and scoliosis. He had been on hemodialysis for chronic renal failure caused by reflux nephropathy since the age of 23 years. At the age of 24, he received bilateral nephrectomy and underwent living renal transplantation from his mother. Hemodialysis was started again at the age of 26, because the renal graft was not functioning. At the age of 34, the patient developed a fever that persisted for a few days. He received antibiotic medication from his physician, but since his condition did not improve he was refered to our hospital. A computed tomography scan examination revealed abscess formation in the left retroperitoneum. Magnetic resonance imaging findings also showed the abscess in the left retroperitoneum. The patient was diagnosed with empyema of the residual ureter and underwent a left ureterectomy procedure.

Il est ennuyeux de dire ou écrire en plusieurs mots une chose que l'on peut penser en un seul. Un mot précis suffit normalement à l'écrivain, mais il le paraphrase parfois pour ceux qui ne comprennent pas son choix. Barthes paraphrase souvent. Mais ce qu’il ajoute ne constitue pas pour ses lecteurs, à proprement parler, un enrichissement du premier mot. Après un mot ambigu, il réécrit avec un autre mot, mais ce nouveau mot nous semble tout aussi ambigu. Quel est donc le meilleur des deux ? S’il y en avait un, Barthes nous le donnerait seul, sans le mettre en comparaison avec un autre mot. Cela signifie qu’aucun des deux mots proposés n’est précis et essentiel. Ces mots choisis par Barthes, il est peu recommandé de vouloir les traduire en autant de mots précis japonais. Le contenu essentiel, que Barthes appelle « intention/pensée », n’est pas enfermé dans les mots-signes mais se trouve au-delà. Pour Barthes, son mot ne peut être précis et unique qu’au moment du choix, et son efficacité disparaîtra dès lors qu’il en choisira un autre pour désigner le même objet. Barthes dit que le contenu du poème est prosaïque. Plutôt que les phrases, temporairement précises, de Barthes en train de penser, ce sont donc ses intentions, ses vues au-delà de ces mots, qui doivent être recherchées. Pour cela, sans doute faudrait-il que la paraphrase en japonais soit assez longue et intelligible, même si l'auteur français la jugerait sans doute redondante et ennuyeuse. Il n'y a pas de grammaire absolue. Vers contre prose, langue française contre langue japonaise, toute transformation langagière doit être relativisée., 補遺 pp. S-1～S-34

Clonal hematopoiesis (CH) in apparently healthy individuals is implicated in the development of hematological malignancies (HM) and cardiovascular diseases. Previous studies of CH analyzed either single-nucleotide variants and indels (SNVs/indels) or copy number alterations (CNAs), but not both. Here, using a combination of targeted sequencing of 23 CH-related genes and array-based CNA detection of blood-derived DNA, we have delineated the landscape of CH-related SNVs/indels and CNAs in 11, 234 individuals without HM from the BioBank Japan cohort, including 672 individuals with subsequent HM development, and studied the effects of these somatic alterations on mortality from HM and cardiovascular disease, as well as on hematological and cardiovascular phenotypes. The total number of both types of CH-related lesions and their clone size positively correlated with blood count abnormalities and mortality from HM. CH-related SNVs/indels and CNAs exhibited statistically significant co-occurrence in the same individuals. In particular, co-occurrence of SNVs/indels and CNAs affecting DNMT3A, TET2, JAK2 and TP53 resulted in biallelic alterations of these genes and was associated with higher HM mortality. Co-occurrence of SNVs/indels and CNAs also modulated risks for cardiovascular mortality. A 20-year-old woman had urinary incontinence since childhood. She self-managed her symptoms by using incontinence pads and she admitted never having been to a urologist. When she consulted a urologist for cystitis, ultrasonography could not locate the presence of a right kidney. She was suspected of having a contracted kidney and was referred to our hospital for further examinations and treatment. An enhanced computed tomography scan showed a contracted right kidney, which was located on the surface of the inferior vena cava. Magnetic resonance imaging showed that the right ureter extended into the vagina. Cystoscopy showed the absence of a right ureteral orifice, and an ectopic orifice was identified in the vagina. Laparoscopic nephroureterectomy was performed based on the diagnosis of a hypoplastic kidney with an ectopic ureter. We removed the ureter as far as the vagina because a residual ureteral segment could cause infection. Postoperatively, the patient had no complications, and her uterine artery was successfully saved. The patient achieved complete continence after the operation.

A 74-year old man presented to us with difficulty urinating. The laboratory examinations revealed an elevated prostate-specific antigen (PSA) level. A prostate biopsy revealed prostate adenocarcinoma. The computed tomography scan showed metastasis to lymph nodes in the pelvic cavity and both adrenal glands. Combined androgen blockade therapy was started, and PSA levels began to decrease gradually. However, 2 months later, he was hospitalized for treatment of cellulitis and was found to have hypertension, hyperglycemia, and hypokalemia. Endocrinological studies showed elevated serum adrenocorticotropic hormone (ACTH) and serum and urinary cortisol levels. The plasma cortisol level was not reduced by either the 1 mg or 8 mg dexamethasone suppression test, and the plasma cortisol and ACTHlevels did not respond to the corticotropin-releasing hormone test. Computed tomography indicated increased metastasis, and endocrinological studies and other examinations suggested a diagnosis of ectopic ACTHsyndrome. Combined androgen blockade therapy was continued, and inhibitors of adrenocortical synthesis were also administered. However, 16 weeks later, he died of prostate cancer. On autopsy, the histological examination revealed small cell carcinoma in the prostate and adrenal glands and other sites of metastasis, while immunohistochemistry findings revealed that the tumors in the prostate and adrenal glands were strongly positive for ACTH.

An 83-year-old man with an indwelling lumbar-peritoneal (L-P) shunt (for idiopathic normal-pressure hydrocephalus) underwent retroperitoneal laparoscopic radical right nephrectomy for renal cell carcinoma (pT1aN0M0). Peritoneal perforation occurred intraoperatively, and he developed postoperative disturbance of consciousness. Computed tomography showed mild ventricular enlargement, which was attributed to L-P shunt failure secondary to increased pneumoperitoneum pressure. His level of consciousness was improved when we raised his head. Few reports have discussed complications observed during retroperitoneal laparoscopic surgery in patients with an indwelling L-P shunt. This case report discusses this topic along with a discussion of previously reported findings.

A 96-year-old woman was diagnosed with colitis in our hospital. Her past history was breast cancer and lung cancer but no diabetes mellitus. She suddenly complained of lower abdominal pain. Abdominal computed tomography (CT) revealed intramural gas in the bladder wall and multiple poor contrasting area in her right kidney, which suggested emphysematous cystitis and acute focal bacterial nephritis. Indwelling urethral catheter was performed for bladder drainage and the treatment with antibiotics started. Urine culture revealed Klebsiella pneumoniae and blood culture was negative. After 3 weeks, the abdominal CT confirmed the decrease of gas within the bladder wall and improvement of contrasting area in the right kidney.

Pour le titre du septième chapitre de ce livre, Barthes a choisi les deux mots sans article : « Écriture et révolution ». Et à la suite, pour le huitième, il en a combiné un mot-clé, le récurrent « écriture » avec un autre mot focalisé, « silence ». À propos de ces deux juxtapositions, on a la même impression de banalité. Cependant on pourrait se demander pourquoi Barthes n’a pas repris le style sans article, « Écriture et silence », mais a opté pour le style avec article, « L’écriture et le silence ». Le style qui ne se répète pas, nous semble-t-il, est paradoxal, puisque la répétition est à la base du « style ». L’indécision de Barthes nous montre un de ses aspects, sa facette pensive. Il en est ainsi de ses présentations doublées avec deux termes différents, comme « l'existence d'un troisième terme, terme neutre ou terme-zéro » et autres. Barthes se sert souvent du conjonctif « ou », non pour la séléction des objets, mais pour celle des termes possibles qui se référencient à l'unique image-objet. Les traducteurs japonais les ont traités à tort comme des objets juxtaposés. D’autre part, des syntaxes anormales, avec échange du rôle qualitatif pour un nom et un adjectif, sont fréquentes chez notre auteur : quand il écrit « l'instrument formel », on pourrait comparer cette syntaxe avec une autre, absente mais potentielle, « la forme instrumentale ». Mais peut-être, n'importe guère la différence entre la syntaxe que Barthes a choisie et celle habituellement sous-jacente. Les deux formations réversibles nous font sentir l'image-objet qu’aucune des deux ne peut pas représenter exactement. C'est la meilleure, voire unique façon d’appréhender l'objet réel qui n'existe pas dans les signes. Ce chapitre montre la combinaison de « l'écriture » et du « degré zéro » comme « l'écriture au degré zéro », une seule fois, excepté l’introduction écrite sûrement après tous les arguments substantiels. Cette syntaxe devrait être comparée au titre de ce livre, « Le degré zéro de l'écriture ». De même, ces deux syntaxes laissent en suspens une interrogation logique : laquelle est plus désirée et essentielle ? Les lecteurs japonais se sentent d'autant plus confus que l'arrangement syntaxique japonais est à l’opposé du français., 補遺 p.S-1～p.S-22

Du mot « utopie », les Japonais retiennent seulement le sens positif d'idéal et de perfection, bien qu'ils connaissent son étymologie en grec, littéralement : « nulle part ». Le sens positif de ce néologisme s'accompagne en effet d'une nuance négative : l'utopie est impossible. Ce mot est donc ambigu en français, c'est-à-dire un degré au dessus dans l'échelle d'abstraction si on le compare au japonais, qui privilégie un seul versant du mot. De la même manière, dans le japonais d'aujourd'hui, nombre de mots de sens simplifié suggèrent l'attitude subjective de la personne, positive ou négative. Les Japonais tentent de contrôler la signification des mots, négligeant parfois leur origine pour les simplifier et instituer des oppositions claires, alors que les Français s'attachent, en paraphrasant, au sens commun des synonymes. Barthes fait fonctionner admirablement la logique de négativité, empruntée à la linguistique, qui voit un troisième terme, norme invisible entre deux contraires. Le « degré zéro » en est un bon exemple. La préparation du roman, dernière activité littéraire de sa vie, ne serait pas le pendant d'une réalisation, mais nous présente plutôt une oeuvre virtuelle de façon négative. Cette absence de la substance même de la création littéraire nous rappelle ces lignes de Barthes à la fin du chapitre de L'écriture du Roman : « Aussi les plus grandes oeuvres de la modernité s'arrêtent-elles le plus longtemps possible, par une sorte de tenue miraculeuse, au seuil de la Littérature, dans cet état vestibulaire... ». Les Japonais sont enclins à penser que Barthes préparait un roman, et qu'il l'aurait donc achevé et publié si son accident mortel ne l'en avait empêché. Et pourtant, le terme « utopie » est un signe qui avait déjà annoncé l'impossibilité d'écrire ce roman. À notre grand regret., 補遺 pp. S-1～S-11

A 37-year-old male with a history of intermittent gross hematuria visited our hospital with asymptomatic macrohematuria. Cystoscopy findings revealed bloody urine from the left ureteric orifice and the cytodiagnosiswasclas sII. Contrast-enhanced computed tomography imaging revealed the inferior vena cava (IVC) running upward along the left side of the aorta, then crossing the aorta between the aorta and superior mesenteric artery (SMA). The crossing portion of the IVC was severely compressed by the SMA, causing the left-sided IVC (LIVC), and two left renal veins (LRV) were entered at the dilated portion of LIVC. On suspicion of nutcracker syndrome (NCS) with LIVC, color doppler ultrasonography and venography examinationswere performed. Although the resultsdid not meet criteria reported for NCS, venography revealed reflux from the IVC to the upper branch of the left renal vein. LIVC is a rare vessel anomaly, with a rate of incidence ranging from 0.2-0.5%, while NCS with LIVC isextremely rare, with only 7 case reports in the English literature. Herein. we report the first known case in Japan and review previous reports.

A 65-year-old man had been hospitalized for examination of melena at a separate facility. Computed tomography (CT) showed left renal mass and he was referred to our hospital. Magnetic resonance imaging (MRI) revealed a tumor without adipose components in the perirenal space. Preoperative diagnosis was retroperitoneal malignant tumor. Tumor excision and left nephrectomy were performed. Pathologic evaluation revealed inflammatory liposarcoma. He has been followed up for 10 months with no additional treatment and no evidence of local recurrence was seen.

本稿は2015年5月青山社発行の雑誌『流域』第76号に筆者が発表した論考型エッセー「日本語とフランス語の間で・・・ －Je cuid et pense en J/F, donc je suis－」に例と考察を加筆し、修正を加えたものである。

À travers cet essai, c’est « le roman » que Barthes pose comme objet central. Lorsqu'il a confronté Balzac et Michelet dans le chapitre « l’écriture du Roman », il a noté la similarité atteinte par le langage de la fiction et celui de l'histoire visant à la réalité substantielle. La réalité qu'il a objectivée dans cette phase n'est pas la condition primordial des faits historiques, mais l'effet langagier qui nous fait sentir « réel » ce que les mots représentent. Le roman n'est rien que la création fictive, et ne contient pas de substance dure. Après ce chapitre, il assure qu'il n'existe pas « d'écriture poétique » et que le poème n'est que le contenu prosaïque décoré par des rimes et des syllabes bien contrôlées. Puis, il retrace cette manipulation délicate qui a permis à l'écriture bourgeoise de survivre à la Révolution. Il nous a montré ce processus où la technique si élaborée des écrivains a produit le Naturalisme et le Réalisme, dont les socialistes se servent le plus efficacement pour lutter contre l’idéologie bourgeoise. Barthes nous suggère que ce paradoxe n’est rien qu'une tricherie du socialisme, qui nous fait croire au bout du compte que la description réaliste est substantiellement réelle. Aujourd’hui, la manipulation politique des mots par les médias est critiquée. Il apparaît que Barthes a cerné la vraisemblance coupable du langage médiatique dans le prolongement de l’écriture littéraire problématique. Ce texte qui nous défie trouble la traduction japonaise, si l’on se focalise sur la surface présentant l'apparence d'un désordre. Il nous faut imaginer les pensées informelles de Barthes, lesquelles s'activent substantiellement avant d'écrire. Par principe, nous avons objectivé non seulement les écrits de Barthes, mais ce qui nous semble avoir agi avant l'écriture. Cependant il faut raisonablement tenir comme objectif le plus important l'histoire substantielle que Barthes ne perd jamais des yeux dans sa recherche, derrière l'enchaînement de ses thèmes particuliers., 補遺 p.S-1～p.S-22

A self-repelling random walk of a token on a graph is one in which at each step, the token moves to a neighbor that has been visited least often (with ties broken randomly). The properties of self-repelling random walks have been analyzed for two dimensional lattices and these walks have been shown to exhibit a remarkable uniformity with which they visit nodes in a graph. In this paper, we extend this analysis to self-repelling random walks on mobile networks in which the underlying graph itself is temporally evolving. Using network simulations in ns-3, we characterize the number of times each node is visited from the start until all nodes have been visited at least once. We evaluate under different mobility models and on networks ranging from 100 to 1000 nodes. Our results show that until about 85% coverage, duplicate visits are very rare highlighting the efficiency with which a majority of nodes in the network can be visited. Even at 100% coverage, the exploration overhead (the ratio of number of steps to number of unique visited nodes) remains low and under 2. Our analysis shows that self-repelling random walks are effective, structure-free tools for data aggregation in mobile ad-hoc networks.

Biotelemetry is widely used to identify the position of aquatic animals. In order to observe micro scale movements of small target species, positioning accuracy is yet to be improved. It is known that frequency modulated signals provides better positioning accuracy. However, the conventional biotelemetry studies have used single-frequency transmitters. The purpose of this study is to develop a new type of ultrasonic transmitter using frequency-modulated tones (FM pinger) to reduce the positioning errors of biotelemetry. We conducted experiments to compare the accuracy of time difference of arrival (TDOA) by using the FM signals and conventional single-frequency signals in an open sea. We tested six patterns of frequency modulation including single-frequency (0, 2, 4, 6, 8, 10 kHz) with two kinds of duration (2, 10 msec.) for each frequency modulation bandwidth. The results showed that the TDOA of frequency-modulated tones provided higher accuracy than single-frequency tones. The average and standard deviation of TDOA of the frequency-modulated tones were close to the theoretical value of TDOA and 0. En général, le schéma ci-dessous est utilisé pour expliquer la signification d'une « métaphore » : Cependant cette superposition décalée de deux expressions ne se limite pas au niveau rhétorique. La « paraphrase » aussi doit présenter, dans une seconde formulation, une idée conforme à celle qui était exprimée dans la première. Et il en est ainsi de la « prose », supposant d'autres types de textes susceptibles de rapporter les mêmes faits. L'objectivisme visé par une langue comme le français est acquis lorsqu'on laisse de côté les différences subjectives et personnelles, en les considérant comme peu importantes. Quels que soient les efforts déployés, la traduction interlangagière ne peut pas non plus échapper à cette différenciation. Il n'existe presque pas de correspondances symétriques entre traduit français et traduisant japonais. Le décalage est donc inévitable. Le vocabulaire français est enclin à généraliser des concepts. Ses signifiés abstraits sont ordinairement englobés comme éléments dans le vocabulaire japonais, qui ne préserve pas le niveau d'abstraction des idées étrangères mais les particularise pour produire des mots plus concrets et pratiques, à l'aide de kanjis riches en informations. C'est ainsi que l'on adapte les concepts occidentaux au Japon. Généralisation et concrétisation sont des principes opposés dans la manipulation du langage. Les Français, ne répétant pas le même mot, se servent de synonymes pour faire une présentation objective, tandis que les Japonais subjectivistes utilisent plutôt les synonymes pour particulariser et différencier les choses. On doit donc tenir compte de cette asymétrie pour que la traduction ne soit pas un remplacement naïf par des mots japonais., 補遺 pp. S-1～S-12

En général, on suppose que l'essentiel de la prose n'est pas dans sa forme mais dans son contenu, contrairement au poème dont c'est la forme même qui représente cet essentiel. Cependant en réalité, il n'y a pas de prose sans aucune signification intérieure de sa forme, ni d'art poétique sans aucun contenu référé à son extérieur. Toute traduction interlangagière cherche à transmettre le contenu seul, puisque des langues différentes ont des formes assez inintelligibles pour nécessiter la version traduite, et que le contenu signalé par ces formes peut être le même constant. Pourtant, cela ne signifie pas que l'on ne puisse connaître l'expression de la forme même. Si les traductions japonaises ne nous présentent pas des pensées compréhensibles de Barthes, on devrait chercher la clé d'un glissement sémantique malcontrôlé dans la forme originale française extérieure au contenu. On trouve ainsi quelques sources du chaos des quatre versions japonaises dans des changements, tels que par exemple ceux indiqués ci-dessous : a) Des directions logiques de « conséquence-cause » à celles narratives de « cause-conséquence » : ce renversement nous trouble en raison de l'enchaînement compliqué des pensées chez Barthes. b) Des idées « générales » à des idées « particulières » : cette particularisation en japonais nous déroute avec des images mal concrètement détaillées. c) D'une syntaxe logique à une syntaxe rhétorique : le sujet non-animé (y compris l'idée abstraite) normal pour les Français nous donne l'illusion d'une personnification très artificielle. Le renversement de ligne causale, la perte de la vision généralisée et l'apparition des objets sémantiques créent un paysage alogique. Mais on s'assure bien du défi barthésien au-delà du masque non-désiré de ce critique.

Leiomyosarcomas of the spermatic cord are rare tumors which cause significant morbidity and mortality. We report a case of leiomyosarcoma in a 67-year-old man who presented with a left scrotal mass. Left orchiectomy with high ligation of the spermatic cord was performed with clinical diagnosis of scrotal tumor. The pathological examination revealed leiomyosarcoma arising from the spermatic cord. He was free of disease two years postoperatively.

We report a case of solitaryneurofibroma of the spermatic cord. A 48-year-old man was referred to our hospital with a complaint of a left inguinal mass. Magnetic resonance imaging (MRI) demonstrated a solid mass in the left inguinal region. The tumor was removed. Histopathological diagnosis was neurofibloma. He had no other findings of Von Recklinghausen disease. Onlya few cases of solitary neurofibromas of spermatic cord have been reported.

In recent years, functional near-infrared spectroscopy (NIRS) has been introduced as a new neuroimaging modality with which to conduct functional brain-imaging studies. With its advanced features, NIRS signal processing has become a very attractive field in computational science. This work explores nonlinear physical aspects of cerebral hemodynamic changes over the time series of NIRS. Detecting the presence of chaos in a dynamical system is an important problem in studying the irregular or chaotic motion that is generated by nonlinear systems whose dynamical laws uniquely determine the time of evolution of a state of the system. The strategy results directly from the definition of the largest Lyapunov exponent. The Lyapunov exponents quantify the exponential divergence of initially close state–space trajectories and estimate the amount of chaos in a system. The method is an application of the Rosenstein algorithm, an efficient method for calculating the largest Lyapunov exponent from an experimental time series. In the present paper, the authors focus mainly on the detection of chaos characteristics of the time series associated to hemoglobin dynamics. Furthermore, the chaos parameters obtained can be used to identify the active state period of the human brain.

We propose novel dynamic network control algorithms that reduce the blocking probability for dynamic bandwidth service provisioning. The expected network services include on-demand broad bandwidth provisioning services and layer one VPN. The important service attribute of fairness in terms of path length is effectively achieved by introducing a simple weighting function that considers path length and link utilizations of intermediate links of candidate paths. The algorithm achieves enhanced network utilization by rerouting existing paths to alternative routes without disruption. Numerical examples demonstrate that the developed algorithms attain not only a high degree of fairness but also low service blocking probability.

前立腺Endometrioid adenocarcinomaの3例を経験した.症例1(71歳男性).排尿困難で受診.肉眼的血尿, PSA上昇, 膀胱尿道鏡で前立腺右葉より尿道へ突出する乳頭上腫瘍を認め, 生検で類内膜腺癌と診断, TAB療法でPSAは低下し, 前立腺全摘除術を行った.しかし約1年3ヵ月後, 排尿困難から膀胱鏡で乳頭状腫瘍を認めて切除, 類内膜腺癌であった.症例2(71歳男性).肉眼的血尿, 膀胱タンポナーデで入院となった.PSA高値, CT・MRIで前立腺左葉より膀胱内に突出し, 精嚢へ浸潤する腫瘍を認め, 生検で前立腺類内膜腺癌と診断, 膀胱前立腺摘除術を施行した.3ヵ月後PSA高値より, CTで多発性肝転移を認め, TAB療法でPSAは低下した.症例3(86歳男性).高血圧, 高脂血症で他院通院中, PSA高値で紹介入院となった.MRIで前立腺両葉に内腺への浸潤を疑うlow intensity areaを認め, 生検で類内膜腺癌と診断, TAB療法でPSAは低下した, We report three cases of endometrioid adenocarcinoma of the prostate. Case 1: A 71-year-old male was admitted with a complaint of micturition pain. Serum prostatic specific antigen (PSA) was 12 ng/ml. Radical prostatectomy following needle biopsy was performed in May 1998. Histopathological diagnosis was prostatic endometrioid adenocarcinoma (pT2bpN0M0). Case 2: A 71-year-old male was admitted with a complaint of macrohematuria. PSA was 18 ng/ml. Cystoprostatectomy and ileal conduit construction following needle biopsy were performed in July 2002. The tumor that was diagnosed as endometrioid adenocarcinoma, invaded the bladder neck (pT4pN0M0). Case 3: An 86-year-old male was referred to our department for elevated PSA (16.8 ng/ml). Prostatic needle biopsy was performed and histopathological diagnosis was endometrioid adenocarcinoma with a part of well differentiated adenocarcinoma (T2bN0M0). PSA dropped to an undetectable range after total androgen blockade therapy.
