1. Milk Adulterants in Quetta: A Threat to Public Health

Abstract

Milk a natural rich lacteal secretion that provides all essential nutrients required by body. This study was designed to detect milk adulterants alongwth various physiochemical factors in milk sold in Quetta market. A total of 120 raw milk samples were collected hygienically and subjected to milk adulteration Kit (UVAS Lahore). Results revealed that overall, 11.66 % samples revealed adulteration with any single chemical adulterant. Water and powdered milk were the major adulterants found almost in all samples. On towns basis Zarghoon town samples were more adulterated (18%) than Chiltan Town samples (5%). Other parameters such as, Electrical Conductivity, pH, Fat percentage, Total Solids and Solid Not Fat were observed below than normal values in most of the samples. Adulterants like Urea, Hydrogen Peroxide, Sorbitol, Boric acid, Quaternary Ammonium Compound, Carbonate, Soap and Alcohol were not detected in any sample. However, Sodium chloride (1.66 %), Formalin (3.33 %), Hypochlorite (2.5 %), Starch (2.5 %) and sugar (4.16 %) were detected. This study explores the potential health hazard due to adulterated milk sold in retail outlets in Quetta market. There is dire need of regular monitoring of milk and its products by concerned food authority. These findings will help to build a concrete strategy to curtail this problem and improve the life standards of people.

Abstract

Purpose: In the critically ill, hospital-acquired bloodstream infections (HA-BSI) are associated with significant mortality. Granular data are required for optimizing management, and developing guidelines and clinical trials. Methods: We carried out a prospective international cohort study of adult patients (≥ 18years of age) with HA-BSI treated in intensive care units (ICUs) between June 2019 and February 2021. Results: 2600 patients from 333 ICUs in 52 countries were included. 78% HA-BSI were ICU-acquired. Median Sequential Organ Failure Assessment (SOFA) score was 8 [IQR 5; 11] at HA-BSI diagnosis. Most frequent sources of infection included pneumonia (26.7%) and intravascular catheters (26.4%). Most frequent pathogens were Gram-negative bacteria (59.0%), predominantly Klebsiella spp. (27.9%), Acinetobacter spp. (20.3%), Escherichia coli (15.8%), and Pseudomonas spp. (14.3%). Carbapenem resistance was present in 37.8%, 84.6%, 7.4%, and 33.2%, respectively. Difficult-to-treat resistance (DTR) waspresent in 23.5% and pan-drug resistance in 1.5%. Antimicrobial therapy was deemed adequate within 24h for 51.5%. Antimicrobial resistance was associated with longer delays to adequate antimicrobial therapy. Source control was needed in 52.5% but not achieved in 18.2%. Mortality was 37.1%, and only 16.1% had been discharged alivefrom hospital by day-28. Conclusions: HA-BSI was frequently caused by Gram-negative, carbapenem-resistant and DTR pathogens. Antimicrobial resistance led to delays in adequate antimicrobial therapy. Mortality was high, and at day-28 only a minority of the patients were discharged alive from the hospital. Prevention of antimicrobial resistance and focusing on adequate antimicrobial therapy and source control are important to optimize patient management and outcomes.

Abstract

The present study was designed to investigate the mycological contamination and aflatoxigenic potential of fungi isolated from the indigenous certified varieties of wheat. Surface spread method was used to determine mycological contamination whereas to determine the toxigenic potential of isolated fungi and screening of wheat grains for aflatoxin contamination thin layer chromatography was used. All the collected samples revealed fungal contamination however none of the fungal isolate showed aflatoxigenic potential. Similarly all the samples showed negativity for aflatoxin. It can be concluded that for human public health, cereal grains must be subjected to quality control and mycotoxicologicalexaminations.

Abstract

Anemia is associated with poor health outcomes, and the prevalence of anemia is a significant public health indicator for both developed and developing countries. Iron-deficiency anemia (IDA) is the most common type of anemia, which often develops during pregnancy. A cross-sectional study was conducted in the urban areas of Quetta city among the women of reproductive age (15-49 years) to update the status of IDA in the region. The study participants ( n = 216 ) were selected on a random basis, and the samples were further distributed by age. Overall, 75% of females were nonanemic, and among those that were anemic, 2% were severe, 13% were moderately, and 10% were mildly anemic. Among the IDA-affected women, 83% were non-pregnant. Age-wise distribution of IDA revealed no significant difference among different age groups, but numerically higher observations were recorded in the age groups of less than 30 years. The highest number of moderately IDA-affected women (15%) were in the age group 15-19 with the following IDA indicating parameters: hemoglobin 9.64 g/dl, mean corpuscular volume 63.11 fl, mean corpuscular hemoglobin 20.40%, red cell distribution 19.28%. This study will be beneficial for illustrating the requirement and the development of a program to raise extended awareness in the Quetta communities to overcome the negative health effects of IDA on the female population.

Abstract

— Using wastewater for agricultural irrigation presents potential risks to public health and the environment. The aim of this study was to compare the microbial burden and nutritional components of cabbage grown in fresh and wastewater in Quetta, Pakistan. Cabbage samples randomly collected from fields irrigated with fresh and wastewater sources were analysed for viable bacterial and fungal counts, pH and vitamin contents. There was a significant difference in viable bacterial (3.0 × 106, 1.9 × 107 CFU g-1) and fungal (2.4 × 103, 1.0 × 105 CFU g-1) counts for cabbage grown in fresh and wastewater, respectively. There were no significant differences in pH (7.88/7.86) and contents of the vitamins thiamin (0.31/0.30 mg/100 g), riboflavin (0.05/0.04 mg/100 g), niacin (0.56/.54 mg/100 g), pyridoxine (0.13/0.12 mg/100 g) and ascorbic acid (57.00/56.60 mg/100 g). Results demonstrated that cabbage grown in wastewater had a significantly higher microbial burden than cabbage grown in fresh water, although there were no significant differences in nutritional components of cabbage from the different water sources. These observations emphasise the potential dangers to public health in using wastewater for agricultural irrigation.

Abstract

This was a prospective study conducted in order to evaluate and compare pain scores through visual analogue scale, length of hospital stay and follow up in patients of benign renal disease and early renal malignancy. This study was conducted in the Department of Surgery, Batra Hospital and Medical Research centre, New Delhi. For the purpose of this study, a total number of 80 patients participated willingly after giving written informed consent. Out of these 40 were operated through open route and 40 patients underwent laparoscopic nephrectomy. Patients were grouped based on their route of surgery into open and laparoscopic groups. The comparison between them was made in relation to need for analgesia, hospital stay and convalescence. Follow up was done at 1 week, 4 weeks, 3 months and every six months so as to look for any delayed complications, chronic pain and recurrence in case of malignant disease. Female patients in the reproductive age group were more common in patients operated by laparoscopic route while as males were more common in open nephrectomy group. Laparoscopic group had lesser pain scores but on statistical analysis there were no signicant differences between the two groups. Mean hospital stay was (6.3 ±1.34) days in open group while as (4.2 ±1.22) days in the laparoscopic group. This study demonstrated that acute post operative pain scores were not different statistically between the two groups. Mean length of hospital stay was more in open group as compared to laparoscopic group. The cost of surgery was more in the laparoscopic group in comparison to open nephrectomy group. However, due to brief hospital stay, lesser morbidity, shorter convalescence and overall costs laparoscopic method was more useful in comparison with open route nephrectomy.

Abstract

Background: Malrotation of gut presenting in adolescents and adults though rare but exists. Also its presentation is nonspecific. Hence its diagnosis needs high index of suspicion. Material and methods: This prospective study was done the of department of surgery SMHS hospital at GMC Srinagar over a period of 5.5 years from July 2012 to December 2017.The patients included all the adolescent and adult patients who proved to be cases of malrotation of gut on intraoperative visualisation. Results: During the study period of 5.5 years, a total of 14 patients with malrotation of gut (belonging to adolescent and adult age group) were recorded. Age of the patients ranged from 16 to 54 years with an average of 26.21± 11.15years (SD=11.149). Majority of the patients belonged to adolescent age group. Preoperatively CT (computed tomography) was done only in 4 patients, thereby making the diagnosis of malrotation preoperatively in these 4 patients. In all other patients, the diagnosis was made only after laparotomy (done for acute abdomen) and intraoperative visualisation. Ladd´s procedure was done successfully in majority of patients with two patients requiring resection of gangrenous small bowl with stoma formation. Conclusion: Malrotation of gut presenting in adolescents and adults is a rare phenomenon and also the symptoms are similar to several other abdominal pathologies, hence it represents a diagnostic challenge. Hence a high index of suspicion is needed to diagnose this rare entity early without undue delay, so as to avoid or limit further complications .Threshold for CT scan should be kept a little low in suspicious cases so as diagnose and intervene in time. Well trained senior person should be involved during the surgical intervention of this rare condition. Further studies and reporting is needed to get familiar with this rare condition. Abstract

Cattle lungs (n=1200) obtained from abattoir of 10 districts of Balochistan were processed for isolation and identification of Mycoplasma species. A total of 156 isolates produced typical fried egg colonies on Modified Hayflick’s agar medium and 87.8% were preliminarily identified as Mycoplasma species, 12.2% species were Acholeplasmas. All the digitonin sensitive isolates were further subjected to different biochemical and PCR tests for further identification. Overall prevalence of M. bovis lungs samples obtained from slaughter house samples was 9%. Among the Mycoplasma isolates; 108 M. bovis, 29 Mycoplasma mycoides subsp. capri (Mmc) and 16 M. arginini were identified through the biochemical tests. M. bovis and Mycoplasma mycoides subcluster members were further validated through PCR and RFLP. Mycoplasma mycoides subspecies mycoides small colony type (Mmm SC) was not isolated from any of the lung samples. Among the Mycoplasma bovis species isolated, the highest number was observed from Quetta district (16%) followed by Pishin (15%), Zhob (11 %) and Kalat (10%). Conversely the lowest number of M. bovis isolates was found in Bolan (2%) district followed by Jaffarabad (3%), 4%, each from Khuzdar, Mustung, Killasaifullah and 7% in Sibi district. Statistical analysis using chi square test, showed a significance difference (χ²=33.38) in the recovery of Mycoplasma bovis from the lungs of cattle slaughtered in 10 districts of Balochistan.

Abstract

Genetic diversity among four indigenous breeds of sheep namely Mengali, Balochi, Beverigh and Harnai of Balochistan was detected by random amplified polymorphic DNA (RAPD) technique. Nineteen RAPD primers were initially applied and seventeen were chosen for further analysis, based on band pattern quality, reproducibility and presence of bands. An average of 92 RAPD fragments were obtained by using 17 primers and out of the 36 fragments (39.13%) illustrated monomorphism while, 56 bands (60.87%) were polymorphic in all the four sheep breeds. The number of bands amplified in all the sheep breeds ranged from 2 to 10. The highest number of polymorphic loci 40 was observed in the Mengali breed, while the lowest 28 was in Balochi. Further, 33 and 31 polymorphic loci were seen in Beverigh and Harnai breeds, respectively. The overall gene diversity was highest in the Mengali (0.1474) while the lowest in Balochi breed (0.0998). Results of genetic similarities showed closer proximity between Balochi and Beverigh (0.992), Balochi and Harnai (0.992), and between Beverigh and Harnai (0.996). The resemblance was observed between Mengali and Balochi (0.918), between Mengali and Beverigh breeds (0.931) and between Mengali and Harnai breeds (0.925). The high level of genetic similarity between Balochi, Beverigh and Harnai sheep indicated the close relationship that might be due to common habitat. Further the present study highlighted the presence of diversity among and within breeds that can be used in the selection or crossbreeding programs of sheep. The present study suggests that RAPD-PCR can effectively be used to determine the genetic distances among the sheep breeds. Abstract

Background. Obstructive sleep apnea (OSA) is associated with coronary artery disease. Intermittent hypoxia associated with OSA increases sympathetic activity and may cause systemic inflammation, which may contribute to atherosclerosis leading to an increase in the size of carotid intima media thickness (CIMT). Methods. PubMed and Cochrane library were reviewed by utilizing different combinations of key words: sleep apnea, carotid disease, intima media thickness, and carotid atherosclerosis. Inclusion criteria were English articles; studies with adult population with OSA and without OSA; CIMT recorded by ultrasound in mean and standard deviation or median with 95% confidence interval; and OSA defined as apnea hypopnea index of ≥5/h. A total of 95 studies were reviewed for inclusion, with 16 studies being pooled for analysis. Results. Ninety-five studies were reviewed, while 16 studies were pooled for analysis; since some studies have more than one data set, there were 25 data sets with 1415 patients being pooled for meta-analysis.
