Introduction. Patients receiving in-center hemodialysis are extremely vulnerable to COVID-19. It is unclear if routine screening of asymptomatic hemodialysis patients is an effective strategy to prevent COVID-19 outbreaks within the dialysis unit. Methods. We conducted a retrospective analysis of in-center hemodialysis patients who underwent bimonthly COVID-19 rapid antigen test screening from February 15th to December 26th, 2021. Nasal rapid antigen testing was performed in all asymptomatic patients. All rapid antigen-positive tests were confirmed by RT-PCR nasopharyngeal swab. Besides universal rapid antigen screening, RT-PCR testing was conducted in all symptomatic patients and contacts of COVID-19 subjects. Results. Overall, 4079 rapid antigen tests were performed in 277 hemodialysis patients on chronic hemodialysis with a mean age of 68.4 ± 14.6 years. Thirty-eight (0.9%) rapid antigen tests resulted positive. Only five (13.8%) positive-rapid antigen tests were also positive by RT-PCR testing. During the same period, 219 patients regularly screened by rapid antigen tests bimonthly underwent 442 RT-PCR nasopharyngeal swabs for clinical reasons. RT-PCR testing yielded a positive result in 13 (5.9%) patients. The time elapsed between PCR and the negative-rapid antigen test was 7.7 ± 4.6 days (range 1.8–13.9 days). At the end of the follow-up, 6.4% of the population on in-center hemodialysis contracted COVID-19, and routine rapid antigen tests detected only 5 out of 18 (27.7%) COVID-19 cases. No outbreaks of COVID-19 were identified within the dialysis unit. Conclusion. Bimonthly rapid antigen screening led to the early diagnosis of COVID-19 in less than one-third of cases. The short incubation period of the new SARS-CoV-2 variants makes bimonthly test screening inadequate for an early diagnosis of COVID-19. More frequent tests are probably necessary to improve the utility of COVID-19 nasal rapid antigen test in patients on hemodialysis.

We investigate the role that the evaporation of light primordial black holes may have played in the production of the baryon asymmetry of the Universe through the high-scale leptogenesis. In particular, for mass of primordial black hole in the range [$10^6$-$10^9$] g, we find a dilution of thermally generated lepton asymmetry via entropy injection in the primordial plasma after the sphaleron freeze-out. As a consequence, we can put strong constraints on the primordial black hole parameters, showing the mutual exclusion limits between primordial black holes and high-scale leptogenesis. Remarkably, we point out an interplay between the upper bound on the initial abundance of primordial black holes and the active neutrino mass scale., Comment: 11 pages, 6 figures

Urinary tract infections are the most common illnesses that impact pregnant women. This study aimed to investigate bacterial urinary tract infections and antibiotic susceptibility profiles in pregnant women. This study was conducted on pregnant women between April and October 2021 in clinics and hospitals in the Damt district. Midstream urine samples were collected, cultured, identified and tested for antibiotic susceptibility. The prevalence of UTI in pregnant women was 210/350 (60%). Escherichia coli accounted for 90/210 (42.9%), Staphylococcus aureus 50/210 (23.8%), and Klebsiella pneumoniae 25/210 (11.9%). Staphylococcus epidermidis, Pseudomonas aeruginosa, and Proteus mirabilis exhibited 15/210 (7.1%) of each with the moderately identified bacterial species. Antimicrobial sensitivity testing showed that bacterial isolates were resistant to amoxicillin (80-100%), ceftriaxone (40-70%), gentamycin (0-60%), amikacin (0-50%), tetracycline, ciprofloxacin, azithromycin, and amoxiclav (0-40%), and azithromycin (0-30%). Significant bacteriuria was associated with age (p = 0.01), pregnancy trimesters (p = 0.00), gestation (p = 0.00), and residence (p = 0.03), whereas there was no association with education (p = 0.05). Most isolates have become resistant toward antibiotic used in the study treatments, especially ceftriaxone and amoxicillin. Therefore, it is recommended that pregnant women undergo health education on the prevalence and causes of urinary tract infections.

Purpose:In search of novel strategies to improve the outcome of advanced prostate cancer, we considered that prostate cancer cells rearrange iron homeostasis, favoring iron uptake and proliferation. We exploited this adaptation by exposing prostate cancer preclinical models to high-dose iron to induce toxicity and disrupt adaptation to androgen starvation.Experimental Design:We analyzed markers of cell viability and mechanisms underlying iron toxicity in androgen receptor–positive VCaP and LNCaP, castration-resistant DU-145 and PC-3, and murine TRAMP-C2 cells treated with iron and/or the antiandrogen bicalutamide. We validated the results in vivo in VCaP and PC-3 xenografts and in TRAMP-C2 injected mice treated with iron and/or bicalutamide.Results:Iron was toxic for all prostate cancer cells. In particular, VCaP, LNCaP, and TRAMP-C2 were highly iron sensitive. Toxicity was mediated by oxidative stress, which primarily affected lipids, promoting ferroptosis. In highly sensitive cells, iron additionally caused protein damage. High-basal iron content and oxidative status defined high iron sensitivity. Bicalutamide–iron combination exacerbated oxidative damage and cell death, triggering protein oxidation also in poorly iron-sensitive DU-145 and PC-3 cells.In vivo, iron reduced tumor growth in TRAMP-C2 and VCaP mice. In PC-3 xenografts, bicalutamide–iron combination caused protein oxidation and successfully impaired tumor expansion while single compounds were ineffective. Macrophages influenced body iron distribution but did not limit the iron effect on tumor expansion.Conclusions:Our models allow us to dissect the direct iron effect on cancer cells. We demonstrate the proof of principle that iron toxicity inhibits prostate cancer cell proliferation, proposing a novel tool to strengthen antiandrogen treatment efficacy.

Neutrino-nucleus coherent scattering measurements by the COHERENT collaboration provide us with a unique capability to test various beyond the standard model scenarios. In this work, we constrain scalar leptoquarks (LQs) using the COHERENT data. LQs arise in many extensions of the Standard Model (SM). Generally, the mass of the LQs is assumed to be very high to avoid the bounds from proton decay. However, there are low-scale LQ models which prohibit proton decay by construction. We consider two electroweak doublet scalar LQ models with hypercharge Y=1/6, and Y=7/6 and provide the bounds in the plane of the Yukawa coupling and the mass of LQ. We also compare the bounds on LQs coming from various other experiments and find that the COHERENT one covers a wide range of LQ masses from MeV to TeV and in certain regions the constraints are competitive with the others., Comment: 9 pages, 4 figures, 2 tables, matches the published version

The Standard Model of particle physics and its description of nature have been recently challenged by a series of precision measurements performed via different accelerator machines. Statistically significant anomalies emerged when measuring the muon magnetic momentum, and very recently when deducing the mass of the W boson. Here we consider a radiative extension of the Standard Model devised to be sufficiently versatile to reconcile the various experimental results while further predicting the existence of new bosons and fermions with a mass spectrum in the TeV energy scale. The resulting spectrum is, therefore, within the energy reach of the proton-proton collisions at the LHC experiments at CERN. The model investigated here allows us to interpolate between composite and elementary extensions of the Standard Model with an emphasis on a new modified Yukawa sector that is needed to accommodate the anomalies. Focusing on the radiative regime of the model, we introduce interesting search channels of immediate impact for the ATLAS and CMS experimental programs such as the associate production of Standard Model particles with either invisible or long-lived particles. We further show how to adapt earlier supersymmetry-motivated searchers of new physics to constrain the spectrum and couplings of the new scalars and fermions. Overall, the new physics template simultaneously accounts for the bulk of the observed experimental anomalies while suggesting a wide spectrum of experimental signatures relevant for the current LHC experiments.

The definitive diagnosis of canine soft-tissue sarcomas (STSs) is based on histological assessment of formalin-fixed tissues. Assessment of parameters, such as degree of differentiation, necrosis score and mitotic score, give rise to a final tumour grade, which is important in determining prognosis and subsequent treatment modalities. However, grading discrepancies are reported to occur in human and canine STSs, which can result in complications regarding treatment plans. The introduction of digital pathology has the potential to help improve STS grading via automated determination of the presence and extent of necrosis. The detected necrotic regions can be factored in the grading scheme or excluded before analysing the remaining tissue. Here we describe a method to detect tumour necrosis in histopathological whole-slide images (WSIs) of STSs using machine learning. Annotated areas of necrosis were extracted from WSIs and the patches containing necrotic tissue fed into a pre-trained DenseNet161 convolutional neural network (CNN) for training, testing and validation. The proposed CNN architecture reported favourable results, with an overall validation accuracy of 92.7% for necrosis detection which represents the number of correctly classified data instances over the total number of data instances. The proposed method, when vigorously validated represents a promising tool to assist pathologists in evaluating necrosis in canine STS tumours, by increasing efficiency, accuracy and reducing inter-rater variation.

The electoral advantage that incumbent legislators enjoy over challengers in the U.S. Congress has been investigated extensively in political science. Very few studies, however, have considered the role of individual differences when it comes to incumbency preferences among voters. Based on theory and research in political psychology, we hypothesized that political conservatives would exhibit stronger preferences than liberals for incumbents over challengers from the same party. Extensive analyses based on more than 150,000 voters from seven election cycles in the United States from 2006 to 2018 support this hypothesis. A significant effect of conservatism on incumbency preferences was observed in the U.S. Senate and House of Representatives, and it was not attributable to Republican Party identification. This ideological asymmetry is consistent with system justification theory and prior research linking conservatism to risk aversion and status quo bias. Practical implications and directions for future research are discussed.

Despite growing evidence that liberals and conservatives differ on a wide range of psychological traits (Jost 2017), it is not clear whether ideological asymmetries exist in motivated reasoning. We argue that this is due to the fact that motivated reasoning should be asymmetrical when either relational motives to maintain homogenous social networks, or when epistemic motives to reduce uncertainty are triggered. We test these mechanisms directly by assigning participants to counter-attitudinal evidence on salient issues, combined with novel manipulations of relational and epistemic motives. Study 2 replicates and extends Study 1, using a larger sample of U.S. adults recruited by Lucid. We test the same hypotheses underlying Study 1, using the same information material and manipulations included in Study 1. In addition, we add 1) a novel manipulation of uncertainty (personal uncertainty), and 2) two additional issues (a carbon tax to fight climate change and the use of the death penalty as a crime deterrent).

Staphylococcus aureus is a Gram-positive bacterium commonly associated with severe infections in hospitalized patients. S. aureus produces many virulence factors leading to local and distant pathological processes. Invasiveness of S. aureus generally induces metastatic infections such as bacteremia, infective endocarditis, osteomyelitis, arthritis, and endophthalmitis. Peritoneal localization from extra-abdominal infection can be a potential consequence of S. aureus infection. Two cases of metastatic peritonitis have been described in patients on peritoneal dialysis with concomitant peripheral vascular catheter-related bloodstream infection. We reported a case of peritoneal metastatic infection caused by methicillin-resistant Staphylococcus aureus (MRSA) in a patient on maintenance hemodialysis. A 37-year-old man was admitted with fever and chill due to jugular central vascular catheter (CVC)-related bloodstream infection caused by MRSA. CVC was placed after switching the patient from peritoneal dialysis to hemodialysis for scarce adherence to fluid restriction. Detection of MRSA on the peritoneal effluent combined with a total white blood cell count of 554 cells/mm3 prompted the diagnosis of satellite MRSA peritonitis. Antibiotic treatment with daptomycin and simultaneous CVC and peritoneal catheter removal resolved the infectious process. No further metastatic localizations were detected elsewhere. In conclusion, S. aureus can induce metastatic infections far from the site of primary infection. As reported in this case, peritonitis can be secondary to the hematogenous dissemination of S. aureus especially in hospitalized patients having a central line.

Feeding disorders (FDs) are a large group of behaviours characterized by selectivity, inadequate intake and refusal of food. These disorders are frequent in children (10-15% < 3 years old) and their causes are multiple and variably combined. FDs have a significant impact in everyday life for the child, their family and caregivers inside or outside the school environment. In the last few years in the Department of Paediatric Dysphagia in Bologna (Italy) an increase in the number of healthy children with insufficient oral intake or extremely selective food behaviour, due to selecting in type, texture, smell or appearance of food, has been observed. These children tend to develop dysfunctional or oppositional behaviours during mealtime. This growing need requires the implementation of a network with local paediatricians that focalize the feeding disorder and refer the family to a multidisciplinary assessment. The multidisciplinary team identifies behavioural strategies, structural changes to the mealtime and at the same time introduces intervention for the sensorial normalization. The primary goal is the caregivers’ training: parents and school staff are constantly supported as they offer the child new, different, pleasant and adequate food proposals, avoiding to stop at first refusal. Parent training plays a significant role during such intervention, as it reduces mealtime stress and improves child-parent relationship. The introduction of new strategies during mealtime in a playful, positive and cheerful setting encourages the child’s willingness to taste new foods or new textures. Moreover, their peer and parent modelling improves the outcome.

Aria is a plant hosting a $${350}\,\hbox {m}$$ 350 m cryogenic isotopic distillation column, the tallest ever built, which is being installed in a mine shaft at Carbosulcis S.p.A., Nuraxi-Figus (SU), Italy. Aria is one of the pillars of the argon dark-matter search experimental program, lead by the Global Argon Dark Matter Collaboration. It was designed to reduce the isotopic abundance of $${^{39}\hbox {Ar}}$$ 39 Ar in argon extracted from underground sources, called Underground Argon (UAr), which is used for dark-matter searches. Indeed, $${^{39}\hbox {Ar}}$$ 39 Ar is a $$\beta $$ β -emitter of cosmogenic origin, whose activity poses background and pile-up concerns in the detectors. In this paper, we discuss the requirements, design, construction, tests, and projected performance of the plant for the isotopic cryogenic distillation of argon. We also present the successful results of the isotopic cryogenic distillation of nitrogen with a prototype plant.

Aria was designed to reduce the isotopic abundance of $^{39}$Ar, a $\beta$-emitter of cosmogenic origin, whose activity poses background and pile-up concerns in the detectors, in the argon used for the dark-matter searches, the so-called Underground Argon (UAr). In this paper, we discuss the requirements, design, construction, tests, and projected performance of the plant for the isotopic cryogenic distillation of argon. We also present the successful results of isotopic cryogenic distillation of nitrogen with a prototype plant, operating the column at total reflux.

Future liquid-argon DarkSide-20k and ARGO detectors, designed for direct dark matter search, will be sensitive also to core-collapse supernova neutrinos, via coherent elastic neutrino-nucleus scattering. This interaction channel is flavor-insensitive with a high-cross section, enabling for a high-statistics neutrino detection with target masses of $\sim$50~t and $\sim$360~t for DarkSide-20k and ARGO, respectively. Thanks to the low-energy threshold of $\sim$0.5~keV$_{nr}$ achievable by exploiting the ionization channel, DarkSide-20k and ARGO have the potential to discover supernova bursts throughout our galaxy and up to the Small Magellanic Cloud, respectively, assuming a 11-M$_{\odot}$ progenitor star. We report also on the sensitivity to the neutronization burst, whose electron neutrino flux is suppressed by oscillations when detected via charged current and elastic scattering. Finally, the accuracies in the reconstruction of the average and total neutrino energy in the different phases of the supernova burst, as well as its time profile, are also discussed, taking into account the expected background and the detector response., 21 pages, 8 figures

This research tests the effect of network diversity on turnout. In our paper, network diversity is defined as exposure to a network of interpersonal communication characterized by a mix of viewpoints on the referendum issues. This study also examines the interplay between network diversity, ambivalence, political knowledge and the effect of two moderating variables, i.e. frequency of discussion and strong-ties homogeneity.

This study analyzes the effects of training in school debating on ability to detect misinformation. The idea that school debating has positive psychological outcomes has been entertained by social scientists for a long time. In an early experimental study, Howell (1948) found that students assigned to debating classes increased their levels of critical thinking, a conclusion supported by decades of further study (e.g., Colbert 1987, McKee 2003). With rising concern about the spread of misinformation in digital societies (Lazer et al. 2018), researchers have asked what traits make people especially prone to fall for falsehood. One such characteristic is the lack of cognitive reflection (Pennycook and Rand 2021), arguably closely related to critical thinking. We therefore hypothesize that engaging in debating has the potential to make students less vulnerable. School debating is a common extracurricular activity across the world. There are debating societies in the United States, United Kingdom, Poland, Thailand, Ghana, and many other countries. Many youth debaters participate in international debating competitions and national debating leagues. School debates can adopt different formats. In a common format (“parliamentary debating”), eight youths are divided in four teams of two. The teams are randomly assigned to the proposition and opposition roles (two teams per role) and are given a motion (e.g. This House Would Welcome Immigrants), which they subsequently endorse or reject, depending on their assigned role in the debate. Each team has about 15 minutes to prepare arguments to defend its position in the debate. The most convincing team wins the contest. The winner is determined by a judge or a group of judges (oftentimes teachers) who are trained to focus on well formulated and meritocratic arguments. Our study draws on evidence from Poland, where, school debating is currently being considered as a possible addition to the high school curricula. Our partner organization (Fundacja Polska Debatuje) is implementing a trial version of the program across 25 high schools in four big cities. The organization invited us to evaluate the effects of the debating training on youth's behaviors, attitudes and ability to detect misinformation.

This study examines public support for “selective repression” as an approach to deal with drug related violence in two Brazilian cities, Rio de Janeiro and São Paulo. We present participants with a short vignette explaining how this approach work, manipulating the specific type of violence it aims to tackle and the policy trade-offs that it may imply.

How does the COVID-19 pandemic affect interpersonal trust? Most evidence shows that natural disasters reinforce trust and cooperation, but the COVID-19 virus differs from other calamities, since it spreads through contact with people, potentially increasing suspicion and distrust, as, according to contemporaneous writers’ accounts, seems to have been the case with the Black Death, the London plague, and the Spanish influenza. We investigate the link between interpersonal trust and individuals exposed to COVID-19, either vicariously through their community or networks or directly by becoming infected. We rely on an original panel survey, including a survey experiment, with a representative sample of adults in Italy, one of the countries hardest struck by the pandemic. Our experimental findings reveal that priming people about the risk that the pandemic poses to their health leads to a substantial increase in their trust in strangers. Our panel data analysis of within-individual effects shows that those who become infected trust strangers more than those who are not infected. Our findings could be explained by people observing higher than expected altruistic behavior or becoming more dependent on other people’s support, consistent with the “emancipation theory of trust.” When people recover from COVID-19, however, they drop to trusting strangers as much as those who were not directly exposed to the virus, an indication that the positive effects on trust during the pandemic have an emotional source. Nonetheless, the evidence suggests that, in the aggregate, there has been a small but significant increase in trust among the general population relative to prepandemic levels.

Necrosis seen in histopathology Whole Slide Images is a major criterion that contributes towards scoring tumour grade which then determines treatment options. However conventional manual assessment suffers from inter-operator reproducibility impacting grading precision. To address this, automatic necrosis detection using AI may be used to assess necrosis for final scoring that contributes towards the final clinical grade. Using deep learning AI, we describe a novel approach for automating necrosis detection in Whole Slide Images, tested on a canine Soft Tissue Sarcoma data set consisting of canine Perivascular Wall Tumours (cPWTs). A patch-based deep learning approach was developed where different variations of training a DenseNet-161 Convolutional Neural Network architecture were investigated as well as a stacking ensemble. An optimised DenseNet-161 with post-processing produced a hold-out test F1-score of 0.708 demonstrating state-of-the-art performance. This represents a novel first-time automated necrosis detection method in the canine Soft Tissue Sarcoma domain as well specifically in detecting necrosis in cPWTs demonstrating a significant step forward in reproducible and reliable necrosis assessment for improving the precision of tumour grading.

BACKGROUND AND AIMS Traditional vaccination strategies had poor effectiveness in host response in haemodialysis (HD) patients [1]. That is why a booster dose of SARS-CoV-2 vaccine is highly recommended to prevent worse outcomes. The aim of the study was to determine the humoral response in HD patients after the booster dose of mRNA-1273 vaccine (Moderna) [2]. METHOD In October 2021, HD patients received the booster dose and in November 2021 Anti-SARS-CoV-2 antibodies (Ab) were assessed during a cross-sectional retrospective study. Clinical data, laboratory tests and immunomodulatory therapies (IT) were collected from medical records. Of 194 patients enrolled, 26 were excluded for IT. SARS-CoV-2 Ab dosage was carried out in 168 patients using DiaSorin LIAISON SARS-CoV-2 trimetricSpike IgG (DiaSorin, Saluggia, Italy). Absence of humoral immunity was diagnosed when the antibodies value was RESULTS The median age was 76 [interquartile range (IQR) 60–83] years, 102 male and 66 female. 20 of these had previous SARS-CoV-2 infection. In 140 patients that received booster dose, Ab were detected after 30 ± 6 days. The response rate was 99.3%. Those who did not have SARS-CoV-2 infection showed an Ab median title of 10 700 BAU/mL (IQR 3482–18 625) while those who had previous infection showed an Ab median title of 18 300 BAU/mL (IQR 1200–37 500) (P >0.05). There was a negative correlation between age and Ab title (P Ab were detected in all patients who assumed only 2 vaccine doses (mean time after last dose, 121 ± 88 days). In this group, Ab title was higher in previously infected patients (18 500 BAU/mL [IQR 15 800–44 700]) than in those who were not infected [172 BAU/mL (IQR 110–750)] (P Focusing on patients with previous SARS-CoV-2 infection, 2 vaccine's doses enhanced Ab title more than infection alone [455 BAU/mL (IQR 212–3500)] (P 0.05). Lastly, SARS-CoV-2 infection significantly enhanced humoral immunity response independently to time elapsed since infection (P >0.05). The distribution of Ab among patients is detailed in Figure 1. CONCLUSION Early response to booster vaccine dose was present in the majority of our HD patients. Ab title was higher in younger and female patients [33]. According to other studies [4], a completed cycle of vaccine (at least 2 doses) was also recommended in patients with a previous SARS-CoV-2 infection because of the decrease of the Ab title in this group of patients. A crucial question remains unsolved: how long vaccine-induced protection will last in HD patients after booster dose?

BACKGROUND AND AIMS A great amount of information has been divulged on the epidemiology and outcome of coronavirus disease 2019 (COVID-19) in patients with ESRD. The majority of the studies have been conducted in patients on maintenance hemodialysis (HD) and kidney transplant recipients. Unfortunately, few studies focused on the outcome of peritoneal dialysis (PD) patients. Information regarding this subset of the population has been extrapolated from aggregated data including a higher percentage of HD patients. As a result, the impact of COVID-19 is indefinite in patients receiving PD. We conducted a study to better understand how patients on PD have been affected by COVID-19. METHOD We conducted a single-center retrospective analysis of 141 PD patients followed at the University Hospital of Modena, Italy from 1 March 2020 to 31 December 2021. The diagnosis of COVID-19 was performed through nasopharyngeal swab RT–PCR testing. Duration of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) shedding measured the time elapsed from diagnosis of COVID-19 to one or two (if available) negative nasopharyngeal PCR tests. Median and interquartile range or mean and standard deviation were used for continuous variables and percentage for categorical variables. A P-value RESULTS During the pandemic, 18 out of 141 (12.7%) patients receiving PD dialysis contracted COVID-19. Median age was 60 (50.2–66.5) years with a predominance of males (72.2%) The percentage of patients on APD accounted for 33.3%. The infection was symptomatic in out of 18 (94.4%) patients. Fever (94.4%) and cough (55.6%) were the most common symptoms. Viral shedding, traced with nasopharyngeal swabs lasted 26 (14.5–3.5) days. Two patients were inactive on the waiting list for kidney transplantation for a mean of 43 ± 1.4 days. COVID-19 caused hospital admission of seven (38.9%) patients. During hospitalization two (11.1%) patients switched from PD to HD for ultrafiltration failure and inadequate solute clearance and two (11.1%) died for septic shock with multiorgan failure. In our cohort of patients, excess death due to COVID-19 was 22.2%. Half of the patients contracted the infection before the availability of SARS-CoV-2 vaccine. There were no statistically significant differences between vaccinated and unvaccinated patients in terms of symptoms, viral shedding and hospital admission or (Table 1). We underline that COVID-19 was fatal only in two unvaccinated patients. CONCLUSION This study reports the monocentric experience of a large PD center during the COVID-19 pandemic. COVID-19 was symptomatic in the majority of patients and led to hospitalization of about 40% of the patients. The rate of symptoms, viral shedding and hospital admission was similar between vaccinated and unvaccinated patients. Two unvaccinated patients died for the severe consequence of COVID-19.

BACKGROUND AND AIMS mRNA-1273 vaccine (previously known as vaccine Moderna) has shown 94.1% efficacy at preventing COVID-19 illness in the general population. Vaccine-related adverse events (AEs) were usually mild or moderate in intensity and resolved within a few days. Nevertheless, the fear of developing AEs led some patients on haemodialysis to deny vaccination or additional booster doses. No studies have been conducted to evaluate the reactogenicity of the mRNA-1273 vaccine in dialysis patients. To inform public health and clinical practice, we investigated the safety of the mRNA-1273 vaccine in a cohort of patients on haemodialysis. METHOD We conducted a retrospective analysis of in-centre haemodialysis patients without a prior COVID-19 diagnosis who underwent mRNA-1273 vaccine from 1 March to 30 April 2021. mRNA-1273 vaccine was performed in all patients without signs of ongoing infection or COVID-19 who provided written consent from 24 March to 30 April 2021. AEs occurring after the first and the second doses were collected and classified as local or systemic. RESULTS Overall, 126 patients on chronic maintenance dialysis were vaccinated with two doses of mRNA-1273 vaccine. Mean age was 68 (IQR, 54.7–76) years and 53.6% of patients were aged ≥65 years (Table 1). AEs occurred in 57.9% and 61.9% of patients after the first dose and second dose, respectively. The most common AEs were injection-site pain (61.9%), erythema (4.8%), itching (4.8%), swelling (16.7%), axillary swelling/tenderness (2.4%), fever (17.5%) headache (7.9%), fatigue (23.8%), myalgia (17.5%), arthralgia (12.7%), dyspnoea (2.4%), nausea/vomiting (7.1%), diarrhoea (5.6%), shivers (4%) and vertigo (1.6%). The rates of local AEs were similar after the first and second doses (P = .8), whereas systemic AEs occurred more frequently after the second dose (P = .001). Fever (P = .03), fatigue (P = .02) and nausea/vomiting (P = .03) were significantly more frequent after the second dose of the vaccine (Figure 1). Analysis of the data detected statistically significant differences in duration of axillary swelling/tenderness (P = .07) and diarrhoea (P = .02) between the first and second. In both cases, these symptoms lasted longer after the second dose of the vaccine. There were no age-related differences in the rate of AEs between older (≥65 years) and younger participants (18–64 years). Lastly, we noted a lower rate of AEs in hemodialysis patients after the first dose (57.9% versus 84.2%) and second doses (61.9% versus 88.6%) compared to the general population. CONCLUSION RNA-1273 vaccine was associated with the development of transient AEs after the first (57.9%) and second doses (61.9%) in patients on haemodialysis. Systemic AEs were more common after the second dose than the first dose of vaccine. The duration of AEs lasted for a few days, without any apparent consequences. These data confirm the safety of the RNA-1273 vaccine in haemodialysis patients and support the promotion of COVID-19 vaccination in hesitant patients.

The militarization of security enjoys strong popular support in various crime-ridden countries. Yet, we know little about the determinants of such support. Do people support militarization even in the face of human fatalities? We tackle this question in the context of Mexico's “war on drugs." In three experimental studies, we manipulate the presence of human costs in a military operation against a drug lord and present arguments either justifying or condemning these costs. We consistently find that, even in successful operations, support for militarization decreases when military operations involve civilian casualties, but not when casualties are cartel-related. This finding holds both for victims and non-victims of cartel-related violence. Yet, arguments that justify these costs in light of the goal of eradicating organized crime increase support. These findings shed light on the public opinion side of the militarization of security debate, with important implications for security policy reform and democratic politics.

Safe and timely discontinuation of quarantine of in-center hemodialysis (HD) patients with a previous severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection is a challenging issue for the nephrological community because current guidelines for ending isolation do not mention dialysis patients. To prevent potentially fatal outbreaks of coronavirus disease 2019 (COVID-19), a cautionary approach has been adopted by most dialysis units. The criteria for ending the isolation in the HD population generally coincide with those recommended for immunocompromised people. Thus, a test-based strategy relying on two consecutive negative reverse transcriptase-polymerase chain reaction (RT-PCR) nasopharyngeal swabs has been adopted to terminate quarantine. This strategy has the disadvantage of prolonging isolation as RT-PCR positivity does not equate to SARS-CoV-2 infectivity. Consequentially, prolonged positivity of SARS-CoV-2 results in excessive workload for the HD staff who must face an increasing number of COVID-19 patients requiring isolation. This condition leads also to serious implications for the patients and their households including work productivity loss, postponement of health-care appointments and an increased risk of COVID-19 reinfection. To counteract this problem, other diagnostic tests should be used to provide the best care to HD patients. Recent results seem to encourage the use of RT-PCR cycle threshold (Ct) values and rapid antigen tests given their better correlation with cell culture for SARS-CoV-2 than RT-PCR testing. Here, we provide an overview of the current scientific evidence on the tests used to verify the infectiousness of the virus in order to stimulate the nephrological community to adopt a streamlined and pragmatic procedure to end isolation in COVID-19 patients on HD.

COVID-19 is an unpredictable infectious disease caused by SARS-CoV-2. The development of effective anti-COVID-19 vaccines has enormously minimized the risk of severe illness in most immunocompetent patients. However, unvaccinated patients and non-responders to the COVID-19 vaccine are at risk of shortand long-term consequences. In these patients, the outcome of COVID-19 relies on an interplay of multiple factors including age, immunocompetence, comorbidities, inflammatory response triggered by the virus as well as the virulence of SARS-CoV-2 variants. Generally, COVID-19 is asymptomatic or mildly symptomatic in young people, but it may manifest with respiratory insufficiency requiring mechanical ventilation in certain susceptible groups of patients. Furthermore, severe SARS-CoV-2 infection induces multiorgan failure syndrome by affecting liver, kidney heart and nervous system. Since December 2019, multiple drugs have been tested to treat COVID-19, but only a few have been proven effective to mitigate the course of the disease that continues to cause death and comorbidity worldwide. Current treatment of COVID-19 patients is essentially based on the administration of supportive oxygen therapy and the use of specific drugs such as steroids, anticoagulants, antivirals, anti-SARS-CoV-2 antibodies and immunomodulators. However, the rapid spread of new variants and the release of new data coming from the numerous ongoing clinical trials have created the conditions for maintaining a continuous updating of the therapeutic management of COVID-19 patients. Furthermore, we believe that a well-established therapeutic strategy along with the continuum of medical care for all patients with COVID-19 is pivotal to improving disease outcomes and restoring healthcare care fragmentation caused by the pandemic. This narrative review, focusing on the therapeutic management of COVID-19 patients, aimed to provide an overview of current therapies for (i) asymptomatic or mildly/moderate symptomatic patients, (ii) hospitalized patients requiring low-flow oxygen, (iii) high-flow oxygen and (iv) mechanical ventilation.

Digital pathology (DP) and whole slide images (WSIs) are rapidly becoming the first option for routine diagnostics. Successful application of artificial intelligence (AI) methods such as machine learning algorithms (ML) to WSIs have the potential to create new supportive diagnostic clinical tools that can improve diagnostic accuracy, reproducibility and objectivity and provide new insights into human and canine cancer., This thesis focused on the detection and characterisation of canine soft tissue sarcomas (cSTSs) using ML. Currently, the diagnosis of cSTSs is based on histological assessment. In particular, by assessing certain histological features such as the degree of differentiation, necrosis score and mitotic score, it is possible to define a final tumour grade, which aids in prognostication for patients. Due to the subjectivity of the scoring system, grade disagreements are reported in human and cSTS cases. These results were confirmed in our study where we found only a fair level (κ = 0.36) of diagnostic concordance between pathologists in grading these tumours illustrating the need for automated image analysis tools. In order to achieve this goal, the first essential step was to create an appropriate and comprehensive digital slide dataset. The study gathered a large-scale dataset of 1166 histopathological WSIs of cSTSs (n=752), canine mast cell tumours (MCTs) (n=359) and canine apocrine gland anal sac adenocarcinoma (AGASACAs) (n=55) with related clinical information. Once the slide panel was assembled, the study focused on providing a tool for automatic detection of tumour necrosis and mitosis in annotated histopathological WSI of STS using ML. The ML algorithm applied (DenseNet161) had an accuracy for necrosis and mitosis detection of 92.7% and 50.0%, respectively. In conclusion, the results presented here have demonstrated that digital pathology and ML algorithms can potentially be used as a diagnostic support tool for the detection and characterisation of cSTSs.

Scholars usually investigate how average levels of trust in institutions vary across countries and over time. Focusing on average levels, however, ignores distributional properties that might be equally relevant for institutional legitimacy and, more broadly, democratic stability. In this study, the authors investigate how the distribution of trust in the European Parliament has changed over time and across European Union member states. Drawing on pooled cross-sectional data from the European Social Survey for the period from 2002 to 2020, the authors find that confidence in the European Parliament has not only declined over time but also polarized as citizens have increasingly moved away from the “average citizen.” Furthermore, the authors find that trust has polarized, especially among the young versus the elderly and the employed versus the unemployed. These findings have implications for the legitimacy of European Union institutions.

In the next decades, ultra-high-energy neutrinos in the EeV energy range will be potentially detected by next-generation neutrino telescopes. Although their primary goals are to observe cosmogenic neutrinos and to gain insight into extreme astrophysical environments, they can also indirectly probe the nature of dark matter. In this paper, we study the projected sensitivity of up-coming neutrino radio telescopes, such as RNO-G, GRAND and IceCube-gen2 radio array, to decaying dark matter scenarios. We investigate different dark matter decaying channels and masses, from $10^7$ to $10^{15}$ GeV. By assuming the observation of cosmogenic or newborn pulsar neutrinos, we forecast conservative constraints on the lifetime of heavy dark matter particles. Many extensions of the Standard Model predict the existence of new charged or neutral gauge bosons, with a wide variety of phenomenological implications depending on the model adopted. The search for such particles is extensively carried through at the Large Hadron Collider (LHC), and it is therefore of crucial importance to have for each proposed scenario quantitative predictions that can be matched to experiments. In this work we focus on the implications of one of these models, the TopFlavor Model, proposing a charged $\text{W}^\prime$ boson that has preferential couplings to the third generation fermions. We compare such predictions to the ones from the so called Sequential Standard Model (SSM), that is used as benchmark, being one of the simplest and most commonly considered models for searches at the LHC. We identify the parameter space still open for searches at the LHC, and in particular we show that the cross section for the processes $pp \to \text{W}^\prime \to \tau \nu$ and $pp \to \text{W}^\prime \to tb$ can be up to two orders of magnitude smaller with respect to the SSM, depending on the free parameters of the model, like the particle mass and its width. This study makes the case for further searches at the LHC, and shows how a complete and systematic model independent analysis of $\text{W}^\prime$ boson phenomenology at colliders is essential to provide guidance for future searches., Comment: 11 pages, 3 figures

Purpose: In search of novel strategies to improve the outcome of advanced prostate cancer, we considered that prostate cancer cells rearrange iron homeostasis, favoring iron uptake and proliferation. We exploited this adaptation by exposing prostate cancer preclinical models to high-dose iron to induce toxicity and disrupt adaptation to androgen starvation. Experimental Design: We analyzed markers of cell viability and mechanisms underlying iron toxicity in androgen receptor–positive VCaP and LNCaP, castration-resistant DU-145 and PC-3, and murine TRAMP-C2 cells treated with iron and/or the antiandrogen bicalutamide. We validated the results in vivo in VCaP and PC-3 xenografts and in TRAMP-C2 injected mice treated with iron and/or bicalutamide. Results: Iron was toxic for all prostate cancer cells. In particular, VCaP, LNCaP, and TRAMP-C2 were highly iron sensitive. Toxicity was mediated by oxidative stress, which primarily affected lipids, promoting ferroptosis. In highly sensitive cells, iron additionally caused protein damage. High-basal iron content and oxidative status defined high iron sensitivity. Bicalutamide–iron combination exacerbated oxidative damage and cell death, triggering protein oxidation also in poorly iron-sensitive DU-145 and PC-3 cells. In vivo, iron reduced tumor growth in TRAMP-C2 and VCaP mice. In PC-3 xenografts, bicalutamide–iron combination caused protein oxidation and successfully impaired tumor expansion while single compounds were ineffective. Macrophages influenced body iron distribution but did not limit the iron effect on tumor expansion. Conclusions: Our models allow us to dissect the direct iron effect on cancer cells. We demonstrate the proof of principle that iron toxicity inhibits prostate cancer cell proliferation, proposing a novel tool to strengthen antiandrogen treatment efficacy.

Adaptation to import iron for proliferation makes cancer cells potentially sensitive to iron toxicity. Iron loading impairs multiple myeloma (MM) cell proliferation and increases the efficacy of the proteasome inhibitor bortezomib. Here, we defined the mechanisms of iron toxicity in MM.1S, U266, H929, and OPM-2 MM cell lines, and validated this strategy in preclinical studies using Vk*MYC mice as MM model. High-dose ferric ammonium citrate triggered cell death in all cell lines tested, increasing malondialdehyde levels, the by-product of lipid peroxidation and index of ferroptosis. In addition, iron exposure caused dose-dependent accumulation of polyubiquitinated proteins in highly iron-sensitive MM.1S and H929 cells, suggesting that proteasome workload contributes to iron sensitivity. Accordingly, high iron concentrations inhibited the proteasomal chymotrypsin-like activity of 26S particles and of MM cellular extracts in vitro. In all MM cells, bortezomib-iron combination induced persistent lipid damage, exacerbated bortezomib-induced polyubiquitinated proteins accumulation, and triggered cell death more efficiently than individual treatments. In Vk*MYC mice, addition of iron dextran or ferric carboxymaltose to the bortezomib-melphalan-prednisone (VMP) regimen increased the therapeutic response and prolonged remission without causing evident toxicity. We conclude that iron loading interferes both with redox and protein homeostasis, a property that can be exploited to design novel combination strategies including iron supplementation, to increase the efficacy of current MM therapies.

Assuming a type-I seesaw mechanism for neutrino mass generation and invoking a baryogengesis via leptogenesis scenario, we consider a reasonable hierarchical structure for Dirac neutrino mass matrix, similar to up-type quark mass matrix. These hypotheses imply a relevant connection between high scale CP violation and low energy one. By requiring a compact heavy neutrino mass spectrum, which allows to circumvent Davidson-Ibarra limit, one can obtain an efficient leptogenesis restricting the allowed region for low energy neutrino parameters. Once the oscillating parameters are taken inside a $3\sigma$ range, through the numerical resolution of the leptogenesis Boltzmann equations one gets the following allowed intervals for the lightest neutrino mass and the Dirac CP phase: $-0.90\pi, Comment: 15 pages, 2 figures

How do voters react to an ongoing natural threat? We address this question by investigating voters’ reactions to the early spread of COVID-19 in the 2020 French municipal elections. Using a novel, fine-grained measure of the circulation of the virus based on excess-mortality data, we find that support for incumbents increased in the areas that were particularly hit by the virus. Incumbents from both left and right gained votes in areas more strongly affected by COVID-19. The results are robust to a placebo test and hold across different methods, including regressions with lagged dependent variables, a differences-in-differences approach and propensity score matching. We also provide indirect evidence for two mechanisms that can explain our findings: an emotional channel related to feelings of fear and anxiety, and a prospective-voting channel, related to the ability of incumbents to act more swiftly against the diffusion of the virus than challengers.

Insufficient vaccine coverage and dominance of the more transmissible severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) variants are the leading causes of the continued spread of coronavirus disease 2019 (COVID-19) worldwide. To curb the surge in infections, COVID-19 vaccination has been advocated as a priority measure, especially for frail populations and people at high risk of exposure. Patients on in-centre maintenance haemodialysis (HD) embody both conditions. They are at high risk of severe COVID-19 consequences due to their advanced age and weakened immune system and carry an increased risk of SARS-CoV-2 transmission within shared dialysis rooms and public vehicles. Vaccination of the entire HD population is therefore the most effective strategy to protect patients from the dire consequences of COVID-19. Unfortunately, a minority of patients still express COVID-19 vaccine hesitancy. The management of this group of patients, who have the full right to HD treatment, poses demanding problems from a patient safety perspective. The placement of unvaccinated patients within the dialysis room and the protection of all vaccinated patients are some of the most urgent problems the nephrologist faces during the COVID-19 pandemic. In light of these COVID-19-driven changes, an ethical reflection on the management of unvaccinated patients appears crucial to act responsibly and contribute to the health promotion of dialysis patients.

Research has documented that individuals display a bias for preserving the status quo across numerous domains of decision-making, including elections for candidates and referendums. Yet, it is not clear whether thinking about a political reform as a change to the status quo actually makes voters less likely to support it. We investigate this possibility in a referendum campaign, in which we prime a representative sample of voters with a “change cue” evoking the modification of the status quo related to a proposed reform. Our findings show that support for a referendum proposal decreases when voters consider that it will change the status quo, but only among right-wing voters. The effect is stronger among less knowledgeable voters on the right (but not on the left) of the political spectrum. Furthermore, we find that the priming manipulation has no effect in the presence of campaign arguments, thus suggesting that voters might discard peripheral cues when substantial policy information is available. These findings have relevant implications for the goal of achieving political change in democratic politics, and highlight the key role of ideology in moderating the status-quo bias in political decision-making.

Voting decisions in high-stakes referendums can have crucial consequences for the fate of national governments and the implementation of major reforms. Prior studies have found that referendum campaigns can substantially influence their outcomes. Yet few have taken into account the fact that the effect of campaign arguments depends on a number of factors, including individual knowledge levels and the degree of uncertainty surrounding the alternatives on the ballot. In this study we investigate how political knowledge and campaign arguments stressing risks and opportunities influenced vote choice in Colombia’s 2016 peace referendum. Drawing on a nationally representative survey (Study 1) and an original experiment (Study 2), we find that stressing the opportunities that the peace deal could bring to the country, rather than the risks associated with failing to conclude it, increased the probability that Colombians voted Yes in the referendum. While highly knowledgeable voters were more likely to support the deal than those with little knowledge, we find that pro-referendum opportunity arguments reduced the gap between these two groups by increasing the likelihood of a Yes vote among those with little knowledge. These findings contribute to research on voting behavior and campaign effects in direct democracies. Additionally, by exploring the crucial issue of attitudes towards peace, our findings also have important implications for countries trying to secure citizens’ approval of high-stakes issues—such as negotiating the end of decades of war—through democratic instruments.

BackgroundKidney transplant recipients with COVID-19 are at high risk of poor outcome because of comorbidities and immunosuppression. The effects of immunosuppressive therapy reduction are unclear in patients with COVID-19.MethodsWe conducted a retrospective study on 45 consecutive kidney transplant recipients followed at the University Hospital of Modena who tested positive for COVID-19 by RT-PCR analysis.ResultsThe median age of patients was 56.1 (interquartile range, [IQR] 47.3-61.1) years with a predominance of male (64.4%). Kidney transplantation vintage was 10.1 (2.7-16) years, and more than half of patients (55.6%) was on triple immunosuppressive therapy. Early reduction of immunosuppression occurred in 62.8% of patients and included antimetabolite (88.8%) and calcineurin inhibitor withdrawal (22.2%).Of the 45 patients, 88.9% became symptomatic and 40% required hospitalization. Overall mortality was 17.8%. There were no differences in outcomes between full- and reduced-dose immunosuppressive therapy at the end of follow-up. One hospitalized patient experienced irreversible graft failure. There were no differences in serum creatinine level and proteinuria in non-hospitalized patients with COVID-19. Admitted patients had better kidney function after dismission (P=0.019). Risk factors for death were age (odds ratio [OR]: 1.19; 95%CI: 1.01-1.39), and duration of kidney transplant (OR: 1.17; 95%CI: 1.01-1.35). One kidney transplant recipient experienced symptomatic COVID-19 reinfection after primary infection and anti-SARS-CoV-2 mRNA vaccine.ConclusionsDespite the reduction of immunosuppression, COVID-19 affected survival of kidney transplant recipients with COVID-19. Age and duration of kidney transplant were independent predictors of death in COVID-19. Early kidney function was favorable in most survivors after COVID-19.

Primordial black holes (PBHs) hypothetically generated in the first instants of life of the Universe are potential dark matter (DM) candidates. Focusing on PBHs masses in the range $[5 \times10^{14} - 5 \times 10^{15}]$g, we point out that the neutrinos emitted by PBHs evaporation can interact through the coherent elastic neutrino nucleus scattering (CE$\nu$NS) producing an observable signal in multi-ton DM direct detection experiments. We show that with the high exposures envisaged for the next-generation facilities, it will be possible to set bounds on the fraction of DM composed by PBHs improving the existing neutrino limits obtained with Super-Kamiokande. We also quantify to what extent a signal originating from a small fraction of DM in the form of PBHs would modify the so-called "neutrino floor", the well-known barrier towards detection of weakly interacting massive particles (WIMPs) as the dominant DM component., Comment: 8 pages; 4 figures
