2. INJURIES IN THE NATIONAL CAPITAL REGION – A CROSS-SECTIONAL STUDY OF THE CHALLENGING EPIDEMIC

- Abstract
Background: Injuries are an emerging public health problem that has grown into epidemic proportions. Many injuries are linked to social, environmental, cultural, and biological issues in causation; recognized as man-made and behaviour-linked disorders, and linked to sociodemographic transition. Prevention, acute care, and rehabilitation are the major challenges we are facing today. Materials & Methods: A crosssectional study was conducted at the National Injury Surveillance Centre (NISC) Department of Dr. Ram Manohar Lohia Hospital, New Delhi. Data was collected from 2861 participants (>18 years) of injury cases during 12 months (Jan-Dec 2019). Participants were selected using the Convenience Sampling Technique. The data were analysed statistically using SPSS software to assess the risk factors and pattern of injury among the population of the National Capital Region, Delhi. Results: In this study, the mean age was 36.6 years. The male to female ratio was 2.1:0.7. The age group 18-45 years is affected predominantly (71.1%). Around 48.8% were students (matriculation/junior school). 39.5% (1131) were unemployed. 1999 (69.9%) were accidental (unintentional), 1671 (58.4%) were having moderate to severe injury, 1675 (58.5%) happened onroad, and 1341 (46.9%) were having soft tissue injury. 1198 (41.9%) were RTI cases and 816 (28.5%) were assault/quarrel cases, the second most common mode of injury. Conclusion: With the realization that injuries are caused by a complex interaction among agents (vehicle, product), and human and environmental factors operating in complex social and economic systems, injury prevention and control depending on evidence-based research is gaining momentum. There is an urgent need for developing comprehensive, integrated, and intersectoral approaches based on scientic understanding.

Background: A guideline to determine pediatric brain death was updated in 2011. It is unknown how pediatric intensivists have accepted and adopted the revised guideline into clinical practice. Methods: We surveyed US pediatric critical care attending physicians July 2013 to September 2013 and February 2020 to May 2020. Brain death testing practices and utilization of the 2011 pediatric and neonatal brain death guideline were assessed. Results: The 2020 respondents found that the revised pediatric brain death guideline were useful in clinical practice (93.7% vs 83.3%, P = .0484) and provided more consistency and clarity (73.2% vs 63.1%, P = .0462) when compared to 2013 respondents. Conclusion: This study demonstrates that with defined criteria, survey participants reported increased clarity and consistency. Findings from our study indicate that in clinical practice there is no significant deviation from the minimum requirements to determine brain death in children as outlined in the 2011 guideline.

Background Investigating the context of trauma and acute care surgery, the article aims at understanding the factors that can enhance some ethical aspects, namely the importance of patient consent, the perceptiveness of the ethical role of the trauma leader, and the perceived importance of ethics as an educational subject. Methods The article employs an international questionnaire promoted by the World Society of Emergency Surgery. Results Through the analysis of 402 fully filled questionnaires by surgeons from 72 different countries, the three main ethical topics are investigated through the lens of gender, membership of an academic or non-academic institution, an official trauma team, and a diverse group. In general terms, results highlight greater attention paid by surgeons belonging to academic institutions, official trauma teams, and diverse groups. Conclusions Our results underline that some organizational factors (e.g., the fact that the team belongs to a university context or is more diverse) might lead to the development of a higher sensibility on ethical matters. Embracing cultural diversity forces trauma teams to deal with different mindsets. Organizations should, therefore, consider those elements in defining their organizational procedures. Level of evidence Trauma and acute care teams work under tremendous pressure and complex circumstances, with their members needing to make ethical decisions quickly. The international survey allowed to shed light on how team assembly decisions might represent an opportunity to coordinate team member actions and increase performance.

Crop yield data is critical for managing sustainable agriculture and assessing national food security. Current study aims to increase Peanut productivity from current levels by analyzing the yield gap of production potential between theoretical yield and actual farmers’ yields. The spatial yield gap of Peanut for Thiruvannamalai district of Tamil Nadu is examined in this paper by integrating the products of microwave remote sensing (SAR Sentinel-1A) with DSSAT CROPGRO peanut simulation model. CROPGRO Peanut model was calibrated and validated by conducting field experiment at Oilseeds Research Station, Tindivanam during Rabi 2019 for predominant cultivars viz. TMV 7, TMV 13, VRI 2 and G 7. Actual attainable yield was recorded by organizing CCE with help of Department of Agriculture Economics and Statistics in the respective monitoring Villages. Regression analysis between maximum recorded DSSAT Leaf Area Index (LAI) at peak flowering stage of peanut and yield recorded by Crop Cutting Experiment (CCE) for spatial yield estimation of Peanut in Thiruvannamalai district of Tamil Nadu during Rabi 2021 was carried out using ArcGIS 10.6 software. The results showed that the simulated potential yield ranged from 3194 to 4843 kg/ha, whereas actual yield ranged from 1228 to 3106 kg/ha, with a considerable disparity between the actual and potential yield levels (1217 to 2346 kg/ha) of the monitored locations. The minimum, maximum and average yield gaps in Peanut for Thiruvannamalai district was assessed as 1890, 2324 and 2134 kg/ha, respectively. To reduce the production difference (Yield gap) of Peanut cultivation, farmers should focus more on management issues such as time of sowing, irrigation or water management, quantity and sources of nutrients, cultivar selection and availability of quality seeds tailored to each region.

The study focuses on the isolation, characterization, and expression analysis of a lectin from the hepatopancreas of Macrobrachium rosenbergii. The protein was isolated by affinity chromatography on a melibiose-agarose column. The molecular weight of the native protein was found to be ~120 kDa which consists of a single polypeptide of ~39.5 kDa. On mass spectrometric analysis, the protein was identified as lipopolysaccharide- and beta-1,3-glucan binding protein (LGBP). LGBP showed hemagglutination with rabbit RBC like a lectin and its carbohydrate-binding specificity was determined by the hemagglutination inhibition test. The protein also showed antibacterial activity against two Gram-negative bacteria Vibrio harveyi and Aeromonas sobria, and one Gram positive bacteria Bacillus cereus in the disc diffusion test. Rabbit antiserum was raised against the purified LGBP and used to develop a sandwich ELISA system for quantitation of the protein in hepatopancreas and serum samples of M. rosenbergii. The expression of the LGBP transcripts in muscle, hepatopancreas, and gill tissues from M. rosenbergii juveniles at 72 h post-challenge of V. harveyi was not modulated as noticed in qPCR analysis. However, significant increases in the concentrations of LGBP protein in hepatopancreas (5.23 ± 0.45 against 3.43 ± 0.43 mg/g tissue in control) and serum (1.08 ± 0.14 against 0.61 ± 0.08 µg/ml in control) were observed in the challenged group of prawns in ELISA suggesting its putative role against bacterial infections. The study for the first time characterized the native LGBP of M. rosenbergii showing a multifunctional role in immunity.

Background:To distinguish post-neurosurgical bacterial meningitis (PNBM) from aseptic meningitis is difficult. Inflammatory and biochemical cerebrospinal fluid (CSF) changes mimic those classically observed after CNS surgery. CSF lactate assay has therefore been proposed as a useful PNBM marker. Objective:To determine the value of cerebrospinal fluid (CSF) lactate level for the identification of bacterial meningitis following cranial surgery. Methods:Between January 2016 and December 2016, a prospective clinical study was done in Department of Neurosurgery, in which all patients with clinical suspicion of PNBM were enrolled. Patients with clinical suspicion of bacterial meningitis BM were categorized, according to preset criteria, into 3 groups: (1) proven BM; (2) presumed BM, and (3) nonbacterial meningeal syndrome. CSF markers were plotted in a receiver operating curve (ROC) to evaluate their diagnostic accuracy. Results:The study included 70 patients. We obtained 65 CSF samples from patients with clinical suspicion of BM by CSF analysis. 20 corresponded to proven BM, 7 to probable BM and 38 to excluded BM. Mean lactate in CSF was: 8.4 ±3.0 mmol /l for proven BM, 4.8 ± 0.99 mmol /l for probable BM and 2.08 ± 0.822 mmol/l for excluded BM (P < .001). Conclusion: CSF lactate level has good predictive value to distinguish BM from aseptic meningitis with sensitivity of 90% and specificity: 87% at cut-off value: 4.0 mmol/l.

In the current exploration, the impact of the 100 to 125 micron size addition of silicon carbide (SiC) on the mechanical performance of Al6061 alloy has been studied. The Al6061 alloy dispersed with 6, 9, and 12 wt.% of SiC particles were synthesized by a two-step stir cast route. Two-step addition of the preheated particles into the melt helps avoid the agglomeration of the particles, which further contributes to enhancing the properties of composites. The orchestrated composites were exposed to microstructural examines and mechanical properties evaluation. Microstructural portrayals of acquired examples were completed by SEM microscopy, EDS, and XRD patterns. The event of SiC particles were affirmed by the XRD patterns. The density of the Al6061-SiC composites was increased with the addition of high-density silicon carbide particles. The hardness, ultimate, and yield qualities of metal composites have been improved with the increase in the content of SiC support. The ductility of SiC reinforced composites decreased with hard ceramic particles' incorporation in the Al matrix alloy. Various fracture mechanisms were observed in the Al6061-SiC composites using SEM.

Background Known risk alleles for epithelial ovarian cancer (EOC) account for approximately 40% of the heritability for EOC. Copy number variants (CNVs) have not been investigated as EOC risk alleles in a large population cohort. Methods Single nucleotide polymorphism array data from 13 071 EOC cases and 17 306 controls of White European ancestry were used to identify CNVs associated with EOC risk using a rare admixture maximum likelihood test for gene burden and a by-probe ratio test. We performed enrichment analysis of CNVs at known EOC risk loci and functional biofeatures in ovarian cancer–related cell types. Results We identified statistically significant risk associations with CNVs at known EOC risk genes; BRCA1 (PEOC = 1.60E-21; OREOC = 8.24), RAD51C (Phigh-grade serous ovarian cancer [HGSOC] = 5.5E-4; odds ratio [OR]HGSOC = 5.74 del), and BRCA2 (PHGSOC = 7.0E-4; ORHGSOC = 3.31 deletion). Four suggestive associations (P Conclusions CNVs in BRCA1 have been previously reported in smaller studies, but their observed frequency in this large population-based cohort, along with the CNVs observed at BRCA2 and RAD51C gene loci in EOC cases, suggests that these CNVs are potentially pathogenic and may contribute to the spectrum of disease-causing mutations in these genes. CNVs are likely to occur in a wider set of susceptibility regions, with potential implications for clinical genetic testing and disease prevention.

We describe a versatile pipeline for processing the data collected by the Ultra-Violet Imaging Telescope (UVIT) on board Indian Multi-wavelength astronomical satellite AstroSat.The UVIT instrument carries out simultaneous astronomical imaging through selected filters / gratings in Far-Ultra-Violet (FUV), Near-Ultra-Violet & visible (VIS) bands of the targeted circular sky field (~ 0.5 deg dia). This pipeline converts the data (Level-1) emanating from UVIT in their raw primitive format supplemented by inputs from the spacecraft sub-systems into UV sky images (& slitless grating spectra) and associated products readily usable by astronomers (Level-2). The primary products include maps of Intensity (rate of photon arrival), error on Intensity and effective Exposure. The pipeline is open source, extensively user configurable with many selectable parameters and its execution is fully automated. The key ingredients of the pipeline includes - extraction of drift in pointing of the spacecraft, and disturbances in pointing due to internal movements; application of various corrections to measured position in the detector for each photon - e.g. differential pointing with respect to a reference frame for shift and add operation, systematic effects and artifacts in the optics of the telescopes and detectors, exposure tracking on the sky, alignment of sky products from multi-episode exposures to generate a consolidated set and astrometry. Detailed logs of operations and intermediate products for every processing stage are accessible via user selectable options. While large number of selectable parameters are available for the user, a well characterized standard default set is used for executing this pipeline at the Payload Operation Centre (POC) for UVIT and selected products are archived and disseminated by the Indian Space Research Organization (ISRO) through its ISSDC portal., Accepted for publication in Journal of Astrophysics & Astronomy, 50 pages, 16 figures

- Abstract
Iterations in design is ubiquitous to drive constant innovation in the pursuit of excellence in product development, service delivery, systems, and subsystems integration. Within a given time frame of a design process, iteration can occur at different scales, forms, types, etc. Iteration was highlighted recently in the innovation framework using the four ‘D’s design scheme, i.e., Discover, Define, Develop, and Deliver. Iteration studies generally focus on its occurrence, modeling, classification, methods, and impact, among others, in the domain of software development, service providing, aesthetics, and functionality. However, aspects of an iteration involving complex systems like a reconfigurable system become more intriguing because of its overall morphology with changing features and the coupled design of several subsystems. This paper reports on the various factors influencing iteration and are illustrated using a case study of the development of a reconfigurable robotic system named Panthera and its subsystems designed for cleaning and maintenance tasks. The case study of Panthera is reported from its initial ideation phase to the three versions development over a span of three years. Following the journey of the system design of Panthera, it details the timeline, rationale, and insight into the iterative processes and how they are applied to the individual subsystems in consecutive design iterations. With no one solution that fits all approaches to the topic, we aim to highlight and stimulate concepts to aid designers in the design iteration.

IL-17-producing Th17 cells play an important role in pathogenesis of rheumatoid arthritis (RA). Aberrant immune activation due to an imbalance between Th17 and regulatory T (Treg) cells is associated with the development of RA and other autoimmune diseases. Targeting pathogenic Th17 cells and their associated molecules is emerging as a promising strategy to treat and reverse RA. Here, we demonstrate that IL-3 inhibits the differentiation of Th17 cells and promotes the development of Treg cells in IL-2-dependent manner. In IL-2 KO mice, we observed that IL-3 has no effect on differentiation of both Th17 and Treg cells. In addition, IL-3 decreases pathogenic IL-17A

28. A Conceptual Model to Share Resources and Align Goals: Building Blockchain Application to Support Care Continuity Outside a Hospital

The increased use of advanced technologies by consumers and hospitals is moving care closer to patients, and the challenge is one of how patient data can be shared with external care providers and patients. To support care continuity, patient data include both clinical data used by external care providers and non-clinical data used by social care providers. Care coordination of a patient outside a hospital requires peer-to-peer connectivity among a number of these clinical and social care providers, using a digital platform that aligns their goals and assigns their resource sharing responsibilities. With no single entity supporting such care coordination, most hospitals currently distribute this responsibility to several of its provider partners and patients. Such a division of responsibility with no real time feedback leads to discontinuous resource sharing, localized data analysis, and challenges in tailoring care to improve health outcomes. The goal of this paper is to propose a blockchain architecture model that uses a number of constructs for creating and assigning ownership to patient data so it can support peer-to-peer resource sharing and uses smart contracts to support goal alignment. Using two blockchain applications implemented in Hyperledger and illustrating their potential representation using the constructs in multi-chain, we develop a conceptual model for developing blockchain applications in general to support continuity of care. The generalizability of this model is illustrated by applying these constructs to four additional healthcare applications. Finally, we conclude the paper with a discussion of the limitations and directions for future research.

Background: There are limited studies pertaining to management of encephalocele in Nepal. So the present study seems justifiable to bridge the gap in the literature on encephalocele from Nepal on its clinical profile and early outcome. This study aims to characterize the clinical profile, management and outcome of largest series of encephalocele at tertiary care center in Nepal. Materials and Methods: A retrospective analysis of encephalocele, managed surgically at two tertiary care centers between 2015 and 2020, was performed. Results: Total of 25 cases was surgically managed in the present study. The median age of study population was 2.5 months. There were 11 male and 14 female with male to female ratio of 1:1.26. Occipital encephalocele was the most common variant. Lump in the head (n=11) was the commonest clinical presentation followed by hyperteliorism (n=10). One patient presented with cleft lip and one had CSF discharge in a case of occipital encephalocele. Bony defect was the common radiological findings. Excision and repair was the most common mode of surgery leading to good outcome. Mortality rate was 4% with morbidity of 20%. Conclusion: Early surgical excision and tight dural closure with repair of bony defect is the standard treatment with relatively good outcome.

The central role of β-catenin in the Wnt pathway makes it an attractive therapeutic target for cancers driven by aberrant Wnt signaling. We recently developed a small-molecule inhibitor, BC-2059, that promotes apoptosis by disrupting the β-catenin/transducin β-like 1 (TBL1) complex through an unknown mechanism of action. In this study, we show that BC-2059 directly interacts with high affinity for TBL1 when in complex with β-catenin. We identified two amino acids in a hydrophobic pocket of TBL1 that are required for binding with β-catenin, and computational modeling predicted that BC-2059 interacts at the same hydrophobic pocket. Although this pocket in TBL1 is involved in binding with NCoR/SMRT complex members G Protein Pathway Suppressor 2 (GSP2) and SMRT and p65 NFκB subunit, BC-2059 failed to disrupt the interaction of TBL1 with either NCoR/SMRT or NFκB. Together, our results show that BC-2059 selectively targets TBL1/β-catenin protein complex, suggesting BC-2059 as a therapeutic for tumors with deregulated Wnt signaling pathway. SIGNIFICANCE STATEMENT This study reports the mechanism of action of a novel Wnt pathway inhibitor, characterizing the selective disruption of the transducin β-like 1/β-catenin protein complex. As Wnt signaling is dysregulated across cancer types, this study suggests BC-2059 has the potential to benefit patients with tumors reliant on this pathway.

Background There is a paucity of data comparing 30-day morbidity and mortality of sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB), and one anastomosis gastric bypass (OAGB). This study aimed to compare the 30-day safety of SG, RYGB, and OAGB in propensity score-matched cohorts. Materials and methods This analysis utilised data collected from the GENEVA study which was a multicentre observational cohort study of bariatric and metabolic surgery (BMS) in 185 centres across 42 countries between 01/05/2022 and 31/10/2020 during the Coronavirus Disease-2019 (COVID-19) pandemic. 30-day complications were categorised according to the Clavien–Dindo classification. Patients receiving SG, RYGB, or OAGB were propensity-matched according to baseline characteristics and 30-day complications were compared between groups. Results In total, 6770 patients (SG 3983; OAGB 702; RYGB 2085) were included in this analysis. Prior to matching, RYGB was associated with highest 30-day complication rate (SG 5.8%; OAGB 7.5%; RYGB 8.0% (p = 0.006)). On multivariate regression modelling, Insulin-dependent type 2 diabetes mellitus and hypercholesterolaemia were associated with increased 30-day complications. Being a non-smoker was associated with reduced complication rates. When compared to SG as a reference category, RYGB, but not OAGB, was associated with an increased rate of 30-day complications. A total of 702 pairs of SG and OAGB were propensity score-matched. The complication rate in the SG group was 7.3% (n = 51) as compared to 7.5% (n = 53) in the OAGB group (p = 0.68). Similarly, 2085 pairs of SG and RYGB were propensity score-matched. The complication rate in the SG group was 6.1% (n = 127) as compared to 7.9% (n = 166) in the RYGB group (p = 0.09). And, 702 pairs of OAGB and RYGB were matched. The complication rate in both groups was the same at 7.5 % (n = 53; p = 0.07). Conclusions This global study found no significant difference in the 30-day morbidity and mortality of SG, RYGB, and OAGB in propensity score-matched cohorts.

Background Emergency surgery represents a unique context. Trauma teams are often multidisciplinary and need to operate under extreme stress and time constraints, sometimes with no awareness of the trauma’s causes or the patient’s personal and clinical information. In this perspective, the dynamics of how trauma teams function is fundamental to ensuring the best performance and outcomes. Methods An online survey was conducted among the World Society of Emergency Surgery members in early 2021. 402 fully filled questionnaires on the topics of knowledge translation dynamics and tools, non-technical skills, and difficulties in teamwork were collected. Data were analyzed using the software R, and reported following the Checklist for Reporting Results of Internet E-Surveys (CHERRIES). Results Findings highlight how several surgeons are still unsure about the meaning and potential of knowledge translation and its mechanisms. Tools like training, clinical guidelines, and non-technical skills are recognized and used in clinical practice. Others, like patients’ and stakeholders’ engagement, are hardly implemented, despite their increasing importance in the modern healthcare scenario. Several difficulties in working as a team are described, including the lack of time, communication, training, trust, and ego. Discussion Scientific societies should take the lead in offering training and support about the abovementioned topics. Dedicated educational initiatives, practical cases and experiences, workshops and symposia may allow mitigating the difficulties highlighted by the survey’s participants, boosting the performance of emergency teams. Additional investigation of the survey results and its characteristics may lead to more further specific suggestions and potential solutions.

Introduction Quality in medical care must be measured in order to be improved. Trauma management is part of health care, and by definition, it must be checked constantly. The only way to measure quality and outcomes is to systematically accrue data and analyze them. Material and methods A systematic revision of the literature about quality indicators in trauma associated to an international consensus conference Results An internationally approved base core set of 82 trauma quality indicators was obtained: Indicators were divided into 6 fields: prevention, structure, process, outcome, post-traumatic management, and society integrational effects. Conclusion Present trauma quality indicator core set represents the result of an international effort aiming to provide a useful tool in quality evaluation and improvement. Further improvement may only be possible through international trauma registry development. This will allow for huge international data accrual permitting to evaluate results and compare outcomes.

Hepatitis B virus has infected a third of the world’s population, and 296 million people are living with chronic infection. Chronic infection leads to progressive liver disease, including hepatocellular carcinoma and liver failure, and there remains no reliable curative therapy. These gaps in our understanding are due, in large part, to a paucity of animal models of HBV infection. Here, we show that rhesus macaques regularly clear acute HBV infection, similar to adult humans, but can develop long-term infection if immunosuppressed. Similar to patients, we longitudinally detected HBV DNA, HBV surface antigen, and HBV e antigen in the serum of experimentally infected animals. In addition, we discovered hallmarks of HBV infection in the liver, including RNA transcription, HBV core and HBV surface antigen translation, and covalently closed circular DNA biogenesis. This pre-clinical animal model will serve to accelerate emerging HBV curative therapies into the clinic.

Obesity induces multifactorial effects such as dyslipidemia, insulin resistance, and arterial hypertension that influence the progression of many diseases. Obesity is associated with an increased incidence of cancers, and multiple mechanisms link obesity with cancer initiation and progression. Macrophages participate in the homeostasis of adipose tissue and play an important role in cancer. Adipose tissue expansion in obesity alters the balance between pro- and anti-inflammatory macrophages, which is a primary cause of inflammation. Chronic low-grade inflammation driven by macrophages is also an important characteristic of cancer. Adipocytes secrete various adipokines, including adiponectin, leptin, IL6, and TNFα, that influence macrophage behavior and tumor progression. Furthermore, other metabolic effects of obesity, such as hyperlipidemia, hyperglycemia, and hypercholesterolemia, can also regulate macrophage functionality in cancer. This review summarizes how obesity influences macrophage–tumor cell interactions and the role of macrophages in the response to anticancer therapies under obese conditions.

The purpose of this paper is to provide a common framework for studying various generalizations of Leavitt algebras and Leavitt path algebras. This paper consists of two parts. In part I we define Cohn-Leavitt path algebras of a new class of graphs with an additional structure called bi-separated graphs, which generalize the constructions of Leavitt path algebras of various types of graphs. We define and study the category \textbf{BSG} of bi-separated graphs with appropriate morphisms so that the functor which associates a bi-separated graph to its Cohn-Leavitt path algebra is continuous. We also characterize a full subcategory of \textbf{BSG} whose objects are direct limits of finite complete subobjects. We compute normal forms of these algebras and apply them to study some algebraic theoretic properties in terms of bi-separated graph-theoretic properties. In part II we specialize our attention to Cohn-Leavitt path algebras of a special class of bi-separated graphs called B-hypergraphs. We investigate their non-stable K-theory and show that the lattice of order-ideals of V-monoids of these algebras is determined by bi-separated graph-theoretic data. Using this information we study representations of Leavitt path algebras of regular hypergraphs and also find a matrix criterion for Leavitt path algebras of finite hypergraphs to have IBN property.

38. Influence of Modified Atmosphere Packaging on the Shelf Life and Postharvest Quality Attributes of King Chili (Capsicum chinense Jacq.) during Storage

Shelf life enhancement and quality attributes changes under passive modified atmospheric packaging (PMAP) during storage of king chili (Capsicum chinense Jacq.) was investigated. The selection and design of packaging was done based on the respiration rate (RR) of king chili and permeability of the packaging film. The king chili were packed in selected unperforated polypropylene (PP) with thickness 35 μm and stored in refrigerator (8 ± 1 °C and 95%RH) and ambient (25–30 °C, 65–75% RH) conditions. King chili stored without packaging and kept in ambient condition at same temperature and RH served as control sample. A passive modified atmosphere of 8.68% O2, 4.02% CO2 and 9.16% O2, 5.12% CO2 were achieved in the PMAP (8 °C) and PMAP ambient storage, respectively. The quality attributes, viz. weight loss, firmness, and ascorbic acid content, were retained in PMAP as compared to the unpacked samples stored at refrigerated and ambient conditions. The color value (L, a, b) changes during storage had significant (p ≤ 0.05) effect on their storage temperature and time. The initial capsaicin content of king chili was observed 2.47% which moderately changes during storage as compared to the unpacked samples. The highest percentage of marketability and a very low percentage of decay were observed in PMAP at 8 °C as compared to all other treatments. Finally, shelf life of king chili was prolonged up to 30 days in PMAP at 8 °C and 25 days in PMAP ambient as compared to that of 20 days and 10 days of the unpacked refrigerator and unpacked ambient storage, respectively. Also, quality attributes were maintained in PMAP at 8 °C during storage, therefore increased marketability of king chili.

39. Optimization Of Concentration Of Clove (Syzygium Aromaticum) Extract On Reduction Of Histamine Content During Fermentation Of Idli Batter

Fermented foods are rich in nutritional profile and idli batter has its attention as it is a short term stored fermented food and now considered globally. The fermented batter may contain histamine which are produced during fermentation. The clove extract was known for its antimicrobial activity along with lots of medicinal value. Hence an attempt has been made to optimize the concentration of clove extract for addition in idli batter during fermentation to reduce the production of histamine. The effect of clove on the physiochemical parameters such as pH, color, titratable acidity was analyzed during storage period (0,2,4,6 hours) and it was found that the physiochemical parameters were not affected during the storage hours. The titratable acidity of the idli batter and clove added idli batter was found to be in the range of 0.16-0.18 and 0.10 - 0.28 % during the storage period. The Microbial load were also observed by analyzing the total plate count during storage hours and it reduced the microbial count from 9.05 CFU/g to 8.60 CFU/g during the storage period of 6 h in the clove extract added idli batter. Histamine producers were isolated using Nivens agar media. The histamine content in the clove extract added idli batter was found to be low at 6th hr (0.06 mg/kg.) The GC-MS analysis on phytocompounds in clove revealed that eugenol is a major compound present in clove which may be responsible for antibacterial activity and reduce the histamine producing organism. The histamine content in the control and clove extract idli batter was analyzed using modified colorimetric method and revealed that the addition of clove extract reduced the histamine content during storage period that can help in increased shelf life of idli batter.

- Abstract
The composite material comprises either two or more constituents having dissimilar physical and chemical properties. The composite is prepared by various techniques, but the stir casting method has been widely used, as it is simple and cost-effective. In the present work, the composites from ADC12 Aluminum Alloy-Boron Carbide (B4C) (wt.5%) and varied wt % of Rice Husk Ash (RHA) were developed with the help of the stir cast technique. Composite having 3, 6, 9 wt.% of RHA were considered for the wear analysis. The wear analysis of hybrid composites was studied with the help of Taguchi method and also optimum values were determined. The tribological study was conducted on the Pin on disk testing apparatus under dry sliding conditions. The L27 orthogonal array has been preferred in the present work to include three factors and three levels. The selected factors are Speed (N), Load (W), RHA, and wear depth as system output (yield). Analysis of variance has been carried out to know the parameters' influence and level of contribution to the wear loss. For, the validation of the analysis results the experimental test was carried out for the optimum values. To understand the wear mechanism in composites the samples were analyzed using Scanning Electron Microscopic (SEM) and it has been observed that both abrasive, as well as adhesive wear, did occur on the contact surface of the specimens.

The hypoeutectic aluminum alloy (AlSi10Mg) is a well-known candidate material used predominantly for its processability and inherent characteristics in metal-based additive manufacturing. Besides, transition metal carbide, such as niobium carbide (NbC), is added to the AlSi10Mg, enhancing its mechanical properties and preferably its wear resistance to the matrix. However, in additive manufacturing, the mixed powder’s flowability is a prerequisite for determining the final properties of the specimens. In this study, mixed powder flowability analysis was carried out through the regular mixing of AlSi10Mg with a varying weight percentage of NbC conducted in the planetary ball mill with different time, speed, and NbC composition following the Central Composite Design (CCD), with a total of 20 experiments. Here, regular mixing was preferred to retain the morphology of the AlSi10Mg (spherical shape) instead of ball milling, which contributes to the degradation of the powder’s shape and size. Finally, based on the combined analysis of apparent density (AD), tapped density (TD), and static angle of repose (SAoR), the flowability characteristics of the mixed powders (AlSi10Mg + X % NbC) were evaluated. The optimum combination (AlSi10Mg + X% NbC) was attained based on the composite desirability criteria.

Powder-based additive manufacturing (PAM) is a potential fabrication approach in advancing state-of-the-art research to produce intricate components with high precision and accuracy in near-net form. In PAM, the raw materials are used in powder form, deposited on the surface layer by layer, and fused to produce the final product. PAM composite fabrication for biomedical implants, aircraft structure panels, and automotive brake rotary components is gaining popularity. In PAM composite fabrication, the aluminium cast alloy is widely preferred as a metal matrix for its unique properties, and different reinforcements are employed in the form of oxides, carbides, and nitrides. However, for enhancing the mechanical properties, the carbide form is predominantly considered. This comprehensive study focuses on contemporary research and reveals the effect of metal carbide’s (MCs) addition to the aluminium matrix processed through various PAM processes, challenges involved, and potential scopes to advance the research.

IntroductionVentriculoperitoneal (VP) shunt is a commonly performed neurosurgical procedure in pediatric surgical practice. It results in a dramatic improvement in patient survival and neurological function but is associated with several complications requiring multiple shunt revisions throughout a patient’s lifetime. The main aimof the study was to evaluate the outcome especially complications of shunt surgery at our center.MethodsA single-institutional, retrospective observational study was conducted in 60 operated cases of congenital hydrocephalus with a minimum follow-up for up to 6 months. Parameters for analysis included demographic data, etiology of hydrocephalus, surgery time, and type of complications.ResultsOut of 86 patients, 60 patients who fulfilled the inclusion criteria were enrolled in the study. The mean age of the patients at the time of VP shunt placement was 3 months, range from 8 days to 15 years with male preponderance. The majority of the patients belonged to the 0-1-year age group (76.8%). Thecommonest etiology of hydrocephalus was congenital aqueduct stenosis. Fifteen patients (25%) developed complications within six months follow-up period. Eight patients (13.3%) developed clinical shunt infection with four cases having culture positive. Four (6.67%) had mechanical complications in the form of shuntobstruction. Three patients had ascites, subdural collection, and shunt extrusion from the anus. The shunt related mortality was 1.67%.ConclusionThe shunt infection and obstruction still remain the most important complications. Despite inherent problems, shunt surgery is the procedure of choice until a safer effective alternative is available.Keywords: Complications congenital hydrocephalus, ventriculoperitoneal shunt

IntroductionSpine pathology involves a wide spectrum of diseases and needs a multidisciplinary approach including surgery, rehabilitation and psychological support. It increases the burden to the family and society. This study describes spine and spinal cord disease from a neurosurgical unit from a tertiary hospital in Nepal.MethodsThis is a retrospective study which meticulously reviewed all spinal cases admitted between April 2019 to February 2019, in the Department of Neurosurgery, Tribhuvan University Teaching Hospital, Kathmandu, Nepal. All adult patients of ≥16 years, diagnosed with various spinal diseases were included. Based on thespectra of causative pathologies, the study population was broadly categorized into trauma, degenerative, tumor, vascular and infection. Demographics, other variables, and outcome (in 1 month) were assessed.ResultsA total of 76 patients included in this study. Male preponderance was observed in all categories. Twentyseven patients (35.5%) had traumatic spine injury and fall from height was the most common mode of injury, accounting ˷two-third of the total spinal trauma patients. Cervical segment was involved in twothirdpatients. More than one-third of the spinal patients (34%) had a degenerative disorder involving lumbar and cervical regions, 57.7% and 42.3%, respectively. Spinal infection was diagnosed in 10.5% of the patients with 62.5% diagnosed as Pott’s spine. Overall complications were seen in 18.4%. Pain improved inall patients while overall good outcome with improvement was noted in 63.2% of the patients.ConclusionOur study demonstrated a large patient burden and a clinical profile dominated by preventable causes such as RTA and fall injury. With early treatment and rehabilitation, significant improvement is seen. Further large scale multicenter studies are required to generalize the whole population of Nepal.Keywords: Burden, Nepal, spine trauma, spine pathology
