3. Clinical presentations among adult patients with tuberculous meningitis

Background: Bangladesh is a country with a high burden of pulmonary as well as extrapulmonary tuberculosis. TBM is a particularly important public health issue in our country. Identifying prognostic predictors would improve the quality of management and ultimately will prevent long-term mortality and morbidity. The aim of the study was to observe and identify the most common clinical presentations among patients with tuberculous meningitis. Methods: This hospital-based prospective cohort study was carried out at the in-patient Department of Clinical Neurology at the National Institute of Neurosciences and Hospital (NINS and H), Dhaka, Bangladesh from October 2016 to September 2017, 1 year. A total of 54 patients admitted to the hospital with features of tuberculous meningitis (TBM) were selected for the study following inclusion and exclusion criteria. Results: Over 70% of patients were adolescents or young adults with a mean age of 28.2±12.3 years. The majority (48.2%) of the participants were from TBM stage II, while 37% were from stage III of TBM. Fever, headache, and systematic symptoms were the most frequent clinical presentations. The median delay in treatment was 30 days. Interruption in anti-TB treatment was reported in 3 (9.3%) cases. Conclusions: The study found that among possible TBM cases, the most frequent clinical features were fever, headache, and systematic symptoms. Focal neurological deficit and cerebral nerve palsy also had a high prevalence among the participants. The majority of the participants were young adults and female prevalence was high in the present study. The majority were from stage II TB among the study participants.

4. Evaluation of Yard-long Bean Genotypes at Varied Sowing Dates in the Sylhet Region, Bangladesh

This experiment was conducted in the experimental field of Department of Horticulture, Sylhet Agricultural University (SAU), Bangladesh during 2022. The objective was to evaluate three yard long bean genotypes (BARI yard long bean-1, BARI yard long bean-2 and Kagornatki) in three planting dates (20 March, 20 April and 20 May) under acidic soil conditions of Sylhet. The two factors field experiment was laid out in Randomized Complete Block Design (RCBD) with three replications. Yard long bean planted on 20 March recorded the minimum number of days to flower (40.33), the minimum number of days to first harvest (53.33), the maximum number of pods per plant (31.01), the highest individual pod weight (12.25 g), pod length (45.98 cm), per plant pod yield (389.47 g) as well as per decimal pod yield (97.34 kg). The genotype BARI yard long bean-1 was the earliest in flowering (41.66 days). BARI yard long bean-1 had the highest number of pods (27.81) as well as pod yield (309.85 kg) per plant. Although individual pod weight and pod length of BARI yard long bean-1 and Kagornatki were statistically similar, the per decimal pod yield (76.86 kg) of BARI yard long bean-1 was significantly higher than Kagornatki (56.31 kg). Interaction effect showed that the genotype BARI yard long bean-1 planted on 20 March showed the best performance in terms of pod yield per plant (506.56 g) and per decimal (127.22 kg). Therefore, the genotype BARI yard long bean-1 can be recommended to cultivate at 20 March sowing date under acidic soil condition of Sylhet.

5. Efficacy of Some Integrated Pest Management (IPM) Techniques against Okra Shoot and Fruit Borer (Earias vittella Fab.)

The experiment was laid out at the central field of Sher-e-Bangla Agricultural University, Dhaka-1207, Bangladesh during February 2022 to June 2022 to determine the efficacy of some integrated pest management tools against okra shoot and fruit borer (Earias vittella Fab.) in okra. The study comprised of seven treatments, viz. T1= Barrier crop (maize), T2= T-Bird perches 2m long@ 3/plot at distinct distances, T3= Pheromone trap@ 15/ha with lures, T4= Hand picking of larvae at regular basis, T5= Bacillus thuringiesis through two applications of Bt toxin on vegetative and flowering stage@ 1g/liter of water, T6= Proclaim 5 SG (Emamectin Benzoate) @ 1g/liter of water and NLE (neem leaf extract) @ 3g/liter of water, T7= untreated control. All the sprayings were done at 7 days interval. The experiment was laid out in Randomized Complete Block Design (RCBD) with three replications. All the treatments were applied in okra at different developmental stages against shoot and fruit borer. Among the treatments, T6 gave maximum number of shoots (15.07) with less infested shoots (1.95) and infestation percentage was minimum (12.93). At the same time, the highest shoot infestation percentage (63.40) was also recorded from T7 treatment. The minimum fruit infestation (8.77) was also obtained from T6 treatment with minimum fruit infestation percentage (24.89), whereas; again, T7 illustrated maximum infestation percentage (44.36) of fruit. The treatment T6 yielded maximum (14.78 t/ha) okra that is very close to T5 (13.72 t/ha) and T4 (13.50 t/ha). So, the treatment T6 showed the best performance compared to rest other treatments. But considering the environmental and human health issues, farmers can use the T5 and T4 as they don’t involve any risk factor compared to chemical pesticides as well as their yield is approximately same as to T6 treatment.

6. Risks Factor after Surgical Site Infection with and without Mesh Incisional Hernia

Background: Systemic problems, mesh infections, and hernia recurrence have all been related to wound infections that appear after a hernia repair. Objective: To assess the risks factor after surgical site infection with and without mesh incisional hernia. Methodology: The study was a Prospective interventional study which was conducted in Department of Colorectal Surgery Bangabandhu Sheikh Mujib Medical University from April, 2019- September, 2020 using a semi-structured questionnaire through face to face interview. Data were analysed using a computer programme SPSS 24.0 version. Result: The mean age of no mesh group was 55.83±8.42 years while the mesh group was 53.29±9.81 years. A majority of male patients in both, no mesh (70.8%) and mesh (66.7%), groups. The mean BMI of no mesh group was 25.13±5.49 while the mesh group was 26.98±4.46. 3 patients at 4th week and 1 patient at 3rd month had infection in no mesh group. And, 7 patients at 4th week, 2 at 3rd month and 1 patient at 6th month had infection in the mesh group. There was no difference statistically between no mesh and mesh groups (p=0.094, p=0.472 and p=0.280 at 4th week, 3rd month and 6th month, respectively. Conclusion: Obesity, tobacco use, and diabetes mellitus are the three main patient comorbidities that can be modified and are substantially linked to postoperative surgical site infection in hernia surgery. Weight reduction, quitting smoking, and diabetes management are all part of preoperative optimization.

7. Effect of bank credit on agricultural gross domestic product

Purpose. This study aimed to provide the central bank of Bangladesh with some empirical data on the impact of bank agricultural credit on agricultural output as a priority sector lending. Methodology / approach. Econometric analysis is applied to discover the nature of the relationship between banks’ agricultural credit and agricultural gross domestic product (GDP) and to find the impact of bank agricultural credit on agricultural output in Bangladesh. Data are extracted from secondary sources, i.e., Bangladesh Bank, Bangladesh Bureau of Statistics, World Bank, and Ministry of Finance for 1991 to 2018. Time-series data are analyzed using the Augmented Dickey–Fuller (ADF) unit root test and the Johansen cointegration test and later examined with Vector Error Correction Model (VECM). Results. The Augmented Dickey–Fuller unit root test confirmed that the variables were non-stationary at their level data and became stationary after taking their first difference value. Since the variables are integrated at I(1), we performed the Johansen Co-integration test. Subsequently, one co-integration equation was found, and Vector Error Correction Model estimation was done afterwards. According to the results of VECM estimation, bank agricultural credit, pesticide consumption, and use of cropped areas have long-run relation with agricultural output. Originality / scientific novelty. This study reveals the nature of relationship and assessment of the impact of bank credit on agricultural GDP with empirically valid techniques and tools. In this context, the data from Bangladesh remains very rare. Moreover, for those works which have been done in the context of Bangladesh, appropriate methods and techniques are not always inherent. Hence, this significant research gap is addressed in our present study. Thus, we expect that the findings of this study from Bangladesh based on empirically valid standard techniques can incrementally contribute to the existing literature. Practical value / implications. Findings of this research, can be used as an information basis by the central bank of Bangladesh. Based on the findings of this research, Bangladesh Bank should initiate new policies and programs regarding agricultural credit for the projected increase in agricultural GDP in Bangladesh.

9. Effects of feeds on selected shellfish (Macrobrachium rosenbergii) and finfishes (Planiliza persia and Rhinomugil corsula) in polyculture system: profitability and viability

The experiment was conducted at earthen ponds in the Bagerhat sadar upazila of Bagerhat to examine the growth, production capacity, and economic return of freshwater prawn (Macrobrachium rosenbergii) with persa (Planiliza persia) and corsula mullet (Rhinomugil corsula) under polyculture system. Four treatments, designated as T1, T2, T3, and T4 were used in the study, each with three replicates. The final weight of the prawns after 120 days of culture was highest in T1 (72.07 g) and lowest in T4 (50.23 g), but there was no significant difference in prawn growth between T1 and T2. In T4, where artificial feed was not employed, prawn, persa, and corsula mullet growth and survival rates were lower. Purpose: To observe the blood picture of patients with COVID 19 infections and to formulate future guidelines for management. Methods: This cross-sectional descriptive study was carried out among patients with COVID 19 infections who were admitted to one of the Covid-dedicated hospitals in Bangladesh from March-2020 to May-2020. Results: A total of 92 patients were included in this study. 53.3% were male and 46.7% were female. Most of the patient’s haemoglobin percentage was found below normal (53.3%), however, the total count of RBC, WBC and Platelet was found within the normal limits in most of the cases (64.1%,80.4% and 87% cases respectively). 75% of patients had high ESR levels. 50% of cases showed Neutrophilia and Lymphocytopenia in 43.5% cases. Although some cases show changes in Monocyte, Eosinophil and Basophil count they were not significant in number. Conclusions: A significant number of patient’s neutrophilia and lymphocytopenia. Blood parameter is an important tool for managing the COVID-19 patients. With or without co-morbidities maintaining normal blood levels is essential in COVID-19 patient management.

12. Effect of Cow Urine on the Biochemical and Microbial Properties of Cow Dung Derived Biogas Slurry

Biogas slurry (BGS) is an anaerobic digested organic material that can be used as an organic fertiliser. As cow urine (CU) is rich in plant nutrients, it may be used as diluting agent in biogas production to enhance the fertiliser quality of BGS. To explore the potency of CU on the fertiliser quality of BGS, four experimental trials were constructed by mixing cow dung (CD) and CU in varying proportion designated as T0 (50% CD + 50% Water) as control, T1 (50% CD + 50% CU), T2 (40% CD + 60% CU), T3 (30% CD + 70% CU) for biogas production. The quality of BGS was evaluated by studying its biochemical and microbial properties. The enzymatic activities revealed that all the CU amended samples showed better activities than control and were increased with the increase in CU. Compared to the control, the increase in urease, protease and phosphatase activities were 11.6% to 64.6%, 4.6% to 29.6% and 22.1% to 50.0%, respectively while cellulase activities were decreased from 25.9% to 3.1%. Most of the bacterial populations also increased in CU amended samples; total bacteria (TB) 20% to 60%, phosphate solubilizing bacteria (PSB) 33% to 67% and nitrogen fixing bacteria (NFB) 0% to 33%. Phytohormone, indole acetic acid (IAA) content and glycemic index (GI) were also increased with increase in CU (IAA- 23.5% to 59.5% and GI- 6.2% to 100.5%). With respect to all parameters analysed, CU amended samples can be considered superior to the control one except for their cellulase activities. Thus, utilization of cow urine improves the quality of BGS as organic fertiliser. This finding will help in reducing environmental pollution by utilizing hazardous cow urine as well as improving fertiliser quality of biogas slurry for agronomic use.

13. Ferulic acid improves glucose homeostasis by modulation of key diabetogenic activities and restoration of pancreatic architecture in diabetic rats

There are increasing concerns on the rising cases of diabetes mellitus with type 2 diabetes (T2D) being of major interest as well as the cost of its treatment. Plant phenolic compounds are natural and potent antioxidants that have been widely reported for their antidiabetic activities properties, one of which is ferulic acid. The effect of ferulic acid (FA) on major diabetogenic activities and pancreatic architecture linked to T2D was investigated in T2D rats. T2D was induced in male Sprague-Dawley rats using the fructose-streptozotocin model. Diabetic rats were treated with FA at 150 or 300 mg/kg bodyweight (bw). Normal control consisted of rats administered with food and water, while diabetic control consisted of untreated diabetic rats. Metformin was used as the standard drug. The rats were humanely sacrificed after 5 weeks of treatment. Their blood, liver, and pancreas were collected for analysis. Total glycogen content and carbohydrate metabolic enzymes activities were analyzed in the liver, while the pancreas and serum from blood were analyzed for oxidative stress biomarkers, purinergic and cholinergic enzyme activities, and amylase and lipase activities. The pancreatic tissue was further subjected to microscopic and histological examinations. FA caused a significant (p 0.05) decrease in blood glucose level, with concomitant increase in serum insulin level. Treatment with FA also led to elevated levels of GSH, HDL-c, SOD, and catalase activities, while concomitantly suppressing malondialdehyde, cholesterol, triglyceride, LDL-c, NO, ALT, AST, creatinine, urea, and uric acid levels, acetylcholinesterase, ATPase, ENTPDase, 5'-nucleotidase, lipase, glycogen phosphorylase, glucose-6-phosphatase, and fructose-1,6-biphosphatase activities. Histology analysis revealed an intact pancreatic morphology in FA-treated diabetic rats. While transmission electron microscopy (TEM) analysis revealed an intact pancreatic ultrastructure and increased number of insulin granules in β-cells. Taken together, these results portray that the antidiabetic potentials of ferulic acid involves modulation of major diabetogenic activities and maintenance of the pancreatic ultrastructure architecture.

Introduction: The lipid profile of a Bangladesh population, which has a lower prevalence of coronary heart disease, compared with others nations, is studied to determine whether lipid-modifying disease prevention programs are necessary. Aim of the Study: The aim of the study was find out the Lipid Profile and Its Association with Risk Factors for Coronary Heart Disease. Methods: This is a cross-sectional study; a total of 172 patients were enrolled and analyzed. The study conducted during January 2020 to December 2020 at the Department of Cardiology in Department of Cardiology, Shaheed M. Monsur Ali Medical College, Sirajganj, Bangladesh. Result: The study population is shown in more than 30% of patients had hypercholesterolemia, 41(23.84%) patients were at the high borderline (5.17-6.19), and 38(22.09%) patients had high-density lipoprotein. Under low-density lipoprotein, 7(4.07%) patients had high LDL-C, and only 2(1.16%) patients had very high LDL-C. The age distribution of the study; 33.72% of patients were from the age group 40-49 years, and 17.44% of patients were aged more than sixty years. Conclusion: WHR is associated with an increase in TG, TC, and LDL-C and supports the view that abdominal obesity may be an important cardiovascular risk factor. This study emphasizes the role of HDL-C, which may avert the CHD risk in females. Further studies with longitudinal data are needed to assess the risk factors for CHD in high-altitude populations.

17. Effect of Genotypes and De-Topping on Growth and Yield of Okra

A field experiment was conducted at the experimental field of the Department of Horticulture in Sylhet Agricultural University to evaluate the growth and yield performance of three okra genotypes (viz. BARI Dherosh-1, BARI Dherosh-2 & RAJ F1) under special management practices (viz. de-topping & non de-topping) from March to July 2022. The two factor experiment was laid out in a Randomized Complete Block Design (RCBD) with three replications. BARI Dherosh-1 showed vigorous growth nature having maximum yield and yield contributing characters viz. the highest plant height (20.46 cm) at 30 DAS (Days After Sowing) and at 1st harvest (108.23 cm ), early flowering (51.33 days), the maximum number of leaves per plant (4.71), number of branches (2.96), number of pods per plant (43.80), the highest individual pod weight (20.96 g), pod yield per plant (916.75 g) as well as per hectare yield (30.53 t) than the other two genotypes. Contrary, the lowest individual pod weight (18.80 g), pod yield per plant (638.72 g), and per hectare yield (21.28 t) were produced by genotype RAJ F1. Due to removing apical parts at 30 DAS, the shortest plants were observed in the de-topping system (85.69 cm). Again, Individual pod weight (20.87 g), pod yield per plant (887.95 g), and per hectare yield (29.57 t) were higher in non de-topping system than the de-topping system. Considering the interaction effect, BARI Dherosh-2 performed better under non de-topping system having the highest pod breadth (2.05 cm) and pod yield per plant (1007.86 g) and per hectare yield (33.56 t). Actually, de-topping at 30 DAS had no beneficial effect on yield or yield contributing characters. So, BARI Dherosh-2 might be recommended for the Sylhet region to ensure maximum benefit without de-topping.

18. Examining the interdisciplinary approach for treatment of persistent post-concussion symptoms in adults: a systematic review

Background: The objective of this review is to examine the evidence for the interdisciplinary approach in treatment of persistent post-concussion symptoms in adults. Methods: This systematic literature search was undertaken according to the Preferred Reporting Items for Systematic and Meta-Analysis (PRISMA) guidelines. Five electronic databases were searched: CINAHL, Informit, ProQuest, PubMed and Scopus. After screening and quality assessment, the review included six studies published in English and peer-reviewed journals, between 2011 and 2021 to return contemporary evidence. Results: The results revealed that there was significant variation between measures used and the timing of the pre- and post-treatment assessment. The studies found an interdisciplinary approach to be beneficial, however, the challenges of inherent heterogeneity, lack of clarity for definitions and diagnosis, and mixed results were apparent. The interdisciplinary interventions applied in all identified studies were found to reduce post-concussion symptoms across the symptom subtypes: headache/migraine, vestibular, cognitive, ocular motor and anxiety/mood. Conclusions: The results demonstrated evidence for a reduction in persistent post-concussion symptoms following interdisciplinary intervention. This evidence will inform health services, clinicians, sports administrators and researchers with regard to concussion clinic and rehabilitation team design and service delivery.

19. Application of ARIMA Model in Forecasting Exchange Rate: Evidence from Bangladesh

This paper attempts to apply the ARIMA time series model to forecast the exchange rate of seven currencies (United States Dollar, Euro, Pound sterling, Australian Dollar, Japanese Yen, Canadian Dollar and Swedish Krona) in terms of Bangladeshi Taka (BDT) and to investigate the accuracy of the model by comparing the forecasted rates with the actual exchange rates. It considered daily currency exchange rates (244 selling price) of seven currencies for twelve months from January 2018 to December 2018 to forecast the subsequent one month (25 selling rate) in January 2019 rate. The Durbin-Watson test result shows an autocorrelation in the daily foreign currency exchange rate with the previous rate. The Augmented Dickey-Fuller test result shows data have unit roots and non-stationary. But the 1st differencing becomes data stationary to apply d equal to 1 in ARIMA model. Also, autocorrelation function considers MA(0) and partial autocorrelation function considers AR(1) for the ARIMA model. So, ARIMA (1,1,0) models are selected based on Ljung-Box test, root mean square error, mean absolute percent error, mean absolute error and R-square values. By using the above ARIMA models, forecasted foreign currency exchange rates next one month calculated and compared with the respective actual rates, which validate with Chi-Square test, mean absolute percent error, mean square error, root mean square error values of Goodness fit test. The result shows that predicted foreign currency exchange rates follow ARIMA (1,1,0) model, which may be applied to forecast the foreign currency exchange rates in Bangladesh.

This experiment was conducted during October, 2013 to March, 2014 in the experimental field of Department of Horticulture, Sylhet Agricultural University (SAU), Bangladesh. The objective was to evaluate the effect of three planting dates (30 October, 15 November and 1 December) on growth, yield and yield attributes of three broccoli varieties (Premium, BR001 and BR002) under acidic soil condition of Sylhet. The two factors field experiment was laid out in Randomized Complete Block Design (RCBD) with three replications. Broccoli planted on 30 October recorded the minimum number of days to first curd harvest (64.77), the tallest plants at harvest (62.76 cm) with denser leaves (17.90), the highest primary curd weight (142.75 g) with increased curd length (11.93 cm), the maximum curd yield of both primary (5.29 t/ha) and secondary (5.24 t/ha). Variety Premium was the earliest in curd initiation (50.33 days) and curd harvest (64.44 days). Premium had the highest number of leaves at harvest (16.07), the maximum primary curd weight (207.59 g) with increased curd length (12.18 cm) and curd diameter (14.28 cm). The highest primary curd yield (7.70 t/ha) and secondary curd yield (4.02 t/ha) of Premium variety out yielded the other two varieties in this experiment. Interaction effect showed that the variety Premium planted on 30 October showed the best performance in primary curd weight (274.13 g), primary curd yield (10.17 t/ha) and secondary curd yield (8.43 t/ha). Therefore, the variety Premium can be recommended to cultivate at 30 October planting date under acidic soil condition of Sylhet.

The urban raw food markets of the Rajshahi City play a pivotal role to supply foods to the urban people. These food markets are very essential places in the commercial areas, which are dispersedly distributed over the city. The markets are found not grown up according to a master plan of the city. From this point of view, this study focuses on the structural characteristics and spatial organisation of the raw food markets of the city. The study also attempts to discuss historical background of the markets and their service area. It is revealed that the structural characteristics of all markets of the city are not sufficiently developed and also their spatial distribution are not well organised.

Objectives : The Objectives of this study is to determine the outcome and effectiveness of Ultra-mini percutaneous nephrolithotomy (PCNL) for treating low-volume renal stone without placement of any stent and nephrostomy tube as a supplement to the conventional PCNL. Material and methods: The Patients who underwent ultra-mini PCNL bwtween July 2018 to December 2018. were studied.This was a prospective study of 14 patients.This study was carried out in urology centre of CMH Dhaka.Before the study ethical clearance was taken from hospital ethical committee. All these patients had their first-line treatment. The patients had calculus limited to either a single calyx or just extending to the pelvis and the stone size was less than 1.5 cm in its maximal dimension. The mean stone size was 10.8+_4.2(5-15mm). An 6 Fr operating nephroscope was used. The patients were placed in prone position. The stones were fragmented using Holmium-YAG laser. Various surgical outcomes including duration of the surgery, stone-free rate and any subsequent complications-if any-were analyzed.The stone free rate was assessed on the 1st day and at three month after surgery by X-ray KUB and ultrasonography. Results: The study includes a series of 14 patients (one patient treated with bilateral renal stone disease). The mean age of the patients was 39.07 years and body mass index was 25.5 kg/m2. Intrarenal stone location was as follows: lower calyx, n=7; middle calyx, n=3, upper calyx, n=1, and pelvis, n=3. Median operative time was 52.66 min (range: 40–65) and the stone-free rate was 93.3% at first day and 97% after three month follow up.. Only one patient had residual fragments and needed subsequent extracorporeal shock wave lithotripsy. Conclusion: Ultra-mini PCNL in prone position with a complete tubeless approach for renal stone disease is a safe method for treating low-volume stone disease. 26. Erythema Induratum of Bazin: A Rare Finding in an Old Woman with Subcutaneous Nodules in the Lower Limbs

Erythema induratum of Bazin (EIB) is a chronic nodular eruption that frequently occurs on lower legs of young or middle-aged women. The clinical manifestations of EIB revealed various dermatological disorders that can be easily leads to misdiagnosis. We report a case of a 50-year-old woman who initially presented with EIB. The histopathological specimen was collected from her medial aspect of let leg. The epidermis is unremarkable. The dermis shows fibro fatty tissue, skin adnexae and foci of granuloma with Langhans type of giant cells and some lymphocytic infiltration in the deep dermis which was suggestive of erythema induratum. Her lesions responded to antitubercular therapy (ATT). This case study may help extend the therapeutic regimens for cutaneous tuberculosis. CBMJ 2023 January: Vol. 12 No. 01 P: 121-123

27. Common Health Problems Among Agricultural Workers in A Selected Rural Area of Mymensingh, Bangladesh

This cross-sectional descriptive study was conducted in Churkhai village of Bhavokhali union during December 2018 to find out common health problems among 200 agricultural workers in a selected rural area of Mymensingh as a part of Residential Field Site Training by 3rd year MBBS students session 2016-17. 63 percent of population lives in rural area. Agricultural workers have illnesses common to general population. Moreover socioeconomic and work condition poses extra burden. The study was carried out on 200 agricultural workers who were selected purposively. Data were collected on a predesigned questionnaire by face to face interviewing agricultural workers. Data analysis was done by SPSS version 20.0. Age distribution of respondents ranged from 19 years to 80 years; mean age 46.53 years and standard deviation 14.891 years. Males were predominant (male: female ratio 506.06: 100. Majority of them (57.50%) were literate. Most of them (65.00%) belonged to middle class family. More than half (55.00%) of agricultural workers were suffering from illness. Respiratory illnesses 16.00% were predominant followed by skin disease 10.50%, gastrointestinal disease 6.50%, conjunctivitis 6.00%, arthritis 6.00%, hypertension 6.00%, diabetes mellitus 5.00%, anemia 4.50% and hearing loss 0.50%. In summer heat exhaustion (14.50%) was predominant followed by heat syncope (10.00%. In rainy season ARI (12.00%) was predominant followed by diarrheal diseases (10.50%). In winter mild hypothermia (27.00%) was predominant followed by ARI (4.50%). In recall of last 1 year the important events were crop loss 12.50%, damage to house 10.00%, and unemployment 2.50%. There was no case of anthrax and bird flu. Prevalence of occupational accidents was 5.00%. which includes fall from height 0.50%, heavy object fallen on body 1%, domestic fire 1%, electrocution in house 1%, electrocution by power pump 0.50%, cattle hitting by horn 0.50% and accidental insecticide poisoning 0.5%. Most of the injury occurred in the current week. Majority of the workers (66.50%) had partial tetanus toxoid immunization. Morbidity in this study was better than several studies conducted in Bangladesh and India. By knowing health problems we can give health care to agricultural workers which will prevent diseases and illnesses of them, prolong their healthy life, promote their health and efficiency. 28. Comparative Study Between Holmium Laser Versus Pneumatic Lithotripsy for the Treatment of Lower Ureteric Calculi

Objective: To compare the efficacy of Holmium laser and pneumatic lithotripsy for the treatment of lower ureteric calculi. Methods: The study included total of 218 patients divided into two groups of laser lithotripsy (LL) and pneumatic lithotripsy (PL). Study was conducted between October 2014 and September 2018. Inclusion criteria were patients with a lower ureteric single stone of size 0.8 to 1 cm and negative urine culture. An x-ray KUB and USG of KUB was mandatory. IVU also done when required. Procedures were done under spinal anesthesia. A 9.5 Fr. semi rigid ureteroscope was used for ureteroscopy in all cases. Holmium laser with 550 ìm fiber was employed in laser group and frequency was set between 20-30 Hz at energy of 0.5 to 1 Joule. Storz lithotripter was used in PL group. Postoperatively patients underwent radiography at 4th week of follow up to assess stone clearance. Results: The mean patient age in LL and PL group was 38.8±9.1(15-58) and 41.2±10.3(16-62) years, male to female ratio 1.75:1and 1.57:1 and stone size 8.94±0.98 and 8.94±0.91 mm respectively. Stone free rate at 4 weeks was 96.36 % in LL group as compared to 88.83 % in PL group(p=0.033). Stone migration up in pelvicalyceal system occurred in one (0.90%) patients of LL group while in five (4.63%) patients of PL group (p= 0.0929). DJ Stent was placed in 43(39.09%) patients in LL group whereas 64(59.25%) patients required it in PL group (p=0.0030). Complication rate was 18.18% (20) in LL group whereas it was 38.88% (42) in PL group (p=0.0038) Conclusion: Holmium laser lithotripsy is a superior technology compared to pneumatic lithotripsy in terms of rate of stone clearance and complications for lower ureteric stones. Objective: In standard PCNL usually placement of a double J stent and a nephrostomy tube is required.Several recent studies have reported the benefits of tubeless percutaneous nephrolithotomy (PCNL). Postoperatively,In standard PCNL patients have an indwelling ureteric stent placed, which is often associated with stent-related morbidity. We performed totally tubeless(without any stent or nephrostomy tube) PCNL.This study was conducted in the urology centre of Combined military Hospital,Dhaka, to evaluate the safety, effectiveness, and feasibility of totally tubeless PCNL and to compare with standard PCNL where both nephrostomy tube and double J stent were placed. Materials and Methods: From January 2018 to June 2019 , total 57 selected patients underwent standard or totally tubeless PCNL.In standard PCNL group, both D-J stent and nephrostomy tube were placed and Neither a nephrostomy tube nor a ureteral stent was placed in the totally tubeless PCNL group. We compared patient demographics and stone characteristics, operation time, length of hospital stay, analgesia requirements, stonefree rate, blood loss, change in creatinine, and postoperative complications between the standard and totally tubeless PCNL groups. Results: There were no significant differences in preoperative patient characteristics, postoperative complications, and the stone-free rate between the two groups, but the totally tubeless PCNL group showed a shorter hospital stay and a lesser analgesic requirement compared with the standard PCNL group. Blood loss and change in serum creatinine level were not significantly different between the two groups. Conclusions: Totally tubeless PCNL appears to be a safe and effective alternative for the management of renal stone in selected patients and is associated with a shorter length of hospital stay and less analgesic requirement. 30. The Effect of Dexmedetomidine on Attenuation of Per-operative Hemodynamic Changes During Laparoscopic Cholecystectomy

Background: Dexmedetomidine is a newer adjuvant anesthetic agent which is gaining its popularity day by day in our perspective. As an anaesthetic adjuvant dexmedetomidine has been shown to provide good perioperative haemodynamic stability during laparoscopic surgeries. But still it is to be judged its efficacy as adjuvant agent in our aspect. Objective: To observe the haemodynamic effects of intravenous dexmedetomidine during laparoscopic cholecystectomy. Methods: Eighty(80) patients (ASA grade l and ll) who underwent laparoscopic cholecystectomy surgery were randomly selected and were scrutinized according to eligibility criteria.The selected patients were divided into two groups on even and odd number basis. Even number Group A (n=40): received a bolus dose of I.V dexmedetomidine 1μg/kg over 10 minutes before induction of Anesthesia and then I.V infusion of 0.5 μg/kg/hr as a maintenance infusion and odd number Group B(n=40): received a bolus dose of same volume of normal saline before induction of Anesthesia and infusion was continued during surgery. General anesthesia was administered in all patients using fentanyl, thiopentone, succinylcholine, nitrous oxide in oxygen, halothane and muscle relaxation maintained with vecuronium bromide. A pre-tested, observation based, peer-reviewed data collection sheet was prepared before study. Results: Intra-operative mean Heart rate of Group B patients were more higher than those of Group A patients and showed statistically significant differences (P=0.003). Mean arterial pressure of Group B patients were far more higher than Group A patients in different events of peroperative period which is statistically significant (P=0.001). Mean diastolic blood pressure in Group B was significantly higher than that of Group A (P=0.0001). Conclusion: Dexmedetomidine is an effective drug that can be used as adjunctive infusion in general anesthesia in an intention to stable the hemodynamic profile in the peroperative period of different surgeries. Specially in laparoscopic cholecystectomy some patients with cardiac risk become vulnerable after CO2 gas in sufflation which can be properly managed using Dexmedetomidine. Expression of the transient receptor potential (TRP) channel genes and their isoforms in the alpha-cells and the beta-cells of the human islets of Langerhans has not been studied in detail. In this study, we have analyzed the RNA sequencing data obtained from purified human alpha-cells and beta-cells to identify the genes and their isoforms that are expressed differentially in these two cell types. We found that TRPC1, TRPC4, TRPC7, TRPM3, and TRPML1 were differentially expressed in these two cell types. TRPC1, TRPM3, and TRPML1 were expressed at a higher level in the beta-cells than in the alpha-cells. TRPC4 and TRPC7 were expressed at a higher level in the alpha-cells than in the beta-cells. The TRPC4-206 isoform was expressed at a 45-fold higher level in the alpha-cells compared to the beta-cells. Expression of TRPM3-202 was 200-fold and TRPM3-209 was 25-fold higher in the beta-cells than in the alpha-cells. Our study has demonstrated the relative abundance of expression of the TRP channel genes and their isoforms in the human alpha-cells and the beta-cells.

32. Ferulic acid mitigates diabetic cardiomyopathy via modulation of metabolic abnormalities in cardiac tissues of diabetic rats

Cardiovascular abnormalities have been reported as a major contributor of diabetic mortality. The protective effect of ferulic acid on diabetic cardiomyopathy in fructose-streptozotocin induced type 2 diabetes (T2D) rat model was elucidated in this study. Type 2 diabetic rats were treated by oral administration of low (150 mg/kg b.w) and high (300 mg/kg b.w) doses of ferulic acid. Metformin was used as the antidiabetic drug. Rats were humanely euthanized after 5 weeks of treatment, and their blood and hearts were collected. Induction of T2D depleted the levels of reduced glutathione, glycogen, and HDL-cholesterol and the activities of superoxide dismutase, catalase, ENTPDase, and 5'nucleotidase. It simultaneously triggered increase in the levels of malondialdehyde, total cholesterol, triglyceride, LDL-cholesterol, creatinine kinase-MB as well as activities of acetylcholinesterase, angiotensin converting enzyme (ACE), ATPase, glucose-6-phopsphatase, fructose-1,6-bisphophatase, glycogen phosphorylase, and lipase. T2D induction further revealed an obvious degeneration of cardiac muscle morphology. However, treatment with ferulic acid markedly reversed the levels and activities of these biomarkers with concomitant improvement in myocardium structural morphology, which had favorable comparison with the standard drug, metformin. Additionally, T2D induction led to the depletion of 40%, 75%, and 33% of fatty acids, fatty esters, and steroids, respectively, with concomitant generation of eicosenoic acid, gamolenic acid, and vitamin E. Ferulic acid treatment restored eicosanoic acid, 2-hydroxyethyl ester, with concomitant generation of 6-octadecenoic acid, (Z)-, cis-11-eicosenoic acid, tridecanedioic acid, octadecanoic acid, 2-hydroxyethyl ester, ethyl 3-hydroxytridecanoate, dipalmitin, cholesterol isocaproate, cholest-5-ene, 3-(1-oxobuthoxy)-, cholesta-3,5-diene. These results suggest the cardioprotective potential of ferulic acid against diabetic cardiomyopathy.

The overarching objective of this scoping review was to explore the breadth of health care literature in attempts to identify current strategies that hospitals adopt to improve patient bed flow, reduce access and exit block while optimizing patient care.PubMed, CINAHL, Embase, Proquest, and Cochrane electronic library databases supported literature search in March 2021. Scholarly articles that met the 3 eligibility criteria-access block causes, effects, and solutions-were considered. Joanna Briggs Institute Guidelines supported first- and second-level literary screening processes.The synthesis included 43 references. Most initiatives addressed access (n = 15), followed by care (n = 16) and then community (n = 9), with a further 3 articles providing commentary across all 3 domains (n = 3). Evidence supported Lean principles in both emergency department and inpatient sector. Lean principles addressing access included physician-led ED triage models, point-of-care testing, overcapacity protocols, mental health team collocation models, and fast-track services. Inpatient care Lean concepts validated gains in multidisciplinary rounds, appropriate allocation of allied health services with a 7-days-a-week model, staggering of elective surgeries, journey boards usage, transit lounges, and lateral transfers. Most literature addressing the backend was narrative in nature, theorized, and advocating for solutions and policy reform.This study addressed aims and identified current strategies that hospitals adopt to tackle access block while guaranteeing patient care. Government-supported research to map out evidence-based models of care that address exit block and demonstrate efficiencies is required to optimize access to care in the community.

34. Nutritional Assessment of Guava for Quality Jelly Production

- Abstract
Guava (Psidium guajava L.) is a Vitamin C (ascorbic acid) rich fruit. Its production is quite high in Bangladesh but post-harvest life is very short. Farmers will benefited by producing different guava products like juice, jelly etc. and selling them to market for year round. The current study has been carried out to observe the nutritional composition of fresh guava, guava juice, and jelly for nutritive jelly production. Shelf life and sensory evaluation of guava and guava jelly were also evaluated. Fresh green guava of four varieties (Swarupkathi Peyera, Kazi Peyera, BAU-2 Peyera and BAU-19 Peyera) were collected from the Germplasm Center, Patuakhali Science and Technology University (PSTU), Bangladesh. The shelf-life (mean value) of fresh Guava, Guava juice, and Jelly were about 11, 9, and 142 days in ambient condition; 24, 30, and 213 days in refrigerated condition; and 42, 142, and 325 days in freezing condition, respectively. Kazi Peyara confined the highest percentage of ascorbic acid (195.59 mg/100g FW), total sugar (10.65 % FW), calcium (17.45 mg/100g), and phosphorus (34.63 mg/100g). However, BAU-19 Peyara contains the highest percentage of magnesium (20.15 mg/100g) and BAU-2 Peyara contains the highest percentage of potassium (392.1 mg/100g). The mean ascorbic acid content in guava juice (29.23 mg/100ml) and jelly (17.65 mg/100gm) were significantly lower compared to the fresh guava (192.77 mg/100gm) and also the ascorbic acid loss during storage was significant. The average score of different sensory parameters for fresh guava and guava Jelly were within the standard range. From the results, it can be concluded that guava is a good source of nutrients and quality jelly could be prepared from it.

Because of modern day overpopulation, rapid industrial and lifestyle growth, and Khulna citys geographic location, natural resource consumption is rising while their availability is deteriorating. This paper has been designed with an attempt to inform the policymakers of Bangladesh regarding the potential of waste management and recycling as a renewable source of energy. Due to economic and environmental initiatives, metal, paper, and rubber waste recycling rates continue to rise.The total daily waste collection volume considering four areas of the city (Fulbarigate, Khalishpur, Sheikhpara, and Zero Point) was found to be9.91tons. Its daily output is made up of metal (7.1tons), paper (2.2tons), and rubber (0.59tons). The substantial amount of these components in Khulnas waste composition emphasizes the significance of frequent collection and disposal.The current daily market value of this waste has been estimated to be BDT 4,36,879 in total. Each year, the income is close to BDT 160million.

The physical education system collapse during the Covid-19 situation in Bangladesh and then online education was becoming popular to overcome the problem. The present study is conducted to measure the effect of online education in Covid-19 situation and the performance of the education system. The educational material, teaching facility and skill, online connection service, student acceptability, social acceptability and digital learning perception are identified as the main factor that influence online education, which are measured as (each five) close ended survey (five point Likert scale) questionnaire. The 400 survey response data are finalized by purposefully sampling in randomized block design from public and private university teachers of different division of Bangladesh and analyzed by descriptive analysis, Kolmogorov–Smirnov test, Shapiro –Wilk test, One-Sample Wilcoxon Signed Rank Test, Spearman's rho correlation, Mann–Whitney test, Kruskal–Wallis H test and ordinal logistic regression model by MS Excel 2016 and IBM SPSS Statistics 26 software. The result shows academic performance of online education, online educational material, online teaching facility and skill, online connection service, online student acceptability, online social acceptability and online digital learning perception are significantly higher than the median test value 3, which are significantly increasing during Covid-19 situation. It is observed that the quality of educational material, teaching facility and skill are significantly affect the academic performance of online education. But online connection service, student acceptability and social acceptability have no significant effect on the academic performance of online education. Also digital learning perception is significantly affect the academic performance of online education. It is observed from the study that the contribution of both male and female gender to the academic performance of online education are same, which are significantly higher perception from the median test value 3. The perception of private university is significantly higher than public university in the academic performance of online education, which are significantly higher perception from the median value. The perception of lower experience group has significantly higher perception on the academic performance of online education gradually. So, the experience of this study may be utilized in the planning and implementing online education management in Bangladesh.

Despite the increased risk of disasters in the Bangladesh, there is a dearth of research about women’s physical and mental health vulnerability following disasters. This study aimed to explore the physical and mental health status and associated risk factors of women in recurrent disaster-affected areas of Bangladesh. Participants (N = 408) were recruited using systematic random sampling from two purposively selected disaster-prone areas (Upazilas of Sagata and Sarankhola), using a structured questionnaire conducted in person. Physical and mental health status was assessed using the health Short Form 12 (SF-12) measure that is comprised of both physical health (PCS-12) and mental health (MCS-12) subscales. Background: Access to oral healthcare is not uniform globally, particularly in rural areas with limited resources, which limits the potential of automated diagnostics and advanced tele-dentistry applications. The use of digital caries detection and progression monitoring through photographic communication, is influenced by multiple variables that are difficult to standardize in such settings. The objective of this study was to develop a novel and cost-effective virtual computer vision AI system to predict dental cavitations from non-standardised photographs with reasonable clinical accuracy. Methods: A set of 1703 augmented images was obtained from 233 de-identified teeth specimens. Images were acquired using a consumer smartphone, without any standardised apparatus applied. The study utilised state-of-the-art ensemble modeling, test-time augmentation, and transfer learning processes. The “you only look once” algorithm (YOLO) derivatives, v5s, v5m, v5l, and v5x, were independently evaluated, and an ensemble of the best results was augmented, and transfer learned with ResNet50, ResNet101, VGG16, AlexNet, and DenseNet. The outcomes were evaluated using precision, recall, and mean average precision (mAP). Results: The YOLO model ensemble achieved a mean average precision (mAP) of 0.732, an accuracy of 0.789, and a recall of 0.701. When transferred to VGG16, the final model demonstrated a diagnostic accuracy of 86.96%, precision of 0.89, and recall of 0.88. This surpassed all other base methods of object detection from free-hand non-standardised smartphone photographs. Conclusion: A virtual computer vision AI system, blending a model ensemble, test-time augmentation, and transferred deep learning processes, was developed to predict dental cavitations from non-standardised photographs with reasonable clinical accuracy. This model can improve access to oral healthcare in rural areas with limited resources, and has the potential to aid in automated diagnostics and advanced tele-dentistry applications.

Background and aims: Interaction between a drug and another element which causes a drug interaction when it prevents the medication from functioning as intended. Usually, an interaction can change how the drug is absorbed, distributed, metabolized and eliminated by the body. The purpose of this study is to assess the type and strength of in-vitro complexation that may be caused by an interaction between Amoxicillin with Antacids and investigation of antimicrobial study of Amoxicillin. Methodology: This work ensures as a possible interaction between Amoxicillin with Mg(OH)₂ (Antacid-1) and CaCO₃ (Antacid-2) which was determined by Job’s plot approach. Investigating the number of Mg(OH)₂ (Antacid-1) and CaCO₃ (Antacid-2) connected in the complexation with Amoxicillin complexes was demonstrated by exploiting several spectrophotometric methods. Ultraviolet studies of these were carried out and balanced. The microbial sensitivity assessment is important to identify whether Amoxicillin’s efficacy has changed as a result of its interaction with antacids. Findings: The study supported that there is a probable interaction between Amoxicillin with antacids which is confirmed by job’s plot method. The Amoxicillin absorbance is verily different from absorbance of Amoxicillin –Mg(OH)₂ (Antacid-1) and Amoxicillin- CaCO₃ (Antacid-2). And the intensity of the peak of Amoxicillin changes particularly when Amoxicillin forms strong 1:1 complex with Mg(OH)₂ (Antacid-1) and CaCO₃ (Antacid-2). These curves were able to indicate strong kinetics of complexation of Amoxicillin with antacids. By Antimicrobial investigation against gram positive bacteria, it was certain that the zone of inhibition of Amoxicillin with Mg(OH)₂ (Antacid-1) and CaCO₃ (Antacid-2) reduced from 15 mm, 11 mm and 9 mm respectively against gram positive bacteria and Amoxicillin with Mg(OH)₂ and CaCO₃ did not exhibit any detectable antibacterial action when tested against gram negative bacteria. Conclusion: It has been demonstrated that the standard absorbance of Amoxicillin differs significantly from the absorbance of drug-antacid and the antimicrobial activity is also reduced due to interaction. The recovery approach involves providing a holistic and integrated service that is centered on and adapts to the aspirations and needs of consumers, who are seen as the expert on their health and well-being. Evidence is needed to address the current ambiguities related to the concept of recovery and its application.A scoping review was conducted to identify papers describing theories, models, and frameworks of recovery to delineate the central domains of recovery.Three literature search strategies were used: electronic database searching; hand-searching of key journals; and a reference list review of included papers. Inclusion criteria outlined theories, models and frameworks developed to support consumers' recovery and those supporting mental health professionals (MHPs) to deliver recovery-oriented services.Twelve studies (eleven articles and one book) were included in the review. The dimensions of recovery were synthesized into a framework named the Consolidated Framework for Recovery-oriented Services (CFRS). There are three domains within the framework: mechanisms/strategies; recovery as an internal process; and recovery as an external process. Each of these domains, as well as their relationships, are discussed.The CFRS can be used by practitioners, researchers, funders, and collaborative members to conceptualize, implement, and evaluate recovery-oriented services.

Since ancient times, plants have been used as green bioresources to ensure a healthier life by recovering from different diseases. Kattosh (Lasia spinosa L. Thwaites) is a local plant with various traditional uses, especially for arthritis, constipation and coughs. This research investigated the effect of Kattosh stem extract (LSES) on streptozotocin-induced damage to the pancreas, kidney, and liver using in vitro, in vivo and in silico methods. In vitro phytochemical, antioxidative and anti-inflammatory effects of LSES were accomplished by established methods followed by antidiabetic actions in in vivo randomized controlled intervention in STZ-induced animal models for four weeks. In an in silico study, LSES phytocompounds interacted with antidiabetic receptors of peroxisome proliferator-activated receptor-gamma (PPAR, PDB ID: 3G9E), AMP-activated protein kinase (AMPK, PDB ID: 4CFH) and α-amylase enzyme (PDB ID: 1PPI) to verify the in vivo results. In addition, LSES showed promising in vitro antioxidative and anti-inflammatory effects. In contrast, it showed a decrease in weekly blood glucose level, normalized lipid profile, ameliorated liver and cardiac markers, managed serum AST and ALT levels, and increased glucose tolerance ability in the animal model study. Restoration of pancreatic and kidney damage was reflected by improving histopathological images. In ligand-receptor interaction, ethyl α-d-glucopyranoside of Kattosh showed the highest affinity for the α-amylase enzyme, PPAR, and AMPK receptors. Results demonstrate that the affinity of Kattosh phytocompounds potentially attenuates pancreatic and kidney lesions and could be approached as an alternative antidiabetic source with further clarification.

Thirty (30) layer birds of 65 weeks of age were randomly divided into three groups (A, B and C) containing 10 birds in each group. Birds were subjected to treatments for a period of 1 month (30 days) as per fol lowing schedule: Group-A treated with 2gm spirulina/kg feed; Group-B: treated with 4gm spirulina/kg feed and Group-C: without any treatment. Piezoelectric materials have found their parts in numerous manufacturing applications such as transducers and sensors. Piezoelectric material shows anisotropy; in addition, its elastic field and electric field are integrated. The analysis of piezoelectric materials has obtained great interest among researchers with the development of smart structures. It is significant to explain the distribution of stress and electric displacement fields along the bonded edge. It has never been obvious how stress singularity and electric displacement fields are distributed at the vertex of piezoelectric dissimilar material joints. Stress and electric displacement distribution near the vertex along the interface of piezoelectric boned joints are investigated in this present research. Numerical analysis of piezoelectric dissimilar material joints is carried out by using Abaqus FEA software. From the numerical analysis, it is observed that stress, displacement, electric potential, electric displacement field evolvement along the interface edge rises with the increment of adhesive layer thickness and slant angle. So, a thin adhesive layer thickness and small slant angle are more reliable for operation.

We investigate whether firm-level political risk affects corporate cash holdings. Taking a sample of 5,424 US firms with 129,750 firm-quarter observations from 2002Q1 to 2021Q3, we find that cash holdings is higher for firms with greater exposure to firm-level political risk. The positive relationship between firm political risk and cash holdings is consistent for financial constraint and non-constraint firms, high and low growth firms, pro-cyclical and counter-cyclical and competitive industries. Further, our findings are consistent to alternative measures of firm-level political risk and cash holdings. In addition, our findings remain robust with different endogeneity tests: a natural experiment, an instrumental variable approach, and a propensity score matching. Overall, we present novel evidence on the determinants of corporate cash holdings.

RNA structure prediction with pseudoknots is an NP-complete problem, in which an optimal RNA structure with minimum energy is to be computed. In past decades, several methods have been developed to predict RNA structure with pseudoknots. Among them, metaheuristic approaches have proven to be beneficial for predicting long RNA structure in a very short time. In this paper, we have used two metaheuristic algorithms; Genetic Algorithm (GA) and Simulated Annealing (SA) for predicting RNA secondary structure with pseudoknots. We have also applied a combination of these two algorithms as GA-SA where GA is used for a global search and SA is used for a local search, and conversely SA-GA, where SA is used for a global search and GA is used for a local search. Four different energy models have been applied to calculate the energy of RNA structure. Five datasets, constructed from the RNA STRAND and Pseudobase++ database, have been used in the algorithms. The performances of the algorithms have been compared with several existing metaheuristic algorithms. Here we have obtained that the combination of GA and SA (GA-SA) gives better results than GA, SA and SA-GA algorithms and all other four state-of-art algorithms on all datasets.
