Background : This study attempted to produce a novel graft composed of bone marrow-derived mesenchymal cell (BMC) layer-gelatin sheets for bladder augmentation cystoplasty. Then, we determined if the grafts could reconstruct bladder tissues. Methods : BMCs harvested from the femurs of green fluorescence protein (GFP)-transfected Sprague-Dawley (SD) rats were adherent and proliferating cells on collagen dishes. The cells were then cultured on temperatureresponsive culture dishes. Following this, the BMCs maintaining cell-cell contacts within the monolayer itself were applied to a gelatin sheet. Two BMC layer-gelatin sheets were overlaid together with the cell sides juxtaposed with one another (bilayered BMC-gelatin graft). Bladder top of SD rats were incised and transplanted with the bilayered BMC-gelatin grafts. Similarly, urinary bladders irradiated with 2 Gy once a week for 5 weeks were also conducted. As control, bilayered acellular-gelatin grafts were used. At 4 weeks after transplantation, the bladders were histologically investigated. Results : At 4 weeks after transplantation into either normal or radiation-injured urinary bladders, incised regions closed. The closed regions of bladder top had reconstructed tissues that were formed with urothelium, and smooth muscle layers. Within the reconstructed tissues, the thickness of the smooth muscle layers in the bilayered BMC-gelatin graft-transplanted bladders were larger compared to controls. The GFP-positive transplanted BMCs were detected. Some of the cells were simultaneously positive for smooth muscle or nerve cell markers. Conclusion : This study showed that the bilayered BMC-gelatin grafts that were experimentally produced could reconstruct bladder tissues. Based on the pole allocation method in control theory, the authors’ previous research found out a fundamental closed-form equation governing a three-lumped-mass stick-shape shear model for a building. Through the selection of appropriate model parameters, the simplified model can represent an earthquake-resistant structure, a base-isolated structure, an interstory-isolated structure, or a passively controlled structure with a tuned mass damper or viscous interstory dampers. The equation explains how the model parameters are related to the control target and generally proves the trade-off relationship in the passive control effect. The present study similarly applies the pole allocation method to a multi-lumped-mass stick-shape model to provide the trade-off relationship with higher generality. The newly introduced equation has the same form as the previous research indicated. Next, this study discusses the installation distribution of interstory dampers from a viewpoint of modal damping as control target.

For a building with a passive control system, an inverse problem is formulated based on the pole allocation method in control theories. The structural system is simplified as a 3DOF damped lumped-mass shear model. The model can express an earthquake resistant structure, a base-isolated structure, an interstory-isolated structure, or a structure with a tuned mass damper or viscous dampers by changing its parameters adequately. The natural frequencies and the corresponding damping ratios in three vibration modes are set as the initial control target for performance-based design. The introduced closed-form expression explains how model parameters are related to the control target, and the mathematical expression generally proves the trade-off relationship in passive control effect. The relationship is extended to an MDOF damped-mass shear model. The numerical examples indicate that the pole allocation has arbitrariness in selecting a solution and a limited solution can be applied to the actual building.

Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disorder (MOGAD) is a newly identified autoimmune demyelinating disorder that is often associated with acute disseminated encephalomyelitis and usually occurs postinfection or postvaccination. Here we report a case of MOGAD after mRNA severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination. A previously healthy 68-year-old woman presented to our department with gradually worsening numbness on the right side of her face, which began 14 days after her second dose of an mRNA-1273 vaccination. The patient's brain MRI revealed a right cerebellar peduncle lesion with gadolinium enhancement, a typical finding of MOGAD. A neurological examination revealed paresthesia on her right V2 and V3 areas. Other neurological examinations were unremarkable. Laboratory workups were positive for serum MOG-IgG as assessed by live cell-based assays and the presence of oligoclonal bands in the cerebrospinal fluid (CSF). The patient's serum test results for cytoplasmic-antineutrophil cytoplasmic antibodies, perinuclear-cytoplasmic-antineutrophil cytoplasmic antibodies, GQ1b-antibodies, and aquaporin-4 antibodies (AQP4-IgG) were all negative. Tests for soluble interleukin (IL)-2 receptors in the serum, IL-6 in the CSF and skin pricks, and angiotensin converting enzyme tests were all unremarkable. The patient was diagnosed with MOGAD after receiving an mRNA SARS-CoV-2 vaccination. After two courses of intravenous methylprednisolone treatment, the patient's symptoms improved and her cerebellar peduncle lesion shrunk slightly without gadolinium enhancement. To date, there have only been two cases of monophasic MOGAD following SARS-CoV-2 vaccination, including both the ChAdOx1 nCOV-19 and mRNA-1273 vaccines, and the prognosis is generally similar to other typical MOGAD cases. Although the appearance of MOG antibodies is relatively rare in post-COVID-19–vaccine demyelinating diseases, MOGAD should be considered in patients with central nervous system (CNS) demyelinating diseases after receiving a SARS-CoV-2 vaccine.

Purpose: To obtain an understanding of the correlation between hemodynamic differences and morphological changes as well as potential sex differences in children with ADHD using multi-delay pseudo-continuous arterial spin labeling (pCASL) imaging and voxel-based morphometry (VBM), especially given that previous findings are limited for girls. Materials and methods: We recruited 23 children with ADHD (mean age, 8.3 years; 19 boys; 4 girls) and 24 children without ADHD (mean age, 9.1 years; 13 boys; 11 girls) as controls. All participants underwent 3D multi-delay pCASL and T1-weighted imaging. The voxel-based statistical parameter mapping (SPM) method was used for group-wise comparisons. Results: Compared with controls, children with ADHD exhibited decreased regional cerebral blood flow (rCBF) and gray matter volume (GMV) in the left middle frontal gyrus and left postcentral gyrus. Analysis by sex revealed reduced rCBF and GMV in the left lingual gyrus and left inferior occipital gyrus in boys with ADHD versus controls and increased rCBF and GMV in the left superior frontal gyrus in girls with ADHD. Conclusion: Although our results are preliminary because of small sample sizes, several brain regions exhibit changes in both cerebral perfusion and GMV in the same direction in patients with ADHD, with boys with ADHD showing decreased activity and girls with ADHD displaying increased activity in the fronto-parietal cortices.

Purpose : To determine the reproducibility of corrected quantitative cerebral blood flow (qCBF) through measurement of transit flow time using multi-delay three-dimensional pseudo-continuous arterial spin labeling (pCASL) in healthy men and women and to evaluate the differences in qCBF between not only men and women, but also the follicular and luteal phases of the women’s menstrual cycle. Methods : The participants were 16 healthy volunteers (8 men and 8 women ; mean age, 25.3 years). Two MRI were conducted for all participants ; female participants were conducted in the follicular and luteal phases. The reproducibility of qCBF values was evaluated by the intraclass correlation coefficient (ICC) and differences between the two groups were estimated by voxel-based morphometry (VBM) analysis. Results : The qCBF values were lower in men than in women, and those in females were significantly different between the follicular and luteal phases (P < 0.05). In VBM analysis, the qCBF values of the lower frontal lobes were significantly higher in women than in men (P < 0.05). The qCBF values of the frontal pole were significantly higher in the follicular phase than in the luteal phase (P < 0.01). Conclusion : Multi-delay pCASL can reveal physiological and sex differences in cerebral perfusion.

航法技術は宇宙機に不可欠な技術であり，宇宙機自身の位置や速度，姿勢といった状態量を知るための技術である．宇宙機のそうした情報を知ろうとした場合，一般的には地上局からのレンジングや慣性航法装置などが使われるが，深宇宙の小天体を探査するミッションなどでは航法精度が足りない場合がある．このようなミッションでは，対象天体に安全かつ高精度に接近するための技術が求められる．画像情報を用いた航法技術はそうしたミッションで使われることが多く，これまでも小惑星探査や小天体フライバイなど多くミッションで実証されてきた．さらに近年では月や火星の豊富な科学データをもとにあらかじめ決めた着陸地点に高精度で着陸するミッションも多く立案されている．そうしたミッションでも画像を用いた航法技術は大いに有用である．本稿は，画像情報を用いた宇宙機の航法技術全般について国内外の探査プロジェクトないしは技術実証プロジェクトのサーベイを行い，その結果をまとめたものである．, Navigation technology is essential for spacecraft, and is a technique to get the state such as the position, velocity, and attitude of spacecraft itself. Generally, ranging from ground stations or inertial navigation systems are used to obtain such information of spacecraft. However in recent mission such like small body exploration in deep space, the navigation accuracy is insufficient. In such missions, technologies for safe and accurate access to the target bodies are required. Vision-based navigation technologies are often used in such missions, and has been demonstrated in many missions such as asteroid exploration and asteroid flyby. In recent years, many missions that aim to land predetermined landing point precisely have been planned based on abundant scientific data of the moon and Mars. Vision-based navigation technologies are also very useful in these missions. In this study, we measured space charge distribution during proton beam irradiation in order to understand the charge accumulation characteristics of space insulating materials by proton beam irradiation. As a result, positive charge accumulation was observed in the sample, and different charge accumulation tendencies were obtained in the proton passage region and the unreachable region. Purpose : To compare data on brain tumors derived from intravoxel incoherent motion (IVIM) and arterial spin labeling (ASL) imaging with multiple parameters obtained on dynamic susceptibility contrast (DSC) perfusion MRI and to clarify the characteristics of IVIM and ASL perfusion data from the viewpoint of cerebral blood flow (CBF) analysis. Methods : ASL-CBF and IVIM techniques as well as DSC examination were performed in 24 patients with brain tumors. The IVIM data were analyzed with the two models. The relative blood flow (rBF), relative blood volume (rBV) corrected relative blood volume (crBV), mean transit time (MTT), and leakage coefficient (K2) were obtained from the DSC MRI data. Results : The ASL-CBF had the same tendency as the perfusion parameters derived from the DSC data, but the permeability from the vessels had less of an effect on the ASL-CBF. The diffusion coefficient of the fast component on IVIM contained more information on permeability than the f value. Conclusion : ASL-CBF is more suitable for the evaluation of perfusion in brain tumors than IVIM parameters. ASL-CBF and IVIM techniques should be carefully selected and the biological significance of each parameter should be understood for the correct comprehension of the pathological status of brain tumors.

The observation of walking improves gait ability in chronic stroke survivors. It has also been suggested that activation of the mirror neuron system contributes to this effect. However, activation of the mirror neuron system during gait observation has not yet been assessed in sub-acute stroke patients. The objective of this study was to clarify the activation of mirror neuron system during gait observation in sub-acute stroke patients and healthy persons. In this study, we sequentially enrolled five sub-acute stroke patients who had undergone gait training and nine healthy persons. We used fMRI to detect neuronal activation during gait observation. During the observation period in the stroke group, neural activity in the left inferior parietal lobule, right and left inferior frontal gyrus was significantly higher than during the rest period. In the healthy group, neural activity in the left inferior parietal lobule, left inferior frontal gyrus, left middle frontal gyrus, left superior temporal lobule and right and left middle temporal gyrus was significantly higher than during the rest period. The results indicate that the mirror neuron system was activated during gait observation in sub-acute stroke patients who had undergone gait training and also in healthy persons. Our findings suggest that gait observation treatment may provide a promising therapeutic strategy in sub-acute stroke patients who have experienced gait training.

研究の目的 : 多変量解析とGISを用いて、乳幼児を対象とする日本の予防接種率の地域差の実態を把握し、その要因を検討する。研究の背景 : 公衆衛生上の課題である予防接種率の地域差の特定と改善を目的として、多様な地域スケールからその要因を分析する研究が行われている。しかしながら、日本における既往研究では個人を対象とした研究が多く、接種率の地域差の検討や関連要因を用いた生態学的研究もほとんど行われてこなかった。研究手法 : 日本の1737の市区町村における7種類19回の予防接種を対象として、重回帰分析と市区町村―都道府県間の階層を用いたマルチレベル分析を行った。被説明変数には推計した2013年から2018年までの予防接種率を、説明変数には9つの個人要因と4つの環境要因を用いた。これらの変数の作成には国勢調査などの政府統計を用いた。予防接種率の分布図と予測モデルの残差プロットを用いて、地理的な関連要因を探索した。結果 : 一人当たり課税所得と乳幼児健診受診率は市区町村レベルで、世帯の児童数は都道府県レベルで全般的に有意な関連を示した。その他の変数は予防接種の種類や接種時期(回数)によって関連の有無が異なった。接種率は西日本で低く、大都市圏で高い傾向がみられた。残差は内陸部と接種率の偏差が大きい地域に多く見られた。考察 : 市区町村の所得格差と都道府県の世帯あたり児童数の差が、地域の接種率の格差に関連すると示唆された。また、人口減少と高齢化が進む小規模地域には接種率に関連する固有の因子が存在する可能性がある。結論 : 接種率の向上には、その地域の社会経済的状況や人口構造、地域の文脈、予防接種の種類、それらの要因の影響するスケールを踏まえた施策が求められる。, Objective: Examining geographical variation and the factors of infant immunization coverage in Japan. Background: Incomplete vaccination coverage is one of the largest public health threats. Many interdisciplinary studies have investigated factors that might increase immunization rates or cause Vaccine Hesitancy. However, most studies in Japan have focused on individuals and few studies have considered regional disparities of vaccination coverage. Methods: We conducted a multiple regression analysis and a multilevel analysis (municipality-prefecture hierarchy) for 19 vaccinations in 1737 municipalities in Japan, during 2013-2018. The dependent variable was the estimated vaccination rate, and nine individual factors and four environmental factors were used as explanatory variables. Data were collected from publicly available sources. We used distribution maps of vaccination rates and residuals of the predictive models to explore geographical factors. Results: In general, per capita taxable income and infant health checkup rate showed generally significant positive associations at the municipal level, and the number of children in the household negative association at the prefectural level. Other factors showed various associations depending on the type and the timing of vaccines. Vaccination rates were likely to be lower in western Japan and higher in metropolitan areas. Residuals were found mostly in inland areas and areas with large deviations in vaccination rates. Discussion: These results suggested that vaccination rates were associated with regional differences in municipal income inequality and in the number of children per household in prefectures. Smaller villages with declining and aging populations might have unique factors. Conclusions: To increase vaccination coverage among infants, it requires to consider the effect size at various spatial scales, in addition to compositional and contextual effects as the socioeconomic status, demographic structure, the local contexts, and the type of vaccination.

In clinical magnetic resonance imaging, gadolinium-based contrast agents are commonly used for detecting brain tumors and evaluating the extent of malignancy. We present a new method to evaluate relaxivity (r1) and contrast agent concentration separately in contrast-enhanced lesions using quantitative parameter mapping (QPM). Furthermore, we also aimed to estimate the extracellular pH (pHe) of tumor lesions. We demonstrated that it is possible to evaluate pathophysiological tumor changes due to therapeutic efficacy by measuring r1 in contrast-enhanced lesions. In this study, the primary brain tumor group showed significantly higher r1 values than other brain disease groups (P < 0.001). Moreover, mean pHe value showed a trend for tumor malignancy having a lower pHe value and primary brain tumor having a significantly lower pHe than other brain diseases (P < 0.001). Our results might suggest that QPM can separately quantify r1 and CA concentration in brain tumors and that pHe brain tumor mapping could serve as a tumor biomarker. In conclusion, our method has potential clinical applications for assessing the treatment effects.

A 71-year-old man visited a local urologist with the complaint of continuous painful erection for three days. Penile cavernosal blood data showed acidosis and hypoxia. Thus he was diagnosed with ischemic priapism. After penile aspiration and injection of phenylephrine, erection was temporary improved. The blood platelet count was 94.5×104/μl and myeloproliferative disease was suspected. He was referred to our hospital and visited us the following day. Since priapism relapsed, we aspirated corpus cavernosum of the penis and injected phenylephrine which was successful. A bone marrow biopsy and genetic test were performed at the department of hematology and the diagnosis of essential thrombocythemia with JAK2 gene mutation was confirmed. By treatment with hydroxyurea, the blood platelet count decreased without priapism recurrence.

Primary effusion lymphoma‐like lymphoma (PEL‐LL) is a rare B‐cell lymphoma that the etiology remains unclear. We describe a case of PEL‐LL with a pleuropericardial effusion. Diagnosis required long period of time as it followed a unique progress of disappearance and recurrence of the body cavity effusion. We finally had a diagnosis of B‐cell lymphoma by the immunocytochemistry of effusion using the cell block procedure. Authors consider that it is valuable to actively try the cell block procedure at the time of the first drainage for early diagnosis, if the body cavity effusion due to the malignancy is suspected.

This strain demonstrated nonsusceptible to all tested antibiotics, including last-line antibiotics such as carbapenems (MICs ≥128 μg/mL) and colistin (MIC =32 μg/mL), except tigecycline (MIC =1 μg/mL). Whole-genome analysis using both MinION and MiSeq data revealed that the strain carried 29 resistance genes. Particularly, mcr-1 and blaNDM-4 genes were carried by different self-conjugative plasmids and able to be transferred to a recipient by conjugation. The colistin resistance of this strain was conferred by mcr-1 and additional chromosomal resistance determinants. Eight amino acid substitutions found in PmrA, PmrB, PmrC, PmrI, and PmrJ, all proteins that are involved in lipopolysaccharide modifications, may be associated with chromosomal colistin resistance. Mucinous tubular and spindle cell carcinoma (MTSCC) is a distinct entity in the World Health Organization classification of kidney tumors since 2004. Herein, we report a case of a patient with MTSCC of the kidney. A 48-year-man visited our hospital with a chief complaint of occult blood in his urine, confirmed by urine occult blood reaction. Computed tomography revealed a solid tumor in the right kidney. The tumor was 40×38 mm in length and was slightly enhanced (cT1aN0M0). Therefore, we performed radical nephrectomy. On analysis of the resected specimen, we found that the number of comparatively small malignant cells had increased markedly, forming branched tubular cuboidal cells. Further more, positive results were obtained on staining the stroma with both PAS and alcian blue stainscharacteristic of papillary renal cell carcinoma ; however, extracellular mucinous material was found to be depleted. Therefore, we needed to differentiate between papillary renal cell carcinoma and MTSCC. Finally, on the basis of the immunohistochemical staining results-vimentin (＋), CK34βE12 (−), and CD10 (−)-MTSCC was confirmed.

We report a case of vesical endometriosis that worsened during the early pregnancy period. A 37-yearold woman had been under treatment for endometriosis (including vesical endometriosis) by a gynecologist during the past 10 years. She was treated for sterility 1 year ago, and became pregnant through in vitro fertilization. In her 8th gestational week, she complained of gross hematuria at our hospital. Cystoscopic findings revealed some tumors that appeared worse than the last findings two years ago. In order to deny malignancy, transurethral resection of the bladder tumor was performed in her 12th gestational week. The pathologic diagnosis was endometriosis. She was able to stay pregnant, and delivered a girl. After delivery, cystoscopic findings revealed reduction of tumors. In most cases pregnancy cures endometriosis ; however, in this case symptoms became worse during the early stage of pregnancy. The reason for this contrary event is discussed.
