Classic adenoid cystic carcinomas (C-AdCCs) of the breast are rare, relatively indolent forms of triple negative cancers, characterized by recurrent MYB or MYBL1 genetic alterations. Solid and basaloid adenoid cystic carcinoma (SB-AdCC) is considered a rare variant of AdCC yet to be fully characterized. Here, we sought to determine the clinical behavior and repertoire of genetic alterations of SB-AdCCs. Clinicopathologic data were collected on a cohort of 104 breast AdCCs (75 C-AdCCs and 29 SB-AdCCs). MYB expression was assessed by immunohistochemistry and MYB-NFIB and MYBL1 gene rearrangements were investigated by fluorescent in-situ hybridization. AdCCs lacking MYB/MYBL1 rearrangements were subjected to RNA-sequencing. Targeted sequencing data were available for 9 cases. The invasive disease-free survival (IDFS) and overall survival (OS) were assessed in C-AdCC and SB-AdCC. SB-AdCCs have higher histologic grade, and more frequent nodal and distant metastases than C-AdCCs. MYB/MYBL1 rearrangements were significantly less frequent in SB-AdCC than C-AdCC (3/14, 21% vs 17/20, 85% P 0.05), despite the frequent MYB expression (9/14, 64%). In SB-AdCCs lacking MYB rearrangements, CREBBP, KMT2C, and NOTCH1 alterations were observed in 2 of 4 cases. SB-AdCCs displayed a shorter IDFS than C-AdCCs (46.5 vs 151.8 months, respectively, P 0.001), independent of stage. In summary, SB-AdCCs are a molecularly heterogeneous but clinically aggressive group of tumors. Less than 25% of SB-AdCCs display the genomic features of C-AdCC. Defining whether these tumors represent a single entity or a collection of different cancer types with a similar basaloid histologic appearance is warranted.

OBJECTIVES: We compared the relative effect of hypertensive disorders of pregnancy and chorioamnionitis on adverse neonatal outcomes in very preterm neonates, and studied whether gestational age (GA) modulates these effects. METHODS: A cohort of neonates 23 to 30 weeks' GA, born in 2008 to 2011 in 82 hospitals adhering to the Italian Neonatal Network, was analyzed. Infants born from mothers who had hypertensive disorders (N = 2096) were compared with those born after chorioamnionitis (N = 1510). Statistical analysis employed logistic models, adjusting for GA, hospital, and potential confounders. RESULTS: Overall mortality was higher after hypertension than after chorioamnionitis (odds ratio [OR], 1.39; 95% confidence interval [CI], 1.08–1.80), but this relationship changed across GA weeks; the OR for hypertension was highest at low GA, whereas from 28 weeks' GA onward, mortality was higher for chorioamnionitis. For other outcomes, the relative risks were constant across GA; infants born after hypertension had an increased risk for bronchopulmonary dysplasia (OR, 2.20; 95% CI, 1.68–2.88) and severe retinopathy of prematurity (OR, 1.48; 95% CI, 1.02–2.15), whereas there was a lower risk for early-onset sepsis (OR, 0.25; 95% CI, 0.19–0.34), severe intraventricular hemorrhage (OR, 0.65; 95% CI, 0.48–0.88), periventricular leukomalacia (OR, 0.70; 95% CI, 0.48–1.01), and surgical necrotizing enterocolitis or gastrointestinal perforation (OR, 0.47; 95% CI, 0.31–0.72). CONCLUSIONS: Mortality and other adverse outcomes in very preterm infants depend on antecedents of preterm birth. Hypertension and chorioamnionitis are associated with different patterns of outcomes; for mortality, the effect changes across GA weeks.

Purpose: The mTOR complex C1 (mTORC1) inhibitor everolimus in combination with the aromatase inhibitor exemestane is an effective treatment for patients with hormone receptor—positive (HR+), HER2-negative (HER2−), advanced breast cancer (HR+/HER2− aBC). However, everolimus can cause hyperglycemia and hyperinsulinemia, which could reactivate the PI3K/protein kinase B (AKT)/mTORC1 pathway and induce tumor resistance to everolimus. Experimental Design: We conducted a multicenter, retrospective, Italian study to investigate the impact of baseline and on-treatment (i.e., during first 3 months of therapy) blood glucose levels on progression-free survival (PFS) in patients with HR+/HER2− aBC treated with everolimus-exemestane. Results: We evaluated 809 patients with HR+/HER2− aBC treated with everolimus-exemestane as any line of therapy for advanced disease. When evaluated as dichotomous variables, baseline and on-treatment glycemia were not significantly associated with PFS. However, when blood glucose concentration was evaluated as a continuous variable, a multivariable model accounting for clinically relevant patient- and tumor-related variables revealed that both baseline and on-treatment glycemia are associated with PFS, and this association is largely attributable to their interaction. In particular, patients who are normoglycemic at baseline and experience on-treatment diabetes have lower PFS compared with patients who are already hyperglycemic at baseline and experience diabetes during everolimus-exemestane therapy (median PFS, 6.34 vs. 10.32 months; HR, 1.76; 95% confidence interval, 1.15–2.69; P = 0.008). Conclusions: The impact of on-treatment glycemia on the efficacy of everolimus-exemestane therapy in patients with HR+/HER2− aBC depends on baseline glycemia. This study lays the foundations for investigating novel therapeutic approaches to target the glucose/insulin axis in combination with PI3K/AKT/mTORC1 inhibitors in patients with HR+/HER2− aBC.

Vortex-induced vibrations represent a potentially critical phenomenon for bridges in a multitude of cases. The increasing optimization of modern bridge structures is giving rise to long spans and slender decks particularly sensitive to vortex shedding, which may generate deck oscillations unacceptable for the comfort of the users even for relatively low wind velocities. At the same time, the variety of geometric details characterizing bridge deck cross sections and the marked influence of an even limited variation of the flow angle of incidence add further difficulties to the prediction of bridge deck VIV response without an accurate and extended experimental campaign. In this context, VIV mathematical modeling represents a challenging but also an attractive issue for bridge decks, not only from a scientific point of view, but hopefully with the practical purpose of developing a mathematical approach able to support or at least partially reduce the amount of wind tunnel tests needed during the design phase. The present dissertation deals with the critical effect of significant cross-section geometric details and flow angle of attack on bridge deck VIV response and these factors are included in an extended study about two mathematical approaches for VIV response prediction applied to different configurations of a realistic bridge section. An overview of representative and meaningful studies provided by scientific literature about the influence of geometric details and angle of attack is firstly proposed, along with a description of mathematical modeling attempts developed over the years for VIV modeling of elongated cylinders with a constant cross section. The variability given by geometric features combined with angle of attack variation over a realistic range of values can be remarkable. This should be preferably taken into account by VIV mathematical modeling, usually developed, on the other hand, for simplified section geometries at zero angle of attack. In this work, a wake-oscillator model, derived from Tamura and Matsui’s one, and a modified version of harmonic model are deeply discussed and employed for a realistic cross-section geometry. For the purpose of exploring the influence of geometric details at different wind angles of incidence, calibrating both models and assessing their performances, wind tunnel tests were performed on a bridge deck sectional model. The modification of bridge section lower corners, the addition of two lateral barrier typologies and the investigation of different angles of attack gave rise to a large amount of tested section layouts. Aerodynamic force measurements were firstly performed on the stationary body and results were employed for mathematical model calibration and to formulate qualitative suppositions about the expected dynamic behavior of different cross-section layouts. Then, aeroelastic tests were carried out, with the model elastically suspended and free to vibrate. Response curves for different values of wind velocity were determined for each geometric layout. They were employed to complete wake-oscillator model calibration and compared to the predictions of both mathematical approaches. A critical effect of lateral barriers installed on the deck was observed, along with marked effects, even unexpected in some cases, produced by limited angle of attack variations. In addition, an estimation of the aerodynamic damping was conducted through free-decay tests. In the last part of the dissertation, VIV response mathematical modeling is addressed specifically. According to what previously stated, a Tamura-type wake-oscillator model and a modified harmonic model were investigated. The first one is a physically based two-degree-of-freedom approach, while the second one is a one-degree-of-freedom simplified model aiming to a quick and relatively easy prediction of the peak response at lock-in. A mathematical connection between the models was explored and both approaches were studied and applied preliminarily for a 4:1 rectangular cylinder and then for the bridge deck geometric configurations experimentally tested at different angles of attack. The modified harmonic model was calibrated only through results of static force measurements, while for the wake oscillator model, in absence of further static tests like flow visualizations, the response curves obtained at low Scruton number were employed to calibrate a part of model parameters. Virtues and limitations of both models are discussed. In particular, the necessity of adapting the wake oscillator design of the Tamura-type model for elongated cross sections, like a 4:1 rectangle or a bridge deck, was observed. Speculations about wake lamina geometry modification were formulated, with the purpose of achieving, for elongated sections, the appropriate values of model parameters without losing their physical origin. On the other hand, wake-oscillator model equations of motion were found able to reproduce a large variety of experimental response curve shapes, while limitations in peak response amplitude prediction were frequently observed, as well as for the modified harmonic model, especially where a considerable oscillation amplitude was found. Such limitations were attributed to the spanwise correlation increase of the vortex-shedding force with the oscillation amplitude; full correlation at lock-in was proposed as upper bound condition for the fluctuating force acting on the body. Pressure measurements for correlation effect quantification and further study about the wake oscillator structure for elongated sections, including the addition of a degree of freedom, are finally proposed as main possible advancements to improve the present work., Wirbelinduzierte Schwingungen stellen für Brücken in einer Vielzahl von Fällen ein potenziell kritisches Phänomen dar. Die zunehmende Optimierung moderner Brückenkonstruktionen führt zu langen Spannweiten und schlanken Überbauten, die besonders empfindlich auf Wirbelablösungen reagieren, woraus schon bei relativ geringen Windgeschwindigkeiten für die Nutzer unzumutbare Schwingungen resultieren können. Gleichzeitig erschweren die Vielzahl geometrischer Details, die den Querschnitt des Brückenüberbaus ausmachen, und der selbst bei begrenzter Variation starke Einfluss des Strömungseinfallswinkels die Vorhersage des VIV-Verhaltens von Brückenüberbauten ohne eine zusätzliche, genaue und umfangreiche Versuchskampagne. In diesem Zusammenhang stellt die mathematische Modellierung des VIV-Verhaltens von Brückenüberbauten eine herausfordernde, aber auch interessante Thematik dar, nicht nur aus wissenschaftlicher Sicht, sondern auch mit dem praktischen Ziel, einen mathematischen Ansatz zu entwickeln, der die in der Entwurfsphase erforderlichen Windkanalversuche unterstützt oder zumindest dessen Umfang reduziert. Die vorliegende Dissertation befasst sich mit dem kritischen Einfluss signifikanter geometrischer Querschnittsdetails und des Anströmwinkels auf das VIV-Verhalten von Brückenüberbauten. Diese Faktoren werden in einer ausführlichen Studie über zwei mathematische Ansätze zur Vorhersage des VIV-Verhaltens berücksichtigt, welche für verschiedene Konfigurationen eines realistischen Brückenabschnitts angewendet werden. Zunächst wird ein Überblick über repräsentative und aussagekräftige Studien aus der wissenschaftlichen Literatur über den Einfluss geometrischer Details und des Anströmwinkels gegeben, zusammen mit einer Beschreibung der mathematischen Modellierungsversuche, die im Laufe der Jahre für die VIV-Modellierung von langgestreckten Zylindern mit konstantem Querschnitt entwickelt wurden. Die Variabilität, die sich aus der Kombination der geometrischen Merkmale und der Variation des Anströmwinkels über einen realistischen Bereich ergibt, kann erheblich sein. Dies ist insbesondere bei der mathematischen VIV-Modellierung zu berücksichtigen, da die Modelle üblicherweise für vereinfachte Querschnitte und einen Null-Grad-Winkel der Anströmung entwickelt wurden. In dieser Arbeit werden ein von Tamura und Matsui abgeleitetes Nachlaufoszillatormodell und eine modifizierte Version des harmonischen Modells eingehend diskutiert und auf eine realistische Querschnittsgeometrie angewendet. Um den Einfluss geometrischer Details bei verschiedenen Windangriffswinkeln zu untersuchen, die beide Modelle zu kalibrieren und ihre Leistungsfähigkeit zu bewerten, wurden Windkanalversuche an einem Sektionsmodell eines Brückenüberbauquerschnitts durchgeführt. Die Veränderung der unteren Ecken des Querschnitts, das Hinzufügen von zwei unterschiedlichen Arten seitlicher Barrieren und die Untersuchung verschiedener Anströmwinkel führten zu einer großen Anzahl getesteter Konfigurationen. Zunächst wurden aerodynamische Kraftmessungen am stationären Körper durchgeführt und die Ergebnisse für die Kalibrierung des mathematischen Modells und der Formulierung qualitativer Vorhersagen über das zu erwartende dynamische Verhalten verschiedener Querschnittsausführungen verwendet. Anschließend wurden aeroelastische Tests durchgeführt, bei denen das Modell elastisch aufgehängt wurde und frei schwingen konnte. Für jede geometrische Ausführung wurden die Antwortkurven bei unterschiedlichen Windgeschwindigkeiten ermittelt. Diese wurden mit den Vorhersagen der beiden mathematischen Ansätze verglichen und verwendet, um das Nachlaufoszillatormodell vollständig zu kalibrieren. Es wurde ein kritischer Effekt der auf der Brücke installierten seitlichen Barrieren beobachtet, zusammen mit deutlichen Effekten, die in einigen Fällen sogar unerwartet waren, die durch begrenzte Variationen des Anstellwinkels erzeugt wurden. Darüber hinaus wurde eine Abschätzung der aerodynamischen Dämpfung anhand von Ausschwingversuchen durchgeführt. Im letzten Teil der Dissertation geht es speziell um die mathematische Modellierung der VIV-Reaktion. Wie bereits erwähnt, wurden ein Nachlaufoszillatormodell nach Tamura und ein modifiziertes harmonisches Modell untersucht. Das erste ist ein physikalisch basierter Ansatz mit zwei Freiheitsgraden, wohingegen das zweite ein vereinfachtes Modell mit nur einem Freiheitsgrad ist, welches auf eine schnelle und vergleichsweise einfache Vorhersage der Spitzenantwort beim Lock-in abzielt. Es wurde eine mathematische Verbindung zwischen den Modellen hergestellt. Beide Ansätze wurden zunächst für einen Reckteckzylinder mit einem Seitenverhältnis von 4:1 und anschließend für die geometrischen Ausführungsvarianten des Brückenüberbauquerschnitts bei unterschiedlichen Anströmwinkeln untersucht und angewendet. Das modifizierte harmonische Modell wurde anhand der Ergebnisse aus den statischen Kraftmessungen kalibriert. Für das Nachlaufoszillatormodell hingegen wurden die bei niedrigen Scrutonzahlen ermittelten Antwortkurven aufgrund des Nichtvorhandenseins weiterer statischer Tests, wie Strömungsvisualisierungen, zur Kalibrierung eines Teils der Modellparameter verwendet. Die Vorzüge und Anwendungsgrenzen beider Modelle werden diskutiert. Insbesondere wurde die Notwendigkeit der Anpassung des Nachlaufoszillatormodells nach Tamura für langgestreckte Querschnitte, wie einen 4:1-Rechteckzylinder oder einen Brückenüberbau, festgestellt. Es wurden Vorschläge zur Modifizierung der Nachlauflamellengeometrie formuliert, um für langgestreckte Querschnitte die geeigneten Werte der Modellparameter zu erhalten, ohne ihren physikalischen Ursprung zu verlieren. Andererseits wurde festgestellt, dass die Bewegungsgleichungen des Nachlaufoszillatormodells in der Lage sind, eine Vielzahl von experimentellen Antwortkurvenformen zu reproduzieren, während bei der Vorhersage der Spitzenantwortamplitude häufig Einschränkungen beobachtet wurden, ebenso wie beim modifizierten harmonischen Modell, insbesondere wenn eine große Schwingungsamplitude vorlag. Diese Einschränkungen wurden auf die über die Spannweite verlaufende Korrelationszunahme der Wirbelablösekraft mit der Schwingungsamplitude zurückgeführt; eine vollständige Korrelation beim Lock-in wurde als obere Grenzbedingung für die auf den Körper wirkende fluktuierende Kraft vorgeschlagen. Druckmessungen zur Quantifizierung des Korrelationseffekts und weitere Studien über die Struktur des Nachlaufoszillators für langgestreckte Querschnitte, einschließlich des Hinzufügens eines Freiheitsgrads, werden schließlich als wichtigste mögliche Schritte zur Weiterentwicklung der vorliegenden Arbeit vorgeschlagen.
