Introduction: Novel post-processing methods allow not only for assessment of brain volumetry or cortical thickness based on magnetic resonance imaging (MRI) but also for more detailed analysis of cortical shape and complexity using parameters such as sulcal depth, gyrification index, or fractal dimension. The aim of this study was to analyze changes in brain volumetry and other cortical indices during aging in men and women.Material and Methods: Material consisted of 697 healthy volunteers (aged 38–80 years; M/F, 264/443) who underwent brain MRI using a 1.5-T scanner. Voxel-based volumetry of total gray matter (GM), white matter (WM), and cerebrospinal fluid (CSF) was performed followed by assessment of cortical parameters [cortical thickness (CT), sulcal depth (SD), gyrification index (GI), and fractal dimension (FD)] in 150 atlas locations using surface-based morphometry with a region-based approach. All parameters were compared among seven age groups (grouped every 5 years) separately for men and women. Additionally, percentile curves for men and women were provided for total volumes of GM, WM, and CSF.Results: In men and women, a decrease in GM and WM volumes and an increase in CSF volume seem to progress slowly since the age of 45. In men, significant GM and WM loss as well as CSF increase start above 55 years of age, while in women, significant GM loss starts above 50 and significant WM loss as well as CSF increase above 60. CT was found to significantly decrease with aging in 39% of locations in women and in 36% of locations in men, SD was found to increase in 13.5% of locations in women and in 1.3% of locations in men, GI was decreased in 3.4% of locations in women and in 2.0% of locations in men, and FD was changed in 2.7% of locations in women compared to 2.0% in men.Conclusions: Male and female brains start aging at the similar age of 45. Compared to men, in women, the cortex is affected earlier and in the more complex pattern regarding not only cortical loss but also other alterations within the cortical shape, with relatively longer sparing of WM volume.

Objective To assess the current framework of interventional and diagnostic neuroradiology in Europe Methods The UEMS (European Union of Medical Specialists) Section of Radiology and the subspecialty UEMS Division of Neuroradiology collected by e-mail a survey on the situation of diagnostic and Interventional Neuroradiology’ training and practice in Europe. The questionnaire was sent to the national delegates from 31 UEMS member countries, belonging to the European Union, the European Economic Area and the Council of Europe. In case of uncertain or discordant replies, the survey envisaged the involvement of neuroradiology scientific societies’ experts for providing a decisive answer. Results A formal post-residency training in diagnostic and interventional neuroradiology is provided respectively by 12/31 and 20/31 of the European countries. Currently, for becoming neuroradiologist in a country without fellowship program, a radiologist should (1) get subspecialty credits, (2) follow training inside national or international neuroradiology departments, or (3) perform the main reporting activity in neuroradiology. In nearly 2/3 of the States included in the survey, the neurointerventional procedures are provided by radiologists (22/31) and in the most frequent scenario a specific training in neurovascular is required to all radiologist or non-radiologist candidates (18/31). Conclusions The European framework of neuroradiology’s training and practice that emerged through this survey is fragmented, but there is an increasing attention by European scientific societies and institutions to create a common path of training and practice that can guarantee high educational and patient care standards.

This document sets out standards for training in interventional neuroradiology (INR) in Europe. These standards have been developed by a working group of the European Society of Neuroradiology (ESNR) and the European Society of Minimally Invasive Neurological Therapy (ESMINT) on the initiative and under the umbrella of the Division of Neuroradiology/Section of Radiology of the European Union of Medical Specialists (UEMS). For this document, the text of guidelines proposed and published earlier by the previous board of UEMS’s Division of Neuroradiology1 2 was used and modified with the consensus of all involved parties. It is well recognized that there are a number of structural and operational differences in the healthcare systems, appointment procedures, and training systems in the various European countries. This document takes into consideration the health system and demographic realities of Europe. At present, the national laws in each of the EU member states regulate the professional activity of each country. Thus, it is important to build a comprehensive INR core curriculum and training program that can be used in the individual countries according to their organization and laws, and that complies with international treaties, the UN Declarations on Human Rights, and the WMA International Code of Medical Ethics. The INR curriculum should constitute a uniform training program, approved by all the European countries, which can be incorporated in a pre-existing national program or serve as the basis for the development of a new program, according to the national regulations. In this document the INR training program will be referred to as ' particular qualification' in INR. A considerable number of specialists in Europe are performing all or some of the INR activities detailed in this training charter. Background A clinical case is described of growth retardation, severe developmental delay, facial dysmorphic features with microcephaly, as well as congenital cataract, schizencephaly, periventricular calcifications, and epilepsy. Methods TORCH infection was suspected, but all tests for toxoplasmosis, rubella, cytomegalovirus, and herpes simplex virus were negative for the child and her mother; however, an increased level of antibodies against parvovirus B19 was detected in the proband. Results Chromosomal analysis and array-CGH showed no aberration. Target capture sequencing for COL4A1 and COL4A2 revealed a de novo COL4A1 mutation (c.2123G>T [p.Gly708Val]). The mutation occurred at a highly conserved Gly residue in the Gly-X-Y repeat of the collagen triple helical domain, suggesting that these mutations may alter the collagen IV α1α1α2 heterotrimers. The mutation was predicted to be damaging. Conclusion We suggest that COL4A1 testing should be considered in patients with schizencephaly as well as with phenotype suggesting TORCH infection without any proven etiological factors. BACKGROUND The posterior cingulate region is an area of the earliest pathological changes in amnestic mild cognitive impairment (aMCI). The utility of FDG-PET imaging in dementia is already well established. OBJECTIVES The aim of the study was to compare FDG-PET with advanced MR measurements: MR spectroscopy (MRS), perfusion weighted imaging (PWI), and diffusion tensor imaging (DTI) within the posterior cingulate region in patients with aMCI. METHODS Fifty-five patients diagnosed with aMCI (66.5 y) and 20 age-matched controls (69 y) underwent MR examination including MRS, PWI, and DTI followed by FDG-PET scanning. Values of MRS metabolite ratios (NAA/Cr, Cho/Cr, mI/Cr), PWI cerebral blood volume (rCBV), and DTI fractional anisotropy (FA) were compared to the FDG-PET rates of glucose metabolism. RESULTS Compared to controls, aMCI patients showed significant (p < 0.05) glucose hypometabolism, and lower rCBV and FA values. FDG-PET results correlated significantly with rCBV values. Compared to FDG-PET, PWI showed similar and DTI greater accuracy in distinguishing aMCI from controls. According to FDG-PET findings, two groups of aMCI patients were established: those with lower (PET-positive) and normal (PET-negative) glucose uptake. PET-positive aMCI subjects showed normal MRS findings, lower rCBV and FA values, while PET-negative patients revealed normal MRS and PWI results but significantly lower FA values. CONCLUSIONS Advanced MR techniques such as PWI and particularly DTI may be regarded as competitive techniques to FDG-PET. DTI was the only method to show alterations in aMCI patients with normal FDG-PET, PWI, and MRS findings. DTI seems to be a very sensitive biomarker of early degeneration in aMCI.

Background The origin of fatigue in multiple sclerosis (MS) remains unclear. Magnetic resonance spectroscopy (MRS) provides an insight into metabolic properties of the brain. Objectives The aim of the study was to evaluate brain MRS measurements in MS patients, with regard to fatigue and cognition. Methods The study comprised 32 MS patients and 43 controls. Fatigue was assessed using the Fatigue Severity Scale (FSS) and the Modified Fatigue Impact Scale (MFIS) and cognition — using the Brief Repeatable Battery of Neuropsychological Tests (parts of BRBNT). MRS voxels were placed in the parietal white matter (PWM) and the posterior cingulate gyrus (PCG); N-acetyl-aspartate (NAA), choline (Cho) and myoinositol (mI) to creatine (Cr) ratios were determined. Relationships were searched between MRS measurements and fatigue as well as BRBNT results. Results MS patients in comparison with controls showed decreased NAA/Cr and increased mI/Cr ratios in PCG and PWM, respectively. No significant relationships between MRS parameters and fatigue measures, BRBNT results or MS-related variables were found. Conclusions The decrease of NAA and increase of mI within white and gray matters in MS patients do not show a significant relationship with cognitive performance or fatigue.

Twenty-two neuropsychiatric (NPSLE) and 13 systemic lupus erythematosus (SLE) patients with a normal appearing brain on plain magnetic resonance (MR) as well as 20 age-matched healthy controls underwent MR spectroscopy (MRS), perfusion-weighted (PWI) and diffusion-tensor imaging (DTI). In MRS NAA/Cr, Cho/Cr and mI/Cr ratios were calculated from the posterior cingulate cortex and left parietal white matter. Introduction: Recent developments of spinal instruments allow to address nearly all components of idiopathic scoliosis. Direct vertebral rotation (DVR) maneuver was introduced to correct apical axial vertebral rotation. It is however still not well established how efficiently DVR affects results of scoliosis correction. The object of the study was to evaluate en bloc apical vertebral rotation (DVR) and its impact on coronal and sagittal correction of the spine in patients undergoing surgical scoliosis treatment. Material and methods: 36 consecutive patients who underwent posterior fusion with pedicle screws only constructs for idiopathic scoliosis. 15 patients (20 curves) were corrected by rod derotation only and 21 patients (26 curves) had both rod derotation and DVR. Curve measurements were performed on x-rays obtained before and post operatively - coronal curves, kyphosis (T2-T12, T5-T12). Spine flexibility was assessed on prone bending x-rays. Apical axial rotation was determined on CT scans obtained intraoperatively and postoperatively. Rotation angle (RAsag) was measured according to Aaro and Dahlborn. Results: We observed reduction of RAsag in all patients, however in DVR group decrease was greater, by 31.8% comparing to non-DVR group, by 8.6% (p=0.0003). Mean coronal correction in DVR group was 68.8% and in rod derotation group without DVR 55% (p=0.002). No significant correlation was found between degree of derotation obtained and coronal correction. In DVR group T2-T12 kyphosis has increased in 28 (65%) patients whereas in non DVR group in 31 (69%) cases. Mean value of T2-T12 kyphosis growth was 16.7% in DVR and 22.1% in non DVR group. These differences however did not occur statistically significant. Conclusions: Direct vertebral rotation (DVR) manoeuvre reduces significantly apical rotation of the spine, enhances ability of coronal correction and it does not reduce thoracic kyphosis.

Background: Epidemiologic data revealed an increasing incidence of papillary carcinomas (PTCs) in the German population. There is some evidence that the size of resected PTCs has decreased during the last few years. Purpose: The aim of the present study was to test the hypothesis that the sonographic characteristics of PTCs vary with size. Material/Methods: Consecutive PTCs were histologically confirmed in 41 patients. Ultrasound examinations of these 50 PTCs were reevaluated retrospectively and classified according to five sonographic criteria. Results: It could be shown that the sonographic shape (p < 0.001), the contour (p = 0.024), the structure (p = 0.04), the echogenicity (p < 0.001) and calcifications (p = 0.008) varied with the size of the neoplasms. By factoranalysis a factor FPTC with an eigenvalue of 1.89 could be extracted from the data on which the sonographic structure, the ontour and the presence of calcifications had the strongest impact with factorloadings of 0.74, 0.68 and 0.61, respectively. Conclusion: The knowledge of the variation of the ultrasound characteristics should be of assistance for the sonographic classification of PTCs. In this context microcarcinomas usually do not display cystic components. However, they may show microcalcifications. Cystic components, hyperechogenicity and the taller-than-wide sign are predominantly seen in larger PTCs.

This study evaluated the damage to the extensive range of white matter tracts in patients with Alzheimer's disease (AD) and mild cognitive impairment (MCI). Thirty-four patients with AD (mean age 71.5 yrs, MMSE 17.6), 23 patients with MCI (mean age 66 yrs, MMSE 27.4) and 15 normal controls (mean age 69 yrs, MMSE 29.8) were enrolled. Diffusion tensor imaging (DTI) was performed in 25 directions on 1.5 T MR scanner. Fractional anisotropy (FA) values were obtained with a small ROI method in several association tracts including posterior cingulum fibers, in commissural tracts (genu and splenium of corpus callosum) and projection tracts (middle cerebellar peduncles and posterior limbs of internal capsules). In MCI significant reductions of FA were found in the inferior longitudinal fascicles, left superior longitudinal fascicle and posterior cingulum fibers compared to normal controls. In AD significantly decreased FA values were detected in the same fascicles as in MCI and additionally in inferior fronto-occipital tracts and commissural tracts. In both AD and MCI the most severe changes were found within posterior cingulum fibers. No abnormalities were detected in projection tracts in both groups. Accuracy of DTI in detecting AD and MCI reached 0.95 and 0.79, respectively. FA measurements strongly correlated with neuropsychological tests. DTI is capable of depicting microstructural changes within white matter fiber tracts in dementia and may aid the differential diagnosis of AD and MCI.

BACKGROUND The proper diagnosis and management of patients after surgery for pituitary tumors are of great importance in clinical practice. The purpose of this study was to investigate the magnetic resonance features of the postoperative sella with fast spin echo T2-weighted imaging and to evaluate the benefits of this sequence compared to the classically performed contrast-enhanced T1-weighted imaging at 1.5T unit. MATERIAL/METHODS The study group consisted of 101 patients who underwent resection of pituitary tumors. There were 58 women (57.4%), aged 22 to 75 (mean age, 52.67 years) and 43 men (42.6%), aged 21 to 79 (mean age, 49 years). In all patients preoperative and multiple postoperative MR studies were performed. Post-contrast T1 and pre-contrast T2 images were interpreted by 2 independent readers (neuroradiologists). The aim was to develop a morphometric and anatomically accurate atlas (statistical shape model) of calcaneus. The model is based on 18 left foot and 18 right foot computed tomography studies of 28 male individuals aged from 17 to 62 years, with no known foot pathology. A procedure for automatic atlas included extraction and identification of common features, averaging feature position, obtaining mean geometry, mathematical shape description and variability analysis. Expert manual assistance was included for the model to fulfil the accuracy sought by medical professionals. The proposed for the first time statistical shape model of the calcaneus could be of value in many orthopaedic applications including providing support in diagnosing pathological lesions, pre-operative planning, classification and treatment of calcaneus fractures as well as for the development of future implant procedures.

Arterio-venous malformations (AVMs) and dural arterio-venous fistulas (AVFs) are relatively rare developmental vascular system disorders and constitute the majority of vascular malformations of the nervous system. The malformations are characterized by a large variety of vascular architecture. Intracranial or intramedullary hemorrhage constitute the most serious complications of the malformations. The algorithm of management aiming at complete recovery or hemorrhage prevention has not been completely established yet due to considerable individual variability of malformations. The obliteration of malformations pathological vessels using cyanoacrylate glue or ethylene vinyl alcohol copolymer (EVOH)--so called endovascular embolization--has been the most dynamically developing treatment method recently. The procedure, unlike classical surgical resection of pathological vessels, is significantly less invasive and associated with shorter hospitalization period. The features and properties of embolization agents (cyanoacrylate and EVOH) as well as application of the agents for treatment of the vascular pathologies of the central nervous system are discussed in the paper. The procedure of endovascular introduction of embolization agents into the lumen of malformation vessels is also presented in the article. The analyzed literature and own experiences allow to claim that the application of cyanoacrylate and EVOH is a relatively safe therapeutic method that in part of the cases enables complete embolization of pathological vessels. The development of endovascular systems and the advances in embolization agents should trigger further intensive improvement of the technique.

During recent years a number of medical diagnosis support systems have been presented. Such systems are important from medical point of view, because they aid physicians in their daily work. Some of those systems, like Computed tomography support systems (CT) rely on image data analysis, making it an interesting research topic from pattern recognition point of view. The paper presents a practical realization of a medical support system, including segmentation, feature extraction and decision processes. Various techniques are used within the system: fuzzy logic, shape descriptors, classifiers and automatic image annotation framework. Series of 2D CT images are an input of the system. As a result, a patientpsilas diagnosis is presented.

The subject of the paper is the evaluation of a brain atrophy depending on computed tomography (CT) images. There are two computer aided methods under investigation. The first one is a semi-automatic volumetric method, the second is a fully automatic one and depends on the fractal dimension. Results of the experiment with 68 patients show that both methods are able to estimate the brain atrophy very similarly to the evaluation made by expert radiologist with the coefficient of correlation in the range of 0.7-0.9. The great advantage of the automatic method comparing to volumetric method is generating results in a very short time, that counts in ordinary clinical practice and could be used to experiment with huge patient groups.

We present a case of 32-year-old woman, who was admitted to the Department of Neurology in the emergency mode, due to: instantaneous pupillary dilation (mydriasis), ptosis of the left eyeball and double vision. We performed plain CT, panarteriography of cerebral vessels, CT angiography with RT3D (volume-rendered three-dimensional) reconstruction images. On the base of imaging studies the diagnosis of giant saccular aneurysm of the left posterior communicating artery was established. The patient was operated on and the giant aneurysm of left posterior communicating artery was clipped, confirming radiological diagnosis. During operation and postoperative period no complications were noted.

Aneurysms of the posterior cerebral artery (PCA) are uncommon (about 1% of all intracranial aneurysms) and their neurosurgical treatment is associated with high operative risk. We report on two saccular aneurysms of the P1 segment of the right PCA, combined with occlusion of the C1 segment of the internal carotid artery (ICA) detected during routine diagnostic studies in a 58-year-old patient with slight sensory loss in the left extremities. The aneurysms were embolized during two consecutive sessions, without subsequent complications. Endovascular intervention is an efficient method in the treatment of multiple aneurysms in the P1 segment of the PCA.

Approximately 10% of persons infected with human immunodeficiency virus (HIV) and 75-90% patients with acquired immunodeficiency syndrome (AIDS) present neurological symptoms. This group causes the greatest diagnostic difficulties among AIDS patients. The purpose of the study was to demonstrate the usefulness of the magnetic resonance imaging (MR) of the central nervous system (CNS) in the diagnosis and differentiation of primary and secondary lesions.Fifty one patients with the symptoms suggesting CNS involvement were enrolled into the study. In 40 of them AIDS diagnosis was established based on the picture of CNS lesions. MR studies were performed using EDGE-ECLIPSE 1.5 T unit with FSE sequences, T1- and T2-weighted images, before and after paramagnetic contrast agent injection, in sagittal, coronal and transverse planes. In almost all cases FLAIR technique was also used. Thirteen persons were examined several times. Altogether we executed 62 studies. Magnetic resonance results were compared with the results of laboratory tests and sometimes histopathological examination at autopsy.Pathological findings were visible on MR of CNS in 37 (92.5%) of 40 patients with AIDS. In 8 (20%) cases the diagnosis of AIDS dementia complex (ADC) was established. Toxoplasmosis was diagnosed in 9 (22.5%) cases, tuberculosis in 6 (15%), cryptococcosis in 5 (12.5%) patients, progressive multifocal leukoencephalopathy (PML) in 3 (7.5%) patients, primary lymphoma also in 3 (7.5%), and atypical mycobacteriosis in 2 patient. In 1 (2.5%) case we could not establish the final diagnosis. We described the patterns of MR appearance in the different types of lesions mentioned above based on our findings and the literature review.Magnetic resonance imaging has a high sensitivity in identifying lesions of the central nervous system. It is very useful, together with the results of other tests, in the final diagnosis of the lesions in the CNS. Magnetic resonance plays also an important role in the treatment planning and monitoring of the efficacy of therapy used in AIDS patients.

The purpose of the study was to evaluate the usefulness of perfusion CT (pCT) in differentiating Alzheimer's disease (AD) from vascular dementia (VaD) and mixed dementia (MixD). pCT was performed in 41 patients (mean age, 68.3 years): 24 with AD, 8 with VaD, and 9 with MixD. Regional perfusion parameters (rCBF, rCBV, and rMTT) were calculated from 31 ROIs in the grey and white matter of the frontal and temporal lobes, basal ganglia, and internal capsules bilaterally. The obtained data for the subgroups of AD, VaD, and MixD patients were compared statistically. Conclusions On the basis of rCBF and rCBV values, pCT may be a valuable method of distinguishing between AD and VaD but it seems to be of little significance in differentiating MixD from VaD and of no usefulness in distinguishing between AD and MixD.

The aim of the study was to assess the relationship between cognitive impairment according to the Mini Mental State Examination (MMSE) and values of cerebral blood flow (CBF), cerebral blood volume (CBV), and mean transit time (MTT) obtained in perfusion CT (pCT).Sixty-four patients with dementia (36 with Alzheimer's disease, 15 with vascular dementia, and 13 with mixed dementia), aged 45-92 years (mean: 69.9 years) with different degrees of cognitive impairment (mean MMSE score: 17.0) were involved in the study. All subjects underwent pCT at the level of the basal ganglia (50 scans, 1 scan/s, 40 ml contrast medium at 4 ml/s, delay: 7 s). CBF, CBV, and MTT values from 31 regions of interest (ROIs) in the gray and white matter of the frontal and temporal lobes, lentiform nuclei, and internal capsules were correlated with the MMSE scores.Statistical analysis of CBF and MMSE revealed significant correlations in 22/31 ROIs including the gray and white matter of the frontal and temporal lobes, both internal capsules, right and left hemispheres, and total gray and white matter. Analysis of CBV and MMSE showed significant correlations in 21/31 ROIs in the gray and white matter of the frontal and temporal lobes, both internal capsules, left lentiform nucleus, right and left hemispheres, as well as total gray matter. There were no significant correlations between MTT and MMSE.CBF and CBV calculated with pCT correlate with cognitive impairment in patients with dementia and thus may play a role in monitoring disease progression or therapeutic response.

Background The aim of the study was to search for correlations between the results of psychological tests and FDG-PET as well as spectroscopic metabolites from the posterior cingulate gyrus (PCG) in patients with amnestic (aMCI) and non-amnestic MCI (naMCI). Material and Methods The study group consisted of 35 aMCI, 16 naMCI and 20 control subjects matched for age, sex and years of education. All subjects underwent a wide range of psychological tests including MMSE, CDR, TYM, DEMTEC, CDR, FAS, IADL and GDS as well neuroimaging brain studies including FDG-PET with the evaluation of glucose metabolism and magnetic resonance spectroscopy (MRS) with the evaluation of several metabolites such as NAA (neuronal marker), choline (marker of myelin turnover) and myoinositol (marker of gliosis). The results of FDG-PET and the levels of MRS metabolites obtained from PCG were compared between the subject groups and then correlated with the scores of psychological tests using Pearson’s coefficient. Results Compared to CG, both aMCI and naMCI subjects showed significantly lower levels of glucose metabolism and NAA within PCG. In the aMCI subgroup the results of FDG-PET studies revealed significant negative correlations with the scores of TYM (p=0.17, r= -0.48) while in the naMCI subgroup lower levels of NAA correlated with lower scores in DEMTEC. Conclusions In both aMCI and naMCI metabolic changes within PCG such as glucose hipometabolism and decreased neuronal activity correlate with the results of single psychological test.

CT perfusion is a new diagnostic method which enables imaging of the hemodynamics of tissues and organs. Until now most of the publications have been focused on the use of this technique to assess acute stroke. The aim of the presented work is to show the role of CT perfusion in the diagnosis of cerebral gliomas. These tumors are characterized by intensive vascularity which results in an increased blood volume (CBV), blood flow (CBF) and permeability surface (PS) values in perfusion CT examination. A correlation has been reported between perfusion parameters and the histopathological tumor grade. CT perfusion provides morphological and functional information which is useful in the diagnosis, planning treatment strategies and monitoring of the therapeutic effect in cerebral gliomas.

Vascular malformations of the spinal canal are rare and their clinical appearance is not specific. The authors demonstrate material of 6 patients, who have been diagnosed with magnetic resonance imaging (MR). MR studies in all cases showed tortuous foci of very low signal, compatible with pathological vessels. In 2 patients malformations were also visible on MR angiography and confirmed with conventional angiography. Besides, in 2 cases features of secondary spinal cord oedema (thickening and hyper-intensity on T2-weighted images) have been shown. One patient had spinal cord haematoma which was not visible on MR and has been diagnosed with computed tomography (CT). In the authors' opinion MR is an efficient method of diagnosing and evaluating spinal canal vascular malformations and should be the first diagnostic study in these cases.

Despite high efficiency of magnetic resonance (MR) imaging in degenerative disease of lumbar spine (DDLS), in many cases its appearance is ambiguous or not compatible with clinical findings. Therefore, different new techniques have been proposed to improve the quality of MR images. One of them is CBASS (Completely Balanced Steady State) sequence. The aim of our study was the evaluation of CBASS sequence in DDLS based on analysis of 78 consecutive cases.The material consists of 78 consecutive patients referred to MR study of the lumbar spine due to low back pain and radicular symptoms. MR examinations, including CBASS sequence, were performed with open low field (0.23 Tesla)MR unit. In all patients, degenerative findings in CBASS and FSE T2-weighted images were scored using 0-3 point scale, and analyzed statistically.The superiority of CBASS to FSE T2-weighted images was found in evaluation of disc herniation and bulging disc outlines, the relationship of disc herniation to the dural sac and nerve roots, visualization of the osteophytes, facet joint degeneration and thick- ening of the ligamenta flava. CBASS was inferior in the assessment of the degenerated disc signal and type 1 degenerative changes of the vertebral bodies.CBASS is superior to T2-weighted images in the evaluation of many symptoms. Therefore, it should be included in routine diagnostic protocol in low-field MR units. In our opinion, CBASS supplements T1-and T2-weighted images in sagittal planes and can entirely replace T2-weighted transverse images.

Magnetic resonance (MR) has become the most important imaging method in the diagnosis of intracranial tumours. However, conventional MR protocol often fails to obtain all morphological details which are necessary for precise planning of the treatment. Therefore, there are attempts of introducing other MR techniques, to gain new information concerning tumour morphology. One such technique is FLAIR sequence. In previous papers we presented the value of plain FLAIR in patients with intracranial tumours. The aim of the present study was evaluation of contrast enhanced FLAIR (CE FLAIR) in assessment of intracranial tumours.MR examinations, including conventional T1 (pre-and postcontrast) and T2 images, as well as plain and contrast-enhanced FLAIR sequences were performed in 27 patients with intracranial tumours. In all patients the degree and localization of contrast enhancement in post-contrast T1-weighted sequence (CE T1w) and CE FLAIR images have been compared.Contrast enhancement of the tumour in both CE T1-w and CE FLAIR sequences was found in 18 patients. In 5 patients the enhancement was visible only on T1-weighted images, while in 2 cases only on FLAIR images. Among 18 patients, who demonstrated enhancement both in T1-weighted and FLAIR sequences, in 9 cases the effect of enhancement was better on T1-weighted images, in 7-CE FLAIR gave superior results, and in 2 patients - both methods were equivalent. In the whole material in 9 cases CE-FLAIR was superior in 14-inferior and in 4-equal to CE-T1-w. Among 9 cases in which CE- FLAIR was superior, in 7 patients the enhancement concerned the border of the tumour or postoperative cave.Contrast-enhanced FLAIR has limited value in assessment of intracranial tumours and should be used as a supplementary technique if there are difficulties in evaluation of tumour morphology in other sequences.

CT-angiography is a non-invasive method, alternative to cerebral angiography in the diagnosis of intracranial aneurysms. The aim of the study was the assessment of the efficiency of CT-angiography in a large material, which has been mostly verified with angiography and/or surgery.CT-angiography was performed in 196 patients with intracranial bleeding. Seventy-three (73) patients underwent also cerebral angiography (CA), 121 were operated on, 74 of them solely on the basis of CT-angiography. A spiral CT scanning (2 mm slices, 1-1.5 pitch, 1 mm-gap reconstruction) combined with an injection of 120 mL of contrast medium (5 mL/sec) was used. Subsequently three-dimensional MIP reconstructions were received, supplemented in some cases with SSD, VRT and MPR.CT-angiography revealed 128 aneurysms in 106 patients, located mostly in anterior communicating, middle cerebral and internal carotid arteries. The size of most aneurysms was 6-10 mm, but nearly one-third of them did not exceed 5 mm. In the group of patients who underwent CA, there was only 1 false negative CT-angiography result, while in 7 patients CT-angiography allowed for the diagnosis of small aneurysms which were not clearly visible on CA. CT-angiography was superior to CA in assessment of aneurysm morphology by 3 independent radiologists in 17 out of 22 cases. CT-angiography findings were confirmed during surgery in all 74 patients.CT-angiography is highly effective in the diagnosis of intracranial aneurysms and the evaluation of their morphology. In our opinion, in most cases, CT-angiography is precise enough to plan a surgical or endovascular procedure.

Acute thrombosis within the lumen of the intracranial artery may occasionally be demonstrated on precontrast CT. The hyperdensity follows the course of the occluded segment of the vessel and an area of low attenuation in the territory of the involved artery is usually present, consistent with infarction. Usually, a single vessel is involved; occlusion of multiple branches occurs less commonly. This report demonstrates a case of diffuse bilateral thrombosis of the main arteries of the anterior circle of Willis.

We present six cases of extra-axial lesions: three meningiomas [including one intraventricular and one cerebellopontine angle (CPA) meningioma], one dural metastasis, one CPA schwannoma and one choroid plexus papilloma which were chosen from a larger cohort of extra-axial tumors evaluated in our institution. Apart from conventional MR examinations, all the patients also underwent perfusion-weighted imaging (PWI) using dynamic susceptibility contrast method on a 1.5 T MR unit (contrast: 0.3 mmol/kg, rate 5 ml/s). Though the presented tumors showed very similar appearance on conventional MR images, they differed significantly in perfusion examinations. The article draws special attention to the usefulness of PWI in the differentiation of various extra-axial tumors and its contribution in reaching final correct diagnoses. Finding a dural lesion with low perfusion parameters strongly argues against the diagnosis of meningioma and should raise a suspicion of a dural metastasis. In cases of CPA tumors, a lesion with low relative cerebral blood volume values should be suspected to be schwannoma, allowing exclusion of meningioma to be made. In intraventricular tumors arising from choroid plexus, low perfusion parameters can exclude a diagnosis of meningioma. In our opinion, PWI as an easy and quick to perform functional technique should be incorporated into the MR protocol of all intracranial tumors including extra-axial neoplasms.

Background The aim was to develop statistical shape models of the main human tarsal bones that would result in novel representations of cuboid, navicular and talus. Methods Fifteen right and 15 left retrospectively collected computed tomography data sets from male individuals, aged from 17 to 63 years, with no known foot pathology were collected. Data were gathered from 30 different subjects. A process of model building includes image segmentation, unifying feature position, mathematical shape description and obtaining statistical shape geometry. Results Orthogonal decomposition of bone shapes utilising spherical harmonics was employed providing means for unique parametric representation of each bone. Cross-validated classification results based on parametric spherical harmonics representation showed high sensitivity and high specificity greater than 0.98 for all considered bones. Conclusions The statistical shape models of cuboid, navicular and talus created in this work correspond to anatomically accurate atlases that have not been previously considered. The study indicates high clinical potential of statistical shape modelling in the characterisation of tarsal bones. Those novel models can be applied in medical image analysis, orthopaedics and biomechanics in order to provide support for preoperative planning, better diagnosis or implant design.
