Introduccion : La lesion de la vesicula biliar secundaria a trauma abdominal cerrado constituye un evento infrecuente de perforacion traumatica de ella, de presentacion tardia; revisamos la literatura cientifica actualmente disponible y ademas describimos un caso. Materiales y Metodo : Utilizando la plataforma PubMed se buscan las siguientes palabras clave: “ Blunt abdominal trauma ”. Se seleccionan las series con lesiones de la vesicula biliar: “ Traumatic gallbladder rupture”. Se seleccionan los reportes de lesiones aisladas de la vesicula biliar: “ Isolated gallbladder rupture ”. Se seleccionan los reportes de presentacion tardia de lesiones aisladas de la vesicula biliar: “ Delayed presentation of isolated gallbladder rupture ”. Resultados: De todas estas publicaciones se seleccionan las que a criterio de los autores son relevantes para el presente caso. Discusion : La mayoria de las perforaciones de la vesicula biliar se producen en vesiculas sanas de paredes delgadas distendidas por el ayuno o el consumo de alcohol. No existe una presentacion clinica clasica. Los estudios imagenologicos son inespecificos y se llega al diagnostico definitivo durante la exploracion quirurgica. El tratamiento de esta lesion es la colecistectomia. Conclusion : El diagnostico no es facil pero la resolucion es relativamente simple y el pronostico es bueno. El presente caso ilustra este tipo de lesiones en pacientes con trauma abdominal cerrado

Objective To report emerging data on the use of highly active antiretroviral therapy (HAART) in Argentina by assessing patterns of HAART access and late vs early treatment initiation in a population-based cohort of adults infected with HIV type-1. Design The Prospective Study on the Use and Monitoring of Antiretroviral Therapy (PUMA) is a study of 883 HIV-positive individuals enrolled in the Argentinean drug treatment program. Individuals were 16 years of age and older and were recruited from 10 clinics across Argentina. Methods Sociodemographic and clinical characteristics were examined using contingency tables (Pearson chi-square test and Fisher exact test) for categoric variables and Wilcoxon rank-sum test for continuous variables. To analyze time to initiation of HAART we used Kaplan-Meier methods and Cox regression. Results Patients who initiated HAART were more likely to be older, have an AIDS-defining illness, be an injection drug user (IDU), have a lower median CD4 cell count, have a higher median viral load, and be less likely to be men who have sex with men (MSM). In multivariate analysis, AIDS-defining illness and plasma viral load were significantly associated with time to starting therapy. Patients who received late access were more likely to be diagnosed with AIDS and have higher median plasma viral loads than those receiving early access. Conclusion Our results indicate that despite free availability of treatment, monitoring, and care in Argentina, a significant proportion of men and women are accessing HAART late in the course of HIV disease. Further characterization of the HIV-positive population will allow for a more comprehensive evaluation of the impact of HAART within the Argentinean drug treatment program.

Heartburn and regurgitation are considered highly specific symptoms of gastroesophageal reflux. A considerable number of patients with these symptoms do not have endoscopic signs of esophagitis.To study the relationship between gastroesophageal reflux symptoms and 24 h esophageal pH mesurement in patients with normal or near normal endoscopic findings.One hundred eighty six patients with persistent reflux symptoms and absence of severe endoscopic esophagitis were studied. Pathological studies of esophageal biopsies, manometry and 24 h esophageal pH measurements were performed in all.Abnormal acid reflux was found in 131 patients (70%). No differences in the frequency of symptoms, gender or pathologic findings were observed between patients with or without abnormal acid reflux. However, a higher frequency of esophageal erosions and a lower resting pressure of the inferior sphincter of the esophagus was observed in patients with abnormal acid reflux.Thirty percent of patients with heartburn and regurgtitation did not have abnormal acid reflux. Therefore, these symptoms are not specific for gastroesophageal reflux.

6. Respuesta en el corto plazo de algunas propiedades físicas a la introducción de cultivos de cobertura

Para evitar la degradación física del suelo se necesitan estrategias de gestión sostenibles. En este sentido, la intensificación en la secuencia de cultivos genera un uso más eficiente de los recursos ambientales, aumentando el aporte de carbono al suelo y mejorando su condición física. El objetivo del presente trabajo fue estudiar en el corto plazo (un año), la incidencia de dos cultivos de cobertura (CC) con características contrastantes (CC Trigo y CC Vicia) y la de una situación bajo barbecho invernal (Barbecho), sobre distintas propiedades físicas del suelo, analizando además su relación con el rendimiento de dos cultivos de verano (maíz y soja). Para cumplir con dichos objetivos, se evaluó la respuesta de un Argiudol bajo siembra directa (SD), ubicado en el partido de San Antonio de Areco (Pcia. de Bs. As.). Se determinó la lámina de agua disponible de los primeros 60 cm de suelo en distintos momentos del ensayo. A la cosecha de los cultivos de verano se determinaron a dos profundidades las siguientes propiedades: densidad aparente, resistencia mecánica a la penetración, estabilidad de agregados, conductividad hidráulica saturada y porosidad estructural, como así también los rendimientos de soja y maíz. Las parcelas ocupadas durante el invierno con CC, no presentaron al momento de la siembra, floración y cosecha de los cultivos de verano, menor contenido hídrico en los primeros 60 cm de suelo respecto al Barbecho. Se comprobó en el corto plazo y sobretodo en el estrato superficial, el efecto favorable de los CC sobre la mayoría de las propiedades físicas evaluadas. En algunos casos se generaron respuestas distintas del suelo, según el CC presente. La estabilidad de agregados de los primeros centímetros de suelo a la cosecha del maíz, fue la variable más sensible para detectar los cambios sufridos, mientras que la resistencia mecánica a la penetración fue la menos eficaz.
