Abstract

Background: Reducing the burden of death due to infection is an urgent global public health priority. Previous studies have estimated the number of deaths associated with drug-resistant infections and sepsis and found that infections remain a leading cause of death globally. Understanding the global burden of common bacterial pathogens (both susceptible and resistant to antimicrobials) is essential to identify the greatest threats to public health. To our knowledge, this is the first study to present global comprehensive estimates of deaths associated with 33 bacterial pathogens across 11 major infectious syndromes. Methods: We estimated deaths associated with 33 bacterial genera or species across 11 infectious syndromes in 2019 using methods from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, in addition to a subset of the input data described in the Global Burden of Antimicrobial Resistance 2019 study. This study included 343 million individual records or isolates covering 11 361 study-location-years. We used three modelling steps to estimate the number of deaths associated with each pathogen: deaths in which infection had a role, the fraction of deaths due to infection that are attributable to a given infectious syndrome, and the fraction of deaths due to an infectious syndrome that are attributable to a given pathogen. Estimates were produced for all ages and for males and females across 204 countries and territories in 2019. 95% uncertainty intervals (UIs) were calculated for final estimates of deaths and infections associated with the 33 bacterial pathogens following standard GBD methods by taking the 2·5th and 97·5th percentiles across 1000 posterior draws for each quantity of interest. Findings: From an estimated 13·7 million (95% UI 10·9–17·1) infection-related deaths in 2019, there were 7·7 million deaths (5·7–10·2) associated with the 33 bacterial pathogens (both resistant and susceptible to antimicrobials) across the 11 infectious syndromes estimated in this study. We estimated deaths associated with the 33 bacterial pathogens to comprise 13·6% (10·2–18·1) of all global deaths and 56·2% (52·1–60·1) of all sepsis-related deaths in 2019. Five leading pathogens—Staphylococcus aureus, Escherichia coli, Streptococcus pneumoniae, Klebsiella pneumoniae, and Pseudomonas aeruginosa—were responsible for 54·9% (52·9–56·9) of deaths among the investigated bacteria. The deadliest infectious syndromes and pathogens varied by location and age. The age-standardised mortality rate associated with these bacterial pathogens was highest in the sub-Saharan Africa super-region, with 230 deaths (185–285) per 100 000 population, and lowest in the high-income super-region, with 52·2 deaths (37·4–71·5) per 100 000 population. S aureus was the leading bacterial cause of death in 135 countries and was also associated with the most deaths in individuals older than 15 years, globally. Among children younger than 5 years, S pneumoniae was the pathogen associated with the most deaths. In 2019, more than 6 million deaths occurred as a result of three bacterial infectious syndromes, with lower respiratory infections and bloodstream infections each causing more than 2 million deaths and peritoneal and intra-abdominal infections causing more than 1 million deaths. Interpretation: The 33 bacterial pathogens that we investigated in this study are a substantial source of health loss globally, with considerable variation in their distribution across infectious syndromes and locations. Compared with GBD Level 3 underlying causes of death, deaths associated with these bacteria would rank as the second leading cause of death globally in 2019; hence, they should be considered an urgent priority for intervention within the global health community. Strategies to address the burden of bacterial infections include infection prevention, optimised use of antibiotics, improved capacity for microbiological analysis, vaccine development, and improved and more pervasive use of available vaccines. These estimates can be used to help set priorities for vaccine need, demand, and development. 11. THE EFFECT OF USING AUTHENTIC AUDIO AND VIDEO MATERIALS ON THE SPEAKING FLUENCY OF AUDITORY AND VISUAL IRANIAN LEARNERS

Abstract

When we talk about learning a second language, we internationally think of conversing with speakers of other languages. This means that speaking is seen as the prime purpose of language learning. This study was undertaken to investigate the effect of authentic auditory and visual materials on speaking fluency of Iranian EFL learners with auditory and visual styles of learning. The quasi-experimental design consisted of 60 Iranian EFL language learners divided into two groups of visual style and auditory style. A questionnaire on perceptual learning styles and a speaking fluency test were used to collect the data. Students with visual style received authentic visual materials and students with auditory style received authentic audio materials. The obtained data was analyzed by employing paired and independent samples t-test. The results of the study indicated that both groups of students significantly progressed in speaking fluency suggesting the positive effect of authentic visual materials on students with visual preferences and the positive effect of authentic audio materials on students with audio preferences. Moreover, both types of materials had equal effects on the participants of the study. The findings suggest that providing instruction that is congruent with students’ learning styles and the use of authentic materials appear to improve speaking fluency.

Abstract

Background: Staphylococcus aureus is one of the most frequently isolated microorganisms from burn wounds. Antimicrobial photodynamic therapy (aPDT) is a new strategy that may improve antimicrobial treatment. Method: This study evaluated three meticillin-resistant Staphylococcus aureus (MRSA) and three meticillin-sensitive Staphylococcus aureus (MSSA) clinical isolates, which produced a biofilm with 0.1mg/ml Toluidine Blue O (TBO) (Sigma-Aldrich, Germany) with an energy density of 45J/cm2 and 90J/cm2, for MRSA and MSSA, respectively. The antibiofilm potential of aPDT with TBO was analysed using crystal violet assays and scanning electron microscopy. Results: TBO-aPDT significantly degraded the biofilm formed by MRSA and MSSA clinical isolates (pConclusion: Our results indicated that aPDT is an effective approach to combat bacterial biofilms associated with burn wound infection. aPDT could provide a supplemental to the treatment of wound and tissue infection, and patients with burns may benefit from combined treatments.

Abstract

The complete mitochondrial genome (mitogenome) of the soft coral Sinularia acuta Manuputty and van Ofwegen, 2007 was sequenced and annotated using Illumina next-generation sequencing (NGS). The mitogenome of S. acuta was 18,730 bp in length and consisted of 14 protein-coding genes (PCGs), two ribosomal RNA genes (rRNA), and only one transfer RNA gene (tRNA-Met). The base composition was 30.18% A, 16.46% C, 19.35% G, and 34.00% T, with a total A + T content of 64.19%. The phylogenetic analysis demonstrated a close evolutionary relationship among Sinularia acuta, Sinularia penghuensis, and Sinularia maxima.

31. Total mechanical energy budget in open-channel transitions with turbulent flows

Abstract

The issue of mechanical energy budget has received a lot of attention in open-channel flows which are frequently turbulent. Turbulent motions play a major role in the total mechanical energy of the flow field. It is difficult to evaluate total mechanical energy balance and energy loss for 3-D turbulent flows in irregular open channels due to the formation of vortices and the presence of secondary currents. In 3-D turbulent flows, the effect of these phenomena has often been considered as the empirical coefficient of energy loss. In this study, these details in the total mechanical energy budget are investigated through theoretical analysis and numerical simulation, a new form of the total mechanical energy equation and a relationship for the energy loss in 3-D turbulent flows are derived, and their applications are examined in open-channel transitions. For this purpose, the Navier-Stokes equations are solved numerically, and the results are used to obtain the various terms in the total mechanical energy balance equation and analyze the details. The relationships derived in this study generally support the concept of the total mechanical energy budget to comprehend the turbulence parameters affecting the mechanical energy balance in various open-channel flows.

33. The importance of surface tension in endodontic irrigation: A review study

Abstract

The surface tension of irrigants determines their ability to penetrate dentinal tubules and access the apical third of root canals. It is defined as an affinity for a liquid's surface area to decline in the case of surface tension between molecules. Irrigating the canal wall is vital for dissolving organic tissues and disinfecting dentin and its tubules in root canal preparation. Even with super elastic rotary preparation, untouched spaces are a typical issue during root canal instrumentation. As a result, the antimicrobial activity and solvent action in these areas is essential to the disinfection of root canals. The ability to diagnose irrigants quickly and treat them successfully depends on a thorough discernment of their surface tension. In this study, surface tension involved in endodontic solutions was examined for etiological factors, features, and possible consequences. The relevant studies were found by searching online databases. We identified 30 articles using the hand search method and summarized the critical factors of each study. It was found that low surface tension agents would penetrate tubules more acceptable and that surface tensions estimated at various temperatures and concentrations affected canal cleaning.

Abstract

Inspired by natural resources, such as peptides and carbohydrates, glycopolypeptide biopolymer has recently emerged as a new form of biopolymer being recruited in various biomedical applications. Glycopolypeptides with well-defined secondary structures and pendant glycosides on the polypeptide backbone have sparked lots of research interest and they have an innate ability to self-assemble in diverse structures. The nanostructures of glycopolypeptides have also opened up new perspectives in biomedical applications due to their stable three-dimensional structures, high drug loading efficiency, excellent biocompatibility, and biodegradability. Although the development of glycopolypeptide-based nanocarriers is well-studied, their clinical translation is still limited. The present review highlights the preparation and characterization strategies related to glycopolypeptides-based copolymers, followed by a comprehensive discussion on their biomedical applications with a specific focus on drug delivery by various stimuli-responsive (e.g., pH, redox, conduction, and sugar) nanostructures, as well as their beneficial usage in diagnosis and regenerative medicine.

37. MIL-101(Cr)-Functionalized TEDA-BAIL: An Efficient and Recyclable Catalyst for the Synthesis of Pyrimido[4,5-b]quinolinetrione Derivatives

Background: In this study, a heterogeneous catalyst containing MIL-101(Cr) functionalized TEDA-BAIL was used to carry out an efficient four-component reaction between aromatic aldehydes, barbituric acid, dimedone, and aryl amines, resulting in the synthesis of a new class of pyrimido[ 4,5-b]quinolinetrione derivatives. Methods: Pyrimido[4,5-b]quinolinetrione derivatives were synthesized through a one-pot fourcomponent reaction between aromatic aldehydes, barbituric acid, dimedone, and aryl amines, in the presence of triethylenediamine-based ionic liquid@MIL-101(Cr) composite as a catalyst under reflux conditions. The TEDA-BAIL@MIL-101(Cr), which is a recyclable catalyst, was fully characterized by Fourier transform infrared spectrophotometry (FT-IR), X-ray diffraction (XRD), scanning electron microscopy (SEM), energy-dispersive X-ray spectroscopy (EDX) thermogravimetric analysis (TGA), Brunauer-Emmett-Teller (BET) and Transmission electron microscopy (TEM). Results: Four-component synthesis of pyrimido[4,5-b]quinolinetriones was catalyzed by TEDABAIL@ MIL-101(Cr) with aromatic aldehydes, barbituric acid, dimedone, and aryl amines under reflux conditions. The obtained experimental results revealed that the employed synthesis approach is a simple method that offers several advantages, including sustainability, facile separation from the reaction medium, and reusability of the catalyst after six consecutive runs without loss of activity. Conclusion: The presented method is an efficient synthesis route for the synthesis of pyrimido[4,5- b]quinolinetriones in the presence of TEDA-BAIL@MIL-101(Cr) under reflux conditions. This procedure provides multiple advantages, such as ease of execution, high yield, clean reaction conditions, shorter reaction time, and catalyst sustainability.

Natural killer (NK) cells are among the most important innate immunity members, which are the first cells that fight against infected cells. The function of these cells is impaired in patients with COVID-19 and they are not able to prevent the spread of the disease or destroy the infected cells. Few studies have evaluated the effects of COVID-19 vaccines on NK cells, though it has been demonstrated that DNA vaccines and BNT162b2 can affect NK cell response. In the present paper, the effects of SARS-CoV-2 on the NK cells during infection, the effect of vaccination on NK cells, and the NK cell-based therapies were reviewed.

Systemic lupus erythematosus (SLE) is a debilitating multisystemic autoimmune disorder with a high prevalence in women of childbearing age; however, SLE incidence, prevalence, and severity are strongly influenced by ethnicity. Although the mystery of autoimmune diseases remains unsolved, disturbance in the proportion and function of B cell subsets has a major role in SLE's pathogenesis. Additionally, colocalizing hyperactive T helper cell subgroups within inflammatory niches are indispensable. Despite significant advances in standard treatments, nonspecific immunosuppression, the risk of serious infections, and resistance to conventional therapies in some cases have raised the urgent need for new treatment strategies. Without the need to suppress the immune system, mesenchymal stem cells (MSCs), as ''smart" immune modulators, are able to control cellular and humoral auto-aggression responses by participating in precursor cell development. In lupus, due to autologous MSCs disorder, the ability of allogenic engrafted MSCs in tissue regeneration and resetting immune homeostasis with the provision of a new immunocyte repertoire has been considered simultaneously. In Brief The bone marrow mesenchymal stem cells (BM-MSCs) lineage plays a critical role in maintaining the hematopoietic stem-cell microstructure and modulating immunocytes. The impairment of BM-MSCs and their niche partially contribute to the pathogenesis of SLE-like diseases. Allogenic MSC transplantation can reconstruct BM microstructure, possibly contributing to the recovery of immunocyte phenotype restoration of immune homeostasis. In terms of future prospects of MSCs, artificially gained by ex vivo isolation and culture adaptation, the wide variety of potential mediators and mechanisms might be linked to the promotion of the immunomodulatory function of MSCs.

Refereed/Peer-reviewed This paper determines the optimal capacities of small wind turbine (SWT) and battery energy storage (BES) for a grid-connected household (GCH) with or without an electric vehicle (EV) to minimize the overall cost of electricity (COE). Rule-based home energy management systems (HEMSs) are developed for two different configurations of the GCH: (i) with only SWT, and (ii) with SWT and BES. For each configuration, the HEMSs are developed for two cases: with and without an EV in the premises of the GCH. Uncertainties are also included in the arrival time, departure time, and initial state of charge (at arrival) of the EV. The above technique is then applied to a typical household in South Australia (SA) using the yearly load profile of the household and actual yearly wind speed data at an interval of one hour. To investigate the effects of stochastic nature of household load, EV, and wind power generation on various results, the optimization process is repeated using 10-year of actual wind speed data and probabilistic load and EV uncertainties. The results of several sensitivity analyses of various system parameters are presented. It has been found that the SWT can effectively decrease the COE of the household for both cases (with and without an EV). However, the current price of battery may not be in favor of further reducing the COE of the household.

Abstract

Background: Using the WHO STEPwise approach to NCD risk factor surveillance (STEPS), first round of Iran’s STEPS completed in 2005. It has been repeated six times afterward. Here we report the results of 2016 round on the population characteristics and prevalence of diabetes and prediabetes, along with an assessment of the country-level performance on diabetes care in Iran. Methods: Using a proportional-to-size cluster random sampling method, the STEPS 2016 included 18947 subjects aged≥25 years who matched the criteria (non-missing information on diabetes self-report, and biomarkers). For the analyses, survey design methods with weighted samples were employed. Different definitions of diabetes (biomarker-based, self-report, anti-diabetes medication use, or a combination) and prediabetes (different cutpoints of the biomarker) were calculated and presented. Results: An estimated 5171035 persons aged≥25 years or 10.6% (95% CI: 10.0%–11.1%) had diabetes according to the serologic diagnosis of diabetes (FPG≥126 mg/dL) or the use of at least one anti-diabetes medication (1896 out of 18947). Employing the serologic diagnosis of diabetes among those who responded no to the self-reported question, 2.7% (2.5%–3.0%) of the population were not aware of their diabetes compared to 11.5% (10.9%-12.0%) who were diabetics according to the just self-reported question. Defining prediabetes as 100≤FPG9%) was found in 18.4% (16.3%-20.6%) of the patients with self-reported diabetes. Conclusion: Since 2005, the prevalence of diabetes in Iran has been on a gradual increase in both genders with an increasing gap between females and males.

Abstract

Background The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic; characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic; and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting System (CRS) and the WHO Global Health Expenditure Database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings In 2019, at the onset of the COVID-19 pandemic, US$9 center dot 2 trillion (95% uncertainty interval [UI] 9 center dot 1-9 center dot 3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7 center dot 3 trillion (95% UI 7 center dot 2-7 center dot 4) in 2019; 293 center dot 7 times the $24 center dot 8 billion (95% UI 24 center dot 3-25 center dot 3) spent by low-income countries in 2019. That same year, $43 center dot 1 billion in development assistance was provided to maintain or improve health. The pandemic led to an unprecedented increase in development assistance targeted towards health; in 2020 and 2021, $1 center dot 8 billion in DAH contributions was provided towards pandemic preparedness in LMICs, and $37 center dot 8 billion was provided for the health-related COVID-19 response. Although the support for pandemic preparedness is 12 center dot 2% of the recommended target by the High-Level Independent Panel (HLIP), the support provided for the health -related COVID-19 response is 252 center dot 2% of the recommended target. Additionally, projected spending estimates suggest that between 2022 and 2026, governments in 17 (95% UI 11-21) of the 137 LMICs will observe an increase in national government health spending equivalent to an addition of 1% of GDP, as recommended by the HLIP. Interpretation There was an unprecedented scale-up in DAH in 2020 and 2021. We have a unique opportunity at this time to sustain funding for crucial global health functions, including pandemic preparedness. However, historical patterns of underfunding of pandemic preparedness suggest that deliberate effort must be made to ensure funding is maintained. Abstract

Lupus is known as a systemic immune-mediated disorder. Like other diseases in this category, its cause and definitive treatment remain unknown. Gold standard therapies, which mainly include immunosuppressive agents, have been able to have therapeutic effects on patients. However, a significant percentage of cases still do not respond to this kind of treatment, resulting in death from complications. Recently, a new source of non-hematopoietic cells, mesenchymal stem/stromal cells (MSCs), with the potency to re-establishment immune homeostasis and tissue regeneration, has been wildly used in both primary and clinical research. One of the remarkable features of MSCs is their anti-inflammatory and immunosuppressive properties and stimulating tissue differentiation programs. Under the influence of background signals, MSCs migrate to inflammatory bioactive substances and then regulate overactive immune responses to restore immune tolerance. MSCs have shown a two-way interaction with most immunocytes, which plays a significant role in resolving sterile inflammation. Restricting the entry of inflamed cells into the site of inflammation and re-educated infiltrated cells to achieve a tolerant phenotype have been reported as mechanisms of MSCs in tissue repair. Stimulation of the endogenous and tissue-dwelling stem cells in addition to releasing immunomodulatory agents, suggests MSCs transplantation as a potential modality in the treatment of future immune-mediated disorders.

Abstract

Scleromyxedema is a rare but important mucinosis disorder of the skin that is presented with dermatological manifestations such as waxy papules, diffuse induration, and nondermatologic involvements like neurological and renal disorders. We report a case series of the data regarding the characteristics and treatment of 14 patients diagnosed with scleromyxedema and their follow-up. Patients entered the study based on scleromyxedema diagnosis criteria. Comorbidities were also recorded to evaluate their effect on the treatment process. Clinicopathological and laboratory findings and responses to their treatment were evaluated separately. There was a significant improvement after administering intravenous immunoglobulin (IVIG). Despite the lack of a definite treatment for this condition, the present study shows that the application of IVIG can improve both cutaneous and systemic symptoms. Younger patients, in particular, responded significantly to the use of IVIG. More studies are required to investigate the potential efficacy of IVIG in the treatment of scleromyxedema.

Abstract

Refereed/Peer-reviewed This article proposes a two-stage optimization algorithm to effectively determine the system configuration at one stage, as well as the capacity of components at the other stage in the middle of the former. This algorithm fits in best with the hybrid systems with more possible types of components. The studied system, in this article, includes diesel generators, wind turbines, photovoltaic arrays, and tidal generators as the power generation components, as well as battery banks and flywheels as the energy storage components. It also includes fuel cells and electrolyzers that either work as batteries or generate electricity in the presence of biomass. When the number of components (decision variables) increases, it becomes difficult to find an optimal solution by the conventional methods. Therefore, in this study, a two-stage multiobjective optimization algorithm is applied to design a cost effective and environmental friendly ac minigrid hybrid power system. In each iteration of the proposed algorithm, first, renewable energy sources and energy storage components are selected to form a hybrid power system along with the diesel generator. Then, the capacities of the components are optimized based on the two objective functions, including levelized cost of electricity and emissions. The optimization model uses real annual data in hourly time intervals for the load, solar insolation, ambient temperature, and wind speed. It is found that the proposed algorithm decreases the susceptibility of the solutions to multiple runs compared with the conventional algorithms and achieves a better optimized power system design.
