3D virtual reality, including the current generation of multi-user virtual worlds, has had a long history of use in education and training, and it experienced a surge of renewed interest with the advent of Second Life in 2003. What followed shortly after were several years marked by considerable hype around the use of virtual worlds for teaching, learning and research in higher education. For the moment, uptake of the technology seems to have plateaued, with academics either maintaining the status quo and continuing to use virtual worlds as they have previously done or choosing to opt out altogether. This paper presents a brief review of the use of virtual worlds in the Australian and New Zealand higher education sector in the past and reports on its use in the sector at the present time, based on input from members of the Australian and New Zealand Virtual Worlds Working Group. It then adopts a forward-looking perspective amid the current climate of uncertainty, musing on future directions and offering suggestions for potential new applications in light of recent technological developments and innovations in the area.

Objective: The aim of this review was to scope the literature for publications on the practice of screening for depression and anxiety in acute coronary syndrome patients in acute care by identifying instruments for the screening of anxiety and/or depression; determining if screening for anxiety and/or depression has been integratedin to cardiac models of care and clinical pathways; and identifying any evidence practice gap in the screening and management of anxiety and/or depression in this population. Introduction: Depression in acute coronary syndrome is bi-directional. Depression is an independent risk factor for cardiovascular disease, and comorbid depression is associated with two-times greater risk of mortality in patients with cardiovascular disease. The presence of acute coronary syndrome increases the risk of depressive disorders or anxiety during the first one to two years following an acute event, and both depression and anxiety are associated with a higher risk of further acute coronary health concerns. Clinical practice guidelines have previously recommended routine screening for depression following a cardiac event; although many current guidelines do not include recommendations for screening in an acute setting. To date there have been no previous scoping reviews investigating depression and anxiety screening of patients with acute coronary syndrome in the acute care setting. Inclusion criteria: Adults (18 years and over) with acute coronary syndrome who are screened for anxiety and/or depression (not anxiety alone) in an acute care setting Methods: A systematic search of the literature was conducted by a research librarian. Research studies of any design published in English from January 1, 2012, to May 31, 2018, were included. Data were extracted from the included studies to address the three objectives. Purposefully designed tables were used to collate information and present findings. Data are also presented as figures, and by narrative synthesis. Results: Fifty-one articles met the inclusion criteria. Primary research studies were from 21 countries and included21,790 participants; clinical practice guidelines were from two countries. The most common instruments used for the screening of depression and anxiety were: 1) the Hospital Anxiety and Depression Scale (n ¼ 18); 2) the Beck Depression Inventory (n ¼ 16); and 3) the nine-item Patient Health Questionnaire (n ¼ 7). Eleven studies included screening for anxiety in 2181 participants (30% female) using the full version of the Hospital Anxiety and Depression Scale. The State-Trait Anxiety Inventory was used to screen 444 participants in three of the studies. Four studies applied an intervention for those found to have depression, including two randomized controlled trials with interventions targeting depression. Of the seven acute coronary syndrome international guidelines published since2012, three (43%) did not contain any recommendations for screening for depression and anxiety, although four(57%) had recommendations for treatment of comorbidities. Conclusions: This review has identified a lack of consistency in how depression and anxiety screening tools are integrated into cardiac models of care and clinical pathways. Background and Aim: This article provides an overview of a phenomenological investigation of spirituality with six people who have experienced mental illness. The study aimed to gain a greater understanding of the phenomenon of spirituality and its relationships to occupation and the experience of mental illness. Methods and Results: Participants were interviewed and the interviews were audiotaped and transcribed verbatim. Findings emerged through multiple readings and gradually constructed meanings from the data. Spirituality was found to be: unique to each participant, a journey through life, vitally important to participants, manifested as ‘spiritual occupations’, the foundation of meaningful doing, important socially as well as individually, and was found to support participants in coping with mental illness. This article focuses on the ‘meta’, integrative theme of spirituality as life sustaining. It concludes with the notion that spirituality is essential to the participants and is a significant aspect of their occupational behaviour. Conclusion: It is recommended that occupational therapists give themselves a ‘licence’ to discuss spirituality in practice and that they commit to learning more about how spirituality is a part of the lives of their clients.

The purpose of this study was to examine absconding behaviour (a patient leaving the hospital without permission) in acute and rehabilitation wards of one Australian psychiatric institution to describe the characteristics of the absconding patient and these events. Absconding is a significant issue in psychiatric inpatient settings, with risks that include patient harm, aggression and violence. In spite of this, limited research has been conducted in Australia on patients who abscond while receiving psychiatric care The study was a retrospective descriptive analysis. Absconding events from three acute and seven rehabilitation wards over a 12-month period were studied. The rate of absconding events by detained patients was 20·82%. Gender was not significantly associated with absconding, although 61·19% of those who absconded were men diagnosed with schizophrenic disorders. Over half of acute care patients who absconded left during their first 21-day detention order. More than half of absconding events were by patients that absconded more than once. There was limited support for the efficacy of locking ward doors. Age and diagnosis emerged as particularly important factors to consider. The study revealed that men are not more likely to abscond than women, that locking ward doors does not deter the determined absconders and that once a person has absconded, they are more likely to do so again. Younger patients and those with a schizophrenic disorder may be particularly likely to abscond. There also appears to be a link between continuing detention orders and an absconding event. Findings provide new data about the profile of absconding patients in Australia. Exploration of the reasons why patients abscond and why many do so repeatedly warrants further investigation. Risk management approaches taking into account factors associated with absconding could be trialled to reduce the incidence of absconding in psychiatric inpatient settings. Refereed/Peer-reviewed

The present qualitative study describes and discusses the perspectives and experiences of young homeless people with mental health problems in relation to their interactions with health and social care services. Working in partnership with Streetlink, a supported accommodation assistance programme in Adelaide, Australia, the authors interviewed 10 homeless young people, aged from 16 to 24 years of age, who had experienced mental health problems. In-depth interviews elicited accounts of the best and worst of the participants’ experiences of health and social care services. Access to services was not identified as being a significant problem in comparison with the participants’ concerns regarding the quality of the services encountered. The central findings stress the importance of a respectful and supportive climate in relation to the qualities of service provision that the young people identified as valuable for their continuing treatment or consultation.

Background Disturbed psychiatric inpatients are managed using a range of containment measures (e.g. seclusion, mechanical restraint) whose use differs by country. Little is known about why these differences exist, or about how staff choose between the different methods available to them. Aims To compare psychiatric professionals attitudes to containment measures between countries with different practices, and to discover what factors have the greatest impact on preparedness to use a containment method. Method Surveys of psychiatric professionals in the United Kingdom, the Netherlands, Finland, and Australia, using the Attitude to Containment Measures Questionnaire. Results Relative approval of different containment measures broadly matched what we know about different practices, with some notable differences. Staff in Finland expressed the highest level of approval of containment, staff in the UK the least, with those in the Netherlands in between. Individuals' preferences for different containment measures were largely determined by whether they considered it (i) safe for the patients undergoing it, (ii) prevented them from injuring others, and (iii) quickly calmed them. Conclusion Future evaluation research on containment measures should use time taken to calm the patient, injury to patients and others, as primary outcomes. National clinical audit and injury reporting systems would also enable the identification of methods that are truly physically injurious to patients, aiding in the rational selection of appropriate containment measures.

The aim of this qualitative research project was to investigate the phenomenon of spirituality within occupation as encountered by six people who have all experienced mental illness. A Heideggerian phenomenological philosophy was utilised as the theoretical background. Data were gathered through in-depth interviews with each participant, which were transcribed verbatim. Data were analysed using a hermeneutic, circular process. Eight themes arose from the investigation. Firstly, the experience of spirituality is diverse and each person has his or her idiosyncratic understanding of what spirituality is. Secondly, spirituality is experienced as a journey that changes across time. The third theme notes that spirituality is vitally important to the study participants, so much so, that participants??? spirituality saved them from suicide and filled their lives with meaning and purpose. The fourth theme is recognition that there are ???spiritual occupations??? that are designed to directly access one???s spirituality. Fifth is that spirituality is a form of being which provides the meaning to purposeful doing and that therefore spirituality can make even ???ordinary??? occupation spiritually meaningful. Theme six is that spirituality is not only important on an individual level, but that social and community dimensions of spirituality are also essential. Theme seven finds that mental illness provided the ???wake-up message??? that ???called??? each participant to a spiritual life. The final theme is that spirituality helps the participants cope with living with mental illness. The thesis concludes that spirituality is an indispensable part of each participant???s life. Spirituality has both irrevocably changed participants??? lives and allowed them find personal meaning in their lives that is life-sustaining and life-enhancing. The findings also reveal that spirituality can be related to occupation both theoretically and practically. Given that spirituality was found to be life-saving and life-sustaining and that it can be conceptualised as an essential dimension of occupation, it is argued that spirituality is an issue of high importance to occupational therapists. However, since some occupational therapists may not feel comfortable or knowledgeable enough to discuss spirituality in practice (Collins, Paul and West-Frasier 2001; McColl 2000) it is noted that this topic area forms an emergent frontier for occupational therapy. Spirituality has been recognized as an important part of nursing practice since its early beginnings. However, debate continues about whether and how nurses and other mental health professionals should include spirituality within their daily work. This paper aims to contribute to the discussion of spirituality within mental health nursing, through considering findings from a Heideggerian phenomenological study conducted with six people with mental illness living in regional Australia. This study aimed to provide a greater understanding of the phenomenon of spirituality by answering a primary research question, 'What does spirituality mean for people with a mental illness?' Participants were interviewed and data analysed using an iterative approach. Findings emerged through multiple readings and meanings were gradually constructed from the data into themes. The themes describe that spirituality is experienced uniquely for the participants, and that spirituality became vitally important to them when they became mentally unwell. In addition, issues of interest to mental health nurses were raised but not completely addressed by the study. The issues relate to potential interactions about spirituality between nurses and their patients. Although participants wanted to discuss their experiences of spirituality with others, they raised concerns about whether their mental health care providers would be accepting of their beliefs. Spirituality was deemed to be a highly individual phenomenon; it could be experienced as a journey and it was life-sustaining. For these reasons, it is proposed that mental health professionals must be prepared to discuss patients' spiritual needs in the context of their health concerns.

Youth homelessness is a growing Australian and international concern associated with considerable health disadvantage, including serious mental health problems. This paper reports findings of a qualitative study that explored young homeless people's experiences of mental health and well-being through in-depth interviews. Thematic analysis identified medication use and management as a central issue for the young people. Reasons identified for medication non-adherence included unwanted side-effects, issues of access and storage, and lack of support from health and social agencies. These problems were compounded by everyday stresses of homelessness. Medication adherence was facilitated by social support, consistent contact with supportive health services, and regular medication supply, often resulting in improved mental health and well-being. For these young people, prioritizing management of medication helped stabilize one aspect of their lives. Health professionals can play an important role in helping them achieving this goal.
