INTRODUÇÃO: a deficiência transversal da maxila é caracterizada pela atresia maxilar, palato ogival, apinhamento e rotação dos dentes, além da mordida cruzada posterior (uni ou bilateral). O tratamento dos pacientes com esta deficiência consiste no alargamento das bases ósseas maxilares. Nos pacientes com maturidade esquelética, a expansão da maxila é obtida através de procedimentos ortocirúrgicos (conhecidos como expansão de maxila cirurgicamente assistida - EMCA) que podem gerar efeitos sobre a cavidade e sobre o septo nasal. OBJETIVO: avaliar o comportamento do septo nasal frente aos procedimentos de EMCA. METODOLOGIA: foram avaliadas radiografias cefalométricas em norma póstero-anterior e radiografias oclusais totais de maxila, obtidas pela técnica convencional e posteriormente digitalizadas, de 16 pacientes submetidos à EMCA pela técnica de osteotomia Le Fort I subtotal. As radiografias foram obtidas no período pré-operatório (inicial) e pós-operatório mediato ou ao final da expansão e foram digitalizadas e mensuradas utilizando o programa DIGORA. Foram medidas, ainda, as distâncias entre os dentes (caninos, primeiro pré-molares, segundo pré-molares, primeiros molares e segundos molares). RESULTADOS: após a análise estatística pode-se constatar o aumento das distâncias interdentárias e intermaxilares, além do aumento da largura da porção basal da abertura piriforme e das distâncias entre a parede lateral da porção basal da abertura piriforme e o septo nasal. CONCLUSÃO: a EMCA é um procedimento eficaz, capaz de alargar a porção basal da abertura piriforme, que parece não influenciar no posicionamento do septo nasal. INTRODUCTION: Maxillary transversal deficiency is characterized by constricted maxillary arch, dental crowding and rotation, along with posterior uni or bilateral crossbite. The treatment of this deficiency is based on the widening of maxillary bone. In skeletal mature patients the maxillary expansion is achieved by means of surgical-orthodontic procedures (known as surgically assisted maxillary expansion - SAME) that can cause some effects on the nasal cavity and septum. AIM: To evaluate the behavior of the nasal septum after the procedures of SAME. METHODS: Post-anterior cephalometric and maxillary occlusal radiographs, taken by conventional technique and therefore digitalized, were analyzed from 16 patients undergone SAME. For all patients, the SAME technique consisted of subtotal Le Fort I osteotomy. The radiographs were taken before the surgical procedure, right after (mediate post-operatory time) or after the expansion, and thereafter digitalized and measured using DIGORA program. Also, the distances between teeth (canines, first premolars, second premolars, first molars and second molars) were registered. RESULTS: The statistical treatment showed an increase in interdental and intermaxillary distances, along with an increase in the width of the base of piriform aperture and in the distances between the base of lateral wall of the piriform aperture and nasal septum. CONCLUSION: SAME is an efficient procedure that seems not to influence the septum position.

Fibrous dysplasia is a benign fibro-osseous disease that affects one or more bones. Although its etiology has been defined, the mechanism of spontaneous resolution is still unclear. There is strong evidence indicating the occurrence of stabilization when bone maturation is completed. Deformities that lead to esthetic and functional disorders are observed in almost all cases. Plastic surgery is often recommended when the maxilla and mandible are involved. In the case of mild deformities, careful follow-up during skeletal growth is recommended. We describe here the 23-year follow-up of a patient with monostotic fibrous dysplasia whose disease had stabilized by 13 years of follow-up.

We have presented a case of eosinophilic granuloma of bone in the region of the angle of mandible which was diagnosed through biopsy and successfully treated with radiotherapy. There was no recurrence after 7 years.

O objetivo da presente pesquisa foi avaliar, comparativamente,as medidas lineares em radiografias panorâmicas obtidas em programas de imagem para planejamento pré-cirúrgico em implantes (Radioimp e Planimp), com as obtidas pelo método manual, utilizando um paquímetro digital. Outro objetivo foi analisar os dois métodos de digitalização utilizados: escaner e câmera digital A amostra constou de 50 radiografias panorâmicas de pacientes edêntulos, nas quais foram marcados pontos de referência para delimitar 14 distâncias anatômicas em cada radiografia. As radiografias foram digitalizadas por meio de um escaner HP Scanjet 4C/T (Hewllett Packard) e uma câmera digital FinePix S602 (Fujifilm). As mensurações lineares das distâncias anatômicas foram realizadas pelo método manual (paquímetro digital) e pelo método digital (programas Radioimp e Planimp). Os dados obtidos foram submetidos à análise estatística de variância, com nível de significância de 5%. Esta análise estatística mostrou que não houve diferença significante entre as medidas manuais e digitais. A partir dos resultados concluiu-se que os programas Radioimp e Planimp apresentaram desempenhos adequados para a calibração da imagem radiográfica, bem como para a mensuração de distâncias lineares. Os sistemas de digitalização de imagens utilizados (escaner e câmera digital) não apresentaram diferenças significativas, sendo adequados para uso com esses programas. The present study evaluated the linear measurements on panoramic radiographs obtained in image softwares for presurgical implant planning (Radioimp and Planimp) compared to the manual technique using a digital caliper. And evaluated the systems for image digitization employed: scanner and digital camera. The sample comprised 50 panoramic radiographs of edentulous patients, on which reference points were traced for delineation of 14 anatomical dimensions on each radiograph. The radiographs were digitized with a HP Scanjet 4C/T scanner (Hewllett Packard) and a digital camera FinePix S602 (Fujifilm). Linear measurements of the anatomical dimensions were achieved by the manual method (digital pachymeter) and digital method (Radioimp and Planimp softwares). The data achieved were submitted to statistical analysis of variance, at a significance level of 5%. This statistical analysis has not demonstrate any significant difference between the manual and digital measurements. The results allowed the conclusion that the Radioimp and Planimp softwares displayed proper performances for calibration of the radiographic image and for measurement of linear dimensions. The systems for image digitization employed (scanner and digital camera) has not present significant differences, being adequate for utilization with such softwares.

Uma avaliação sócio-odontológica de pessoas idosas foi realizada com o objetivo de conhecer as características de saúde bucal e geral da terceira idade, e de se elaborar uma ficha clínica específica para seu atendimento. A amostra foi constituída por trezentas pessoas com 50 anos e mais, vindas de três instituições, públicas e privadas, diferentes. Todos os participantes da pesquisa eram funcionalmente independentes ou parcialmente dependentes. O estudo foi constituído por uma entrevista direta e por um subseqüente exame físico da cavidade bucal. Na entrevista direta foram levantados : a história médica pregressa, o grau de satisfação em relação à saúde bucal e a ocorrência de queixas de problemas bucais, cujos resultados foram relacionados aos graus de escolaridade, às diferentes rendas mensais e à presença de ansiedade. No exame físico, foram avaliadas as alterações de mucosa bucal mais prevalecentes e as condições das próteses utilizadas pelos idosos. Os resultados obtidos proporcionaram as seguintes conclusões: 1- As alterações bucais mais prevalecentes na população estudada foram aquelas associadas ao uso de próteses, principalmente a candidíase crônica eritematosa de palato, especialmente nas mulheres; as alterações da língua, como a língua saburrosa e a língua varicosa; e a gengivite. 2- O grau de satisfação em relação à saúde bucal relatado pelos idosos foi superestimado pois não se mostrou condizente com a avaliação clínica das condições bucais e das próteses utilizadas. Odontalgias, problemas gengivais e a sensação de boca seca foram as principais queixas bucais acusadas. 3- A auto-percepção da saúde bucal e a ocorrência de queixas bucais não estavam relacionadas ao grau de escolaridade, à renda mensal ou à presença de dentes naturais. As queixas de problemas bucais tiveram associação estatisticamente significantes com os casos de queixa de ansiedade. 4- As doenças sistêmicas mais freqüentes nos idosos avaliados em Bauru foram; as doenças cardiovasculares, as doenças reumáticas, a ansiedade, a depressão e a diabetes mellitus. Os medicamentos rotineiramente mais utilizados pela população idosa avaliada foram : as medicações para o controle de desordens cardiovasculares, os antiinflamatórios (incluindo-se as fórmulas manipuladas), os psicotrópicos e os medicamentos hipoglicêmicos para o controle da diabetes mellitus . 5- O edentulismo apresentou uma prevalência de 45,3% nos idosos avaliados em Bauru. Houve uma associação estatisticamente significante do edentulismo com a procedência rural, com o analfabetismo e com rendimentos mensais entre 1 e 5 salários mínimos. 6- A maioria das próteses removíveis parciais e/ou totais apresentavam condições insatisfatórias de função, de aparência e de limpeza. 7- A literatura consultada e os resultados obtidos permitiram a idealização de uma ficha específica para o atendimento odontológico de pessoas idosas. A sociodental investigation was carried out with 300 elderly people, aging 50 years or older, living at home in Bauru. The aim of the present study was to acknowledge the general and dental health, as well as to elaborate a specific clinical file for the elderly. The study was composed of a direct interview and an oral cavity exam. The clinical investigation was based on an established criteria: their medical records, their satisfaction with specific aspects of oral health and their complaints. The results were related to different levels of income and education and the presence of anxiety. The oral exam checked the main alterations of the buccal mucosa and the elderly´s prosthesis conditions. The conclusions were: 1) The buccal alterations found were related to the use of prosthesis, mainly denture stomatitis, specially in women; tongue varicosis, saburral tongue and gingivitis. 2) The level of satisfaction related by the elderly was super estimated and was not compatible with the findings of the clinical examination. Toothache, gengival bleeding and dry mouth were the main buccal problems referred. 3) Anxiety had a statistically significant association with the buccal complaints. 4) The most frequent medical conditions were cardiovascular diseases, arthritis, anxiety, depression, and diabetes. The most frequently prescribed medications were cardiacs, anti-inflammatory, CNS agents, oral hypoglycemics agents. 5) The prevalence of endentulousness was 45.3% in the elderly population of Bauru. There was a statistically significant association of edentulousness with the elderly coming from the countryside, illiteracy, and income ranging from 1 to 5 minimal salaries. 6) The majority of the prosthesis did not show satisfactory functioning, appearance and cleaning conditions. 7) The bibliography reviewed and the results of the present study allowed the idealization of a specific file directed to the buccal exam of the elderly.

Uma pesquisa sobre o cisto ósseo traumático foi realizada com os seguintes objetivos: (1) Estabelecer a epidemiologia da amostra estudada, quanto ao gênero, raça, faixa etária, história clínica, localização e conformação da imagem radiográfica. (2) Discutir as possibilidades de cura espontânea da doença, sugerindo critérios para a proservação das lesões não tratadas cirurgicamente. (3) Estudar a prevalência do cisto ósseo traumático em pacientes tratados ortodonticamente e discutir sua possível relação etiopatogênica. O material constituiu-se de duas amostras. A amostra número 1, de procedência heterogênea, foi composta por 28 casos de cisto ósseo traumático, sendo 19 comprovados cirurgicamente, e nove casos compatíveis com a doença, pelos aspectos clínicos, radiográficos e de proservação. A amostra número 2 foi constituída por 8 casos encontrados entre 956 pacientes com tratamento ortodôntico concluído. Os resultados referentes à amostra número 1 permitiram verificar que: (1) não houve uma diferença significante na prevalência do cisto ósseo traumático em relação ao gênero; (2) o cisto ocorreu predominantemente na raça branca, porém houve uma relação entre lesões bilaterais e mulheres negras; (3) 22 casos apareceram em pacientes na 2a década de vida; (4) 69,23% dos pacientes apresentavam história de tratamento ortodôntico e 30,77% de trauma; (5) a localização principal da lesão foi o corpo mandibular com imagens radiográficas uniloculares e de forma oval; (6) a proservação dos casos compatíveis com o diagnóstico de cisto ósseo traumático revelou imagens estáticas ou em regressão; (7) os seguintes critérios de proservação foram sugeridos: (a) exame clínico e radiográfico 20 dias após a descoberta da lesão, sem sintomatologia constatável e com imagem radiográfica estável comparável à imagem de diagnóstico; (b) exame clínico e radiográfico de 60 dias mantendo as características iniciais ou mostrando involução da lesão; (c) exame clínico e radiográfico de seis meses mantendo as características iniciais ou de involução da lesão; (d) controle de um ano ou mais, repetindo aspectos já mencionados. Os resultados referentes à amostra número 2 mostraram uma alta prevalência do cisto ósseo traumático em pacientes tratados ortodonticamente, porém 50% dos casos foram detectados na documentação ortodôntica inicial. Assim pode-se concluir: (1) o maior acesso da população ao tratamento odontológico especializado permite a detecção de lesões, até então, consideradas raras; (2) o ortodontista desempenha um importante papel no diagnóstico do cisto ósseo traumático. A research about the traumatic bone cyst (TBC) of the jaws was realized with the following purpose: (1) To determine the epidemiology of the sample studied regarding gender, race, age, clinical history, location, and radiographic image shape. (2) To discuss the possibility of self-healing of the TBC, determining criteria for diagnosis and follow-up without surgical intervention. (3) To study the frequency of TBC in orthodontic treated patients and discuss its possible etiopathogenic relationship. The material was constituted by two sample. The sample number one was composed of 28 cases of TBC, being 19 cases confirmed by surgical procedure, and 9 valid cases of TBC determined by clinical and radiographic features and follow-up information. The sample number two was constituted by cases found in 956 orthodontic treated patients. The results concerning sample number one allowed us to verify that: (1) there was a lack of gender predilection; (2) TBC affected most frequently white patients, although bilateral lesions were more likely to occur in black women; (3) 22 patients were in second decade; (4) 69.23 per cent of the reported cases were involved in orthodontic treatment, and 30.77 per cent showed a positive history of trauma; (5) the favorite anatomic location was the body of the mandible, showing unilocular oval-shaped radiolucencies; (6) the follow-up of the lesions without surgical intervention suggested static images or images in regression. The results concerning sample number two showed a high frequency of TBC in orthodontic treated patients, though 50 per cent of the cases were detected in the initial orthodontic records. Therefore, it is possible to conclude that: (1) access of the population to specialized dental treatment reveals more lesions than it was previously thought there could be; (2) the orthodontist plays an important part in the diagnosis of the TBC.
