3. SOD2 V16A amplifies vascular dysfunction in sickle cell patients by curtailing mitochondria complex IV activity

Abstract: Superoxide dismutase 2 (SOD2) catalyzes the dismutation of superoxide to hydrogen peroxide in mitochondria, limiting mitochondrial damage. The SOD2 amino acid valine-to-alanine substitution at position 16 (V16A) in the mitochondrial leader sequence is a common genetic variant among patients with sickle cell disease (SCD). However, little is known about the cardiovascular consequences of SOD2V16A in SCD patients or its impact on endothelial cell function. Here, we show SOD2V16A associates with increased tricuspid regurgitant velocity (TRV), systolic blood pressure, right ventricle area at systole, and declined 6-minute walk distance in 410 SCD patients. Plasma lactate dehydrogenase, a marker of oxidative stress and hemolysis, significantly associated with higher TRV. To define the impact of SOD2V16A in the endothelium, we introduced the SOD2V16A variant into endothelial cells. SOD2V16A increases hydrogen peroxide and mitochondrial reactive oxygen species (ROS) production compared with controls. Unexpectedly, the increased ROS was not due to SOD2V16A mislocalization but was associated with mitochondrial complex IV and a concomitant decrease in basal respiration and complex IV activity. In sum, SOD2V16A is a novel clinical biomarker of cardiovascular dysfunction in SCD patients through its ability to decrease mitochondrial complex IV activity and amplify ROS production in the endothelium.

Abstract: Background Artificial intelligence (AI) is gaining traction in medicine and surgery. AI-based applications can offer tools to examine high-volume data to inform predictive analytics that supports complex decision-making processes. Time-sensitive trauma and emergency contexts are often challenging. The study aims to investigate trauma and emergency surgeons’ knowledge and perception of using AI-based tools in clinical decision-making processes. Methods An online survey grounded on literature regarding AI-enabled surgical decision-making aids was created by a multidisciplinary committee and endorsed by the World Society of Emergency Surgery (WSES). The survey was advertised to 917 WSES members through the society’s website and Twitter profile. Results 650 surgeons from 71 countries in five continents participated in the survey. Results depict the presence of technology enthusiasts and skeptics and surgeons' preference toward more classical decision-making aids like clinical guidelines, traditional training, and the support of their multidisciplinary colleagues. A lack of knowledge about several AI-related aspects emerges and is associated with mistrust. Discussion The trauma and emergency surgical community is divided into those who firmly believe in the potential of AI and those who do not understand or trust AI-enabled surgical decision-making aids. Academic societies and surgical training programs should promote a foundational, working knowledge of clinical AI.

6. Predictive models in extracorporeal membrane oxygenation (ECMO): a systematic review

Abstract: Purpose Extracorporeal membrane oxygenation (ECMO) has been increasingly used in the last years to provide hemodynamic and respiratory support in critically ill patients. In this scenario, prognostic scores remain essential to choose which patients should initiate ECMO. This systematic review aims to assess the current landscape and inform subsequent efforts in the development of risk prediction tools for ECMO. Methods PubMed, CINAHL, Embase, MEDLINE and Scopus were consulted. Articles between Jan 2011 and Feb 2022, including adults undergoing ECMO reporting a newly developed and validated predictive model for mortality, were included. Studies based on animal models, systematic reviews, case reports and conference abstracts were excluded. Data extraction aimed to capture study characteristics, risk model characteristics and model performance. The risk of bias was evaluated through the prediction model risk-of-bias assessment tool (PROBAST). The protocol has been registered in Open Science Framework (https://osf.io/fevw5). Results Twenty-six prognostic scores for in-hospital mortality were identified, with a study size ranging from 60 to 4557 patients. The most common candidate variables were age, lactate concentration, creatinine concentration, bilirubin concentration and days in mechanical ventilation prior to ECMO. Five out of 16 venous-arterial (VA)-ECMO scores and 3 out of 9 veno-venous (VV)-ECMO scores had been validated externally. Additionally, one score was developed for both VA and VV populations. No score was judged at low risk of bias. Conclusion Most models have not been validated externally and apply after ECMO initiation; thus, some uncertainty whether ECMO should be initiated still remains. It has yet to be determined whether and to what extent a new methodological perspective may enhance the performance of predictive models for ECMO, with the ultimate goal to implement a model that positively influences patient outcomes.

Abstract: Background: The aim of this study was to test the association between periprostatic adipose tissue (PPAT)—apparent diffusion coefficient (ADC) value recorded at multiparametric magnetic resonance imaging (mpMRI) and determinants of prostate cancer (PCa) aggressiveness in the preoperative setting. Methods: Data from 219 consecutive patients undergoing prostate biopsy (PBx) for suspicion of PCa, between January 2020 and June 2022, at our institution were retrospectively evaluated. Only patients who had mpMRI performed before PBx were included. The distribution of demographics and clinical features among PPAT-ADC values up to vs. above the median was studied using both parametric and non-parametric tests, according to variables. Linear and logistic regression models tested the association between PPAT-ADC values and determinants of PCa aggressiveness and the presence of intermediate-high risk PCa, respectively. Results: Of 132 included patients, 76 (58%) had PCa. Median PPAT-ADC was 876 (interquartile range: 654 − 1112) × 10−6 mm2/s. Patients with PPAT-ADC up to the median had a higher rate of PIRADS (Prostate Imaging—Reporting and Data System) 5 lesions (41% vs. 23%, p = 0.032), a higher percentage of PBx positive cores (25% vs. 6%, p = 0.049) and more frequently harbored ISUP (International Society of Urological Pathology) > 1 PCa (50% vs. 28%, p = 0.048). At univariable linear regression analyses, prostate-specific antigen (PSA), PSA density, PIRADS 5, and percentage of PBx positive cores were associated with lower PPAT-ADC values. PPAT-ADC up to the median was an independent predictor for intermediate-high risk PCa (odds ratio: 3.24, 95%CI: 1.17–9.46, p = 0.026) after adjustment for age and body mass index. Conclusions: Lower PPAT-ADC values may be associated with higher biopsy ISUP grade group PCa and a higher percentage of PBx-positive cores. Higher-level studies are needed to confirm these preliminary results.

Abstract: Photoluminescent color conversion by quantum dots (QDs) makes possible the formation of spectrum-on-demand light sources by combining blue LEDs with the light generated by a specific blend of QDs. Such applications, however, require a near-unity photoluminescence quantum efficiency since self-absorption magnifies disproportionally the impact of photon losses on the overall conversion efficiency. Here, we present a synthesis protocol for forming InP-based QDs with +90% quantum efficiency across the full visible spectrum from blue/cyan to red. The central features of our approach are as follows: (1) the formation of InP core QDs through one-batch-one-size reactions based on aminophosphine as the phosphorus precursor, (2) the introduction of a core/shell/shell InP/Zn(Se,S)/ZnS structure, and (3) the use of specific interfacial treatments, most notably the saturation of the ZnSe surface with zinc acetate prior to ZnS shell growth. Moreover, we adapted the composition of the Zn(Se,S) inner shell to attain the intended emission color while minimizing line broadening induced by the InP/ZnS lattice mismatch. The protocol is established by analysis of the QD composition and structure using multiple techniques, including solid-state nuclear magnetic resonance spectroscopy and Raman spectroscopy, and verified for reproducibility by having different researchers execute the same protocol. The realization of full-spectrum, +90% quantum efficiency will strongly facilitate research into light-matter interaction in general and luminescent color conversion in particular through InP-based QDs.

9. Differential diagnosis in Rosai-Dorfman disease: A rare case of isolated hepatic presentation mimicking a metastatic tumor with positive 18-FDG uptake

Abstract: Rosai-Dorfman disease (RDD) is also called sinus histiocytosis with massive lymphadenopathy, and it is caused by a histiocytic disorder with unclear etiology. It usually involves cervical lymph nodes, but it may also present with extranodal involvement. We report a rare condition of isolated hepatic RDD without nodal involvement, clinically manifested with three-month abdominal pain and tenderness of the right hypochondrium. CT- and PET-CT scans were compatible with a secondary lesion from an unknown primary tumor. Therefore, the patient underwent an atypical liver resection. Immunohistochemistry and histological results were compatible with a diagnosis of RDD. RDD is characterized by phenomena of emperipolesis, histiocytic proliferation and positive immunostaining for CD14, CD68 and S-100 protein. Cases of isolated gastrointestinal localization of RDD are particularly rare, especially in the liver. Instrumental exams might confuse RDD with other malignancies. RDD is a rare entity, which might be misdiagnosed using PET-CT due to its similarities with malignant tumors. An accurate multidisciplinary approach may help to clear diagnostic clues of this uncommon disease.

Abstract: Background: Intra- and juxta-articular osteoid osteomas are rare, representing less than 10% of all osteomas. Compared to the classic diaphyseal or metaphyseal site of long bones, they often have an atypical onset, a longest diagnostic delay, and frequent initial misdiagnoses, with pictures that can mimic inflammatory monoarthritis. We aimed to describe a case series, and to provide a literature review of this uncommon and misleading tumor location. Methods: We performed a retrospective analysis of patients referred to three pediatric rheumatology centers, with a final diagnosis of articular osteoid osteoma. A review of the literature was additionally conducted. Results: We included 10 patients with a mean age of 14 years. All patients with unusual sites (olecranon fossa, lumbar vertebra, distal phalanx of the toe, fibula) had a misdiagnosis, and cases with initial suspicion of monoarthritis had the longest diagnostic delay, up to 24 months. The literature review confirms the significant risk of misdiagnosis, and an average time from symptom onset to diagnosis ranging from 0.4 to 1.8 years. Conclusions: Articular osteoid osteoma may mimic arthritis, especially in adolescence. Knowledge of the atypical forms of presentation, and of the clinical and radiological pitfalls, reduces the risk of diagnostic error.

11. Influence of Masonry Infills on Seismic Performance of an Existing RC Building Retrofitted by Means of FPS Devices

Abstract: The safety assessment of existing structures in areas with a relevant seismic hazard is one of the major topics for engineers since many existing reinforced concrete structures have been realized disregarding seismic design with minimal details with respect to present practice. In this context, seismic assessment is a primary issue in order to identify the best retrofitting solution with the aim of enhancing the efficiency of existing buildings. In recent years, with the aim to enhance the seismic behavior of reinforced concrete (RC) structures (with particular care to existing ones), the system of seismic isolation adopting friction pendulum (FPS) devices proved to be among the most diffuse and effective solutions. The purpose of this paper is to explore the effectiveness of the refurbishment using FPS with single concavity devices on the performance of one irregular existing RC building placed in a highly seismic area of central Italy. First, the geometric and material characteristics of the building have been determined within the approach based on the “knowledge levels”. Second, a suitable numerical model based on a fiber-modeling approach has been established using SAP2000, including relevant mechanical non-linearities. Then, a set of 21 natural seismic inputs, inclusive of 3 accelerations over vertical and horizontal directions, was adopted with the aim of performing non-linear (NL) dynamic simulations. The NL dynamic simulations have been performed considering the structural system, both inclusive and not inclusive of the FPS isolator devices. The influence of the actual distribution of infill masonry panels on the overall behavior of the structure has also been evaluated in both of the cases mentioned above. Finally, the outcomes deriving from the NL dynamic simulations were helpful in assessing the advantages of the intervention of retrofitting to improve the seismic performance of the building, highlighting the influence of masonry infills.

Abstract: Cigarette smoke (CS) is the most common risk factor for chronic obstructive pulmonary disease (COPD). The present study aimed to elucidate whether mtDNA is released upon CS exposure and is detected in the plasma of former smokers affected by COPD as a possible consequence of airway damage. We measured cell-free mtDNA (cf-mtDNA) and nuclear DNA (cf-nDNA) in COPD patient plasma and mouse serum with CS-induced emphysema. The plasma of patients with COPD and serum of mice with CS-induced emphysema showed increased cf-mtDNA levels. In cell culture, exposure to a sublethal dose of CSE decreased mitochondrial membrane potential, increased oxidative stress, dysregulated mitochondrial dynamics, and triggered mtDNA release in extracellular vesicles (EVs). Mitochondrial DNA release into EVs occurred concomitantly with increased expression of markers that associate with DNA damage responses, including DNase III, DNA-sensing receptors (cGAS and NLRP3), proinflammatory cytokines (IL-1β, IL-6, IL-8, IL-18, and CXCL2), and markers of senescence (p16 and p21); the majority of the responses are also triggered by cytosolic DNA delivery in vitro. Exposure to a lethal CSE dose preferentially induced mtDNA and nDNA release in the cell debris. Collectively, the results of this study associate markers of mitochondrial stress, inflammation, and senescence with mtDNA release induced by CSE exposure. Because high cf-mtDNA is detected in the plasma of COPD patients and serum of mice with emphysema, our findings support the future study of cf-mtDNA as a marker of mitochondrial stress in response to CS exposure and COPD pathology.

Abstract: Chronic obstructive pulmonary disease (COPD) is characterized by continuous and irreversible inflammation frequently caused by persistent exposure to toxic inhalants such as cigarette smoke (CS). CS may trigger mitochondrial DNA (mtDNA) extrusion into the cytosol, extracellular space, or foster its transfer by extracellular vesicles (EVs). The present study aimed to elucidate whether mtDNA is released upon CS exposure and in COPD. We measured cell-free mtDNA (cf-mtDNA) in the plasma of former smokers affected by COPD, in the serum of mice that developed CS-induced emphysema, and in the extracellular milieu of human bronchial epithelial cells exposed to cigarette smoke extract (CSE). Further, we characterized cells exposed to sublethal and lethal doses of CSE by measuring mitochondrial membrane potential and dynamics, superoxide production and oxidative stress, cell cycle progression, and cytokine expression. Patients with COPD and mice that developed emphysema showed increased levels of cf-mtDNA. In cell culture, exposure to a sublethal dose of CSE decreased mitochondrial membrane potential, increased superoxide production and oxidative damage, dysregulated mitochondrial dynamics, and triggered mtDNA release in extracellular vesicles. The release of mtDNA into the extracellular milieu occurred concomitantly with increased expression of DNase III, DNA-sensing receptors (cGAS, NLRP3), proinflammatory cytokines (IL-1β, IL-6, IL-8, IL-18, CXCL2), and markers of senescence (p16, p21). Exposure to a lethal dose of CSE preferentially induced mtDNA and nuclear DNA release in cell debris. Our findings demonstrate that CS-induced stress triggers mtDNA release and is associated with COPD, supporting cf-mtDNA as a novel signaling response to CS exposure.

Abstract: Robustness of structures is fundamental to limit progressive collapse of buildings in case of accidental loss of columns due to explosions, impacts or materials deterioration. Modelling of progressive collapse response of reinforced concrete (RC) frame structures needs considering extreme geometric and mechanical nonlinearities. Moreover, in the case of infilled frames the collapse mechanism becomes more complex because of the frame-infill interaction. This paper presents a numerical study aimed at proposing: a) an appropriate fiber-section modeling methodology for reinforced concrete frames under large displacement progressive collapse events; b) a new multi-strut fiber macro-element model to account for the influence of masonry infills in the progressive collapse response. Proposed numerical models are developed using the OpenSees software platform. The predictive capacity of the proposed methodology is widely validated in the paper through comparisons with experimental test results and refined numerical simulation pushdown test results. Results show that the new equivalent-strut modeling approach can be suitably employed as a simple assessment method when numerical simulation of progressive collapse scenarios is needed for bare and infilled reinforced concrete frames.

Abstract: Acclimation to acute hypoxia through cardiorespiratory responses is mediated by specialized cells in the carotid body and pulmonary vasculature to optimize systemic arterial oxygenation and thus oxygen supply to the tissues. Acute oxygen sensing by these cells triggers hyperventilation and hypoxic pulmonary vasoconstriction which limits pulmonary blood flow through areas of low alveolar oxygen content. Oxygen sensing of acute hypoxia by specialized cells thus is a fundamental pre-requisite for aerobic life and maintains systemic oxygen supply. However, the primary oxygen sensing mechanism and the question of a common mechanism in different specialized oxygen sensing cells remains unresolved. Recent studies unraveled basic oxygen sensing mechanisms involving the mitochondrial cytochrome c oxidase subunit 4 isoform 2 that is essential for the hypoxia-induced release of mitochondrial reactive oxygen species and subsequent acute hypoxic responses in both, the carotid body and pulmonary vasculature. This review compares basic mitochondrial oxygen sensing mechanisms in the pulmonary vasculature and the carotid body.

Abstract: Background: Phase 1 clinical trials represent a critical phase of drug development because new candidate therapeutic agents are tested for the first time on humans. Therefore, international guidelines and local laws have been released to mitigate and control possible risks for human health in agreement with the declaration of Helsinki and the international Good Clinical Practice principles. Despite numerous scientific works characterizing the registered clinical trials on ClinicalTrials.gov, the main features and trends of registered phase 1 clinical trials in Europe have not been investigated. This study is aimed at assessing the features and the temporal trend of distribution of phase 1 clinical studies, carried out in the five largest European countries over a ten-year period (2012–2021), and to evaluate the impact of the Italian regulatory framework on the activation of such studies. Methods: The main data and characteristics of phase 1 clinical studies registered on the ClinicalTrials.gov database for France, Germany, Italy, Spain and the United Kingdom have been investigated and subsequently compared. The above-mentioned countries were selected based on similarities in terms of demographic and Gross Domestic Product (GDP) data available on official government websites. (3) Results: A total number of 6878 phase 1 clinical trials were registered for the five selected countries in the ClinicalTrials.gov database during the ten years analyzed; the studies were predominantly randomized (39.33%) and for-profit (76.64%). The most represented area of investigations was oncology (52.15%), followed by hematology (24.99%) and immunology (12.04%). The variability observed between the analyzed countries showed that the UK, Germany and France presented the highest reduction in the number of phase 1 clinical trials, while for Spain and Italy, a stable/increased trend was observed, although with a lower number of trials registered on the ClinicalTrials.gov database. (4) Conclusions: Italy displayed the lowest number of registered phase 1 clinical trials, even though it showed a stable trend over the years. In this regard, the Italian regulatory framework must urgently be adapted to that of other European countries (Spain has been the first country to implement the new Regulation (EU) No 536/2014) and streamline the process of clinical trial application to increase the attractiveness of the country. Moreover, nonprofit phase 1 clinical trials (which represent 19.81% of the total number of phase 1 clinical trials registered in Italy vs. 80.19% of profit phase 1 clinical studies) should be promoted and supported by the institutions, even from a financial point of view, to allow independent researchers to develop new therapeutic drugs.

Abstract: Plants and other organisms, but not insects or vertebrates, express the auxiliary respiratory enzyme alternative oxidase (AOX) that bypasses mitochondrial respiratory complexes III and/or IV when impaired. Persistent expression of AOX from Ciona intestinalis in mammalian models has previously been shown to be effective in alleviating some metabolic stresses produced by respiratory chain inhibition while exacerbating others. This implies that chronic AOX expression may modify or disrupt metabolic signaling processes necessary to orchestrate adaptive remodeling, suggesting that its potential therapeutic use may be confined to acute pathologies, where a single course of treatment would suffice. One possible route for administering AOX transiently is AOX-encoding nucleic acid constructs. Here we demonstrate that AOX-encoding chemically-modified RNA (cmRNA), sequence-optimized for expression in mammalian cells, was able to support AOX expression in immortalized mouse embryonic fibroblasts (iMEFs), human lung carcinoma cells (A549) and primary mouse pulmonary arterial smooth muscle cells (PASMCs). AOX protein was detectable as early as 3 h after transfection, had a half-life of ~4 days and was catalytically active, thus supporting respiration and protecting against respiratory inhibition. Our data demonstrate that AOX-encoding cmRNA optimized for use in mammalian cells represents a viable route to investigate and possibly treat mitochondrial respiratory disorders.

Abstract: Patients are the most important actors in clinical research. Therefore, patient preference information (PPI) could support the decision-making process, being indisputable for research value, quality, and integrity. However, there is a lack of clear guidance or consensus on the search for preference studies. In this blueprint, an openly available and regularly updated patient preference management system for an integrated database (PPMSDB) that contains the minimal set of data sufficient to provide detailed information for each study (the so-called evidence tables in systematic reviews) and a high-level overview of the findings of a review (summary tables) is described. These tables could help determine which studies, if any, are eligible for quantitative synthesis. Finally, a web platform would provide a graphical and user-friendly interface. On the other hand, a set of APIs (application programming interfaces) would also be developed and provided. The PPMSDB, aims to collect preference measures, characteristics, and meta-data, and allow researchers to obtain a quick overview of a research field, use the latest evidence, and identify research gaps. In conjunction with proper statistical analysis of quantitative preference measures, these aspects can facilitate formal evidence-based decisions and adequate consideration when conducting a structured decision-making process. Our objective is to outline the conceptual infrastructure necessary to build and maintain a successful network that can monitor the currentness and validity of evidence.

Abstract: Background Surgical strategies are being adapted to face the COVID‐19 pandemic. Recommendations on the management of acute appendicitis have been based on expert opinion, but very little evidence is available. This study addressed that dearth with a snapshot of worldwide approaches to appendicitis. Methods The Association of Italian Surgeons in Europe designed an online survey to assess the current attitude of surgeons globally regarding the management of patients with acute appendicitis during the pandemic. Questions were divided into baseline information, hospital organization and screening, personal protective equipment, management and surgical approach, and patient presentation before versus during the pandemic. Results Of 744 answers, 709 (from 66 countries) were complete and were included in the analysis. Most hospitals were treating both patients with and those without COVID. There was variation in screening indications and modality used, with chest X‐ray plus molecular testing (PCR) being the commonest (19·8 per cent). Conservative management of complicated and uncomplicated appendicitis was used by 6·6 and 2·4 per cent respectively before, but 23·7 and 5·3 per cent, during the pandemic (both P, The COVID‐19 pandemic required reorganization of surgical services, affecting patients with common surgical diseases including acute appendicitis. No evidence is available on the topic. This study found global variation in screening policies, use of personal protective equipment and intraoperative directives. Abstract: Background and Objectives: Published comparisons of minimally invasive approaches to colon surgery are limited. The objective of the current study is to compare the effectiveness of robotic-assisted and laparoscopic sigmoid resection. Methods: A multicenter retrospective comparative analysis of perioperative outcomes from consecutive robotic-assisted and laparoscopic sigmoid resections performed between 2010 and 2015 by six general and colorectal surgeons, who are experienced in both robotic-assisted and laparoscopic surgical techniques and who had >50 annual case volumes for each approach. Baseline characteristics and surgical risk factors between the two groups were balanced using a propensity score methodology with inverse probability of treatment weighting. Mean standardized differences were reported, and in all instances, a p-value < 0.05 was considered statistically significant. Results: Three hundred thirty-six cases (robotic-assisted, n = 211; laparoscopic, n = 125) met eligibility criteria and were included in the study. Following weighting, patient demographics and baseline characteristics were comparable between the robotic-assisted (n = 344) and laparoscopic (n = 349) groups. The laparoscopic group was associated with shorter operating room and surgical times. The robotic-assisted group had lower estimated blood loss and shorter time to first flatus compared to the laparoscopic group. Rates of complications post discharge to 30 d tended to be lower for the RA group: 5.1% vs 8.6% [p = 0.0657]. The RA group also had lower rates of readmissions and reoperations: 4% vs 8% [p = 0.029] and 0.5% vs 5.1% [p = 0.0003], respectively. Conclusions: Robotic-assisted sigmoid colon resection is clinically effective and provides a minimally invasive alternative to the laparoscopic approach with improved intraoperative and postoperative outcomes for colorectal patients.

Abstract: Circulating, cell-free mitochondrial DNA (ccf-mtDNA) and nuclear DNA (ccf-nDNA) are under investigation as biomarkers for various diseases. Optimal ccf-mtDNA isolation parameters, like those outlined for ccf-nDNA, have not been established. Here, we optimized a protocol for both ccf-mtDNA and ccf-nDNA recovery using a magnetic bead-based isolation process on an automated 96-well platform. Using the optimized protocol, our data show 6-fold improved yields of ccf-mtDNA when compared to the starting protocol. Digestion conditions, liquid handling characteristics, and magnetic particle processor programming all contributed to increased recovery and improved reproducibility. To our knowledge, this is the first high-throughput approach optimized for mtDNA and nDNA recovery and serves as an important starting point for clinical studies.Graphical Abstract

Abstract: In many countries around the world a huge number of existing reinforced concrete (RC) structures have been realized without account for seismic detailing, even if they are located in areas subjected to high seismicity. In this context, several passive seismic protection techniques have been developed and applied to retrofit these structures such as, for an example, seismic isolation. The aim of this work is to characterize in probabilistic terms the seismic performance of a framed RC building retrofitted by means of sliding friction pendulum (FPS) devices. Specifically, the response of an existing RC building located in a high seismicity area in Italy is investigated. After the description of the main available information about the structure, a non-linear numerical model has been defined by means of fiber-elements approach. Then, non-linear dynamic analyses considering multiple recorded ground motions with the three accelerometric components have been carried out to assess the seismic response of the building with and without the retrofitting intervention composed of FPS isolators. Finally, the results are processed to achieve a probabilistic assessment of the seismic performance of the retrofitting intervention.

Abstract: According to Eurocode, robustness is the ability of a structure to withstand events like fire, explosions, impact or the consequences of human error, without being damaged to an extent disproportionate to the original cause.Avoiding the progressive collapse of a building in presence of accidental loading conditions is one of the challenges for the designers.The tie-force method is actually one of the most used design techniques for resisting progressive collapse, whereby a statically indeterminate structure is designed with reference to local simplified models determined in accordance to the failure mode considered.In this work a computational study of a reinforced concrete frame is presented. The beam-column assembly represents a portion of the structural framing system of a ten-story reinforced concrete frame building and is subjected to monotonically increasing vertical displacement of the center column to simulate a column removal scenario.Two different finite element models, with distinct levels of modeling, are used in order to compare the numerical results with the experimental ones coming from a full-scale test, and evaluate the ability of the models to simulate the structural behavior of the frame.

Abstract: Mitochondrial disorders associated with genetic defects of the ATP synthase are among the most deleterious diseases of the neuromuscular system that primarily manifest in newborns. Nevertheless, the number of established animal models for the elucidation of the molecular mechanisms behind such pathologies is limited. In this paper, we target the Drosophila melanogaster gene encoding for the ATP synthase subunit c, ATPsynC, in order to create a fruit fly model for investigating defects in mitochondrial bioenergetics and to better understand the comprehensive pathological spectrum associated with mitochondrial ATP synthase dysfunctions. Using P-element and EMS mutagenesis, we isolated a set of mutations showing a wide range of effects, from larval lethality to complex pleiotropic phenotypes encompassing developmental delay, early adult lethality, hypoactivity, sterility, hypofertility, aberrant male courtship behavior, locomotor defects and aberrant gonadogenesis. ATPsynC mutations impair ATP synthesis and mitochondrial morphology, and represent a powerful toolkit for the screening of genetic modifiers that can lead to potential therapeutic solutions. Furthermore, the molecular characterization of ATPsynC mutations allowed us to better understand the genetics of the ATPsynC locus and to define three broad pathological consequences of mutations affecting the mitochondrial ATP synthase functionality in Drosophila: i) pre-adult lethality; ii) multi-trait pathology accompanied by early adult lethality; iii) multi-trait adult pathology. Abstract: Downregulation of Jun N-terminal kinase (JNK) signaling inhibits cell migration in diverse model systems. In Drosophila pupal development, attenuated JNK signaling in the thoracic dorsal epithelium leads to defective midline closure, resulting in cleft thorax., Downregulation of Jun N-terminal kinase (JNK) signaling inhibits cell migration in diverse model systems. In Drosophila pupal development, attenuated JNK signaling in the thoracic dorsal epithelium leads to defective midline closure, resulting in cleft thorax. Here we report that concomitant expression of the Ciona intestinalis alternative oxidase (AOX) was able to compensate for JNK pathway downregulation, substantially correcting the cleft thorax phenotype. AOX expression also promoted wound-healing behavior and single-cell migration in immortalized mouse embryonic fibroblasts (iMEFs), counteracting the effect of JNK pathway inhibition. However, AOX was not able to rescue developmental phenotypes resulting from knockdown of the AP-1 transcription factor, the canonical target of JNK, nor its targets and had no effect on AP-1-dependent transcription. The migration of AOX-expressing iMEFs in the wound-healing assay was differentially stimulated by antimycin A, which redirects respiratory electron flow through AOX, altering the balance between mitochondrial ATP and heat production. Since other treatments affecting mitochondrial ATP did not stimulate wound healing, we propose increased mitochondrial heat production as the most likely primary mechanism of action of AOX in promoting cell migration in these various contexts.

Abstract: α-Phenylcinnamic acid converts into polycyclic derivatives by interaction with high valent transition metal halides in chloroform at reflux temperature. Different products, featured by local structural differences depending on the nature of the metal halide system (i. e. WCl6, PCl5/NbF5or PCl5/NbCl5), are easily isolable after treatment with water. In particular, WCl6configures as an efficient multitasking agent, working as 1) chlorinator of the carboxylic acid function, 2) catalytic precursor for intra- and intermolecular rearrangements, and 3) catalytic precursor for Baeyer-Villiger lactonization in water at ambient temperature, exploiting air as oxidant. The products have been purified by alumina chromatography, and identified by analytical and spectroscopic (IR, NMR) techniques, and by single crystal X-ray diffraction in three cases. Their formation probably involves a common reaction pathway which has been rationalized by DFT calculations.

Abstract: In structural design, structures are often modeled using the finite elements method (FEM). One of the most common element type is the shell, which is used to model surfaces in a three dimensional space as far as the surface thickness is smaller than the other two dimensions. Many decisions must be taken during the design process, and many physical and loading constraints must be satisfied. Designers are generally interested in providing a solution that respects all the problem constraints, without trying to further improve it. In fact optimization is not trivial, even if it could yield a huge benefit both from the economic and the construction point of view. Additionally, saving materials is one of the fundamental criteria for the sustainable approach to the design. In this paper, we address the Skew Reinforcement Design in Reinforced Concrete Two Dimensional Elements (SRD2D). It consists of determining the minimum reinforcement required to respect all the constraints given by the geometric properties and the internal actions working on it. As this problem is strongly nonlinear and non-convex it cannot be easily solved using exact methods, while heuristics and meta-heuristics are suitable to this purpose. We propose a Genetic Algorithm (GA) and an enhanced version of it, a memetic algorithm, in which we apply an intensification heuristic to the solution obtained by the GA, where each variable is separately optimized by applying a first improvement based on local search strategy. We report computational results showing both the effectiveness of the proposed method, and the benefit of combining GAs with intensification methods.

38. Implicazioni e possibili motivazioni della scelta di non quotarsi da parte delle medie imprese italiane (Earnings Management and Financial Constraints for Listed and Not-listed Firms. Is There Any Rationality for Not Going Public?)

Abstract: Italian Abstract: Il presente lavoro mette a confronto su un periodo di dieci anni (2002-2011) i bilanci di un campione rappresentativo di medie imprese italiane quotate in borsa con un campione di imprese non quotate simili per settore industriale e per dimensione. I dati mostrano che le societa quotate hanno registrato tassi di crescita notevolmente superiori rispetto alle non quotate in termini di fatturato, investimenti e occupazione, anche grazie a una maggiore possibilita di ricorso al capitale di debito; le societa quotate, peraltro, oltre ad aver generato una maggiore redditivita, hanno risentito in misura piu attenuata degli effetti della crisi finanziaria del 2007 e della conseguente recessione che ha avuto inizio nel 2009. Nelle evidenze internazionali, la “qualita” dei dati contabili e uno degli aspetti che piu marcatamente differenzia le societa quotate da quelle a controllo privato. I risultati confermano, anche per il mercato italiano, una minore “qualita” dei dati contabili per le societa non quotate che puo essere ricondotta a quell’insieme di pratiche note con il nome di earnings management, sebbene esse sopportino un carico fiscale simile (se non superiore) a quello delle societa quotate. Le societa non quotate hanno infine maggiori vincoli finanziari in termini di capacita di raccolta di capitale di rischio e vi sono chiare evidenze di una maggiore difficolta di accesso al credito bancario per le non quotate. Per tali motivi, le loro opportunita di investimento sono maggiormente vincolate alla capacita di autofinanziamento, come indicato dalla piu elevata sensitivita degli investimenti al cash flow rispetto alle societa quotate. Complessivamente, queste evidenze indicano che la scelta di «non quotarsi» implica costi molto elevati in termini di minore crescita e redditivita e, allo stesso tempo, sembra solo in parte riconducibile alla volonta del management di sfruttare maggiori possibilita di manipolazione dei dati contabili (earnings management) per estrarre benefici privati o condizionare i contratti con gli stakeholders. I risultati del lavoro portano quindi a ritenere che la scarsa propensione alla quotazione delle medie imprese italiane – che si riflette nell’esiguita del nostro listino – non sembra giustificata dalla prevalenza dei benefici (maggior discrezionalita nella redazione della rendicontazione contabile e finanziaria) sui costi (minor redditivita, minor crescita, piu severo razionamento e dipendenza da fonti di finanziamento interne) connessi con la conservazione dello status di “non quotata”, ma sia da ricondurre ad altri fattori, anche culturali, verosimilmente legati alla percezione dei costi complessivi dell’apparato regolamentare che disciplina la quotazione in borsa. Questi risultati evidenziano l’importanza dei numerosi recenti interventi di semplificazione delle regole e di incentivo allo sviluppo di canali di finanziamento alternativi al credito bancario. English Abstract: This paper compares a sample of ten years (2002-2011) mid-size Italian listed companies with a sample of similar unlisted companies with comparable size and operating in a similar industrial sector. Data shows that listed companies recorded growth rates considerably higher than unlisted in terms of turnover, investment and employment, also thanks to a greater use of debt capital. Listed companies, moreover, in addition to having generated more profitability, have been less affected by the effects of the financial crisis which spread out in 2007 and the resulting recession that began in 2009. The results confirm, also for the Italian market, a lower "quality" of the accounting data for the unlisted companies that can be traced back to the set of practices named as “earnings management”, although they bear a similar tax burden (if not higher) than that of listed companies. Unlisted companies also have greater financial constraints in terms of raising risk capital and there is clear evidence of reduced access to bank credit for non-listed companies. For these reasons, their investment opportunities are more constrained to self-financing, as indicated by the higher sensitivity of cash flow investments to listed companies.

Abstract: While a large body of literature argues that financial intermediaries exert a causal impact on long-run growth, it doesn’t investigate the links between financial architecture and technological change. We seek to shed light on these links by exploring the relationship between financial architecture (FA) and what is assumed to be one of the main drivers of economic growth, the technological change (TC). We apply the stochastic frontier analysis (SFA) to estimate and decompose total factor productivity growth (TFP) into its main components: efficiency change (EC) and technological change (TC). As a second step we regress the technological change (TC) on a set of variables capturing the financial characteristics (“ financial architecture ”) of a sample of OECD countries in order to identify which features of the financial system affect the country’s rate of technological change. Our results confirm that better functioning financial systems – open and competitive – improve resource allocation and accelerate the country’s rate of technological change with positive impact on long-run economic growth.

Abstract: In jawed vertebrates the V-(D)-J rearrangement is the main mechanism generating limitless variations of antigen-specific receptors, immunoglobulins (IGs), and T-cell receptors (TCRs) from few genes. Once the initial diversity is established in primary lymphoid organs, further diversification occurs in IGs by somatic hypermutation, a mechanism from which rearranged TCR genes were thought to be excluded. Here, we report the locus organization and expression of the T-cell receptor gamma (TCRG) genes in the Arabian camel (Camelus dromedarius). Expression data provide evidence that dromedary utilizes only two TCRG V-J genomic arrangements and, as expected, CDR3 contributes the major variability in the V domain. The data also suggest that diversity might be generated by mutation in the productively rearranged TCRGV genes. As for IG genes, the mutational target is biased toward G and C bases and (A/G/T)G(C/T)(A/T) motif (or DGYW). The replacement and synonymous substitutions (R/S) ratios in TCRGV regions are higher for CDR than for framework region, thus suggesting selection toward amino acid changes in CDR. Using the counterpart human TCR γδ receptor as a template, structural models computed adopting a comparative procedure show that nonconservative mutations contribute to diversity in CDR2 and at the γδ V domain interface.

Abstract: Deterioration process due to corrosion of steel in concrete affects the performance of reinforced concrete structures both in service and ultimate conditions. Corrosion reduces the rebar section, deteriorates the surrounding concrete with the oxides expansion product and it also alters bond between steel and concrete. Moreover, in structures subjected to cyclic loading, the damage due to corrosion can be combined with the mechanical action that is present in service. In this case the initial crack pattern due to load action and to the rheological phenomena is further modified by the expansion of the oxides and by the interaction among those causes. In the present work, the results of an experimental campaign on reinforced concrete elements subjected to simultaneous corrosion and cyclic loading are shown. It is put in evidence the loss of structural performance, by effect of chemical degradation and mechanical action. The key role of the combined effect of those causes of deterioration is also confirmed by static tests performed under the condition of static loading and simultaneous corrosion.

Abstract: In the present work, an experimental campaign on reinforced concrete elements subjected to simultaneous loading and degradation due to corrosion is presented. The specimens are loaded in tension and the cycling action is designed in order to simulate in terms of stress peak and stress variation, the effect of the passage of vehicles under fatigue design situations. The environmental action is simulated by means of corrosion induced with an electrochemical process; the corrosion rate is chosen in order to obtain about 27 years of equivalent carbonation attack within 25 days of test. In the mean time, 6.5·106 loading cycles are achieved. In addition to those standard tests, also other static tests were conducted in order to have a comprehensive analysis of the deterioration processes. Evolution of transversal crack due to loading and of longitudinal cracks due to corrosion is monitored. They can be related to the internal bond condition between steel and concrete. Results highlight the differences in terms of corroded and uncorroded specimens, static and cycling test and also different loading amplitude. Finally it can be observed as the growing of the damage is significantly different when a cycling action, combined with a chemical attack, is present.

Recent studies suggest potential short-term outcomes advantages for the intracorporeal anastomosis technique.MethodsThis is a multicenter retrospective propensity score-matched comparison of intracorporeal and extracorporeal anastomosis techniques for laparoscopic and robotic-assisted right colectomy between January 11, 2010, and July 21, 2016.ResultsAfter propensity score-matching, there were a total of 1029 minimal invasive surgery cases for analysis-379 right colectomies (335 robotic-assisted and 44 laparoscopic) done with an intracorporeal anastomosis and 650 right colectomies (253 robotic-assisted and 397 laparoscopic) done with an extracorporeal anastomosis. There were no significant differences in any preoperative patient characteristics between groups. The minimally invasive intracorporeal anastomosis group had significantly longer operative times (pl0.0001), lower conversion to open rate (p = 0.01), shorter hospital length of stay (p = 0.02) and lower complication rate from after discharge to 30-days (p = 0.04) than the extracorporeal anastomosis group.ConclusionsThis comparison shows several clinical outcomes advantages for the intracorporeal anastomosis technique in minimally invasive right colectomy. These data may guide future refinements in minimally invasive training techniques and help surgeons choose among different minimally invasive options.

In this work an experimental analysis on reinforced concrete structures under both cyclic load and corrosion of reinforcing bars is presented. Three couples of reinforced concrete ties are connected in series and subjected to the same stress variation in order to produce the cracking conditions and to activate the bond mechanism. However, while one of the two reinforced concrete ties is only subjected to cyclic load, the second one is also corroded using an accelerated electrochemical corrosion process. The simultaneous effect of the cyclic load and corrosion is evaluated monitoring the crack opening on the structures during the test and by means of visual inspection of the sample. The test results show the correlation between the mechanism of bond and the average level of stresses for an amplified stress range.

Abstract: Summary Leber's Hereditary Optic Neuropathy (LHON) is a mitochondrial disease due to homoplasmic mtDNA point mutations in complex I, affecting prevalently young males with incomplete penetrance. We recently documented that increase of mitochondrial biogenesis drives incomplete penetrance in LHON, further enhanced by estrogen protection in females. To shed light on nuclear genetic modifiers of LHON penetrance, we combined linkage analysis and association studies with tag and functional SNPs, MitoExome and microarray expression analysis. After computation of relevant co-variates (age, sex, smoke and mtDNA copy number) we obtained a list of candidate genes, the most interesting being implicated in ROS detoxification, mitochondrial biogenesis and cell quality control. A prevalent role of tobacco smoking in penetrance also emerged from our studies, showing that tobacco toxicity triggers LHON by depressing mtDNA copy number and oxidative phosphorylation. Overall, our results indicate that penetrance in LHON is modulated by variants in different genes rather than by a single mutation, and that a complex interaction with environmental factors such as tobacco smoking plays also a major role, representing a confounder for genetic studies. 47. Portal Vein Thrombosis following Splenectomy: Identification of Risk Factors

Abstract: Portal vein thrombosis (PVT) following splenectomy is a potentially life-threatening complication, and the true incidence of PVT in splenectomized patients is unknown. The objective of this study was to determine the incidence of symptomatic PVT after splenectomy. The hospital database was searched to identify cases of PVT associated with splenectomy from January 1990 to May 2002. Six hundred eighty-eight patients underwent splenectomy during this period, 321 of them for hematologic diseases. Eleven of the 688 patients had PVT associated with splenectomy, and the charts of these patients were reviewed. Six patients developed PVT after splenectomy. Five had hematologic diseases. Symptoms were abdominal pain (6), ileus (5), fever (3), or diarrhea (2). Diagnosis was confirmed by computed tomography (CT) (4), duplex ultrasonography (1), and magnetic resonance imaging (1). The indications for splenectomy included hemolytic anemia (3), thalassemia (1), and myelofibrosis (1). One patient had an incidental splenectomy during gastrectomy. There were four laparoscopic and two open splenectomies. The median interval between splenectomy and diagnosis of PVT was 40 days (range, 13–741). One patient died of pulmonary embolism. Five of six patients with postsplenectomy PVT had splenomegaly and hemolysis. We conclude that the risk of PVT is higher in patients with hematologic conditions associated with splenomegaly and hemolysis.

49. Competition Versus Efficiency: What Drives Bankss Spreads in Italian Banking System?

Abstract: Economists have long debated the relationships between market structure of banking sector and the profitability. General consensus asserts that more concentrated market is associated with higher profitability: banks with higher market share generally achieve higher profits. This empirical evidence can however hinge on two opposite explanations: in the first case banks increase their market share (via mergers and acquisitions) in order to exploit the resulting stronger market power and impose higher prices to their clients; the second explanation tells that more cost-efficient banks are able to lower the prices applied to their clients and therefore to gain new clients and finally enlarge their market share. In both cases there is a positive link between market share and profitability, but in the former what actually plays a crucial role is a non-competitive force (Relative Market Power Hypothesis, henceforth RMP), in the latter the relationship is driven by the greater efficiency of banks which enlarge their market share by reducing prices (X-Efficiency Hypothesis, ESX). We run a 4-years panel data analysis (2008-2011) in order to disentangle the above alternatives hypothesis (RMP versus ESX) for the Italian market. First we estimate the cost efficiency for a sample of more than 200 banks by applying the Stochastic Frontier Analysis (SFA). In the second step we regress bank spreads on efficiency scores and market structure variables in order to identify which process leads the price setting of Italian banks. Difference in the legal organization of bank (mutual, cooperative, and commercial banks) and type of business relationships established are controlled for.

Abstract: Leber’s hereditary optic neuropathy (LHON), the most frequent mitochondrial disease, is associated with mitochondrial DNA (mtDNA) point mutations affecting Complex I subunits, usually homoplasmic. This blinding disorder is characterized by incomplete penetrance, possibly related to several genetic modifying factors. We recently reported that increased mitochondrial biogenesis in unaffected mutation carriers is a compensatory mechanism, which reduces penetrance. Also, environmental factors such as cigarette smoking have been implicated as disease triggers. To investigate this issue further, we first assessed the relationship between cigarette smoke and mtDNA copy number in blood cells from large cohorts of LHON families, finding that smoking was significantly associated with the lowest mtDNA content in affected individuals. To unwrap the mechanism of tobacco toxicity in LHON, we exposed fibroblasts from affected individuals, unaffected mutation carriers and controls to cigarette smoke condensate (CSC). CSC decreased mtDNA copy number in all cells; moreover, it caused significant reduction of ATP level only in mutated cells including carriers. This implies that the bioenergetic compensation in carriers is hampered by exposure to smoke derivatives. We also observed that in untreated cells the level of carbonylated proteins was highest in affected individuals, whereas the level of several detoxifying enzymes was highest in carriers. Thus, carriers are particularly successful in reactive oxygen species (ROS) scavenging capacity. After CSC exposure, the amount of detoxifying enzymes increased in all cells, but carbonylated proteins increased only in LHON mutant cells, mostly from affected individuals. All considered, it appears that exposure to smoke derivatives has a more deleterious effect in affected individuals, whereas carriers are the most efficient in mitigating ROS rather than recovering bioenergetics. Therefore, the identification of genetic modifiers that modulate LHON penetrance must take into account also the exposure to environmental triggers such as tobacco smoke.
