However, these findings come from studies including active smokers, while results in ex-smokers are scarce and contradictory. The purpose of this study was to evaluate if frequent acute moderate exacerbations are associated with an accelerated decline in forced expiratory volume in one second (FEV1) and impairment of functional and clinical outcomes in ex-smoking COPD patients.Methods: A cohort of 100 ex-smoking patients recruited for a 2-year follow-up study was evaluated at inclusion and at 6-monthly scheduled visits while in a stable condition. Evaluation included anthropometry, spirometry, inspiratory capacity, peripheral capillary oxygen saturation, severity of dyspnea, a 6-minute walking test, BODE (Body mass index, airflow Obstruction, Dyspnea, Exercise performance) index, and quality of life (StGeorge’s Respiratory Questionnaire and Chronic Respiratory Disease Questionnaire). Severity of exacerbation was graded as moderate or severe according to health care utilization. Patients were classified as infrequent exacerbators if they had no or one acute exacerbation/year and frequent exacerbators if they had two or more acute exacerbations/year. Random effects modeling, within hierarchical linear modeling, was used for analysis.Results: During follow-up, 419 (96% moderate) acute exacerbations were registered. At baseline, frequent exacerbators had more severe disease than infrequent exacerbators according to their FEV1 and BODE index, and also showed greater impairment in inspiratory capacity, forced vital capacity, peripheral capillary oxygen saturation, 6-minute walking test, and quality of life. However, no significant difference in FEV1 decline over time was found between the two groups (54.7±13 mL/year versus 85.4±15.9 mL/year in frequent exacerbators and infrequent exacerbators, respectively). This was also the case for all other measurements.Conclusion: Our results suggest that frequent moderate exacerbations do not contribute to accelerated clinical and functional decline in COPD patients who are ex-smokers.Keywords: chronic obstructive pulmonary disease, acute exacerbations, disease progression, FEV1, BODE index, health status

Background/Aims: Hidradenitis suppurativa (HS), a chronic inflammatory skin disease of the hair follicle, can lead to scarring and disability. With an estimated European prevalence of 1%, few epidemiological studies of HS have been performed, and none focused on hospitalisations. We aimed to study the time trends of HS hospitalisations and to evaluate the demographic characteristics, hospital incidence rate, readmissions, length of stay, comorbidities and risk factors of hospitalised HS patients. Methods: We performed a retrospective observational study using a national administrative database in Portugal, with discharges between 2000 and 2014. All the inpatients aged 5 years or more with a diagnosis of HS were included. Variables analysed were age, sex, admission and discharge date, discharge outcome and diagnoses. Results: A total of 1,177 patients were hospitalised in this time period (48 were aged 18 years or younger) with a male-to-female ratio of 1:1.17. There was a hospital incidence rate of 0.83 patients with HS per 100,000 person-years (95% CI = 0.78-0.88). The age group with the highest incidence rate was 20-29 years among women and 40-49 years among men. We recorded an increasing trend in the number of new hospitalised patients and in the hospital incidence rate of HS. Tobacco was the most common comorbidity/risk factor. Eighty-three percent of our population underwent HS surgery. Conclusion: This hospital-based incidence study showed that admission for HS is increasing and that the majority of the HS inpatients were surgical cases. In the future, prospective studies will be important to assess risk factors for hospitalisations and complications.

Peer relations constitute a main developmental context for adolescents. Peers offer an instance for identity definition and set the norms of acceptable and valued characteristics, behaviors, and attitudes, representing a societal model that allows and restrains avenues for adolescents' socioemotional development. The present article departs from these considerations to review research on adolescents' peer relations in Latin America from a socioemotional perspective. First, approaches to adolescence are discussed, with a main focus on attachment and identity theories, based on a bioecological framework. Then, a review of research in Latin America on friendships, school climate, and intergroup relations is presented. The discussion addresses the tension between theories and evidence generated in developed societies and highlights the particularities of Latin American youth, stressing the need for collecting local data.

The subtype of violence behavior named as bullying is a serious problem recurrent in schools nowadays, bringing negative short, medium and long term consequences to development process. Considered as a public health issue, this and other forms of peer aggression are attracting media, parents, teachers and scientists attention as well as attention from responsible for public policies. Therefore, in a digital world, the new information and communication technologies allow a new kind of bullying to take place, a violence that situates in cyberspace and is supported by interactive technological tools, called cyberbullying. In this sense, the present paper aims to analyze and discuss aspects that are related to this electronic, intentional aggressive behavior, focusing in conceptual differences from bullying, impacts, risk and protective factors related to this process. To that, studies selected from Pepsic, Science Direct, BVS, PsychInfo, Scielo and PubMed about prevalence, characteristics and cyberbullying consequences are presented and discussed. The result was an unsystematic international and national literature revision toward cyberbullying that allowed an adequate reflection about this kind of violence and possible ways to a better comprehension and effective strategies to deal with cyberbullying nowadays. The importance of the researches about this topic in Brazil and Latin America is reinforced to provide a broader understanding about this process and to generate data that support the development of focus and preventive interventions.

Background: In recent years it has been suggested that in COPD, lung volumes can be modified more than expiratory flows, with bronchodilators. Aim: To study the acute effects of salbutamol on FEV1 and lung volumes at rest. Subjects and Methods: Forty stable COPD patients were studied using a single dose of salbutamol (200 µg). Forced expiratory volumen in 1 second (FEV1), slow vital capacity (SVC), forced vital capacity (FVC), and inspiratory capacity (IC) were measured at baseline and after salbutamol administration. Results: After salbutamol, 39/40 patients exhibited a clinically significant increase in volumes (SVC, FVC or IC ž10% predicted). A significant increase in FEV1 (ž10% predicted) was observed in only 13 patients. Conclusions: Our results demonstrate that changes in lung volumes, and consequently in dynamic lung hyperinflation, take place more frequently than changes in maximal expiratory flows during the spirometric test in patients with COPD. Assessment of spirometric reversibility based only on changes in FEV1 underestimates the effect of bronchodilator drugs in these patients (Rev Méd Chile 2004; 132: 787-93)

Department of Clinical Haematology, Hospital Geral de São João, Porto, Portugal. npp46740@mail.telepac.pt Abstract We report a case of CD8(+)/V beta 5.1(+) T-cell large granular lymphocyte leukemia (T-LGL leukemia) presenting with mild lymphocytosis, severe autoimmune neutropenia, thrombocytopenia, polyarthritis and recurrent infections with a chronic disease course. Immunophenotyping showed an expansion of CD3(+)/TCR alpha beta(+)/CD8(+bright)/CD11c(+)/CD57(-)/CD56(-) large granular lymphocytes with expression of the TCR-V beta 5.1 family. Southern blot analysis revealed a clonal rearrangement of the TCR beta-chain gene. Hematopoietic growth factors, high dose intravenous immunoglobulin and corticosteroids were of limited therapeutic benefit to correct the cytopenias. During the disease course, the patient developed a severe cutaneous leg ulcer and bilateral vascular mammary skin lesions. Treatment with 2-deoxycoformycin resulted in both clinical and hematological complete responses, including the resolution of vascular skin lesions. Combined immuno-staining with relevant T-cell associated and anti-TCR-V beta monoclonal antibodies proved to be a sensitive method to assess the therapeutic effect of 2-deoxycoformicin and to evaluate the residual disease. O objetivo deste trabalho foi avaliar o efeito do boro na nodulação da ervilha (Pisum sativum L. cv. Torta de Flor Roxa), cultivada em solos de várzea em condições de casa de vegetação, entre maio e julho de 1998. Amostras de Neossolo Flúvico, Gleissolo Háplico, Gleissolo Melânico e Organossolo Mésico artificialmente drenado foram coletadas na camada de 0-20 cm. Essas amostras foram previamente cultivadas com rabanete, e receberam calcário, P, K, S, Cu e Zn e seis doses de B (0,0, 0,25, 0,5, 1,5, 3,0 e 6,0 mg dm-3). Por ocasião do cultivo da ervilha aplicou-se apenas K (100 mg dm-3). O delineamento experimental foi inteiramente casualizado, em esquema fatorial 4x6, com três repetições. Foram cultivadas três plantas por vaso, cujas sementes, na época da semeadura, foram submetidas à inoculação com estirpes de Rhizobium leguminosarum (BR 618 e BR 619). Para verificação da existência de estirpes nativas de rizóbio, cultivaram-se sem inoculação nas sementes, três vasos, de cada solo, que haviam recebido a mesma dose de calcário, a mesma adubação básica e 0,5 mg dm-3 de boro. Aos 45 dias após o plantio, em pleno florescimento, colheu-se o experimento. Não foram observadas estirpes nativas nos solos avaliados. Nos solos Gleissolo Háplico e Organossolo Mésico artificialmente drenado, as doses de B influenciaram a nodulação e a atividade da nitrogenase.
