ObjectifÉlaborer des recommandations pour l’examen clinique pelvien en gynécologie et obstétrique.Matériel et méthodesUn groupe de travail multidisciplinaire de 45 experts a été constitué, comprenant des représentants d’associations de patients et d’usagers du système de santé. L’ensemble du processus de ces recommandations a été mené indépendamment de tout financement. Il a été conseillé aux auteurs de suivre les règles du système GRADE® (Grading of Recommendations Assessment, Development and Evaluation) pour évaluer la qualité des preuves. Les limites potentielles de faire des recommandations fortes en présence de preuves de faible qualité ont été soulignées. Le comité a étudié 40 questions dans 4 domaines pour les femmes symptomatiques ou asymptomatiques (urgence, consultation gynécologique, maladies gynécologiques, obstétrique et grossesse). Chaque question a été formulée dans un format PICO (Patients, Intervention, Comparaison, Résultat) et les éléments de preuve ont été détaillés. La revue de la littérature et les recommandations ont été réalisées selon la méthodologie GRADE®.RésultatsLe travail de synthèse des experts et l’application de la méthode GRADE ont abouti à 27 recommandations. Parmi les recommandations formalisées, 17 présentaient un accord fort, 7 un accord faible et 3 un accord professionnel. Treize questions ont donné lieu à une absence de recommandation en raison du manque de preuves (pas de réponse dans la littérature).ConclusionsLes 27 recommandations ont permis de préciser quand un examen clinique est requis pour différentes situations cliniques gynécologiques et obstétricales. Ces recommandations intéressent tout professionnel impliqué dans la santé des femmes. La nécessité de réaliser un examen clinique chez certaines patientes dans certaines situations a été fondée sur des preuves scientifiques. Des recherches supplémentaires sont nécessaires pour étudier les avantages dans d’autres situations.

Estimates on the asymptotic behaviour of solution to linear integro-differential equations are fundamental in understanding the dynamics occuring in many nonlocal evolution problems. They are usually derived by using precise decay estimates on the heat kernel of the considered diffusion process. In this note, we show that for some generic jump diffusion and particular initial data, one can derive a lower bound of the asymptotic behaviour of the solution using a simple PDE argument. This is viewed as an independant preliminary brick to study invasion phenomena in nonlinear reaction diffusion problems.

Women’s preoperative perceptions of pelvic-floor disorders may differ from those of their physicians. Our objective was to specify women’s hopes and fears before cystocele repair, and to compare them to those that surgeons anticipate. We performed a secondary qualitative analysis of data from the PROSPERE trial. Among the 265 women included, 98% reported at least one hope and 86% one fear before surgery. Sixteen surgeons also completed the free expectations-questionnaire as a typical patient would. Women’s hopes covered seven themes, and women’s fears eleven. Women’s hopes were concerning prolapse repair (60%), improvement of urinary function (39%), capacity for physical activities (28%), sexual function (27%), well-being (25%), and end of pain or heaviness (19%). Women’s fears were concerning prolapse relapse (38%), perioperative concerns (28%), urinary disorders (26%), pain (19%), sexual problems (10%), and physical impairment (6%). Surgeons anticipated typical hopes and fears which were very similar to those the majority of women reported. However, only 60% of the women reported prolapse repair as an expectation. Women’s expectations appear reasonable and consistent with the scientific literature on the improvement and the risk of relapse or complication related to cystocele repair. Our analysis encourages surgeons to consider individual woman’s expectations before pelvic-floor repair.

Manual rotation of fetuses in occiput posterior positions at the start of the second stage of labor does not increase the rate of spontaneous vaginal delivery., OBJECTIVE: To evaluate whether manual rotation of fetuses in occiput posterior positions at full dilation increases the rate of spontaneous vaginal delivery. METHODS: In an open, single-center, randomized controlled trial, patients with a term, singleton gestation, epidural analgesia, and ultrasonogram-confirmed occiput posterior position at the start of the second stage of labor were randomized to either manual rotation or expectant management. Our primary endpoint was the rate of spontaneous vaginal delivery. Secondary endpoints were operative vaginal delivery, cesarean delivery, and maternal and neonatal morbidity. Analyses were based on an intention-to-treat method. A sample size of 107 patients per group (n=214) was planned to detect a 20% increase in the percent of patients with a spontaneous vaginal delivery (assuming 60% without manual rotation vs 80% with manual rotation) with 90% power and alpha of 0.05. RESULTS: Between February 2017 and January 2020, 236 patients were randomized to either manual rotation (n=117) or expectant management (n=119). The success rate of the manual rotation maneuver, defined by conversion to an anterior position as confirmed by ultrasonogram, was 68%. The rate of the primary endpoint did not differ between the groups (58.1% in manual rotation group vs 59.7% in expectant management group (risk difference −1.6; 95% CI −14.1 to 11.0). Manual rotation did not decrease the rate of operative vaginal delivery (29.9% in manual rotation group vs 33.6% in expectant management group (risk difference −3.7; 95% CI −16.6 to 8.2) nor the rate of cesarean delivery (12.0% in manual rotation group vs 6.7% in expectant management group (risk difference 5.3; 95% CI −2.2 to 12.6). Maternal and neonatal morbidity was also similar across the two groups. CONCLUSION: Manual rotation of occiput posterior positions at the start of second stage of labor does not increase the rate of vaginal delivery without instrumental assistance. We study acceleration phenomena in monostable integro-differential equations with ignition nonlinearity. Our results cover fractional Laplace operators and standard convolutions in a unified way, which is also a contribution of this paper. To achieve this, we construct a sub-solution that captures the expected dynamics of the accelerating solution, and this is here the main difficulty. The detection of a stimulus in the environment largely depends on its salience (e.g. an intense flash of light is easier to detect than a faint light) but the state of the brain has also a strong influence on the perception and response to stimuli. Sleep is a typical and extreme example of such influence. It is thought that the sensory isolation that operates during sleep serves vital functions and that the processing of irrelevant stimuli is impaired during sleep. Yet, what makes a stimulus relevant for the sleeper remains to be elucidated. Sleep is a composite state divided in several sleep stages with distinct behavioral, physiological, electrophysiological, and phenomenal manifestations that reflect the action of different functions. The perceptual dimensions that make a stimulus relevant should therefore depends on the sleep functions that operate and therefore, on sleep stages. In this thesis, I investigated the relation between vigilance states and which perceptual dimensions are relevant to the sleeping brain by recording the cerebral response during sleep to stimuli of different natures. First, I recorded the brain response to interoceptive signals (i.e. heartbeat) in sleep stages with distinct degrees of sensory disconnection. I found a consistent change of brain response to visceral signals between wakefulness and sleep and between sleep states of high and low sensory disconnection. Thus, although interoceptive signals convey to the brain crucial information about the body and would be expected to be constantly relevant, their processing changes with sleep stages. Then, I investigated the brain response to intrinsically aversive stimuli (screams) during sleep. I found that screams evoked more sleep oscillations than controlled “neutral” vocalizations. This result suggests that intrinsically alarming signals conserve their relevant status during sleep. Next, I operated a conditioning procedure on participants to associate an auditory cue with humorous pictures and another auditory cue with neutral pictures. These participants were medically resistant epileptic patients implanted with intracranial electroencephalography for pre-surgical evaluation. I benefited from rare intracranial recordings to investigate the response of specific brain regions to the two conditioned auditory cues during sleep. I found that the orbitofrontal cortex, a brain region crucially implicated in the evaluation of the affective properties of stimuli evoked more sleep oscillations in response to the auditory cues paired with emotional items. This result suggests that emotion-related brain regions react to relevant stimuli during sleep. Finally, I recorded participants with electroencephalography and polysomnography while they were stimulated with odors overnight. I recorded a response of the brain to odors in wakefulness and sleep which supports the detection of odors by the sleeping brain. In addition, odors modified the sleep architecture: they reduced the sleep latency but fragmented deep sleep. This new knowledge informs about which stimuli can fragment human sleep, can influence sleep state, or can evoke complex sensory representations. In the long term, they could serve to inform public health policies and design overnight stimulation to treat disorders including insomnia.

International audience; Objectives - To provide up-to-date evidence-based guidelines for the management of smoking cessation during pregnancy. Methods - Systematic review of the international literature. We identified papers published between January 2003 and April 2019 in Cochrane PubMed, and Embase databases with predefined keywords. All reports published in French and English relevant to the areas of focus were included and classified according the level of evidence ranging from 1 (highest) to 4 (lowest). The strength of the recommendations was classified according to the Haute Autorité de santé, France (ranging from A, highest to C, lowest). Results - "Counseling", involving globally all kind of non-pharmacological interventions, has a modest benefit on smoking cessation, birth weight and prematurity. Moderate physical activity did not show a significant effect on smoking cessation. The systematic use of feedback by measuring the expired air carbon monoxide concentration do not influence smoking abstinence but it may be used in establishing a therapeutic alliance. The use of self-help interventions and health education are recommended in helping pregnant smokers quit. The prescription of nicotine replacement therapies (NRT) may be offered to any pregnant woman who has failed stopping smoking without medication This prescription can be initiated by the health care professional taking care of the pregnant woman in early pregnancy. There is no scientific evidence to propose the electronic cigarette for smoking cessation to pregnant smokers; it is recommended to provide the same advice and to use methods that have already been evaluated. The use of waterpipe (shisha/narghile) during pregnancy is associated with decreased fetal growth. It is recommended not to use waterpipe during pregnancy. Breastfeeding is possible in smokers, but less often initiated by them. Although its benefit for the child's development is not demonstrated to date, breastfeeding allows the mother to reduce or stop smoking. The risk of postpartum relapse is high (up to 82% at 1 year). The main factors associated with postpartum abstinence are breastfeeding, not having a smoker at home, and having no symptoms of postpartum depression. Conclusions - Smoking during pregnancy concerns more than hundred thousand women and their children per year in France. It is a major public health burden. Health care professionals should be mobilized for reducing or even eradicating it.

Common established causes include uterine anomalies, antiphospholipid syndrome, hormonal and metabolic disorders, and cytogenetic abnormalities. Other etiologies have been proposed but are still considered controversial, such as chronic endometritis, inherited thrombophilias, luteal phase deficiency, and high sperm DNA fragmentation levels. Over the years, evidence-based treatments such as surgical correction of uterine anomalies or aspirin and heparin for antiphospholipid syndrome have improved the outcomes for couples with recurrent pregnancy loss. However, almost half of the cases remain unexplained and are empirically treated using progesterone supplementation, anticoagulation, and/or immunomodulatory treatments. Regardless of the cause, the long-term prognosis of couples with recurrent pregnancy loss is good, and most eventually achieve a healthy live birth. However, multiple pregnancy losses can have a significant psychological toll on affected couples, and many efforts are being made to improve treatments and decrease the time needed to achieve a successful pregnancy. This article reviews the established and controversial etiologies, and the recommended therapeutic strategies, with a special focus on unexplained recurrent pregnancy losses and the empiric treatments used nowadays. It also discusses the current role of preimplantation genetic testing in the management of recurrent pregnancy loss. International audience; The time discretization of gradient flows in metric spaces uses variants of the celebrated implicit Euler-type scheme of Jordan, Kinderlehrer and Otto. We propose in this Note a different approach which allows to construct two second-order in time numerical schemes. In a metric space framework, we show that the schemes are well defined and prove the convergence for one of them under some regularity assumptions. For the particular case of a Fokker–Planck gradient flow in the Wasserstein space, we obtain (theoretically and numerically) the second-order convergence.; La discrétisation temporelle des flots de gradient dans des espaces métriques utilise des variantes du schéma d’Euler implicite issu du travail séminal de Jordan, Kinderlehrer et Otto. Nous proposons dans cette Note une approche différente permettant de construire deux schémas numériques d’ordre deux en temps. Dans le cadre d’un espace métrique, nous montrons que les schémas sont bien définis et prouvons la convergence de l’un d’entre eux sous des hypothèses de régularité. Objective: The main objective of this work was to evaluate the risk factors of incidence and remission of urinary incontinence (UI) in a cohort of women at midlife, taking into account the type of UI (Stress urinary incontinence - SUI – Urge urinary incontinence – IUU - and mixed urinary incontinence - IUM) and the severity of symptoms.Material and Methods: Data were obtained from the survey questionnaires "women and their health," the annual questionnaires cohort GAZEL and specific questionnaire "urinary problems" posed in 2000 and 2008. A Cox model was used between 1990 and 2008 from the data of the question "Have you trouble holding your urine? ". Logistic regression models were used between 2000 and 2008 from the issue validated UI "Over the last 12 months, did you have any involuntary loss of urine ? ".Results: In our population, the annual rate of incidence of UI was 5.5% and the annual rate of remission was 6.2%, confirming that the UI is a dynamic condition with possible periods of worsening and of improvement. These rates vary with the type of UI: between 2000 and 2008, incidence rates were 14.9% for SUI, 3.2% for UUI and 3.1% for the MUI. SUI is the most likely type to experience a possible remission of the UI compared to UUI and MUI. Factors associated with the occurrence of UI are social factors (high educational level), obstetrical (parity), hormonal (menopause), and factors in relation with physical or mental health status (weight gain, onset of depressive symptoms, impairment in health-related quality of life - energy dimension and social isolation dimension of NHP score). Complete remission is even less common as women gain in age and in weight. For each type of UI, the influence of risk factors seems different. Thus, educational level (baccalaureate), BMI (at baseline and during follow-up) and a depressive syndrome at baseline are associated with the onset of SUI. An antecedent of surgical procedure for UI is associated with the occurrence of UUI and MUI. Surgical correction of the UI during follow-up is, as expected, associated with remission of symptoms of SUI. By contrast, pelvic floor muscle training exercises and vaginal childbirth, are associated with persistence of symptoms of SUI. The vaginal delivery was also associated with the persistence of MUI. MUI and UUI are the more severe types of UI at baseline. Furthermore, the aggravation is more pronounced in case of UUI or MUI that for SUI. Aside from the type, the factors associated with worsening symptoms are a prior surgery for a UI (at baseline) and the onset of a depressive syndrome. A worsening of UI is negatively associated with alcohol consumption at baseline and incontinence surgery during the study period. The role of social factors, such as the baccalaureate for example, is associated with the appearance of a slight UI, while obstetric factors such as a vaginal childbirth is associated with the appearance of a severe UI.Conclusion: The UI is a dynamic condition with possible periods of worsening and of partial or total improvement. The analysis of risk factors associated with the UI must incorporate a differentiation of the type and severity. Data on the epidemiology of UI in women between 45 and 60 years are still too few. Ainsi, le niveau d’étude (être titulaire du baccalauréat), l’IMC (à l’inclusion, et au cours du suivi) et un syndrome dépressif à l’inclusion sont associés à l’apparition d’une IUE. Un antécédent de prise en charge chirurgicale de l’IU est associé à l’apparition d’une IUU et d’une IUM. La correction chirurgicale de l’IU pendant le suivi est, comme attendu, associée à une rémission des symptômes d’IUE, mais en revanche la rééducation périnéale et un antécédent d’accouchement par voie vaginale, sont associés à une persistance des symptômes d’IUE. L’accouchement par voie vaginale était également associé à la persistance de l’IUM. L’IUU et l’IUM sont les types d’IU les plus graves à l’inclusion. De plus, l’aggravation est plus marquée en cas d’IUU ou d’IUM que d’IUE. Hormis le type, les facteurs associés à une aggravation des symptômes sont une operation pour une IU (à l’inclusion) et l’apparition d’un syndrome dépressif. L’aggravation des symptômes est négativement associée à la consommation d’alcool à l’inclusion et une chirurgie de l’incontinence pendant la période d’étude. Le rôle des facteurs sociaux, comme l’obtention du baccalauréat par exemple, est associé à l’apparition d’un IU légère, alors que des facteurs obstétricaux comme un antécédent d’accouchement par voie basse est associé à une apparition de l’IU grave.Conclusion : L’IU est un phénomène dynamique avec des périodes possibles d’aggravation et d’amélioration partielle ou totale. L’analyse des facteurs de risque associés à l’IU doit intégrer une différentiation du type et de la gravité. Les données concernant l’épidémiologie de l’IU chez les femmes entre 45 et 60 ans sont encore trop peu nombreuses. D'autres enquêtes longitudinales incluant un nombre plus important de femmes sont essentielles afin de confirmer nos résultats et mieux conseiller les femmes en présentant les symptômes.

Un antécédent de prise en charge chirurgicale de l’IU est associé à l’apparition d’une IUU et d’une IUM. La correction chirurgicale de l’IU pendant le suivi est, comme attendu, associée à une rémission des symptômes d’IUE, mais en revanche la rééducation périnéale et un antécédent d’accouchement par voie vaginale, sont associés à une persistance des symptômes d’IUE. L’accouchement par voie vaginale était également associé à la persistance de l’IUM. L’IUU et l’IUM sont les types d’IU les plus graves à l’inclusion. De plus, l’aggravation est plus marquée en cas d’IUU ou d’IUM que d’IUE. Hormis le type, les facteurs associés à une aggravation des symptômes sont une operation pour une IU (à l’inclusion) et l’apparition d’un syndrome dépressif. L’aggravation des symptômes est négativement associée à la consommation d’alcool à l’inclusion et une chirurgie de l’incontinence pendant la période d’étude. Le rôle des facteurs sociaux, comme l’obtention du baccalauréat par exemple, est associé à l’apparition d’un IU légère, alors que des facteurs obstétricaux comme un antécédent d’accouchement par voie basse est associé à une apparition de l’IU grave.Conclusion : L’IU est un phénomène dynamique avec des périodes possibles d’aggravation et d’amélioration partielle ou totale. L’analyse des facteurs de risque associés à l’IU doit intégrer une différentiation du type et de la gravité. Les données concernant l’épidémiologie de l’IU chez les femmes entre 45 et 60 ans sont encore trop peu nombreuses. D'autres enquêtes longitudinales incluant un nombre plus important de femmes sont essentielles afin de confirmer nos résultats et mieux conseiller les femmes en présentant les symptômes.

International audience; OBJECTIVE: Urinary incontinence (UI) is often considered to be an age-related disease that develops gradually as women grow older. Much remains to be learnt about factors that promote its incidence or its remission. Our objective was to assess its incidence and risk factors. DESIGN: Longitudinal cohort study. SETTING: French GAZEL cohort. POPULATION: A cohort of 4127 middle-aged women (aged 47-52 years at baseline) over an 18-year period (1990-2008). METHODS: UI was defined as 'difficulty retaining urine'. The question was asked at baseline and repeated every 3 years over an 18-year period. Two groups (UI incidence and remission) were analysed according to status at baseline (continent or incontinent). A multivariable analysis (Cox model) was used to estimate the risk factors for UI incidence and remission. MAIN OUTCOME MEASURES: Annual incidence and remission rates and risk factors for UI incidence and remission. RESULTS: The annual incidence and remission rates for UI were 3.3% and 6.2%, respectively. High educational level (hazard ratio [HR] = 1.28; 95% confidence interval [95% CI] = 1.05-1.55), parity, i.e. at least one baby versus no baby (HR = 1.64; 95% CI = 1.19-2.27), menopause (HR = 5.44; 95% CI = 4.47-6.63), weight gain, i.e. for each kilogram change in weight (HR = 1.00; 95% CI = 1.00-1.02), onset of depressive symptoms (HR = 1.31; 95% CI = 1.09-1.57) and impairment in health-related quality of life incidence (social isolation dimension [HR = 1.29; 95% CI = 1.04-1.60] and energy dimension [HR = 1.41; 95% CI = 1.17-1.70]) were associated with an increased probability of UI. The factors associated with persistent UI were age (HR = 0.58; 95% CI = 0.55-0.61), weight gain (HR = 0.99; 95% CI = 0.98-0.99) and transition to menopausal status (HR = 1.54; 95% CI = 1.19-1.99). CONCLUSIONS: Our study suggests that, in our population of middle-aged women, age, menopause, weight gain, onset of depression and impaired health-related quality of life may promote UI.

The works presented in this document are concerned with the numerical simulation of acoustic propagation in a moving fluid, in the frequency domain. The model retained for these studies is the Galbrun's equation which describes the linear wave propagation in presence of a perfect fluid flow in adiabatic motion and uses the lagrangian displacement as a variable. Mathematical analysis shows that a nodal finite element method is not, in general, suited to approximate the solution of the equation, the computed results being strongly polluted by spurious numerical modes. In the first part of the thesis, we propose a regularization method for which we prove the convergence of a nodal finite element method for diffraction problems in a duct and in presence of uniform or sheared subsonic flows. The second part of the document is devoted to the construction and analysis of perfectly matched absorbing layers for the radiation of a compact source, placed in a duct in a uniform flow. We successively treat the case of an acoustic source, which leads to a scalar problem, and of a more general source of perturbations. A limiting absorption principle is established in the general case and we prove that the convergence of the PML method is exponential with respect to the size of the layers. Numerical results that illustrate these two approaches are presented., Les travaux de cette thèse concernent la simulation numérique de la propagation acoustique dans un fluide en écoulement, en régime périodique établi. Le modèle retenu est l'équation de Galbrun, qui modélise la propagation linéaire d'ondes en présence d'un écoulement de fluide parfait en évolution adiabatique et porte sur le déplacement lagrangien. L'analyse mathématique montre qu'une méthode d'éléments finis nodaux ne permet pas, en général, d'approcher la solution de l'équation, les résultats étant alors fortement pollués par des modes numériques "parasites". Dans la première partie de la thèse, nous proposons une méthode de régularisation de l'équation pour laquelle nous prouvons la convergence d'une approximation par éléments finis nodaux pour des problèmes de diffraction dans un conduit en présence d'écoulements subsoniques uniforme ou cisaillé. La deuxième partie du document est consacrée à la construction et l'étude de couches absorbantes parfaitement adaptées, dites PML, pour le rayonnement d'une source localisée en présence d'un écoulement uniforme et dans un conduit. Nous traitons successivement le cas d'une source irrotationnelle, qui conduit à un problème scalaire, et celui d'une source quelconque. Un principe d'absorption limite est établi dans le cas général et nous démontrons un résultat de convergence exponentielle de la méthode de PML en fonction de la longueur des couches. Des résultats numériques illustrant ces approches sont présentés.

This paper is devoted to the resolution of the time-harmonic linearized Galbrun's equation, which models, via a mixed Lagrangian-Eulerian representation, the propagation of acoustic and hydrodynamic perturbations in a given flow of a compressible fluid. We consider here the case of a uniform subsonic flow in an infinite, two-dimensional duct. Using a limiting amplitude process, we characterize the outgoing solution radiated by a compactly supported source. Then, we propose a Fredholm formulation with perfectly matched absorbing layers for approximating this outgoing solution. The convergence of the approximated solution to the exact one is proved, and error estimates with respect to the parameters of the absorbing layers are derived. Several significant numerical examples are included.

In this paper, we propose and analyze perfectly matched absorbing layers for a problem of time harmonic acoustic waves propagating in a duct in presence of a uniform flow. The absorbing layers are designed for the pressure field, satisfying the convected scalar Helmholtz equation. A difficulty, compared to the Helmholtz equation, comes from the presence of so-called inverse upstream modes which become unstable, instead of evanescent, with the classical Bérenger's PMLs. We introduce here a new PML model which makes all outgoing waves evanescent. We then analyse the error due to the truncation of the domain and prove that the convergence is exponential with respect to the size of the layers, for both the classical and the new PML models. Numerical validations are finally presented.

We deal with the well-posedness of the transient Maxwell equations in a particular class of heterogeneous chiral material modeled by the Drude-Born-Fedorov constitutive relations. A new formulation of the underlying evolution problem allows us to correct a previous result establishing the existence and uniqueness of the electromagnetic fields in a homogeneous medium.
