1. Selective Inhibition of Soluble TNF using XPro1595 Improves Hippocampal Pathology to Promote Improved Neurological Recovery Following Traumatic Brain Injury in Mice

Abstract

Aims: To determine the efficacy of XPro1595 to improve pathophysiological and functional outcomes in a mouse model of traumatic brain injury (TBI). Background: Symptoms associated with TBI can be debilitating, and treatment without off-target side effects remains a challenge. This study aimed to investigate the efficacy of selectively inhibiting the soluble form of TNF (solTNF) using the biologic XPro1595 in a mouse model of TBI. Objectives: Use XPro1595 to determine whether injury-induced solTNF promotes hippocampal inflammation and dendritic plasticity and associated functional impairments. Methods: Mild-to-moderate traumatic brain injury (CCI model) was induced in adult male C57Bl/6J WT and Thy1-YFPH mice, with XPro1595 (10 mg/kg, S.C.) or vehicle being administered in a clinically relevant window (60 minutes post-injury). The animals were assessed for differences in neurological function, and hippocampal tissue was analyzed for inflammation and glial reactivity, as well as neuronal degeneration and plasticity. Results: We report that unilateral CCI over the right parietal cortex in mice promoted deficits in learning and memory, depressive-like behavior, and neuropathic pain. Using immunohistochemical and Western blotting techniques, we observed the cortical injury promoted a set of expected pathophysiology’s within the hippocampus consistent with the observed neurological outcomes, including glial reactivity, enhanced neuronal dendritic degeneration (dendritic beading), and reduced synaptic plasticity (spine density and PSD-95 expression) within the DG and CA1 region of the hippocampus, that were prevented in mice treated with XPro1595. Conclusion: Overall, we observed that selectively inhibiting solTNF using XPro1595 improved the pathophysiological and neurological sequelae of brain-injured mice, which provides support for its use in patients with TBI.

Abstract

We report on the 1,000-element transition-edge sensor (TES) x-ray spectrometer implementation of the TOMographic Circuit Analysis Tool (TOMCAT). TOMCAT combines a high spatial resolution scanning electron microscope (SEM) with a highly efficient and pixelated TES spectrometer to reconstruct three-dimensional maps of nanoscale integrated circuits (ICs). A 240-pixel prototype spectrometer was recently used to reconstruct ICs at the 130 nm technology node, but to increase imaging speed to more practical levels, the detector efficiency needs to be improved. For this reason, we are building a spectrometer that will eventually contain 3,000 TES microcalorimeters read out with microwave superconducting quantum interference device (SQUID) multiplexing, and we currently have commissioned a 1,000 TES subarray. This still represents a significant improvement from the 240-pixel system and allows us to begin characterizing the full spectrometer performance. Of the 992 maximimum available readout channels, we have yielded 818 devices, representing the largest number of TES x-ray microcalorimeters simultaneously read out to date. These microcalorimeters have been optimized for pulse speed rather than purely energy resolution, and we measure a FWHM energy resolution of 14 eV at the 8.0 keV Cu K$\alpha$ line., Comment: 5 pages, 4 figures, submitted to IEEE Transactions on Applied Superconductivity

5. Porcine Bladder Replacement with a Bilayer Silk Fibroin Enhanced Prosthetic Reservoir: A Feasibility Study

Abstract

The creation of synthetic reservoirs for bladder replacement has been limited by challenges of interfacing synthetic materials and native tissue. We sought to overcome this challenge by utilizing a novel bilayer silk fibroin scaffold (BLFS) as an intermediary towards the development of an acellular prosthetic reservoir.Under institutionally-approved protocols, 3D-printed reservoirs were implanted in six juvenile female pigs after cystectomy. BLFS was attached to the in-situ prosthetic reservoir serving as an intermediary to native ureteral and urethral tissue anastomoses. Our first protocol allowed four pigs to be survived up to seven days, and the second protocol allowed two pigs to be survived for up to one year. At the first sign of functional decline or the end of the study period, the animals were euthanized, and kidneys, ureters, prosthetic bladder, and urethra were harvested en-bloc for histopathology.The first two pigs had anastomotic urine leaks due to design flaws resulting in early termination. The third pig had acute renal failure resulting in early termination. The artificial bladder design was modified in subsequent iterations. The fourth pig survived for seven days and upon autopsy, had intact urethral and ureteral anastomoses. The fifth and sixth pigs survived for 11 and 12 weeks, respectively, before they were sacrificed due to failure to thrive. One animal developed an enteric fistula. The other animal had an intact anastomosis, and the BLFS was identified at the ureteral and urethral anastomoses on histopathogic analysis.Replacing the porcine bladder with a prosthetic bladder was successfully achieved for up to three months, the second longest survival period for a nonbiological bladder alternative. Bilayer silk fibroin scaffold was used for the first time to create an interface between synthetic material and biologic tissue by allowing ingrowth of urothelium onto the acellular alloplastic bladder.

Voksenuddannelse har historisk set blandt andet været tiltænkt en rolle i forhold til at mindske den sociale ulighed i uddannelse ved at tilbyde en ny chance til dem, der i første omgang ikke havde fået så meget uddannelse. I dette kapitel diskuteres udviklingen i det politiske fokus på voksenuddan¬nel¬se, samt hvorfor voksenuddannelse i dag reelt ikke synes at mindske den sociale ulighed, hvilket leder frem til nogle afsluttende overvejelser om, hvad der skal til for at voksenuddannelse kan udfylde den rolle. Through the years, different aims has been attributed to adult education, including formation of democratic citizens, development of (basic) competences like reading and writing, and more work related training. A common theme over the years, however, has been a focus on those with least education prior to their adulthood, whether from a perspective of fairness and education as a human right, giving the least educated a second chance, or from a human capital perspective stressing the need of the national economy for highly qualified workers. Based on prior studies as well as an analysis of select policy papers, this article looks into the political interest in adult education in Denmark; the social inequality in participation in adult education; and why those intended to participate, those with least prior education, are also those least likely to participate. Closing the article is a discussion of whether social inequality in participation in adult education can be reduced, concluding that instead of looking into barriers and describing the least education as lacking something, it might be more helpful to see the group as resourceful and look into what would motivate the group to take up education.

13. The role of exercise in obesity-related cancers: Current evidence and biological mechanisms

Abstract

Cancer ranks among the five leading causes of death in almost all countries and has important repercussions for individual and public health, the healthcare system and society in general. Obesity increases the incidence of many types of cancer, but growing evidence suggests that physical activity may decrease risk for developing a variety of obesity-related cancer types, and in some cases also, improve cancer prognosis and mortality rates. This review summarizes recent evidence on the effect of physical activity on obesity-related cancer prevention and survival. For some cancers, including breast, colorectal, and endometrial cancer, there is strong evidence for a preventative effect of exercise, but for many others, including gallbladder and kidney cancer, and multiple myeloma, evidence is inconsistent or largely lacking. Though many potential mechanisms have been proposed to explain the onco-protective effect of exercise, including improved insulin sensitivity, alterations in sex hormone availability, improved immune function and inflammation, myokine secretion, and modulation of intracellular signaling at the level of AMP kinase, the exact mechanism(s) of action within each cancer subtype remains poorly defined. Overall, a deeper understanding of how exercise can help against cancer and of the exercise parameters that can be tweaked to optimize exercise prescription is necessary and should be the subject of future investigation.

Abstract

Progress in scientific disciplines is accompanied by standardization of terminology. Network neuroscience, at the level of macro-scale organization of the brain, is beginning to confront the challenges associated with developing a taxonomy of its fundamental explanatory constructs. The Workgroup for HArmonized Taxonomy of NETworks (WHATNET) was formed in 2020 as an Organization for Human Brain Mapping (OHBM)-endorsed best practices committee to provide recommendations on points of consensus, identify open questions, and highlight areas of ongoing debate in the service of moving the field towards standardized reporting of network neuroscience results. The committee conducted a survey to catalog current practices in large-scale brain network nomenclature. A few well-known network names (e.g., default mode network) dominated responses to the survey, and a number of illuminating points of disagreement emerged. We summarize survey results and provide initial considerations and recommendations from the workgroup. This perspective piece includes a selective review of challenges to this enterprise, including 1) network scale, resolution, and hierarchies; 2) inter-individual variability of networks; 3) dynamics and non-stationarity of networks; 4) consideration of network affiliations of subcortical structures; and 5) consideration of multi-modal information. We close with minimal reporting guidelines for the cognitive and network neuroscience communities to adopt.

18. The role of community science in orthopteran research

Abstract

Orthopterans are commonly encountered in rural, suburban, and urban landscapes and have charismatic songs that attract the public’s attention. These are ideal organisms for connecting the public with science and critical concepts in ecology and evolution, such as habitat conservation and climate change. In this review, we provide an overview of community science and review community science in orthopterans. Best practices for orthopteran community science are provided, with a focus on audio recordings and highlighting new ways in which scientists who study orthopterans can engage in community science. Before the modern era, scientific discovery was commonly made by people who were not scientists by profession (Brenna 2011, Miller-Rushing et al. 2012). This began to change in the middle of the nineteenth century when science became highly academic, with greater “gatekeeping” of knowledge, and data collection became increasingly expensive. As a result, much of the knowledge gained during that time has been effectively withheld from non-scientists in difficult-to-obtain scientific journals, and there were few opportunities for the public to directly engage with scientific research. In recent years, there has been a concerted effort from the scientific community to change the way we engage with the public. These “citizen” or “community” science projects are filling gaps in the modern approach to scientific inquiry (Jordan et al. 2012, Toomey and Domroese 2013, Johnson et al. 2014). Here, we provide an overview of community science and highlight the exciting and unique role that community science can play in orthopteran research. We focus on how acoustic surveys can be used to study orthopteran biodiversity, provide best practices for orthopteran community science, and suggest future avenues for research.

23. Factors Associated With Blood Pressure Control Among Patients in Community Health Centers

Abstract

Understanding the multilevel factors associated with controlled blood pressure is important to determine modifiable factors for future interventions, especially among populations living in poverty. This study identified clinically important factors associated with blood pressure control among patients receiving care in community health centers.This study includes 31,089 patients with diagnosed hypertension by 2015 receiving care from 103 community health centers; aged 19-64 years; and with ≥1 yearly visit with ≥1 recorded blood pressure in 2015, 2016, and 2017. Blood pressure control was operationalized as an average of all blood pressure measurements during all the 3 years and categorized as controlled (blood pressure140/90), partially controlled (mixture of controlled and uncontrolled blood pressure), or never controlled. Multinomial mixed-effects logistic regression models, conducted in 2022, were used to calculate unadjusted ORs and AORs of being in the never- or partially controlled blood pressure groups versus in the always-controlled group.A total of 50.5% had always controlled, 39.7% had partially controlled, and 9.9% never had controlled blood pressure during the study period. The odds of being partially or never in blood pressure control were higher for patients without continuous insurance (AOR=1.09; 95% CI=1.03, 1.16; AOR=1.18; 95% CI=1.07, 1.30, respectively), with low provider continuity (AOR=1.24; 95% CI=1.15, 1.34; AOR=1.28; 95% CI=1.13, 1.45, respectively), with a recent diagnosis of hypertension (AOR=1.34; 95% CI=1.20, 1.49; AOR=1.19; 95% CI=1.00, 1.42), with inconsistent antihypertensive medications (AOR=1.19; 95% CI=1.11, 1.27; AOR=1.26; 95% CI=1.13, 1.41, respectively), and with fewer blood pressure checks (AOR=2.14; 95% CI=1.97, 2.33; AOR=2.17; 95% CI=1.90, 2.48, respectively) than for their counterparts.Efforts targeting continuous and consistent access to care, antihypertensive medications, and regular blood pressure monitoring may improve blood pressure control among populations living in poverty.

Abstract

Cystic fibrosis (CF) is a recessive disorder arising from mutations in the gene encoding the cystic fibrosis transmembrane conductance regulator (CFTR) protein. CFTR is expressed in numerous tissues, with high expression in the airways, small and large intestine, pancreatic and hepatobiliary ducts, and male reproductive tract. CFTR loss in these tissues disrupts regulation of salt, bicarbonate, and water balance across their epithelia, resulting in a systemic disorder with progressive organ dysfunction and damage. Pancreatic exocrine damage ultimately manifests as pancreatic exocrine insufficiency that begins as early as infancy. Pancreatic remodeling accompanies this early damage, during which abnormal glucose tolerance can be observed in toddlers. With increasing age, however, insulin secretion defects progress such that CF-related diabetes (CFRD) occurs in 20% of teens and up to half of adults with CF. The relevance of CFRD is highlighted by its association with increased morbidity, mortality, and patient burden. While clinical research on CFRD has greatly assisted in the care of individuals with CFRD, key knowledge gaps on CFRD pathogenesis remain. Furthermore, the wide use of CFTR modulators to restore CFTR activity is changing the CFRD clinical landscape and the field’s understanding of CFRD pathogenesis. For these reasons, the National Institute of Diabetes and Digestive and Kidney Diseases and the Cystic Fibrosis Foundation sponsored a CFRD Scientific Workshop, 23–25 June 2021, to define knowledge gaps and needed research areas. This article describes the findings from this workshop and plots a path for CFRD research that is needed over the next decade.

26. Sexual harassment disproportionately affects ecology and evolution graduate students with multiple marginalized identities in the United States

Abstract

Sexual harassment within academic institutions has profound impacts that may lead to the attrition of groups historically excluded from the biological sciences and related disciplines. To understand sexual harassment's effects on vulnerable communities within academia, we examined graduate student experiences with sexual harassment. In a survey of ecology and evolutionary biology programs across the United States, we found that 38% of the graduate student respondents were sexually harassed during their time in these programs. Sexual harassment disproportionately affected graduate students with multiple intersecting marginalized identities, and these experiences led to delays in completing graduate programs and shifts away from their desired careers. Our research highlights the need for academic institutions, and science more broadly, to make widespread changes to sexual harassment policies, including treating sexual harassment as scientific misconduct and creating resources for individuals within students’ informal support networks, in tandem with efforts to dismantle barriers to advancing diversity, equity, and inclusion.

27. A taxonomy for deep cerebral cavernous malformations: subtypes of thalamic lesions

Abstract

OBJECTIVE Anatomical taxonomy is a practical tool to successfully guide clinical decision-making for patients with brain arteriovenous malformations and brainstem cavernous malformations (CMs). Deep cerebral CMs are complex, difficult to access, and highly variable in size, shape, and position. The authors propose a novel taxonomic system for deep CMs in the thalamus based on clinical presentation (syndromes) and anatomical location (identified on MRI). METHODS The taxonomic system was developed and applied to an extensive 2-surgeon experience from 2001 through 2019. Deep CMs involving the thalamus were identified. These CMs were subtyped on the basis of the predominant surface presentation identified on preoperative MRI. Six subtypes among 75 thalamic CMs were defined: anterior (7/75, 9%), medial (22/75, 29%), lateral (10/75, 13%), choroidal (9/75, 12%), pulvinar (19/75, 25%), and geniculate (8/75, 11%). Neurological outcomes were assessed using modified Rankin Scale (mRS) scores. A postoperative score ≤ 2 was defined as a favorable outcome and > 2 as a poor outcome. Clinical and surgical characteristics and neurological outcomes were compared among subtypes. RESULTS Seventy-five patients underwent resection of thalamic CMs and had clinical and radiological data available. Their mean age was 40.9 (SD 15.2) years. Each thalamic CM subtype was associated with a recognizable constellation of neurological symptoms. The common symptoms were severe or worsening headaches (30/75, 40%), hemiparesis (27/75, 36%), hemianesthesia (21/75, 28%), blurred vision (14/75, 19%), and hydrocephalus (9/75, 12%). The thalamic CM subtype determined the selection of surgical approach. A single approach was associated with each subtype for most patients. The main exception to this paradigm was that in the surgeons’ early experience, pulvinar CMs were resected through a superior parietal lobule–transatrial approach (4/19, 21%), which later evolved to the paramedian supracerebellar-infratentorial approach (12/19, 63%). Relative outcomes implied by mRS scores were unchanged or improved in most patients (61/66, 92%) postoperatively. CONCLUSIONS This study confirms the authors’ hypothesis that this taxonomy for thalamic CMs can meaningfully guide the selection of surgical approach and resection strategy. The proposed taxonomy can increase diagnostic acumen at the patient bedside, help identify optimal surgical approaches, enhance the clarity of clinical communications and publications, and improve patient outcomes.

32. Xylem Functionality Is Not a Direct Indicator of Apple Preharvest Fruit Drop

Abstract

Apple (Malus ×domestica) growers can incur significant economic losses when fruit drop before they can be harvested [preharvest fruit drop (PFD)]. In some years and cultivars, more than 30% of potential yield can be lost. Growers frequently apply plant bioregulators to reduce PFD, either via delay in maturity [aminoethoxyvinylglycine (AVG), 1-methylcycolpropene] or via inhibition in production of cell hydrolysis enzymes in the fruit pedicel [naphthalene acetic acid (NAA)]. Finding a physiological indicator of PFD would allow growers to assess the susceptibility of fruit to PFD. Due to its lignification, xylem is believed to be the last tissue to break down in the fruit pedicel, leading to PFD. To determine whether loss in xylem functionality can be used as an indicator of PFD potential, studies were conducted in 2020 and 2021 with ‘Red Delicious’ treated with AVG (132 µL·L−1), NAA (10 µL·L−1), and an ethylene-producing compound [ethephon (150 µL·L−1 in 2020, 200 µL·L−1 in 2021)] to generate a range of PFD potentials. Xylem functionality was assessed in the fruit cortex. Internal ethylene content (IEC), fruit maturity indices, and PFD rates were quantified weekly throughout the harvest period. Expression of genes encoding for cell hydrolysis enzymes (MdEG1 and MdPG2) was quantified to relate xylem functionality to fruit abscission mechanisms. In 2020 and 2021, AVG reduced PFD compared with the untreated control by decreasing IEC. Although ethephon did not result in higher PFD than untreated fruit, NAA reduced PFD in 2020 but not 2021. For all treatments in both years, there was a linear decrease in xylem functionality throughout the measurement period. Cumulative PFD exponentially decreased as xylem functionality neared zero and the climacteric rise in ethylene began. Concurrent with the rise in IEC and PFD was an increase in the expression of MdEG1 and MdPG2 in the fruit pedicel of the control compared with AVG-treated fruit. AVG-treated fruit lost xylem functionality at a similar rate to the untreated control but had lower expression of MdEG1 and MdPG2. These results indicate that xylem functionality is not a sole direct indicator of PFD. The concurrent increase in PFD and expression of MdEG1/MdPG2 supports previous research indicating that these two genes may serve as potential markers for PFD.

Abstract

Background: Recently, common genetic risk factors for intracranial aneurysm (IA) and aneurysmal subarachnoid hemorrhage (ASAH) were found to explain a large amount of disease heritability and therefore have potential to be used for genetic risk prediction. We constructed a genetic risk score to (1) predict ASAH incidence and IA presence (combined set of unruptured IA and ASAH) and (2) assess its association with patient characteristics. Methods: A genetic risk score incorporating genetic association data for IA and 17 traits related to IA (so-called metaGRS) was created using 1161 IA cases and 407 392 controls from the UK Biobank population study. The metaGRS was validated in combination with risk factors blood pressure, sex, and smoking in 828 IA cases and 68 568 controls from the Nordic HUNT population study. Furthermore, we assessed association between the metaGRS and patient characteristics in a cohort of 5560 IA patients. Results: Per SD increase of metaGRS, the hazard ratio for ASAH incidence was 1.34 (95% CI, 1.20–1.51) and the odds ratio for IA presence 1.09 (95% CI, 1.01–1.18). Upon including the metaGRS on top of clinical risk factors, the concordance index to predict ASAH hazard increased from 0.63 (95% CI, 0.59–0.67) to 0.65 (95% CI, 0.62–0.69), while prediction of IA presence did not improve. The metaGRS was statistically significantly associated with age at ASAH (β=−4.82×10 −3 per year [95% CI, −6.49×10 −3 to −3.14×10 −3 ]; P =1.82×10 −8 ), and location of IA at the internal carotid artery (odds ratio=0.92 [95% CI, 0.86–0.98]; P =0.0041). Conclusions: The metaGRS was predictive of ASAH incidence, although with limited added value over clinical risk factors. The metaGRS was not predictive of IA presence. Therefore, we do not recommend using this metaGRS in daily clinical care. Genetic risk does partly explain the clinical heterogeneity of IA warranting prioritization of clinical heterogeneity in future genetic prediction studies of IA and ASAH.

37. A Prospective Study of Brief Resolved Unexplained Events: Risk Factors for Persistent Symptoms

Abstract

OBJECTIVE The risk of persistent symptoms after a brief resolved unexplained event (BRUE) is not known. Our objective was to determine the frequency and risk factors for persistent symptoms after BRUE hospitalizations. METHODS We conducted a prospective longitudinal cohort study of infants hospitalized with an admitting diagnosis of BRUE. Caregiver-reported symptoms, anxiety levels, and management changes were obtained by questionnaires during the 2-month follow-up period. Clinical data including repeat hospitalizations were obtained from a medical record review. Multivariable analyses with generalized estimating equations were conducted to determine the risk of persistent symptoms. RESULTS Of 124 subjects enrolled at 51.6 ± 5.9 days of age, 86% reported symptoms on at least 1 questionnaire after discharge; 65% of patients had choking episodes, 12% had BRUE spells, and 15% required a repeat hospital visit. High anxiety levels were reported by 31% of caregivers. Management changes were common during the follow-up period and included 30% receiving acid suppression and 27% receiving thickened feedings. Only 19% of patients had a videofluoroscopic swallow study while admitted, yet 67% of these studies revealed aspiration/penetration. CONCLUSIONS Many infants admitted with BRUE have persistent symptoms and continue to access medical care, suggesting current management strategies insufficiently address persistent symptoms. Future randomized trials will be needed to evaluate the potential efficacy of therapies commonly recommended after BRUE.

Abstract

PURPOSE Representativeness in acute leukemia clinical research is essential for achieving health equity. The National Cancer Institute's mandate for Comprehensive Cancer Centers (CCCs) to define and assume responsibility for cancer control and treatment across a geographic catchment area provides an enforceable mechanism to target and potentially remediate participatory inequities. METHODS We examined enrollee characteristics across 15 Cancer and Leukemia Group B/Alliance cooperative group adult acute leukemia clinical trials (N = 3,734) from 1998 to 2013, including participation in optional companion biobanks. We determined enrollment odds by race-ethnicity for all participants adjusted for national incidence, and for those enrolled at CCCs adjusted for catchment area incidence. We modeled biobank participation by sociodemographics using logistic regression. RESULTS Non-Hispanic (NH)-White patients were more likely to be enrolled than NH-Black, NH-Asian, or Hispanic patients (odds ratio [OR], 0.75, 0.48, and 0.44, respectively; all P < .001), but less likely than NH-Native American patients (OR, 1.91; P < .001), adjusted for national incidence. Enrollment odds were lower for NH-Black, NH-Asian, and Hispanic patients at CCCs adjusted for catchment area incidence (OR, 0.57, 0.26, and 0.32, respectively; P < .001); differences were driven by overenrollment of NH-White patients from outside self-defined catchment areas (18.1% v 12.3%; χ2 P = .01) and by CCCs with less absolute enrollee diversity (rank sum P = .03). Among all enrollees, NH-White race-ethnicity and lower neighborhood deprivation correlated with biobank participation (OR, 1.81 and 1.45, respectively; P = .01 and .03). For CCC enrollees, the correlation of race-ethnicity with biobank participation was attenuated by a measure accounting for their site's degree of enrollment disparity but not neighborhood deprivation. CONCLUSION Acute leukemia clinical research disparities are substantial and driven by structural trial enrollment barriers at CCCs. Real-time CCC access and enrollment monitoring is needed to better align research participation with local populations.

Abstract

Background: Reducing the burden of death due to infection is an urgent global public health priority. Previous studies have estimated the number of deaths associated with drug-resistant infections and sepsis and found that infections remain a leading cause of death globally. Understanding the global burden of common bacterial pathogens (both susceptible and resistant to antimicrobials) is essential to identify the greatest threats to public health. To our knowledge, this is the first study to present global comprehensive estimates of deaths associated with 33 bacterial pathogens across 11 major infectious syndromes. Methods: We estimated deaths associated with 33 bacterial genera or species across 11 infectious syndromes in 2019 using methods from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, in addition to a subset of the input data described in the Global Burden of Antimicrobial Resistance 2019 study. This study included 343 million individual records or isolates covering 11 361 study-location-years. We used three modelling steps to estimate the number of deaths associated with each pathogen: deaths in which infection had a role, the fraction of deaths due to infection that are attributable to a given infectious syndrome, and the fraction of deaths due to an infectious syndrome that are attributable to a given pathogen. Estimates were produced for all ages and for males and females across 204 countries and territories in 2019. 95% uncertainty intervals (UIs) were calculated for final estimates of deaths and infections associated with the 33 bacterial pathogens following standard GBD methods by taking the 2·5th and 97·5th percentiles across 1000 posterior draws for each quantity of interest. Findings: From an estimated 13·7 million (95% UI 10·9–17·1) infection-related deaths in 2019, there were 7·7 million deaths (5·7–10·2) associated with the 33 bacterial pathogens (both resistant and susceptible to antimicrobials) across the 11 infectious syndromes estimated in this study. We estimated deaths associated with the 33 bacterial pathogens to comprise 13·6% (10·2–18·1) of all global deaths and 56·2% (52·1–60·1) of all sepsis-related deaths in 2019. Five leading pathogens—Staphylococcus aureus, Escherichia coli, Streptococcus pneumoniae, Klebsiella pneumoniae, and Pseudomonas aeruginosa—were responsible for 54·9% (52·9–56·9) of deaths among the investigated bacteria. The deadliest infectious syndromes and pathogens varied by location and age. The age-standardised mortality rate associated with these bacterial pathogens was highest in the sub-Saharan Africa super-region, with 230 deaths (185–285) per 100 000 population, and lowest in the high-income super-region, with 52·2 deaths (37·4–71·5) per 100 000 population. S aureus was the leading bacterial cause of death in 135 countries and was also associated with the most deaths in individuals older than 15 years, globally. Among children younger than 5 years, S pneumoniae was the pathogen associated with the most deaths. In 2019, more than 6 million deaths occurred as a result of three bacterial infectious syndromes, with lower respiratory infections and bloodstream infections each causing more than 2 million deaths and peritoneal and intra-abdominal infections causing more than 1 million deaths. Interpretation: The 33 bacterial pathogens that we investigated in this study are a substantial source of health loss globally, with considerable variation in their distribution across infectious syndromes and locations. Compared with GBD Level 3 underlying causes of death, deaths associated with these bacteria would rank as the second leading cause of death globally in 2019; hence, they should be considered an urgent priority for intervention within the global health community. Strategies to address the burden of bacterial infections include infection prevention, optimised use of antibiotics, improved capacity for microbiological analysis, vaccine development, and improved and more pervasive use of available vaccines. These estimates can be used to help set priorities for vaccine need, demand, and development. 41. Retrospective analysis of risk and timing of death from right heart failure in United States and Canadian feedlot necropsied cattle

Abstract

OBJECTIVE To evaluate the risk and timing of right heart failure (RHF) in feedlot cattle. ANIMALS Study population consisted of 1,717,356 cattle (5,527 cohorts) in 13 US and Canadian feedlots. There were 1,336 RHF diagnosed at necropsy. PROCEDURES Multivariable models were utilized to evaluate risk and timing of RHF death. RESULTS Arrival year was associated with RHF and was influenced by arrival quarter on the magnitude of risk of RHF (P < .01), but no linear increase over years was identified. The impact of feedlot elevation on RHF was modified by breed (beef, dairy, or dairy-cross; P < .01) with beef cattle in the highest elevation category having 0.54 times the risk of RHF as dairy cattle in the same elevation category (LCL = 0.31; UCL = 0.962). Cattle that died due to RHF and were treated for bovine respiratory disease died 11 days (LCL = 1.33, UCL = 20.2 days) sooner than cattle never treated for bovine respiratory disease (P = .03). Cattle breed was associated with RHF timing (P = .01), and dairy-cross cattle RHF cases died approximately 37 days earlier (SE = 13.0 days; P = .01) compared to beef cattle. CLINICAL RELEVANCE This research showed demographic factors associated with RHF and their respective influence on risk and timing of RHF. Risk rates of RHF were similar to previous research. This could allow for comparisons across different feedlot populations, using different diagnoses at necropsy and RHF risk/rates do not appear to be increasing.

Abstract

Background: Prior studies into the association of head trauma with neuropathology have been limited by incomplete lifetime neurotrauma exposure characterization. Objective: To investigate the neuropathological sequelae of traumatic brain injury (TBI) in an autopsy sample using three sources of TBI ascertainment, weighting findings to reflect associations in the larger, community-based cohort. Methods: Self-reported head trauma with loss of consciousness (LOC) exposure was collected in biennial clinic visits from 780 older adults from the Adult Changes in Thought study who later died and donated their brain for research. Self-report data were supplemented with medical record abstraction, and, for 244 people, structured interviews on lifetime head trauma. Neuropathology outcomes included Braak stage, CERAD neuritic plaque density, Lewy body distribution, vascular pathology, hippocampal sclerosis, and cerebral/cortical atrophy. Exposures were TBI with or without LOC. Modified Poisson regressions adjusting for age, sex, education, and APOE ɛ4 genotype were weighted back to the full cohort of 5,546 participants. Results: TBI with LOC was associated with the presence of cerebral cortical atrophy (Relative Risk 1.22, 95% CI 1.02, 1.42). None of the other outcomes was associated with TBI with or without LOC. Conclusion: TBI with LOC was associated with increased risk of cerebral cortical atrophy. Despite our enhanced TBI ascertainment, we found no association with the Alzheimer’s disease-related neuropathologic outcomes among people who survived to at least age 65 without dementia. This suggests the pathophysiological processes underlying post-traumatic neurodegeneration are distinct from the hallmark pathologies of Alzheimer’s disease.

46. Parameter Transfer for Quantum Approximate Optimization of Weighted MaxCut

Abstract

Finding high-quality parameters is a central obstacle to using the quantum approximate optimization algorithm (QAOA). Previous work partially addresses this issue for QAOA on unweighted MaxCut problems by leveraging similarities in the objective landscape among different problem instances. However, we show that the more general weighted MaxCut problem has significantly modified objective landscapes, with a proliferation of poor local optima. Our main contribution is a simple rescaling scheme that overcomes these deleterious effects of weights. We show that for a given QAOA depth, a single “typical” vector of QAOA parameters can be successfully transferred to weighted MaxCut instances. This transfer leads to a median decrease in the approximation ratio of only 2.0 percentage points relative to a considerably more expensive direct optimization on a dataset of 34,701 instances with up to 20 nodes and multiple weight distributions. This decrease can be reduced to 1.2 percentage points at the cost of only 10 additional QAOA circuit evaluations with parameters sampled from a pretrained metadistribution, or the transferred parameters can be used as a starting point for a single local optimization run to obtain approximation ratios equivalent to those achieved by exhaustive optimization in 96.35% of our cases.

48. Are Students Coming to Class Prepared? The Importance of Pre-Class Learning in a Flipped Classroom

Abstract

Despite extensive calls for the use of flipped classrooms in accounting, prior research has given little attention to the role of pre-class learning. Effective pre-class learning is essential to ensuring students still learn the basic class content that has been removed from a classroom to free up time for more active learning activities. If pre-class learning is not achieved, the value of the flipped classroom strategy is diminished. Through a review of existing literature, we provide accounting educators with a practical design guide for effectively implementing best-practices pre-class work in flipped classrooms. This article provides resources, design recommendations, and examples of flipped classroom implementation in an accounting education setting to foster more effective pre-class learning. We conclude with a call for research on various aspects of the flipped classroom in accounting education.
