Introduction In recent years, a phenomenon known as "vaccine hesitancy" has spread throughout the world, even among health workers, determining a reduction in vaccination coverage (VC). A study aimed at evaluating VC among healthcare workers (HCWs) in 10 Italian cities (L'Aquila, Genoa, Milan, Palermo, Sassari, Catanzaro, Ferrara, Catania, Naples, Messina) was performed. Materials and methods Annex 3 of the Presidential Decree n. 445 of 28 December 2000 was used to collect information on the vaccination status of HCWs. The mean and standard deviation (SD) were calculated with regard to the quantitative variable (age), while absolute and relative frequencies were obtained for categorical data (sex, professional profile, working sector, vaccination status). Aim: To examine the relationship between inflammatory and coagulation biomarkers and cardiac autonomic function (CAF) as measured by heart rate variability in persons with HIV. Materials & methods: This analysis included 4073 HIV-infected persons from the Strategies for Management of Antiretroviral Therapy study. We examined the association between IL-6, high-sensitivity C-reactive protein (hsCRP) and d-dimer with heart rate variability measures (SDNN and rMSSD), both cross-sectionally and longitudinally. Results: Cross-sectional analysis revealed significant inverse associations between IL-6, hsCRP and d-dimer with SDNN and rMSSD (p < 0.01 for all comparisons). However, longitudinal analysis failed to show a significant association between baseline IL-6, hsCRP and d-dimer with change in CAF over time. Conclusion: Cross-sectionally, higher levels of inflammatory and coagulation biomarkers were associated with lower levels of CAF in the Strategies for Management of Antiretroviral Therapy trial. Although deterioration in CAF was observed during followup, baseline levels of inflammatory and coagulation markers were not predictive of the decline in CAF over time.

We present results on low-resolution mid-infrared (MIR) spectra of 70 IR-luminous galaxies obtained with the infrared spectrograph (IRS) onboard Spitzer. We selected sources from the European Large Area Infrared Survey with S 15 > 0.8 mJy and photometric or spectroscopic z > 1. About half of the samples are quasi-stellar objects (QSOs) in the optical, while the remaining sources are galaxies, comprising both obscured active galactic nuclei (AGN) and starbursts. Redshifts were obtained from optical spectroscopy, photometric redshifts and the IRS spectra. The later turn out to be reliable for obscured and/or star-forming sources, thus becoming an ideal complement to optical spectroscopy for redshift estimation. We estimate monochromatic luminosities at several rest-frame wavelengths, equivalent widths and luminosities for the polycyclic aromatic hydrocarbon (PAH) features, and strength of the silicate feature in individual spectra. We also estimate integrated 8-1000 μm IR luminosities via spectral energy distribution fitting to MIR and far-IR (FIR) photometry from the Spitzer Wide-Area Infrared Extragalactic survey and the MIR spectrum. Based on these measurements, we classify the spectra using well-known IR diagnostics, as well as a new one that we propose, into three types of source: those dominated by an unobscured AGN, mostly corresponding to optical quasars (QSOs), those dominated by an obscured AGN and starburst-dominated sources. Starbursts concentrate at z ∼ 0.6-1.0 favoured by the shift of the 7.7-μm PAH band into the selection 15-μm band, while AGN spread over the 0.5 < z < 3.1 range. Star formation rates (SFR) are estimated for individual sources from the luminosity of the PAH features. An estimate of the average PAH luminosity in QSOs and obscured AGN is obtained from the composite spectrum of all sources with reliable redshifts. The estimated mean SFR in the QSOs is 50-100 M ⊙ yr -1, but the implied FIR luminosity is 3-10 times lower than that obtained from stacking analysis of the FIR photometry, suggesting destruction of the PAH carriers by energetic photons from the AGN. The SFR estimated in obscured AGN is two to three times higher than in QSOs of similar MIR luminosity. This discrepancy might not be due to luminosity effects or selection bias alone, but could instead indicate a connection between obscuration and star formation. However, the observed correlation between silicate absorption and the slope of the NIR to MIR spectrum is compatible with the obscuration of the AGN emission in these sources being produced in a dust torus. © 2009 RAS.

We report results from our Spitzer GO-1 program on IRS spectroscopy of a large sample of Luminous Infrared Galaxies and quasars selected from the European Large Area ISO Survey (ELAIS). The selected ELAIS sources have a wide multi-wavelength coverage, including ISOCAM, ISOPHOT, IRAC and MIPS (from SWIRE), and optical photometry. Here we present the sample selection and results from the IRS spectroscopy.

There is controversy as to whether the double-balloon or the Inoue technique of percutaneous mitral balloon valvuloplasty (PMBV) provides superior immediate and long-term results. This study was undertaken to analyze the effect of the learning curve of the Inoue technique of PMBV in the immediate and long-term outcome of PMBV. The learning curve of Inoue PMBV was analyzed in 233 Inoue PMBVs divided into 2 groups: "early experience" (n = 100) and "late experience" (n = 133). The results of the overall Inoue technique were compared with those of 659 PMBVs performed with the double-balloon technique. Baseline clinical and morphologic characteristics between early and late experience Inoue groups were similar. Post-PMBV mitral valve area (1.89 +/- 0.56 vs 1.69 +/- 0.57 cm(2); p = 0.008) and success rate (60% vs 75.9%; p = 0.009) were significantly higher in the late experience Inoue group. Furthermore, there was a trend for less incidence of severe post-PMBV mitral regurgitationor = 3+ in the late experience group (6.8% vs 12%; p = 0.16). Although the post-PMBV mitral valve area was larger with the double-balloon technique (1.94 +/- 0.72 vs 1.81 +/- 0.58 cm(2); p = 0.01), the success rate (71.3% vs 69.1%; p = NS), incidence ofor = 3+ mitral regurgitation (9% vs 9%), in-hospital complications, and long-term and event-free survival were similar with both techniques. In conclusion, there is a significant learning curve of the Inoue technique of PMBV. Both the Inoue and the double-balloon techniques are equally effective techniques of PMBV because they resulted in similar immediate success, in-hospital adverse events, and long-term and event-free survival.

Patients with renal failure undergoing percutaneous coronary intervention (PCI) experience reduced procedural success rates and increased in-hospital and long-term follow-up major adverse cardiac events. This study was designed to determine whether the severity of preprocedural renal failure influences the outcomes of patients with renal failure undergoing PCI. We compared the immediate and long-term outcomes of 192 patients with mild renal failure (creatinine 1.6 to 2.0 mg/dl, mean 1.76) with those of 131 patients with severe renal failure (creatinine >2.0 mg/dl, mean 2.90), selected from 3,334 consecutive patients undergoing PCI between 1994 and 1997. Background —Patients with end-stage renal disease undergoing conventional balloon angioplasty have reduced procedural success and increased complication rates. This study was designed to determine the immediate and long-term outcomes of patients with varying degrees of renal failure undergoing percutaneous coronary intervention in the current device era. Methods and Results —We compared the immediate and long-term outcomes of 362 renal failure patients (creatinine >1.5 mg/dL) with those of 2972 patients with normal renal function who underwent percutaneous coronary intervention between 1994 and 1997. Patients with renal failure were older and had more associated comorbidities. They had reduced procedural success (89.5% versus 92.9%, P =0.007) and greater in-hospital combined major event (death, Q-wave myocardial infarction, emergent CABG; 10.8% versus 1.8%; P P P P Conclusions —Although patients with renal failure can be treated with a high procedural success rate in the new device era, they have an increased rate of major events both in hospital and at long-term follow-up. Nevertheless, utilization of stenting and debulking techniques improves immediate and long-term outcomes.

OBJECTIVES The purpose of this study was to assess the immediate and long-term outcome of repeat percutaneous mitral balloon valvuloplasty (PMV) for post-PMV mitral restenosis. BACKGROUND Symptomatic mitral restenosis develop in 7% to 21% of patients after PMV. Currently, most of these patients are referred for mitral valve replacement. However, it is unknown if these patients may benefit from repeat PMV. METHODS We report the immediate outcome and long-term clinical follow-up results of 36 patients (mean age 58 ± 13 years, 75% women) with symptomatic mitral restenosis after prior PMV, who were treated with a repeat PMV at 34.6 ± 28 months after the initial PMV. The mean follow-up period was 30 ± 33 months with a maximal follow-up of 10 years. RESULTS An immediate procedural success was obtained in 75% patients. The overall survival rate was 74%, 72% and 71% at one, two, and three years respectively. The event-free survival rate was 61%, 54% and 47% at one, two, and three years respectively. In the presence of comorbid diseases (cardiac and noncardiac) the two-year event-free survival was reduced to 29% as compared with 86% in patients without comorbid diseases. Cox regression analysis identified the echocardiographic score (p = 0.03), post-PMV mitral valve area (p = 0.003), post-PMV mitral regurgitation grade (p = 0.02) and post-PMV pulmonary artery pressure (p = 0.0001) as independent predictors of event-free survival after repeat PMV. CONCLUSIONS Repeat PMV for post-PMV mitral restenosis results in good immediate and long-term outcome in patients with low echocardiographic scores and absence of comorbid diseases. Although the results are less favorable in patients with suboptimal characteristics, repeat PMV has a palliative role if the patients are not surgical candidates.

Management of in-stent restenosis has become a significant challenge in interventional cardiology. The results of balloon angioplasty have been disappointing due to the high recurrence of restenosis at follow-up. Debulking of the restenotic tissue within the stents using directional coronary atherectomy (DCA) may offer a therapeutic advantage. We report the immediate clinical and angiographic outcomes and long-term clinical follow-up results of 45 patients (46 lesions), mean age 63+/-12 years, 73% men, with a mean reference diameter of 2.9+/-0.6 mm, treated with DCA for symptomatic Palmaz-Schatz in-stent restenosis. DCA was performed successfully in all 46 lesions and resulted in a postprocedural minimal luminal diameter of 2.7+/-0.7 mm and a residual diameter stenosis of 17+/-10%. There were no in-hospital deaths, Q-wave myocardial infarctions, or emergency coronary artery bypass surgeries. Four patients (9%) suffered a non-Q-wave myocardial infarction. Target lesion revascularization was 28.3% at a mean follow-up of 10+/-4.6 months. Kaplan-Meier event-free survival (freedom from death, myocardial infarction, and repeat target lesion revascularization) was 71.2% and 64.7% at 6 and 12 months after DCA, respectively. Thus, DCA is safe and efficacious for the treatment of Palmaz-Schatz in-stent restenosis. It results in a large postprocedural minimal luminal diameter and a low rate of both target lesion revascularization and combined major clinical events at follow-up.

Background Early loss of minimal luminal diameter of >0.3 mm after successful percutaneous transluminal coronary angioplasty (PTCA) is associated with a higher incidence of restenosis. The underlying mechanism of this early loss is unknown and thrombus may be a contributing factor. Methods We performed a prospective study using quantitative computerized planimetry on coronary tissue specimens obtained by directional coronary atherectomy of 24 lesions in which early loss occurred 22 ± 9 minutes after successful PTCA. Results Thrombus was present in 9 (37%) of 24 coronary specimens. Segmental areas (mm 2 ) and percentage of total area were distributed as follows: sclerotic tissue, 4.07 ± 0.7 mm 2 (63% ± 6%); fibrocellular tissue, 0.97 ± 0.27 mm 2 (16% ± 4%); hypercellular tissue, 0.99 ± 0.29 mm 2 (12% ± 3%); atheromatous gruel, 0.18 ± 0.07 mm 2 (3% ± 0.1%); and thrombus, 0.24 ± 0.15 mm 2 (6% ± 0.4%). There was no difference in the relative early loss index between lesions with or without thrombus (35% ± 7% vs 26% ± 2%, respectively; P = .87). Multiple stepwise regression analysis did not identify any histologic predictors of relative early loss index. Conclusion Histopathologic analysis of coronary lesions with early loss after successful PTCA suggests that thrombus may not play a significant role in this angiographic phenomenon. Objective. This study sought to compare two strategies of revascularization in patients obtaining a good immediate angiographic result after percutaneous transluminal coronary angioplasty (PTCA): elective stenting versus optimal PTCA. A good immediate angiographic result with provisional stenting was considered to occur only if early loss in minimal luminal diameter (MLD) was documented at 30 min post-PTCA angiography. Background. Coronary stenting reduces restenosis in lesions exhibiting early deterioration (>0.3 mm) in MLD within the first 24 hours (early loss) after successful PTCA. Lesions with no early loss after PTCA have a low restenosis rate. Methods. To compare angiographic restenosis and target vessel revascularization (TVR) of lesions treated with coronary stenting versus those treated with optimal PTCA, 116 patients were randomized to stent (n = 57) or to optimal PTCA (n = 59). After randomization in the PTCA group, 13.5% of the patients crossed over to stent due to early loss (provisional stenting). Results. Baseline demographic and angiographic characteristics were similar in both groups of patients. At 7.6 months, 96.6% of the entire population had a follow-up angiographic study: 98.2% in the stent and 94.9% in the PTCA group. Immediate and follow-up angiographic data showed that acute gain was significantly higher in the stent than in the PTCA group (1.95 vs. 1.5 mm; p Conclusions. The strategy of PTCA with delay angiogram and provisional stent if early loss occurs had similar restenosis rate and TVR, but lower cost than primary stenting after PTCA.

We present the final band-merged European Large-Area ISO Survey (ELAIS) Catalogue at 6.7, 15, 90 and 175 mum, and the associated data at U, g', r', i', Z, J, H, K and 20 cm. The origin of the survey, infrared and radio observations, data-reduction and optical identifications are briefly reviewed, and a summary of the area covered and the completeness limit for each infrared band is given. A detailed discussion of the band-merging and optical association strategy is given. The total Catalogue consists of 3762 sources. 23 per cent of the 15-mum sources and 75 per cent of the 6.7-mum sources are stars. For extragalactic sources observed in three or more infrared bands, colour-colour diagrams are presented and discussed in terms of the contributing infrared populations. Spectral energy distributions (SEDs) are shown for selected sources and compared with cirrus, M82 and Arp220 starburst, and active galactic nuclei (AGN) dust torus models. Spectroscopic redshifts are tabulated, where available. For the N1 and N2 areas, the Isaac Newton Telescope ugriz Wide Field Survey permits photometric redshifts to be estimated for galaxies and quasars. These agree well with the spectroscopic redshifts, within the uncertainty of the photometric method [similar to10 per cent in (1 + z) for galaxies]. The redshift distribution is given for selected ELAIS bands and colour-redshift diagrams are discussed. There is a high proportion of ultraluminous infrared galaxies (log(10) of 1-1000 mum luminosity L(ir) > 12.22) in the ELAIS Catalogue (14 per cent of 15-mum galaxies with known z), many with Arp220-like SEDs. 10 per cent of the 15-mum sources are genuine optically blank fields to r' = 24: these must have very high infrared-to-optical ratios and probably have z > 0.6, so are high-luminosity dusty starbursts or Type 2 AGN. Nine hyperluminous infrared galaxies (L(ir) > 13.22) and nine extremely red objects (EROs) (r - K > 6) are found in the survey. The latter are interpreted as ultraluminous dusty infrared galaxies at z similar to 1. The large numbers of ultraluminous galaxies imply very strong evolution in the star formation rate between z = 0 and 1. There is also a surprisingly large population of luminous (L(ir) > 11.5), cool (cirrus-type SEDs) galaxies, with L(ir) L(opt) > 0, implying A(V) > 1.

We present the first estimate of the evolution of type 1 AGNs in the IR (15 mum) obtained from the ELAIS survey in the S1 region. We find that the luminosity function (LF) of Type 1 AGNs at 15mum is fairly well represented by a double power-law function with a bright slope of 2.9 and a faint slope of 1.1. There is evidence for significant cosmological evolution according to a pure luminosity evolution model L-15(z)proportional to(1+z)(k), with k=3.00(-0.20)(+0.16) in a (Omega(m),Omega(Lambda))=(1.0,0.0) cosmology. This evolution is similar to what is observed at other wavebands. From the luminosity function and its evolution, we estimate a contribution of similar to 2% from Type 1. AGN to the total Cosmic Infrared Background (CIRB) at 15 mum.

Objectives, This study examined the association between the presence of tricuspid regurgitation and immediate and late adverse outcomes in patients undergoing balloon mitral valvuloplasty. Background. Significant tricuspid regurgitation has an adverse impact on morbidity and mortality in patients undergoing mitral valve surgery for mitral stenosis. Methods. We studied 318 consecutive patients (mean [± SD] age 54 ± 15 years) who underwent ballon mitral valvuloplasty and had color Doppler ecnocardiographic studies before the procedure. Patients were classified into three groups: 221 with no or mild (69%), 60 with moderate (19%) and 37 with severe (12%) tricuspid regurgitation. Clinical follow-up ranged from 6 to 62 months. Results. Before mitral valvuloplasty, increasing degrees of tricuspid regurgitation were associated with a smaller initial mitral valve area (p < 0.05), higher echocardiographic score (p < 0.05), lower cardiac output (p < 0.01) and higher pulmonary vascular resistance (p < 0.01). Although the initial success rate did not differ significantly between groups, patients with a higher degree of tricuspid regurgitation had less optimal results, as reflected by a smaller absolute increase in mitral valve area (1.02 vs. 0.9 vs. 0.7 cm2, p < 0.01). The estimated 4-year event-free survival rate (freedom from death, mitral valve surgery, repeat valvuloplasty and heart failure) was lower for the group with severe tricuspid regurgitation (68% vs. 58% vs. 35%, p < 0.0001). At 4 years, 94% of patients with mild tricuspid regurgitation were alive compared with 90% and 69%, respectively, of patients with moderate or severe tricuspid regurgitation (p < 0.0001). Cox proportional analysis identified tricuspid regurgitation as an independent predictor of late outcome (p < 0.001). Conclusions. Patients with mitral stenosis and severe tricuspid regurgitation undergoing mitral valvuloplasty have advanced mitral valve and pulmonary vascular disease, suboptimal immediate results and poor late outcome.

We describe the European Large Area ISO Survey (ELAIS). ELAIS was the largest single Open Time project conducted by ISO, mapping an area of 12 deg(2) at 15 mu m with ISOCAM and at 90 mu m with ISOPHOT. Secondary surveys in other ISO bands were undertaken by the ELAIS team within the fields of the primary survey, with 6 deg(2) being covered at 6.7 mu m and 1 deg(2) at 175 mu m. This paper discusses the goals of the project and the techniques employed in its construction, as well as presenting details of the observations carried out, the data from which are now in the public domain. We outline the ELAIS 'preliminary analysis' which led to the detection of over 1000 sources from the 15 and 90-mu m surveys (the majority selected at 15 mu m with a flux limit of similar to 3 mJy), to be fed into a ground-based follow-up campaign, as well as a programme of photometric observations of detected sources using both ISOCAM and ISOPHOT. We detail how the ELAIS survey complements other ISO surveys in terms of depth and areal coverage, and show that the extensive multi-wavelength coverage of the ELAIS fields resulting from our concerted and on-going follow-up programme has made these regions amongst the best studied areas of their size in the entire sky, and, therefore, natural targets for future surveys. This paper accompanies the release of extremely reliable subsets of the 'preliminary analysis' products. Subsequent papers in this series will give further details of our data reduction techniques, reliability and completeness estimates and present the 15- and 90-mu m number counts from the 'preliminary analysis', while a further series of papers will discuss in detail the results from the ELAIS 'final analysis', as well as from the follow-up programme.

Previous studies demonstrated changes in aortic valve area calculated by the Gorlin equation under conditions of varying transvalvular flow in patients with valvular aortic stenosis (AS). To distinguish between flow-dependence of the Gorlin formula and changes in actual orifice area, the Gorlin valve area and 2 other measures of severity of AS, continuity equation valve area and valve resistance, were calculated under 2 flow conditions in 12 patients with AS. Transvalvular flow rate was varied by administration of dobutamine. During dobutamine infusion, right atrial and left ventricular end-diastolic pressures decreased, left ventricular peak systolic pressure and stroke volume increased, and systolic arterial pressure did not change. Heart rate increased by 19%, cardiac output by 38% and mean aortic valve gradient by 25%. The Gorlin valve area increased in all 12 patients by 0.03 to 0.30 cm2. The average Gorlin valve area increased from 0.67 +/- 0.05 to 0.79 +/- 0.06 cm2 (p0.001). In contrast, the continuity equation valve area (calculated in a subset of 6 patients) and valve resistance did not change with dobutamine. The data support the conclusion that flow-dependence of the Gorlin aortic valve area, rather than an increase in actual orifice area, is responsible for the finding that greater valve areas are calculated at greater transvalvular flow rates. Valve resistance is a less flow-dependent means of assessing severity of AS.

We address the question of how to deal with confusion limited surveys in the mid-infrared domain by using informations from higher frequency observations over the same sky regions. Such informations, once applied to apparently extended mid-infrared sources, which are indeed ``blends'' of two or more different sources, allow us to disentangle the single counterparts and to split the measured flux density into different components. We present the application of this method to the 24 micron Spitzer archival data in the GOODS-EN1 test field, where apparently extended, ``blended'' sources constitute about 20% of a reliable sample of 983 sources detected above the 5-sigma threshold down to 23 microJy. As higher frequency data-set we have considered the public IRAC images and catalogues on the same field. We show that the 24 micron sample is almost unbiased down to ~40 microJy and the careful application of the deblending procedure does not require any statistical completeness correction (at least at the flux level considered). This is probed by direct comparison of our results with those of Chary et al. (2004), who analysed the same data-set through extensive Monte Carlo simulations. The deblending procedure reduces of about 30% the confusion limit of the MIPS 24 micron survey, allowing one to obtain reliable source counts down to ~40 microJy. The extrapolation of the source counts down to fainter fluxes suggests that our 24 micron sample is able to resolve ~62% of the cosmic background down to a flux level of 38 microJy., Comment: 10 pages, submitted to MNRAS on July 2005

We present the optical identification of mid-IR and radio sources detected in the European Large Area ISO Survey (ELAIS) areas N1 and N2. Using the r' band optical data from the Wide Field Survey we apply a likelihood ratio method to search for the counterparts of the 1056 and 691 sources detected at 15 micron and 1.4 GHz respectively, down to flux limits of S_{15}=0.5 mJy and S_{1.4 GHz}=0.135 mJy. We find that ~92% of the 15 micron ELAIS sources have an optical counterpart down to the magnitude limit of the optical data, r'=24. All mid-IR sources with fluxes S_{15} >= 3 mJy have an optical counterpart. The magnitude distribution of the sources shows a well defined peak at relatively bright magnitudes r'~18. About 20% of the identified sources show a point-like morphology; its magnitude distribution has a peak at fainter magnitudes than those of galaxies. The mid-IR-to-optical and radio-to-optical flux diagrams are presented and discussed in terms of actual galaxy models. Objects with mid-IR-to-optical fluxes larger than 1000 are found that can only be explained as highly obscured star forming galaxies or AGNs. We present the first determination of the 15micron luminosity functio galaxies from the European Large Area ISO survey (ELAIS) southern fields. We have adopted a new criterion to separate the quiescent, non-evolving and the starburst, evolving populations based on the ratio of mid-infrared to optical luminosities. Strong evolution is suggested by our data for the starburst galaxy population, while normal spiral galaxies are consistent with no evolution. The starburst population must evolve both in luminosity and in density with rates of the order L(z) (1+z)^(3.5) and rho(z) (1+z)^3.8 up to z~1. The evolutionary parameters of our model have been tested by comparing the model predictions with other observables, like source counts at all flux density levels (from 0.1 to 300 mJy) and redshift distributions and luminosity functions at high-z (0.7 < z < 1.0 from HDF-N data). The agreement between our model predictions and the observed data is remarkably good. We use our data to estimate the star-formation density of the Universe up to z=0.4 and we use the luminosity function model to predict the trend of the star-formation history up to z=1., 35 pages, accepted by ApJ

We present the Final Analysis of the European Large Area ISO Survey (ELAIS) 15 $��$m observations, carried out with the ISOCAM instrument on board the Infrared Space Observatory (ISO). The data reduction method, known as LARI method, is based on a mathematical model of the detector's behaviour and was specifically designed for the detection of faint sources in ISO-CAM/PHOT data. The method is fully interactive and leads to very reliable and complete source lists. The resulting catalogue includes 1923 sources detected with $S/N > 5$ in the {0.5 -- 100 mJy} flux range and over an area of 10.85 {deg$^2$} split into four fields, making it the largest non-serendipitous extragalactic source catalogue obtained to date from ISO data. This paper presents the concepts underlying the data reduction method together with its latest enhancements. The data reduction process, the production and basic properties of the resulting catalogue are discussed. The catalogue quality is assessed by means of detailed simulations, optical identifications and comparison with previous analyses., Accepted by MNRAS. We present the final band-merged European Large-Area ISO Survey (ELAIS) Catalogue at 6.7, 15, 90 and 175 μm, and the associated data at U, g′, r′, i′, Z, J, H, K and 20 cm. The origin of the survey, infrared and radio observations, data-reduction and optical identifications are briefly reviewed, and a summary of the area covered and the completeness limit for each infrared band is given. A detailed discussion of the band-merging and optical association strategy is given. The total Catalogue consists of 3762 sources. 23 per cent of the 15-μm sources and 75 per cent of the 6.7-μm sources are stars. For extragalactic sources observed in three or more infrared bands, colour-colour diagrams are presented and discussed in terms of the contributing infrared populations. Spectral energy distributions (SEDs) are shown for selected sources and compared with cirrus, M82 and Arp220 starburst, and active galactic nuclei (AGN) dust torus models. Spectroscopic redshifts are tabulated, where available. For the N1 and N2 areas, the Isaac Newton Telescope ugriz Wide Field Survey permits photometric redshifts to be estimated for galaxies and quasars. These agree well with the spectroscopic redshifts, within the uncertainty of the photometric method [∼ 10 per cent in (1 + z) for galaxies]. The redshift distribution is given for selected ELAIS bands and colour-redshift diagrams are discussed. There is a high proportion of ultraluminous infrared galaxies (log 10 of 1-1000 μm luminosity L ir > 12.22) in the ELAIS Catalogue (14 per cent of 15-μm galaxies with known z), many with Arp220-like SEDs. 10 per cent of the 15-μm sources are genuine optically blank fields to r′ = 24: these must have very high infrared-to-optical ratios and probably have z > 0.6, so are high-luminosity dusty starbursts or Type 2 AGN. Nine hyperluminous infrared galaxies (L ir > 13.22) and nine extremely red objects (EROs) (r - K > 6) are found in the survey. The latter are interpreted as ultraluminous dusty infrared galaxies at z ∼ 1. The large numbers of ultraluminous galaxies imply very strong evolution in the star formation rate between z = 0 and 1. There is also a surprisingly large population of luminous (L ir > 11.5), cool (cirrus-type SEDs) galaxies, with L ir - L opt > 0, implying A v > 1.

We present a new analysis of the ISOCAM 14.3 micron deep survey in a 20x20 square arcmins area in the Lockman Hole. This survey is intermediate between the ultra-deep surveys and the shallow surveys in the ELAIS fields. The data have been analyzed with the method presented by Lari et al. (2001). We have produced a catalogue of 283 sources detected above the 5-sigma threshold, with fluxes in the interval 0.1-8 mJy. The catalogue is 90% complete at 1 mJy. The positional accuracy, estimated from the cross-correlation of infrared and optical sources, is around 1.5 arcsec. The search for the optical counterparts of the sources in the survey is performed on a medium-deep r' band optical image (5-sigma depth of r'=25), making use of the radio detections when available. The photometry has been checked through simulations and by comparing the data with those presented in a shallower and more extended ISOCAM survey in the Lockman Hole, that we have presented in a companion paper. Only 15% of the 14.3 micron sources do not have an optical counterpart down to r'=25 mag. We use the 6.7/14.3 micron colour as a star/galaxy separator, together with a visual inspection of the optical image and an analysis of the observed Spectral Energy Distribution of the ISOCAM sources. The stars in the sample turn out to be only 6% of the sample. We discuss the 14.3 micron counts of extragalactic sources, combining our catalogue with that obtained from the shallower ISOCAM survey. The data in the two surveys are consistent, and our results fully support the claims in previous works for the existence of an evolving population of infrared galaxies, confirming the evident departure from non-evolutionary model predictions., Comment: Version accepted by Astronomy and Astrophysics. We report results from a new analysis of a deep 95-’¦Ìm imaging survey with the photopolarimeter ISOPHOT on-board the Infrared Space Observatory, over a 40 × 40 arcmin2 area within the Lockman Hole. To this end we exploit a newly developed parametric algorithm able to identify and clean spurious signals induced by cosmic rays impacts and by transient effects and non-linearities in the detectors. These results provide us with the currently deepest - to our knowledge - far-infrared (far-IR) image of the extragalactic sky. Within the survey area, we detect 36 sources with signal-to-noise ratio S/N > 3 (corresponding to a flux of 16 mJy), making up a complete flux-limited sample for S95’¦Ìm>= 100 mJy. Reliable sources are detected, with decreasing but well-controlled completeness, down to S95’¦Ìm~= 20 mJy. The source extraction process and the completeness, the photometric and astrometric accuracies of this catalogue have been tested by us with extensive simulations accounting for all the details of the procedure. We estimate source counts down to a flux of ~30 mJy, at which limit we evaluate that 10-20 per cent of the cosmic IR background (CIRB) has been resolved into sources (contributing to the CIRB intensity ~=2.0 × 10-9 W m-2 sr-1). We report results of a new analysis of a deep 95 mu imaging survey with the photo-polarimeter ISOPHOT on board the Infrared Space Observatory, over a half square degree area within the Lockman Hole. The 95mu galaxy counts reveal a steep slope at fluxes lower than 100 mJy (alpha~1.6), in excess of that expected for a non-evolving source population. The shape of these counts agrees with those determined by ISO at 15 and 175 mu, and starts setting strong constraints on the evolution models for the far-IR galaxy populations., 37 pages, LaTeX file (mn style), 15 figures. We present the 15 micron extragalactic source counts from the Final Analysis Catalogue of the European Large Area ISO Survey southern hemisphere field S1, extracted using the Lari method. The large number of extragalactic sources (about 350) detected over this area between 0.5 and 100 mJy guarantee a high statistical significance of the source counts in the previously poorly covered flux density range between IRAS and the Deep ISOCAM Surveys. The bright counts in S1 (> 2mJy) are consistent with a flat, Euclidean slope, suggesting the dominance of a non-evolving population. In contrast, at fainter fluxes (< 2 mJy) our counts show a strong departure from no evolution models, with a very steep super-Euclidean slope down to our flux limit. Strong luminosity and density evolution is needed at least for the population of star-forming galaxies, together with a luminosity break around 10^10.8 Lsun in their local luminosity function in order to reproduce our sharp increase of the counts below 2 mJy and the redshift distributions observed for 15 micron sources at different flux levels. The contribution of the strongly evolving starburst population (down to 50 microJy) to the 15 micron cosmic background is estimated to be 2.2 nW m^-2 sr^-1 (67% of the total mid-infrared background estimate)., 14 pages, LaTeX, MNRAS style, 16 postscript figures, Accepted for publication in MNRAS

Background — Percutaneous mitral balloon valvuloplasty (PMV) results in good immediate results, particularly in patients with echocardiographic scores (Echo-Sc) ≤8. However, which variables relate to long-term outcome is unclear. Methods and Results — We report the immediate and long-term clinical follow-up (mean, 4.2±3.7 years; range, 0.5 to 15) of 879 patients who underwent 939 PMV procedures. Patients were divided into 2 groups, Echo-Sc ≤8 (n=601) and Echo-Sc >8 (n=278). PMV resulted in an increase in mitral valve area from 1.0±0.3 to 2.0±0.6 cm 2 in patients with Echo-Sc ≤8 and from 0.8±0.3 to 1.6±0.6 cm 2 in patients with Echo-Sc >8 ( P P 8, age, prior surgical commissurotomy, NYHA functional class IV, pre-PMV mitral regurgitation ≥2+, and higher post-PMV pulmonary artery pressure as independent predictors of combined events at long-term follow-up. Conclusions — The immediate and long-term outcome of patients undergoing PMV is multifactorial. The use of the Echo-Sc in conjunction with other clinical and morphological predictors of PMV outcome allows identification of patients who will obtain the best outcome from PMV.

We present the 15 micron luminosity function of type 1 AGN (QSO + Seyfert 1). Our sample of 21 high-redshift sources is selected from the Preliminary Analysis catalogue in the S1 field of the European Large Area ISO Survey (ELAIS). To study the cosmological evolution of the AGN1 luminosity function, our sample has been combined with a local sample of 41 sources observed by IRAS. We find that the luminosity function of AGN1 at 15 micron is fairly well represented by a double-power-law-function. There is evidence for significant cosmological evolution consistent with a pure luminosity evolution (PLE) model L(z) (1+z)^k, with k=3.0-3.3. The value of k depends on the existence or not of an evolution cut-off at redshift ~2, and on the adopted cosmology. From the luminosity function and its evolution we estimate a contribution of AGN1 to the Cosmic Infrared Background (CIRB) of nuI_nu ~ 6 x 10^{-11}W m^{-2} sr^{-1} at 15 micron. This corresponds to ~2-3% of the total observed CIRB at this wavelength. OBJECTIVES The purpose of this study was to determine whether small reference diameter of the culprit coronary artery influences the outcome of an attempted percutaneous revascularization procedure in the current era of interventional cardiology. BACKGROUND Although the interventional strategy is largely determined by the size of the culprit coronary artery, earlier quantitative studies have not shown a worse acute outcome for small reference vessel diameter (≤2.5 mm). METHODS A total of 2,306 patients undergoing percutaneous coronary revascularization was divided in groups with reference diameters ≤2.5 mm (n = 813) or >2.5 mm (n = 1,493). Success and in-hospital major adverse cardiac event (death, Q-wave myocardial infarction and emergency coronary artery bypass graft) rates between both groups were compared. RESULTS Patients with lesions in small vessels were older and presented more frequently with female gender, diabetes mellitus, heart failure, peripheral vascular, multivessel coronary disease and American Heart Association/American College of Cardiology (AHA/ACC) lesion type C (p ≤ 0.01, each). Further, utilization of interventional devices differed markedly. In contrast to stents (18.5% vs. 41.9%) and directional atherectomy (3.7% vs. 13.5%), conventional balloon angioplasty (73% vs. 50%) and rotational atherectomy (16.1% vs. 8.3%) were used more often in smaller vessels (p ≤ 0.0001, each). Success rate was lower in the small vessel group (92% vs. 95%; p = 0.006). Major adverse cardiac events occurred more frequently in small than large vessels (univariate 3.4% vs. 2.0%, p = 0.03; multivariate odds ratio 2.1, p = 0.02), particularly when proximal coronary segments were compared. CONCLUSIONS Lesions in vessels with small reference diameter represent a distinct group with respect to clinical and morphologic characteristics as well as device utilization. These lesions have lower chances of successful percutaneous intervention and carry relatively higher risks, specifically when located in proximal coronary segments.

There is controversy as to whether the double-balloon or Inoue technique of percutaneous mitral balloon valvotomy (PMBV) provides superior immediate and long-term results. This study compares the immediate procedural and long-term outcomes of patients undergoing PMBV using the double-balloon versus the Inoue techniques. Seven hundred thirty-four consecutive patients who underwent PMBV using the double-balloon (n = 621) or Inoue technique (n = 113) were studied. There were no statistically significant differences in baseline clinical and morphologic characteristics between the double-balloon and Inoue patients. The double-balloon technique resulted in superior immediate outcome, as reflected in a larger post-PMBV mitral valve area (1.9 +/- 0.7 vs 1.7 +/- 0.6 cm2; p = 0.005) and a lower incidence of 3+ mitral regurgitation after PMBV (5.4% vs 10.6%; p = 0.05). This superior immediate outcome of the double-balloon technique was observed only in the group of patients with echocardiographic score < or = 8 (post-PMBV mitral valve areas 2.1 +/- 0.7 vs 1.8 +/- 0.6; p = 0.004). Despite the difference in immediate outcome, there were no significant differences in event-free survival at long-term follow-up between the 2 techniques. Our study demonstrates that compared with the Inoue technique, the double-balloon technique results in a larger mitral valve area and less degree of severe mitral regurgitation after PMBV. Despite the difference in immediate outcome between both techniques, there were no significant differences in event-free survival at long-term follow-up.

This study compares the immediate and long-term outcomes of a primary coronary stenting strategy with primary balloon angioplasty with stent bailout in the treatment of patients with acute myocardial infarction (AMI). One hundred forty-seven consecutive patients who underwent primary balloon angioplasty with stent bailout (n = 94) or primary stenting (n = 53) for AMI were clinically followed for 8.1 +/- 5.7 and 8.5 +/- 4.5 months, respectively. Immediate results, as well as in-hospital and long-term ischemic events (death, reinfarction, and repeat revascularization) were compared between both groups. Angiographic success was 91.5% in the balloon angioplasty group and 94% in the stent group. In-hospital and late follow-up combined ischemic events were 22 of 94 (23%) versus 0 of 53 (0%); p < 0.001 and 33 of 78 (42%) versus 13 of 53 (25%), p = 0.04 for the balloon angioplasty and stent groups, respectively. At 6 months, the cumulative probability of repeat target lesion revascularization was higher in the balloon angioplasty group (47% vs 18%, p = 0.0006) as was the probability of late target revascularization (36% vs 18%, p = 0.046); the cumulative event-free survival after 6 months was significantly lower in the balloon angioplasty group (44% vs 80%, p = 0.0001). This study demonstrates that a primary stent placement strategy in patients with AMI is safe, feasible, and superior to primary balloon angioplasty with stent bailout. Primary stenting results in a larger postprocedural minimal luminal diameter, a lower early and late recurrent ischemic event rate, and a lower incidence of target lesion revascularization at follow-up.

Traditionally, procedural risks associated with conventional balloon coronary angioplasty have been largely attributed to unfavorable lesion morphology. However, factors predicting adverse events in the current practice of percutaneous coronary revascularization are unclear. The present study was undertaken to determine factors predicting major adverse events (death or Q-wave myocardial infarction or emergency bypass surgery) in 3,335 consecutive patients undergoing percutaneous coronary revascularization in the current practice of percutaneous coronary revascularization. During the period of observation, the rate of lesions treated successfully increased from 91% to 95% (P < 0.0001), whereas the rate of major adverse events (MACE) decreased from 3.6% to 1.6% (odds ratio [OR], 0.70 per year). Using multiple stepwise logistic regression analysis, cardiogenic shock (OR, 8.59; confidence interval [CI], 4.27-17.27), renal disease (OR, 3.33; CI, 1.95-5.69), evolving myocardial infarction (OR, 2.80; CI, 1.47-5.31), congestive heart failure (OR, 2.18; CI, 1.23-3.86), total number of lesions treated (OR, 1.28; CI, 1.03-1.59), age (OR, 1.03; CI, 1.01-1.06), and history of prior coronary intervention (OR, 0.51; CI 0.26-0.99) were identified as independent predictors of MACE. In addition, vascular disease (OR, 2. 48; CI 1.37-4.50) and unstable angina pectoris (OR, 0.44; CI 0.25-0. 79) were related to adverse events when patients in cardiogenic shock were excluded from the model. With the exception of most unfavorable lesion morphology (AHA/ACC lesion type C; OR, 2.05; CI, 1.19-3.52), anatomic parameters added no further information. In the present era of device technology, success rates of percutaneous coronary revascularization procedures have increased and remain to be determined by lesion morphology. In contrast, the rate of MACE is declining and best predicted by easily identified patient characteristics.

Objective. This study sought to compare two strategies of revascularization in patients obtaining a good immediate angiographic result after percutaneous transluminal coronary angioplasty (PTCA): elective stenting versus optimal PTCA. A good immediate angiographic result with provisional stenting was considered to occur only if early loss in minimal luminal diameter (MLD) was documented at 30 min post-PTCA angiography.Background. Coronary stenting reduces restenosis in lesions exhibiting early deterioration (>0.3 mm) in MLD within the first 24 hours (early loss) after successful PTCA. Lesions with no early loss after PTCA have a low restenosis rate.Methods. To compare angiographic restenosis and target vessel revascularization (TVR) of lesions treated with coronary stenting versus those treated with optimal PTCA, 116 patients were randomized to stent (n = 57) or to optimal PTCA (n = 59). After randomization in the PTCA group, 13.5% of the patients crossed over to stent due to early loss (provisional stenting).Results. Baseline demographic and angiographic characteristics were similar in both groups of patients. At 7.6 months, 96.6% of the entire population had a follow-up angiographic study: 98.2% in the stent and 94.9% in the PTCA group. Immediate and follow-up angiographic data showed that acute gain was significantly higher in the stent than in the PTCA group (1.95 vs. 1.5 mm; p < 0.03). However, late loss was significantly higher in the stent than the PTCA group (0.63 ± 0.59 vs. 0.26 ± 0.44, respectively; p = 0.01). Hence, net gain with both techniques was similar (1.32 ± 0.3 vs. 1.24 ± 0.29 mm for the stent and the PTCA groups, respectively; p = NS). Angiographic restenosis rate at follow-up (19.2% in stent vs. 16.4% in PTCA; p = NS) and TVR (17.5% in stent vs. 13.5% in PTCA; p = NS) were similar. Furthermore, event-free survival was 80.8% in the stent versus 83.1% in the PTCA group (p = NS). Overall costs (hospital and follow-up) were US $591,740 in the stent versus US $398,480 in the PTCA group (p < 0.02).Conclusions. The strategy of PTCA with delay angiogram and provisional stent if early loss occurs had similar restenosis rate and TVR, but lower cost than primary stenting after PTCA.

OBJECTIVESThe purpose of this study was to examine the effect of atrial fibrillation (AF) on the immediate and long-term outcome of patients undergoing percutaneous mitral balloon valvuloplasty (PMV).BACKGROUNDThere is controversy as to whether the presence of AF has a direct negative effect on the outcome after PMV.METHODSThe immediate procedural and the long-term clinical outcome after PMV of 355 patients with AF were prospectively collected and compared with those of 379 patients in normal sinus rhythm (NSR).RESULTSPatients with AF were older (62 ± 12 vs. 48 ± 14 years; p < 0.0001) and presented more frequently with New York Heart Association (NYHA) class IV (18.3% vs. 7.9%; p < 0.0001), echocardiographic score >8 (40.1% vs. 25.1%; p < 0.0001), calcified valves under fluoroscopy (32.4% vs. 18.8%, p < 0.0001) and with history of previous surgical commissurotomy (21.7% vs. 16.4%; p = 0.0002). In patients with AF, PMV resulted in inferior immediate and long-term outcomes, as reflected in a smaller post-PMV mitral valve area (1.7 ± 0.7 vs. 2 ± 0.7 cm2; p < 0.0001) and a lower event free survival (freedom of death, redo-PMV and mitral valve surgery) at a mean follow-up time of 60 months (32% vs. 61%; p < 0.0001). In the group of patients in AF, severe post-PMV mitral regurgitation (≥3+) (p = 0.0001), echocardiographic score >8 (p = 0.004) and pre-PMV NYHA class IV (p = 0.046) were identified as independent predictors of combined events at follow-up.CONCLUSIONSPatients with AF have a worse immediate and long-term outcomes after PMV. However, the presence of AF by itself does not unfavorably influence the outcome, but is a marker for clinical and morphologic features associated with inferior results after PMV.
