A new species, Piarosoma arunachalensis sp. nov., is described and illustrated based on external characters and the genitalia of two male specimens from Tale Wildlife Sanctuary, Lower Subansiri District, Arunachal Pradesh, India. The new species is distinguished from other Piarosoma by a combination of: a) presence of large hyaline patches on the wings, b) the absence of any white or yellow markings on the patagia and the abdominal segments, and c) differences in male genitalia.

The Indo-Australian genus Caltoris has over 15 species distributed from India through south China and SE Asia into New Guinea and Solomon Islands. Based on wing colouration, characters of male genitalia, and early larval stages on the host plant Phragmites karka, a new subspecies of Caltoris bromus (Leech, 1894), Caltoris bromus sadasiva ssp. nov., is described from the coastal lakes and mangrove associated swamps of Kerala, southern India on the western slopes of the Western Ghats. This is the first record of C. bromus from Western Ghats and Peninsular India. This extends the distribution range of the species from NE India to south-western India, adding it to the butterfly fauna of the Western Ghats.

The total synthesis of racemic incarvilleatone has been achieved by utilizing unexplored accelerated Rauhut–Currier (RC) dimerization. The other key steps of the synthesis are oxa-Michael and aldol reactions in a tandem sequence. Racemic incarvilleatone was separated by chiral HPLC and the configuration of each enantiomer was determined by single-crystal X-ray analysis. In addition, a one-pot synthesis of (±)-incarviditone has been achieved from rac-rengyolone by using KHMDS as a base. We have also assessed the anticancer activity of all the synthesized compounds in breast cancer cells nonetheless, they exhibited very limited growth suppression activity.

In The Pune Maternal Nutrition Study, vitamin B12 deficiency was seen in 65% of pregnant women, folate deficiency was rare. Maternal total homocysteine concentrations were inversely associated with offspring birthweight, and low vitamin B12 and high folate concentrations predicted higher offspring adiposity and insulin resistance. These findings guided a nested pre-conceptional randomised controlled trial ‘Pune Rural Intervention in Young Adolescents’. The interventions included: (1) vitamin B12+multi-micronutrients as per the United Nations International Multiple Micronutrient Antenatal Preparation, and proteins (B12+MMN), (2) vitamin B12 (B12 alone), and (3) placebo. Intervention improved maternal pre-conceptional and in-pregnancy micronutrient nutrition. Gene expression analysis in cord blood mononuclear cells in 88 pregnancies revealed 75 differentially expressed genes between the B12+MMN and placebo groups. The enriched biological processes included G2/M phase transition, chromosome segregation, and nuclear division. Enriched pathways included, mitotic spindle checkpoint and DNA damage response while enriched human phenotypes were sloping forehead and decreased head circumference. Fructose-bisphosphatase 2 (FBP2) and Cell Division Cycle Associated 2 (CDCA2) genes were under-expressed in the B12 alone group. The latter, involved in chromosome segregation was under-expressed in both intervention groups. Based on the role of B-complex vitamins in the synthesis of nucleotides and S-adenosyl methionine, and the roles of vitamins A and D on gene expression, we propose that the multi-micronutrient intervention epigenetically affected cell cycle dynamics. Neonates in the B12+MMN group had the highest ponderal index. Follow-up studies will reveal if the intervention and the altered biological processes influence offspring diabesity.

Mikroskopisch kontrollierte Chirurgie (MKC) umfasst verschiedene Methoden, welche eine histologisch nachgewiesene vollständige Entfernung maligner Tumoren bei gleichzeitig größtmöglicher Schonung des tumorfreien Gewebes in direkter Umgebung ermöglichen. Allen unter MKC subsummierten Verfahren gemein ist die Markierung des Exzidates zur topographischen Orientierung, wodurch eine Zuordnung verbliebener Tumorreste möglich ist. Indikationen für MKC sind maligne Hauttumoren in Problemlokalisationen sowie aggressive Subtypen von Hauttumoren. Zu den etablierten Indikationen für MKC zählen unter anderem Basalzellkarzinome, kutane Plattenepithelkarzinome, Morbus Bowen sowie Bowen-Karzinome, das Dermatofibrosarcoma protuberans, Melanome in chronisch lichtgeschädigter Haut sowie akral lentiginöse Melanome und Merkelzellkarzinome. Für weitere Tumoren wie dem extramammären Morbus Paget und verschiedene kutane Sarkome liegen Hinweise vor, dass MKC Vorteile beispielsweise hinsichtlich der Lokalrezidivrate zeigt. Darüber hinaus ist MKC indiziert, wenn absehbar ist, dass eine komplexe Verschlusstechnik erforderlich ist und eine vollständige Resektion des Tumors sicher gewährleistet sein muss. Es wurden verschiedenen Methoden der MKC beschrieben, zu denen beispielsweise die 3D-Histologie, die Horizontal-Methode oder die Mohs-Chirurgie zählen. Purpose: To identify the challenges faced by virtual teams and study the impact of employee engagement practices on performance, team effectiveness of Virtual workforce/Virtual teams. Research Problem: With the Gig Economy coming in and the worldwide disruption in the operation of many organizations due to Geo-political or Biological Factors, organizations should be well prepared to continue to give an uninterrupted performance. Under such circumstances, managing the virtual workforce becomes very important. Due to technological advances, people all across the globe can collaborate without traveling. However, as a result, organizations operate change and require developing a new set of competencies from both managers and employees. Therefore, to ensure the smooth running of an organization, it is imperative to have a concrete performance management system in place as far as the virtual workforce is concerned. Execution of virtual work calls for extensive scientific research because managing a virtual team is different from managing an ordinary team. Research Methodology: The stratified sampling method has been used as the mode of data collection for this research, which has been done by floating a set of questionnaires among the sample set of respondents. Stratified sampling would help classify the sample set into different strata as required and optimize the sample size accordingly to have better precision. In this case, the sample has been categorized primarily into two strata of Gender (Male and Female). The responses have been then analyzed through SPSS by Factor analysis to reach the objective from a grass root level. In this case, the respondents were selected from only those organizations where the work-from-home nature is a viable option. Then the respondents were selected and segregated based on gender to ensure a diverse sample set. Once the data has been run through factor analysis and the independent variables have been identified, linear regression analysis has then been performed on the data to understand the relationship between the deduced independent variables and team effectiveness. Finally, based on the literature review and the responses received through the questionnaire, this piece of research will analyze the inputs and recommend suitable practices in leading virtual teams, taking the challenges faced by the virtual teams into consideration. Expected Outcome/Findings: Through this research, it is expected to conclude the challenges that virtual teams face and the factors that affect the performance of the virtual teams, It is then required to map the associated factors influencing performance to recommend viable, practical, and virtually implementable employee engagement practices. Practical / Managerial Implications: The study shall help virtual team managers better identify the needs of the employees, which would lead to better employee engagement and, in turn, improve productivity. It would help the Human Resource specialists pay specific attention to certain areas of employee engagement and motivational factors while introducing a virtual work ecosystem to optimize the workforce’s performance. Ergonomics is said to have a great potential when it comes to designing of any artefact. Office ergonomics, a branch of ergonomics, seeks to understand how physical characteristics of humans are relevant in designing the workplace and is often used by organisations as an input to minimize the amount of discomfort faced by the employees. The provision of this input by the organisation generates a need to understand if it has any impact on the output produced by the employees. This paper seeks to understand this relationship and study the impact of office ergonomics on the performance of the employees. Additionally, it tries to discover the presence of participatory ergonomics (PE) in the organisation. 75 employees were randomly drawn from different departments of Larsen and Toubro, Surat, India and a questionnaire was administered to assess the relationship between 9 ergonomic elements (independent variables) and employee performance (dependent variable) by building a multiple regression model in R. The regression was first carried out for the 75 employees, post which the type of department that employees belong to (manufacturing or non-manufacturing) was added in the multiple regression model to test if department type moderates the relationship between the independent variables and the dependent variable. The study also tried to understand the implementation of PE in the organisation. Overall, for all employees, architectonic details and colour of the walls showed significant impact on performance. When departmental setup was introduced in the model as a moderating variable, it did not change the results of the model. Thus, departmental setup was found to be irrelevant in moderating the relationship between the variables.

Organizations focus a lot on their culture to attract the best talent to work with them. Leaders of these organizations expound their values on social media, employ PR agents and consulting firms to fine-tune their values, commonly stated through their websites, newsletters, and official communications, but how do people working there actually perceive these values? How are they driven from leadership to the lowest level of employees? This article attempts to address these issues by a qualitative research methodology based on Grounded Theory. Responses of employees working at leading organizations discuss their perceptions of culture in action, how values are driven, their anticipations for the future, and areas of improvement in their organizations. A thematic frequency-based analysis method is used to derive common themes, which can infer common patterns in action across 23 organizations with a strong Indian presence. The respondents span mainly across HR and marketing, with responses from finance, strategy and technology also included.

Prey seldom rely on a single type of antipredator defence, often using multiple defences to avoid predation. In many cases, selection in different contexts may favour the evolution of multiple defences in a prey. However, a prey may use multiple defences to protect itself during a single predator encounter. Such “defence portfolios” that defend prey against a single instance of predation are distributed across and within successive stages of the predation sequence (encounter, detection, identification, approach (attack), subjugation and consumption). We contend that at present, our understanding of defence portfolio evolution is incomplete, and seen from the fragmentary perspective of specific sensory systems (e.g., visual) or specific types of defences (especially aposematism). In this review, we aim to build a comprehensive framework for conceptualizing the evolution of multiple prey defences, beginning with hypotheses for the evolution of multiple defences in general, and defence portfolios in particular. We then examine idealized models of resource trade-offs and functional interactions between traits, along with evidence supporting them. We find that defence portfolios are constrained by resource allocation to other aspects of life history, as well as functional incompatibilities between different defences. We also find that selection is likely to favour combinations of defences that have synergistic effects on predator behaviour and prey survival. Next, we examine specific aspects of prey ecology, genetics and development, and predator cognition that modify the predictions of current hypotheses or introduce competing hypotheses. We outline schema for gathering data on the distribution of prey defences across species and geography, determining how multiple defences are produced, and testing the proximate mechanisms by which multiple prey defences impact predator behaviour. Adopting these approaches will strengthen our understanding of multiple defensive strategies. peerReviewed

Electrochemotherapy has been proven to be an efficient treatment for cutaneous metastases of various cancers. Data on breast cancer (BC) patients with cutaneous metastases were retrieved from the INSPECT database. Patients were divided by their receptor status: HER2+, HR+ (ER/PgR+), and TN (triple negative). Groups were similar for histological subtype and location of the nodules. Most patients were previously treated with surgery/systemic therapy/radiotherapy. We found no differences in the three groups in terms of response ratio (OR per patient 86% HER2+, 80% HR+, 76% TN, p = 0.8664). Purpose: The purpose is to analyze the hospitals and their processes for knowing their functions and commonalities for the benefit of the best hospital choice for patients. In this study, five hospitals of Mangalore have been considered for the analysis. There are a few common things among these hospitals and certain competitive advantages that a few of them have. Hence, analysis is done based on various approaches like marketing, finance, HR, Strategy as well as the service in general. It is also to know the growth, uniqueness, competitive advantage, patient satisfaction, social contribution, and sustainability measures as well as to know the survival ability of hospitals. Design: This study is designed to collect information from various stakeholders of the respective hospitals and to analyze the same using various statistical tools. The data is then interpreted to ensure there is a perfect cross-sectional analysis of each organization from top to bottom as well as bottom-to-top approach. Findings: The findings are based on the data collected about the commonalities and differentiation of services that these five hospitals offer. KMC stands out to be number one among the five. Originality: This is a first of its kind approach to analyze five hospitals in one city in a paper and the entire study is done by the researcher as part of a bigger study. Employee holistic well-being is attracting more interest and consideration from businesses as the employers started realizing the gains and impact this implementation of holistic wellness policies for workers in the workplace can have on the business. This study aims to analyze the impact of the holistic wellness of employees on their job satisfaction and work productivity. This paper aims to assess the approaches adopted by organizations towards employees’ holistic wellness and how it contributes to their job satisfaction and quality of work. The study aims to identify current employee well-being in organizations, employee wellness dimensions being covered through workplace wellness programs, and analyze the relationship between employees’ holistic well-being and work productivity, job satisfaction, and organization performance. The approach of the study includes a quantitative survey, qualitative interviews, and empirical evidence. Primary research is done at the organizational level using structured interviews with HR leads and employees. Secondary research is done by reviewing existing literature to explore industry best practices in holistic well-being. The deductive approach has been used to explain explanations and arguments backed by empirical observations in organizations and relevant theoretical concepts. The findings from the study will help the organizations who are in the process of streamlining and structuring the employee wellness programs to understand the link between employees’ holistic wellness and their productivity and job satisfaction and use these findings to increase job satisfaction and productivity of the employees. The data gathered not only from quantitative surveys and qualitative interviews but also through empirical observations. These findings can provide valuable pointers for human resource managers in other organizations.

The role of prophylactic hyperthermic intraperitoneal chemotherapy (p-HIPEC) in serosa invasive gastric cancers without gross or microscopic peritoneal disease, to reduce the rate of peritoneal relapse is an area of ongoing research. Although p-HIPEC is effective in reducing the rate of peritoneal relapse and improving disease free and overall survival with or without adjuvant chemotherapy, when added to curative surgery in locally advanced, non-metastatic gastric cancers, the available literature is at best, heterogeneous, centre-specific and skewed. Apart from that, variations in the systemic therapy used, and the presence of the associated nodal disease further complicate this picture. To evaluate the role of p-HIPEC the PubMed, Cochrane central register of clinical trials, and the American Society of Clinical Oncology (ASCO) meeting library were searched with the search terms, “gastric”, “cancer”, “hyperthermic”, “intraperitoneal”, “chemotherapy”, prophylactic”, “HIPEC” in various combinations, and a critical review of the available evidence was done. Although p-HIPEC is a promising therapy in the management of locally advanced gastric cancers, the current evidence is insufficient to recommend its inclusion into routine clinical practice. Future research should be directed towards identification of the appropriate patient subset and towards redefining its role with current peri-operative systemic therapies.

The aim of this research was to evaluate and compare the effect of a packaged orange juice on the two most commonly used restorative materials in pediatric dentistry.Fifteen samples each of 6 mm diameter and 2 mm thickness of a glass ionomer cement (GIC) and an esthetic restorative material were prepared using silicone rings. These were exposed to a packaged orange fruit juice and then placed in a standard culture ofResults from the study show that there is a decrease in the colony forming unit (CFU) after exposure to the packaged fruit juice as opposed to the studies using carbonated acidic drinks, which have shown a consistent rise in the CFU due to a change in the surface morphology.Within the limitations of this study, it was seen that the consumption of fruit juice may not cause a deteriorating effect on the restorative materials considered. However, the results were not statistically significant and further research is necessary to come to a conclusion regarding the reduction in the bacterial count after exposure to the fruit juice.With further research, such studies can help in improving the diet counseling practices.Patel A, Belsare SP, Jajoo SS

Advanced internet technology and data-hungry applications encourage us to use Multipath TCP (MPTCP). MPTCP can use many homogenous or heterogeneous interfaces like LTE, Wi-Fi, Ethernet, etc. simultaneously. This inherently introduces out of order packets(OFO) scenario at the receiver side. If packets are not collected in sequence at the receiver then after a timeout, retransmission and spurious retransmission may occur thus increasing overhead and decreasing the quality of user experience. To optimize this, an efficient MPTCP scheduling algorithm becomes essential. This article studies and compares a few MPTCP schedulers theoretically and practically using key performance metrics. Comparative study of data points suggests that known schedulers of MPTCP do perform well in uniform network environments, but are inflexible to adapt to dynamic environments with the constant flux in network states of wired/wireless and homogeneous/heterogeneous setup. This calls for a flexible algorithm that can learn the network state and adapt to it for optimized network performance.

Design: Amazon's business models are studied following the deductive and inductive approaches. Inductive: by checking the assumptions and coming to a conclusion, Deductive: by checking the theories and proving the hypothesis. Objective: To find out the competitive digital strategies of Amazon, that could be learned and copied for other organizational applications. Findings/Result: The findings show that there are several consumer complaints and service issues about Amazon's influence, notably in regard to pricing, customer service, in-store staff, and post-purchase support. The paper claims that Amazon's development and profitability have been greatly aided by business strategies combining technology, distribution networks, and customer service. These tactics have also assisted Amazon in differentiating itself from its competitors in the brick-and-mortar and online retail industries. Gene silencing by feeding double-stranded (dsRNA) holds promise as a novel pest management strategy. Nonetheless, degradation of dsRNA in the environment and within the insect gut, as well as inefficient systemic delivery are major limitations to applying this strategy. Branched amphiphilic peptide capsules (BAPCs) complexed with dsRNA have been used to successfully target genes outside and inside the gut epithelium upon ingestion. This suggests that BAPCs can protect dsRNA from degradation in the gut environment and successfully shuttle it across gut epithelium. In this study, our objectives were to 1) Determine whether feeding on BAPC-dsRNA complexes targeting a putative peritrophin gene of P. japonica would result in the suppression of gut peritrophin synthesis, and 2) gain insight into the cellular uptake mechanisms and transport of BAPC-dsRNA complexes across the larval midgut of P. japonica. Our results suggest that BAPC-dsRNA complexes are readily taken up by the midgut epithelium, and treatment of the tissue with endocytosis inhibitors effectively suppresses intracellular transport. Further, assessment of gene expression in BAPC- peritrophin dsRNA fed beetles demonstrated significant downregulation in mRNA levels relative to control and/or dsRNA alone. Our results demonstrated that BAPCs increase the efficacy of gene knockdown relative to dsRNA alone in P. japonica adults. To our knowledge, this is the first report on nanoparticle-mediated dsRNA delivery through feeding in P. japonica.

Background Hidradenitis suppurativa is an inflammatory disease of the inverse skin regions that occurs particularly in young women and affects approximately 1% of the population. Outpatient care is often inadequate and usually cannot prevent progression. Objective The aim of the EsmAiL trial was to evaluate whether an innovative care concept can decrease disease activity and burden, as well as improve patient satisfaction. Methods EsmAiL was conducted as a two-arm, multicentre, prospective randomised controlled trial including 553 adults with HS. Inclusion criteria were a minimum of three inflammatory lesions and at least moderate impact of the disease on the quality of life. The control group (CG) remained in standard care, whereas the intervention group (IG) was treated according to a trial-specific, multimodal concept. Primary endpoint was the absolute change in the International Hidradenitis Suppurativa Severity Score System (IHS4). Results 279 patients were randomised to the IG and 274 to the CG. Of those, 377 attended final assessment after 12-month of intervention. Participants in the IG (n=203) achieved a mean improvement in the IHS4 of 9.3 points while the average decrease in the CG patients (n=174) amounted to 5.7 points (p=0.003). Design/Methodology/Approach: This study includes various Kubernetes service providers in the industry. The documentation available in the provider website and the published whitepapers are used to understand the Secret management technology. Findings/Result: Software applications and their data are protected in managed cloud services using Secret keys. Across service providers there is a trend and a common approach in managing the Secrets. External or remote services integration is an improvement area for the providers. Operators have the responsibility of protecting Secrets and ensuring it does not leak out. Originality/Value: This paper studies the efforts of different vendors to protect the micro service Secrets. This ensures that the organization's critical assets are protected as per regulations and do not lead to financial losses. Cloud misconfiguration is one of the most common mistakes leading to security attacks, the knowledge of vendor's features helps to configure the cloud services securely. Paper Type: Case study

The present investigation was carried out to study the fauna of aquatic beetles of Koundinya wildlife sanctuary (India). A total of forty two species belonging to four families was recorded. The highest number of species was found in the family Dytiscidae followed by Hydrophilidae, Gyrinidae and Noteridae. All the species are recorded for the first time from the wildlife sanctuary and 9 species are new from the state of Andhra Pradesh.

Purpose This study aims to explore the patterns of corporate social responsibility (CSR) practices and investments across different ownership groups and relevance of CSR practices in the vision and mission (V&M) statements of firms. Design/methodology/approach The paper uses the neo-institutional theory approach, which explains similarities and differences in the CSR practices of organisations embedded within (and between) similar sectoral contexts. The study accounts the CSR activities of the top 100 companies listed on the Bombay Stock exchange (BSE) based on their ownership and checks the overlap of the CSR activities conducted by the companies with the ongoing social development schemes launched in India during the same of time. The time period between 2017 and 2020 is chosen to analyse the CSR studies. The study uses content analysis technique to derive conclusions. A textual analysis of top 100 listed firms across all ownership groups aimed at understanding patterns of CSR practices opted by the different groups and coherence of CSR patterns in the V&M statements. CSR related keywords were analysed in the V&M statements to understand what influence reporting of CSR practices in the strategic communication of firms. Findings Overall analysis indicated that top 100 firms prefer to invest in the areas of “Education”, “Sustainability” “Skill” where public-owned firms preferred towards “Sanitation” and “Environment/Sustainability” showing concurrence with local development goals. Private and foreign groups preferred to park their CSR funds in “Education” and “Skill” development showing coherence with the global agendas. Public-owned firms tend to report more CSR related specifically “Environment’ and “Sustainability” in the strategic documents. However, private and foreign firms do not pay any significance to CSR related keywords in their V&M statements. Research limitations/implications Findings suggest that despite of huge CSR investments, private and foreign-owned firms lack CSR focus and communication in their V&M statements, which may create disintegration in the CSR investment and strategic alignment of near-term and future goals. The paper suggests that private and foreign firms should also communicate their CSR practices through their V&M to stakeholders so that CSR practices may not remain mere 2% mandated expenditure by the Government of India. Originality/value The study contributes in confirming the success of the CSR policy mandate in supplementing government’s social development programmes along with indications on the role of family firms in accelerating the process of community development as compared to foreign firms. The study also favours integration of CSR disclosures in the V&M statements to gain long-term benefit out of these investments.

Retroperitoneal tumours arising from the inferior vena cava (IVC) are rare tumours often requiring large vessel resection for complete surgical excision. Limited exposure to such tumours often discourages surgeons from offering surgical resection to these patients, depriving them of the only potentially curative modality. We present here the surgical technique for resection of a large IVC sarcoma without IVC reconstruction.A 53-year-old lady presented with a large retroperitoneal sarcoma encasing the infra-hepatic IVC with tumour thrombus extension into the hepatic cloaca as well as the left renal vein. Surgical resection was planned as the disease remained stable after 2 cycles of neoadjuvant chemotherapy with adriamycin and ifosfamide.Complete surgical excision of the tumour was achieved by performing a resection of the entire length of infra-hepatic IVC and right kidney, without IVC reconstruction. Left renal vein was divided after careful preservation of a draining collateral. Tumour thrombus was extracted from the hepatic cloaca, and proximal IVC stump closure was achieved with preservation of right hepatic vein insertion. Total blood loss during the procedure was 2300 mL, and the patient recovered without compromise of renal function or development of lower limb oedema.IVC resection without reconstruction can be safely performed for large retroperitoneal sarcomas involving major vascular structures. Familiarity with the retroperitoneal, retro-hepatic and supra-hepatic anatomy is paramount to achieving good surgical outcomes.

To examine trends in the direct acting antiviral (DAA) uptake in a multi-state Medicaid population with hepatitis C virus (HCV) prior to and after ledipasvir/sofosbuvir (LDV/SOF) approval and changes in prior authorization (PA) requirements.Analyses utilized enrollment, medical, and pharmacy claims in four states, December 2013-December 2017.An interrupted time series examined trends in uptake (1+ claim for a DAA) before and after two events: LDV/SOV approval (October 2014) and lifting of PA requirements for 40% of members (July 2016). Analyses were also performed in subgroups defined by the number and dates of change in PA requirements in members' Medicaid plans.Members aged 18-64 years with an ICD code for HCV were included in the sample from diagnosis date until treatment initiation or Medicaid disenrollment.The annual sample size ranged from 38,302 to 45,005 with approximately 30% ages 18-34 years and 40% female. In December 2013, 0.08% was treated, rising to 0.74% in December 2017 (p 0.001). Uptake increased from 0.34%/month in October 2014 to 0.70%/month after LDV/SOF approval, (p 0.001), and increased relative to the pre-LDV/SOV trend through June 2016 (p = 0.04). Uptake increased to 1.18%/month after PA change, (p 0.001) and remained flat through 2017 (p = 0.64). Cumulatively, 20.1% were treated by December 2017. In plans with few/no requirements through 2017, uptake increased to 1.19%/month after LDV/SOF approval (p 0.001) and remained flat through 2017 (p = 0.11), with 22.2% cumulatively treated. Among plans that lifted PA requirements from three to zero in mid-2016, uptake did not increase after LDV/SOF approval (p = 0.36) but did increase to 1.41%/month (p 0.001) after PA change, with 18.1% cumulatively treated.HCV Treatment increased through 2017. LDV/SOF approval and lifting PA requirements led to an increase in uptake followed by flat monthly utilization. Cumulative uptake was higher in plans with few/no PA requirements relative to those with three requirements through mid-2016.

Elektrochemotherapie (ECT) ist eine wirksame lokale Behandlung von Hauttumoren. Ziel dieser Studie war es, die Wirksamkeit der ECT bei ulzerierten gegenüber nichtulzerierten Tumoren zu vergleichen und den Effekt auf tumorassoziierte Symptome zu untersuchen.20 Krebszentren des International Network for Sharing Practices on Electrochemotherapy (InspECT) sammelten prospektiv Daten. Die ECT wurde nach dem ESOPE-Protokoll durchgeführt. Das Therapieansprechen wurde anhand der Entwicklung der Läsionsgröße bewertet. Zusätzlich wurden Schmerzen, Symptome, Leistungsstatus (ECOG-Index) und Gesundheitszustand (EQ-5D-Fragebogen) untersucht.716 Patienten mit ulzerierten (n = 302) und nichtulzerierten (n = 414) Hauttumoren und Metastasen wurden eingeschlossen (Mindest-Nachsorge 45 Tage). Nicht-ulzerierte Läsionen sprachen besser auf die ECT an als ulzerierte Läsionen (vollständiges Ansprechen: 65 % gegenüber 51 %, p = 0,0061). Nur 38 % (115/302) der Patienten mit ulzerierten Läsionen vor der ECT wiesen bei der letzten Nachuntersuchung ulzerierte Läsionen auf. Patienten mit ulzerierten Läsionen berichteten über stärkere Schmerzen und schwerere Symptome im Vergleich zu Patienten mit nichtulzerierten Läsionen, die sich nach der ECT signifikant und kontinuierlich besserten. Bei Patienten mit nichtulzerierten Läsionen hingegen nahmen die Schmerzen während der Behandlung vorübergehend zu. Es wurden keine schwerwiegenden Nebenwirkungen beobachtet.Die ECT ist eine sichere und wirksame lokale Behandlung von Hauttumoren. Während die ECT die Symptome insbesondere bei Patienten mit ulzerierten Läsionen verbessert, sollte auf Basis der Daten die Implementation eines perioperativen Schmerzmanagements besonders bei nichtulzerierten Läsionen während der ECT erwogen werden.

Background: CDK 4/6 inhibitors (CDKI) with Endocrine therapy (ET) is mainstay treatment for hormone receptor-positive (HR+) Her2- metastatic breast cancer (MBC). Despite excellent efficacy, most patients develop resistance to CDKI limiting its utility. CDK4/6 are thought to act by upregulating VEGF causing tortuous angiogenesis and promoting cancer progression. Recent studies have shown aggressive tumors possess a higher density and tortuous vasculature. In this study, we evaluated if vascular radiomic features computationally extracted from liver CT scans pre- and post CDKI treatment could predict patient survival and treatment response. Method: From a registry of 350 patients on treatment with CDKI at institution 1(S1), 51 pts with HR+, Her2-, MBC patients with evidence of liver mets and disease progression (PFS) data were identified. 30 pts discontinued treatment due to progression or death, with a median time to progression of 195 days. Pre-treatment and first post-treatment CT exams were analyzed from 25 and 34 patients, respectively. Median time between scans was 128 days. To validate the prognostic value of our signature, a cohort of 29 patients with available OS data was identified from institution 2 (S2). A publicly available pre-trained deep learning model was applied to isolate liver metastases and vessels. Next, fast marching algorithm was applied to reduce vessels to their centerlines and divide the vasculature into constituent branches. 7 quantitative metrics were computed measuring vascularity of metastases and 3-D shape of hepatic vessels. First, the number (f1) and percentage (f2) of hepatic vessels arising from the tumor were computed. Vessel tortuosity - measuring the degree of twisting across a vessel - was computed separately for each branch. The mean (f3), standard deviation (f4), maximum (f5), skewness (f6), and kurtosis (f7) tortuosity values were calculated to summarize these measurements at patient level. The features were individually assessed at pre- and post-treatment for association with PFS at S1 in univariable Cox proportional hazards models. Features found to be associated in S1 were evaluated for association with OS in S2. Results: On the initial post-treatment scan, features of both tumor vascularization (f6 - HR=1.115 [1.039-1.196]; f7 - HR=10.646 [2.539-44.641]), as well as two features of vessel tortuosity (f3 - HR=0.011 [0.001-0.199]; f4 - HR=0.545 [0.313-0.949]) were significantly associated with PFS. Both tortuosity features were also significantly associated with OS in S2 (f3 - HR=0.085 [0.009-0.780]; f4 - HR=0.331 [0.130-0.842]). In addition the percentage (f7 - HR=6.445 [2.001-20.753]]),of vessels feeding the lesions was also significant in S2 while the number (f6 - HR=1.070 [0.966-1.184]), of vessels was not. No vessel metrics from the pre-treatment baseline exam were significantly associated with OS. Conclusions: Radiomic analysis of tumor vascularity and vessel tortuosity on CT scans post-CDK treatment was associated with patient survival and treatment response.
