Magnetic Resonance Imaging (MRI) produces excellent soft tissue contrast, albeit it is an inherently slow imaging modality. Promising deep learning methods have recently been proposed to reconstruct accelerated MRI scans. However, existing methods still suffer from various limitations regarding image fidelity, contextual sensitivity, and reliance on fully-sampled acquisitions for model training. To comprehensively address these limitations, we propose a novel self-supervised deep reconstruction model, named Self-Supervised Diffusion Reconstruction (SSDiffRecon). SSDiffRecon expresses a conditional diffusion process as an unrolled architecture that interleaves cross-attention transformers for reverse diffusion steps with data-consistency blocks for physics-driven processing. Unlike recent diffusion methods for MRI reconstruction, a self-supervision strategy is adopted to train SSDiffRecon using only undersampled k-space data. Comprehensive experiments on public brain MR datasets demonstrates the superiority of SSDiffRecon against state-of-the-art supervised, and self-supervised baselines in terms of reconstruction speed and quality. Introduction Immune checkpoint inhibitors (ICIs) are being commonly used to treat solid tumours such as renal cell carcinoma. Hypophysitis is an acute or chronic inflammation of the pituitary gland and nivolumab or pembrolizumab induced hypophysitis is markedly lower compared to ipilimumab. Case report We present a novel case of a patient with mRCC who was diagnosed with nivolumab induced hypophysitis based on clinical suspicion due to his hormonal profile and a range of symptoms that he developed during nivolumab immunotherapy. Management and outcome He was treated with high dose of hydrocortisone administered intravenously, subsequently changed to the oral route and physiologic dose. Discussion Nivolumab induced hypophysitis is a rare condition that usually presents with fewer symptoms. High degree of clinical suspicion and a multidisciplinary team required to diagnose and treat such cases.

Deep MRI reconstruction is commonly performed with conditional models that de-alias undersampled acquisitions to recover images consistent with fully-sampled data. Since conditional models are trained with knowledge of the imaging operator, they can show poor generalization across variable operators. Unconditional models instead learn generative image priors decoupled from the imaging operator to improve reliability against domain shifts. Recent diffusion models are particularly promising given their high sample fidelity. Nevertheless, inference with a static image prior can perform suboptimally. Here we propose the first adaptive diffusion prior for MRI reconstruction, AdaDiff, to improve performance and reliability against domain shifts. AdaDiff leverages an efficient diffusion prior trained via adversarial mapping over large reverse diffusion steps. A two-phase reconstruction is executed following training: a rapid-diffusion phase that produces an initial reconstruction with the trained prior, and an adaptation phase that further refines the result by updating the prior to minimize reconstruction loss on acquired data. Demonstrations on multi-contrast brain MRI clearly indicate that AdaDiff outperforms competing conditional and unconditional methods under domain shifts, and achieves superior or on par within-domain performance.

AMAÇ:Akromegalik erkek hastalarda spermlerin niceliksel ve niteliksel olarak incelenmesini, hastalığın aktif ve remisyon dönemlerinde androjen salınımında ve spermiogramda farklılık olup olmadığını öğrenmeyi hedefledik.YÖNTEM:Otuz hastalığı aktif, 32 hastalığı inaktif erkek akromegali hastası ile kontrol grubu için 10 NFA, 10 sağlıklı gönüllü erkek çalışmaya alındı.Çalışmaya alınan hastalardan; serumda GH, IGF-1, FSH, LH, prolaktin, SHBG, albümin, testesteron düzeylerine bakmak için kan ve aynı gün semen örneği alındı.Semen örneğinde semen hacmi, sperm motilitesi, morfolojisi, sperm konsantrasyonu, TMSS parametrelerine bakıldı.BULGULAR:Hastaların ortalama yaşı 42.4 idi.Sağlıklı gönüllüler, NFA, aktif ve remisyondaki akromegali hastaları arasında fertilite açısından p=0.026 istatiksel anlamlı fark görüldü.Yine 4 grup arasında bakılan serumda GH(p=0.00), IGF-1(p=0.00), FSH(p=0.00), testesteron(p=0,00), albumin(p=0.036) ve semen volümü(p=0.001), sperm konsantrasyonu (p=0.00), sperm motilitesi(p=0.003), TMSS(p=0.00), morfoloji(p=0.001) değerleri için anlamlı fark saptandı. Akromegalik erkek hastalarda hastalık aktif olsun olmasın sağlıklı bireylerle karşılatırdığımızda hem FSH, testosteron değerlerinin düşük, hem de sperm fonksiyonlarının bozulduğunu tespit ettik.Kendi içinde tedavi yanıtlarına göre kıyasladığımızda ise, aktif hastalık grubunda bu olumsuz etkilenmenin daha belirgin olduğunu saptadık.Ek olarak GH yüksekliğiyle testesteron (r=-0,290,p=0.01), FSH (r=-0.220,p=0.05), semen volümü (r=-0,234, p=0.005), sperm motilitesi (r=-0,263,p=0,005), TMSS (r=-0,275,p=0,05) ve sperm morfolojisi (r=-0,315,p=0.01) arasında anlamlı negatif korelasyon bulduk. SONUÇ:Yüksek GH ve IGF-I seviyelerine maruz kalmanın germ hücrelerinde,testis üzerinde yarattığı olumsuz etki ile hastalık kontrolü sağlandığında dahi sağlıklı bireylere kıyasla androjen salınımında ve sperm fonksiyonlarında olumsuz etki yarattığı gösterilmiştir.Hastalığın aktif döneminde serum testosteron, FSH düzeyi ve semen volümü, sperm konsantrasyonu, motilitesi, toplam motil sperm sayısı GH yüksekliğiyle anlamlı negatif korele bulunmuştur.Ancak tedaviyle birlikte, çoğu testiküler anomalinin düzeldiği ve testesteron düzeylerinin arttığı izlenmiştir. OBJECTIVE:The aim of this study is to determine the quantitative and qualitative examination of sperms in acromegalic male patients and to detect differences in androgen secretion and spermiograms during active and remission periods of the disease.METHOD:Active(n:30), inactive(n:32) disease with acromegaly, NFA(n:10) and healthy volunteer males(n:10) were included.Serum GH, IGF-1, FSH, LH, prolactin, SHBG, albumin, and testosterone were measured.On the same day,semen samples were taken.Semen volume, motility, morphology, sperm concentration, total motile sperm count (TMSC) were analyzed.RESULTS:The mean age of patients was 42.4 years.There was statistically significant difference between four groups for fertility p=0.026.In addition, there was statistically significant difference found between four groups for GH(p=0.00),IGF-1(p=0.00), FSH(p=0.00), testesterone(p =0.00), albumin (p=0.036) and semen volume(p=0.001), sperm concentration(p=0.00),sperm motility(p=0.003), TMSC(p=0.00), sperm morphology (p=0.001).We found that FSH,testosterone and sperm function were impaired in acromegaly patients with or without active disease,when compared with healthy individuals.We found that these dysfunctions were prominent in active disease group.In addition, there was significant negative correlation seen between testosterone(r=-0.290,p=0.01), FSH(r=-0.220,p=0.05), semen volume(r=-0.234,p=0.005), sperm motility(r=-0.263,p=0.005), TMSC(r= -0.275,p= 0.05), morphology (r=-0.315,p=0.01) and serum GH level.CONCLUSION:Exposure to elevated levels of GH and IGF-I in acromegalic male patients has been shown to have an adverse effect on androgen secretion,sperm function compared to healthy persons even when disease control is achieved.This can be explained by adverse effects on germ cells, testis.Serum testosterone,FSH level and semen volume,sperm concentration, motility, and TMSC were significantly lower in the active period of the disease.However, with treatment, most testicular anomalies improved and testosterone levels increased. 57

Bu tez sosyal sorumluluk kavramı, sosyal sorumluluk alanları, sosyal sorumluluk düzeyleri ile sosyal sorumluluk standardı olan SA 8000 gibi konuları irdelemiş bir çalışmadır.Araştırmanın amacı Adıyaman organize sanayindeki firmaların sosyal sorumluluk düzeylerini incelemek, şirketlerin bu konudaki fikir ve görüşlerini açıklayıp, Adıyaman'daki sosyal sorumluluk profilini ortaya çıkartmaktır.Yapılan araştırma kavramsal ve nitel araştırma olmak üzere iki bölümden oluşmaktadır. Kavramsal bölümde sosyal sorumluluk kavramı hakkında kuramsal bilgi verilirken, ikinci bölümde ise, Adıyaman'da faaliyet gösteren firmaların yöneticileri ile sosyal sorumluluk kapasitelerini ortaya çıkaran veriler elde etmek amacıyla yüz yüze mülakat yapılmıştır. Elektriki iletkenlik (Eİ) testi bezelye için onaylanmış bir test olup diğer baklagil tohumları ve endospermik olmayan tür tohumları içinde tavsiye edilen bir tohum gücü testidir. Bu çalışmada, çimlenme oranı %80 ve üzeri olan beyaz tohum kabuğuna sahip 11 farklı börülce tohum partisinin tarla çıkışını tahmin etmek için tek tohum ve yığın tohum elektriki iletkenlik testleri gerçekleştirilmiştir. Tohum partilerinin yığın tohum elektriki iletkenlik değerleri 17.3 ile 41.6 µS cm-1 g-1 arasında değişirken, tek tohum iletkenlik değerleri 63.7 ile 130.5 µS cm-1 g-1 arasında değişmiştir. Tarla çıkış testleri (TÇ-1 ve TÇ-2) 2012 yılı erken ilkbahar döneminde iki farklı tohum ekim tarihinde gerçekleştirilmiştir. Tohum partilerinin tarla çıkışları TÇ-1'de %53 ile %79 arasında ve TÇ-2'de %61 ile %93 arasında değişmiştir. Hem tek tohum hemde yığın tohum iletkenlik testleri her iki tarla çıkışı ile yüksek negatif korelasyona sahip bulunmuştur (sırasıyla, TÇ-1'de r=-0.794** ve r=-0.891**, p80%, using single and bulk conductivity methods. Bu çalışmada AC Motorun Profibus yoluyla uzaktan denetimi gerçekleştirilmiştir. Tasarımı, merkezi denetim, AC motor ve bu ikisi arasındaki haberleşmeyi sağlayan Profibus ağı oluşturmaktadır. Motorun denetimi geri beslemeli olarak tasarlanmıştır. Ağ yoluyla alınan motor hız bilgisi kullanılarak denetim bilgisi elde edilerek bu bilgi aynı ağ omurgası üzerinden motora iletilmektedir. Bu tasarımda gerçek zamanlı kontrol sistemi uygulanmış olup, motorun kullanım amacına uygun yumuşak kalkış ve durma işlemini gerçekleştirilerek hassas denetim elde edilmektedir. Profibus yüksek hızlı veri iletimi sağladığından, ağ gecikmesinden kaynaklanan denetim hataları düşük oranda gerçekleşmiştir. In this study, the remote control of induction motor have been implemented by using Profibus network. The design is composed of central control, AC motor and Profibus network which provides the communication between them. The control of the motor has been designed as a feedback control. The control data is obtained by the usage of motor speed data that is received via network, and this data is transferred to the motor through network backbone. In this design, realtime control system has been applied, and hence for a sensitive control that perform soft starting and stop operation which is suitable intended use of the motor have been obtained. By reason of high velocity data transfer of Profibus, the position errors due to the network delay have occured at lower ratio.

Hücresel ağlar ve uydu sistemleri gibi kablosuz iletişim teknolojilerinin kullanımının yaygınlaşması ile ev otomasyon ve güvenlik sistemlerinin çeşitliliğinde ve kullanımda son yıllarda hızlı bir artış meydana gelmiştir. Özellikle hücresel ağ teknolojisinin kullanıldığı GSM ile mobil iletişim alanında her geçen gün yeni gelişmeler meydana gelmektedir. Bu çalışmada elektrikli cihazlar ve güvenlik sistemlerinin cep telefonu ile uzaktan denetlenmesi uygulaması gerçekleştirilmiş ve iletişim hattı olarak GSM kullanılmıştır. Gerçekleştirilen uygulama ile sisteme bağlanacak dört adet cihazın aç/kapa denetimi gerçekleştirilmekte, denetim işlemlerinin ardından, cihazların durumu kullanıcılara sesli mesajlar ile iletilerek geri besleme sağlanmaktadır. Bununla birlikte algılanan alarm durumları kullanıcılara sesli mesajlar ile iletilmektedir. Yapılan bu çalışmada, bazı üstünlüklerinden dolayı endüstride çok kullanılanasenkron motorların stator sarım yöntemleri geniş bir şekilde araştırılarakaralarındaki farklar açıklanmıştır. 1,1 BG gücündeki altı adet NM 90S-4standardındaki ASM'un statorlarına, altı değişik metotla sarılmış sargıyerleştirilerek şebekeden ve invertörden beslenmeleri durumundakiperformansları ve uygun sarım yöntemleri belirlenmiştir. Deney ve ölçmelerdeliteratürde geçerliliği kabul gören standartlar kullanılmıştır. Six different types ofwindings are placed into the stators of the six standard induction motors having1.1 HP power. The performances and appropriate winding techniques havebeen determined according to the network and inverter feeding conditions.Internationally valid standards have been used in the measurements andexperiments. Measurement tools developed with high technology have beenused. Harmonics resulting from source voltage and power loss increasing havebeen showed to be related to the winding shape. Supervised reconstruction models are characteristically trained on matched pairs of undersampled and fully-sampled data to capture an MRI prior, along with supervision regarding the imaging operator to enforce data consistency. To reduce supervision requirements, the recent deep image prior framework instead conjoins untrained MRI priors with the imaging operator during inference. Yet, canonical convolutional architectures are suboptimal in capturing long-range relationships, and priors based on randomly initialized networks may yield suboptimal performance. To address these limitations, here we introduce a novel unsupervised MRI reconstruction method based on zero-Shot Learned Adversarial TransformERs (SLATER). SLATER embodies a deep adversarial network with cross-attention transformers to map noise and latent variables onto coil-combined MR images. During pre-training, this unconditional network learns a high-quality MRI prior in an unsupervised generative modeling task. During inference, a zero-shot reconstruction is then performed by incorporating the imaging operator and optimizing the prior to maximize consistency to undersampled data. Comprehensive experiments on brain MRI datasets clearly demonstrate the superior performance of SLATER against state-of-the-art unsupervised methods.

Multi-institutional efforts can facilitate training of deep MRI reconstruction models, albeit privacy risks arise during cross-site sharing of imaging data. Federated learning (FL) has recently been introduced to address privacy concerns by enabling distributed training without transfer of imaging data. Existing FL methods for MRI reconstruction employ conditional models to map from undersampled to fully-sampled acquisitions via explicit knowledge of the imaging operator. Since conditional models generalize poorly across different acceleration rates or sampling densities, imaging operators must be fixed between training and testing, and they are typically matched across sites. To improve generalization and flexibility in multi-institutional collaborations, here we introduce a novel method for MRI reconstruction based on Federated learning of Generative IMage Priors (FedGIMP). FedGIMP leverages a two-stage approach: cross-site learning of a generative MRI prior, and subject-specific injection of the imaging operator. The global MRI prior is learned via an unconditional adversarial model that synthesizes high-quality MR images based on latent variables. Specificity in the prior is preserved via a mapper subnetwork that produces site-specific latents. During inference, the prior is combined with subject-specific imaging operators to enable reconstruction, and further adapted to individual test samples by minimizing data-consistency loss. Comprehensive experiments on multi-institutional datasets clearly demonstrate enhanced generalization performance of FedGIMP against site-specific and federated methods based on conditional models, as well as traditional reconstruction methods.
